▼ Acute kidney injury (AKI) is serious and complicates up to 1 in 7 hospital admissions. It is usually diagnosed from rapidly deteriorating blood tests. Much of the focus of clinical research into AKI has been on strategies to improve recognition and timely intervention. However, emerging evidence suggests that even when people survive AKI, they remain at an elevated risk of poor long-term outcomes. The aim of this thesis was to determine which people with AKI have an ongoing increased risk of poor outcomes (mortality, kidney failure, recurrent illness episodes) after hospital discharge. The design was a population-based data-linkage cohort study involving the Grampian Laboratory Outcomes Morbidity and Mortality Study (GLOMMS-II). Data linkages included population biochemistry, hospital episode data, mortality records, intensive care records and renal registry data from 1999-2013. A cohort of 17,630 people hospitalised in 2003 were followed through to 2013. Outcomes were mortality, progression of kidney disease and unplanned hospital readmission episodes. There have been several novel research outputs. I evaluated and adapted international AKI criteria for use in large population biochemistry datasets. I developed a clinical risk prediction model for unplanned readmissions after hospital discharge, for which AKI was a strong independent predictor. I described long-term survival after AKI, showing that people with AKI (vs no AKI) have a substantially higher risk of death in the first year, but diminishing excess risk thereafter. Finally, I conducted a novel analysis of renal prognosis after AKI, showing that mortality and non-recovery are more common than subsequent renal progression after AKI, but that renal progression is nevertheless increased after AKI. Overall, AKI is a serious condition and marker of people who have a long lasting poorer prognosis. The first year after discharge is a period of particularly heightened risk that could potentially be targeted with initiatives to improve care.

▼ Abstract Cisplatin is one of the most effective chemotherapeutic agents used in treatment of a variety of human solid tumors. The most common adverse side effect limiting the use of Cisplatin is nephrotoxicity. Recent studies indicate that inflammatory and oxidative signaling play critical role in pathogenesis of Cisplatin related nephrotoxicity. Cisplatin-induced nephrotoxicity is associated with lipid peroxidation and the superoxide anion and hydroxyl radical scavenger could prevent acute renal failure through both attenuation of tubular damage and enhanced regenerative response of the damaged tubular cells. It has been shown that green tea polyphenols with antioxidant properties inhibit inflammatory and oxidative responses in mice. However, the evidence indicating the protective effect of epigallocatechin gallate (EGCG), the major polyphenol found in green tea, on Cisplatin-induced nephrotoxicity is lacking. The present study is to evaluate the effect of EGCG injection on Cisplatin-induced nephrotoxicity in rats. The male rats were divided into four groups (n = 6 each); control group, Cisplatin group, EGCG group and Cisplatin + EGCG group. The control group received only intraperitoneal normal saline injection. Cisplatin (6 mg/kg) was given single dose intraperitoneally at day 0, EGCG (10 mg/time) was given subcutaneously at day 4, day 2 and day 0 before Cisplatin challenge and day 2 and day 4 after Cisplatin injection. Nephrotoxicity was evaluated by biochemical analysis of blood and histopathological observations of rat kidney. Nuclear factor-kappa B (NF-ÎºB) activation, inducible nitric oxide synthase (iNOS) expression and malonyldialdehyde (MDA) content were also determined in rat kidney. Cisplatin injection induced an increase in serum blood urea nitrogen, creatinine and tubular necrosis, and upregulation of NF-ÎºB and iNOS expression and MDA content in kidney. All the increases were significantly inhibited by EGCG treatment. The results suggest that EGCG may attenuate Cisplatin induced nephrotoxicity through the anti-inflammatory/oxidative effects. ▼ According to traditional Chinese medicine , Cordyceps sinensis (CS) can prevent subjects from renal failure. The aim of this study was to investigate the protective effect of CS preconditioning on ischemic renal acute failure in rats and to assess its mechanism. The animal model of ischemic acute renal failure was performed by left nephrectomy and clamping right renal vessel for 45 mins in S-D rats. Cordyceps group had been pretreated with two-day 600 mg/kg CS before I/R injury. Rats were sacrificed at 1, 3, 6, 16, 48 and 120 h after reperfusion for evaluation of renal function and histopathological PASD staining. The immunohistochemistry and Western blotting of SDF-1Î±, CXCR4 and Ki67 were also performed. Î-galactosidase activity was detected with the senescence staining. The results showed that the level of creatinine in Cordyceps group were significant lower after 48 hours I/R injury (p =0.04). PSAD staining in Cordyceps group revealed less tubular necrosis, tubular dilatation, and cast formation at 6 and 16 hour than in control group. Immunohistochemistry of SDF-1Î± in Cordyceps group demonstrated staining in the distal tubules and collecting ducts at 1, 3, 6, and 16 h. The CXCR4 signal of control group had gradually intensified from 1 to 6 hr after I/R . In Cordyceps group, the CXCR4 expression had been stabilized until 16 h after I/R. The Î²-galactosidase activity was higher in control group at 1, 3 and 6 hours. However, the senescence was presented at 1 and 3 hours in Cordyceps group. The nuclear staining of repair enzyme Ki67 in Cordyceps group showed higher density than in control group. Pathologic morphology indicated CS may protect subjects from ischemic acute renal failure. CS also induced SDF-1Î± expression in early stage of I/R injury, and maintained the stable CXCR4 expression. CS can not only reduce the activity of senescence-related Î²-galactosidase, but also regulate the expression of repair enzyme Ki67, indicating that CS may alleviate the ischemic-induced senescence and enhance renal repair. ▼ Galectin-3 is a 30-42 kDa [beta]-galactoside-binding lectin with putative functions in cell growth, adhesion and neoplastic transformation. Galectin-3 is expressed in the human ureteric bud/collecting duct lineage and is upregulated in human autosomal recessive kidney disease (ARPKD). ARPKD is a congenital kidney malformation that affects 1 in 40,000 births, whereby cysts develop from the collecting ducts. Previous investigations have shown that exogenous galectin-3 protein levels modulates the growth of cysts derived from Madin Darby Canine Kidney (MDCK) collecting duct-derived cells in vitro. In this thesis, therefore, I investigated the expression patterns and functions of galectin-3 in normal mouse kidney development, and in an animal model for ARPKD, the cpk/cpk mouse. I also begun preliminary investigations into the roles of galectin-3 in kidney regeneration, following acute renal failure. During normal mouse development, galectin-3 protein was detected in the developing ureteric bud/collecting duct lineage, and downregulated in mature postnatal collecting ducts. Utilising mouse metanephric organ culture, I functionally implicated galectin-3 in the control of ureteric bud branching by demonstrating retardation of ureteric bud branching and decreased nephron formation, when extracellular galectin-3 protein levels were altered. (Bullock et al. 2001). In cystic kidneys postnatally, galectin-3 expression patterns, as assessed by in situ hybridisation, immunohistochemistry and western blot, was markedly upregulated in collecting duct derived cysts from the cpk/cpk mice. Paclitaxel (Taxol), a microtubule-stabilising drug, had previously demonstrated therapeutic affects which reduced cyst growth in vivo. Therefore during these investigations, I sought to investigate the effects of paclitaxel on galectin-3 expression within aberrant cystic epithelia. Paclitaxel treatment delayed cyst development and this was associated with decreased expression of galectin-3 in vivo and increased secretion of galectin-3, into the medium, by cystic epithelial cells in-vitro. Based on these results, I propose that in normal development, galectin-3 is implicated in normal ureteric bud differentiation, and upregulation of galectin-3 is associated with the aberrant growth and expansion of cystic collecting duct epithelia in ARPKD.

Comparar o desfecho tardio de pacientes com LRA e DRC estágio 5 sobrevivendo à internação em UTI. 0pWRGRV: Todos os pacientes admitidos em uma UTI de um hospital universitário terciário entre 1998 e 2000. Foram analisados 239 pacientes com LRA e 28 com DRC que sobreviveram à internação. O tempo de seguimento para 231 pacientes com LRA foi 8,7 anos e de 8,4 anos para 25 pacientes com DRC. Os dados foram apresentados como porcentagem (%) ou média ( [ ) desvio padrão (DP) e foram comparados por teste t Student, análise de variância (Anova), Mann Whitney, Kruskal Wallis, Qui-Quadrado ou Fischer, como apropriado. 5HVXOWDGRV: Variáveis analisadas no tempo de internação da UTI mostraram que os pacientes com LRA eram mais velhos 58,6 (14,5-98,6) versus 43,2 (16,1-75,9) anos em DRC (p=0,001); apresentavam mais homens 65,4 % versus 48 % em DRC (p=0,086); ficaram internados por um tempo maior 8 (1-89) versus 5 (2-45) dias em DRC (p=0,032); tinham menor Cr à admissão na UTI 1,3 mg/dl versus 6,7 mg/dl em DRC (p<0,001) e maior prevalência de choque 66,7% versus 26% em DRC, (p=0,002). Os pacientes com DRC tinham maior prevalência de hipertensão 84% versus 66,7% em LRA (p=0,077), maiores porcentagem de admissão por causas clínicas na UTI 100% versus 71% em LRA (p=0,002), maior prevalência de diálise 100% versus 32% em LRA (p<0,001) e maior prevalência de insuficiência respiratória 76% versus 10,4% (p<0,001). Número de falências orgânicas, prevalência de diabetes e de doenças cardiovasculares, porcentagem de readmissão na UTI, prevalência de infecção, acidose metabólica e hipercalemia foram similares nos dois grupos. As curvas Kaplan-Meier de sobrevida de pacientes com LRA e DRC foram diferentes (log-rankp=0,015). A mortalidade tardia foi maior no grupo LRA: 80,5% versus 48% em DRC (p<0,001). O modelo proporcional de COX mostrou que idade (p=0,038) e LRA (p=0,044) foram as variáveis associadas à mortalidade tardia. &RQFOXVmR: Pacientes com LRA que sobreviveram à internação em UTI apresentaram mortalidade tardia significativamente maior do que pacientes com DRC que sobreviveram à internação no mesmo período.

Background: The increased incidence of renal impairment and worsening renal function (WRF) in patients with acute heart failure (AHF) as well as, their correlation with adverse outcomes, raises the emerge for early diagnosis and management. The use of novel biomarkers, including cystatin C, which can estimate more accurately glomerular filtration rate (GFR) and are less influenced by confounding nonrenal factors, is crucial for these patients. Levosimendan, a calcium-sensitizing inotrope, has been correlated with reduction in serum creatinine, whereas its impact on cystatin C, compared with placebo or other inotropes, remains undefined.Aim: The objective of this study was to evaluate the effects of levosimendan on markers of kidney function in patients with acutely decompensated heart failure and renal impairment.Methods: The study included 96 patients hospitalized for acutely decompensated heart failure (ADHF) with symptoms of NYHA class III-IV, systolic dysfunction (ejection fraction ≤ 35%), and renal impairment [estimated glomerular filtration rate (eGFR) <60 ml/min] on admission. Half of these patients received levosimendan and the rest (age-, sex- and cardiac and renal severity-matched) patients received standard treatment (diuretics or/and other inotropes). WRF was defined as an increase in serum ceatinine ≥ 0.3 mg/dl from baseline.Results: Patients were on average 70±9 years old. Serum creatinine decreased but without any significant interaction with treatment (-0.13±0.25 έναντι -0.10±0.58, p=NS, respectively). Cystatin C decreased only in the levosimendan arm (-0.14±0.46 έναντι 0.06±0.51, F=2.985, p=0.088) and there was a trend towards a statistically significant interaction. WRF was less frequently observed with levosimendan administration (4.3% vs. 18.8%, p=0.03).Furthermore, in the overall population, in-hospital rise in cystatin C ≥ 0.4 mg/l was related with higher incidence of major adverse cardiac events (MACE) at 60 days (75% vs. 36% chi-square test p = 0.035; log-rank test p=0.05). Apart from this prognostic factor, BNP value at discharge ≥ 356 pg/ml was also an independent predictor of adverse outcomes (HR=4.457, 95% CI=1.292-15.379, p=0.018). Combining those two parameters (cystatin C change < or ≥ 0.4 mg/l and discharged BNP < or ≥ 356 pg/ml) stratified further the risk of events at 60 days (log-rank test p=0.016).Conclusions: In patients with ADHF and renal dysfunction on admission, levosimendan administration was related with improvement in renal biomarkers, compared to standard therapy. However, large-scale prospective studies are required to clarify the clinical significance of this observation and its impact on prognosis of heart failure patients. Interestingly, in this population in-hospital rise in cystatin C levels and BNP discharge levels had additive prognostic value at short-term.

A nefropatia induzida por contraste (NIC) é uma lesão renal aguda (LRA) tóxica, que consiste em vasoconstrição intra-renal, toxicidade tubular direta com liberação de espécies reativas de oxigênio (EROs). A NIC está diretamente associada a doenças crônicas que comprometem a oxigenação da região da medula renal, como a disfunção renal preexistente, o Diabetes Mellitus (DM) e insuficiência cardíaca congestiva. Esse estudo investigou os mecanismos fisiopatológicos que caracterizam a NIC em ratos diabéticos. Foram utilizados ratos Wistar, adultos e machos. No protocolo DM foi realizada a nefrectomia unilateral esquerda (Nefré) no 1º dia, para potencializar o efeito tóxico da hiperglicemia crônica no rim. O DM foi induzido pela administração intravenosa (i.v.) de 65 mg/kg de estreptozotocina (STZ, diluída com citrato) no 20º dia, e o contraste iodado (CI) ioxitalamato de meglumina, 6 ml/kg, foi administrado (intraperitoneal, i.p.) no 85º dia. Foram avaliados parâmetros fisiológicos (ingestão de ração e água, peso, glicemia capilar, peso do rim e peso relativo do rim); a albuminúria (método de imunodifusão), a função renal (FR) (clearance de creatinina, método de Jaffé), a lesão oxidativa (peróxidos urinários-PU, FOX-2; substâncias reativas com o ácido tiobarbitúrico-TBARS, tióis no tecido renal) e análise histológica renal (lesão tubulointersticial). Observou-se que os grupos diabéticos apresentaram polifagia, polidipsia, hiperglicemia e redução do peso corporal (p<0,05), além de redução do clearance de creatinina com elevação de PU e TBARS, manutenção de tióis e elevação da albuminúria. O tratamento com CI nos animais diabéticos determinou redução da FR, elevação dos PU e TBARS e redução dos tióis. Quanto à histologia renal, demonstrou-se que apenas o grupo Nefré+DM+CI apresentou lesão tubulointersticial. Os achados dessa investigação confirmaram o efeito tóxico do CI dose única sobre a função renal de ratos com hiperglicemia crônica, pressupondo que o DM seja fator de risco para essa nefropatia.

Contrast-Induced Nephropathy (CIN) is a toxic acute kidney injury (AKI) that consists in intrarenal vasoconstriction, direct tubular toxicity with generation of reactive oxygen species (ROS). The CIN is associated with the decreased tissue oxygen tension in renal medula in preexisting renal dysfunction, Diabetes Mellitus (DM) and congestive heart failure. This study investigated the pathophysiologic mechanisms in the CIN in diabetic rats. Adult, male, Wistar rats were used. It was performed left uninephrectomy (Nx) on the 1st day in the DM group to potentialize the toxic effect of the chronic hyperglycemia. Nephrotoxicity is the main adverse effect of aminoglycoside use. There are few information about its prevalence and risk factor in intensive care unit patients. Objectives: To assess the prevalence, mortality and risk factors for aminoglycoside nephrotoxicity in ICU patients. Casuistic and Method: In order to assess the prevalence of, and risk factors for aminoglycoside nephrotoxicity in the ICU, 360 consecutive patients starting aminoglycoside therapy in the ICU with a baseline calculated GFR (cGFR) ≥30 ml/min/1.73 m2 were evaluated. Results: Of them, 209 (58%) developed aminoglycoside-induced nephrotoxicity (AKI, decrease in cGFR >20% from baseline), while 151 did not (non-AKI). Both groups had similar baseline cGFR. The AKI group developed a lower cGFR nadir (45 27 vs. 79 39 ml/min/1.73 m2, p<0.001), was older (56 18 y vs. 52 19 y, p=0.033), had a higher prevalence of diabetes (19.6% vs. 9.3%, p=0.007), used other nephrotoxic drugs (51% vs. 38%, p=0.024) and iodinated contrast more frequently (18% vs. 8%, p=0.0054), showed higher prevalence of hypotension (63% vs. 44%, p=0.0003), shock (56% vs. 31%, p<0.0001), and jaundice (19% vs. 8%, p=0.0036). Mortality was 44.5% in the AKI and 29.1% in the non-AKI groups (p=0.0031). A logistic regression model identified as significant (p<0.05) independent factors affecting aminoglycoside-induced nephrotoxicity baseline cGFR<60 ml/min/1.73 m2 (OR 0.42), diabetes (OR 2.13), simultaneous use of other nephrotoxins (OR 1.61) or iodinated contrast (OR 2.13), and hypotension (OR 1.83). Conclusion: The AKI was frequent among ICU patients using aminoglycoside, and it was associated with high mortality. The presence of diabetes, hypotension, simultaneous use of other nephrotoxic drugs, and iodinated contrast were independent risk factors for the development of aminoglycoside-induced nephrotoxicity.

Existem poucas informações sobre a prevalência e os fatores de risco para nefrotoxicidade por aminoglicosídeos em paciente internados em unidades de terapia intensiva. Objetivos: Avaliar a prevalência, a mortalidade e os fatores de risco para nefrotoxicidade por aminoglicosídeos em pacientes internados em Unidade de Terapia Intensiva geral. Casuística e Métodos: Foram avaliados a prevalência, os fatores de risco e a mortalidade da nefrotoxicidade por aminoglicosídeo de 360 pacientes internados em terapia intensiva com filtração glomerular calculada por fórmula (MDRD, RFG) basal ≥30ml/min/ 1,73m2, que iniciaram o uso do antibiótico na terapia intensiva; nefrotoxicidade foi definida como queda >20% na RFG em relação ao RFG basal. Resultados: Entre os pacientes estudados 209 (58%) desenvolveram nefrotoxicidade (IRA) e 151 não alteraram a função renal (não IRA). Injúria renal aguda (IRA) é doença de elevada incidência, associada a altas taxas de morbimortalidade. Sepse, pós-operatório de grandes cirurgias e baixo débito cardíaco são as principais causas de IRA em todo o mundo. Na maioria destas situações, expansão volêmica é parte do manejo preventivo e terapêutico da IRA. Contudo, a manutenção de uma estratégia de infusão liberal de fluidos pode causar balanço hídrico positivo (BH+), que tem sido associado a desfechos desfavoráveis em pacientes criticamente enfermos. BH+ frequentemente ocorre nestes pacientes que recebem grandes infusões de volume, mesmo que apresentem volume urinário considerado satisfatório ou acima de 0,5ml/kg/h. Nesta situação, se não houver elevação da creatinina sérica, não será feito o diagnóstico de IRA pelos critérios do Kidney Disease Improving Global Outcome (KDIGO), ainda que haja claro déficit na eliminação da sobrecarga hidrossalina. Este estudo observacional prospectivo, com controle pareado por dias de exposição ao BH+ avaliou a associação entre BH+ e diagnóstico subsequente de IRA (pelos critérios do KDIGO) e mortalidade em 233 pacientes admitidos em uma unidade de terapia intensiva (UTI) geral. Observamos por análise de regressão logística que cada 100 ml de aumento no BH se associou a elevação de 4% na chance de desenvolver IRA (OR 1,04; IC 95% 1,01 a 1,08). Comparado ao primeiro quartil de BH médio, o quarto quartil de BH médio (BH > +1793 ml/dia) se associou a chance 3,12 vezes maior de desenvolver IRA (OR 3,12; IC 95% 1,13 a 8,65). Comparado ao BH de zero até +1500 ml/dia, o BH médio > +1500 ml/dia se associou a chance 3,4 vezes maior de desenvolver IRA, (OR 3,4; IC 95% 1,56 a 7,48). Um modelo de efeito fixo mostrou que BH+ estava presente pelo menos seis dias antes do diagnóstico de IRA pelos critérios do KDIGO. Para avaliar o desfecho óbito, consideramos o BH durante toda internação na UTI. Observamos que cada 100 ml de aumento no BH se associou a incremento de 7% na mortalidade (OR 1,07; IC 95% 1,02 a 1,12). Comparado ao primeiro quartil, o quarto quartil de BH médio (BH > +1652 ml/dia) se associou a chance 2,8 vezes maior de evoluir para óbito (OR 2,8; IC 95% 1,04 a 7,66). Comparado aos pacientes com BH de zero a +1500 ml/dia, os pacientes com média de BH > +1500 ml/dia apresentavam chance 3,8 vezes maior de evolução para óbito (OR 3,8; IC 95% 1,55 a 9,16). Em conclusão, BH+ como variável contínua, em quartis ou utilizando ponto de corte maior do que +1500 ml/dia se associou de maneira independente a maior chance de desenvolvimento subsequente de IRA e evolução para óbito em pacientes criticamente enfermos. ▼ L’administració precoç d’antibioteràpia apropiada ha demostrat ser la intervenció més eficaç per reduir la mortalitat en pacients crítics amb xoc sèptic i síndrome de disfunció multiorgànica (SDMO). Malgrat la seva rellevància, però, la dosificació antibiòtica en els pacients amb SDMO incloent insuficiència renal aguda (IRA) que requereixen teràpia continua de suport renal (TCSR) encara representa un repte diari pels professionals de la salut. Al nostre medi, els antibiòtics beta[lactàmics d’ampli espectre meropenem i piperacilelina (en combinació amb tazobactam) són els antibiòtics més prescrits a aquests pacients d’altíssima complexitat i gravetat. L'impacte del xoc sèptic, la IRA i la TCSR en els requeriments de dosis d'aquests fàrmacs és vital, ja que tant la pròpia malaltia com les intervencions mèdiques produeixen alteracions significatives en la seva farmacocinètica (FC), que duen a variacions en els perfils concentració[temps i, conseqüentment, comprometen l'assoliment de concentracions del fàrmac dins del rang terapèutic. No obstant això, individualitzar la dosificació de meropenem i piperacilelina en pacients amb xoc sèptic, IRA i requeriment de TCSR és encara molt complex. HIPÒTESI: La dosificació de meropenem i piperacilelina en pacients crítics amb xoc sèptic i IRA que requereixen TCSR és sub[òptima degut a les variacions en el comportament FC dels fàrmacs produïdes tant per la malaltia com pel maneig mèdic d’aquesta. Aquestes variacions FC poden comprometre l'assoliment de concentracions terapèutiques. OBJECTIUS: 1. Avaluar la idoneïtat de les recomanacions actuals sobre dosificació de meropenem i piperacilelina en pacients crítics amb xoc sèptic i IRA que requereixen TCSR; 2. Identificar les fonts de variabilitat que comprometen l’exposició òptima a aquests antibiòtics en la nostra població de pacients; i 3. Desenvolupar noves recomanacions per individualitzar la dosificació d’aquests antibiòtics tenint en compte aquestes fonts de variabilitat. METODOLOGIA: En base a la hipòtesi i els objectius, s’han desenvolupat els tres estudis següents: Estudi 1: Revisió de la literatura. S’ha realitzat una revisió sistemàtica i avaluació crítica de l'evidència disponible sobre la dosificació de meropenem i piperacilelina en pacients crítics amb xoc sèptic, IRA i requeriment de TCSR. Estudis 2 i 3: Caracterització de la FC de meropenem i piperacilelina en pacients crítics amb xoc sèptic i IRA que requereixen TCSR. S’han realitzat dos estudis farmacocinètics multicèntrics, oberts, prospectius observacionals, a les Unitats de Medicina Intensiva de tres hospitals espanyols de tercer nivell. S’han inclòs a l’estudi 30 pacients amb xoc sèptic, IRA i TCSR que rebien meropenem i 19 pacients que rebien piperacilelina. Amb les dades procedents d’aquests pacients, s’han desenvolupat i validat dos models FC poblacionals, a partir dels quals s’han realitzat simulacions de Monte Carlo de diferents esquemes terapèutics (mitjançant el software NONMEM v.7.3®). ▼ IntroduÃÃo: Nas Ãltimas dÃcadas, a insuficiÃncia cardÃaca (IC) emergiu como um problema de saÃde pÃblica mundial. Dados do MinistÃrio da SaÃde de 2006 sugerem prevalÃncia de dois milhÃes de portadores de IC, sendo esta uma das principais causas de hospitalizaÃÃo entre as doenÃas cardiovasculares no Sistema Ãnico de SaÃde. LesÃo renal aguda (LRA) Ã uma complicaÃÃo comum em pacientes admitidos por IC. Portanto, sabe-se que a diminuiÃÃo da funÃÃo renal estÃ associada com o aumento do risco de Ãbito, hospitalizaÃÃes e eventos cardiovasculares com importantes implicaÃÃes socioeconÃmicas. Apesar da terapia avanÃada para IC, o prognÃstico de pacientes com IC continua reservado. Objetivo: Avaliar o biomarcador syndecan-1 na admissÃo hospitalar em pacientes internados por IC descompensada e a sua associaÃÃo com lesÃo renal aguda/crÃnica. Metodologia: Trata-se de um estudo prospectivo observacional com 201 pacientes apresentando IC descompensada no departamento de emergÃncia. O estudo foi realizado em um hospital pÃblico, na cidade de Fortaleza/CE entre abril e setembro de 2013. Foi aprovado pelo ComitÃ de Ãtica em Pesquisa do Hospital de Messejana Doutor Carlos Alberto Studart Gomes. Os pacientes foram comparados com um grupo controle composto por 15 indivÃduos sadios. A anÃlise foi realizada em amostras de soro por meio da tÃcnica de imunoensaio ligado Ã enzima (ELISA) e pelos ensaios bioquÃmicos. A anÃlise estatÃstica foi realizada atravÃs do programa SPSS 19.0 para Windows. Resultados: A idade mÃdia foi 64,2 Â 13,5 anos e a fraÃÃo de ejeÃÃo (FE) calculada foi de 41.5 Â 14.4 no momento da admissÃo. Considerando todos os pacientes, 80 (39,8 %) tinham doenÃa renal crÃnica (DRC) e 62 pacientes (37,8 %) desenvolveram ou pioraram da LRA durante a internaÃÃo. Dos pacientes com DRC, 43 pacientes mantiveram funÃÃo renal estÃvel durante a internaÃÃo hospitalar. A mÃdia de internaÃÃo hospitalar foi de 8,7 Â 4,9 dias e mortalidade intra-hospitalar foi de 5,5%. Os pacientes IC descompensada tinham syndecan-1 sÃrico aumentado na admissÃo hospitalar (133,7 Â 95,0 vs. 18,3 Â 9,2, p<0,001) em comparaÃÃo ao grupo controle. O aumento do nÃvel de syndecan-1 foi observada em pacientes com LRA (mÃdia de 248,7 Â 165.6ng/mL). Pacientes com estÃgio 2/3 de LRA (n =13) apresentaram maior nÃvel syndecan-1 em comparaÃÃo aos pacientes com estÃgio 1 de LRA (n = 63) - 443,2 Â 281,1 vs. 178,9 Â 100,8 ng/mL, p <0,001). A curva ROC para a previsÃo LRA foi 0,741 (IC 95% 0,669-0,812, p <0,001). Os resultados foram ainda melhores quando considerados apenas os graus mais elevados de severidade da LRA (estÃgio2/3) â curva ROC 0,840 (IC 95% 0,733-0,948, p <0,001) Na anÃlise uni variada, a concentraÃÃo de syndecan-1 como variÃvel contÃnua se correlacionou de forma significativa com a mortalidade hospitalar (OR = 1,046 com 95% CI 1,025-1,067, p<0,001 para cada 10ng/mL). Biologija - fiziologija životinja i čoveka / Biology - Animal and human physiology

Akutna bubrežna insuficijencija (ABI) se definiše kao nagli gubitak bubrežne funkcije. Uzroci nastanka ABI su mnogobrojni, a mehanizmi razvoja još uvek nedovoljno jasni. Karakterišu je iznenadna pojava, brzi tok, neizvesna i često loša prognoza krajnjeg ishoda bolesti. ABI je česta pojava kod hospitalizovanih pacijenata, a učestalost njenog nastanka na odeljenjima intenzivne nege se kreće do 30%. Udružena sa drugim oboljenjima, poput hipertenzije, uzrokuje visok stepen mortaliteta. Savremena istraživanja pokazuju da hipertenzija i oksidativni stres imaju važnu ulogu u progresiji bubrežnih oboljenja, što je usmerilo i stavilo akcenat naših istraživanja na uloge ova dva patogenetska faktora u nastanku i progresiji ABI. Sistem renin angiotenzin aldosteron (RAAS) ima značajnu ulogu u održanju krvnog pritiska i homeostatskih mehanizama bubrega. On u velikoj meri reguliše bubrežnu hemodinamiku i vaskularnu reaktivnost, a kako je intrarenalna vazokonstrikcija jedan od glavnih mehanizama razvoja ABI, očita je njegova uloga u patogenetskim mehanizmima nastanka ove bolesti. Oksidativni stres predstavlja disbalans između produkcije reaktivnih vrsta kiseonika (RVK) i sposobnosti biološkog sistema da ih uklanja ili da reparira oštećenja nastala produkcijom ovih molekula. Reaktivne vrste kiseonika su izuzetno reaktivne supstance usled postojanja nesparenih elektrona, i kao takve mogu da uzrokuju oštećenja na DNK, RNK, kao i nekim proteinima koji imaju ulogu u procesu starenja. Pokazano je da oksidativni stres ima veliku ulogu u razvoju mnogih oboljenja, kao što su Alchajmerova i Parkinsonova bolest, i neki kardiovaskularni poremećaji. Takodje je dokazano da, prilikom ishemičnih povreda organa i tkiva, nakon hipoksije, a tokom reperfuzije, dolazi do povećane produkcije molekula RVK. Iz pomenutih razloga, sasvim je izvesno da u ishemičnoj formi ABI povećana koncentracija ovih molekula, doprinosi nastanku i razvoju ovog teškog oboljenja...

▼ Septic acute kidney injury (AKI) is a leading cause of renal impairment in the critical care setting and is independently associated with increased mortality and morbidities. As the knowledge on pathophysiological mechanisms of AKI increases, its clinical management is evolving. The published literature on critically ill septic AKI patients has predominantly come from Caucasian patients. There is little data on the clinical features, management and outcome of critically ill septic AKI in Chinese patients. Results from my single-centred cross-sectional survey (n=3687) showed that septic AKI affected 27% of critically ill Chinese patients. Among those with documented sepsis upon ICU admission or during ICU stay, 71.6% developed AKI. Compared with non-septic AKI patients, septic AKI patients were older, had higher disease severity scores and required more intensive/invasive organ support. Renal replacement therapy (RRT) was used more often in septic AKI (23.3% vs. 12.6% for non-septic AKI, p<0.001), but full or partial renal recovery was also more common (97.5% vs. 93.6%, p<0.001) compared with non-septic AKI. However, the optimal timing for starting RRT among critically ill septic AKI patients remained controversial. My study (n=120) found no significant difference in organ function, defined as Sequential Organ Failure Assessment (SOFA) score changes within 48 hours after the initiation of RRT (improvement by 1.5 points for “early” vs. 2 points for “late”, p=0.523), and mortality (at 28-day 48.4% for “early” vs. 48.3% for “late”, p=0.994) when the initiation of RRT was classified using renal function-based criteria. APACHE score was the only independent predictor for organ function improvement. The application of global assessment tools as RRT initiation criteria warrant further investigation. Blood purification has emerged as an adjunctive therapy in critically ill septic AKI patients as they are effective in clearing endotoxin or inflammatory mediators. Four different techniques were assessed. Firstly, among those septic AKI patients due to gram-negative sepsis (n=30), SOFA score was improved by 37% at 48 hours post initiation of endotoxin/ cytokine adsorptive continuous venovenous haemofiltration (CVVH) compared to a deterioration of 3% among the control group (p=0.013). The treatment was well tolerated. Secondly, among those septic AKI patients with shock due to intra-abdominal sepsis (n=15), Alteco endotoxin haemoadsorption offered no extra benefit over the control group in terms of SOFA score changes at 48 and 72 hours. Thirdly, polymethylmethacrylate (PMMA) based CVVH (n=11) may delay organ function recovery, as assessed by SOFA score, when compared with septic AKI patients with shock who received polysulfone (PS) based CVVH (n=14). The 28-day mortality (55% vs. 21%, p=0.115) showed a trend in favour of the PS-CVVH groups. Finally, among septic AKI patients with shock (n=7), high cut-off CVVH achieved good cytokine modulation. ▼ INTRODUCTION: Despite the technological and pharmaceutical advances made during the last decades, both incidence and mortality associated with acute kidney injury (AKI) in the critically ill patients remain unchanged. Serum creatinine (sCr), which is the primary biomarker of renal dysfunction in use to date, has many disadvantages, such as the time delay in its increase in the serum, and the influence of other factors on its level. Hence, there is a need for identification of better renal biomarkers in order to provide timely intervention for prevention and treatment of acute kidney damage. Two such promising biomarkers are neutrophil gelatinase-associated lipocalin (NGAL) and cystatin C (CysC).PURPOSE: To compare the predictive abilities of admission pNGAL, pCysC, uNGAL, sCr and their combinations for the prediction of the following outcomes: a) Primary outcome: AKI development within 7 days post-ICU admission b) Secondary outcomes: renal replacement therapy need during ICU stay and ICU mortality. Moreover, the predictive ability of the peak value of pNGAL, pCysC and sCr for the aforementioned outcomes will be estimated.METHODS: This was a prospective observational study of adult patients admitted to a 30-bed general Academic Critical Care Department of ‘Evangelismos’ General Hospital of Athens. All consecutive patients admitted to the ICU were screened for eligibility. Exclusion criteria were the following: End stage renal disease, known chronic kidney disease or nephrectomy or renal transplantation, expected ICU stay or imminent death in less than 48 h, transfer from another ICU or HDU, brain death, age < 18 years, inability to draw blood or urine (anuria). The lowest sCr level found within 12 months prior to ICU admission was used as baseline. If it was not available, then the lowest of the on-admission sCr or final ICU sCr was used. AKI was defined according to RIFLE classification. Demographics, co-morbidities, admission diagnosis, outcome measures, disease severity scores and routine laboratory data were recorded on a daily basis. Blood samples for pNGAL, pCysC, sCr and urine sample for uNGAL were collected within 1 h, at the latest, of enrolment. In addition to that, blood samples were obtained 24 and 48 h after admission for pNGAL, pCysC and sCr. pNGAL was measured by Triage® NGAL test, pCysC by MNII nephelometer, while uNGAL by the ARCHITECT platform. Statistical analysis: the following tests were used: multiple logistic regression, ROC analysis, DeLong test for AUCs comparison, Youden’s test, log-rank-test and Cox regression analysis. Introdução: A insuficiência renal (IRen) aguda ou renal crônica podem levar a complicações respiratórias, que podem culminar com insuficiência respiratória aguda (IRpA), severa hipoxemia e alterações de mecânica respiratória (MR). O tratamento hemodialítico, pode desencadear um processo inflamatório pulmonar entretanto, a correção da hipervolemia que proporciona, pode melhorar a IRpA. O objetivo deste estudo foi avaliar a influência da hemodiálise (HD) na MR de pacientes com IRen sob ventilação mecânica invasiva (VMI). Materiais e Métodos: Pacientes com IRen, idade 18 a 75 anos, em VMI e em HD. Parâmetros analisados: clínicos gerais, laboratoriais (hemograma, gasometria arterial, função renal), características da HD e MR (complacência estática e dinâmica, e resistência do sistema respiratório). Os parâmetros foram analisados antes do início da HD e 4 horas após seu início. Resultados: (média ± DP) Foram analisados 37 pacientes, idade 51 ± 17 anos. Houve perda de peso, e melhora dos parâmetros laboratoriais (p < 0,05) exceto a PaO2 e a saturação de O2. A HD não alterou a pressão arterial e nem a MR, porém o delta da complacência dinâmica apresentou correlação com o delta de creatinina (p = 0,02). Conclusão: A HD por 4 horas não altera a MR de pacientes sob VMI.

Introduction: Renal failure (RF), acute or chronic, can induce respiratory complications that can evolve to acute respiratory failure (ARpF), hypoxemia and severe changes in respiratory mechanics (RM). Hemodialysis (HD) can produce pulmonary inflammation and worse the ARpF. On other hand, HD corrects hypervolemia and thus can improve the ARpF. The objective of this study was evaluating the HD role in RM of RF patients in use of invasive mechanical ventilation (IMV). Materials and Methods: Patients with RF, age 18 to 75 years, in use of IMV and HD. Analyzed parameters: age, gender, HD characteristics, respiratory and renal laboratory tests, and RM evaluation (static and dinamic compliance and resistence). Parameters were evaluate before HD and 4 hours after it started. Results (mean ± SD): We studied 37 patients, age 51 ± 17 years. During HD the patients lost body weight and presented an improvement in pulmonary and renal parameters (p < 0.05) except for PaO2 and arterial O2 saturation (p > 0.05). HD did not induce hemodynamic instability. There was correlation (p = 0.02) between the changes in plasma creatinine and the changes in dynamic compliance. Conclusion: HD for 4 hours did not modify respiratory mechanics in patients in use of IMV.

A ventilação mecânica (VM) tem sido recentemente associada ao desenvolvimento de falências orgânicas distais, como um fator contribuinte para a falência renal em pacientes com trauma e fator de risco para diálise e mortalidade em unidade de terapia intensiva (UTI). A estratégia ventilatória adotada pode influenciar estes efeitos. O objetivo do presente estudo é explorar a hipótese de que a estratégia de ventilação mecânica adotada pode influenciar na função renal. Delineamento: Randomizado, investigação animal experimental. Casuística: Ratos machos Wistar, anestesiados, paralisados e ventilados mecanicamente. Intervenção: Dois grupos com seis animais cada foram randomizados para receberem ventilação mecânica com volume corrente (VT) de 8ml/kg (VT8) ou 27ml/kg (VT27). Os parâmetros ajustados para grupo foram: a) VT 8ml/kg; Freqüência respiratória (FR) 60±7 rpm; pressão positiva expiratória final (PEEP) 3 cmH2O; Pico de pressão inspiratória (Pwap) 11.8±2 cmH2O; Pressão média de vias aéreas (Pawm) 6,33±0,22 e b) VT 27ml/kg; FR 30±5 rpm; PEEP 0 cmH2O; Pwap 22.7±4 cmH2O; Pawm 6,50±0,22. Mensurações e Resultados: O grupo VT27 apresentou redução significativa no clearance de inulina após 60 minutos de VM, indicando insuficiência renal aguda (0.6±0.05 ml/min/100g de peso corporal (PC)), e ainda mais acentuada após 90 minutos de VM (0.45±0.05 ml/min/100g de PC) comparada aos valores basais (0.95±0.07 ml/min/100g de PC), p<0.001. Nenhum dos dois grupos sofreram variações significativas em relação as variáveis hemodinâmicas e gasométricas. Conclusões: Observamos que o ritmo de filtração glomerular (RFG) mensurado pelo clearance de inulina é afetado pela estratégia de volume corrente empregado após 60 minutos de VM com 27ml/kg e caindo a valores mais baixo após 90 minutos de VM

Mechanical Ventilation (MV) has been recently associated with development of distal organ failure and it is also a contributor factor for renal failure in trauma patients, and risk factor for dialysis and mortality rate in intensive care unit. The ventilatory strategy adopted might be influenced this effect. The aim of the present study was to explore the hypothesis that mechanical ventilatory strategy may contribute to decreased renal function. Design: Randomized animal laboratory investigation. Subjects: Anesthetized, paralyzed, and mechanically ventilated male Wistar rats. Interventions: Two groups of six rats each were randomized to receive tidal volume of either 8ml/kg or 27 ml/kg. ▼ Racional. A Síndrome de Baixo Débito Cardíaco (SBDC) representa uma etapa evolutiva complexa e grave no contexto fisiopatológico da insuficiência cardíaca. O fluxo sanguíneo regional se altera conforme o território analisado, tendo particular impacto no rim. A instalação da insuficiência renal aguda em pacientes com SBDC é freqüente e representa fator independente de aumento da morbidade e mortalidade. Objetivo. O objetivo primário do estudo foi avaliar o efeito do tratamento precoce sobre a função renal de miocardiopatas com SBDC e disfunção renal baseado em critérios clínicos pré-definidos e monitorização hemodinâmica invasiva. Os objetivos secundários contemplaram o efeito do tratamento sobre o lactato sérico, escore prognóstico APACHE II e os dias de internação em unidade de tratamento intensivo(UTI). Casuística e métodos. O estudo foi prospectivo, randomizado, unicêntrico e incluiu 31 pacientes em SBDC com fração de ejeção do ventrículo esquerdo (FEVE) ao ecocardiograma menor que 35%, pressão arterial média (PAM) menor que 65 mmhg, sinais de má perfusão periférica, diurese(DU) menor que 0,5 ml/kg/h e creatinina sérica maior que 1,4 mg/dl. Constituiu-se 2 grupos: a) tratamento convencional (CV) baseado em protocolo clínico da UTI e b)tratamento objetivo-dirigido (OD) cujos pacientes receberam monitorização invasiva com catéter intra-arterial e catéter de artéria pulmonar, propiciando o controle contínuo por 72 horas com a finalidade de se atingir saturação arterial de oxigênio maior que 95%, pressão de oclusão de artéria pulmonar maior que 18 mmhg, pressão arterial média(PAM) maior que 65 mmhg, hematócrito maior que 30% e saturação venosa de oxigênio maior que 60%. Os dados foram registrados sequencialmente durante 72 horas (6, 12, 24, 48 e 72 h) e realizada observação diária até o 28° dia. Resultados. Na admissão os grupos CV (n=16) e OD (n=15) foram estatisticamente comparáveis no que se refere à idade (49,1±11,2 e 52,0±11,3 anos p= 0,483), sexo (feminino 6,2 e 20% p= 0,33 e masculino 93,7 e 80% p= 0,33), índice de massa corpórea (24,1±1,9 e 23,3±2,1 Kg/h2 p= 0,354), FEVE (24,1±7,1% e 21,6±4,8% p = 0,286), escore APACHE II (14,5±3,1 e 15,6±4,1 p = 0,423), creatinina sérica (1,7±0,3 e 1,7±0,3 mg/dl p=0,354), hemoglobina (13,1±0,9 e 12,4±0,9 mg/dl p=0,066), lactato sérico (19,8±6,2 e 23,7±7,4 mg/dl p=0,666), PAM (71,0±7,2 e 66,9±3,8 mmhg p=0,479), pressão venosa central(PVC) (14,5±4,2 e 14,7±4,8 mmhg p=0,457), DU (0,5±0,4 e 0,3±0,2 ml/kg/h p= 0,257) e diferentes quanto à saturação venosa central (SVcO2) e saturação venosa mista (SVO2) de oxigênio (48±17% e 37±8% p= 0,034). Os registros de 6, 12, 24, 48 e 72 horas para os 2 grupos foram comparáveis e evidenciaram aumento da DU, diminuição da creatinina, adequação dos níveis de PAM, PVC, e aumento das SVcO2 e SVO2. No controle de 72 horas observou-se redução do escore APACHE II, diminuição do lactato sérico e equivalência no tempo de internação na UTI. Conclusões. ▼ Der akute Nierenschaden (AKI, acute kidney injury) ist eine plötzlich auftretende Verschlechterung der glomerulären Filtrationsrate (GFR). Viele unterschiedliche Ursachen sind bekannt. Klassischerweise werden sie in prärenal, renal und postrenal eingeteilt. Seit vielen Jahren ist die Biomarker Forschung beim AKI von großem Interesse. Themen mit besonders hoher Beachtung sind die frühzeitige Diagnosestellung des AKI, die Identifkation von Ursachen des Nierenschadens und die Prognoseabschätzung. In meinen Arbeiten haben wir uns mit der Identifikation solcher Biomarker beschäftigt und dazu unter anderem difference gel electrophoresis (DIGE) als analytische Methode eingesetzt. Wir haben mit DIGE differentiell regulierte Proteine nach herzchirurgischen Operationen im Urin identifiziert, welche auf die Ursache eines Nierenschadens hinweisen. Ausserdem haben wir Marker für die Prognose des AKIs gesucht und insulin-like growth factor binding protein 7 (IGFBP-7) als neuen Biomarker für die renale Prognose identifiziert. In einer weiteren Studie zeigte sich die Kynurenin-Säure als potentieller Marker für die renale Prognose beim AKI. In einer klinischen Studie haben wir die GFR vor und nach perkutanem Aortenklappenersatz beobachtet. Dabei konnten wir feststellen, dass sich bei mehr als 50 % der Patienten die GFR nach Behebung der Aortenstenose verbesserte. Wir konnten mehrere Risikofaktoren für das Auftreten eines AKIs bei diesen Patienten identifizieren, wie den transapikale Zugang, das Auftreten einer Inflammation mit erhöhtem CRP oder erniedrigter Thrombozytenzahl sowie die Transfusion von Erythrozyten. Bei Patienten mit Aortenaneurysma aufgrund eines Marfan Syndroms haben wir mit DIGE eine erhöhte Fragmentierung von Filamin-A festgestellt. Ursache dieser Fragmentierung war eine erhöhte Aktivität der Protease Calpain-2. Zusammengefasst haben wir durch unterschiedliche Methoden Biomarker bei renalen und vaskulären Erkrankungen identifiziert.
