学童期の齲蝕は減少傾向を示し,12歳児での一人平均DMF歯数は1歯台になったが,齲蝕有病者率は6年生でいまだ半数近くを占めており,近年は齲蝕をもつ者ともたない者の格差が取りざたされてきている.このような現状において,学校歯科保健の現場ではリスクに応じ,ポピュレーションストラテジーとハイリスクストラテジーの両面での対策が必要になってきており,とくに学童期を通してハイリスク児童の検出が重要である.そのため,縦断的に変動する齲蝕リスクについて,唾液中のミュータンス連鎖球菌(SM)レベルの変化を指標に,永久歯齲蝕発生への影響について検討を行った.M県下の某小学校において,年1回の定期健診時に口腔内診査と唾液中SM量の検査を実施し,6年間すべての診査,検査を受診した児童291名のうち,1年生時に永久歯齲蝕が発生していなかった284名を対象とした.SMレベルは,Class 0(<104 CFU/mL)をLow SM,Class 1(104〜105 CFU/mL)以上をHigh SMと評価し,1年生時から6年生時までのSMレベルの変化パターンを解析し,永久歯齲蝕発生状況との関連について検討した.各学年におけるカリエスフリーからの永久歯齲蝕発生者率は,いずれの学年時も1割前後であり,学年間の有意差は認められなかった.6年間における永久歯カリエスフリーからの齲蝕発生者の1年前のSMレベルを検索した結果,High SMが82.9%であった.また,1年生時のSMレベル別に検索した結果では,1年生時にHigh SMであった場合,齲蝕発生者の94.6%は直前のSMレベルがHigh SMであったのに対し,Low SM群の場合,直前のSMレベルがHigh SMである確率は22.2%であり,Low SMである確率の方が高かった.以上の結果より,1年生時にLow SMと評価された者については,その後のSMレベルの変動はほとんど齲蝕発生に影響しないが,1年生時にHigh SMと評価された者は,学童期を通じてHigh SMを示した翌年に永久歯齲蝕を発生する確率がきわめて高いことがわかった.
