▼ BACKGROUND: Schizophrenia is a chronic mental disorder presenting with psychotic and cognitive symptoms that lead to impairments in independent
living and psychosocial functioning. Individuals with schizophrenia demonstrate cognitive deficits in areas of attention, executive functioning, memory, and language. Additionally, schizophrenia has been associated with impairments in
activities of
daily living (ADLs) such as toileting and the ability to feed one’s self and instrumental
activities of
daily living (IADLs) such as taking medication, financial management, communication, and transportation.
METHODS: Twenty-six participants diagnosed with schizophrenia or schizoaffective disorders were recruited from the University of Texas Southwestern Medical Center’s Division of Translational Neuroscience of Schizophrenia’s IRB approved Database Registry for Psychotic Disorders and completed a neuropsychological test battery which included the Texas Functional
Living Scale (TFLS) and University of California San Diego (UCSD) Performance-based Skills Assessment (UPSA). IBM SPSS Statistics (SPSS v. 19.0) was used to perform Pearson correlation coefficients and multiple regression analyses to identify which subscale(s) of the TFLS had the highest predictive ability for examining IADLs to create a possible short form and to identify which subscales of the TFLS long form have the strongest correlation to neurocognitive measures used in the study. The present pilot study used the Type I error rate at .10; a 90% confidence interval.
RESULTS: Results of the analysis indicated that the Time and Money Calculation subscales of the TFLS long form significantly correlated with more neurocognitive measures than the UPSA. Specifically, these two subscales had a higher number of moderate to strong correlations with the neurocognitive measures compared to the UPSA. Results also indicated the TFLS short form to have a stronger correlation with the UPSA (r =.59, p < .003) compared to baseline correlations of the TFLS long form and the UPSA (r = .34, p < .112), which suggests that the Time and Money Calculations subscales of the TFLS can be used as a valid short form of the TFLS in the assessment of IADLS in schizophrenia.
DISCUSSION: Overall, the short form of the TFLS appears to be a valuable addition to standard neuropsychological assessment batteries given its numerous correlations with neurocognitive measures. Results also suggest that the TFLS short form is a stronger measure for detecting IADL impairments compared to its original long form and the UPSA.

▼ Background: Little is known about the impact cardiac
rehabilitation (CR) exercise programs have on daily functional
abilities. The purpose of this study was to determine the impact of
CR on elderly patients’ ability to perform common household tasks.
Methodology: Twenty-two post myocardial infarction patients (10
female, 12 male; age 75 ± 6.3 years) were studied. Patients were
tested prior to entering and again after completion of a CR program
(20 combined aerobic + strength training sessions over 8 to 10
weeks; 45 min/day). Physical function was assessed using the
Continuous Scale – Physical Function Performance 10 test battery
(PFP-10). Results: Post-CR the global PFP-10 score increased
significantly (59 + 14 vs. 52 ± 17; p = 0.003). Prior to CR 7
patients scored above the threshold for independent living, as
defined by a global score ≥ 57 units. Post-CR, 12 patients scored
above the threshold. Conclusions: A 20 session exercise-based CR
program significantly enhanced the physical function of elderly
patients. More importantly, 55% of patients scored above the
threshold for independence post-CR, suggesting that CR may enhance
elderly patient’s ability to live independently.

▼ Chronic pain is a widespread problem, especially in the older population, and can affect various aspects of daily living. At a time when it has been acknowledged that the population is increasingly ageing, research regarding the effects of chronic pain on the daily living of older adults is essential. Furthermore, the development of innovative technology is changing the way that much research is being conducted, and can lead to the retrieval of novel information, using a fresh approach. The adoption of this technology in the field of chronic pain research has the potential to examine various aspects of the daily living of older adults living with chronic pain using a different approach to previous research. This study is underpinned by a Critical Realist ontology and Hermeneutic epistemology and follows a Generic Qualitative Research methodology (Caelli, et al., 2003). The aim of the study was not to generalise the findings but to gather a deep theoretical description of the outcomes and offer an explanation of these findings based on an analysis of the multiple research methods used within the study. This study had two main aims and was split into two sections according to the aims. Firstly, Part A of this study aimed to explore a range of day-to-day patterns and experiences of functioning in older adults suffering from chronic pain. Part B aimed to explore the usability, acceptance and experience of the technology used to measure functioning as part of the first aim of this study. Part B also aimed to look at the practicalities the participants were faced with when using the technology. A mixed methods design was used for Part A in which 15 older adults (65+) living with chronic pain (pain >3 months) took part in an in-depth study lasting seven days. As well as the 15 core participants that took part in the study, two older adults (65+) without chronic pain and two younger adults (<65) with chronic pain took part in the study in order to provide some insight into the effects of either pain, or age, on functioning. Part A used four data collection techniques to gather data upon the daily functioning of older adults with chronic pain; the Daily Reconstruction Method diary (Kahneman, Krueger, Schkade, Schwarz, Stone, 2004), the Sensecam (also known as the Vicon Revue, Vicon©), the LifeShirt (Vivometrics Inc) and a semi-structured interview. However, although the LifeShirt was validated, as part of this PhD, and used throughout the study, the gathered data was not analysed due to multiple problems with the data. The Daily Reconstruction Method, Sensecam and the semi-structured interview were each analysed separately before the results of the Daily Reconstruction Method and Sensecam were integrated into the themes derived from the semi-structured interviews. The integrated results led to the development of two themes, each with sub-themes; ‘effect on daily living’ and ‘managing pain and functioning’. Background: We assessed the association between activities of daily living (ADL) and mortality among nursing home residents in Japan. Methods: This 1-year prospective cohort study investigated 8902 elderly adults in 140 nursing homes. Baseline measurements included age, sex, height, weight, body mass index (BMI), ADL, and dementia level. ADL levels were obtained by caregivers, using the Barthel Index (BI), after which total BI scores were calculated (higher scores indicate less dependence). Information on dates of discharge and mortality was also obtained to calculate personyears. The Cox proportional hazards model was used to estimate hazard ratios (HRs). Results: Mean age was 84.3 years, and mean total BI score was 38.5. The HRs of mortality adjusted for sex, age, BMI, and type of nursing home were 7.6 (95% CI: 3.3–17.8) for those with a BI score of 0 (totally dependent), 3.9 (1.7–9.0) for those with a score of 1 to 10, 3.5 (1.4–8.7) for those with a score of 11 to 40, 2.7 (1.4–5.1) for those with a score of 41 to 70, and 1.3 (0.7–2.4) for those with a score of 71 to 99 (P for trend <0.001), as compared with those with a score of 100. Multivariate analysis revealed that BI, sex, age, and BMI were significantly associated with mortality rate. Conclusions: There was a clear inverse association between ADL level and mortality. In conjunction with other risk factors, ADL level might effectively predict short-term mortality in institutionalized elderly adults.

▼ This study examined the relationship between five neurocognitive domains, including memory, visual-spatial skills, executive functioning, language, and attention, and three instrumental activities of daily living (IADLs), (financial management, medication management, and driving), as well as three basic activities of daily living (ADLs), including dressing, feeding, and grooming.
Subjects were community dwelling older adults diagnosed with Alzheimer's Disease (AD) ranging in severity from mild to severe. Correlation and regression models were used to analyze the relationship between neurocognitive domains and self-care activities. All of the neurocognitive domains were significantly but mildly correlated with declines in IADLs, and all of the neurocognitive domains, except memory, were significantly but mildly correlated with ADL decline. None of the regression models, except driving, produced significant results.
The hypotheses were based on the belief that overlap occurs between declines in cognitive domains and self-care skills. The results of this study support the assertion that global cognitive decline occurs followed by declines in IADLs and later declines in ADLs. Thus, instead of specific neurocognitive domains predicting decline in individual self-care skills, this research indicates that the degree of severity of cognitive decline is predictive of impairments in IADLs or ADLs with milder global impairment predicting IADLs and more pronounced decline predicting declines in ADLs. Degree of severity may have been more predictive because while individuals vary in cognitive symptom presentation, the course of AD always progresses from mild to more severe.
The role of over-learning also potentially impacted the results of the study. Individuals learn and daily practice ADLs at a young age causing them to require less higher level cognitive skills (over-learned). Since over-learned IADLs require less higher level cognition, they were preserved longer in AD course, thus declining after global cognitive impairment. Clinicians need to thoroughly assess the degree to which an individual has over-learned a task in order to make the most accurate recommendations. If an individual has milder global decline, IADLs are likely to be impaired, and if an individual has moderate global decline with memory reaching a floor, ADLs are likely to be impaired.

▼ People of today live in an information- and technology society and are surrounded by everyday technology in our daily activities in different ways and at different levels of difficulty. Everyday technology (ET) includes a range of products and services from a toaster to more advanced computer systems and services. Previous studies have shown that people with cognitive impairment may experience difficulties in the use of ET in their daily activities. However, there are no studies demonstrating relevance and difficulties with ET for younger adults without cognitive impairment. With this study we wanted to describe relevance and difficulty in the use of ET among younger adults without known cognitive impairments. Interviews were conducted with 40 volunteers with interview instrument ETUQ II (Everyday Technology use Questionnaire), divided into 9 areas that included 116 objects and services. The result showed that the majority of ET is seen as relevant for the group, and that in certain areas, there was a gender difference. It also showed that young adults without cognitive impairments reported and described only a few difficulties in the use of ET. This knowledge including what kind of ET that has higher relevance to the group are valuable for occupational therapists (OT) in supporting clients in the same age with these impairments to enhance their participation in daily activities in society.

▼ Rotator cuff tears are a highly prevalent musculoskeletal injury affecting 20-50% of older adults, and prevalence increases with more advanced age. Symptoms of a rotator cuff tear include muscle atrophy, fatty infiltration, decreased strength, and a loss of functional ability. However, a healthy aging process is also associated with atrophy, reduced strength, and declines in physical function. The goals of this dissertation were to understand how muscle volume and strength are affected with healthy aging, and how the symptoms of a rotator cuff tear may further compound age-associated changes to muscle morphology, strength, and function.

▼ With aging there is a decrease in a person’s ability to perform
activities of
daily living (ADL) which may be most effectively addressed using training patterns that are biomechanically similar to ADL. Since aquatic exercise offers the opportunity to provide resistance with a high level of safety, the pool may afford the ideal environment for ADL-specific training in an aging population. Purpose: The purpose of this investigation was to compare a traditional aquatic exercise program (TRAD) to an aquatic program tailored to target ADL (ADLspec). Methods: Eighteen independently
living individuals (68.7 + 7.5 years) were randomly assigned to a TRAD or ADLspec aquatic exercise group. The exercise groups attended 1 hr exercise sessions, 2 times per week for 8 weeks. ADL ability was assessed using the short version of the Continuous-Scale Physical Functional Performance Test (PFP-10); while strength and power were assessed using the 30s arm curl and 30 sec. chair stand tests. Results: Mixed design ANOVAs revealed a significant group x time interaction for floor sweep time with the ADLspec group outperforming the TRAD and control (CON) groups (p = .043). Additionally, the ADLspec group improved the pan weight and scarf time components of
the PFP-10 (p < .020), while the TRAD group improved pan time and laundry time (p < .046). Both training groups showed similar improvements for jacket time, grocery
weight, and 6-min walk, (p < .046). The ADLspec and TRAD groups also made similar improvements in upper and lower body strength, as well as lower body power across time, (p < .043). A student’s t-test revealed the TRAD group spent more time exercising during the hour session than the ADLspec group (p < .05). Conclusion: The results indicate that performing an ADLspec aquatic exercise program can increase performance of ADL that require more complex sequential movements; however, ADL more dependent on fitness may be better addressed using a TRAD intervention. These results can be helpful when designing a periodized aquatic training program to increase independence in older persons.

▼ Despite a large prevalence of rotator cuff impingements or tears in the elderly population, little research has focused on understanding how this population adapts to perform tasks of daily living. Past research has focused on the analysis of upper limb kinematics of young healthy individuals while performing these essential tasks (Magermans, 2004, Murray and Johnson., 2004). The purpose of this thesis was to identify kinematic and shoulder loading differences between elderly mobile individuals and elderly individuals with rotator cuff impingements during specific activities of daily living. Motion capture techniques were used in combination with the Shoulder Loading Analysis Modules (Dickerson, 2005, Dickerson et al., 2007) to estimate thoracohumeral kinematics and calculate external joint moments. Two-tailed t-tests with injury status as the factor determined that differences in active range of motion in flexion/extension and humeral rotations existed between the two populations. Results of the ADL analysis showed that the impinged population tended to have decreased plane of elevation and humeral rotations during ADLs. Task was also a main factor for most variables examined. Perineal care, hair-combing and reaching tasks were the most demanding in terms of range of motion necessary to complete the task. The reaching tasks resulted in the highest shoulder moment. K-means clustering techniques proved to be unsuccessful in identifying different motion strategies between the two study groups. This investigation showed that developing adaptations for perineal care, hair-combing and reaching tasks should be considered a priority when working with patients with rotator cuff impingements, as these tasks demanded the largest ranges of motion as well as high shoulder moments.

▼ A neuropatia diabética é umas das principais complicações crônicas da Diabetes Mellitus que causa prejuízo nos sistemas somatossensorial e motor resultando em alterações biomecânicas nos padrões de locomoção. Atividades de locomoção da vida diária que provocam maior desafio mecânico e de estabilidade podem ser consideradas como um fator de risco para formação de ulcerações em diabéticos neuropatas. Assim, para o real entendimento da interferência da neuropatia diabética na realidade do paciente diabético, torna-se essencial avaliar a biomecânica de membro inferior durante a execução de tarefas motoras da vida real do paciente. O objetivo deste estudo foi investigar a influência da neuropatia diabética nas características discretas e das séries temporais do padrão de ativação muscular, por meio da EMG de superfície, e da cinemática sagital de membro inferior, por meio de eletrogoniômetros, de diabéticos neuropatas (GD) e não diabéticos (GC) (n=46, GC=23, GD=23). Na análise discreta foram analisadas variáveis discretas dos envoltórios lineares dos músculos VL, GM e TA e da variação angular sagital de quadril, joelho e tornozelo. Estas variáveis foram comparadas entre grupos, em cada tarefa motora, por testes t independentes (=0,05). A análise das séries temporais eletromiográficas e de variação angular sagital foi realizada por meio da correlação cruzada de cada série entre os grupos. Também se avaliou a força da relação de fase entre a atividade muscular de VL, TA e GM com a variação angular de joelho e tornozelo, em cada grupo. Os coeficientes de correlação dessas relações foram comparados entre grupos por testes t independentes (=0,05). Os resultados demonstraram que durante o subir, os diabéticos apresentaram menor extensão de tornozelo ao final do apoio e, ao se projetarem para frente na primeira metade do apoio, fletiram menos o tornozelo. Em decorrência disto, houve um prejuízo no posicionamento ideal de tornozelo, indispensável para a ação inicial eficiente de VL. A desvantagem mecânica de VL observada no GD no início do apoio no degrau desencadeou uma ativação aumentada deste músculo ao final da fase de apoio na perna contralateral. No descer, o GD apresentou menor extensão de tornozelo no início do apoio, mas não apresentou alterações na EMG. Nos resultados da análise das séries temporais das EMG houve alta similaridade entre os grupos tanto no subir quanto no descer. A correlação entre a atividade muscular e a variação angular em todas as relações avaliadas foram significativamente mais fracas para o GD no subir para o VL joelho e GM tornozelo (relações fracas em ambos os grupos), exceto para a relação TA e tornozelo, em que o GD obteve relação moderada, enquanto o GC obteve relação fraca. ▼ As transformações demográficas e epidemiológicas do século anterior trouxeram significativas modificações sociodemográficas e de saúde, em todo o mundo, com o aumento de idosos na população. Ao processo de envelhecimento pode estar associado às doenças crônicas não transmissíveis, dentre elas a doença pulmonar obstrutiva crônica (DPOC), um problema de saúde pública, parcialmente reversível, progressivo e incapacitante. Assim, os objetivos do estudo foram caracterizar os idosos com diagnóstico de DPOC nos estádios I a IV, atendidos no Ambulatório de Pneumologia Geral do HCFMRP/USP, segundo as variáveis sociodemográficas, história de tabagismo e condição de saúde; identificar o grau de independência funcional; identificar a presença de sintomas de depressão; analisar a correlação entre o grau de independência funcional, idade, número de morbidades, tempo e grau de DPOC e a associação entre a presença de sintomas de depressão, as variáveis sociodemográficas, o grau de DPOC e as comorbidades. Trata-se de um estudo descritivo, transversal e correlacional. A coleta dos dados ocorreu no período de março a maio de 2010. Utilizou-se um instrumento para caracterização da amostra, a Medida da Independência Funcional (MIF) e a Escala de rastreamento de sintomas depressivos (CES-D). Foram estudados 84 idosos, média de idade 70,4 anos (s=7,3); 61,9% homens; 51,2% casados; 36,9% viúvos; 36,9% sabiam ler e escrever informalmente/analfabetos; 82,8% eram aposentados; 73,2% recebiam um salário mínimo; 54,8% moravam com a família; 75% deixaram de fumar, 20,2% ainda fumavam e 4,8% nunca fumaram; a carga tabágica foi superior para os homens, sendo a diferença significativa (p<000,1); 100% tinham DPOC, 51,2% pressão alta, 39,3% problemas para dormir e 33,3 problemas cardíacos; a média de morbidades foi de 3,4(s=2,4); 41,7% referiram internação nos últimos 12 meses; 34,5% por exacerbação da DPOC e 34,3% por pneumonia. Houve predomínio da DPOC nos estádios III (40,5%) e II (35,7%), verificou-se prevalência de homens (79,5%) no estádio III e de mulheres (56,7%) no II; os homens apresentaram níveis de DPOC mais elevados que as mulheres, sendo a diferença estatisticamente significativa (p=0,05); tempo médio de DPOC foi 8,3 anos (s=6,4). O escore médio da MIF total foi 117,0 pontos; 96,4% com nível de Independência completa/modificada; a correlação de Pearson entre número de morbidade e MIF (total e motora) foi inversa e significativa (p=0,02). A CES-D classificou 71,4% com sintomas de depressão. Houve diferenças significativas entre presença de sintomas de depressão e as variáveis, sexo (p=0,01), estado conjugal (p=0,02) e problemas para dormir (p=0,01). ▼ Attention deficits are common in traumatic brain injury and stroke. These difficulties can impact the individual’s everyday life, affecting
activities of
daily living such as running errands, increasing cognitive errors such as failing to notice signs, and minimising community integration. They also call for specific training to improve these functions. We investigated the relationship between attention functions and these
daily living aspects, as well as the correlates of the CogniPlus VIG training programme, in 12 people with stroke and traumatic brain injury. Results suggested that sustained and divided attention might predict
activities of
daily living and cognitive errors, as well as community integration. There was also a high correlation between CogniPlus outcome measures and sustained and selective attention performance, suggesting a specific and non-specific effect of this training. We concluded that attention could be an important function for managing everyday demands, and that CogniPlus VIG outcomes measures could indicate improvement in sustained and selective attention when used as training.

▼ Alzheimer's disease (AD) is a common disease among the elderly. Physical activity may be beneficial for people with AD as it may slow the rate of decline in cognitive and motor abilities. This retrospective correlational study explored the association between physical activity, cognitive function, and performance of
activities of
daily living (ADLs) in early-stage AD. The sample consisted of 112 participants divided on 2 groups; AD and non-demented controls. Results showed that the AD group exhibited significant declines in cognition, function, and practice of physical activity over a period of 2 years compared to the control group (p<.002). Less practice of household
activities and physical leisure
activities was significantly associated with decline in the performance of ADLs (p <.05) in people with AD. Additionally, cognitive function significantly predicted functional decline (p<.001) in AD. In conclusion, physical activity may be an important part of maintaining independence in people with early AD.

▼ The long term goal of this research is to protect knee
joint surfaces after knee surgery, thereby reducing the incidence
of osteoarthritis. The objective of this dissertation was to
determine if the ovine stifle joint is a suitable preclinical
biomechanical model for the human knee. Using a 6 degree of freedom
(DOF) robot, we applied simulated human and ovine in vivo motions
to human knee and ovine stifle joints to measure the 3D joint and
ACL kinetics. In addition, we investigated the biomechanical
contributions of the other major knee structures. The in vivo
studies were designed to determine the effect of surgically
implanting motion and force sensors on ovine gait by monitoring the
vertical ground reaction forces (VGRFs). Following surgery, we
simultaneously measured VGRFs, knee kinematics, and the output from
an arthroscopically implantable force probe (AIFP) which was
implanted into the ACL. The kinematics were then simulated and
applied to the operated joints, while measuring the 3D joint forces
and moments. The AIFP output was used to validate the reproduced
motions. Finally, we determined the effect of perturbing a
simulated in vivo motion on 3D joint and ACL kinetics, which
allowed us to investigate the potential effect of motion recording
and reproduction errors on force and moment measurements.
Applying simulated human and ovine in vivo motions to
human knee and ovine stifle joints resulted in few significant
kinetic differences between the human and ovine intact joints and
ACLs. For a simulated 6 DOF ovine motion applied to the ovine
stifle joint, the bony interaction and medial meniscus were the
major restraints during the stance phase of gait, whereas the MCL
and ACL were the key restraints during the swing phase. The
contributions of the ovine structures for a simulated 6 DOF in vivo
motion are similar to the roles previously reported for the human.
The in vivo studies revealed that surgery to implant motion and
force sensors decreased average and peak VGRFs less than 10% and
20%, respectively, across all combinations of speed and grade. VGRF
measurements acquired before and after surgery were consistent
among animals, with a coefficient of variation averaging no more
than 18% for all
activities. Increasing treadmill speed only
increased hind limb peak VGRFs, whereas increasing treadmill grade
significantly increased hind limb average and peak VGRFs. Finally,
we investigated the effect of motion recording and recreation
errors on kinetic measurements acquired using a robot. The starting
position of the in vivo ovine motion was adjusted (perturbed) in
each degree of freedom to levels comparable to the extents of our
motion recording and recreation errors. Perturbing a simulated in
vivo motion in each degree of freedom indicated that only
translational perturbations significantly affected the intact knee
and ACL kinetics. Also, the average ACL resultant forces across all
subjects and perturbations were less than 10% of the average ACL

▼ Studies have supported that executive function is a more sensitive and stronger predictor of functional impairment than other cognitive abilities. Since improvements in executive function may bring about improved functional status, before developing an intervention, it is important to know what aspect of executive function is most closely related to functional status. Prior studies found three components of executive function; shifting, updating, and inhibition. The purpose of this study is to identify which component of executive function among updating, shifting, and inhibition is most closely related to functional abilities in older adults. 41 healthy community dwelling older adults aged from 66 to 90 years participated in this study. DAFS was used as a performance-based measure of instrumental
activities of
daily living (IADLs), including communication, financial, shopping, and medication tasks. A computerized battery of six executive function tasks was administered: Antisaccade and Stroop tasks tapping inhibition, Letter Memory and Keep Track tasks tapping updating, and Category Switch and Color Shape tasks tapping shifting. The results indicated that the measures of executive function predicted IADL functioning significantly. Measures of inhibition were the best predictors of ability to perform IADLs. This study suggests that inhibition may be important to perform IADLs, and the measures of inhibition might serve as a detector of initial functional decline among older adults.

▼ Despite pharmacological advances, exercise remains a cost effective preventative for cardiovascular disease (CVD) by acutely and chronically lowering systolic blood pressure (SBP). Although numerous studies have investigated aerobic and resistance training’s role in eliciting an acute SBP response termed post-exercise hypotension (PEH), few researchers have investigated how performing both cardiovascular and weight training in a single session (concurrent training) or how different prescriptions for order of exercise modality will elicit this PEH response; no known studies have attempted altering the order of exercise types within each session. This study seeks to determine if the order of exercise type will affect a PEH response following concurrent exercise. Participants (n=13), considered low risk for cardiovascular disease participated in a control session, graded exercise test (GXT) and two concurrent sessions, with concurrent sessions counterbalanced for order of exercise mode. Recovery SBP was analyzed in the laboratory for sixty minutes and for twenty-four hours thereafter using an ambulatory blood pressure monitor. All exercise conditions elicited a depression in SBP; however, only the cardiovascular-weight training (CVWT) concurrent session elicited PEH (p = 0.05). However, there were no differences in SBP attenuation between conditions. There was no main effect for PEH between conditions during twenty-four hour assessment. With no statistically significant differences in the magnitude of PEH twenty-four hours after exercise, it may be determined that the order of exercise does not vi affect the preventative attributes of aerobic and resistance exercise in regards to acute SBP response. Furthermore, only exercise involving resistance training (CVWT) provoked PEH during the first 60 minutes; therefore it may be argued that individuals should pair cardiovascular exercise with weight training in order to elicit the greatest cardiovascular benefits. Future studies should consider evaluating the effect of time of day and PEH following concurrent exercise to determine if pairing exercise modalities will at different times will affect the blood pressure response.

▼ The aim of the study was to describe occupational therapists experiences in promoting older people´s participation in activities of daily living in residential care. To answer the aim of the study a qualitative approach was selected and data were collected through semi-structured interviews with seven occupational therapists. To analyze the data, a qualitative content analysis was used who resulted in four categories: Assessments that describe the older people's needs; Using the older people's abilities in order to promote participation; Various aspects to consider when prescribing assistive devices; Create a functioning collaboration with health professionals. The results showed that an equal approach and a good collaboration with health professionals was a key requirement. Furthermore, supervision and education of health professionals were seen as important factors in the occupational therapists' work to promote participation in daily activities. Finally assistive devices were a large part of the occupational therapists’ work when prescribing assistive devices in residential care for older people. The authors consider that there is a need for studies that illustrate how the occupational therapists’ presence in residential care could favor the promotion of activities in daily living for older people.

▼ Background: Inpatient rehabilitation facility (IRF) goals are to optimize functional independence and discharge patients to community
living. Debility, or deconditioning associated with hospitalization, is the fourth most common impairment group, accounting for about 10% of IRF cases. Objectives were to provide benchmark data for patients with debility, consider trends with respect to health care policy changes, identify risk factors for discharge to acute or subacute care, and examine readmission to acute care after discharge to community. Aim 1: National benchmark data for years 2000 – 2010 were retrospectively analyzed for 260,373 patients from 830 IRFs contributing to the Uniform Data System for Medical Rehabilitation. Trends from 2000 to 2010 included decrease in mean (SD) FIM® instrument (“FIM”) total admission ratings from 73.9 (16.2) to 62.5 (15.8). FIM total discharge ratings decreased from 95.0 (19.7) in 2000 to 88.2 (19.8) in 2010. Mean length of stay decreased from 14.3 (9.1) in 2000 to 12.1 (6.2) days in 2010. FIM efficiency increased from 1.9 (1.7) in 2000 to 2.4 (1.9) in 2010. Discharge to community decreased from 80% in 2000 to 75% in 2010. Health policy changes may have influenced trends. Aims 2 & 3: Centers for Medicare and Medicaid Services data (years 2006 to 2009) were analyzed for factors associated with discharge to acute or subacute care (N = 67,626) and readmissions for 90 days following discharge (N = 45,424). Discharge setting was 76% community, 13% subacute, and 11% acute care. Significant risk factors for both acute and subacute discharge settings were lower FIM motor subscale, male gender,
living alone, comorbidity tier, weight loss, and fluid/electrolyte disorders. Rehospitalization rates were 19% at 30 days and 34% at 90 days. Congestive heart failure, renal failure, and chronic pulmonary disease were common causes of hospital readmission and independent risk factors for reshospitalization. Conclusions: National data indicate the number of debility cases is increasing with diverse etiologic diagnoses. A high proportion of patients discharged to acute or subacute care. One-third of patients who discharged to the community experienced acute hospital readmission within 90 days. Functional independence is an important indicator for discharge setting and rehospitalization.

▼ Older adults with mild cognitive impairment (MCI) often have difficulty performing complex instrumental activities of daily living (IADLs). Smart environment prompting technologies have gained popularity over the last decade and have the potential to assist these individuals with IADLs in order to live independently. In the current study, we developed and experimented with a graded hierarchy of prompts to examine whether individuals with MCI would benefit from receiving prompts when errors occurred during completion of eight scripted IADLs in an experimenter-assisted smart apartment environment. Thirty individuals who met criteria for multi-domain MCI, 23 individuals who met criteria for single-domain MCI, and 50 healthy older adult controls participated in this study. Results showed that the multi-domain MCI group received more prompts and required that more activity steps be prompted than the single-domain MCI and healthy older adult groups. Similar to the other two groups, the multi-domain MCI group responded accurately to the indirect prompts and did not more often need a higher level of prompting to reengage with the tasks. The total number of prompts received was best predicted by executive abilities in single-domain MCI and verbal memory abilities in multi-domain MCI. Experimenter activity quality ratings were best predicted by executive abilities in healthy older adults and individuals with single-domain MCI. These findings suggest that future IADL prompting technologies can be tailored specifically for individuals with MCI.

▼ The study investigated an organized semantic fluency task, (the Controlled Animal Fluency Task - CAFT) as a measure of executive functioning (EF) in adults, and the relationship with instrumental
activities of
daily living (IADL). Participants (N = 266) consisted of a clinical sample (n = 142) utilizing neuropsychological assessment data collected at an outpatient psychological center, and a community sample (n = 124). The clinical sample was a heterogeneous mixed neurological group including a variety of health conditions and comorbid anxiety and depression. The CAFT Animals by Size demonstrated a significant positive correlation with Category Fluency (r = .71, n = 142, p < .001) , Animal Fluency (r = .70, n = 142, p < .001), and with other, established neuropsychological measures. The CAFT Animals by Size condition demonstrated a significant moderate negative correlation with IADL for the sample as a whole (r = -.46, n = 248, p < .001), and for the clinical sample (r = -.38, n = 129, p < .001), but not for the community sample. In a hierarchical regression analysis, CAFT Animal by Size explained additional variance in IADL (ΔR2 = .15). In a hierarchical regression analysis predicting IADL with the control variables entered first, followed by Category Fluency, with CAFT Animal by Size entered last, CAFT Animals by Size did not make a significant additional contribution. A stepwise forward regression indicated Category Fluency, education, and Category Switching are better predictors of IADL than CAFT Animals by Size. Normative data for the CAFT were calculated separately for age groups and education levels. Simple logistic regression indicated CAFT Animal by Size was a significant predictor of clinical or community group membership. A second logistic regression analysis indicated the CAFT Animal by Size condition improved the prediction of membership in the clinical versus the community group, compared to the MMSE alone. Applications of the CAFT are discussed.

▼ The purpose of this dissertation was (i) to investigate the prevalence of older Canadian adults living with coexisting vision and mobility impairments, and (ii) to describe how disability associated with both impairments relate to health status, activities of daily living, physical activity participation, assistive technology use, and health care services. These goals were achieved by using mixed methodology approach.
The first manuscript was based on secondary analyses of the 2006/2007 National Population Health Survey (NPHS). The results of the secondary analysis of the NPHS revealed that approximately 3% of the Canadian population over the age of 65 was living with coexisting vision and mobility impairments. Also, older adults with both conditions required more assistance with activities of daily living and they reported the lowest levels of physical activity participation compared to those living with one or no impairment.
The second part of the core component was based on secondary analyses of the 2006 Participation and Activity Limitations Survey (PALS). The PALS was specifically designed for individuals who reported they were living with a disability. As a result, a greater proportion of respondents indicated the presence of both target conditions – approximately 15% of older Canadians who reported having a disability were living with coexisting vision and mobility impairments. The analysis of the PALS data revealed that older adults with both impairments required more assistance with activities of daily living and they reported a higher use of assistive technology; however, there was a higher level of unmet needs for vision related assistive technology.
The final manuscript consisted of in-depth interviews conducted to add the personal perspective of older adults with both impairments, and to expand and clarify the quantitative findings. The analyses of the transcripts revealed four main themes among participants:
(i) the meaning behind vision, mobility, and coexisting impairments;
(ii) adaptation of desired activities;
(iii) external support for engaging in activity; and
(iv) internal support for engaging in activity.
The results of this dissertation contribute knowledge about living with coexisting vision and mobility impairments and offer a starting point to guide rehabilitation services for clients with multiple impairments.

▼ Introdução: O declínio cognitivo representa uma das principais causas de comprometimento funcional e da qualidade de vida do idoso. A busca por preditores de alterações cognitivas precoces ao diagnóstico de demência tem despertado cada vez mais o interesse dos pesquisadores. Tem-se como hipótese que menor desempenho de Atividades Avançadas de Vida Diária (AAVD) poderia ser marcador de declínio cognitivo futuro. As AAVD reúnem as atividades sociais, produtivas e de lazer, e estariam relacionadas ao topo da hierarquia da complexidade das atividades cotidianas, com melhores níveis funcionais e cognitivos. Objetivo: Analisar a associação entre o desempenho de atividades avançadas de vida diária por idosos paulistanos e a incidência de declínio cognitivo. Métodos: Estudo longitudinal com uma amostra de idosos não institucionalizados que participaram da segunda (2006) e terceira (2010) ondas do estudo de múltiplas coortes Saúde, Bem estar e Envelhecimento em São Paulo, Brasil. A amostra final contou com 819 idosos. O declínio cognitivo foi avaliado com o Miniexame do Estado mental abreviado. Foram consideradas 12 AAVD. Outras covariáveis abordaram as condições sociodemográficas, saúde, estilo de vida e incapacidade funcional dos participantes no início do seguimento. A análise estatística incluiu frequência, análises bivariadas e multivariadas, com o uso de Regressão Logística. O programa Stata 12.0 foi utilizado para a análise e a correção para os efeitos do desenho do estudo foi feita pelo comando survey para analisar os dados originados de uma amostra complexa. Resultados: Após quatro anos de seguimento a incidência de declínio cognitivo foi de 7.9 por cento . A análise múltipla evidenciou que os indivíduos com maior número de AAVD no início do estudo apresentavam menor risco de declínio cognitivo ao final do seguimento OR 0.83 [0.71; 0.96]. Outras características foram associadas à incidência de declínio cognitivo no modelo final: 75 anos e mais OR 4.54 [IC 2.52; 8.17], possuir dificuldades nas atividades instrumentais e básicas de vida diária OR 2.54 [1.32; 4.87]. Possuir 8 anos e mais de estudo mostrou-se como fator protetor ao declínio cognitivo OR 0.16 [IC 0.05; 0.52] Conclusões: O desempenho de maior número de AAVD mostrou-se como fator protetor à incidência de declínio cognitivo, independentemente de condições sociodemográficas, condições gerais de saúde, funcionalidade e estilo de vida.

▼ Alzheimer?s disease (AD) progressively impairs cognitive and functional abilities. Research on pharmacological treatment of AD is shifting to earlier forms of the disease, including preclinical stages. However, assessment methods traditionally used in clinical research may be inappropriate for these populations. The Alzheimer Disease Assessment Scale-cognitive (ADAS-cog), a commonly used cognitive battery in AD research, is most sensitive in the moderate range of cognitive impairment. It focuses on immediate recall and recognition aspects of memory rather than retention and delayed recall. As clinical trials for dementia continue to focus on prodromal stages of AD, instruments need to be retooled to focus on cognitive abilities more prone to change in the earliest stages of the disease. One such domain is delayed recall, which is differentially sensitive to decline in the earliest stages of AD. A supplemental delayed recall subtest for the ADAS-cog is commonly implemented, but we do not know precisely where along the spectrum of cognitive dysfunction this subtest yields incremental information beyond what is gained from the standard ADAS-cog. An item response theory (IRT) approach can analyze this in a psychometrically rigorous way. This study?s aims are twofold: (1) to examine where along the AD spectrum the delayed recall subtest yields optimal information about cognitive dysfunction, and (2) to determine if adding delayed recall to the ADAS-cog can improve prediction of functional outcomes, specifically patients? ability to complete basic and instrumental
activities of
daily living.
Results revealed differential functioning of ADAS-cog subtests across the dimension of cognitive impairment. The delayed recall subtest provided optimal information and increased the ADAS-cog?s measurement precision in the relatively mild range of cognitive dysfunction. Moreover, the addition of delayed recall to the ADAS- cog, consistent with my hypothesis, increased covariation with instrumental but not basic
activities of
daily living. These findings provide evidence that the delayed recall subtest slightly improves the ADAS-cog?s ability to capture information about cognitive impairment in the mild range of severity and thereby improves prediction of instrumental functional deficits.

▼ Purpose: The aim of the study was to evaluate the
effects of NAGs for pain, pain in available end range, various
ranges of motion, certain activities of daily living (ADL) and
anxiety associated with cervical spine pain and stiffness.
Relevance: The studies on NAGs and its functional outcomes have not
been reported in literature. Several questions regarding NAGs
remain unanswered especially about its efficacy in pain and
stiffness affecting ADL. However, a number of therapists throughout
the world use this technique as an integral component of their
practice. A limited number of papers including case studies and
case series have documented a rapid reduction in pain and stiffness
of other parts of the spine and the peripheral joints using
Mulligan Concept. In the era of evidence based practice, there is
urgent need to cross validate the claims made by the proponents of
this technique. This study is an attempt to establish a scientific
evidence to use NAGs for the benefit of the patients and future
research. Participants: One hundred subjects (N=100) of age group
30 to 60 years, attending OPD of orthopedic department, suffering
from cervical pain and stiffness, without radicular pain, diagnosed
as having cervical Spondylosis. Those who met the predefined
criteria were included in the sample with written informed consent.
The sampling was incidental; subjects were randomly assigned to the
four groups (n=25). Methods: Ethical approval was sought and
obtained. It is a repeated measure design, with double blind
controlled trials. Visual Analogue scale (VAS) score, Range of
Motion (ROM), Neck Disability Index (NDI), and State-trait anxiety
inventory (STAI) were the dependent measures. McGill Pain
Questionnaire (MPQ) was used for the assessment and screening of
the subjects for the study and to develop demographic data. All
groups received hot packs for 12 minutes along with set of active
exercises from day one to day twelve.

The aim : To identify small movement
disorders and pain impact on daily business activity and quality of
life in traumatic hand injuries. Study objectives: first to
evaluate the dynamics of fine motor rehabilitation period in

▼ INTRODUÇÃO: A doença pulmonar obstrutiva crônica (DPOC) é uma doença respiratória com comprometimento sistêmico, que cursa com dispneia, intolerância ao exercício e dificuldade para realizar as atividades de vida diária (AVDs). Uma importante medida terapêutica é a reabilitação pulmonar (RP), porém os efeitos desta intervenção sobre as AVDs são pouco conhecidos. OBJETIVOS: Estudar os efeitos da RP sobre a capacidade de exercício, a qualidade de vida, as alterações do humor, as AVDs e avaliar a relação entre as mudanças neste diferentes desfechos em pacientes com DPOC. MATERIAL e MÉTODOS: Pacientes com diagnóstico de DPOC foram submetidos a um programa de RP de 12 semanas. Foram realizados as seguintes avaliações antes e depois da intervenção: teste de caminhada de seis minutos (TC6), Saint George’s Respiratory Questionnaire (SGRQ), inventário de ansiedade (BAI) e depressão de Beck (BDI) e testes que reproduzem AVDs como levantar da cadeira (TSL), levantar do chão, subir escadas e equilíbrio. Um valor de p ≤ 0,05 foi considerado como significativo. RESULTADOS: Foram estudados 52 pacientes com DPOC, com volume expiratório forçado no primeiro segundo (VEF1) de 0,99±0,43 l e 38,1±8,4 % do previsto e idade de 66,2 ±8,5 anos. Com a RP houve melhora na distância percorrida no TC6 (367,6 ± 87,4 vs 412,5 ± 82,6 metros; p<0,001), nos escores do BAI e BDI (10 vs 5 pontos; p<0,001 e 10,5 vs 6 pontos; p<0,001), no escore total do SGRQ (49,4 vs 39,5 pontos; p<0,001). Após a RP aumentou o número de repetições no TSL (10 vs 11 vezes; p<0,001), reduziu o tempo para levantar do chão (5,9 vs 5,4 segundos; p = 0,003) e subir escadas (9,0 vs 7,9 segundos; p<0,001) e aumentou o tempo de equilíbrio (10,1 vs 14,5 segundos; p = 0,014). O TSL se relacionou com todos os domínios do SGRQ (rs entre -0,282 e 0,370, p<0,05). Não houve associação entre testes de AVDs e distância percorrida no TC6 ou escores do BAI e BDI. CONCLUSÕES: A capacidade física, a qualidade de vida, os sintomas depressivos, a ansiedade e os testes de AVDs melhoraram com a RP. A melhora observada nas AVDs (no tempo de equilíbrio, para levantar do chão e no TSL) se associou com mudanças na qualidade de vida.
BACKGROUND: Chronic obstructive pulmonary disease (COPD) is a respiratory disease with systemic involvement that leads to dyspnea, exercise intolerance, and difficulty in performing activities of daily living (ADL). O envelhecimento manifesta-se pelo declínio das funções dos diversos órgãos, associado à prevalência de doenças crónicas e incapacitantes que leva a uma diminuição da capacidade funcional e perda de independência, frequentemente responsáveis por uma baixa perceção da qualidade de vida (Direcção-Geral da Saúde, 2004; Fleck, 2008; Netto, 2000).
Tendo em conta, estes pressupostos, o presente estudo teve como objetivo geral estudar o efeito da dependência funcional na qualidade de vida dos idosos. E por objetivos específicos, avaliar o nível de capacidade funcional dos idosos, caraterizar a dependência funcional em termos de sexo e idade, avaliar a perceção que os idosos têm da sua qualidade de vida e caraterizá-la em termos de sexo e idade.
Para alcançar esses objetivos foi selecionada uma amostra de 50 idosos do Centro Social do Orvalho, com idades compreendidas entre os 65 e 97 anos.
Os dados foram recolhidos através de um questionário sociodemográfico (elaborado para o efeito), a Escala de Barthel Modificada e a Escala WHOQOL-OLD. A informação obtida foi tratada no programa Statistical Package for the Social Sciences (SPSS), versão 17.0 para Windows, tendo sido utilizado a estatística descritiva para caraterizar a amostra e o coeficiente de correlação de Pearson (r).
Em termos de resultados obteve-se relação estatisticamente significativa entre sexo e qualidade de vida (r=0,45 e:ρ:0001):verificando-se uma associação moderada entre estas duas variáveis e entre a idade e qualidade de vida (r=-041: e: ρ: 0003): também com associação moderada entre elas. Verificou-se que não existe relação estatisticamente significativa entre o sexo e: dependência: funcional: (r=0225: e: ρ: 0,385), nem entre a de idade e dependência funcional (r=-0295: e: ρ: 0251): acontecendo o mesmo entre a dependência funcional e a qualidade de vida (r=0,234 e ρ:0366)
Assim, pode-se concluir que se confirmaram duas das hipóteses colocadas, tendose constatado neste estudo, uma moderada relação, estatisticamente significativa, entre o sexo e a qualidade de vida, em que os homens apresentam melhor perceção de qualidade de vida quando comparados com as mulheres. Verificou-se também que ao longo da idade a qualidade de vida dos idosos vai diminuindo, com moderada relação entre as duas variáveis. As restantes hipóteses colocadas, não se confirmaram, uma vez que não se confirmou a existência de relação entre as variáveis dependência funcional, sexo e idade e dependência funcional e qualidade de vida.

▼ This thesis examined the importance of physical activity and fitness, in the performance of
daily living activities (ADLs). The research comprised 600 adults over 50 year old and 79 young adults, of whom 106 old adults were followed-up for a 5 years period. Falls and injuries, fear of falling, disability in performing ADLs, usual physical activity, intrinsic (physical fitness, body composition and health status) and extrinsic risk factors (environmental hazards) for these events/behaviors were evaluated. Data were analyzed by using comparisons, correlations and regression analyses. Results showed that frailty regarding intrinsic factors leads to an increase in the risk of extrinsic factors and limit the performance of ADLs. The number of co-morbidities was the main intrinsic risk factor for falls and ADL disability. Fear of falling was found to be a higher risk for ADL disability than falls and injuries. Decreased balance was the physical fitness parameter that
contributed most for falls, while reduced aerobic endurance was the most important parameter for ADL disability.

▼ A doença de Alzheimer (DA) é a demência mais prevalente entre os idosos, com manifestações como alterações cognitivas, comportamentais e funcionais. A presença de sintomas neuropsiquiátricos é frequente na demência e pode resultar em sofrimento para os pacientes e seus familiares, além de ocasionar sobrecarga para quem cuida. É comum o idoso apresentar DA associada à demência vascular, caracterizando a demência mista (DM). Assim, o objetivo geral deste estudo foi: analisar a relação entre a sobrecarga do cuidador familiar, o desempenho funcional e a presença de sintomas neuropsiquiátricos em idosos com diagnóstico médico de DA ou DM, atendidos em um Ambulatório de Geriatria e Demências de um Hospital Geral Terciário. Trata-se de um estudo quantitativo, não experimental, descritivo, transversal e exploratório, realizado com 96 idosos com DA ou DM e seus respectivos cuidadores familiares. Para a coleta de dados, utilizaram-se um questionário para caracterização dos idosos e de seus cuidadores, o Índice de Katz, a Escala de Lawton e Brody, o Inventário Neuropsiquiátrico (NPI) e a Escala de Sobrecarga do Cuidador. Quanto aos idosos, 68,7% eram mulheres, média de idade 80,8 anos, 50,0%, viúvos(as), 82,3%, aposentados; 56,2% com diagnóstico de DA e 43,7%, com DM; 43,7%, com CDR1 (demência leve). No desempenho para as atividades básicas de vida diária (ABVDs), 70,8% eram dependentes para uma ou mais funções e 29,2% independentes; nas atividades instrumentais de vida diária (AIVDs), 74,0% apresentaram dependência parcial. Houve associação entre os estágios da demência e o desempenho dos idosos para as ABVDs (p<0,01) e as AIVDs (p<0,01), evidenciando que quanto maior a gravidade da demência maior a dependência dos idosos. Quanto aos cuidadores, 90,6% eram mulheres, média de idade de 56 anos, 65,6%, casados(as); 79,2%, cuidadores primários, 70,8% cuidavam do pai/mãe e 64,6% moravam com o idoso. Os sintomas neuropsiquiátricos mais prevalentes entre os idosos foram apatia/indiferença (63,5%), disforia/depressão (59,4%), agitação/agressividade (50,0%) e comportamento motor aberrante (50,0%), com média de 5,0 por idoso. Identificou-se forte correlação (r=0,82) entre o escore total do NPI e o escore total do Inventário Neuropsiquiátrico Desgaste (NPI-D), ou seja, quanto maiores a frequência e a gravidade dos sintomas neuropsiquiátricos apresentados pelos idosos maior é o desgaste do cuidador (p<0,01) e forte correlação (r=0,80) entre o escore total do NPI-D e o número de sintomas neuropsiquiátricos, o que indica que quanto maior o número de sintomas neuropsiquiátricos maior é o desgaste do cuidador (p<0,01). A média de sobrecarga dos cuidadores foi de 25,2 pontos, e 48,9% deles apresentaram sobrecarga pequena. Evidenciaram-se maiores médias de sobrecarga para os que cuidavam de idosos com dependência para as ABVDs (27,2) p=0,04 e para os totalmente dependentes para as AIVDs (31,4), p=0,03.
