▼ Dans les régions tempérées de l’hémisphère nord, les tiques représentent le premier vecteur d’agents infectieux d’importance médicale et vétérinaire. Parmi elles, Ixodes ricinus, abondante en Europe, transmet bon nombre de microorganismes aux Hommes et aux animaux, aussi bien des bactéries, des virus, que des protozoaires, pour la plupart responsables de zoonoses considérées comme émergentes. Ces microorganismes sont Borrelia spp., Anaplasma phagocytophilum, Rickettsia spp., Babesia spp. ou encore le virus de l’encéphalite à tique (TBEV). Depuis les années 80’, plus d’une dizaine d’agents pathogènes pour l’Homme ont été découverts dans les tiques en Europe. En Suisse, de nouvelles espèces comme Babesia venatorum, Rickettsia monacensis, Borrelia miyamotoi ou B. spielmanii ont fait leur apparition durant la dernière décennie. De plus, un cas de septicémie dû à Candidatus Neoehrlichia mikurensis, une bactérie transmise par I. ricinus, a récemment été signalé chez un homme résident en Suisse, sans que la bactérie n’ait jamais été décrite dans les tiques du pays. Face à l’émergence de ces pathogènes, nous avons évalué leur distribution géographique ainsi que leur prévalence dans les tiques dans l’Ouest de la Suisse afin d’identifier les zones à risque. Nous avons recherché Borrelia spp., Rickettsia spp., Babesia spp., A. phagocytophilum et Candidatus N. mikurensis dans les tiques en quête récoltées dans 11 sites. Globalement, 34.2% (505/1’476) des tiques étaient infectées par au moins un pathogène. Borrelia spp., Rickettsia spp. et Candidatus N. mikurensis étaient présents dans tous les sites investigués avec des prévalences de 22.5%, 10.2% et 6.4%, respectivement. A l’inverse, Babesia spp. et A. phagocytophilum ont démontré une répartition géographique plus restreinte et une prévalence plus faible (1.9% et 1.5%). Des co-infections, impliquant le plus souvent Borrelia spp. et Rickettsia helvetica, ont été détectées dans 19.6% des tiques infectées. Nous avons identifié Candidatus N. mikurensis pour la première fois dans les tiques sur le territoire helvétique ainsi que des espèces rarement signalées comme R. monacensis, B. lusitaniae et B. spielmanii. En Suisse, ces dernières années, des cas humains d’encéphalite à tiques ont été déclarés hors du périmètre qui délimitait les foyers de TBEV jusque-là, dans l’Ouest du pays. Ainsi, à partir des années 2'000, de nouvelles zones endémiques au TBEV ont été répertoriées dans cette région. Nous avons confirmé la présence du TBEV dans les tiques libres dans l’une de ces nouvelles zones endémiques au virus, la Plaine de l’Orbe avec une prévalence globale de 0.1% (6/6’120). Parmi les cinq foyers identifiés, la prévalence du virus variait de 0.21 à 0.95. ▼ L’encéphalite à tiques (TBE) est une maladie qui touche le système nerveux central causée par un virus (TBEV) qui est principalement transmis par des tiques du genre Ixodes. Présent uniquement sur le continent eurasien, la particularité du TBEV est de se localiser sous forme de foyers. En Suisse, on parle de foyer lorsqu’au minimum 3 cas cliniques ont été déclarés auprès de l’Office fédéral de la santé publique dans un rayon de 10 à 15 Km et/ou si des tiques infectées par le TBEV sont détectées. De 1969 à 2005, tous les foyers de TBEV étaient regroupés dans le nord et l’est de la Suisse. Cependant, en 2006, de nouveaux foyers ont émergé à l’ouest du pays avec l’apparition de cas sporadiques, notamment dans la Plaine de l’Orbe/VD. Cette étude a permis de confirmer l’endémicité de cette région par la détection du TBEV chez les tiques en quête dans 5 (Agiez, Vugelles, l’Abergement et deux sites à Montcherand) des 26 sites investigués avec une prévalence de 0.1% (6/6120) variant de 0.29-0.59% selon les sites. Parallèlement, on a observé une augmentation du nombre de cas de TBE principalement en 2006, phénomène également rencontré dans d’autres pays européens. Les raisons de cette augmentation ont tout d’abord été attribuées aux changements climatiques qui auraient favorisé la survie de la tique Ixodes ricinus. Néanmoins, notre étude a montré que cette augmentation n’était pas directement liée à un réchauffement climatique mais semblait plutôt liée à un changement du comportement humain. En réponse à une augmentation des températures observées en 2006, l’être humain aurait augmenté les contacts avec des tiques infectées en se rendant plus fréquemment en forêt. On sait cependant que le climat peut exercer une influence sur le comportement des tiques et favoriser le principal mode de transmission du TBEV, le « co-feeding » : le TBEV est transmis à des larves par des nymphes infectées. Néanmoins, en raison des conditions microclimatiques extrêmes observées au printemps 2007 dans certains sites étudiés du canton de Berne (Belp, Kiesen, Trimstein), une partie de la population de nymphes en quête a souffert de dessiccation. Ceci a eu pour conséquence de diminuer la proportion de rongeurs infestés par des larves et des nymphes réduisant ainsi les chances de transmission par « co-feeding ». Des printemps chauds à répétition pourraient donc faire disparaître des foyers de TBEV à certains endroits comme l’avait prédit Randolph (2001). Nous avons également recherché la présence du TBEV dans le canton de Berne par l’analyse de tiques en quête et de tiques nourries sur des rongeurs. Dans deux sites connus pour héberger le TBEV (Thun et Belp), la prévalence du TBEV chez les tiques en quête était de 0.14%-0.2%, respectivement. Concernant les deux autres sites dont le statut de foyer était inconnu, le TBEV a pu être mis en évidence pour la première fois chez des tiques en quête à Kiesen avec une prévalence de 0.04% contrairement à Trimstein où sa présence n’a pas pu être démontrée. ▼ This study analyzes the socioeconomic factors behind HIV prevalence and risky sexual behaviors using the 2000 and 2005 Ethiopian Demographic and Health Surveys. The study also examines changes in risky sexual behaviors occurred between the two surveys. Econometric models used in the study include Heckman Probit model for HIV prevalence, Proportional Hazard model for age at first intercourse, and Probit model for condom use and number of sexual partners. According to the results of Heckman Probit model, Women who attend higher education are at lower risk of HIV infection, whereas those who attend primary education are at higher risk. However, education appears to have insignificant effect for men’s HIV infection. On the other hand, wealth has a protective effect against HIV for men, whereas women of highest wealth quintile are at higher risk of HIV infection. Marriage for women and urban residence for both sexes consistently predicts higher HIV prevalence. With regard to the effect of education and wealth on risky sexual behaviors, both factors are associated with lower risky behaviors and the protective effects of these factors are more apparent in 2005 data. The exception to this fact is that education increases hazard rate of early sexual initiation for males in both survey data. Marriage and urban residence significantly increase risky behaviors, with exceptions that marriage reduces multiple partnerships and urban residence increases the probability of condom use. Changes in sexual behaviors are more pronounced among men than women. Compared to the earlier survey (2000), men have experienced later age at first sex, lower probability of multiple partners and higher probability of condom use in the later survey (2005). With exception of lower probability of multiple partners, women, however, exhibited earlier sexual initiation in 2005 than in 2000. Hence, the study assures that there are still opportunities to reduce HIV prevalence by devising policies which promote behavioral changes among women and the poor and illiterate society. ▼ ABSTRACT Background: Obesity and overweight pose major risk for serious diet-related chronic diseases, including type2 diabetes, cardiovascular disease, hypertension and stroke, and certain forms of cancer. Even though there is no well-documented information recently, chronic diseases that are directly or indirectly related to overweight and obesity become increasing in Ethiopia especially in urban areas. Objective: the main aim of this study is to determine the prevalence of childhood overweight and obesity and its determinant factors. Methods: A cross-sectional survey was conducted among 463 elementary school children in Addis Ababa. Study participants were selected using multi stage sampling from 10 public and 10 private elementary Schools. Overweight and obesity was determined using CDC 2000, BMI percentile chart. Socio-demographic and other determinants of childhood overweight were assessed. Data were collected using combination of interview of children and self-administered questionnaire to their parents. Finally data were processed and analyzed using Epi info version 3.5.4and SPSS version 21. Result: A total 463 children and 463 parents participated in the study. The overall prevalence of underweight, normal, overweight and obesity is 44(9.5%), 360(77.8%), 46(9.9%) and 13(2.8%) respectively. Sex specific prevalence of overweight and obesity shows 7.7% and 3.2% for male and 12%and 2.5% for female respectively. Significant association with overweight was observed among car ownership of the family (p<0.001), number of snacks per day (p=0.03), sweet food preference (p<0.001), buying ice cream (p=0.014), eating breakfast regularly (p=0.034), walking or riding bicycle for at least 10 minutes per day (p=0.009) and family participation in regular physical activity (p=0.023). Conclusion and Recommendations: The prevalence of overweight was found to be high even computable with the global prevalence. Among the factors identified are unhealthy dietary pattern like preferring sweet foods, eating breakfast irregularly, buying ice cream and frequent snacking are strongly associated with overweight. In addition physical inactivity are also important factors impacting on the risk of obesity in children. If Primary preventive measures is not taken immediately, prevalence of overweight and obesity among children in Addis Ababa might increase rapidly in the coming few years. Early interventions on modifiable risk factors by promoting active lifestyles and healthy diets are likely to decrease the rate of childhood obesity. ▼ Background Hyponatremia is a common electrolyte disturbance in hospitalized patients with heart failure. It is associated with high morbidity, longer in-hospital stay and is a well known predictor of mortality in patients with heart failure. There is a paucity of data on the prevalence of hyponatremia in patients with heart failure in Kenya. Objectives The aim of the study was to determine the prevalence of hyponatremia in patients admitted with heart failure in the medical wards at the Kenyatta National Hospital. Methodology This was a prospective, observational cohort study conducted over a period of about 3 months in patients above 13 years with heart failure in the medical wards. Patients were screened using the Modified Framingham „Criteria for eligibility and recruited upon signing an informed consent or assent. Sodium levels were done within 24 hours of admission and patients were followed up to determine their outcome at two weeks. The outcomes included, in hospital mortality, discharged home and prolonged stay in hospital beyond the 2 weeks. Results The study was conducted from December 2015 to February 2016. Eighty nine patients with heart failure based on the Modified Framingham‟s Criteria were recruited. 53.9% were males and the mean age of the patients was 51.6±18.8 years with a range of 14-86 years. On admission 94.4% of the patients were in NYHA class 3 and 4. 79.8% (95% CI 71.9-87.6) of the patients had hyponatremia at admission. The mean sodium level was 128.2±8.1 mmol/L. Of these, 84.3% were on a loop diuretic at admission while 6.7% were on a thiazide diuretic. During the 2 week follow up period, 13.5% of all the patients admitted with heart failure died while 48.5% were discharged from the hospital. 38.2% of the patients had a prolonged stay and were still undergoing treatment at the lapse of the 2 weeks. Of those who died 66.7% had hyponatremia while 88.2% of those who were still admitted at the lapse of the 2 weeks had hyponatremia. Conclusion This study found a high prevalence of hyponatremia in patients admitted with heart failure at the KNH. Hyponatremia did not affect the short term outcomes in a statistically significant way; however, the trend suggests worse outcomes in the hyponatremia patients.

Objetivos: Determinar a prevalência das maloclusões, sua gravidade e necessidade de tratamento em adolescentes e adultos jovens entre 15 e 24 anos de idade do interior do Uruguai e sua associação com variáveis demográficas, sociais e de qualidade de vida e alguns indicadores de risco. Métodos: Foi realizado um estudo transversal com dados da "Primeira Pesquisa Nacional de Saúde Bucal da população jovem e adulta uruguaia", realizada entre 2010 e 2011. 438 pessoas foram examinadas. Análise estatística: As diferenças na prevalência de maloclusões entre as categorias de co- variáveis foram testadas com o teste do qui-quadrado para heterogeneidade com linearização de Taylor. Resultados: A prevalência das maloclusões foi de 33,8% no período. Maloclusões mostraram associação com qualidade de vida. Conclusões: Os resultados deste estudo são os primeiros realizados com uma amostra representativa dessa população e, portanto, de interesse para os gestores de saúde pública e outros pesquisadores no campo.

Objectives: Determine the prevalence of malocclusion, their severity and need for treatment in adolescents and young adults between 15 and 24 years of age outside the capital of Uruguay and its association with social demographic and quality of life variables, and risk indicators Methods: A cross sectional study was performed using data from the "First National Survey of Oral Health in young and adult Uruguayan population" conducted between 2010 and 2011. It was examined 438 people. Statistical analysis: The differences in prevalence of malocclusion among categories of covariates were tested with the chi-square test for heterogeneity with Taylor linearization. Results: The prevalence of malocclusion was 33,8 % in the period. Malocclusions showed association with quality of life. Conclusions: The results of this study are the first ones conducted with a representative sample in this population and therefore of interest for public health managers and other researchers in the field.

▼ Dental caries is a widespread global public health problem with significant health and financial implications. The purpose of this study was to determine dental caries prevalence, experience and severity, as well as to investigate disparities by geographic (urban/rural) location, dental health care utilization and oral hygiene practices in seven to eight year old Nova Scotia school children in 2006-2007. The study results demonstrated that over half (57.3%) of the seven to eight year old population was afflicted by dental caries in the overall dentition. Primary dentition caries prevalence was 55%, with a mean deft score of 2.58 ± 0.08 SE. Prevalence of permanent dentition caries was 14%, with a mean DMFT score of 0.26 ± 0.02 SE. The mean defs score was 6.86 ± 0.28 SE and the average DMFS score was 0.47 ± 0.04 SE. Geographic location was not found to have a major influence on dental caries prevalence, experience and severity in this population. However, a significant relationship was observed between dental caries and dental visit frequency, with higher overall caries prevalence and severity among those who visited the dentist less than once per year. Disparities in dental caries were also found by frequency of brushing, with significantly lower caries rates in those who brushed at least twice per day. Health promotion measures such as pit and fissure sealants and topical fluoride application in susceptible and high-risk children, and appropriate health education regarding recommended frequency of brushing twice per day using fluoridated toothpaste, as well as preventive and regular dental visits are merited to help promote children’s overall health and well-being. ▼ Agricultural lands are often interspersed with areas of natural land cover (e.g., forests, wetlands) that may serve as environmental reservoirs of pathogens. However, the potential for natural areas to serve as reservoirs of food-associated pathogens is poorly understood. As well, Listeria species have been isolated from diverse environments, often at considerable prevalence, and are known to persist in food processing facilities. The presence of Listeria spp. has been suggested to be an indicator of L. monocytogenes contamination. In the studies presented here, we investigated the distribution of Listeria monocytogenes, Listeria spp., Salmonella, and Shiga toxin-producing Escherichia coli (STEC) in five study sites representing natural environments across New York State (NYS). Data from an additional study was also used to compare the prevalence and diversity of Listeria spp. between produce production and natural environments. Geographical and meteorological factors that affect the prevalence of L. monocytogenes and Listeria spp. in these environments were also explored. We found that (i) L. monocytogenes is found at considerably higher prevalence than Salmonella and STEC in natural areas of NYS, (ii) the prevalence of L. monocytogenes shows considerable variation associated with season, geospatial, and meteorological factors, and (iii) the effects of proximity to water and pasture lands on L. monocytogenes prevalence differ between locations. Differences in baseline values of L. monocytogenes prevalence among the five study sites suggested that different sets of ecosystem dynamics influence the presence of L. monocytogenes in different study sites. We also found that the prevalence of Listeria spp. was approximately 34% and 33% for samples obtained from produce production (201/588) and natural environments (245/734), respectively. The co-isolation of L. monocytogenes and at least one other species of Listeria from a sample was 9% in produce production environments, compared to 3% in natural environments. Soil moisture and proximity to water and pastures were identified as important factors for detection of Listeria spp. in produce production environments, while elevation, study site and proximity to pastures were identified as important predictors for detection of Listeria spp. in natural environments, as determined by randomForest models. Our data show (i) that Listeria spp. were prevalent in both agricultural and non-agricultural environments and (ii) that geographical and meteorological factors associated with Listeria spp. detection were considerably different between the two environments. Educational modules were also developed, inspired in part by the research presented here. ▼ The human population has experienced tremendous growth over the past 300 hundred years. It took from the start of human history up to the 1800 before the world reached 1 billion in population; in but 210 years another six billion people were added to the population. This exponential growth has been well documented since the time of Thomas Malthus at the turn of the 19^(th) century and continuing with his modern day counterparts in the 1960’s, particularly Paul Ehrlich. Interestingly, a simultaneous population trend emerged in the 1960’s, namely, fertility decline. However, many academics, as well as the public in general, have been largely fixated on high fertility and overpopulation while in reality fertility continues to plummet in most regions of the world. This dissertation addresses the fertility decline reported in 178 countries between 1960 and 2011. I draw on data from the World Bank Data Bank; my research focuses on the role of contraceptive prevalence in facilitating the vast fertility decline observed over this time period. I estimate multilevel quadratic growth curve models to analyze the effect of contraceptive prevalence on total fertility rates among countries. My results document the substantial fertility decline over this time period and indicate that contraceptive prevalence is a significant predictor of fertility decline even after controlling for known correlates of fertility decline such as development, urbanization, economic growth, and declining mortality. This dissertation also includes a discussion of missing observations and the unique complications that missing observations present in a longitudinal framework. Furthermore, I also discuss the logic of multilevel modeling and how multilevel models may be employed in longitudinal research. Finally, I include a discussion of theoretical implications stemming from this dissertation. The focus of these implications is centered on the need for a paradigmatic shift in how we approach the study of fertility in favor of more effective fertility paradigms such as the “Low Fertility Trap Hypothesis.” I argue that a paradigm shift in research orientation is essential as a growing number of countries face the stark reality of population aging, which has occurred as a result of declining and sustained low fertility. ▼ Retrospective cohort study (1997-2002) was conducted to determine the prevalence, incidence and risk factors for Herpes simplex virus type 2 (HSV-2) infections among factory workers at two sites in Ethiopia. Among enrolled participants in a cohort study of HIV incidence and disease progression, 1222 (71.3%) were males and 491 (28.7%) were females. The median age was 35 years for males (range, 19-62) and 33 years (range, 19-46) for females. The serologic status of each stored plasma sample for HIV and syphilis has been routinely determined. Antibody to HSV-2 was detected by using the HerpeSelect TM 2 ELISA IgG, Focus Technologies, USA. The HSV-2 prevalence at enrollment was 41.1%, 57.2% among females and 34.6% among males (p<0.001). Seventeen-percent of females and 9.7% of males younger than 25 years had evidence of HSV-2 infection. Independent risk factors for HSV-2 seropositivity were HIV seropositivity, positive TPPA serology, older age, female sex, and ever being married. The incidence of HSV-2 during follow-up was 1.2/100 person-years (95%CI, 1.0-1.4), 2/100 person-years (95% CI, 1.2-3.2) among females and 0.9/100 personyears (95%CI, 0.7-1.3) among males (p=0.02, assuming Poisson distribution). Positive HIV serology at enrollment was an independent risk factor for HSV-2 seroconversion (adjusted hazard ratio [HR], 2.5; 95% CI, 1.1-5.6). Most of HSV-2 infections were asymptomatic or had minor symptoms, which were not recognized. The study showed high prevalence and incidence of HSV-2 infection. This large number of herpes infected individuals may continue to engage in unprotected sexual activity despite their infection facilitating the sexual transmission of HSV-2 and HIV making the vicious cycle. In the absence of protective vaccine or effective antiviral therapy, prevention of HSV-2 infection will rely on the widespread use of condoms and reduction in the number of sexual partners with emphasis to prevention of HSV-2 infection at early ages. ▼ Background: Hepatitis B Virus (HBV) infection and its sequelae (cirrhosis and liver cancer) are major global health problems. Overall prevalence in Ethiopia varies from 4.7-16.8% for HBsAg and 70-76.4% for at least one marker positive. During recent years evidence has accumulated that in patients with diabetes mellitus, phagocytosis by polymorpho nuclear leukocytes is impaired and disturbances in cell-mediated immune responses can be demonstrated. Cell-mediated immunity is involved in the defense against viruses as well as in that against Mycobacterium and Fungi. However, no increased incidence of viral infections like HBV has yet been reported in patients with diabetes. Objective: To determine the prevalence of HBsAg in patients with diabetes mellitus and to compare those with the non diabetes. It also asses associated factors and liver function tests (LFT); and compare Diabetic and non Diabetic subjects; and HBV positive and negative participants. Methods: The study was a comparative cross sectional study design conducted at Woldiya General Hospital using 108 consented study populations from each study group during the period November, 2010 through January, 2011. A convenience sampling method was used. A total of 216 samples were tested for HBsAg serostatus using VISITECT HBsAg rapid test kit and LFT tests using Humastat 80 chemistry analyzer. Data entered to SPSS-16 and then analyzed using the same soft ware. Multivariate logistic regression was used to see the association of HBV with clinical history of participants and sociodemographic variables. All tests were two-sided with α-level of 0.05 and 80% power. Results: prevalence of HBsAg was equal between diabetes and non diabetes, 3.7% indicating that there was no difference between the two groups. No any Sociodemographic and clinical history of participants were associated with HBV infection (p>0.05) except chronic liver disease. Conclusion: In this study a positive relation was not indicated between HBV and Diabetes and DM in the study area did not predisposed to HBV infection than the rest of the population. ▼ A survey study was conducted on 142 and 60 apparently healthy slaughtered sheep and goats, respectively at an export abattoir, Modjo, Ethiopia from October 2007 to April 2008. The objectives were to determine prevalence and serotype distribution of Salmonella in slaughtered sheep and goats and abattoir environment, investigate the associations between some potential risk factors and Salmonella contamination rates of carcasses and forward strategies to minimize them. A total of 1240 samples consisting of skin swabs, eviscerator’s hand swabs, eviscerating knife swabs, mesenteric lymph nodes, cecal contents, carcass swabs and water samples were collected. The samples were examined for the presence of Salmonella following standard techniques and procedures outlined by the International Organization for Standardization. In a total of 202 animals examined for Salmonella, 18 (8.9%) were positive of which 11 (7.7%) were sheep and 7 (11.7%) were goats. In a total of 1240 different samples, Salmonella was isolated in 89 (7.2%) samples of which 25 (12.4%) were carcass swabs, 11 (5.5%) mesenteric lymph nodes, 8 (4.0%) cecal contents, 10 (5.0%) skin swabs, 18 (8.9%) eviscerator’s hand swabs, 15 (7.4%) eviscerating knife swabs and 2 (7.1%) water samples. Salmonellae were detected in all test samples obtained from sheep and goats. Although the associations of carcass contamination with potential risk factors were assessed, no statistically significant results were observed except with eviscerating knife swab, which was found to be significantly associated with carcass contamination. Sheep and goat carcasses that were eviscerated using Salmonella positive knives were 4.175 times more likely to be contaminated with Salmonella compared to those that were eviscerated with Salmonella negative knives. The 89 isolates of Salmonella recovered were composed of 12 different serotypes. Salmonella Infantis (16.9%), S. Typhimurium (15.7%) and S. Heidelberg (13.5%) were prevalent in test samples. Other serotypes isolated included S. Reading (10.1%), S. Braenderup and S. Enteritidis (9.0% each), S. Butantan (6.7%), S. Anatum (5.6%), S. Newport and S. Poona (4.5% each), S. Give (3.4%) and S. Hadar (1.1%). Results of the present study showed that Salmonella are widespread in slaughtered sheep and goats, eviscerator’s hands and the abattoir environment. Furthermore, eviscerating knife was the main source of carcass contamination. Therefore, proper sterilization of knives at 82 °C should be implemented at abattoirs in order to reduce contamination of the meat and other edible offals during slaughtering operations. ▼ A cross sectional study was conducted from January 2015 to May 2015 in and around kombolcha town, Eastern Amhara, to isolate and identify methicilin resistant S. aureus and their resistance to different antimicrobials and also identify risk factors associated with the occurrence of dairy cow mastitis. A total of 150 dairy cows were included during the study period. A total of 600 quarters were examined to detect clinical and subclinical mastitis by physical examinations of udder and milk and California mastitis Test, respectively. The prevalence of mastitis was 56%. Out of this, 10% and 46% were clinical and subclinical respectively. Among potential risk factors considered from the farm attributes, age, milking hygiene, parity, and floor system had significant (p=<0.05) effect on the prevalence of mastitis. However, breed, previous treatment and lactation period have not been a significant potential risk factor on the occurrence of mastitis. Staphylococcus aureus was isolated at a rate of 11 (73.3%) and 29 (42%) in clinical and subclinical mastitis, respectively. The result showed that the occurrence of Staphylococcus aureus in clinical mastitis were found to be significantly higher than subclinical mastitis (p=0.028). The present study showed that S. aureus was resistant to penicillin G (100%), Amoxicillin (100%), cefoxtin (42.7%) and tetracycline (77.4%) however all the isolates were found to be totally (100%) susceptible to the gentamycin disc. Out of the resistance S. aureus isolates, 24 (45.3%) were found to be multidrug resistant against 4 antibiotic discs primarily to penicillin G, ampicillin, cefoxtin, and erythromycin. The development of antimicrobial resistance is nearly always as a result of repeated therapeutic and/or indiscriminate use of them. Regular antimicrobial sensitivity testing helps to select effective antibiotics and to reduce the problems of drug resistance development towards commonly used antibiotics. In conclusion, among different risk factors, age of the animal, milking hygiene, parity and floor type were critical for the occurrence of mastitis. Mastitis caused by S. aureus is one of the major problems of dairy cows in milk production in the study area. Advisors/Committee Members: Dr. Asmelash Tassew (advisor).

▼ Background: Chronic abdominal pain is one of the most common presenting complaints to primary care providers and pediatricians. Chronic abdominal pain in children is generally believed to be functional, and its treatment cause can be a challenge. Studies in developed countries have shown that constipation is one of the most common diagnoses in children presenting with chronic abdominal pain. Constipation and its associated symptoms adversely affect the quality of life of the child and his or her family if left untreated. There is paucity of data on the prevalence of constipation in children with recurring abdominal pain in our region. Objective: To determine the prevalence of constipation in 4-13 year-old children referred to gastroenterology clinic with chronic abdominal pain and to describe the pharmacological and non-pharmacological management of children with constipation at Kenyatta National Hospital. Methods: A cross-sectional hospital based study was carried out on children aged 4 to 13 years referred to the paediatric gastroenterology clinic at Kenyatta National Hospital between Julyto December 2014. A questionnaire was administered to children with chronic abdominal pain who fulfilled Apley’s criteria and constipation was determined using Rome III criteria. SPSS version 19.0 was usedfor analyzing the data. Descriptive analysis was used to show the frequency and proportion of variables. Results: A total 84 children with chronic abdominal pain were seen, 47 (55.95%) were girls and the median age was nine years. The prevalence of constipation in children with chronic abdominal pain with two or more Rome III criteria was 69 out of 84 (82%, 95%CI: 72%95%), out of whom 37(53.62%) were females and 32(46.3%) were males. ix The difference between the proportion of boys and girls was not statistically significant. Pharmacological management was more commonly used than non-pharmacological management. The medication given to children with constipation include: lactulose syrup with milk of magnesium, dietary advice, water intake and behavioral change which was given to 29(42.0%) of the children, lactulose/ dietary advice/water which was given to 18(26.1%) of the children, while dis-impaction/water intake/ dietary advice was given to 6(8.7%), while lactulose alone was given to 16(23.2%) of the children. Conclusion The prevalence of constipation in children with chronic abdominal pain is 82%. Pharmacological management was more commonly used than the non-pharmacological and the commonest drug prescribed was lactulose which was used for treating 91.3% of children with constipation. Recommandations: Constipation to beconsidered as an important aetiologyin children presenting with chronic abdominal pain.

Over the years more specialists have been trained and as a result more transplants are done each year and with the improved post transplant care and immunosuppressant therapy, these patients have longer survival rates. As a consequence of this, a resurgence of latent forms of viral infections occurs, in this regard human papillomavirus, leading to an upsurge of anogenital malignancies such as cervical cancer, vulval cancer and anal cancer. The risk of cervical cancer increases by 14 fold in renal transplant recipients yet this can be prevented with more vigilant cervical cancer screening. Early detection of precancerous lesions can lead to timely treatment and reduce the burden of disease that may occur as a result of multiple illnesses. Owing to the multiplicity of complications associated with renal transplant, cervical cancer screening may take a back seat in this population. This study will determine the prevalence of HPV and cervical cytological abnormalities in the renal transplantation population in a bid to emphasize the need for yearly cervical cancer screening. Broad Objective The main objective of this study was to determine the prevalence of cervical cytological abnormalities and HPV infection in renal transplant recipients at KNH. Methods This was a cross-sectional study to determine the prevalence of cervical cytological abnormalities and HPV infection among renal transplant recipients. Data collection was done by the principal investigator and individuals were identified through consecutive sampling. A consent form was filled followed by administration of a structured questionnaire to assess the socio-demographic, reproductive and clinical history of participants. Thereafter, a pap smear and liquid based cytology sample for collection of HPV was done. Data was analysed using SPSS version 22, t-test and chi test for the continuous and categorical variables, Fischer‟s test and chi square was used for associations. xv Results Thirty two female renal recipients who met the eligibility criteria were enrolled into the study. There median age was 38.5 years (range 24-64 years). The mean duration since transplant was 4.89 years (SD 5.15). The prevalence of hrHPV infection was 33% (10/32) whereas the prevalence of abnormal cervical cytology was 12.5% (4/32)). The marital status, parity and lifetime number of partners were found to be statistically significant Conclusion The prevalence of premalignant cervical lesions and hrHPV infection was found to be high among female renal transplant recipients attending the post transplant clinic at KNH. Though few, these findings raise awareness of the magnitude of this problem among this population. The use of immunosuppressive therapy is also inevitable, however health awareness measures aimed at preventing the acquisition of HPV and other STIs as well as reduction in the number of sexual partners will greatly impact on the occurrence of HPV and subsequent premalignant cervical lesions in this population.…

▼ Fascioliasis (liver fluke) is listed as one of the most important parasitic infections in humans and animals in the developing world, posing a considerable burden to human public and veterinary health globally (Mas-Coma, 2005). In Vietnam, the prevalence of fascioliasis has increased rapidly since the beginning of the 21st century and it has become a zoonosis of great public health importance, with Central Vietnam being most severely hit by the disease (Trieu, 2011). The rising prevalence of the disease has been reported in all 15 regional provinces of Vietnam, increasing from 1,500 cases during 2004-2005 to 9,985 cases in 2011 (Nguyen, 2011). To date, comprehensive strategies for fascioliasis control are not in place, and the only control of the disease is reliance on chemotherapy with triclabendazole (TCZ), recommended by the WHO as the drug of choice for the treatment of both acute and chronic fascioliasis in humans (WHO, 2007b). However, TCZ resistance has been reported in animals and in humans (Ortiz, Scarcella, Cerna, Rosales, Cabrera, Guzman, Lamenza and Solana, 2013). Although alternative fascioliasis control measures are recommended elsewhere, no typical example has been proven to be effective in practice (Nithiuthai, Anantaphruti, Waikagul and Gajadhar, 2004; Saba and Korkmaz, 2005). Furthermore, various models of foodborne trematode infection control have been introduced and tested for their effectiveness but none of them are complete and applicable to all situations (Nguyen, Nguyen, Bui and Tran, 2011). In the context of the unavailability of effective control measures of fascioliasis, the study of a broadly-based control model of fascioliasis in Central Vietnam was conducted to address the burden of the disease and promote the community’s health. The model, selectively adapted from the trial control model of fascioliasis by Nguyen et al. (2011) and the intervention model by Molyneux (2006), comprised five main components: vector control, health education, improvement of local health systems, involvement of concerned bodies, and chemotherapy. This study aims to implement and evaluate a broadly-based control model for fascioliasis in a community with high prevalence of fascioliasis in Central Vietnam (1) to determine the seroprevalence of human fascioliasis by laboratory methods prior to the interventions; (2) to describe the risk factors associated with fascioliasis infections by conducting surveys on knowledge, attitudes and practices (KAP), and undertaking household observations and snail counts;…

▼ Obesity is a multifaceted condition that is caused by a combination of environmental, medical and psychological factors. It is one of today’s noticeable yet neglected public health problem with serious health consequences such as hypertension, type 2 diabetes and cardiovascular diseases that affect individuals of all ages globally. Adolescents are particularly prone to obesity owing to their reduction in physical activity, as well as to the greater accessibility of foods void of nutritional value. Additionally, adolescents’ susceptible nature and changing bodies also contribute to their increase weights. The life course perspective postulates that, excessive body weight can persist from adolescence to adulthood, and increase the risk of NCDs. Among the adolescents, the school environment is a crucial setting for the development of and, or engagement in unhealthy dietary and physical activity behaviour. The primary objective of this study was to determine the prevalence and risk factors of obesity among Senior High School Students in the Adansi North District in the Ashanti Region. Also, the study sought to assess the physical activity levels and the food consumption pattern of the students, as well as establishing the relationship between the independent variables and obesity. A descriptive cross- sectional survey was conducted among 306 adolescents aged 12- 19 years. The adolescents were recruited from the five senior high schools in the district using stratified random sampling technique. Data was collected by the use of structured questionnaire and anthropometric data sheet to calculate for BMI levels. STATA version 11.1, and Microsoft Excel was used to analyze the data collected. Results from the chi-square test indicated that, there was a significant relationship between leisure activities and obesity such as playing computer games (x2=7.5086, df=9, p<0.05), and TV watching (x2=6.3576, df=2, p<0.05). There was no significant relationship between playing and assisting in household chores and obesity (x2=9.5706, df=7, p>0.05), (x2=7.2145, df=2, p>0.05) respectively. There was also a significant relationship between food consumption pattern and obesity (x2= 21.6181, df= 9, p< 0.05), a significant relationship was seen between mode of transportation and obesity (x2=30.6244, df=15, p<0.05) and there was a significant relationship between income level of parents and obesity (x2=8.3189, df=6, p<0.05). Overall prevalence of obesity among the adolescents was found to be 47.06%. Factors that were associated with the development of obesity were; physical inactivity levels and consumption of foods that were dense in fats and carbohydrates. Even though majority 62.75% of the respondents ate three times in a day, their meals were mainly energy dense foods and fats. PURPOSE: To investigate the prevalence, incidence, and bilateral occurrence of impacted teeth in Koreans. MATERIALS & METHODS: From January 2010 to December 2015, we retrospectively reviewed the Korean patients who visited the Department of Orthodontics, Dental Hospital, Ajou University Hospital. The patient's initial electronic chart and panoramic radiographs were used to evaluate the diagnosis of the impacted tooth, the eruption pattern of the opposit impacted tooth of the impacted tooth On panoramic radiographs, only complete impaction of the permanent tooth was included, and impaction of the deciduous tooth, supernumerary tooth, third molar and systemic disease, if the permanent tooth is not erupted due to the retention of the deciduous tooth, but the eruption of the permanent tooth is expected when the deciduous tooth is extracted were excluded from the study. RESULTS: The prevalence rate was 16.7% and there was no statistically significant difference between to gender(p=0.745). The frequency of occurrence by age was the highest 57.7% under 12 years with statistically significant difference(p=0.001). The average number of impacted tooth per patient was 1.35 ± 0.68 and the minimum was 1, the maximum was 6. Two hundred seventy two impacted teeth were found in maxilla, and 122 were in mandible. The incidence in maxilla was the order of canine, second premolar, central incisor, In mandible, the incidence as the order of second molar, second premolar, and first molar(p=0.001). The frequency of occurrence of left and right impacted tooth was 189 on the left side and 205 on the right side, and the frequency was similar in the order of second premolar, second molar, and canine(p=0.899). The unilateral impaction frequency was 236 80.5%. The eruption pattern of the opposit impacted tooth of the impacted tooth were the most frequent in type I 63.7%. CONCLUSION: This study investigated the incidence and occurrence of impacted teeth. As a result of eruption pattern of the opposit impacted tooth of the opposed teeth, 32.2% of the impacted tooth occurred in both right and left sides, and 2.3% in the case of the left and right teeth missing.

목적: 한국인에서 제3대구치 이외 치아의 매복치 유병률, 발생빈도 및 양측성 발생 여부를 연구한다. 대상 및 방법: 2010년 1월부터 2015년 12월까지 아주대학교 치과병원 치과교정과에 내원한 한국인 환자를 대상으로 하였다. 환자의 초진 전자차트기록과 파노라마 방사선사진을 통해 매복치의 진단, 매복치의 반대 측 동명치아에 대한 맹출 정도 및 맹출 경로를 평가하였다. 파노라마 방사선사진 상에서 영구치의 완전 매복만을 포함시켰으며 유치, 과잉치, 제3대구치, 전신질환자, 유치의 잔존으로 영구치가 맹출 되지 못하고 있으나 유치 발치 시 영구치의 정상 맹출이 예상되는 경우는 연구대상에서 제외하였다. 결과: 매복치 보유 비율은 남자가 822명 중 140명, 여자가 930명 중 153명으로 16.7%의 유병률을 보였으며 성별에 따른 발병률은 통계적으로 유의한 차이를 나타내지 않았다(p=0.745). 연령별 발생 빈도는 12세 이하가 169명 57.7%로 가장 많았으며 통계적으로 유의한 차이를 나타내었다(p=0.001). 1인당 매복치 보유수는 1개가 211명 72.0%로 가장 많았다. 환자 1인의 평균 보유 매복치 수는 평균 1.35±0.68개, 최소 개수는 1, 최대 개수는 6이었다. 상하악 매복치 빈도는 상악이 272개, 하악이 122개로 상악이 높은 빈도를 보였으며 상악은 견치, 제2소구치, 중절치 순으로, 하악은 제2대구치, 제2소구치, 제1대구치 순으로 높은 매복치 발생 빈도를 보였다(p=0.001). 좌우측 매복치 빈도는 좌측이 189개, 우측이 205개로 비슷한 빈도를 보였으며 좌우측 둘 다 제2소구치, 제2대구치, 견치 순으로 높았다(p=0.899). 매복치 양측성 및 편측성 발생 빈도는 236개 80.5%로 편측이 더 높았다. 매복치의 반대 측 동명치아에 대한 맹출 정도 및…
