pain remains a significant healthcare issue, delaying discharge and prolonging convalescence periods, resulting in high social and economic costs. Depending on the type of surgical procedure performed, up to half of patients can experience moderate to severe
pain in the early hours and days following surgery. Inguinal hernia repair is one of the most common surgical procedures and is increasingly being performed on an ambulatory (day patient) basis. The growth of ambulatory surgery however, is limited by the ability to provide adequate and satisfactory
pain relief in the early postoperative period. The development of more anatomically relevant animal models of
acute postoperative
pain with improved translational and predictive validity will improve our understanding of the mechanisms mediating this condition and allow for the development of more effective
pain management protocols. The aims of the work presented in this thesis were to develop and characterise a novel anatomically relevant rat model of
acute postoperative
pain associated with inguinal hernia repair, and to investigate postoperative
pain behaviour using in vivo and ex vivo approaches
Under isoflourane anaesthesia, the Lichtenstein inguinal hernia repair technique was employed in male Lister-Hooded rats to model inguinal hernia repair. Post-surgical behavioural characterisation involved extensive investigation of home cage and open field locomotor activity (horizontal and vertical) using the Opto M3 infra-red beam break system. Post-surgical mechanical hypersensitivity using von Frey filaments was also assessed. Bupivacaine, ibuprofen, morphine, carprofen and paracetamol were all investigated for their ability to attenuate surgery-induced deficits in locomotor activity. Further studies interrogated the affective dimension of
acute postoperative
pain using the place escape avoidance paradigm (PEAP). Immunohistochemistry and liquid chromatography coupled with tandem mass spectrometry (LC/MS-MS) were used to investigate the molecular and neurochemical mechanisms underlying the postoperative behavioural phenotype.
Inguinal hernia repair surgery resulted in robust and reproducible deficits in horizontal and vertical locomotor activity in the home cage and open field in the early postoperative period. There was some evidence of slightly increased mechanical hypersensitivity of the inguinal area following surgery, though it was not as robust or reproducible as the locomotor activity deficits. Morphine, carprofen and paracetamol were all capable of attenuating these deficits at discrete time points following surgery. The studies also demonstrate for the first time, an affective component of
acute postoperative
pain. Furthermore, surgery was associated with increased c-Fos expression in the dorsal horn of the spinal cord and altered levels of endocannabinoids and N-acylethanolamines (NAEs) in the amygdala thalamus and PAG, which may contribute to the observed postoperative behavioural phenotype.

Worldwide incidence of adult thermal burns is 3-10%. Thermal burns cause significant tissue
injury leading to acute pain which if not adequately controlled leads to delayed wound
healing, prolonged hospital stay and psychological disturbances like depression and
posttraumatic stress disorder. There is paucity of literature concerning burn pain assessment
and adequacy of its control in our setup. This study aimed to assess the adequacy of pain
control in adult patients with thermal burns admitted to KNH.
Methods:
A descriptive study with a sample size of 138 adult patients admitted to KNH due to thermal
burns were recruited for this study. Study duration was six months from February to July
2015. The data collected was demographic data, aetiology of burns, pain scores using Visual
Analogue Scale, degree of burn and mode and type of analgesics offered. Data was analysed
using STATA v.11.2, frequencies, means, medians were used to describe data. Student’s ttest
& chi-square were used to test for statistical significance with p-value of < 0.05 showing
statistical association.
Results:
Median age of the study population was 28 (IQR 22-38), majority were males 65%. Sixtyfive
percent sustained moderate to major burns. Etiologies were mainly flame and scald each
contributing 35% respectively. Pain assessment was not done in majority at A&E and wards
98% & 95% respectively. The correlation between TBSA and intensity of pain and degree of
burn and intensity of pain was not statistically significant. Pain control at KNH was found to
be inadequate with only 17% having adequate background control of pain and 7% having
adequate procedural pain control. There was judicious use of analgesics at A&E (96%) but
this not a common practice during change of dressing (29%). Mostly the mode of offering
analgesia was unimodal 77% at A&E and during dressing. The choice of analgesics was
opioids in both places.
Conclusion:
Pain assessment is not a common practice at KNH hence its management was very arbitrary.
Pain control is inadequate amongst patients with thermal burns at KNH. These findings are
contrary to the recommendation from other burn centres. Judicious use of analgesics is
recommended to control procedural pain, but at KNH only 26% of the patients received
analgesics during change of dressing yet the VAS scores remained at moderate pain to severe
pain despite the use of procedural analgesics.

▼ Pain recognition and management in horses has become more important in recent years. Interest in the animal’s welfare has increased. Horses are prey animals, and therefore will not easily show their
pain. This study attempted to create and validate to objectively score and interpret
pain in horses with
acute colic. We have used an adapted Composite
Pain Scale (CPS) system previously developed and tested by others on horses with orthopedic or post-operative somatic
pain.
Eight horses with
acute colic were scored using the colic CPS and these scores were compared to 18 healthy
pain free horses (farrier clients and recipient mares). Two observers scored the animals, and scores were compared between observer 1 and observer 2 in order to determine the CPS’s reliability. Videos of all horses were scored by other veterinarians using a Visual Analogue Scale (VAS) after which these scores were compared to the scores given using the CPS.
Animals not experiencing any
pain were shown to score low, whereas horses with
acute colic
pain scored significantly higher on the CPS.
Pain scores for colic patients decreased over time during their stay at the clinic. Scoring based on the CPS was significantly more reliable between observers than scoring performed with the VAS.
This study found that the CPS system could become a reliable method to use in the objective assessment of
pain in horses with
acute colic, and therefore benefit clinicians as well as owners and caretakers.

▼ Diverse behavioural cues have been proposed to be useful indicators of infant
pain, but there is a paucity of evidence based on formal psychometric evaluation to establish their validity for this purpose. We aimed to examine two widely-used
pain scales, the Neonatal Facial Coding System (NFCS) and the Modified Behaviour
Pain Scale (MBPS), by examining their factor structures with factor analysis. The results indicated that an item reduced NFCS scale with three items produced a one-factor
pain model that maintained the good psychometric properties of the 7-item scale. In addition, it was found that MBPS also has challenging internal consistency, with items that are weakly correlated and highly redundant. Redefinition of MBPS with a single indicator was suggested. This analysis provides new iterations of NFCS and MBPS that improve construct validity and internal consistency. These versions also increase the feasibility of both measures and improve their potential for clinical use.

Acute gout typically presents as an extremely painful intermittent arthritis affecting the foot. The impact of
acute gout flares on musculoskeletal function is not well described. The aim of this study was to evaluate the impact of
acute gout on foot
pain, impairment and disability.
Design: Prospective observational study.
Methods: A total of 20 patients (17 males, 3 females, mean age of 54.4 years) were recruited from hospital wards and rheumatology outpatient clinics within Auckland and Counties-Manukau District Health Boards. Patients were recruited at the time of an
acute flare (baseline visit) and then reassessed at a follow-up visit after the flare had resolved 6-8 weeks later. Clinical characteristics including tender joint count, swollen joint count, patient global assessment, C-reactive protein and serum urate were assessed at both study visits. General and foot-specific outcome measures were used to assess
pain, impairment, function and disability. These included the Health Assessment Questionnaire (HAQ-II), Foot Function Index, Lower Limb Tasks Questionnaire and the Leeds Foot Impact Scale. Data was analysed using paired t-tests.
Results: At the baseline assessment, 14 (70%) of patients were suffering from
acute flares affecting the foot.
Acute flares were associated with high levels of
pain, impairment and disability. All measures of
pain, impairment, function and disability displayed improvement at the follow-up visit.
Pain and disability scores did not return to normal levels after the resolution of the
acute flares.
Conclusion:
Acute gout is associated with high levels of foot
pain, impairment and disability. This suggests the management of gout needs to be improved, particularly foot related health.

Uvod: Pored antinocicepcije, opioidi ispoljavaju
značajne efekte i na telesnu temperaturu. Brojni dokazi podržavaju
postojanje endogenog glutamatergičkog sistema koji moduliše telesnu
temperaturu preko N-metil-D-aspartat (NMDA) receptora. Ketamin i
magnezijum, NMDA antagonisti, su poznati po svojim anestetičkim,
anagletičkim svojstvima, kao i dejstvu protiv drhtavice. Takođe,
oni mogu povećati antinociceptivne efekte opioidnih analgetika u
različitim modelima bola kod životinja, kao i kod ljudi. Cilj
ispitivanja: Cilj ovog ispitivanja bilo je poređenje
antinociceptivnih i hipertermnih odgovora između dve grupe agonista
μ-opioidnih receptora: derivata fentanila (4-anilinopiperidinski
tip) i morfina (fenantrenski tip) kod pacova. Takođe, ova studija
imala je za cilj ispitivanje efekata ketamina i magnezijum sulfata
na telesnu temperaturu i akutni nociceptivni bol kod pacova, da
utvrdi tip interakcije između ova dva leka i da ispita da li
magnezijum sulfat dodat ketaminu ili kombinaciji morfin-ketamin,
povećava analgetički efekt ovih lekova i njihov efekt na telesnu
temperaturu. Metode: Analgetička aktivnosti procenjivana je testom
potapanja repa mužjaka Wistar pacova (200-250 g) u toplu vodu.
Distalnih 5 cm repa stavljano je u toplu vodu (55 ± 0,5°C) i kao
odgovor mereno je vreme za koje životinja povuče rep iz tople vode.
Telesna temperatura je merena stavljanjem termometarske sonde u
dužini od 5 cm u debelo crevo. Rezultati: Fentanil, (±)-cis-3-metil
fentanil (CM), (±)-cis-3-karbometoksi fentanil, (±)-
trans-3-karbometoksi fentanil i (±)-cis-3-butil- fentanil, kao i
morfin, oksikodon (O), tebakon i 6,14-etenomorfinan-7-metanol,
4,5-epoksi-6-fluoro-3-hidroksi-,,17-trimetil-, (5,7) izazivali
su dozno-zavisno povećanje antinocicepcije i hipertermije. Derivati
morfina (fenantrenski tip) i fentanila (4-anilinopiperidinski tip)
izazivali su hipertermiju kod pacova u dozama koje su oko 2 puta
niže i 6-11 puta više od njihovih srednjih antinociceptivnih
doza...

▼ Background: Pain is common in hospitalized patients. Despite the availability of various treatment methods and effective drugs foroptimal pain alleviation, research has revealed that pain treatment remains suboptimal. Aim: e overall aim was to investigate the prevalence and treatment of pain in hospitalized patients, and whether interventions can improve pain management. Methods: A combination of methods was employed for data collection and analysis. Study I is a quantitative cross-sectional study. An intervention comprising the use of pain guidelines, education and nurses with responsibility for pain management was implemented between baseline (n=) and follow up (n=). Study II is a cross-sectional study that examined the occurrence and treatment of pain in hospitalized children and adolescents aged - years. Study III is a qualitative interview study where content analysis was employed. Out of adult patients who had undergone orthopaedic surgery, nine received oral information and nine both oral and written information about pain treatment. Study IV is a controlled intervention study comprising patients with cancer pain, half of whom were allocated to a control group and the other half to an intervention group. A multidimensional pain questionnarie was used in which the patients rated their pain. After the baseline measurements, the completed questionnaires were given to the care team, while only the researchers had access to those of the control group. e follow up measurement took place - days later. Results: Study I revealed significant differences in terms of increased use of pain rating and improved drug prescription. Study II highlighted deficiencies in pain rating and drug prescription in hospitalized children and adolescents. e children reported that drugs as well as different non-pharmacological methods provided pain relief. Study III demonstrated that the patients perceived that oral and written information provided them with knowledge that enabled involvement in their own pain treatment, whereas oral information alone was difficult to remember and use postoperatively. Study IV showed a significantly lower pain intensity and less influence on other pain-related dimensions and symptoms, as a result of drug adjustments in the intervention group. Conclusion: e present thesis contributes knowledge about the occurrence and treatment of pain in hospitalized patients. Interventions can facilitate improved pain management, both in terms of pain rating and treatment. Improved pain treatment can reduce pain and lead to less impact on other dimensions and symptoms. More research in the area is needed to design several interventions that can alleviate pain, increase knowledge and raise awareness of the need for greater focus on ethical and humanitarian aspects of caring for patients with pain.

Pain is a leading cause of disability and the most common reason for clinical care. The field of pain research has focused on sex differences in the recent years with an expansive body of literature demonstrating sex-related differences in pain behavior and responsiveness to pharmacological interventions. Prescription opioids are potent analgesics and the mainstay for the clinical management of moderate-to-severe acute and chronic pain conditions. However, the long-term clinical use of prescription opioids for chronic pain remains controversial due to concerns about severe adverse effects, including tolerance, dependence, and addiction associated with opioid use. The non-medical use and abuse of prescription opioids has become a public health crisis, the problem even arising in a subset of chronic pain patients receiving opioid therapy. The vulnerability factors, specifically the role of pain in the propensity to prescription opioid abuse, are poorly understood. The present research project sought to investigate the propensity to opioid reward as a function of pain in male and female mice by incorporating acute (acetic acid-induced) visceral nociceptive and chronic chemotherapy (paclitaxel)-induced peripheral neuropathic pain models. Sexually dimorphic variations in the sensitivities of mice to nociceptive and allodynic behaviors were initially assessed using the two putative pain models. Following that, the two prescription opioids, morphine and oxycodone were examined under both pain contexts and the capacity of the two prescription opioids to produce reward-related behavioral effects were measured using drug discrimination, conditioned place preference, and intravenous drug self-administration procedures. The presence of acute noxious state but not chronic pain selectively attenuated the discriminative stimulus effects of the prescription opioid, morphine in male mice. The magnitude of modulation of the stimulus effects of opioids by the acute noxious state were further observed to be inversely related to the relative intrinsic antinociceptive effectiveness of the two opioids in reversing the acute noxious state and sex-specific sensitivities of mice to opioid-induced antinociception. In contrast, while no change was observed in opioid-reward as a function of the acute noxious state in both sexes, the presence of paclitaxel-induced chronic pain opioid-selectively and dose-selectively enhanced the conditioned rewarding effect of morphine (0.3 mg/kg dose), and the effect was more pronounced in male relative to female mice. These data were further supported by the self-administration results, in that the reinforcing efficacy (breakpoints under progressive ratio (PR) responding) and the incentive-motivational salience of morphine significantly increased in the presence of chronic pain in male mice, while non-selectively increasing regardless of the presence/absence of pain in female mice. children requiring painful procedures to diagnose and treat their
illness, management of this pain must be a priority for nurses. The
role nurses assume in the current undermanagement of children's
pain requires further examination. In the first paper, a
comprehensive review of the available literature on pediatric pain
management was conducted in order to provide the context in which
this issue is situated. The second paper is a qualitative inquiry
seeking nurses' accounts of the individual level factors they
identify that influence their choices for distraction to manage
children's procedural pain. Nurses described the three key
determinants of nursing knowledge, experience and relational
capacity as influencing their practice. These descriptions provided
an extended understanding on nurses' choices for using distraction
to manage children's procedure-related pain. Nurses disclosed using
distraction for themselves, as well as for the child experiencing a
painful procedure.

Introduction: Pain is one of the main reasons why older people (65 years and over) present to hospital. They have the highest rate of admission to hospital involving a multitude of medical and/or surgical procedures and many are associated with acute pain. Older people also often present with histories of existing and multiple pain sources and types. The most common reason for unrelieved pain in acute care settings is the failure of health care providers to systematically assess and treat pain. Pain affects an older person’s health, their ability to function at their optimal capacity both cognitively and physically, and also reduces their quality of life. Older people who are in pain are at risk of sleep disturbances whilst they are an inpatient as well as delayed rehabilitation. Furthermore, it has been shown that an older person who has a higher postoperative pain score, will have a longer stay in hospital and this can lead to chronic functional impairment. Methods: The aims of the study were to explore the culturally mediated practices of registered nurses (RN’s) in acute care settings when assessing and managing pain in older people and to explore the culturally mediated barriers and facilitators to practice. A Qualitative research design was used underpinned by Focused Ethnography. Methods included 1,040 hours in the field with 73 hours of focused observations of nurses (n=9) who cared for older patients (n=42) conducted over a 6 month period. Semi-structured interviews were also held with nurses (n=23) and older patients (n=12). Documents including ward policies, procedures, patient histories and medical notes were also reviewed. The settings for the study were eight wards: medical, surgical, sub-acute and oncology, of two large tertiary referral hospitals in New South Wales Australia. Data were analysed in a range of ways beginning with a qualitative descriptive approach directed by the undertaking of taxonomies, typologies guided by the work of Spradley (1980) and the use of Leininger’s culture care theory (1988), before a content analysis and thematic analysis. Analysis of data showed that when caring for older patients with pain, nurses showed leadership by acting as problem solvers around pain issues and concerns, that they mentored new nurses in relation to pain care, they questioned them about the rationales for their approaches and they provided education to staff, albeit within the limitations of their own knowledge. There were tales of success, how they assessed and rated pain, pain that they acknowledged as real and genuine, and stories of how quality improvement audits shaped what they did. Older patients talked of not being heard by nurses when in pain, of enduring pain, of having to retell their pain histories often, of being moved around from ward to ward, and of being a good patient or an unpopular patient. Low back pain is a common complaint and has the highest global disability burden when measured as years lived with a disability. After an episode of low back pain, up to two-thirds of people will experience variable levels of chronic pain after one year and around 10% will be significantly disabled in association with low back pain. Recent research has revealed that people with chronic low back pain are characterised by widespread pain hypersensitivity, suggesting that neuroplastic changes at the central nervous system underlie this condition. While this knowledge has enhanced our understanding of pathophysiological processes in chronic low back pain, it is currently unclear how early these somatosensory changes develop. Therefore, the broad aims of this thesis are: to investigate the time course of somatosensory changes from the acute stage of low back pain without serious pathology; to examine the prognostic utility of this information in low back pain; and to address methodological aspects of such assessment using quantitative sensory testing (QST).
In order to meet these aims, several research approaches have been undertaken. Two systematic reviews of the literature were carried out to establish whether somatosensory changes are a feature of acute low back pain compared to healthy controls (Chapter 2) and to investigate the prognostic ability of QST in low back pain (Chapter 5). An inception cohort study, using a comprehensive QST assessment, was carried out to inform whether early somatosensory changes can be detected soon after low back pain onset compared to pain-free individuals (Chapter 3). The assessment included evaluation of endogenous pain modulation (Chapter 4) and tracked changes in somatosensory function over time, until 4 months after onset (Chapter 7). This comprehensive data set has also enabled the evaluation of important methodological issues related to the stability of QST over time in healthy individuals (Chapter 6).
Overall, the work presented in this thesis has contributed to the body of evidence regarding the evaluation of somatosensory function in the early stages of low back pain, as well as providing novel methodological insights into QST testing. This scholarly work has specific implications for clinicians and researchers addressing low back pain, the condition of highest disability burden worldwide.

an important global issue, and a phenomenon of oligoanalgesia,
unfortunately, is also common. Different researchers conducted
studies and suggested different ways to eradi¬cate this phenomenon.
However, the question why patients with acute abdominal pain are
treated inadequately, even with clear evidence that the treatment
will not cause any adverse effects so far remains unanswered.
Researchers of this study have hypothesis that the well-established
local tradition or people's attitudes can lead to the fact that
evidence-based guidelines are not fully applied in practice. This
study investigates clinical characteristics of pain in patients
with acute undifferentiated abdominal pain and to identify the
factors influencing effective management of acute abdominal pain in
the ED at university hospital. Objectives of the study 1. To
investigate clinical characteristics of pain in patients with acute
undifferentiated abdominal pain upon arrival and departure from the
ED at university hospital. 2. To evaluate the trends of acute
abdominal pain relief in the ED at university hospital and to
compare them with current guidelines. 3. To identify differences in
subjective pain intensity rating between patients with abdominal
pain and assessment of nurses and doctors. 4. To investigate effect
of strong analgesic drugs (morphine) for acute abdominal pain
management in the ED and compare their efficacy versus NSAIDs
(diclofenac) or placebo.

Measuring pain in infants is important but challenging for researchers and health care professionals. The measurement of skin conductance (SC) is considered as a measure of stress and a surrogate indicator of pain.
Purpose
This dissertation provides insight on the validity of SC for pain measurement in infants and consists of two studies: 1) a scoping review synthesizing the methods and findings of previous studies on validating or using SC for measuring pain in infants; 2) a primary study evaluating the validity of SC for measuring pain in mechanically ventilated infants.
Methods
Arksey and O’Malley’s framework informed the methods of the scoping review. Nine electronic databases were searched. Data were analyzed and presented descriptively. The primary study used a prospective cross-sectional observational design. Eligible infants were those up to 12 months of age, hospitalized in intensive care units, who were mechanically ventilated, and required painful and non-painful procedures.
Results
Scoping review: Twenty-eight studies with 1061 infants were included, including 23 cross-sectional observation studies and five interventional studies. The validity evidence of SC was tested in relation to referent pain measures (13 variables), stimuli (13 variables), age (2 variables) and other contextual variables (11 variables). Fifteen studies evaluated the validity evidence in relation to phase of painful procedure, and SC increased significantly during painful procedures in most studies (n=14/15). However, inconsistent findings on other validity evidence and wide variation in methods existed across studies.
Primary study: SC showed good validity in relation to the category of procedure, the phase of procedure and the referent pain measures in critically ill mechanically ventilated infants. The findings from diagnostic test accuracy showed that SC had good capacity of detecting moderate to severe pain. However, the values of SC need to be used with caution, due to the imperfect correlations with the referent pain measures and imperfect positive predictive value.
Conclusions
SC is a promising approach to measuring pain in critically ill infants. Further research testing the validity of SC in relation to pain treatments and advancing the technology of measuring and analyzing SC is needed before it can be recommended for clinical use.

pain 24 hours post-breast cancer surgery (BCS). Before surgery 86 women completed demographic, health status, and psychological questionnaires and blood was drawn to measure baseline cytokine levels. Numeric Rating Scale-Rest (NRS-R), NRS-Movement (NRS-M), Short-Form McGill
Pain Questionnaire (SF-MPQ) and Short-Form Neuropathic
Pain Questionnaire (SF-NPQ) were completed 24 hours post-BCS. Backward regression models found significant correlates for NRS-R: younger age, increased
pain catastrophizing and bilateral surgery; NRS-M: younger age, increased trait anxiety, bilateral surgery, and mastectomy; SF-MPQ: increased
pain catastrophizing, bilateral surgery, and previous breast surgery; and SF-NPQ: decreased interleukin-10 and increased
pain catastrophizing. These results support the biopsychosocial model of
pain and the importance of measuring multiple
pain outcomes. Variables accounting for the most variance in each outcome (
pain catastrophizing [NRS-R; SF-MPQ], trait anxiety [NRS-M] and baseline IL-10 [SF-NPQ]) are potentially modifiable.

▼ Pain is the universal symptom of illness and trauma. It affects people of all ages, cultures, and backgrounds, causing distress and suffering. Appearing in a plethora of diagnoses, almost all patients will experience some type of
pain as a related symptom during their lifetime. The ubiquitous nature of
pain renders it likely that a wide variety of healthcare providers will treat patients reporting
pain in both the
acute care and longitudinal settings. Many institutions and governing bodies in the medical sphere have emphasized the duty of the medical field to treat
pain and thereby alleviate suffering.
It is common for Emergency Medicine (EM) physicians to manage patients presenting to the Emergency Department (ED) in
pain. Although these patients’ etiologies for their
pain may differ, most will be experiencing
pain from an
acute insult. Emergency Medical Services (EMS) is the extension of EM into the prehospital setting. As such, EMS providers interact with many of the same patients experiencing
acute pain. Despite the prevalence of
pain and the importance of alleviating it,
acute pain management has often been inadequate. Improving
pain management should continue to be a high priority.
Opioid analgesics have long been the standard of care for
acute pain management. The first opiate, morphine, was isolated in the early 1800’s. Opioids are potent analgesics and are titratable to effect. However, they have a significant adverse effect profile. Among other adverse effects, opioids can cause hypotension and respiratory depression. In addition, the United States opioid epidemic has placed increased pressures on EMS and the entire healthcare profession to utilize opioid alternatives while continuing to improve the quality of
acute pain management provided to patients. As a result, non-opioid analgesics have gained increased attention and use in EMS. They generally have fewer adverse effects than opioids and are not typically associated with a potential for addiction and abuse. However, the individual and subjective nature of the
pain experience increases the difficulty of achieving improved analgesia.
EMS providers must weigh these various factors and the complexity of the
pain experience when determining the most appropriate treatment for
acute pain. This review seeks to determine if non-opioid analgesics have potential for use as first line treatment by EMS over opioid analgesics, the standard of care for
acute pain management. The purpose of this review of the current literature, especially comparison studies, is to investigate the common EMS analgesics: morphine, fentanyl, acetaminophen, ketorolac, ibuprofen, nitrous oxide, methoxyflurane, and ketamine. The findings are discussed in relation to four important outcome measures identified: effect on
pain severity, rescue analgesic use, patient satisfaction, and the consideration of risks. Due to the paucity of research on this important topic, a general recommendation cannot be made for the use of non-opioid analgesics as first line treatment for

Smärtbedömning är en viktig del i smärtbehandlingen och kan göras med hjälp av observationer av fysisk status och smärtskattningsinstrument. Eftersom ett stort trauma även innebär en psykologisk påfrestning för patienten är det viktigt att lindra både smärtan och det psykiska lidandet. Syfte: Att beskriva ambulanssjuksköterskors uppfattningar om traumapatienters upplevelser av smärta samt beskriva ambulanssjuksköterskors erfarenheter av smärtbehandling och omvårdnadsåtgärder som lindrar lidande hos dessa patienter. Metod: En intervjustudie med elva ambulanssjuksköterskor. Intervjuerna analyserades med kvalitativ innehållsanalys. Resultat: Ambulanssjuksköterskans arbete vid smärtlindring vid stort trauma präglades av avvägningar och prioriteringar. Dessa gjordes utifrån ambulanssjuksköterskans egna bedömningar samt de riktlinjer som fanns inom ambulanssjukvården. Omvårdnadsåtgärder var lika viktiga som de farmakologiska åtgärderna, i arbetet för att lindra patientens smärta och lidande. Majoriteten av ambulanssjuksköterskorna upplevde smärtan vid stort trauma som underbehandlad. Detta på grund av otillräckliga läkemedelsdoser men också på grund av prioritering av livsuppehållande åtgärder. Slutsats: Studiens resultat visar att ambulanssjuksköterskans arbete stämmer väl överens med kompetensbeskrivningen. Smärtlindring vid stora trauman är komplext och de upplevda hindren är många. Ambulanssjuksköterskans egen bedömning, med utgångspunkt från gällande riktlinjer, är kärnan i deras arbete med traumapatienter. Smärtlindring vid stora trauman är ett viktigt ämne och ett relativt outforskat område. Det är därför angeläget med vidare forskning.
Introduction: Research shows that pain at major traumas are poorly treated, which can lead to a prolonged course of the disease and lead to complications for the patient. Pain assessment is an important part of pain treatment and can be done either through observations of the patient’s physical status or via pain assessment tools. A major trauma involves also a psychological stress for the patient, therefore it’s important to relieve both the physical pain as well as the mental suffering. Aim: To describe ambulance nurses’ perception regarding trauma patients’ experiences of pain and to describe ambulance nurses’ experiences of pain treatment and nursing care that ease the suffering for these patients. ▼ Socioemotional selectivity theory posits that as people age, they become motivated and successful at maximizing positive emotions and minimizing negative ones. Yet, 70% of older adults report physical pain, which is associated with negative affect. The strategies and resources that older adults use to maintain positive affect in the face of pain remain largely unknown. Specific positivity-enhancing strategies include recalling, recognizing, and responding to positive stimuli and prioritizing close over knowledgeable social partners. Executive functions (EF, i.e., task-switching, working memory, and inhibition) and heart rate variability (HRV) may be important resources for coping with pain. The current project used two studies to test whether older adults used positivityenhancing strategies and maintained emotional wellbeing following pain more than younger adults; associations with EF and HRV were also investigated. In Study 1, 50 older and 50 younger adults experienced a control and a pain condition, were given the chance to employ positivity-enhancing strategies, and provided EF and HRV data. Study 2 used longitudinal data from community-dwelling older adults (n =150) to test whether task-switching moderated the within-person relationship between pain and wellbeing. In Study 1, after the pain condition, younger adults demonstrated lesser preference toward knowledgeable social partners than older adults (γ = -0.15, p = .016). No other age group x pain condition x valence interactions were found. Older and younger adults did not differ in changes in positive or negative affect following pain. Task-switching and HRV were both associated with reduced preference for knowledgeable social partners following pain, but no other significant EF or HRV interactions were found. Study 2 failed to support the hypothesis that task-switching protected against pain-related declines in wellbeing. Future research on strategies that older adults use to maintain emotional wellbeing in the face of pain is needed.

pain qualities and neuropathic
pain 24 hours post-breast cancer surgery (BCS). Before surgery 86 women completed demographic, health status, and psychological questionnaires and blood was drawn to measure baseline cytokine levels. Numeric Rating Scale-Rest (NRS-R), NRS-Movement (NRS-M), Short-Form McGill
Pain Questionnaire (SF-MPQ) and Short-Form Neuropathic
Pain Questionnaire (SF-NPQ) were completed 24 hours post-BCS. Backward regression models found significant correlates for NRS-R: younger age, increased
pain catastrophizing and bilateral surgery; NRS-M: younger age, increased trait anxiety, bilateral surgery, and mastectomy; SF-MPQ: increased
pain catastrophizing, bilateral surgery, and previous breast surgery; and SF-NPQ: decreased interleukin-10 and increased
pain catastrophizing. These results support the biopsychosocial model of
pain and the importance of measuring multiple
pain outcomes. Variables accounting for the most variance in each outcome (
pain catastrophizing [NRS-R; SF-MPQ], trait anxiety [NRS-M] and baseline IL-10 [SF-NPQ]) are potentially modifiable.

▼ Cardiovascular indices of
pain are pervasive in the hospital setting. However, no research has examined the development of cardiac responses to acutely painful procedures in the first year of life. Our main goal was to synthesize evidence regarding the development of cardiovascular responses to acutely painful medical procedures over the first year of life in preterm and term born infants. A systematic search retrieved 6994 articles to review against inclusion criteria. A total of 41 studies were included in the review. In response to acutely painful procedures, most infants had an increase in mean heart rate (HR) that varied in magnitude both across and within gestational and postnatal ages. Research in the area of HR variability has been inconsistent, limiting conclusions. Accordingly, longitudinal research is needed to further understand the inherent variability of cardiovascular
pain responses across and within gestational and postnatal ages, and the causes for the variability.

▼ Pain is the most conspicuous aspect of sickle cell disease (SCD) to those that are affected that even African tribes have been calling it onomatopoetic names describing its
pain for at least hundreds of years. The
acute pain crisis is the hallmark symptom of SCD and the leading cause of hospital admissions among patients. It is an unpredictable event that may have some triggers including stress, dehydration, weather, and infection, but occurrence is very heterogeneous. Chronic
pain in sickle cell disease is typically caused by bone infarction, avascular necrosis, chronic leg ulcers, among others.
Pain heterogeneity is seen in patients reporting baseline
pain and this variability is also not completely understood in SCD. Race, gender, environment, socio-economic status, trauma, and stress contribute to the variability in
pain seen between individuals, however,
pain sensitivity and risk are complex traits that are up to 30-76% heritable and polygenic. Several studies have observed the genetic components that contribute to phenotypic diversity in SCD.
Pain is a complex phenotype that has multiple contributions made by genes and the environment. In this study, candidate
pain single nucleotide polymorphisms and
pain genes are prioritized to further investigate the role multiple genes may play in
pain variability. Particular focus is on the monoamine neurotransmitter system because of its high relevance and comorbidities seen in
pain and other neurological disorders. Each candidate SNP is analyzed to ascertain individual contributions, and then a polygenic approach is taken to observe the contributions made to a complex phenotype.

Akut smärta beskrivs som subjektiv och individuell vilket styrker nödvändigheten av ökad kunskap och förståelse för akut smärta och patienters olika upplevelser av den. Detta skulle kunna generera i en bättre omvårdnad, där utvecklingen från akut smärta till en kronisk långvarig smärta samt ett lidande för patienten skulle kunna minimeras. Syfte: Syftet var att beskriva patienters upplevelse av akut smärta och vårdpersonalens betydelse i samband med den. Metod: En litteraturöversikt som baseras på 13 artiklar, sju kvalitativa och sex kvantitativa. Resultat: Fyra teman identifierades; "Vikten av adekvat smärtbehandling", "Vårdpersonalens kompetens inger trygghet", "Att underkasta sig vårdpersonalen" samt "Betydelsen av kommunikation och information". Konklusion: Akut smärta är fortfarande ett stort problem och alldeles för många patienter lider av akut smärta. Vårdpersonalen har stor betydelse för dessa patienters upplevelse då det kan lindra smärtan och förebygga onödigt lidande. För att förbättra vården för patienter med akut smärta behövs en ökad förståelse och kunskap för deras subjektiva upplevelser men även betydelsen av hur sjuksköterskan agerar, behandlar och utvärderar är viktig.
Background: Pain is one of the most common reasons why patients seek healthcare. Acute pain is described as subjective and individual, which proves the need for greater knowledge and understanding of the acute pain and the patient's different experiences of it. This could generate in a better care, where the development of acute pain to chronic prolonged pain and the suffering of the patient could be minimized. Aim: The aim was to describe the patients’ experience of acute pain and the significance of health care professionals associated with it. Method: A literature review based on 13 articles, seven qualitative and six quantitative. Results: Four themes where identified; "The importance of adequate pain management",”To submit to the health care professionals”, “How the health care professionals’ competence incites a sense of safety” and ”The significance of communication and information”. Conclusion: Acute pain is still a major problem and too many patients suffers from it. The health care professionals have a great importance for these patients’ experience since they can ease the pain and prevent unnecessary suffering. To improve care for patients with acute pain, a greater understanding and knowledge of their subjective experiences is required but also the significance of how the nurse act, treats and evaluates should be of importance.

Metod: Litteraturstudie med ett systematiskt arbetssätt. Resultat: Val av smärtskattningsinstrument: VAS och NRS är tillförlitliga instrument för att bedöma akut smärta. Professionell smärtbehandling: Användandet av smärtskattningsinstrument förkortade tiden till smärtbehandling samtidigt som fler blev smärtbehandlade. Upplevd kontra tolkad smärta: Sjuksköterskor underskattade ofta patienters akuta smärtintensitet. Kontinuerligt dokumenterade smärtanalyser: För att tillgodose att patienter får en så bra smärtbehandling som möjligt krävs att smärtanalyser utförs och dokumenteras vid inskrivning och kontinuerligt till utskrivning. Med regelbunden smärtskattning och dokumentation kan patienters smärtutveckling och eventuella behandlingsresultat följas. Slutsats: Förutsättningen för en adekvat smärtbehandling är att en smärtanalys utförts med ett smärtskattningsinstrument. Användandet av smärtskattningsinstrument leder till att patienter får adekvat smärtbehandling fortare. Smärtskattningsskalor är goda redskap för att bedöma smärta. En metod för att förbättra smärtbehandlingen kan vara att ha tydliga riktlinjer och rutiner som förespråkar användandet av smärtskattningsinstrument.
Background: Pain is one of the most common reasons for seeking emergency care. Adequate pain assessment is a prerequisite for good pain management. Aim: To illustrate the results of the nurse's use of pain assessment tools for the assessment of acute pain in adult patients in the emergency department. Method: Literature study with a systematic approach. Results: Choice of pain assessment tool: VAS and NRS are reliable tools for the measurement of acute pain. Professional pain management: The use of pain assessment tools shortened the time to pain relief, while more patients received analgesics. Experienced versus interpreted pain: Nurses often underestimated the patients' pain intensity. Continuously documented pain assessments: To assure that patients get the best pain management possible, documented pain assessments at enrollment and continuously to discharge are required. With regular pain assessment and documentation patients' pain development and possible treatment outcomes can be followed. Conclusion: The prerequisite for adequate pain management is a pain analysis performed with a pain assessment tool. The use of pain assessment tools leads to more patients getting adequate pain relief more rapidly. Pain rating scales are excellent tools to assess pain. ▼ The centre of Catania joined Pain Out International , a prospective observational project (now involving more than 60 centres) aiming to improve management of postoperative pain (PP) by providing feedback and benchmarking of patient reported outcomes regarding PP [www.pain-out.eu].
The residents of the School of Anaesthesia and Intensive Care of the University of Catania collected data in 8 different Hospitals involving 4 different surgical specialities (general surgery, orthopaedics, gynaecology and urology). We collected information from medical records and asked adult patients to fill in an anonymous questionnaire on postoperative day 1 regarding the PP treatment. We analysed data collected between March 2010 and April 2012. Non parametric tests Chi-square test with Yates correction (X2) were used for the analysis. We considered as significant a p<0.05.
In total we collected data from 2706 patients (almost 10% of the international data collection) and 2441 filled in the anonymous questionnaire. Many difference were found. In such a large database we found some interesting results in the first analysis. Among them:
1. Postoperative pain after thyroidectomy seems to be treated better in the private Healthcare System. There are also consistent differences of patient s outcomes regarding postoperative pain management between all the Hospitals in our territory
2. The use of remifentanil provided worse outcomes after thyroidectomy or open abdominal uterine surgery.
3. In patients undergoing caesarean section and receiving spinal anaesthesia, morphine and fentanyl as adjunct to the local anaesthetic did not produce differences among the patient s outcome, even though patients receiving morphine required less frequently a combination of drugs to manage PP.
As main results we showed the feasibility of data collection through a model based on the residents of the School of Anaesthesia and Intensive Care of the University of Catania. Moreover, Catania s contribution to the overall worldwide data collection has been outstanding. We will discuss our results with the Hospitals and surgical specialties involved in the attempt to improve the management of PP in our territory. We are aware that small sample size and study design may have affected our analysis by hiding some other significant difference. We believe that the differences seen in our subgroup analysis should be investigated in larger samples. We are planning to update our analyses once the sample of data collection in Catania will be larger and also to consider some analysis in the same subgroups (i.e. remifentanil anaesthesia) of the overall data collected so far in Pain Out Project.

Pediatric acute pain is a serious global health issue and remains unexplored mainly in low and middle-income countries (LMIC). Children in LMIC are at high risk to suffer from inadequately managed acute pain, but it is challenging to quantify the magnitude of the problem, evaluate resource availability for acute pain management, and address possible factors influencing acute pain recognition, assessment and treatment. Pediatric acute pain management is complex, if not adequately managed could lead to life-long consequences. Therefore, the purpose of this dissertation is to examine pediatric acute pain management practices in Botswana. Using a three-article format, this dissertation in Chapter 2 presents a synthesis of existing literature to describe the state of the science of pain management practices in LMIC. Chapter 3 reports on pain prevalence, intensity and nature, and pain management practices in Botswana using observational data and a retrospective review of hospital health records. In Chapter 4, children's and parents/guardians’ experiences and perceptions of pediatric pain management practices in Botswana are explored through a convergent mixed-method design, integrating data from a descriptive cross-sectional survey and descriptive qualitative study. The principle findings were: 1) critical gaps and limitations exist in the available literature, particularly in sub-Saharan Africa; 2) resource availability, children and parents/guardians’ voices are mostly absent in literature, and healthcare providers are not prepared to adequately deal with pediatric pain; 3) visible efforts are evident in the development and validation pain tools and novel treatment strategies, but not in clinical applications; 4) acute pain prevalence is high among hospitalized children, and acetaminophen is the drug of choice, despite a substantial proportion of children having moderate-severe pain; 5) reports of severe pain are rare and maximum pain intensity documented in health records is moderate pain; 6) pain assessment was not well documented; 7) children and parents/guardians were content with pain care, but expect adequate acute pain treatment and child comfort; and 8) children and parents/guardians demonstrate significant understanding of pediatric acute pain, child risk factors, consequences, and management strategies. Pediatric acute pain is a significant problem in Botswana and requires a comprehensive strategy to improve its management.

▼ Surgical injury induces a series of comlex response in endocrine-metabolic and hormonal cascade systems which may pose a risk to the patient due to increased physiological demands , with subsequent organ dysfunction conntributing to morbidity, the need for hospitalization. A peri-operative pathophysiology, optimized pain relief, improved fluid management, early oral nutrition and mobilization ,has been produced into the concept of multimodal rehabilitation as ERAS programs. Colorectal syrgery using laparoscopic approach has gained wide acceptance because it is associated with reduced pain and analgesic consumption, lower morbidity, a faster recovery, a shorter hospital stay. Optimal pain therapy for patients undergoing laparoscopic colorectal surgery remains controversial. Purpose: in order to reduse postooperative administration and pain levels in patients submitted to laparoscopic colectomy, we assessed the efficacy of preemptive use of pregabalin, as part of multimodal analgesia sscheme, in a randomized controlled trial setting. Method: overall, 50 adult patients scheduled for laparoscopic colectomy were included and randomized in our trial. In the experimental group, 23 patients received preoperatively 2 doses of 150mg pregabalin(PG) per os, whereas the control group consisted of 27 cases, where a matching to PG placebo was administered at the sama scheme. The two groups had identical analgesia and anesthesia regimes otherwise. Our study endpoints included postoperative morphine consumption, postoperative pain and complication rates. Results: patients in the PG group displayed a significantly reduced morrhine consumption at 8h,24h,48h postoperatively. The two groups were comparable in terms of postoperative pain (rest and movement assessment) and side effects. Conclusions: the preoperative addition og PG resulted in a significant reduction of the postoperative opioid consumption in patients undergoing laparoscopic colectomy. However, an association with the postoperative scores was not identified.

Ο οξύς μετεγχειρητικός πόνος προκαλεί ένα πλήθος δυσάρεστων αισθητικών , συναισθηματικών και νοητικών εμπειριών που συνοδεύονται από αντιδράσεις του ΑΝΣ. Τα παραπάνω παρατείνουν την παραμονή του ασθενή στο νοσοκομείο, ελαττώνουν την ικανοποίηση του και μπορεί να αυξήσουν τη θνητότητα. Ο πόνος για μεγάλο ποσοστό επεμβάσεων φαίνεται πως δεν ελέγχεται επαρκώς. Dentists frequently treat and manage patients with
acute dental
pain. Patients often seek care to treat their
pain. As a part of that treatment, it is common for providers to use both prescription and non-prescription medications in the management of the patient’s dental
pain. This study focused on understanding the patient’s knowledge, history and expectations regarding prescription and non-prescription medication in the management of their dental
pain.
Methods
A self-report survey was developed, using Likert-type scales, consisting of 32 items, including standardized, psychometrically-tested instruments. The survey was administered to 108 adult outpatients who reported to the West Virginia University School of Dentistry urgent care clinic.
Results
Expected
pain relief ratings of six opioid medications was compared to that of five non-opioid medications (including four that are over-the-counter drugs). Expected relief from opioid medications (
M = 7.89,
SD = 1.86) was significantly higher than that of non-opioid medications (
M = 4.98,
SD = 2.0), with greater
pain relief expected from the opioid medications versus non-opioid ones,
t(37) = 7.30,
p < .0005. Across all 108 patients, the average dental
pain level, on a 0-10 scale, was 5.42 (
SD = 2.68); of these patients, 39 expected that an opioid prescription would be necessary for their present dental
pain relief. A binary logistic regression was conducted with
pain level as the predictor and expectation of an opioid as the outcome variable. The model was significant, χ
2(1,
N = 108) = 9.86,
p = .002, indicating that
pain level could distinguish whether patients would expect an opioid medication prescription at their appointment, with higher
pain levels associated with greater expectation of an opioid. The expected
pain relief rating of opioid medications was compared to the combination of ibuprofen/acetaminophen; the mean for the opioid medications was 7.69 (
SD = 2.23), while the mean for ibuprofen/acetaminophen combination mean was 5.92 (
SD = 2.62). As predicted, these means differed significantly, with greater
pain relief associated with opioid medications,
t(38) = 4.12,
p < .0005.
Conclusions
Patients believe that opioids provide superior dental
pain relief than non-opioid medications. With higher dental
pain levels, patients’ expectation to receive an opioid
pain medication is greater. In general, urgent care dental patients have limited knowledge of both opioid and non-opioid analgesic medications.

▼ This program of research examines the experience of chronic pain in a community sample. While, it is clear that like patient samples, chronic pain in non-patient samples is also associated with psychological distress and physical disability, the experience of pain across the total spectrum of pain conditions (including acute and episodic pain conditions) and during the early course of chronic pain is less clear. Information about these aspects of the pain experience is important because effective early intervention for chronic pain relies on identification of people who are likely to progress to chronicity post-injury. A conceptual model of the transition from acute to chronic pain was proposed by Gatchel (1991a). In brief, Gatchel’s model describes three stages that individuals who have a serious pain experience move through, each with worsening psychological dysfunction and physical disability. The aims of this program of research were to describe the experience of pain in a community sample in order to obtain pain-specific data on the problem of pain in Queensland, and to explore the usefulness of Gatchel’s Model in a non-clinical sample. Additionally, five risk factors and six protective factors were proposed as possible extensions to Gatchel’s Model. To address these aims, a prospective longitudinal mixed-method research design was used. Quantitative data was collected in Phase 1 via a comprehensive postal questionnaire. Phase 2 consisted of a follow-up questionnaire 3 months post-baseline. Phase 3 consisted of semi-structured interviews with a subset of the original sample 12 months post follow-up, which used qualitative data to provide a further in-depth examination of the experience and process of chronic pain from respondents’ point of view. The results indicate chronic pain is associated with high levels of anxiety and depressive symptoms. However, the levels of disability reported by this Queensland sample were generally lower than those reported by clinical samples and consistent with disability data reported in a New South Wales population-based study. With regard to the second aim of this program of research, while some elements of the pain experience of this sample were consistent with that described by Gatchel’s Model, overall the model was not a good fit with the experience of this non-clinical sample. The findings indicate that passive coping strategies (minimising activity), catastrophising, self efficacy, optimism, social support, active strategies (use of distraction) and the belief that emotions affect pain may be important to consider in understanding the processes that underlie the transition to and continuation of chronic pain.

pain and understand the underlying mechanisms, animal models of
pain have been generated over the past few decades. This dissertation presents two new models of
acute pain developed to evaluate drug effects on nociceptive responses in cats and dogs. The first model determines the MAC sparing effect of an agent during visceral noxious stimulus of the ovary and ovarian ligament in the anesthetized cat. This technique was developed for dogs and modified subsequently to investigate the anesthetic sparing effect of different drugs in cats. The second method evaluates the efficacy of analgesic medications in conscious dogs using nociceptive threshold testing devices. One thermal and two mechanical nociceptive threshold testing devices were utilized to evaluate the antinociceptive effect of different drugs such as buprenorphine in dogs. Both models are promising to test the analgesic effect of different drugs. Maropitant, NK-1 antagonist, reduced significantly the anesthetic requirements during the ovary and ovarian ligament stimulation in cats. This indicates that maropitant may have the antinociceptive properties encouraging and supporting further investigation of this agent in clinical trials. Orotransmucosal buprenorphine increased thermal and mechanical nociceptive thresholds in dogs using the three testing devices. These findings show potential of the OTM route as an alternative administration of buprenorphine for
pain treatment in dogs.

Given cancer causes pain and suffering impacting the quality of life of patients the use of evidence-based guidelines (EBGs) for pain management is essential for improving patient experiences. The Cancer Pain Management Guideline (CPMG) in South Korea was introduced in 2004 following many concerns of healthcare professionals, including nurses, about the suffering of patients with cancer due to pain. South Korean healthcare professionals believed that the introduction of the CPMG would influence the care and treatment of patients with cancer pain. This study aims to critically explore the impact of the introduction of the CPMG on clinical practice of the nurses in cancer care facilities. Further, the study aimed to uncover the roles and functions of nurses and the factors that influenced or hindered the adoption/spread/uptake of the CPMG. To answer the research question ‘What is the impact of the introduction of the Cancer Pain Management Guidelines on nursing practice in South Korea healthcare context?’ Critical Ethnography was deemed the most appropriate methodology, because it allowed the researcher to observe the practice, examine documents, explore reports on experiences and critically examine the emergent themes from the storylines provided by the 10 nurse participants working in acute cancer care units in South Korea. A total of 13 themes were classified into three superordinate themes and some themes contained further subordinate themes, which included themes relating to the uptake of the CPMG, nurses’ usual practices and the nurses’ experiences of managing pain. There was little evidence of the impact of the introduction of the CPMG on nursing practice. The findings also revealed that nurses’ usual practice was observed as ‘transactional’ and not always demonstrating an advanced level of decision-making informing practice to serve the patients’ true needs based on evidence. Nurses themselves were not evaluating their own practice and embracing a comprehensive set of roles and functions related to pain management. Nursing practice was influenced by various issues in terms of nurses’ decision-making. These included: 1) the medically driven nature of the development processes for a set of guidelines suitable for use as EBGs; 2) an absence of evidence of the organisation’s role in support of EBGs and subsequent support for quality care and a supportive work culture; 3) the valuing of the nursing contribution, and an apparent undermining of the nurses’ role by all involved; and 4) lack of patient-family centred care aligned with the concept of a therapeutic partnership and mutual respect. Recommendations include the need for an integrated approach at four levels: Policy informing the national guidelines; organisational support for implementation of EBGs; involvement of nurses in the EBP through the use of EBGs; and recognition of patients and families’ involvement in education and decision-making about their care.

▼ När en individ drabbas av en demenssjukdom och kognitiv svikt kan det leda till oförmågan att uttrycka smärta, på ett sätt som omgivningen kan förstå. Personer med demenssjukdom är ofta äldre och vårdas inom akutsjukvård på grund akut sjukdomstillstånd eller fall i hemmet som kräver omedelbar smärtbedömning och smärtlindring. Smärtbedömning samt smärtlindring för patienter med demenssjukdom är komplext och en utmaning för sjuksköterskor inom akutsjukvården. Syftet med studien var att belysa smärtbedömning av patienter med demenssjukdom inom akutsjukvård. En litteraturöversikt valdes som metod för att identifiera och lyfta fram aktuell forskning inom området. Mellan december 2015 och januari och februari 2016 gjordes sökningar i databaserna CINAHL, PubMed och PsykINFO. Totalt inkluderades 15 vetenskapliga artiklar, publicerade mellan 2004-2015. Artiklarna kvalitetsgranskades och analyserade utifrån syftet med studien och kring likheter och skillnader. Resultatet kom att beskriva tre huvudteman: kroppsspråk och beteendeförändringar; smärtskattninginstrument och sjuksköterskans erfarenhet. Slutsatsen från studien är att smärta orsakar lidande, vidare att smärtbedömning av patienter med demenssjukdom inom akutsjukvård är komplicerad. Flera smärtbedömningsinstrument identifierades som kan vara till hjälp vid smärtbedömning av patienter med demenssjukdom. Vårdrelation, personcentrerat förhållningssätt samt användning av flera strategier kan säkerställa en korrekt smärtbedömning av patient med demenssjukdom vid vård inom akutsjukvården. Fortsatt forskning behövs för att utvärdera olika smärtbedömningsinstrument inom svensk sjukvård.
