▼ Hepatitis B virus (HBV) infects over 300 million people worldwide and despite the availability of a vaccine, it remains the second biggest carcinogen in the world and a serious global health problem. The majority of adult infections are resolved while 90% of neonates fail to clear infection. Persistently-infected individuals harbour a risk of developing liver disease and hepatocellular carcinoma (HCC). Treatment of HBV is met with several problems including poor tolerance and antiviral resistance and therefore, novel well-tolerated and effective therapies are required.
Innate lymphocytes include NK, NT, NKT and T cells and comprise a group of MHC-unrestricted lymphocytes that can elicit potent cytokine production and cytotoxicity at a very early stage of virus infection. We proposed that a better understanding of the role of innate lymphocytes in the control of HBV infection might eventually facilitate the development of new cell-based immunotherapies.
We have quantified the frequencies and cytokine profiles of natural killer (NK), natural T (NT), natural killer T (NKT) and T cells in a group of HBV patients with relatively low viral load and little evidence of liver disease in order to ascertain the role of these cells in controlled HBV infection. We found expansions in the frequencies of circulating NK and NT cells and the V2 and V1 subsets of T cells in these patients, compared to uninfected control subjects. Furthermore, expansions of interferon- (IFN-)-producing NT, V2 T and conventional T cells were significantly higher in this group as were the frequencies of total IFN--producing lymphocytes. Higher levels of IFN- expression in HBV were confirmed using qRT-PCR and we, therefore, propose that such expansions are indicative of an active antiviral immune response and that IFN- plays an important role in the control of HBV replication. Higher frequencies of IL-10-producing NK cells observed in the HBV patients may indicate a possible regulatory role of these cells in asymptomatic HBV infection. IL-17-producing NT and T cells exhibit enhanced responses to in vitro stimulation in HBV suggesting that IL-17 plays a supporting role in the control of HBV infection and that such Th17-biased cells can expand rapidly following sufficient stimulation. Since IL-10 and IL-17 can elicit inhibitory effects on IFN- production, we propose that the increased potential of IL-10 and IL-17 production in these patients might represent a regulatory mechanism that limits the antiviral immune response to prevent liver injury. We have also found reduced expression of the cytotoxicity-associated receptor NKG2D by V2 T cells
which may be another mechanism by which immune-mediated damage is limited in these HBV patients. Furthermore, we have found that HBV proteins have negative effects on IFN--production by V2 T cells suggesting that HBV can inhibit the effector mechanisms of these cells.

▼ Toll like receptor (TLR) signalling is central in controlling innate immune responses, with dysregulation of TLR pathways associated in both autoimmune and inflammatory diseases. Numerous reports suggest that cannabinoids, known to exhibit general immunosuppressive properties, have potential therapeutic value in the treatment of inflammatory conditions. In relation to multiple sclerosis (M.S.), cannabinoids palliate M.S. patient symptoms and the severity of clinical signs associated with experimental autoimmune encephalomyelitis (EAE), an animal model of M.S. However, the precise molecular mechanism for these effects is not understood. While the synthetic cannabinoid R(+)WIN55,212-2 exerts established anti-inflammatory effects in vitro and in vivo, the role of cannabinoid compounds in modulating TLR signalling events is unknown. Here, using a variety of cell model systems, evidence is provided that R(+)WIN55,212-2, in the presence of the TLR3 ligand, Poly(I:C), targets interferon regulatory factor 3 (IRF3), a transcription factor essential for regulating type I interferon (IFN) expression. These findings also demonstrate that protective effects of R(+)WIN55,212-2 in EAE mice exist, as evidenced by reduced clinical scores, demyelination and inflammation. Furthermore, the anti-inflammatory effects of R(+)WIN55,212-2 in this model are IFN- dependent. In addition to blunting pro-inflammatory signaling induced by Poly(I:C) in PBMCs from both healthy donors and M.S. patients, R(+)WIN55,212-2 robustly enhanced IFN- production in these cells, an event restricted to M.S. patients.
This study also highlights the anti-inflammatory potential of R(+)WIN55,212-2 by virtue of its inhibitory effects on the NFB pathway. R(+)WIN55,212-2 can inhibit TLR3/4-induced activation of NFB. This likely makes a major contribution to the inhibitory effects of R(+)WIN55,212-2 on pro-inflammatory gene expression. Indeed, it is also demonstrated that R(+)WIN55,212-2 blunts TLR3/4 induction of TNF-. The regulatory effects of R(+)WIN55,212-2 on TLR3/TLR4-induced activation of IRF3, IFN- and NFκB are independent of the cannabinoid receptors and evidenced is presented suggesting a potential role for peroxisome proliferator activated receptors in mediating the regulatory effects R(+)WIN55,212-2.
Overall these findings highlight that R(+)WIN55,212-2, by targeting IRF3 and IFN- downstream of TLR3 stimulation, may exert anti-inflammatory properties and provide evidence that cannabinoid administration may offer therapeutic potential in the treatment of M.S.

▼ In many ways, the design of domestic cooking and heating products reflects the
zeitgeist of Irish culture throughout the 20th and into the 21st century. From
domesticity to materialism, these products have evolved to meet fundamental human
needs within the home. Concurrent with this, the methods and processes designers
use to create domestic artefacts have evolved and changed. The emergence of Design
Ethnography illustrates an evolution where Design has appropriated an established
method of Anthropology for its own particular objectives. However, the integrity of
the ethnography practised by designers has been criticised by many, e.g. Dourish
questions whether it creates forms of “discount ethnography” (2006, p.548). The
designer ethnographer has different objectives to the anthropologist and the
particular principles, methods, and understanding of design ethnography have not
been fully elucidated for use by professional designers. Bichard and Gheerawo
observe “…if anthropologists and ethnographers appear to be becoming designers as
such, then perhaps designers should allow themselves to reflect on their ‘field’ and
‘work’ more as anthropologists and ethnographers”(2011, p.55). The identification
and construction of a design ethnography, epistemology, and methodology that is
distinct from, while respectful of that of anthropology, is required.
Design ethnography has been heralded for its ability to investigate future complex
issues for humanity, and to produce powerful, democratising, and radical effects
(Plowman, 2003). Meeting the demands of a growing older population will be one of
these future complexities. Ireland is rapidly becoming an ageing society with its
population living longer in ill health (McGill, 2010). 89% of older Irish people
would prefer to live at home rather than in institutional care (McGee et al., 2005).
Therefore developing healthy environments in which they grow old must be a
priority. Particular attention must be given to the design of domestic products that
provide older people with basic daily requirements. Domestic cooking and heating
products offer basic health and wellbeing needs in the form of nutrition and heat.
Prior to designing these products a deep understanding of older people’s needs must
be determined. To achieve this, empathy and sensitivity are essential (Newell et al.,
2010). Comprehensive field studies such as ethnographies are important in gaining
understanding and eliciting true user insights (Seidel, 2009, Newell et al., 2010)
This thesis contributes to two domains. Firstly, it identifies and develops an
understanding of the essence of design ethnography, and a process by which
designers can harness ethnographic methods for the purposes of design practice.
Secondly, it provides an example of a designer ethnographic approach to product
design for older people, producing insights and product design requirements for
cooking and heating products. The research involved design ethnographic fieldwork

▼ This research investigated the role of mathematics in engineering practice and whether there is a relationship between students’ experiences with school mathematics and their choice of engineering as a career. The study was inspired by the observation that there is a lacuna in the scholarly literature concerning the nature of mathematics’ role, if any, as a significant cause of the declining number of students entering professional engineering courses. Additionally there is currently no broad picture of the mathematical expertise required or used by practising engineers.
The population of interest in this study comprises professional engineers practising in Ireland. A sequential explanatory mixed methods design, where the subsequent collection and analysis of interview data builds on the survey findings, is employed. Engineers’ use of mathematics is considered in three parts: curriculum mathematics, mathematical thinking, and engaging with mathematics. Curriculum mathematics usage is measured by a derivation of de Lange’s mathematics assessment pyramid and with reference to three dimensions: mathematics domain, usage type, and academic level. Thinking usage relates to mathematical modes of thinking. Engaging usage is the motivation to take a mathematical approach. Engineers’ experiences of school mathematics, factors that contributed to their engagement with school mathematics and the impact of their feelings about mathematics on their choice of engineering careers are investigated.
The findings show that (i) engineers’ feelings about mathematics are a major influence on their choice of engineering as a career; (ii) teachers, affective factors and sociocultural influences are the main contributors to engineers’ interest in and learning of mathematics; (iii) while almost two thirds of engineers use high level curriculum mathematics in engineering practice, mathematical thinking has a greater relevance to engineers’ work compared to curriculum mathematics; (iv) professional engineers’ curriculum mathematics usage is dependent on the interaction of engineering discipline and role and their mathematical thinking usage is independent of discipline and role; (v) engineers show high affective engagement with
mathematics and their usage of mathematics in engineering practice is influenced by the value given to mathematics within their organisation; and (vi) the focus on “objective” solutions at the expense of tacit knowledge in mathematics education reduces the value of mathematics in engineering practice.

▼ This thesis generates new insights and understandings of the concept of reflexive management learning (RML). To date, most scholarship has taken the form of prescriptive theory suggesting what RML should be and should mean for managers and organisations. In the main, however, RML remains empirically under-theorised and as a result rather detached from real world contexts and from the practitioners that would constitute its intended learning audience.
This thesis helps to rebalance the scholarly scales by presenting the results of a two-year reflexive insider’s ethnographic study of RML in the Irish subsidiary of a services industry MNC. This heretofore unavailable methodological vantage point provides a novel perspective on RML which elucidates its highly contextualised character. The concern here has been to make managerial voice in RML clearer and more pronounced. Doing this highlights the need for more political reflexivity in RML theorising: defined as the adequate recognition of the complex role of context in RML and the accompanying need to direct this learning approach towards practice-based concerns which matter to the managers involved, without allowing its inherently critical character to be unduly censored by contact with the organisational status quo.
The key findings of this thesis report on how challenging RML is as told through the experiences of the managers involved. But these experiences also portray something which has been less prominent in previous empirical work: RML’s promise-laden potential and despite the difficulties and risks involved, paradoxically even because of them, its ability to translate into reflexive action attempted beyond the learning space. These findings led to, and are articulated in this thesis through, a new theoretical framework for RML which also draws from the work of political theorist Hannah Arendt. Titled the ‘Reflexive Space of Appearance’, this new spatial and political theoretical framework provides RML with a much needed theory of action, or praxis, which can make new contributions to scholarship and practice.

▼ This thesis describes and analyses the PhD candidates in the Humanities at the Friedrich-Wilhelms-Universität zu Berlin and at Columbia University, New York, from 1871 to 1913 as well as the reforms related to the PhD programs at said institutions. The thesis uses primary sources such as the theses and curriculum vitaes of the PhD candidates but also the reports of the universities, statistics released, census records of government institutions as well as newspapers and biographical collections. The goal is to compare the PhD candidates at these two universities according to their numbers, age, gender, religion, place of birth and social background. It further includes a comparison of the reforms and transformation of the two universities with a focus on those which most affected PhD candidates. Instead of focusing on the careers of PhD candidates after they acquired their degree (as in most other studies), this thesis focuses on the background and the life of PhD candidates before they received their degree from their university. By doing so, this thesis will contribute to the understanding of the development of the universities and societies in the late 19th and early 20th centuries, taking into account the debates regarding the German Sonderweg, the professionalisation of education and cross-border exchange among academics wherever possible.

▼ Research undertaken within the Sociology of Education frequently highlights concerns about the underachievement of Black students in education and, later, within the labour market. Yet, there are a number of shortcomings associated with research in this area. Firstly, there is a tendency to homogenise the achievement levels of all Black students. Thus observations made about the outcomes of African-Caribbean students are often applied to all other Black groups. When distinctions between African and African Caribbean groups have been made, the achievement levels of students from different African backgrounds are often merged, creating a misleading impression of their different academic outcomes. Secondly, studies seeking to provide explanations for the low attainment levels of Black students are often critical of life within Black families, in particular their assumed use of an „authoritarian‟ parenting style, which is seen as creating psychological problems in children and as hindering their achievement. Effectively, such notions serve to pathologise Black families in Britain. This thesis presents a critique of existing studies concerning Black families in Britain and the academic achievement of Black (African) children, and also seeks to address existing gaps in the knowledge about Black Africans residing in Britain. Life history interviews were conducted with 25 British-Ghanaians who have achieved highly in their academic and professional pursuits. The findings suggest that not all parents adopted an „authoritarian‟ approach when raising their children, and that those who did were influenced by their own socialisation experiences in Ghana. While some respondents experienced some socio-emotional problems resulting from their „authoritarian‟ socialisation, these were generally resolved and did not have a long-term impact on their attainment. The thesis also suggests that the use of discipline, associated with this parenting style, may have had some beneficial effects in relation to respondents‟ academic and professional outcomes.

▼ The randomised controlled education intervention study recruited 430 adults aged 25-65 years with established diagnosis of T2DM (M = 130, F = 309) via a multi-stage double-blind stratified sampling procedure from 22 hospitals and health centres in Doha, Qatar. The intervention group (n = 215) were assigned to a six-week, 2-hourly structured educational class based on the specifically designed DPET, plus their usual one-to-one routine clinical care; the control group (n = 215) used the DPET for home self-study, plus their usual one-to-one routine clinical care.
Adherence to the programme in the intervention group (n = 109; M = 40, F = 69) was 50.7% compared to 84% among controls (n = 181; M = 50, F = 131); an overall non-adherence rate of 32.6%. Repeated measures ANOVA showed a highly significant change in each of diabetes knowledge, attitudes and practice among intervention compared to controls at 12 month follow-up (p<0.0001).
The intervention had no significant overall impact on systolic blood pressure (p = 0.632) nor diastolic BP (p = 0.421) but improvements in BMI among the intervention group (p = 0.001). Repeated measures ANOVA also showed differences in overall change in HbA1c (p = 0.012), fasting blood glucose (p = 0.022), HDL-cholesterol (p<0.0001) and albumin-creatinine ratio (p<0.0001) in the intervention group but not total cholesterol (p=0.204), LDL-cholesterol (p = 0.203) and total triaclyglycerol (p = 0.200) from baseline values after 12 months follow-up. Post-sessional self-assessment tests of knowledge, attitudes, practices and goal-setting used as a proxy measurement of empowerment showed a significant improvement in the empowerment scores from baseline in the intervention group (p<0.0001).

▼ Widespread physical inactivity and resultant increases in cardiovascular and metabolic disease is a serious public health concern in the developed World. Unsurprisingly, the vast majority of research evidence suggests that physical activity is an effective intervention in addressing this state of affairs. An apparently strong case exists for the widespread clinical prescription of physical activity (PA). The application of PA in both preventative and remedial health is often termed ‘exercise is medicine’.
Whilst on the basis of a large volume of laboratory data there is some consensus regarding the optimal delivery of health related PA, there is an apparent discrepancy between data emanating from laboratory and/or clinical studies and those emanating from real world interventions. In short, real world interventions do not appear to be as effective in promoting health as laboratory research suggests should be the case. This situation is compounded by a relative paucity of peer reviewed research studies reporting real world PA research, and furthermore by even less clinically relevant data. On this basis, a clear picture of the degree of translation from laboratory to the field is not yet possible. It is however not unreasonable to argue that the setting of the vast majority of research studies investigating the exercise is medicine hypothesis - that is laboratories, hospitals and clinics – might theoretically limit the translation of these findings to real world public health settings, and on that basis, more real world research is warranted.
In Chapters 1 and 2 of this thesis the above arguments are developed into a case for a large scale ecologically valid translational study to investigate the effects of exercise on clinically relevant health variables. Chapter 3 presents the results of a pilot study that assessed the comparative effectiveness of structured PA (STRUC), unstructured PA (FREE), and PA counselling (PAC), among sedentary individuals in a community fitness centre setting. Significant improvements were observed in cardiovascular risk factors in all three groups, with no significant between-group differences.
Chapters 4, 5 and 6 report data from a large scale, ecologically valid, longitudinal (48 week), multi-centre (n=26) investigation comparing the three interventions above with a measurement only condition. Participants were 1146 previously sedentary individuals. The ecological validity of the exercise is medicine hypothesis was tested from a clinical (Chapter 4) and behavioural (Chapter 5) perspective. Survey data pertaining to factors influencing the effectiveness of the interventions are explored in Chapter 6.
Data suggest that the baseline health status of participants mediated effects over time, with participants most at risk of cardiovascular disease experiencing clinically significant improvements in health (e.g. VO2max: STRUC High -7.52% vs Low 32.03% (P=0.005), FREE High -4% vs Low 24.31% (P=0.023), PAC High -8.19 vs Low 35.8% (P=0.007), COM High -5.22% vs Low 8.17% (P=0.663).…

▼ In April 2002, five years after the Blair government’s proposals to create a ‘New NHS
[National Health Service]’, the government outlined the key priorities that would mark the
NHS reform. The main reforms involved patient choice supported by a system of ‘Payment
by Results’ (PbR) under which hospitals would be funded on the activity they undertook.
PbR is a case based payment system, a type of system increasingly being adopted as
the main form of provider payment across industrialised countries. The literature on this
type of payment system and experiences from other countries identifies many di!erent
behavioural incentives that can have both positive and negative impacts on quality of
care. This thesis investigates the quality implications observed so far in England, for
seven conditions which represent a spectrum of important clinical areas that are admitted
through both emergency and elective admissions.
In order to identify changes in quality, this thesis first considers how to construct an appropriate measure of quality. The first part of the thesis utilizes two di!erent methodological techniques used for quality measurement; a latent variable approach and a technique
put forward by McClellan and Staiger (1999) using Vector Autoregressions. The results
from these techniques indicate that quality measurement approaches di!er markedly with
regards to how much measurement and systematic error they are able to filter out of raw
outcome data. Finally, the new indicators created by these techniques are used to evaluate the quality impact the introduction of PbR as the main form of hospital payment has
had in England. The analysis indicates that since the policy’s implementation, there have
been di!erential quality e!ects on the di!erent conditions. However, for the most part this
indicates an improvement in mortality outcomes, and a reduction in the variation of outcomes across hospitals. As found, the interpretation of readmissions has to be approached
with caution as more severe patients being kept alive through quality improving measures
on mortality create more mixed signals for the readmission indicators. In two conditions
we find changes in activity that are indicative of e"ciency gains, in the form of better
coding and adoption of new technology, both as a result of differences in reimbursement
categories.

▼ This thesis explores conceptualisations of ethical subjectivity in the stories of people
who have, or have had, cancer and who use complementary therapies and self-help
(complementary self-health). In England the increasing emphasis on individual
responsibility and choice in healthcare means that those with cancer are now in the
position where they have to make many of their own decisions about their treatment.
For the people with cancer in this research this included choosing complementary
self-health.
The thesis explores the stories and experiences of people with cancer who used
complementary self-health. The aim of the thesis is to document and make visible the
many original ways people come to understand themselves as able to ‘heal’ their
health, self and life. The thesis also reflects on the highly unusual position of the
author, who was himself diagnosed with cancer mid-way through his fieldwork. The
problems and challenges to the research are explored in the thesis, which became a
story in itself.
The thesis finds the ‘price that is paid’ for the way that the interviewees came to
think of their selves in their subjectivity. The interviewees told open and ongoing
stories of ‘balanced living’ and how they found ways to listen to their ‘embodied
guides’. They also described how understanding ‘cancer as an opportunity for
change’ helped to transform and transcend their previous ways of living. In doing so
they provided themselves with new narratives of the past and future, which were
situated by ‘living in the now’. The thesis explores the ethico-political repercussions
of these new formations of subjectivity. In doing so it argues that the ‘spiritual’
component of the stories remains unaccounted for and under conceptualised in
contemporary sociological theory of health. It concludes with suggestions of how to
move contemporary analysis beyond its present ethical concerns with the subject.

▼ In Europe, Health Technology Assessment (HTA) bodies produce coverage decisions
that guide public funding of pharmaceuticals. This thesis examines and weights those
factors that drive HTA coverage decisions, focusing on the National Institute for Health
and Clinical Excellence (NICE) in England and Wales, the Scottish Medicines
Consortium (SMC), the Dutch College voor Zorgverzekeringen (CVZ), and the French
Haute Autorité de Sante (HAS). To address the research question, a dataset of
approximately 1000 HTA coverage decisions by NICE, SMC, CVZ and HAS from the
period 2004-2009 was created, containing more than 30 clinical, economic, process and
socio-economic factors extracted from published HTA reports. A three-category
outcome variable was used, defined as the decision to ‘recommend’, ‘restrict’ or ‘not
recommend’ a technology. Multivariate analyses were conducted to assess the relative
contribution of the explanatory variables on coverage decisions both within and
between HTA bodies.
Results demonstrate that different combinations of clinical/economic evidence, process
and socio-economic factors drive HTA coverage decisions by NICE, SMC, CVZ and
HAS. In addition, the same factor may behave differently according to the nature of the
coverage decision. The analysis further suggests there is a significant difference
between HTA bodies in the probability of reaching a ‘restrict’ or ‘not recommend’
decision outcome relative to a ‘recommend’ outcome, adjusted for evidence, process
and context factors. This thesis contributes to the understanding of factors driving
HTA coverage decisions by examining multiple European HTA bodies, enhancing the
comprehensiveness of the factors examined through descriptive and multivariate
analyses and by identifying and weighting the key drivers of the coverage decisions
made by the four HTA bodies between 2004 and 2009. This research further provides
relevant insights to variation among HTA bodies in the determination of patient access
to pharmaceuticals, and implications for collaboration between European HTA bodies.

▼ Schizophrenia is a chronic illness which has severe consequences for the lives of patients and their families. The costs associated with treating individuals with schizophrenia are considerable. This thesis examined the relationship between non-adherence to medication, patient-, environmental- and medication-related factors and the costs associated with health and social care services used and the wider societal costs in treating individuals with schizophrenia. Analysis was undertaken of data from the 1993-4 and 2000 Psychiatric Morbidity Surveys and the Quality of Life following Adherence Therapy for People Disabled by Schizophrenia and their Carers study.
An individual’s level of education, having had a recent inpatient stay and alcohol abuse were found to be associated with a greater likelihood of non-adherence in individuals taking antipsychotics. These results were not observed in analyses of individuals taking antidepressants. Common factors associated with non-adherence across individuals taking antipsychotics and antidepressants included experiencing side-effects and severity of illness.
Community-based services were found to be used more by individuals with interruptions in their antipsychotic medication. In this group there may also be additional costs in hospitalisations and overall health and social care services attributable to non-adherence. Benefits to patients may be accrued by enabling health and social care professionals, particularly those working in the community, to encourage medication adherence in individuals with schizophrenia and to provide information on new interventions that are cost-effective in improving adherence.
National Institute of Clinical Excellence (NICE) guidelines for treating individuals with schizophrenia, revised in 2009, address some key findings in my analyses, such as emphasising the role of carers and family members in successful management of the illness, the potentially adverse impact that illicit drug use can have on therapeutic effects and issues around service provision to individuals from ethnic minorities.
Further analysis of data from long-term studies is required to determine the clinical, economic and personal consequences of non-adherence.

Lumbar disc disease is of high importance with health and social impact leading to rapid decrease of the work capacity, impaired social adaptation, a lowering of one's quality of life and in some cases may be even the cause of the disabilities. The adequate therapeutic method is consistent with the stage if the illness and has a key role when it comes to successful recovery of the patients. Physical therapy has various tools via which it can act both symptomatically and pathogenically. One of them is traction used since Hippocrates’ time.
Extensive research of the efficiency of the complex appliance of pre-formal physical factors and traction on the functional recovery of patients suffering from lumbar disc disease has been conducted, based on scientific evidence, for the purpose of this research. Assessment of the treatment effect is performed by dynamic tracking of both subjective and objective neurological status, indicators for active volume of mobility in the lumbar region, questionnaire by Roland-Morris for assessment of disability and questionnaire for assessment of the psycho-functional condition of the patient – CAM (confidence, activity, mood): before and after treatment; at the end of the first and the third months. The analysis of the results proves that the developed complex physiotherapeutic program applied during the right stage of the disease has high treatment efficiency and contributes for the proper recovery of the patients suffering from lumbar disc disease.

Поясната дискова болест е забoляване с голяма медико-социална значимост, водещо до рязко снижаване на работоспособността, нарушаване на социалната адаптация, намаляване качеството на живот, а в редица от случаите е причина и за инвалидизация. Адекватния терапевтичен подход съобразен със стадия на заболяването е от решаващо значение за ранно и пълноценно възстановяване на пациентите. Физикалната терапия разполага с разнообразни средства, посредством които има възможност да въздейства както симптоматично, така и патогенетично. Едно от тях е екстензионната терапия прилагана още от времето на Хипократ. В дисертационния труд въз основа на научни доказателства е проведено задълбочено проучване на ефективността на комплексното приложение на преформирани физикални фактори и екстензионна терапия върху функционалното възстановяване на пациенти с поясна дискова болест. Оценката на ефекта от проведеното лечение се извършва посредством динамично проследяване на субективния и обективен неврологичен статус, показатели за активен обем на движение в лумбален отдел, въпросник на Ролан-Морис за оценка на инвалидност и въпросник за оценка на психо-функционалното състояние на индивида „САН“ (самочувствие, активност, настроение) – преди, след лечение, в края на първия и третия месец. Importance and timeliness of interest to the subject of this thesis is pertinent with the increasing need for motion therapy of patients in the operative and postoperative recovery period of acute and chronic, single or multiple diseases. Bulgaria, as well as a number of EU member states, is facing a serious challenge posed by an aging population increasing the need for integrated care, hence a substantial public spending for these services required. Concurrently, ensuring high quality services by the existing specialized institutions turns out to be an extremely complex issue. Successfully overcoming the challenges requires comprehensive measures and actions aimed at improving the quality of services providing real access to them and creating new, integrated cross-sectoral services (with an emphasis on the mutual health and social care commitment), by focussing on the individual needs of the target groups.
Assessment of Bulgarian medium shows lack of uniformity in the formation of professional competence of the medical professionals practicing motion therapy. Universities training such personnel determine their competencies according to the relevant program: rehabilitation, physical therapy, ergotherapy, occupational therapy, massage, etc.
Deciphering the place and role of the physiotherapist in public health and the development of integrated care is essential in order to address the need of trained professionals. Kinesiotherapy is a regulated medical profession with recognized qualification within the European Union. Kinesiotherapy training programs should be accredited by the schools of Public Health and the higher education medical schools on account of the close patient-physiotherapist relationship and the implementation of the patient-centred approach. Well-timed and adequate rehabilitation of multidisciplinary task team held in a hospitals, outpatient clinic or home treatment will help reduce the risk of disability. The expected results are associated with reducing the financial costs of health and social systems in our country.
This study aims to explore the place and role of the physiotherapist in public health and in the development of integrated care.
To achieve the above objective we set ourselves the following tasks:
To scrutinize kinesiotherapy as a profession and education wise in Bulgaria and abroad.
To reveal opportunities for practical application of physiotherapy in Bulgaria.
To analyze the practical models for development of integrated care in Bulgaria and abroad.
To present and discuss the clinical experience of patients with arthrosis - users of integrated care at home.
To investigate the level of satisfaction of the two groups of patients: users of integrated care provided by a multidisciplinary team, and users of rehabilitation services funded by the NHIF.

Current development of psychiatry in conditions of deinstitutionalization of mental health services and heightened opportunities to control the symptoms of mental disorders, makes patients’ socialization increasingly important. Along with the "objective" quantitative and qualitative characteristics of social interaction, subjective life experiences (physical, psychological and social interactions) and welfare, defined by us as “quality of life”, are gaining critical importance as well. This is a relatively new perspective in the study of the mentally ill in social settings.
A contribution to better quality of life for these patients would be a full-bodied rehabilitation providing social adaptation and resocialization of persons with mental illnesses after treatment of an acute episode.
Key priority for complex rehabilitation is the potential of ensuring equal and adequate access to mental health care for people with mental disorders; ensuring continuity of care and social inclusion; ensuring their own and public safety; combating the stigma and discrimination of the mentally ill, ensuring the reduction of the economic and social burden of mental illness. The above objectives set forth in various strategic documents cannot be achieved without well-trained teams of specialists for psychosocial rehabilitation.

На съвременното ниво на развитие на психиатрията, в условията на деинституционализация на психично-здравните услуги и повишени възможности за контрол на симптомите на психичните разстройства, все по-актуален става проблема, свързан със социализацията на пациентите. В последните години, наред с «обективните» количествени и качествени характеристики на социалното функциониране, все по-голямо значение придобиват субективните преживявания от живота (физическо, психично и социално функциониране) и благополучия, получили определението „качество на живот“. Това е относително нов аспект от изучаването на психично болните в социален план.
Принос към по-доброто качество на живот на тези болни би била пълноценната рехабилитация, която да подпомогне социалната им адаптация и ресоциализация на лицата с психични заболявания след лечението на острия епизод.
Основни приоритети при комплексната рехабилитация са осигуряване на равен и адекватен достъп до психични здравни грижи на хората с психични проблеми; осигуряване на приемственост на грижите и социалното им включване; гарантиране собствената им и на обществото безопасност; борба със стигмата и дискриминацията на психично болните, гарантиращи снижаването на икономическото и социалното бреме от психичната болест. Тези цели, заложени в стратегически документи не могат да се постигнат без добре подготвени специалисти в екипи за психосоциална рехабилитация.

Modern science describes metabolic health as the ability of an organism to resist to ever changing conditions of the environment. It is considered that the faster and efficient phenotypic adaptation and well maintained homeostasis means a healthier organism. This portrays the so called phenotypic flexibility of the individual.
At present there are no parameters and biomarkers standardized for evaluation of the changes in phenotypic flexibility. Therefore, we aimed to examine the changes in the phenotype of individuals in response to nutrition intervention studies (caloric restriction and herbal infusion intake) and stress tests (oral glucose tolerance test and stimulation of peripheral blood mononuclear cells (РВМС) with bacterial lipopolysaccharides (LPS), in order to identify biomarkers for assessment of the phenotypic flexibility.
The study established that the caloric restriction for 12 weeks leads to favorable changes in the metabolic phenotype represented by decreased levels of TAG, total cholesterol, LDL- cholesterol and leptin in overweight volunteers and in the oxidative phenotype represented by decreased levels of 8-OHdG in РВМС as a marker for DNA oxidation. The applied model of LPS stimulated PBMC and herbal intervention enables identification of differences in the response of normal- and overweight subjects to stress tests and intervention. It is established that the glucose load exerts long-term effects on the metabolic and oxidative phenotype represented by changes in the levels of total and LDL-cholesterol, glutathione and MDA in erythrocytes. Biomarkers are identified for evaluation of the changes in phenotypic flexibility of normal- and overweight subjects after an intervention study.

Съвременната наука описва метаболитното здраве като способността на организма да устои на непрекъснато променящите се условия на околната среда. Смята се, че колкото по-ефективно и бързо организмът адаптира фенотипа си към променените условия и стресорите на средата и възстановява хомеостазата, толкова по-здрав е той. Това представлява т. нар. фенотипна пластичност на индивида.
До момента не е правен подбор на маркери и няма стандартизирани параметри за проследяване на промените при изучаване на фенотипната пластичност. Именно затова и настоящото проучване цели проследяване промяната във фенотипа на индивиди в отговор на хранителни интервенции (калорийна рестрикция и билкова интервенция) и стресови тестове (орален глюкозо-толерантен тест и стимулиране на периферни мононуклеарни кръвни клетки (РВМС) с бактериални липополизахариди (LPS), за да се идентифицират биомаркери за оценка на фенотипната им пластичност.

Children with special needs are a sensitive topic for society because they are subject to a policy of radical change from isolation in special institutions to deinstitutionalization and development of alternative care and other social services for them and their families.
The present study accentuates on the global trends in child protection and realization of children’s rights, on the advantages of raising children with disabilities in family environment, the legal framework governing the status of children with mental and physical disabilities. Modelling of social services is analysed with emphasis on preventive services. A practice review of different EU countries with services for disabled children is presented, along with various forms of provision of health and social care for children with special needs aged 0-6 years old.
The aim of this thesis is: To investigate the opinion of parents, medical professionals and social workers on the quantity and quality of care for children with special needs aged 0-6 years old, and based on the results to propose a model of nursing service which supports family environment care for these children.
Tasks: To elicit the specific characteristics of children from orphanages in Varna, Shumen, Dobrich, Stara Zagora and Bourgas; parents` survey about their needs for special training and support; to explore the view of medical professionals and social workers who collaborate with families about the quality of services for children with special needs. Different sources of information have been given to the targeted groups: historical, documentary (many official documents from various state and municipal institutions and NGO`s are analysed), sociological, statistical and other. The presented stages of the study show the sequence of its organizational steps. The methodology includes concepts vocabulary and instruments developed for each of the surveyed groups: parents (61), medical specialists from orphanages (107), and health visitors trained in UNICEF project (43), experts from the social sphere (95).
Results give insights on the contingent of children`s centres nosological affiliation (mostly CP), the needs for medical intervention and procedures and corresponding type of staff - speech therapists, psychologists, masseurs, physiotherapists. Parents’ expectations of health status outcome are putted together with actual achievements. These data, along with the opinion of the specialists for the barriers in communication with parents, emphasise the need of training and guidance for the families of children with special needs. Positive impact on the satisfaction of professionals working with special needs children is the emotional attitude of providing care; negative footprint is the workload and time deficit for quality service. Factors and practices designated to increase the satisfaction with the process of care of children with special needs have been derived for each of the studied groups. The thesis reviews models of family functioning in systems theories. It presents the theoretical aspects of parent/child relationships and describes the specifics of the expression of anxiety in normal children and children with pathological conditions.
Research models of the link between parenting style and anxiety in children are presented, along with data published between 1990 and 2002. Empirical evidences of three traditional parenting categories: parental acceptance, control and anxiety modelling are displayed. Basic terms as “preschool anxiety”, “parenting style” and “parental behaviour” are defined.

• Student’s T-test for differentiation of averages in the variables of various assessments;

• Regression analysis.
Psychometric characteristics of SFI (Self-Report Family Inventory: Version II) in Bulgarian settings are presented in an adapted family relationship questionnaire. The questionnaire was created by R. Beavers to examine how families function. It features 36 items that form five areas: health/competence, conflict, cohesion, leadership and emotional expressiveness. The questionnaire is used as a screening study of the perception of a family member of how the family functions. It is based on the R. Beavers Systems Model of Family Functioning. Exploratory factor analysis was used to produce a family relationship scale with Cronbach’s alpha of 0.794.
The psychometric characteristics of the author’s scale of preschool behavioural anxiety in Bulgarian settings are presented in an adapted preschool behavioural anxiety questionnaire. This thesis aims to assess the feasibility of a comprehensive prognostic screening model for early detection of eating disorders.
Attainment of that objective provokes the following tasks:
• To analyse the status and relevance of the problem of eating disorders (ED).
• To explore contemporary literature and experience with applied tools for ED screening.
• To establish and justify prognostic screening model to capture eating disorders.
• Based on empirical research to validate the test screening methods for early detection of eating disorders.
Hypotheses:
• The proposed methodology is eligible for reliability in Bulgarian conditions.
• It is assumed that the factor structure of the methodology used for Bulgarian conditions is similar to the factor structure in other countries.
• Assuming there is comorbidity between eating disorders and depression.
• It is believed that a more pronounced eating pathology is closely associated with higher values of negative body image.

Целта на дисертацията е оценка на приложимостта на комплексен прогностичен скрининг модел за превенция на хранителните разстройства.
ЗАДАЧИ
Постигането на целта провокира решаването на следните задачи:
• Да се направи анализ на състоянието и актуалността на проблема, свързан с хранителните нарушения (ХН).
• Да се проучи съвременната литература и опит по отношение на прилаганите инструменти за скрининг на ХН.
• Да се създаде и аргументира прогностичен скрининг модел за улавяне на хранителни нарушения.
• На основата на емпирични изследвания да се валидизират тестови скрининг методики за ранно откриване на хранителни нарушения.
ХИПОТЕЗИ
• Предлаганата методика отговаря на условията за надеждност в български условия.
• Предполага се, че факторната структура на използваната методика за български условия е сходна с факторната структура в другите страни.
• Предполага се наличието на коморбидност между хранителни нарушения и депресивни състояния.
• Предполага се, че по-изразената хранителна патология е тясно свързана и с по-високи стойности на нарушена представа за собственото тяло.

При събирането на данни са използвани следните методики: скрининг/ въпросниците:The SCOFF Questionnaire (Morgan, 1999) - (Sick, Control, Fat, Food), Body Shape Questionnaire (BSQ; Cooper at al.,1987), Eating Disorder Diagnostic Scale (EDDS) и Въпросник на Beck за депресия (Beck Depression Inventory II- BDI; Beck et al., 1961; 1988).

The purpose of this thesis is to investigate the medical and social aspects of nursing in outpatient care for MS patients and to develop an algorithm for complex nursing activities and a model for a training centre for nurses involved with these patients. The following tasks have been formulated:
to investigate the medical and social needs of patients with newly diagnosed MS and of those on disease-modifying treatment; to investigate the views of patients' relatives about the need and benefits of specialized nursing care; to investigate the assessment of general practitioners, neurologists and healthcare specialists regarding the provided complex care; to study the quality of life self-assessment in MS patients with the MSQL-54 questionnaire; to investigate the opinion of MS patients who have undergone individual self-injection training. The study explored the opinion of 296 responders divided into 4 groups.
The following methods have been applied: sociological, including documentary; medicostatistical inquiries, SWOT analysis; own experience.
Contributions: The medical and social needs of MS patients of timely and complete information on the choice of medication, training and monitoring the condition during the course of the disease were established; New information has been obtained through the MSQOL-54 questionnaire; The role of the nurse in the overall process of planning long-term care in relation to the course of the disease - training, monitoring and evaluation of the outcomes of care - was established; The need for measures to overcome the barriers of inequality between MS patients with complex quality health care has been demonstrated.
An algorithm for complex nursing activities aimed at MS patients with outpatient care has been developed, which improves the monitoring, treatment and social adaptation of patients with MS by unifying and regulating certain activities.
A Model of a Training Centre for Specialized Training for Healthcare Professionals has been proposed that provides highly specialized nursing services for MS patients and sustainable product development through ongoing nursing training. Participation in a team of authors to compile a patient's diary to track regular injections of Interferon Beta and to develop a step-by-step guide to the new Interferon Beta system.

The musculoskeletal disorders are common conditions among children and are usually caused by the long-term effects of poor posture.
The purpose of this study is to examine the measures for prevention of spinal deformities and to evaluate the effects of the application of remedial gymnastics to improve the posture in preschool-age children.
The study was held in the period 2014-2015. It involved 300 children aged between 5 -7 from the city of Varna. Their posture was evaluated through somatoscopy, functional tests of Matthiass and Kiel and balance durability test which were conducted both in the beginning and at the end of the study. The children were divided in equal numbers into three groups: experimental group (EG) 1, which included exercises with sports hammock, EG 2 which included standard remedial gymnastics and control group (CG). To analyse the data we used descriptive methods, methods of testing the hypotheses (Student's t-test, ANOVA; ?2) and correlation analysis.
After application of the methods, the reduction in the frequency of the asymmetries and improvement of the functional durability in the children of EG1 were statistically significant when compared to the results obtained in the CG (p <0.05).
The regular classes with sports hammock for at least six months are recommended in the pre-school children for correction and prevention of musculoskeletal asymmetries.

Мускулно-скелетните нарушения са често срещани състояния сред децата, причинени от дългосрочните ефекти на неправилната поза.
Целта на проучването е да се проучат мерките за профилактика на гръбначните изкривявания и да се оцени ефекта от приложението на изправителната гимнастика за подобряване позата при деца в предучилищна възраст.
Изследването се проведе в периода 2014г.–2015г. с 300 деца на възраст между 5 и 7 години в гр. Варна. Позата им беше оценена чрез соматоскопия и функционални тестове: на Matthiass, на Kiel и за равновесна устойчивост както в началото, така и в края на проучването. Децата бяха разпределени по равен брой в три групи: ЕГ 1 с провеждане на гимнастика със спортен хамак, ЕГ 2 стандартна изправителна гимнастика и КГ. За анализ на данните са използвани дескриптивни, методи за тестване на хипотези (Student’s t-test; ANOVA; ?2) и корелационен анализ .
След приложението на методиката, намаляването честотите на асиметриите и подобрението функционалната издържливост при децата в ЕГ1 е статистически значимо при сравняването ? с резултатите от КГ (р<0,05).
Системните занятия със спортен хамак в продължение на поне 6 месеца са препоръчителни за децата от предучилищна възраст за корекция и профилактика на мускуло-скелетните асиметрии.

Aim of this thesis is to carry out a complex study on the impairments of the musculoskeletal and the peripheral nervous systems of persons employed in apparel production industry and suggest an algorithm for diagnosing, treatment and prevention of the conditions under consideration.
The object of the study is a group of patients with impairments of the musculoskeletal system and the peripheral nervous system, employed in the apparel production industry. For the purposes of the study, 103 persons employed in apparel production with impairments of the musculoskeletal system and the peripheral nervous system, were investigated. The subjects investigated are residents of Pleven, Lovech, Gabrovo and Veliko Tarnovo districts. They were admitted to the Department of Occupational Diseases, of Georgi Stranski University Hospital in Pleven during the period 2014-2016.
Clinical, paraclinical, diagnostic imaging, neurophysiological and statistical methods of investigation were used. Medication and physiotherapy were applied.
The onset and frequency of subjective complaints and the severity of course impairments of the musculoskeletal system and the peripheral nervous system correlated with the age and length of exposure of persons employed in apparel production industry.
The algorithm of clinical, laboratory, imaging, vegetologic and neurophysiological investigations contributes to early and timely diagnosis of occupation-related impairments of the musculoskeletal and peripheral nervous systems in workers in the apparel production.

Цел на научното проучване е да се извърши комплексно проучване на уврежданията на опорно–двигателния апарат и периферната нервна система на работещи в шивашката промишленост и предложи алгоритъм за диагностика, лечение и превенция на разглежданите заболявания.
Обект на проучването са пациенти с увреждания на опорно – двигателния апарат и периферната нервна система, работещи в шивашката промишленост. За целите на проучването са изследвани 103 лица, работещи в шивашката промишленост, с увреждания на опорно-двигателниа апарат и периферната нервна система. Изследваните лица са жители на областите Плевен, Ловеч, Габрово и Велико Търново и са хоспитализирани в Отделението по Професионални болести,УМБАЛ - Плевен през периода 2014 – 2016 г.
Използвани са клинични, параклинични ,образни, неврофизиологични и статистически методи на изследване. Приложено е медикаментозно и физиолечение.
Появата и честотата на субективните оплаквания и тежестта на протичане на уврежданията на опорно–двигателния апарат и периферната нервна система корелират с възрастта на работещите и продължителността на експозицията в шивашкото производство.
Отклоненията на периферната нервна система, установени при неврофизиологичните и вегетологични изследвания корелират с възрастта и експозицията в шивашкото производство.

We conducted an open, randomized, prospective interventional clinical pilot research with a control group. All patients were suffering from panic disorder (according the definition of DSM-IV-TR) and were receiving standard therapy for the disorder by their psychiatrists. The patients were randomly distributed into two groups – a psychodrama group (n = 20) and a control group (n = 20), with a stratification for demographic parameters and illness characteristics. A semi-structured interview according to the diagnostic criteria of DSM-IV-TR, was used for diagnostics of the patients. Afterwards, all the participants in the study were examined with 3 tools – HAM-A, SAI-R and CORE-OM, and the evaluations of the patients were performed three times: the first at the baseline; the second at the 6th month (at the end of the period of the group therapy); and the third at the 6-months follow-up, after the end of the period of the group therapy.
Conclusions: The advantages of parallel therapy with psychodrama and pharmacotherapy in panic disorder are: a significantly higher degree of improvement in anxiety symptoms compared to pharmacotherapy alone, 55% of patients experienced no clinically significant anxiety symptom after 6-month treatment course; greater improvement in overall clinical outcome, reduction of problems and symptoms and increased mental well-being. The improvement achieved is significant and 6 months after the end of the psychodrama treatment course.
The presence of a significant increase in spontaneity in patients treated in parallel with psychodrama and pharmacotherapy is confirmed. Increased spontaneity correlates with reduced anxiety, improvement in overall clinical outcome and functioning and increased mental well-being, which supports the hypothesis of its importance for mental health in people with panic disorder.

Материали и методи: Отворено, рандомизирано, пилотно, проспективно, интервенционално, клинично изследване с контролна група. Пациентите бяха разпределени на случаен принцип в 2 групи – група с психодрама (n = 20) и група с контроли (n = 20), със стратификация за равенство по демографски показатели и характеристики на боледуването. Всички пациенти получават своята фармакотерапия (стандартната за паническото разстройство) при личния си психиатър. За диагностика на пациентите бе използвано полуструктурирано интервю, проведено от клиницист, според дефиницията на DSM-IV-TR. За оценяване на тежестта на разстройството бе използван Hamilton for anxiety, а за измерване на промените в симптомите, качеството на живота и социалното функциониране - CORE‐OM (Evans et al., 2000) и SAI‐R (David Kipper 2008).

▼ The Kingdom of Saudi Arabia follows the holy Qur’an as its Constitution, and virtually every sphere of human activity is governed by the Shariah. Owing to the particular nature of the tenets of Shariah, several forms of business activity are held to be illegal and forbidden, including anything that pertains to gambling, pornography and alcohol. Moreover, the Shariah has its own code with regard to loans and investments, and the charging or paying of interest are both prohibited.
Foreign direct investment (FDI) is almost universally regarded as significant for the growth of an economy. Nations vie with one another to attract FDI from multinational corporations. Numerous studies have suggested a link between FDI and the growth rate of the GDP or gross domestic product of a nation. Globalisation has caused a marked degree of homogeneity in the world economy and the Kingdom of Saudi Arabia offers various business opportunities to foreign investors. However, the intricacies of the workings of the Shariah need to be clearly understood by overseas investors.
In this thesis, an attempt is made to study the investment climate of the KSA, and to explore the various key factors of the Shariah, the desirability of FDI for development, the forces of globalisation, and the mechanisms by which the KSA can uphold its laws as well as profit from an increasing infusion of funds by overseas investors. The research yields several interesting insights into various aspects of the KSA investment climate, and concludes with a series of recommendations that are designed to reconcile the apparently conflicting interests and pressures.

▼ Obstructive sleep apnoea (OSA) is a chronic sleep-related breathing disorder that if left
untreated leads to serious adverse health consequences, poor quality of life, and also impacts
negatively on society. Continuous positive airway pressure (CPAP) is widely acknowledged as
the best available treatment for moderate to severe OSA. CPAP treatment has been linked to
reduced co-morbidities as well as improved quality of life. However, adherence to CPAP therapy
is a major obstacle to effective long-term treatment. The aim of this study was to explore and
identify predictors of CPAP adherence in a sample of patients with moderate to severe OSA.
Specifically the study explored; 1) the combination of psychological factors—mood, personality
self-efficacy, health locus of control, and health belief—that best predicted adherence and nonadherence
to CPAP use; 2) the impact of adherent CPAP use on mood following the
implementation phase; and 3) the impact of adherent CPAP use on sleep-related variables
collected from polysomnography at the diagnostic phase. Traditionally, much of the research on
OSA and treatment adherence has focussed on sleep-related variables that are likely to predict
CPAP adherence. In contrast, the current study explored the predictive efficacy of psychological
factors. A total of 156 sleep study patients were invited to participate in the present study with 69
adherent patients participating in both the diagnostic and implementation phase and 87 nonadherent
patients only participating in the diagnostic phase. The sample comprised mainly of
men (65%) diagnosed with moderate to severe OSA, with a mean age of 49 years, and a mean
body mass index of 32. Predictor variables included mood, self-efficacy, personality, health
locus of control, and health beliefs. Results from a discriminant function analysis revealed that
anger/hostility, vigour/activity and depression/dejection on the mood measure and self-efficacy,
internal health locus of control, and perceived susceptibility and perceived benefits on the health
belief measure were significant predictors accounting for 59% of the variance of CPAP iii
adherence. Cross-validated classification showed that the overall predictive accuracy was 88%.
The results also showed a positive and strong statistically significant reduction in the Apnoea-
Hypopnoea Index as well as a positive and strong statistically significant increase in O2
saturation at implementation of CPAP use that demonstrated that CPAP treatment continues to
remain an effective treatment option for OSA sufferers. While more research is still needed to
exploring the predictive value of a range of psychological factors in relation to CPAP nonadherence
in moderate to severe OSA patients the present study provides initial useful
information for predicting adherence and non-adherence. This information is likely to be vital to

Cambodians shaped their individual and social perception as well as performance of
identity. It examines the English language as an increasingly dominant cultural and
linguistic presence in Cambodia and endeavours to fill the epistemic gap in what Gee
(2008, p. 1) has identified as the ‘other stuff’ of language. This other stuff includes
‘social relations, cultural models, power and politics, perspectives on experience,

▼ Visitors are increasingly travelling to destinations in search of culinary experiences. Food functions not merely as physiological sustenance but also as a destination experience enhancer, providing opportunities to learn about destination culture through direct encounters with local cuisines. Although a substantial amount of research has investigated food-related visitor experiences, such experiences have not yet been addressed from the theoretically comprehensive perspective of three stages of the visitor experience (pre-, during, and post-). Moreover, most research in this domain has been conducted in more developed tourist destinations, neglecting emerging destinations, including Indonesia. Considering these shortcomings, the overarching aim of this PhD project is to improve understanding of dining on local food that international visitors experience when travelling to a destination, using Indonesia as the research context.

▼ Consumer demand for perfumes in Saudi Arabia is strong but with the demand increasingly focused on perfumes imported from Western countries rather than on traditional perfumes made in Saudi Arabia. There is, however, little information to understand why Saudi Arabian consumers appear to prefer Western perfumes. It may be because Saudi Arabian consumers are being increasingly exposed to, and have interactions with, other cultures both in Saudi Arabia and overseas. As such, they may be more cosmopolitan in their attitudes and consumption, and their perception of what is authentic is changing.
