▼ Introduction: The purpose of this randomized controlled trial was to evaluate the effect of AcceleDent Aura device on maxillary and mandibular arch alignment and pain during orthodontic treatment with SureSmile. Methods: Seventy-one subjects undergoing non-extraction comprehensive orthodontic treatment with SureSmile were randomly assigned into experimental group that used AcceleDent device and control group that used no additional device. Little’s irregularity index was used to evaluate maxillary and mandibular anterior teeth alignment at four time points – initial, 3 months, 6 months, and 12 month or end of treatment. Pain intensity and compliance with AcceleDent were recorded. Results: Out of total initial sample, data from sixty-one subjects were analyzed. There were no statistical significant differences in the initial upper and lower arches irregularity indices between the two groups, p-value > 0.05. At all follow up time points, the mean irregularity indices were consecutively smaller for both upper and lower arches. Within each group these differences were statistically significant with p-value <0.001. However, the mean differences in irregularity index were not statistically significant between the experimental and control groups at any time points. There was no difference in pain levels between the two groups during initial 3 months of the treatment for the 3 days evaluated; however, pain was significantly lower in the experimental group from the third month until the end of the treatment, p-value = 0.006. Average compliance with AcceleDent use was 78%. Conclusion: The AcceleDent Aura device had no effect on arch alignment. Likewise, device did not have any effect on pain level during initial three months. After 3 months until the end of the treatment, AcceleDent group had significantly less pain compared to the control group during the initial 3 days post-adjustment appointments. ▼ Biomarkers of orthodontic tooth movement with fixed appliances and vibration device: A randomized clinical trial Marie-Claude Chouinard D.M.D. ABSTRACT Background: On average, comprehensive orthodontic treatments last approximately 21-27 months in non-extraction cases and 25-35 months when teeth elected for extraction. Longer treatment times have been associated with multiple detrimental effects such as white spot lesions, root resorption, gingival inflammation and dental caries. Recently, some evidence has shown that the application of mechanical vibration to the dentition can increase the rate of tooth movement. However, the biological mechanism underlying this effect is not clearly understood. The aims of this study were to determine if the addition of vibration to the regular fixed orthodontic appliances could alter the expression of biological factors involved in bone remodeling as well as the speed of orthodontic tooth movement and to evaluate its role in the control of pain in patients. Materials and Methods: A total of 14 patients, aged from 15 to 35 years old, in good health and exhibiting at least 5 mm of crowding were randomly allocated to two groups: one group used a vibration device 20 minutes per day in combination with fixed appliances and the other group solely had fixed appliances treatment. Biomarkers in saliva, the regularity index, the mobility of the teeth, the oral quality of life as well as the level of pain experienced during treatment were examined during the trial for an overall period of 3 months. Results: Overall, no statistically significant difference in the rate of tooth movement was found between the control and the experimental groups. Multivariate linear regression was also performed to assess any potential correlations between the initial irregularity, age, sex and type of intervention on the reduction of the irregularity index. The only significant difference appreciated among the groups was attributed to the gender at T0 and T1, with the female group experiencing statistically significant less crowding. There was no significant difference in pain levels as well as the oral quality of life of the patients at any time points between both groups. Tooth mobility at the level of the incisors significantly increased after T0, peaking at the 5 weeks’ adjustment appointment. No statistically significant difference was seen in the expression of IL-1B, IL-8 and TNF-α when control group was compared to Acceledent group. However, great variability was seen between patients. Conclusions: Application of vibration during orthodontic treatment did not show a statistically significant difference in the rate of tooth movement, the level of pain experienced by the patient as well as the oral quality of life assessment compared to the fixed appliances only group. ▼ In the field of orthodontics, gingival crevicular fluid (GCF) is one of the main targets for assessing biomarkers. The term GCF describes the fluid that escapes from between the tooth surface and the epithelial lining of the periodontal ligament (PDL). There are over 100 regulatory proteins that can be detected in GCF, and they can be categorized on the basis of their biological significance such as bone resorption or formation. OBJECTIVES: To investigate the effect of vibratory stimulation (AcceleDent(R) Aura) on the expression of biomarkers in GCF using a human tooth movement model with clear aligners. METHODS: Prospective, single-center, randomized controlled crossover study comparing an active vibration device to a sham device. Participants received 6 Zendura(R) plastic aligners to move a target tooth 1.98 mm over 12 weeks. Patients were crossed over to the opposite device at the 4th aligner. GCF was collected at T0 (baseline 1), T1 (baseline 2), T2 (week 1), T3 (week 7, crossover) and T4 (week 12). 11 biomarker levels quantified by multiplex bead immunoassay. A mixed model analysis was used to compare the effects of active versus sham device on GCF composition. RESULTS: 16 males and 23 females completed the protocol. No statistically significant differences were found between the biomarker levels of the Sham-Active (SA Group) and Active-Sham (AS Group). The results of each group were then combined and biomarker concentrations were compared to tooth movement. There was a negative correlation between the baseline level of IL-6 and the total amount of tooth movement. A negative correlation was seen between the baseline levels of MMP-9 and tooth movement between weeks 1-2 followed by a positive correlation between the concentration of MMP-9 at T4 and the amount of tooth movement over the course of the study. CONCLUSIONS: The results suggest that vibration does not influence the secretion of GCF biomarkers; nevertheless, tooth movement with clear aligners seems to alter the proteomic composition of GCF. This study was registered on https://clinicaltrials.gov (ClinicalTrials.gov Identifier: NCT01962012). It was approved by the Western Institutional Review Board (WIRB20131856).
