▼ Postoperative delirium (POD) is a common and often unrecognized cerebral dysfunction following surgery. Among predisposing and precipitating risk factors, blood pressure-related aspects, such as intraoperative hypotension (IOH), were associated with a higher incidence of POD. POD has been shown to increase mortality rates, treatment costs and dementia. One study used variance to link blood pressure fluctuations to higher rates of POD. The literature cannot provide a definition nor an operationalization of these fluctuations. Most operationalizations are based on the mean blood pressure or chronological trends. The aim of this study was to investigate an association between blood pressure fluctuations, independent of the mean and pathological blood pressure values, and POD.
Methods
This study is a secondary analysis of the "Surgery Depth of anesthesia and Cognitive Outcome" (SuDoCo)-study, registered under International Standard Randomised Controlled Trial Number (ISRCTN) 36437985, which assessed an association between the depth of anesthesia and POD in 1277 patients. From this study population, 1000 patients were randomly selected. Using the intraoperative blood pressure values, we defined blood pressure fluctuations as the sum of the absolute magnitude changes between every two successive measurements. The multivariate analysis of the association between blood pressure fluctuations and POD had a total of 12 adjustment variables, including length of anesthesia, intraoperative hypotension, the use of antihypertensive medication, and variables describing patient health status.
Results
Data on POD was available for 961 patients, and complete data sets for all adjustment variables were available for 924 patients. In total, POD was detected in 206 patients (21.4%). Intraoperative systolic, diastolic, and mean arterial blood pressure (MAP) fluctuations were significantly higher in the POD group. In the multivariate analysis, there was a significant association between intraoperative systolic blood pressure fluctuations and POD (per 100 mmHg: odds ratio (OR) 1.12 (95% confidence interval (95% CI) 1.01-1.23), p = 0.03) and a tendency between MAP fluctuations and POD (per 100 mmHg: OR 1.17 (95% CI 0.99-1.37), p = 0.07). Other significant predictors of a POD included longer duration of anesthesia and hospitalization periods, increased age, higher number of secondary diagnoses, the use of an arterial catheter, higher ASA scores, and the use of antihypertensive medication.
Conclusion
Intraoperative fluctuations in systolic blood pressure were independently associated with a POD, while MAP fluctuations showed a tendency towards higher POD rates. Future studies should be performed to assess whether minimization of intraoperative systolic blood pressure fluctuations reduce the incidence of POD.

▼ For many decades, mast cells are primarily known for their detrimental role in
the elicitation of allergic reactions. Although physiological functions have
been postulated ever since the first description of mast cells by Paul Ehrlich
1877, it took until 1996 to provide first evidence for a protective role of
mast cells in bacterial infections. In the work described in the manuscripts
presented herein, we were able to identify and characterize other, until then
unknown important physiological functions of mast cells. For example, mast
ells act as important sensors and effectors contributing to protect the host
from various dangers such as bacterial infections, chronic UV damage, or
endogenous and exogenous toxins.

Introduction: Past decades have confronted humankind with new and emerging diseases of usually unknown pathogenic potential for the human population, nearly 70% of them being with zoonotic origin. In 1992, a new microorganism was identified in acanthamoeba and was called Bradfordcoccus due to its enormous size and its behavior in Gram staining tests. In 2003 French scientists proofed this pathogen to be a virus and it was named Mimivirus, derived from microbe mimicking virus. So far the pathogenicity of Mimivirus for humans is not understood. A few studies show that~ 2% of the healthy population can be seropositive for Mimivirus, increasing in patients with respiratory diseases and correlating best to pneumonia. However, Mimivirus DNA was only proven once in a pneumonia patient. A lab accident of a person working with Mimivirus lead to seroconversion and pneumonia. Moreover, the only Mimivirus isolate from a human could be obtained from a Tunisian patient suffering from pneumonia.
Methods: This study aimed at the establishment of a diagnostic system for Mimivirus to elucidate the prevalence of Mimivirus German samples.
Results: A duplex real-time PCR targeting two different genome regions of Mimivirus in combination with the modified sample preparation protocol was able to detect 140 genome equivalents of Mimivirus in 1 mL of a respiratory specimen. Serology was based on an immunofluorescence test which used the covering of WI-38 cells with Mimivirus particles, because tested human cells are not susceptible to Mimivirus. A more specific Western Blot could be used for confirmation. The seroprevalence of Mimivirus in blood donors could be determined as 2.3 %. Also, 3 out of 46 patients (6,5 %) with respiratory diseases were seropositive, while none of the 24 corresponding respiratory specimens was DNA positive for Mimivirus. From 18 children suffering from hematological disorders 68 respiratory specimens and 31 sera could be collected. Three patients were seropositive and 3 others were DNA positive in respiratory specimens. Interestingly, one patient was positive for Mimivirus antibodies as well as Mimivirus DNA prior to transplantation.
Take-home message: Although the case numbers from 2 years of monitoring are low, children with hematological disorders may have a higher risk to become positive for Mimivirus. The prevalence of Mimivirus in Germany in healthy blood donors and patients suffering from respiratory diseases see to be comparable to other countries. Finally there is no evidence for an increased pathogenic potential arising from Mimivirus for humans.

▼ Since the 1950s lung cancer is the type cancer with the highest mortality rate
and causes more deaths than the next three most frequent types of cancer
together. One of the main problems of radiation treatments of lung cancer
poses the high mobility of lung tumors due to breathing and heart beat. The
tumor motion due to breathing can be as much as 2,5 cm in cranio-caudal
direction. So far, methods to address this kind of tumor motion are very
insufficient. Because of that large radiation fields are being used to
guarantee that the tumor receives the minimal required radiation dose despite
of the tumor motion. As a result a large amount of healthy tissue receives
radiation, which is especially critical for the radiation sensitive lung
tissue. Conventional radiation delivery systems and linear accelerators are up
to now not able to solve this fundamental problem. Based on the x-ray and
infrared based positioning technology of the Novalis linear accelerator a
system for a respiration triggered tumor treatment was developed and brought
to clinical routine use. The system is applicable for all indications, which
are
subject to breathing induced motion with a special focus on lung and liver
tumors. Two x-ray tubes with two detectors make it possible to use bony
anatomy or implanted markers for patient positioning. The detection of
breathing motion by external markers and the correlation to an implanted
marker make it possible to predict tumor position and motion based on the
motion of the external markers. Using this method it is possible, via the
close to real-time information of patient / tumor position, to perform a
respiration triggered treatment. The developed concepts and methods were, in a
first step, verified on an especially constructed gating phantom. In a second
step the system was used for patient treatments to verify clinical usability,
achieved accuracy as well potentially achieved clinical gain. The achieved
results impressively show the potential of the system. The overall system
accuracy to position a moving target is within 1 mm and therefore at an
identically high level as the positioning accuracy of the Novalis system for
non-moving targets. As shown in comprehensive measurements, gated treatments
even allow the application of complex IMRT treatment on a moving target with
only little deviation of the applied to the planned dose distribution. The
total loss of the planned dose distribution when treating a moving target
without gating can be prevented and a homogeneous dose coverage of the tumor
as well as best possible protection of critical structures can be restored.
The developed gating phantom did prove to be very effective and user friendly,
offering functionalities that exceed the requirements for routine quality
assurance by far. The clinical usability of the gating system was proven by
the completion of this work on more than 20 treated patients with more than
200 treatment fractions. Clinical validation was performed on 19 patients (11

2007. Up to the 1980s the number of publications increase gradually. Only in
the last twenty years happened a explosive proliferation that is partly due to
the increasing importance of epilepsy research, partly to the inclusion of
abstracts since 1991 in the database, which simultaneously means a greater
match of the keyword search. There are extreme publication maximas in the
years 1995, 1999 and 2005 due to a very high number of meeting abstracts
published in the journal Epilepsia. Meeting abstracts make a total of 20% of
all publication formats since 1945, the year from their first appearance.
Articles make out 60%, they are the oldest publication type listed since 1900.
In this study stands out a negative trend of the number of articles and a
continuous increase in the number of meeting abstracts. The majority of
epilepsy-specific publications are published in the journal Epilepsia. The
highest citation rates, however, experience the epilepsy-specific publications
that appeared in the Annals of Neurology, presumably due to the lower
specialization. For the great importance of Annals of Neurology and Neurology
speak their high journal impact factors. The assignment of the epilepsy-
specific publications on topics takes place mainly in Clinical Neurology,
Neurosciences and Psychiatry. The average of two publications are assigned to
three topics. In 2007 an average of five authors are involved in a
publication. The highest number of publications are released by Frederick
Andermann. For the importance of his research militates in favour his h-index
of 70. On the other hand he is mentioned in more than 66% of the publications
as co-author. In the mean, authors with the highest numbers of items
published, where mentioned to 20% as first-, 40% as co- and for 40% as senior-
authors. The highest h-indexes are combined in a very few authors. Values
greater than ten are made out only by about 1.3% of all authors. 47% of the
authors have an h-index of one. Less than 20% of all authors have higher
h-Indexes than 3. The publication language english makes out more than 91% of
all cases with a trend towards a further increase, while the next most common
publication languages are french and german. The most important institutions
in the wordwide epilepsy research are the University of California at Los
Angeles with the highest number of publications and Yale University with the
highest citation rate. The most common cooperations exist between University
of Toronto and the Hospital of Sick Children, Toronto. Worldwide, there is a
steady increase in cooperation articles. In 81% of all cooperation articles
participate two countries. The United States of America are the most frequent
partner. Most of all collaborations are seen between the U.S. and Germany. In
Europe, most cooperative articles exist between Britain and Germany.

▼ In 2007 approximately 33 million people were living with HIV, 67% of them in
sub-saharan africa. Nearly all of the 370 000 children newly infected with HIV
in 2007 aquired the virus by maternal-infant transmission. Programmes to
prevent mother-to-child transmission of HIV (PMTCT) include the components
voluntary counselling and testing for pregnant women, antiretroviral
prophylaxis during childbirth and postnatal counselling about infant feeding
options. The present study was undertaken to assess the level of awareness
regarding mother-to-child transmission of HIV and the attitude towards HIV-
preventive measures among the population of a future project region of a PMTCT
programme in western uganda. Basic knowledge about the means of HIV
transmission and HIV prevention were also enquired. Associations between the
level of knowledge and awareness and sociodemographic characteristics should
be identified. The data are drawn from interviews with clients and stuff of
health institutions, residents of rural villages
und traditional birth
attendants. Sexual transmission of HIV was spontaneously mentioned by 91.2 %
of the male and female clients and villagers, mother-to-child transmission of
HIV by only 3.1%. Mother-to-child transmission of HIV was significantly less
mentioned by female
und less educated participants (p<0.001). That prevention
of a HIV infection is impossible to avoid was reported by one fifth (20.9%) of
the female participants. The more educated a participant, the more preventive
measures of HIV infection could be named (p<0.001). The proportion of male and
female clients and villagers previously tested for HIV was 10.3%. The
willingness of the untested participants to undergo a HIV test was high
(91.3%). 67.5% of the male clients and villagers would accept replacement
feeding if this could prevent the newborn from getting infected with HIV.
Farmers were less likely to accept replacement feeding than other
professionals (p=0,004). All of the participating health workers but only
64.3% of traditional birth attendants were aware of mother-to-child
transmission of HIV. All awareness-raising measures and information regarding
a PMTCT programme should be assessible for individuals with little or no
education. As the uptake of voluntary testing for HIV is low but the
willingness to be tested is high in the study population, routine offer of HIV
testing must be implemented in more settings and on a wider scale. HIV
education programmes should be established for both health care professionals
and traditional birth attendants. The findings of this investigation
illuminate the need of an increased HIV knowledge, especially regarding the
mother-to-child transmission of HIV.

▼ The evaluation and validation of the „Tremormeter“ for characterizing and
quantifying the physiologic tremor (PT) in children was the goal of this
dissertation. The “Tremormeter” consists of a metal plate with nine holes of
decreasing diameter. Subjects have to hold a metal stylus into each hole for
30 seconds without touching the edge. I examined 1.142 healthy students aged
six to 25 years. In this age, motor development must be taken into account
when evaluating the test performance. For this reason, I stratified the values
age- and gender-specifically. The subjects also completed the Frostig
Developmental Test of Visual Perception (Subtest I). This was to quantify the
influence of the eye-hand-coordination on the results of the “Tremormeter”. I
also evaluated the influence of other factors (handedness, consumption of
nicotine, drugs and caffeine and the duration of sleep during the night before
the test). After this test series, I also assessed the influence of the “real”
tremor as objectified with accelerometry. I examined 102 adults with the
“Tremormeter” as well as with triaxial accelerometry. My hypotheses were the
following: (1) a high tremor frequency in accelerometry results in many
contacts between the metal stylus and the edge of the hole and (2) a high
tremor amplitude in accelerometry results in a contact between stylus and edge
in a hole with a comparatively large diameter. These hypotheses could not be
proven. I employed a multiple linear regression analysis to quantify the
influence of the different factors (age, gender, handedness, consumption of
nicotine, drugs and caffeine and the duration of sleep during the night before
the test) on the “Tremormeter” results. For the subjects aged six to 18 years,
I found a highly significant (p<0,001) influence of age and gender on the test
results. This influence could not be shown in the adult subjects. I could not
find any influence by the consumption of nicotine, drugs or caffeine. This
might also be due to the only qualitative (not quantitative) analysis of these
factors. Handedness and sleep duration showed a significant influence on some
parameters of the “Tremormeter” test. The eye-hand-coordination as assessed by
the Frostig Developmental Test of Visual Perception (Subtest I) showed to be a
significant influence on the performance in the “Tremormeter” test. The eye-
hand-coordination (together with the factors age, gender, handedness,
consumption of nicotine, drugs and caffeine and sleep duration) accounts for
28-42% of the “Tremormeter” results. The results of accelerometry (together
with age, gender, handedness, consumption of nicotine, drugs and caffeine)
account for 10-36% of the results. Further factors are probably influencing
the test results. These could be metabolic, hormonal or toxicologic factors.
But since age and gender have proven to be the most important influences,
especially motor development is probably very strongly influencing the test

▼ Ballet dancers are competitive athletes who undergo extreme physical and
mental stress and work according to an irregular schedule, with long days of
training, rehearsal, and performance. Their most significant potential risks
entail physical injury and altered sleep. The elaborate training requirements
for ballet dancers do not allow regular chronobiological patterns or a normal
sleep-wake rhythm. The aim of the study was to investigate the sleep-wake
rhythm and sleep quality during rehearsal phases prior to a ballet premiere.
Wrist actigraphy and sleep diaries were used for a period of 77 days before
the ballet premiere performance to study 24 classical ballet dancers. To
assess quality of sleep, aspects of cognitive performance and health status
the Epworth Sleepiness Score (ESS), the Pittsburgh Sleep Quality Index (PSQI),
the SF-12 Quality of life Assessment, and the d2 Test of Attention have been
applied. It was noted a significant reduction in sleep duration and sleep
efficiency (from 81 % to 79 %) over the 77-day course of the rehearsal. It was
also found a decline in time in bed and an increase in wakefulness after sleep
onset. However, the changes in sleep as documented by actigraphy were not
reflected by the subjective data of the sleep diaries and sleep scores. As a
result of the facts that total sleep efficiency and sleep duration values were
already lower than usual for the dancers' age group at the beginning of the
study and that mental acuity, concentration, and speed were likewise impaired,
it was observed an exacerbated health deterioration in terms of sleep
deprivation in ballet dancers during preparation for a premier. It was
concluded that individual activity-rest schedules, including daytime naps, may
be helpful – especially during the stressful training and rehearsal
experienced prior to ballet premieres.

▼ alpha-Mannosidase I N-linked protein glycosylation represents an important
cellular process for modifying protein properties. It resembles a cascade of
various enzymatic reactions, in which class I alpha−mannosidases play a
central role. It is well established that N-glycosylation plays a major role
for immune functions. Interestingly, Sawitzki and colleagues identified alpha-
mannosidase I as being highly expressed in graft infiltrating cells of
tolerance developing recipients. I now studied the expression and function of
alpha-mannosidase I in total CD4+, naïve and memory T cells by analysing
alpha-mannosidase I transcription and activity. Alpha-mannosidase I function
was altered by i) treatment with Kifunensine, a specific inhibitor for class I
alpha-mannosidases, ii) downregulation of alpha-mannosidase I gene expression
using siRNA transfection, and iii) overexpression utilising retroviral gene
transfer. T cell activation was evaluated studying CD69 expression, IL-2 and
IFN-gamma production. My results demonstrate that alpha-mannosidase I
transcription is transiently down-regulated following T cell activation, and
that alpha-mannosidase I exerts an inhibitory effect on T cell activation.
Most interestingly, these effects were restricted to naïve CD4+ T cells, while
memory cells remained nearly unaffected. Thus complex N-glycans generated by
enzymes such as alpha-mannosidase I inhibit the activation of naïve T cells.
These findings could be used to improve the ex vivo priming of naïve T cells
for adaptive T cell therapies. Receptor for hyaluronan-mediated motility
(RHAMM) Sawitzki and colleagues identified the receptor for hyaluronan-
mediated motility (RHAMM) as being highly expressed in transplantat
infiltrating leucocytes of acutely rejecting recipients. I now studied the
expression and function of RHAMM during T cell activation in vitro and in
vivo. My results demonstrate that RHAMM transcription is highly up-regulated
during T cell activation. Furthermore, inhibition of RHAMM expression
utilising siRNA transfer resulted in decreased IFN-gamma expression by T cells
as well as diminished their migratory potential and effector capacity in a
model of transfer colitis. The results also show that in the murine system
more than three RHAMM isoforms are transcribed. However, their transcriptional
regulation and role during T cell activation are still unknown. Using siRNA
technology transcription of RHAMM can be suppressed, providing a possibility
for a more detailed examination of its role during T cell activation.

endemically spread in more than 20 countries in the Latin American region.
About 100 million people live in the endemic area; 16-18 million people are
chronically infected. The infected people are lifelong carrier of T.cruzi
after the infection and whit a moderate acute phase of the disease. 30 % of
the infected patients develop symptoms (chronic Chagas’ disease) after 10-20
years of a symptom-free interval (asymptomatic phase, indeterminate phase,
latency phase). 90 % of the chronic Chagas patients develop the live
threatening heart disease (Chagas cardiomyopathy) while the remaining 10 %
develop gastrointestinal complications (Megacolon, Megaesophagus). The late
complications of the chronic Chagas’ disease are taken together an enormous
socio-economic burden for the Latin American countries. The aim was,
therefore, the finding of marker which enable 1\. the early detection of
patients which possess a high risk for the development of late complications
and 2\. the early detection of the transition of the asymptomatic phase into
the symptomatic phase of the Chagas’ disease. Based on the pathogenesis of the
late complications, autoantibodies (AAB) against G-protein coupled receptors
such as the ß1-adrenoceptor (ß1-AAB), the ß2-adrenoceptor (ß2-AAB), and the
muscarinergic receptor 2 (M2-AAB) might be of special prognostic importance.
Unspecific inflammation marker and marker of the oxidative stress are
important for the early diagnostic of the late complications while for the
heart diseases additionally specific marker such as NT-proBNP and hs TnT are
applicative. T. cruzi positive patients were enrolled in Bolivia over a period
of two years. The T. cruzi positive patients were divided into the following
groups: 1. asymptomatic patients, 2. Chagas cardiomyopathy patients, 3.
Megacolon patients, and 4. patients suffering from cardiomyopathy and
megacolon according to the medical history, Holter-ECG, and radiologic
investigations. A typical sign for the Chagas cardiomyopathy was the
occurrence of ß1-AAB and M2-AAB, for the megacolon the combination ß2-AAB and
M2-AAB. One third of the patients in the latency period showed serum AAB, of
which 85 % had the Chagas cardiomyopathy pattern while the remaining 15 %
showed the megacolon pattern. Taken together the percentages of AAB positive
patients in the latency period in total and the percentages of the specific
AAB patterns for cardiomyopathy and megacolon, these data correspond
completely with the epidemiological data of the percentages developing late
Chagas complications. The usefulness of the AAB diagnostic for an early risk
assessment in the asymptomatic phase for an early identification of the
development of late complications is therefore postulated. Prospective studies
are, however, necessary in order to verify this hypothesis. NT-proBNP and

▼ Objective: Patients with isolated secundum atrial septal defect (ASDII) or
isolated patent foramen ovale (PFO) were to be phenotyped and screened for the
GATA4 S377G polymorphism (SNP). Its prevalence within the study cohort was to
be defined. We hypothesized that the GATA4 S377G SNP may be associated with
either ASDII or PFO and that it may alter the clinical parameters of these
atrial defects. Methods: In a retrospective cross-section study, 92 patients
with isolated ASDII and 95 patients with isolated PFO were recruited between
June 2005 and July 2006 at the Department for Congenital Heart
Disease/Paediatric Cardiology of the German Heart Centre Berlin (DHZB).
Clinical parameters were pseudonymized and entered into an online database.
Genotyping was performed by the workgroup of Posch MG & Özcelik C et al.
(Cardiogenetic Lab., Experimental and Clinical Research Centre) using single-

highly overrepresented in patients with PFO (p<0.0001). The prevalence of the
G-allele was significantly higher in PFO patients compared to ASDII patients
(p<0.00001) and controls without ASDII or PFO (p<0.00001). Surprisingly,
genotypes did not significantly differ between ASDII patients and controls
(p=0.73). Furthermore, two mutations in GATA4 were identified in a patient
with PFO, A346V and D425N, both previously described in patients with ASD by
other authors. As to the clinical characterization, all differences between
patients with ASDII and PFO were due to the pathophysiology of the particular
defect. Based on permanent left to right shunting in ASDII patients from
birth, clinical parameters which indicate a possible overcharge of the right
heart, such as size of the right ventricle/atrium, right ventricular pressure,
deviated electric axis of the heart and right bundle branch blocks were
significantly higher in patients with ASDII than in those with PFO. Symptoms
at diagnosis differed in both groups but can only be explained by
pathophysiological differences. The GATA4 S377G SNP was not associated with
any defect related phenotype. In neither one of the defect groups, ASDII or
PFO, clinical parameters differed between patients with or without the GATA4
S377G SNP. Only in patients with the homozygote variant of the GATA4 S377G
SNP, some peculiarities could be identified. Within this group defects were
always localized in the centre of the atrial septum and always closed during
cardiac catheterization. Atrio-ventricular Blockage never occurred in these
patients. Yet, the number of cases in this subgroup is not large enough for
any reliable assessment of a genotype-phenotype association. Conclusions: In
the present study, a highly significant association of the GATA4 S377G SNP
with large PFO was found. For the first time, we could identify mutations of
the GATA4 gene in a patient with PFO. Known clinical effects of isolated ASDII

▼ T cell depleting strategies are an integral part of immunosuppressive regimens
used in solid organ transplantation. Although it is known that the polyclonal
rabbit antithymocyte globulin (rATG) or the monoclonal anti-CD52 antibody
alemtuzumab induce lymphocytopenia, little is known about the effects of these
therapeutic antibodies on Natural Killer (NK) cells in more detail. An
induction therapy with rATG results in a rapid depletion of both T cells and
NK cells in patients with combined kidney/pancreas transplantation. Based on
this observation the mode of action of rATG and alemtuzumab was analyzed with
continuative in vitro experiments. Treatment of NK cells with rATG and
alemtuzumab leads to an impairment of cytotoxicity and induction of apoptosis.
In this context it is of clinical relevance that NK cells are 10 to 100 times
more susceptible to both therapeutic antibodies than T or B cells. By
generating Fc-parts of rATG and alemtuzumab it could be illustrated that their
ligation to FcγRIIIa (CD16), expressed exclusively on CD3-CD56dim NK cells, is
sufficient for induction of apoptosis, degranulation and massive inflammatory
cytokine release suggesting that that NK cells are important mediators of the
clinically observed cytokine release syndrome. As NK cells are functionally
relevant for the effective combat of long-term immunosuppression complications
including opportunistic viral infections or tumors these findings should be
considered for the generation of new therapeutic antibodies in the future and
in defining the optimal treatment dosage in clinical settings. Furthermore the
effect of rATG on regulatory T cells was analyzed as these cells were
comprehensively discussed in the context of diminishing the risk of rejection.
Interestingly, by determining the methylation status of the transcription
factor FOXP3 it could be shown that an induction therapy with rATG depletes
bona fide regulatory T cells after kidney transplantation. In addition it has
been shown that murine regulatory T cells express Fcγ-receptors in a SHIP-
deficient mouse model suggesting a potential responsiveness to antibodies or
reactivity towards opsonized cells.

▼ Multiple Sclerosis is next to epilepsy the most common neurological disease of
the young adult in North America and Europe. Characteristic for the disease
are dissiminated lesions of demyelination and inflammation around medium sized
vessels in the CNS. Today, auto reactive T-cells are assumed to cause the
disease, after being activated in the periphery and having crossed the blood
brain barrier. We know only little about the fate of immune cells once they
cross the blood brain barrier. In the present analysis, different T-cell
subsets have been tracked by two photon microscopy on their way through the
CNS parenchyma of non inflamed murine hippocampi. This work demonstrates
differences in the motility of T-cells depending on their activation state,
and a distinct migrational behaviour of CD4+ and CD8+ T-cells. CD4+ T-cells
show a CXCL12-dependent movement along the parenchymal side of the vessels,
while CD8+ T-cells move throughout the parenchyma. These results are in
agreement with recent analyses of human organotypic slice cocultures, and they
point to the limitations and possibilities of therapeutic CXCR4 antagonists to
manipulate the movement and action of immune cells in the brain.

▼ The allogenic bone mark transplantation, a treatment modality for malignant
diseases of the hematopoietic system, has the disadvantage of a Graft-versus-
Host disease (GvHD), even under a complete congruency within the MHCs, caused
by minor histocompatibility antigens (mHAgs), additional transplantation
antigens, existing outside the MHCs. However, by the identification of only
hematopoietically expressed mHAgs and by the stimulation of donor lymphocytes
which are applied as a therapy on a KMT (donor lymphocyte infusion) with
peptides processed of transcription products of such mHAgs a GvL (Graft versus
Leukaemia) effect without GvHD could be reached. In order to test that a mouse
model was developed where two mouse strains (C.B10
und B 6) identical in MHC
distinguish from each other in 5 mHAgs. The select H 13 mHAg exists in the
C.B10 strain (donor) as H 13b-Haplotype or at the B6 strain (recipient) as
haplotype H13a. The peptides processed of these two haplotypes, SIL 9 at the
donor strain and SVL 9 at the recipient, show the difference on an amino acid
position (isoleucine instead of valine) due to the substitution of a methyl
group and they are both represented by MHC-class I Db–molecules. Recipients
were sublethally irradiated and i.v. with the hematopoietic cells of the donor
injected in a mixture of B6-strain-derived leukaemia cells, EL 4, or its
ovalbumin-transfected variant EG 7. The tumour presence and the dynamics of
its growth were weekly examined by the bioluminescence-imaging method and the
treatment was started according to one of the two basic principles: either the
donor was immunised in vivo by the B6 splenetic cells and SVL 9 Peptide and
afterwards restimulated in vitro, or the donor lymphocytes were stimulated
only once in vitro with SVL 9. Both therapeutic options were applied to the
mouse groups standardized due to the size of the tumour and the presence of
the remaining tumour cells were examined by the ß-galactosidase test (Gal-X)
after the treatment. In both therapeutic options the treatment with the
immunised donor lymphocyte infusion resulted in a longer survival (more than
half of the mice survived more than 80 days), compared to the treatment with a
non-immunised donor lymphocyte infusion (39-70 days) or to the control group
(22 – 34 days). In mice difficult to treat the regulatory CD4+/CD25+- and
CD4+/Foxp3 T-lymphocytes were examined and in mice who survived more than 80
days the presence of SVL 9–specific CD 8+–T-lymphocytes was examined by CFSE-
in vivo cytotoxicity tests.

▼ Biomechanical factors in fracture healing are yet not been completely
finalised and current clinical methods for monitoring fracture healing are
often invasive and inaccurate. This study evaluates the non-invasive
measurement of the interfragmentary movements and ground reaction forces in
two monolateral external fixators with different stiffness to define the ideal
factors that optimise the fracture healing. Standardised 3 mm diaphyseal bone
defects were created in the right tibia of 64 female sheep and stabilised with
either a rigid monolateral external fixator or a more flexible variant. Over a
nine week healing period gait parameters were measured using a pressure
sensitive platform and interfragmentary movements at the fracture site were
monitored. The tibiae were tested biomechanically after sacrifice. All animals
unloaded the operated and overloaded the contralateral hindlimb. Callus
mineralization and stiffness, as well as limb loading increased during healing
whilst interfragmentary movements were reduced. Larger interfragmentary
movements resulted in a slower fracture healing rate. Micro movements in the
axial plain between 0.5 mm and shear movements smaller than 0.8 mm stimulate
the callus formation and optimise the fracture healing.

▼ Drowning is a constant global problem which claims about half a million
victims worldwide each year, mainly in developing countries. The number of
near-drowning victims is considerably higher. The previous and current
literature of the
subject “drowning” will be analysed quantitatively and
qualitatively by using scientometric tools. There were 2381 articles listed in
the ISI-Web, using the search term “drowning” and including the years 1900
until 2006. The publication effort has increased constantly in the past few
years. Almost 93 per cent of these articles were published in English,
approximately 71 per cent are original papers. With 800 articles about
drowning, the United States are the most publishing nation by far. According
to the WHO data of the year 2002, the highest numbers of drowning victims are
recorded in China (113000) and India (69000). However, the highest drowning
rates are allotted to African states. According to the citation analysis, most
citations (36) are allotted to the publications of the year 1921. Regarding
the citations of citation years, most citations (1770) are recorded for the
year 2006. The trend analysis demonstrates an increase of citations for the
past years. English is not only the dominant publication language but also the
most cited language with an average of 8.84 citations per article. Without
exception, also all journals publishing to the topic drowning are in English.
Among them is the JAMA (Journal of the American Medical Association), a very
famous medical journal with an impact factor of 23.175 for the year 2006. The
analysis of the
subject area shows that most papers (16.67 per cent) are
related to the category Medicine, General & Internal. The subheading analysis
on the PubMed database demonstrates that among the ten most publishing
nations, the main subheadings are epidemiology, etiology and diagnosis of
drowning incidents. According to the author analysis, the most productive
author is Modell, JH with 34 articles about the
subject drowning. Interest and
feedback on the part of the research community on the topic drowning have been
rising continuously in the past three decades. It can be assumed that,
according to the actuality of drowning incidents, e.g. because of natural
catastrophes, and the climate change, the scientific turning to the
subject
drowning will achieve even greater dimensions.

▼ Purpose: Metastases from uveal melanoma have an extremely poor prognosis.
Factors predicting development of metastases and survival are of critical
importance for future adjuvant trials investigating innovative
immunotherapeutic approaches in patients with uveal melanoma. Patients and
Methods: We analyzed all patients with uveal melanoma, who received primary
treatment in 1994 and 1995 in our hospital, from whom we previously had
reported 5-year follow-up data. Data on primary tumor, local therapy and
follow-up were collected. Results: Of 271 patients 78.2% of patients were
available for 10 years follow up. Median follow-up was 123 months. A total of
68 patients (25.1%) developed metastases. Kaplan-Meier analysis revealed a
10-year progression free survival (PFS) of 65% for the whole patient cohort.
10-year PFS was 25% in patients with extraocular tumor growth (EOG). Patients
with ciliary body involvement (CBI) or a largest tumor diameter (LTD) > 14mm
had a 10-year PFS of 56.3% and 52%, respectively, which was significantly
lower than in patients without ciliary body involvement or LTD < 14mm. 10-year
overall survival was 57.5% for the whole patient cohort. 10-year survival was
20% in patients with EOG and 42.3% in patients with CBI and 43.2% in patients
with LTD > 14mm. Univariate analysis revealed EOG, CBI and LTD > 14mm to be
predictive for overall survival (p<0.02, log-rank test for all variables).
Conclusion: This study confirmed our previous 5-year follow up data and
determined ciliary body involvement and a largest tumor diameter > 14mm to be
the most important clinical risk factor for metastases and survival. Therefore
patients with ciliary body involvement and LTD > 14 mm are candidates for
adjuvant therapeutic interventions within future trials.

▼ Abstract Invasive ventilation remains an important treatment option for
patients with acute respiratory failure. However, it involves the risk of
various complications such as ventilator-associated pneumonia or weaning
failure. It has been known for many years that delayed weaning from
ventilation presents a high risk for the outcome of ventilated patients. Many
factors have an effect on the course of ventilation. Different constitutional
situations make it necessary to adapt artificial ventilation individually to
patients. The pathophysiological properties not only of overweight, but of
underweight as well, require different weight-adapted management of the
ventilated patients. Despite the huge amount of physiological information, the
role of body weight and related evidence-based strategies during ventilation
and weaning has been the
subject of some controversy. In recent years, there
has been increasing evidence that being overweight has a positive effect on
the clinical outcome of intensive care patients. The clinical disadvantage of
underweight is currently the
subject of research. Methods, Clinical Question
This is a report of a retrospective, multi-center observational study
including a total of 923 patients in Germany. The following clinical questions
were discussed in this study: Were patients ventilated differently depending
on their BMI? Were any differences found with respect to the ventilation
parameters among the various BMI groups? Did ventilation and weaning periods
differ depending on BMI? What role did BMI play during ventilation and
weaning? First, demographic data were recorded for ventilated patients who
fulfilled the inclusion criteria. In the next step, the patients were assigned
to BMI groups according to the WHO classification. During the 28-day
observation period, the modes and parameters of ventilation were recorded and
documented for the duration of the controlled ventilation period and the
weaning period. These parameters were analyzed on the day of intubation, day
of extubation, and during the course of ventilation. Results Our investigation
yielded the following results: There were only slight differences among the
BMI groups regarding demographic data, ventilation methods, and causes leading
to ventilation. The use of lung-protective ventilation with small tidal
volumes of 6ml/kgPBW was not adhered to in patients of all BMI groups.
Patients were given higher tidal volumes the higher their BMI was. Related to
their weight, patients with a high BMI were given the lowest tidal volumes.
These results suggest that tidal volumes were applied depending on the BMI
after a subjective estimate of the body weight, completely independently of
objectively measurable parameters. The BMI was not taken into consideration
when setting the positive end-expiratory pressure. The relatively low PEEP in
comparison with the BMI was associated with a lower oxygenation index in

▼ Patients with chronic kidney disease (CKD) often show other organic
disturbances concomitantly. Primarily, cardiovascular diseases (CVD) lead to
complications during therapy. Amongst other complications accumulation of
uremic retention solutes which under normal circumstances are removed by the
healthy kidney seem to be accountable for the observed cardiovascular
symptoms. In this PhD thesis 74 known uremic retention solutes were tested
concerning their vasoactivity in an isolated perfused rat kidney. Through this
systematic testing nine uremic substances could be identified, yet unknown to
have vasoactive potential. The results showed a vasoconstrictive effect for
the uremic retention solutes 1-methyladenosine, endothelin-1, methylguanidine-
HCl, neuropeptide Y, phenol, p-cresol and uridine. Adrenomedullin, atrial
natriuretic peptide, urea, hydrochinone, potassium indoxyl sulfate and
p-cresol sulfate showed a vasodilatation. For the first time a vasoactive
effect in the rat isolated kidney was shown for the uremic toxins
1-methyladenosine, urea, hydrochinone, potassium indoxyl sulfate,
methylguanidine-HCl, phenol, p-cresol, p-cresolsulfat and uridine. The
recorded dose vs blood pressure curves of potassium indoxyl sulfate, p-cresol
and p-cresol sulfate showed a linear progression whereas the other vasoactive
substances rather showed a sigmoidal course. P-cresol (∆p +17,4 mmHg) and its
conjugated derivate p-cresol sulfate (∆p -38,7 mmHg) showed an opposite
vasoactivitiy at the same dose of 5 x 10-5 mol. The relative efficiency was
ranked by comparing uremic retention solutes with a similar ED50 value to the
application of the same amount of test-bolus. The ranking for vasoconstrictive
substances with an ED50 value of -6±2 log mol and a test-bolus of 10-6 mol
showed the following correlation: uridine (51,8 mmHg) > methylguanidine-HCl
(38,7 mmHg) > 1-methyladenosine (32,3 mmHg) > phenol (14,2 mmHg) > p-cresol
(4,0 mmHg). The ranking for vasodilatory substances was as follows: p-cresol
sulfate (-26,0 mmHg) > potassium indoxyl sulfate (-24,9 mmHg) > hydrochinone
(-16,9 mmHg) > urea (-0,6 mmHg). Additionally, pharmacological
characterization of receptor specificities for the nucleotides
1-methyladenosine, methylguanidine-HCl and uridine was performed. As it turned
out, vasoconstriction of 1-methyladenosine and uridine was mainly mediated by
A1-receptors. Whereas P2-receptors didn’t seem to be involved in this
regulatory mechanism. Altogether, results of this PhD thesis showed uremic
retention solutes to contribute to the development of CVD in patients with CKD
by influencing the vascular tone. Upcoming, more detailed in vitro experiments
with various organs, cell tissues, and isolated enzyme cascade systems will
provide further insight into the molecular action of uremic substances in
vivo. The data generated here and the ongoing identification of particularly
unfavorable uremic substances should finally proceed to a more specific
therapy by showing fewer side effects at the same time for patients suffering

▼ For this work we measured the frequencies of specific T-cells after hybrid
cell vaccination of 29 melanoma stage III and IV patients. There is a lot of
research about immune therapies against malignant melanoma. Amongst others
there are studies with autologous, allogeneic or peptide vaccines. In this
case we extracted tumour materials to produce an autologous vaccine, which we
fusioned with donored dendritic cells. This kind of vaccine shall expose the
immune system of the patient again with the tumour antigens to induce an
immune reaction. Focus of this immune response are tumour-specific cytotoxic
T-cells, which are considered to be the major effector cells of the anti-
tumour immune response. The frequencies of these tumour specific T-cells are
measured by FACS analysis before vaccination, and 24h and 48h afterwards,
respectively. The first three vaccination cycles have been evaluated.
Additionally different injection spots and differing amounts of injected
hybrid cells are compared to each other. Moreover the question came up, if
there is a correlation between the clinical disease development of the
individual patients. After stimulation of single tumour antigens partly
frequencies of the specific T-cells in the range of 0,01% - 8% had been
metered, partly specific T-cells could not be verified. Depending on the
patient HLA-A2 or HLA-A1 restricted tumour antigens had been examined. A
correlation with clinical disease development could not be recognised. Nor
have there been a relevant difference in choosing a certain injection spot or
a certain amount of injected cells. This work illustrates, that one is still
trying to understand and to improve the respective immune therapies against
malignant melanoma. One part of the above is the analysis of the therapeutic
induced immune response, e.g., the frequency determination of induced T-cell.

▼ In the present paper the influence of Clostridium botulinum C3-protein on the
glutamate uptake and release in cultivated mice-astrocytes was determined.
First of all the results of previous works on the influence of C3bot on
astrocyte morphology and the associated decrease of RhoA-proteinlevels could
have been validated. In C3bot-treated cultures a significant increase of the
uptake of tritium-labelled Glutamate could have been measured. The following
studies of the glutamate-transporters showed that the expression of
transporter GLT-1 was increased significantly and the expression of GLAST was
rarely affected. Pharmacological studies could verify, that the increase in
glutamate-uptake relies on the activity of this transporter. Furthermore it
was shown that also vesicular glutamate transporters such as VGLUT1-3 were
expressed, but not influenced by C3bot in the analyzed cultures. Despite this
the increased inhibition of glutamate uptake by bafilomycinA1 after C3bot-
treatment suggests an involvement of vesicular storage processes. The C3bot-
treatment also led to an increase of Ca2+-dependent secretion, as shown with
the Ca2+-Ionophore Ionomycin. In further studies a significantly increased
expression of SNARE-protein Synaptobrevin could have been demonstrated.
Visualization and quantification of vesicular fusion events with the plasma
membrane via AM1-43 dye supplied evidence of augmented numbers of these events
after treatment with C3bot. These indications of increased exocytotic
glutamate release were confirmed via Clostridium botulinum Neurotoxin D, which
could block most of the C3bot-associated release. The conducted experiments
proved a determing role for Rho-proteins in regulation of uptake and release
of the most important excitatory transmitter and therefor also for the whole
glutamate-homeostasis in the CNS. They also suggest that previous shown
effects of C3bot-proteins on neuronal growth could possibly also rely on
indirect-mediated, glial effects.

▼ This habilitation thesis investigated how the use of computed tomography (CT)
with subsequent 2D and 3D reformations can contribute in animal experiments
and to the 3R concept in terms of reduction, refinement and replacement.
Especially computed tomographic scans have a high value in the comprehensive
analysis of animal experimental projects due to the short acquisition time.
The low-risk imaging of the animal by CT and postprocessing of the acquired
volume data sets make it possible to answer questions even far beyond the
initial question. The habilitation thesis presented here demonstrates the
value of CT for animal studies and especially in the areas of replacement,
reduction and refinement according to the 3R principles of Russell and Burch:
We could detect pulmonary changes on CT representing influence of non-
physiological positioning on lung perfusion and ventilation (see page 13).
Also we were able to image the reasons for limitations of veno-venous
hemofiltration and present alternatives (see page 23). For refinement of the
animal model "Minipig" the central vessels were measured in vivo by CT at
different time points and then correlated to histology. For the first time a
data pool was generated, which -without restriction of fixation and shrinkage-
is comparable to what will be encountered during surgery or interventional
procedures (see page 35). Furthermore, the influence of weight and growth on
the vascular system of minipigs aged between 16-20 months could be
relativized. The evaluation of a vein cuff, which has been tested according
its position, function and biocompatibility, was done exclusively on CT data
sets. With the help of CT most of the questions could be answered during the
course of the trial without killing the animals. The aspect of reduction (less
animals) was successfully implemented here (see page 40). Two aspects of
physiological and pathophysiological changes were successfully represented by
means of computed tomographic volume data sets : It has been shown that -and
how- a complete occlusion of the aorta can be completely compensated without
clinical symptoms in female domestic pigs due to specific collaterals (see
page 51). Finally, the concept of the critical defect size was called into
question observating bone defects in the mandible of minipigs. Different
defect sizes far beyond the size considered as critical showed extensive new
bone formation in CT and in volumetric assessment (see page 65). At the same
time the limitations of an animal model and the consequences were addressed in
this study. The knowledge gained here can be used to prevent experimental
projects, as these data shows once again that a well-established animal models
need to be examined further because of insufficient physiological description.
Additionally, a pool of data acquired from all studies in domestic pigs and
minipigs enables further evaluation with new questions and without further
scans or measurements. For example, one given publication did not require any

▼ Chronic neck pain in the elderly is frequent, costly and lowers the quality of
life. This work evaluates the effects of qigong and neck exercises compared
with no treatment on the quality of life in elderly patients with chronic neck
pain. 121 elderly patients (>55 years) with chronic neck pain (>6 months) were
randomly assigned to one of the three groups (qigong, neck exercise or waiting
list). Qigong and neck exercises were provided by professional therapists (24
sessions over 3 months). Patients filled out standardized questionnaires at
baseline, after 3 and 6 months. The most important secondary outcome parameter
was quality of life, measured by the SF-36 after 3 and 6 months. Additional
parameters were depression (measured by the ADS), questions on quality of
sleep and digestion, global satisfaction with treatment, and practicing
habits. 117 patients were included in the ITT-analysis. The mean age of the
study population was 76 ± 8 years, the mean duration of neck pain was 19 ± 5
years and the mean neck pain intensity was 51 ± 20 mm on a visual analogue
scale (VAS 1-100). Most patients were female (95%). After 3 months, there was
no significant difference for SF-36 scale means between the qigong and the
waiting list group ((mean difference (95%CI) after 3 months = 1.6 mm (-1.7 to
4.9) P = .347 (physical component summary PCS); 1.9 (-3.3 to 7.1) P = .476
(mental component summary MCS)) respectively between the qigong and the neck
exercise group ((-0.5 (-3.6 to 2.5) P = .735 (PCS); 2.3 (-2.8 to 7.4) P = .372
(MCS)). Moreover, after 6 months there was no significant difference between
the qigong and the waiting list group ((mean difference (95%CI) after 6
months: 0.3 (-3.0 to 3.6) P = .855 (PCS); 2.4 (-2.7 to 7.5) P = .353 (MCS))
respectively between the qigong and the neck exercise group ((0.4 (-2.8 to
3.6) P = .789 (PCS); 1.1 (-4.0 to 6.2) P = .661 (MCS)). Also, we did not find
any significant changes in depression (ADS), sleep habits or digestion.
Contrary to what we might have expected from these results, most patients
continued practising qigong respectively neck exercises after the end of the
study. They also recommended their therapy and were pleased with both, therapy
and teacher. Qigong and neck exercises, compared with no treatment, have no
effects on quality of life (SF-36) in elderly patients with chronic neck pain.
However, patients were very satisfied with the therapies. It is still
subject
to further scrutiny, if the contrary results were due to non-adequate
questionnaires, old age, very chronic neck pain, short duration of
intervention or other influencing factors.

▼ As cause of the „sudden (cardiac-) death“ within younger dead (≤ 35 years), it
is more and more the hypertrophic cardiomypathy, an autosomal-dominant
transmitted group of disease, to be discussed as a reason of death.
Responsible are mutations of at least eleven different genes, which lead to
changes of the sarkomeres and therefore to cardiac dysfunction. The risk to
die of a sudden cardiac death of all patients with hypertrophic cardiomyopathy
is estimated at 1% per year [8, 13]. Most of the time these patients were not
diagnosed before their death since they presented no or only unspecific
symptoms. To explore the question, if cases of death with a hypertrophic heart
or unknown cause of death can be classified as hypertrophic cardiomyopathy,
the postmortem examination files of the three Berlin institutes of forensic
medicine dated January 1st 2000 to December 31st 2004 were looked and taken
into consideration at as well as the evaluation of “sudden cardiac death”
studies. Out of 11.535 obducted dead, 145 met the including criteria. The
causes of dead were: related to the coronary arteries (n=29), hypertrophy of
the heart/ hypertrophic cardiomyopathy (n=17), other cardiac causes (n=40),
unknown cause of death (n=36), convulsions (n=15), metabolic disorder (n=8). A
similar distribution was also found in the studies of the “sudden cardiac
death” [3,5,6]. Given a closer look to the weight of the heart within the
different groups, it is shown that the clearly heaviest heart was found in the
group hypertrophic cardiomyopathy with 541g. The group “unknow cause of death”
averaged the age but is below the average of body- and heartweight of the
examined dead. Further was the average age of the dead with coronary
heartdisease higher, in the case of the hypertrophy/ hypertrophic
cardiomyopathy lower than the average age of the total examined patients. 71%
of the hypertrophy/ hypertrophic cardiomyopathy patients died within an hour.
For the most cases within the group “unknown cause of death” it is not
certain, if they suffered of cardiac specific symptoms before they died. As
possible etiology of the unknown causes of death, diseases of the excitation-
and conducting system for instance the Long-QT-syndrome or the Brugada-syndrom
were discussed in studies, because of the common lack of a morphological
substrate [26]. From the medical point of view, it should be aimed to examine
corresponding cases with molecular diagnostic methods even if this correlates
to costlier and more time expanding examinations. The benefit, if one
succeeded to classify the unknown causes of cardiac death, would be the
identification of groups with higher risk and the possibility to therapy them
und prevent the sudden death.

▼ In this study the influence of Flt-3L on the immune response after DNA
vaccination, in particular the modification of the T cell-mediated immune
response was investigated. FLT3-ligand (Flt-3L) is a 1991 discovered cytokine
that stimulates in vivo proliferation and differentiation of hematopoietic
progenitor cells, particularly dendritic cells. Dendritic cells play a central
role in the induction and Th polarization of the immune response. The
application of recombinant Flt-3L in mice leads to an expansion of myeloid and
lymphoid dendritic cells. To investigate the (adjuvant) effect of Flt-3L on
the immune response after DNA vaccination C57/BL6-mice were immunized
intramuscularly with a plasmid coding for bacterial β-galactosidase.
β-Galactosidase served as a well-characterized model antigen for which MHC-
restricted immunogenic peptides are known. The mice received daily 10
micrograms of recombinant Flt-3L subcutaneously for a period of 12 days. On
the seventh day the mice were vaccinated intramuscularly with a
β-galactosidase coding plasmid. The treatment with Flt-3L resulted in a
significant increase of the antigen-specific proliferative immune response
after in vitro restimulation of the splenocytes. The analysis of the cytokine
profile secreted by restimulated splenocytes showed that the administration of
Flt-3L did not lead to a Th1 polarized immune response. Rather there was a
tendency towards a Th2 polarization of the immune response, corresponding to
the humoral immune response which was primarily characterized by IgG1
antibodies. A cytotoxic T cell response of restimulated splenocytes from Flt-
3L-treated mice against β-galactosidase peptide-pulsed target cells could not
be observed. To clarify the cause of the lack of Th1 polarization the
phenotype of the dendritic cells was characterized. It was found that the
dendritic cells showed an immature phenotype, which was associated with a lack
or low expression of the accessory molecules CD80, CD86 and CD40. The results
of this study show that Flt-3L as an adjuvant in DNA vaccination is indeed
able to enhance immune responses but does not lead by itself to a Th1
polarization. Therefore it can be assumed that the sole administration of Flt-
3L as an adjuvant in tumor vaccination is not a promising approach, but in
combination with additional dendritic cell maturation signals, Flt‑3L may play
a role in future oncological immunotherapy.

The CBF was assessed using the duplex ultrasound method by measuring the blood
flow in the external brain feeding vessels. Results were correlated to the
LVEF, CI, HMV, age, gender and MAP. Results: 1\. Directlymeasured CBF of our
patients was 472 ± 93 ml/min. After application of the CO2 correction the
values reduced significantly to 412 ± 105 ml/min. Similar results were
reported in the studies on patients with heart failure. Our hypothsis is that
combination of high age and heart failure of our patients led to the presented
results. 2\. Comparison of the g-CBF between patients with and without
arrhythmia revealed that CBF, but also LVEF and HMV were significantly lower
in the former. 3\. Consideration of the measured EtCO2-Values and other
factors such as blood pressure, BMI, heart rate, LVEF, HMV, CI yield to
significant correlation of the CBF to the LFEV, but not to HMV or CI. This
fact is most likely explained by higher number of patients in the study for
which LVEF data were availbale compared to lower number of patients with HMV
values known. After all our results indicate a relationship between heart
output and CBF in the sense of exhaustion of the cerebral auto regulation in
case of distinctive heart failure in combination with arrhythmia

factors of persisting diabetes after a pregnancy with gestational diabetes.
22% of the women were diagnosed with pathologic glucose tolerance within the
first year after a pregnancy with GDM, thus highlighting the necessity of
early postpartum screenings. The following independent risk factors for
pathologic postpartum oGTT were indentified: a maternal BMI > 30kg/m², a
gestational age < 24 weeks of pregnancy at diagnosis of GDM, insulin therapy,

macrosomy lose their predicative character when controlling for maternal
glucose values in the multiple regression analysis. Although 51 % of the women
in the study repeated the test within the first year, we found significant
differences in the return rates of the individual study sites depending on the
social and ethnic makeup of the population. While return rates were highest
for a private practice with an urban, middle class population with few
immigrants, only 23 % of women of an obstetric clinic in a socially
disadvantaged district in Berlin participated in the test. Unfortunately,
convincing women to take part in the postpartum tests tends to be particularly
difficult in those parts of the population that have a high prevalence of
postpartum glucose tolerance dysfunctions. Recommending postpartum glucose
testing to all women with GDM is an important measure of secondary prevention
because the test can provide valuable information regarding the individual
risk for developing diabetes later in life. Confining the test to fasting
blood glucose is insufficient; determining fasting blood glucose without oGTT
testing significantly reduces the explanatory power for a subsequent glucose
tolerance dysfunction. As a widespread introduction of postpartum oGTT testing
is in reality difficult, we developed an algorithm with the aim of ensuring
and improving compliance with and economic efficiency of postpartum glucose
testing as well as identifying the majority of women with a high risk for
postpartum glucose dysfunctions. Ideally, it should be possible to assess each
woman’s risk for diabetes already prior to nativity by using easily available
antenatal risk factors. Based on the prevalence of pathologic ppoGTT and
according to the number of existing risk factors, we defined three risk
categories. Women with a low risk showed a low prevalence of 11 % of
pathologic oGTT testing; only 1.2 % had diabetes. This group represented 60 %
of our population. This may vary in other populations, depending on, for
example, adiposity and existing type 2 diabetics. Where means or compliance of
patients is limited, ppoGTT testing of women with a low risk could be skipped
without missing a significant number of women with diabetes. However, one

▼ Cardial hypertrophy is one of the most important clinical predictors for
cardiovascular morbidity and mortality. Also age and gender are two well known
cardiovascular risc factors. Many studies have shown that calcineurin plays a
central role in the development of cardial hypertrophy. Activated calcineurin
consists of the catalytic subunit A (58-59 kDa) and the regulatory subunit B
(19 kDa). The genes for the catalytic subunit are found on the chromosomes 4,
10 and 8 and code for the three isoforms calcineurin A α, β
und γ
respectively. Through alternative splicing at least seven splice variants
originate (CnA α 1, 2 and 3, CnA β 1 and 2, CnA γ 1 and 2). Calcineurin is
activated by different agonists through an intracellular rise of calcium
together with calmodulin. Activated calcineurin then attaches to specific
transcription factors and together they translocate into the nucleus, where
they cause a change in gene expression which leads to cardial hypertrophy. The
aim of this study was to determine the gene and protein expression of
calcineurin A α and β and their splice variants and of the markers of
hypertrophy ANP and BNP in human cardial hypertrophy following aortic stenosis
and in cardiac failure due to ischemic and dilative cardiomyopathy. These
results were also checked for correlations between the groups and to clinical
parameters of the patients as well as differences in age and gender.
Leftventricular samples of myocardium were analyzed from 14 patients with
aortic stenosis, from 27 patients with cardiac failure due to dilative
cardiomyopathy and from 7 patients with heart failure due to ischemic
cardiomyopathy. As controls 15 potential donor hearts were used, which were
rejected for technical reasons. After rna-isolation and reverse transcription
the semiquantitative real-time rt-pcr was performed on the TaqMan® PE 5700. As
external standard gapdh was used. After protein isolation protein measurement
was performed using western blot. The m-RNA of two splice variants of
calcineurin A α was significantly upregulated in the AS (166 % vs. controls,
p<0,05), while the measurement of all three splice variants of calcineurin A α
showed a significant upregulation in the DCMP (189 % vs. controls, p<0,05).
The calcineurin A β results showed a stronger upregulation of m-RNA in the AS
(176 % and 174 % vs. controls respectively, p< 0,01 and p<0,05 respectively)
than in the DCMP (148 % and 150 % vs. controls respectively, p<0,01 and p<0,05
respectively). One splice variant showed a gender specific regulation in the
AS with a stronger upregulation in the male patients, while another splice
variant an age specific regulation showed in the DCMP with a stronger
upregulation in the younger patients (< 40 ys.). The m-RNA expression of ANP
was stronger upregulated than BNP in all patients, especially in the AS. The
results of the protein expression of calcineurin A (not isoform specific)

▼ The Osteoradionecrosis (ORN) of the jaws is one of the most serious
complications after radiotherapy of the head and neck area. In this study, the
data from 150 patients were analyzed in the framework of the treatment of
squamous cell carcinoma of the head and neck area, a primary or adjuvant
Radio(chemo)therapie received . After follow-up for 6 Years, the incidence of
ORN was 24 % (n = 36), over 85 % of ORN developed in the first 2 years, over
98% were in the mandible. The ORN was more common in men, Co-morbidities such
as diabetes mellitus and metabolic bone disorders increased the incidence of
ORN. Alcohol consumption after radiotherapy increased the risk of ORN, the
postoperative smoking behavior played no role in this cohort. The smaller the
distance of the tumor to the jaw was, the higher the ORN risk was, the highest
incidence was in tumors of the mouth´s floor and the mandibular alveolar
ridge. Analysis of dose-volume histograms showed that, the highest absolute
exposure in the mandible was, the total radiation dose in the damaged areas
was on average 66.4 Gy, doses more than 64Gy were associated with a rising
risk. Similarly, the fractionation of the irradiation was related to the
emergence of ORN, normal fractionation was associated with a higher incidence
compared to hyperfractionated accelerated radiotherapy. A previous surgical
intervention carried a clear risk for the development of ORN, particularly
when a marginal mandibular resection is performed. Also, the removal of the
cervical lymph nodes increased the risk of ORN- significantly. The highest
incidence of ORN could therefore be determined in the implementation of
adjuvant radiotherapy compared to the radiation of recurrence tumor and of the
primary RT / RCT. An important measure for the prevention of ORN is the dental
therapy before the start of radiotherapy, the highest incidence of ORN is
found in patients without prior dental therapy. This can be explained by the
fact that, the ORN incidence increased significantly, when the oral surgical
procedures are performed after radiotherapy. We found that flows in the risk
of developing ORN, the professional expertise, so that surgical procedures
after irradiation should be carried out by experienced dentists or
specialists. Regarding the shape of the prosthetic dentures after
radiotherapy, it was shown significantly that the mucosa-supported dentures
was more often associated with the incidence of ORN compared with implant-
supported dentures, the pressure sores under the dentures increased the risk
of ORN.

▼ The Proximal Tibia Less Invasive Stabilization System is an angle stable plate
system for the treatment of proximal tibia fractures which acts like an
internal fixator according to the principles of minimally invasive surgery.
From July 1998 to March 2002 45 fractures at 44 patients were treated in the
Department of Traumatology at the Charité Berlin with the new Proximal Tibia
LISS and the prospective course of healing was documented. The clinical data
were recorded at follow-up examinations 3, 6 and 12 month post-operatively. At
these times, the progress of healing was also documented radiographically. The
inclusion criteria were defined as proximal tibia shaft fractures and
intraarticular proximal tibia fractures of all degrees of severity. A total of
27 proximal tibia fractures and 18 metaphyseal fractures were treated, 18 of
these presented with open soft tissue damage. 37 of the 44 patients were
followed-up (84%) completely. The average age of the patients was 43 years.
The patient sample included 33 men and 11 women (3:1). Fracture healing was
completed at 36 patients in time. Full weight bearing was possible after an
average of 17,8 weeks (9-52 weeks). Complications were observed at 18
patients, in 5 cases reoperation were necessary. The one case of implant
loosening on the distal shaft was stabilized again using bicortical screws.
Knee prothesis were implanted in two patients with posttraumatic
osteoarthrosis. In one patient, the fracture had only been partially bridged
and secondary bone grafting teas performed. One soft tissue infection healed
uneventfully after a revision operation with the implant in situ. There were 2
varus and 3 valgus malalignments. The average Lysholm score was 82. The
average Tegner score decreased 1 point pre- to postoperatively. In summary the
Less Invasive Stabilization System is a good ostheosynthesis procedure for the
treatment of proximal tibia fractures and ensures a stable osteosynthesis to
fracture healing.
