糖尿病は、糖尿病性網膜症や糖尿病性腎症、糖尿病性末梢神経障害を引き起こす。この病態機序の背景には血液臓器関門機能の障害がある。一般に血液臓器関門は、内皮細胞、それを取り囲む周皮細胞、グリア細胞にて構成される。糖尿病性網膜症では、このうち、周皮細胞の喪失が重要とされる。神経系では主として末梢神経障害について論じられている。しかし、糖尿病が認知症やAlzheimer病の発症リスクとなることから、血液脳関門も同様に障害されることが推定されるが、その研究、特に、周皮細胞の障害についての研究は進んでいない。そこで、本研究では、1型糖尿病モデルマウスを用い、脳小血管障害について、病理学的に検討した。 方法として、streptozotocinの投与により1型糖尿病モデルマウスを作成し、1か月後に脳標本を作製し、免疫蛍光染色を行った。大脳皮質・海馬・線条体でレーザー共焦点顕微鏡により画像を取得し、3次元画像を構築して定量解析を行った。周皮細胞障害は小血管内皮細胞を取り囲む周皮細胞の体積比率(被覆率)を、血管内皮細胞障害は形態学的変化と体積を、血液脳関門の機能障害は、血漿成分fibrinogenの血管壁への沈着と、同部位の活性化ミクログリアを、定量的に解析した。 その結果、大脳皮質では、周皮細胞の被覆率が低下し、内皮細胞障害、fibrinogenの血管壁への沈着と活性化ミクログリアの肥大化を認めた。海馬でも、周皮細胞の被覆率の低下と、fibrinogenの血管壁への沈着を認めたが、内皮細胞障害と活性化ミクログリアの肥大化は認めなかった。一方、線条体では、いずれの変化も認めなかった。 以上の結果から、本研究では、1型糖尿病モデルマウスの脳小血管で、周皮細胞被覆率の低下と、fibrinogenの血管壁への沈着を伴うことを示し、周皮細胞の被覆率の低下が、本モデルの脳小血管障害の早期の変化であることを示した。周皮細胞の喪失は、糖尿病における網膜小血管の障害でも早期に認めることから、高血糖による小血管障害の普遍的な機序である可能性がある。一方、脳の部位により、高血糖に対する小血管の脆弱性は異なり、小血管障害には脳内で多様性があることが示された。高血糖下での周皮細胞の脆弱性に関わる分子病態機序の解明は、糖尿病における小血管障害の治療標的に繋がることが期待される。
