▼ In order to ensure long-term adoption and use of augmentative and alternative communication (AAC) technologies, they must be designed to support children to participate within everyday activities and routines that are prioritized by families. Nine parents of children with cerebral palsy who used AAC technologies participated in semi-structured interviews to provide their perspectives of how AAC technologies were integrated into the functional contexts of everyday life. Five major themes emerged from the discussions: (a) integrating AAC into life, (b) AAC technologies, (c) child needs and skills, (d) parent responsibilities and priorities, and (e) AAC process and decision-making. Children were able to use AAC technologies within a variety of everyday contexts with various partners; however, challenges included access to AAC within physical and outdoor activities and partners who lacked knowledge regarding operational competencies and effective interaction strategies. In order to integrate AAC technologies into life, parents prioritized technology features including ease of programming, improved physical design (e.g., lighter, wearable, easy to mount, durable), features to enhance efficiency and ease of access (e.g., brain-computer interface, sentence prediction), and availably of multiple functions and features (e.g., facial recognition software, augmented reality, projection capabilities, capability to control wheelchair). AAC manufacturers and mainstream technology developers should work to ensure that technologies are responsive to the supports, limitations, and ideal features identified by parents. Future research should seek input from a larger group of stakeholders and use longitudinal methods to examine perceptions of AAC technologies over time. ▼ PUPRPOSE: Individuals with disabilities, who do not have reliable motor control to manipulate a standard computer mouse, require alternate access methods for complete computer access and for communication as well. The Camera Mouse system visually tracks the movement of selected facial features using a camera to directly control the mouse pointer of a computer. Current research suggests that this system can successfully provide a means of computer access and communication for individuals with motor impairments. However, there are no existing data on the efficacy of the software’s communication output capabilities. The goal of this case study is to provide a comprehensive evaluation of Camera Mouse as a computer access method for Augmentative and Alternative Communication (AAC) for an individual with cerebral palsy, who prefers to use her unintelligible dysarthric speech to communicate her desires and thoughts despite having access to a traditional AAC system. METHOD: The current study compared the Camera Mouse system, the Tobii PCEye Mini (a popular commercially available eye tracking device) paired with speech generating technology, and natural speech using a variety of tasks in a single dysarthric speaker. Tasks consisted of two questionnaires designed to measure psychosocial impact and satisfaction with assistive technology, two sentence intelligibility tasks that were judged by 4 unfamiliar listeners, and two language samples designed to measure expressive language. Each task was completed three times—once for each communication modality in question: natural speech, Camera Mouse-to-speech system, and Tobii eye tracker-to- speech system. Participant responses were recorded and transcribed. RESULTS: Data were analyzed in terms of psychosocial effects, user satisfaction, communication efficiency (using intelligibility and rate), and various measures of expressive output ability, to determine which modality offered the highest communicative aptitude. Measures showed that when paired with an orthographic selection interface and speech-generating device, the Camera Mouse and Tobii eye tracker resulted in greatly increased intelligibility. However, natural speech was superior to assistive technology options in all other measures, including psychosocial impact, satisfaction, communication efficiency, and several expressive language components. Though results indicate that use of the Tobii eye tracker resulted in a slightly higher rate and intelligibility, the participant reported increased satisfaction and psychosocial impact when using the novel Camera Mouse access system. CONCLUSION: This study is the first to provide quantitative information regarding the efficiency, psychosocial impact, user satisfaction, and expressive language capabilities of Camera Mouse as a computer access system for AAC. This study shows promising results for Camera Mouse as a functional access system for individuals with disabilities and for future AAC applications as well. Advisors/Committee Members: Stepp, Cara E. Os desafios colocados aos Recursos Humanos são cada vez maiores. A par da crescente evolução tecnológica e das suas consequências na organização do trabalho, no conteúdo dos empregos e no aumento das qualificações dos trabalhadores, é reconhecida a necessidade dos empregadores investirem em mão-de-obra qualificada, para permitir uma maior qualidade e desempenho dos seus recursos humanos. Assim, urge implementar programas de ação destinados a promover a formação profissional contínua, dinamizando a conceção da formação e a modernização na gestão, nas metodologias e nos equipamentos necessários à formação profissional contínua, que respondam à evolução das necessidades de formação. Neste sentido, o presente estudo centrou-se numa abordagem das novas tecnologias aplicadas à formação em contexto organizacional. O objeto de estudo foi a AMA – Agência para a Modernização Administrativa, que gere entre outros, as Lojas do Cidadão e Lojas de Empresa, serviços descentralizados geograficamente ao longo do País. Como objetivos pretendeu-se analisar a importância e a eficácia da formação b-learning enquanto instrumento de desenvolvimento dos trabalhadores nas Lojas do Cidadão, no contexto de uma organização com serviços descentralizados, e perceber de que forma as novas tecnologias de informação e comunicação podem ser uma ferramenta preponderante na formação contínua dos trabalhadores. Utilizou-se a metodologia qualitativa como suporte ao processo de investigação, com um carácter exploratório. Para tal, recorreu-se à metodologia do estudo de caso, a qual pretendeu verificar o impacto registado na organização e nos indivíduos, dos processos de formação e das aprendizagens realizadas. Para recolha de informação utilizou-se a técnica de entrevista semiestruturada (ao responsável da formação, formador/tutor e formandos das ações), a análise documental e análise de conteúdo de diferentes tipos de documentação produzida pela organização no decorrer das ações. A análise dos dados permitiu averiguar o impacto positivo no processo de aprendizagem e de melhoria do desempenho daqueles que integraram os grupos de formação da ação piloto realizada em 2008. A implementação da metodologia de formação b-learning, permitiu à organização fundamentar a criação de um departamento de formação (2010) para responder eficazmente às necessidades da mesma, com a introdução de novos cursos, considerando a missão a organização, os seus valores e os padrões de desempenho exigidos aos trabalhadores que prestam atendimento ao público na Agência para a Modernização Administrativa em geral, e nas Lojas do Cidadão e Lojas de Empresa, no período de 2008 a 2012.

▼ Toddlers who demonstrate significant speech impairments and use augmentative and alternative communication (AAC) often demonstrate concomitant expressive language impairments. These toddlers receive AAC intervention due to difficulties with speech production at the motor level, and receive language intervention due to difficulties with vocabulary development at the linguistic level. One language intervention approach with a large base of evidence supporting efficacy in young children who have language delays, but do not use AAC, is responsivity education combined with focused stimulation. Another language intervention approaches with a large base of evidence supporting efficacy in young children who use AAC is responsivity education combined with aided AAC modeling. However, the effectiveness of responsivity education and focused stimulation has not been systematically studied, nor has the approach been compared to responsivity education and focused stimulation with the addition of AAC modeling in toddlers who use AAC.A challenge of language intervention is to provide effective and efficient treatment for the expressive language impairment. To address this critical challenge with toddlers who use AAC, the current investigation compared the effectiveness and efficiency of these language intervention approaches in teaching new vocabulary production to toddlers between the ages of two and three who received AAC intervention. Specifically, the study examined the possible added value of including AAC modeling to an intervention involving responsivity education and focused stimulation. The study used an adapted alternating treatment design across participants. Four 2-year-old toddlers, who used AAC to communicate, participated in the study. One toddler used a visual grid, manual sign, and word approximations/words to communicate; one toddler used manual sign and word approximations/words to communicate; one toddler used a communication board, manual sign, and word approximations/words to communicate; and a final toddler used a flip chart and word approximations/words to communicate.The results of the study provided preliminary evidence that the language intervention approach of responsivity education and focused stimulation was more effective for improving the vocabulary production in three of four participants in the study. The added value of including AAC modeling in the intervention resulted in more effective vocabulary production with the fourth participant. In addition, the results of the investigation provided preliminary evidence that the language intervention approach of responsivity education and focused stimulation was more efficient for improving the vocabulary production in two of the four participants in the study. ▼ In this study a new hybrid voltage source converter based topology is modeled, simulated and controlled. This new topology is usually known as alternate arm converter. For the simulation developing MATLAB/Simulink software is used. With the MATLAB/Simulink software the model of alternate arm converter is developed from the scratch. Firstly the simple one-phase average mathematical model is developed and implemented in the simulation. Then the balancing control between the arms voltages is implemented in the one-phase model. The simple one-phase with a simplified control model is evaluated. Then the three-phase model is created in the same MATLAB/Simulink software. Firstly the three-phase model is evaluated as working in the island mode, which means that it is working alone, not connected to the grid. The improved control is implemented in this 3-phase model in order to increase the performance of the converter. The 3-phase model with improved control model is connected to the grid by using MATLAB/Simulink software. The grid connection is simulated with the voltage sources and phase locked loop control. The phase locked loop constantly adjust the voltage in order to lock onto the phase and frequency. Furthermore it is extract a voltage angle for the used dq0 transformation, which simplifies control. The grid connected model is simulated and evaluated. After this evaluation the MATLAB/Simulink simulation model of the alternate arm converter, together with control is connected in the point to point scheme. The point to point scheme represents the energy transmission from the one point to another or in the different case it can also transmit the energy in the other side. The point to point system is represented in the MATLAB/Simulink model, together with additional droop control, which controls the DC voltage accordingly to the active power. If the active power reaches the maximum value the DC voltage decreases accordingly to the drop value and the way around. The six study cases are simulated in the MATLAB/Simulink software, with the point to point connection. In the first case the operation points are evaluated of the system and the behaviour of the AC voltages, AC currents, DC voltage, arm voltages, arm currents and circulating currents are observed in the terminal 1 and terminal 2, which is AC/DC and DC/AC converters, respectively. The second study case is aimed to inspect the DC side capacitance effect to the DC voltage. In this study case is evaluated that by increasing the DC side capacitance the DC voltage ripple reduces, therefore the AC currents with less harmonic distortions are presented. The third case shows the droop control behaviour for the different drop values. When the drop value is increasing the gain value of the droop control decreases and the smaller deviation of the active power is presented. ▼ Background: The Stroke and Aphasia Quality of Life Scale-39 (SAQOL-39) is a valid, reliable quality of life (QoL) assessment for PWA (Hilari, 2003; Hilari, Byng, Lamping, & Smith, 2003). However, individuals with severe aphasia are unable to use it because their ability to comprehend the text is too severely impaired for accurate self-report (Hilari & Byng, 2009; Hilari et al., 2003; Hilari, Owen, & Farrelly, 2007). Proxy respondents often report on QoL measures for these individuals; however, studies suggest differences between proxy-reported and self-reported scores in less severe populations (e.g., Cruice, Worrall, Hickson, & Murison, 2005; Engell, Hütter, Willmes, & Huber, 2003; Hilari et al., 2007). Therefore, proxy-reported scores may not be reliable substitutes (Cruice et al., 2005). Incorporating images may increase accessibility of text to individuals with severe aphasia by drawing upon intact visuo-spatial abilities (Dietz, Hux, McKelvey, Beukelman, & Weissling, 2009; Dietz, McKelvey, & Beukelman, 2006; Elmore-Nicholas & Brookshire, 1981; Engell et al., 2003; Rautakoski, Korpijaakko-Huuhka, & Klippi, 2008). Objective: This study assessed similarities between high-context, colored photographs and SAQOL-39 questions. Methods: This study employed a non-experimental, within-group design. Twenty individuals between 65-85 with no history of aphasia completed a 7-point Likert scale rating task wherein they rated the degree of similarity between photographs and SAQOL-39 questions. Three expert reviewers evaluated the photographs before being used as stimuli. Results: Results of the 7-point Likert scale rating task revealed a mean rating of 6.06 (range 5.05 – 6.70) for all 42 photograph-question pairings (i.e., three training items plus 39 items). Thirty-nine of the total 42 photographs (93%) were rated > 6 at least 60% of the time. Thirty-seven of the 39 actual scored SAQOL-39 questions (95%) were rated > 6 at least 60% of the time. The mean standard error of the mean (SEM) for all 42 photographs was 1.44. The average mode was 7.00, and the average median was 6.74. Conclusion: Results indicated that photographs were rated as being similar to SAQOL-39 questions. Further research is warranted to evaluate if the photographs enhance accessibility of the SAQOL-39 to individuals with severe aphasia allowing for self-report.

▼ Amyotrofiseen lateraaliskleroosiin (ALS) liittyy usein motorisia puhehäiriöitä. Useissa tutkimuksissa puheen ja kommunikoinnin häiriötä sekä niiden muutoksia on tutkittu potilailla, joiden ALS diagnoosi on jo varmistunut. Monilla ALS-potilailla on selkeitä puhemotoriikan vaikeuksia ennen diagnoosin varmistumista osan menettäessä puhekyvyn diagnosointiprosessin aikana. Kliinisessä työssä tulisi tavoitella oikea-aikaisia puheterapiainterventioita, jotta potilaiden kyky kommunikoida tehokkaasti ja viestintätarpeiden mukaisesti pystyttäisiin varmistamaan. Tämän väitöskirjatutkimuksen tavoitteena oli tutkia alimpien aivohermojen oireita, puheentuoton vaikeutta sekä kommunikoinnin tehokkuutta lähtötilanteessa, joksi asetettiin ensimmäinen puheterapia-arvio sekä seurata kommunikoinnin muutoksia kahden vuoden ajan kansainvälisen toimintakykyluokituksen (International Classification of Functioning, Disability and Health (ICF)) toimintakyky ja rajoitteet osa-alueen näkökulmasta. Lisäksi tavoitteena oli selvittää vaikuttaako diagnoosin varmuusaste tai sairauden alkamismuoto (bulbaari- tai spinaalialkuinen) puheentuottoon ja kommunikointiin. Tutkimukseen osallistui peräkkäisotannalla 30 ALS-potilasta, joilla oli vähintään yksi bulbaarioire. Aineisto koostuu 157 puheterapeutin tekemästä puheen ja kommunikoinnin arviosta sekä 148 kommunikoinnin tehokkuuden itsearvioinnista. Puheterapia-arvioinnit sisälsivät puheen motorisen tuoton, puhenopeuden, puheen ymmärrettävyyden sekä motorisen puhehäiriön vaikeusasteen arvioinnit. Lisäksi kartoitettiin toiminnallinen kyky kommunikoida, tarve kommunikointia tukeville ja korvaaville (AAC) keinoille sekä käytössä olevat AAC-keinot. Potilaat itsearvioivat kommunikoinnin tehokkuuden sekä puheella että mahdollisilla AAC-keinoilla. Aineiston analysoinnissa käytettiin sekä määrällisiä että laadullisia menetelmiä. Alkuarviossa tutkittavilla esiintyi eriasteisia motorisen puheentuoton oireita, kolmella potilaalla oli jo käytössä AAC-keino heikentyvän puheen tukena. Puheentuoton ja kommunikoinnin häiriöt olivat vaikeampiasteisia potilailla, joiden ALS-diagnoosi ei ollut vielä varmentunut lähtötilanteessa. Näillä potilailla myös aika bulbaarioireiden ilmenemisestä puheterapiaintervention toteutumiseen sekä diagnostiikan varmistumiseen oli pitempi. Suurimmalla osalla näistä potilasta sairaus oli alkanut bulbaarioireilla. Puheen ja kommunikoinnin vaikeudet lisääntyivät sekä puheen tehokkuus heikentyi oletetusti sairauden edetessä lähes kaikilla potilailla. Motorinen puhehäiriö muuttui vaikea-asteiseksi 18 kk:n kuluessa bulbaarioireiden alkamisesta 60%:lla potilaista, kun taas 40%:lla potilaista säilyi kyky kommunikoida adekvaatisti puheella. Vähintään yhtä manuaalista tai tekniikkaa hyödyntävää AAC-keinoa käytti 57%:a tutkittavista. Yli 60%:lla tutkittavista toiminnallinen kyky kommunikoida heikentyi seurannan aikana. Thereâre lots of special B&Bs recently, which not only serve for accommodations but serve in-depth tour as well, and make passengers can totally consider the local culture during the tour. B&Bs differ from hotels' emphasis on professional service, the operation of B&Bs stress breadth,speed, and customization service. Thus, this research will study the training needs and contents for B&Bs in Taiwan. The goal of this research will study B&Bs' value of training nowadays and deeply study the training ways and needs suited for B&Bs. The data of hotels and B&Bs in this study is from "Taiwan Host Event" held by Tourism Bureau, MOTC, R.O.C.. Meanwhile, this study chooses eight B&Bs to analyze through interview, documentary analysis, and then used literature analysis to conclude. The definition of B&Bs in this study is self-characteristic, delicate, special ,and creative tourist and lodging house. B&Bs' business model is differ from general hotels and the training needs should be 4 points as follows: 1.OJT/ Off-JT strengthening: With the development of information technology, people can get more information and much interaction through the internet. The training of B&Bs should be focus on interactive study ; on-the-job training and off-job-training would be the key point. 2.B&Bs Social time planning: Not only being familiar with SOP and customers accommodation, but the emergency management is the best time to show quality of service as well. Thus, emergency management is the most significant; B&Bs can share with each experience and interact through workshop, and can easily learn how to deal with emergency situation during the training class. 3.Service Style establishment: The most important thing to build up each B&B own style is creation and getting into the swing of B&Bs industry. Therefore, how to strengthen the ability of perception, innovation and creation, is much more important than SOP training. 4.Cross-Field class connecting and planning: Tourism service has its own specific characteristics, and B&Bs cannot meet the whole tour needs. The urinary tract infections (UTI) have high incidence in the population, mainly affecting women, the elderly and pregnant women. In most cases an UTI is caused by Enterobacteriaceae, especially Escherichia coli. The main antimicrobials used in the treatment of UTIs, are the quinolones and fluoroquinolones. Since its discovery in the 60s, such antimicrobials were improved and increasing its spectrum of activity, but also the micro-organisms has evolved to develop resistance to these drugs. The resistance comes down to two mechanisms: changes in targets of quinolones or decreased concentration of the drug within the cell. Quinolones are antimicrobial agents that target two enzymes involved in transcription and replication of bacterial DNA, DNA Gyrase, consisting of two subunits GyrA and two GyrB products of the genes gyrA and gyrB respectively and Topoisomerase IV, also composed of two ParC and two ParE subunits, encoded from genes parC and parE. Mutations in these genes can confer high-level resistance to these antimicrobials. Other mechanisms involve genes carried in plasmids such as qnr s, responsible for the production of proteins that protect the target enzymes of quinolones. The gene aac (6 )-Ib-cr also present in plasmids can alter the chemical structure of certain fluoroquinolones interfering efficiency. The association of resistance to quinolones and β-lactams due to the presence of Extended Spectrum β-lactamases (ESBL) has been widely reported in the literature. The main objective of this study was to evaluate the patterns of resistance to quinolones and fluoroquinolones in 55 samples of uropathogenic E. coli collected in Curitiba, PR, Brazil. The resistance to nalidixic acid, norfloxacin, ciprofloxacin and ofloxacin was observed in 69.1% of the samples. Molecular analysis of the resistant strains showed mutations in gyrA (codons 83 and 87) and parC (codon 80). Also investigated was the presence of genes qnr s and aac (6 )-Ib-cr and their association with ESBLs. Two of the 55 samples were positive for aac (6 )-Ib-cr. The presence of this gene in Brazil was first reported in 2012 in Belo Horizonte, MG. No samples were positive for the qnr s genes. These results show a high incidence of resistance to quinolones and the ability to horizontally spread of acquired resistance between the bacterial strains.

▼ Communication partners with AAC training are known to increase the ability of a person with aphasia to understand language and participate in everyday social interactions (Genereaux et al., 2004; Hickey, Bourgeois, & Olswang, 2004; Kagan, Black, Duchan, Simmons-Mackie, & Square, 2001; Lyon et al., 1997; Rayner & Marshall, 2003; Rose, Worrall, & McKenna, 2003; Simmons-Mackie, Kearns, & Potechin, 2005). Healthcare providers, when trained in AAC strategies, serve as important communication partners in the acute care setting. Given more effective communication, they increase involvement in treatment planning by individuals with communication impairments (Kagan et al., 2001; Legg, Young, & Byer, 2005; Simmons-Mackie et al., 2007). Previous studies have identified barriers and facilitators to AAC support use in acute care settings for nurses (Baker & Melby, 1999; Bergbom-Engberg & Halajamae, 1993; Hemsley et al., 2001, Leathart, 1994; Simmons-Mackie et al., 2007). While knowledge of nursing-specific is valuable, because nurses spend a substantial amount of time with patients, nurses are one of many healthcare professionals that interact with patients during their stay in the acute care unit. There is little research investigating possible communication barriers between patients and other healthcare staff. The aims of this study were to identify potential barriers that facilitators, physical therapists, and occupational therapists encounter when implementing AAC strategies in an acute care setting and to determine if or how the barriers and facilitators differ by disciplines. Five healthcare providers participated in individual semi-structured interviews. This study identified 18 different barriers and 19 different facilitators. Results indicated that physical and occupational therapy disciplines felt most challenged by using AAC strategies to support individuals with severe communication impairments. Healthcare providers identified their own knowledge of and skill using AAC, the availability of AAC resources, and the patient health status and ability as the most substantial barriers to using AAC supports with patients with severe impairments. Likewise, they identified colleague AAC knowledge and skill as an important facilitator to enhancing their own AAC knowledge and skill. Findings from this study support the need for additional opportunities for training and information sharing between healthcare providers. Both of these recommendations will increase healthcare providers' AAC knowledge and skill, which in turn will help the providers feel more competent and confident implementing communication supports.

Augmentative and alternative communication (AAC) is defined as “all forms of communication (other than oral speech)…used to express thoughts, needs, wants and ideas” (“Augmentative and Alternative Communication (AAC),” 2012). Working memory is a temporary cognitive process, which briefly maintains and manipulates information while it is being encoded as a part of long-term memory (Engle, Nations, & Cantor, 1990; “Introduction to Working Memory”, 2007). It has been suggested that based upon the unique skill set and needs of users of AAC systems, the design of these systems should reflect knowledge gleaned from the cognitive sciences (Light & Lindsay, 1991) with training and implementation of AAC incorporating an understanding of the cognitive processes impacting memory, learning, and visual processing (Light & Lindsay, 1991; Wilkinson & Jagaroo, 2004). This study sought to examine how users of AAC managed and perceived the cognitive load associated with working memory demands while communicating and what specific strategies and/or design features users of AAC perceived they used during conversation when using AAC. Results revealed an overall large amount of variability in participants’ responses. Length of symbol/word sequences, word prediction, seeing the message as it is being created, attention to the conversational topic, and attempting to remember what their conversational partner said appeared to be judged as having the highest degree of importance for the use of a speech generating device and success and message completion in conversation. Errors in conversational while using a speech generating device and stressors during the conversational process appeared to be most closely related to reported lack of time to create messages and the time it takes to create messages. Users of AAC did not report high frequency of actives attention to the working memory processes and design features.

The aim of this study was evaluated the frequency of the plasmid mediated quinolone resistance genes qnr, aac(6)Ib-cr and qepA. Were evaluated 231 Enterobacteriaceae isolated during the June of 2008 by the Central Laboratory of the São Paulo Hospital. Were analyzed by multiplex PCR followed by sequencing of the respective amplicons determinants qnrA, qnrB, qnrS, qnrC, qepA and aac(6)Ib. Into all the 231 enterobacterial isolates analyzed, nine had 99% homology with qnr genes described in the literature, among them, one E. aerogenes showed identical sequence to the gene qnrS1, three K. pneumoniae isolates showed sequences identical to qnrS1 and qnrB2, one E. cloacae showed sequence identical with qnrS1 and three E. coli presents sequence identical with qnrB19 and qnrS1. The qepA genes were not found by the PCR methodology. The aac(6)Ib gene was found in 14 samples and the sequence of the amplicons demonstrated that five samples showed the variant aac(6)Ib-cr, among them, two E. coli, two K.pneumoniae and one E.cloacae. In analyzing the transfer of qnr determinants by conjugation technique, samples of E. aerogenes carrier determinant qnrS1, K. pneumoniae and E. coli carrier qnrS1 had transferred its determinants in the recipient cell. The transfer of genes contained in conjugative plasmids was confirmed by PCR technique. The analysis of genetic similarity among isolates positives for the qnr determinants by the methodology of PFGE profiles showed clonal and related determinants qnr different. However we can conclude that the frequency of the determinants qnr and aac(6)ib-cr was low (3,46% and 2,16% respectively), the determinant qepA was not found; qnrS the determinant was more prevalent among the enterobacterial studied and was the first report of this gene in Brazilian samples and in E. aerogenes species. The determinant qnrB2 and qnrS1 was conjugated to recipient cells indicate that may be contained in conjugative plasmids, but more studies are needed to characterize the genetic context. The analysis of genetic similarity revealed that samples with the same genetic profile qnr genes are different, indicating that they were probably contained in mobile elements.

Este estudo foi desenhado com o objetivo de avaliar a frequência dos genes plasmideais que conferem resistência às quinolonas. Foram avaliadas 231 enterobactérias isoladas durante o mês de junho de 2008 pelo Laboratório Central do Hospital São Paulo. Foi avaliada pela técnica da PCR multiplex seguida do sequenciamento dos respectivos amplicons os determinantes qnrA, qnrB, qnrS, qnrC, qepA e aac(6)Ib. De 231 enterobactérias avaliadas 9 possuíam o determinante qnr e o sequenciamento demonstrou homologia de 99% com genes descritos, entre elas, 1 E. aerogenes apresentou sequência idêntica ao gene qnrS1, 3 K. pneumoniae apresentaram sequências idênticas aos genes qnrS1 e qnrB2. Hospitals worldwide are becoming increasingly plagued by antibiotic-resistant pathogens; concomitantly, the number of patients who die from antibiotic-resistant pathogens is increasing. The rise of multi-drug resistant (MDR) pathogens has rendered many antibiotics obsolete. The streptogramin and aminoglycoside antibiotics are drugs of last resort against life-threatening, MDR Gram-positive (e.g., methicillin resistant Staphylococcus aureus (MRSA)) and -negative (e.g., Pseudomonas aeruginosa) pathogens, respectively; however, as microbial drug resistance continues to emerge, the efficacy of these two important drug classes is decreasing. One of the most common mechanisms by which pathogens become resistant to streptogramin and aminoglycoside antibiotics is enzymatic inactivation: the Vat and AAC(3) acetyltransferases are employed by pathogens to inactivate streptogramin and aminoglycoside antibiotics, respectively. There currently is a dire need to not only develop new antibiotics, but to find new, creative strategies to outwit microbial resistance mechanisms. One of these strategies is to rescue the activity of antibiotics through the discovery of antibiotic adjuvants. In the current study, adjuvants which rescue the activity of streptogramin and aminoglycoside antibiotics through inhibition of the resistance acetyltransferases, VatD and AAC(3)-Ia, have been discovered—through the development of a cell-based screening method, we have found the first inhibitors of VatD, as well as of AAC(3)-Ia and its homologues, AAC(3)-Ib, AAC(3)-Ic, and AAC(3)-Id. We have demonstrated that streptogramin and aminoglycoside resistance can be reversed both in vitro and in vivo by the protein kinase inhibitors, GW5074 and rottlerin. Steady state kinetics revealed these compounds to inhibit VatD and AAC(3) enzymes mainly through noncompetitive or mixed mechanisms. This study has also demonstrated that eukaryotic kinase inhibitor libraries may be repurposed for the discovery of not only new antibiotic adjuvants, but also new antimicrobial targets. The inhibitors described herein may someday serve as effective adjuvants of streptogramin and aminoglycoside antibiotics.
