▼ Background: Hepatitis B Virus (HBV) infection and its sequelae (cirrhosis and liver cancer) are major global health problems. Overall
prevalence in Ethiopia varies from 4.7-16.8% for HBsAg and 70-76.4% for at least one marker positive. During recent years evidence has accumulated that in patients with diabetes mellitus, phagocytosis by polymorpho nuclear leukocytes is impaired and disturbances in cell-mediated immune responses can be demonstrated. Cell-mediated immunity is involved in the defense against viruses as well as in that against Mycobacterium and Fungi. However, no increased incidence of viral infections like HBV has yet been reported in patients with diabetes.
Objective: To determine the
prevalence of HBsAg in patients with diabetes mellitus and to compare those with the non diabetes. It also asses associated factors and liver function tests (LFT); and compare Diabetic and non Diabetic subjects; and HBV positive and negative participants.
Methods: The study was a comparative cross sectional study design conducted at Woldiya General Hospital using 108 consented study populations from each study group during the period November, 2010 through January, 2011. A convenience sampling method was used. A total of 216 samples were tested for HBsAg serostatus using VISITECT HBsAg rapid test kit and LFT tests using Humastat 80 chemistry analyzer. Data entered to SPSS-16 and then analyzed using the same soft ware. Multivariate logistic regression was used to see the
association of HBV with clinical history of participants and sociodemographic variables. All tests were two-sided with α-level of 0.05 and 80% power.
Results:
prevalence of HBsAg was equal between diabetes and non diabetes, 3.7% indicating that there was no difference between the two groups. No any Sociodemographic and clinical history of participants were associated with HBV infection (p>0.05) except chronic liver disease.
Conclusion: In this study a positive relation was not indicated between HBV and Diabetes and DM in the study area did not predisposed to HBV infection than the rest of the population.

Background: Factors that influence physical restraint use need to be identified to reduce this questionable practice. However, due to limitations of previous studies, it is unknown whether an
association exists between the
prevalence and attitude regarding restraint use.
Aim and research questions: To explore the
association between the
prevalence of physical restraint use and attitude of physicians and nurses towards restraint use in a Dutch acute hospital. The research questions were: what is the
prevalence of restraint use, what is the attitude of physicians and nurses towards restraint use, and is there an
association between the
prevalence and attitude regarding restraint use?
Method: A cross-sectional, correlational study was conducted. Unannounced, one researcher observed patients on 17 units on restraint use to determine the
prevalence. Then, physicians and nurses working in these units were asked to complete the Maastricht Attitude Questionnaire (MAQ) to determine their attitude towards restraint use. Subsequently, the Spearman’s rank correlation coefficient was used to explore the
association between the
prevalence rates and mean MAQ scores at the unit level. Units with a response lower than 40% and/or a
prevalence based on less than five patients were excluded from this analysis to increase the usability of the results.
Results: A moderate to strong positive significant
association (rs=0∙608, p=0∙027) was found, indicating that units with a positive attitude tended to use more restraint than units with a negative attitude. Physicians and nurses had a relatively neutral to slightly positive attitude regarding restraint use, but they considered the use of restraint as appropriate clinical practice. Physicians and nurses considered the mostly used restraint measure (bilateral bedrails) as moderately restrictive.
Conclusion: Attitude may be an influencing factor of physical restraint use.
Recommendations: Information programs for units with positive attitudes and high
prevalence rates should be aimed at changing attitudes against restraint use in order to reduce the use of restraint.

▼ Salmonella in pork is a concern. This study characterized pig antibody responses on-farm and their
association with Salmonella shedding/tissue colonization and host genetic variants. Fourteen cohorts of pigs from eight farms (total = 809) were tested five times (weaning to slaughter). Blood and feces/tissues were analyzed via ELISA and culturing. Data were analyzed using mixed-effects logistic regression models. Overall, 20.7% of sera tested positive for Salmonella antibodies and 12.6% of fecal samples tested positive for Salmonella. Older pigs and pigs shedding Salmonella were more likely to test seropositive (p < 0.05). There was no
association between seropositivity and Salmonella at slaughter. Variants in/near PTPRJ (p = 0.0000066), ST6GALNAC3 (p = 0.0000099), and DCDC2C (p < 0.0000086) were associated with Salmonella susceptibility, while variants in AKAP12 (p < 0.0000358) and RALGAPA2 (p = 0.0000760) may be associated. This work may aid research in reducing Salmonella on-farm, benefiting researchers and the public.

▼ Sleep-related problems are rapidly growing public-health concerns that often result in adverse short-term consequences (e.g., reduced quality of life) and long-term health effects (e.g., cardiovascular disease). However, the relation between sleep problems and chronic kidney disease (CKD) has been under-investigated. To better understand this relation, we addressed three aims.
The first aim was to examine temporal trends in the
prevalence of 5 self-reported sleep problems in U.S. adults and their associations with CKD and all-cause mortality, using data from 5 National Health and Nutrition Examination Surveys (2005-14). The
prevalence of trouble sleeping and diagnosed sleep disorder increased over the decade, while nocturia (urinating ≥2 times/night), inadequate sleep (<7 hours/night), and excessive sleep (>9 hours/night) remained stable. All sleep problems, except inadequate sleep, were more common among adults with CKD than without CKD, especially for excessive sleep and nocturia, which were positively associated with all-cause mortality.
Second, we conducted a large retrospective cross-sectional study of obstructive sleep apnea (OSA) in U.S. veterans who sought care in Veteran Administration (VA) facilities in fiscal year (FY) 2018 to better understand the population burden of OSA and its relation with CKD and its risk factors. Using data from 6.2 million veterans for FY2014-18, we estimated OSA point
prevalence at the last visit to a VHA facility in FY2018 (index time T) and period
prevalence of
OSA (excluding prevalent cases at T) going back to the start of FY2014. Period
prevalence for each duration was estimated by taking into account left censoring of veterans followed back for different durations. At time T, OSA point
prevalence was 24.9% in veterans with CKD and 15.2% in those without CKD. The overall 60-month period
prevalence was 11.6% and was positively associated with CKD, obesity, being male, having hypertension, or diabetes, and inversely associated with age ≥65.
Lastly, we conducted a large retrospective cohort study of U.S. veterans to test the hypothesis that CKD is a mediator in the causal pathway linking race/ethnicity with OSA incidence. Four statistical methods of mediation analysis with different advantages and limitations were used: informal difference method, 4-way decomposition, flexible mediation analysis, and dynamic path analysis. Blacks and Hispanics had higher incidence rates than did non-Hispanic Whites. The percentages of the total race/ethnicity effects mediated by CKD were small and similar using all 4 mediation methods; e.g., using flexible mediation analysis, the percentage of the Black/White effect on OSA incidence mediated by CKD was 5.8%. However, when CKD and its 3 risk factors were treated jointly as mediators in flexible mediation analysis, the percentage mediated increased to 30.3%.

Data presentation methods;-tables, charts and graphs and narratives are used.
Conclusion: - There is bullying in public District Schools in Kikuyu Division of Kiambu
District. Bullying also leads to psychological Distress in the schools studied.
Application of results: - Will be used in formulating appropriate policies to deal with bullying from the school level to the national level

▼ This dissertation consists of three different projects concerning three different aspects of measuring social welfare and poverty, when the measurement is based on more than one attribute or dimension of well-being. The first project develops a class of multidimensional social welfare indices that is sensitive to two distinct forms of inequality across the population. One form of inequality is concerned with the dispersion of the distributions of achievements and the other is concerned with the
association across the attributes. This project also applies these newly developed indices to the Indian context analyzing how the consideration for the sensitivity to inequality may alter the relative performance of the Indian states. The second project is concerned with the composite indices that evaluate the overall performance of societies by taking a weighted average of their performance in different attributes and rank the societies accordingly. The selection of weights is crucial because a choice of different weights other than the one initially used often alters the ranking and thus resulting in ambiguous or non-robust comparisons. This project develops a technique that can gauge the extent of robustness of each pair-wise comparison using a 0-100 percent scale. It is further shown that the
prevalence of robustness is affected by the
association across the attributes. The third project is concerned with the evaluation of multidimensional poverty in the context of India when the attributes are mostly dichotomous or categorical. The project uses the poverty index proposed by Alkire and Foster (2008), which is appropriate for this purpose, and shows how the multidimensional evaluations are different from the purely income based evaluations.
