▼ Terrorism and atrocities have scarred the public memory in the late twentieth and early twenty-first centuries. Three atrocities, the 1995 Oklahoma City bombing, the 1996 massacre at Port Arthur Historic Site in Tasmania, Australia, and the 2002 Bali bombings, had a significant impact on the communities they most affected. How did the differing governments and communities at these sites respond to the sudden loss of life? How were the competing agendas of these groups managed ? Are there shared and distinctive characteristics in the memorialisation of atrocitites across these countries at the turn of the millenium? In responding to these questions, this study analyses cultural differences in memorialisation at contemporary atrocity sites. It examines the differing responses at the case study sites to the planning and the timing of memorials, the engagement of those affected, the memorial designs and the management of the memorials, including tributes. It is an original comparative study of contemporary memorialisation by a heritage professional directly involved in the management of memorials at contemporary atrocity sites. The original research includes the identification of the role the internet in contemporary memorialisation, an in-depth analysis of the memorialisation of the 1996 massacre at Port Arthur Historic Site, and the memorialisation in Bali and across Australia of the 2002 Bali bombings. It extends the current scholarship on the memorialisation of the Oklahoma City bombing through identifying the impact of the internet in the memorialisation and in the timeframe of the analysis through to the 15th anniversary in 2010. The comparative analysis of the management of tributes at all the sites identified issues not previously considered in Australian scholarship: that tributes and the response to them is part of the memorialisation and management of contemporary atrocity sites. A combined research method based on an interpretive social science approach was adopted. A range of methodogies were used, including literature reviews, analysis of electronic material, site visits, unstructured in-depth interviews, and participant-observation at memorial services. Studies on history, memory and memorialisation provided the framework for my analysis and led to an original proposal, that all three sites have shared histories of the memorialisation of war and ‘missing’ memorialisation. These shared histories, I argue, strengthened the justification for this comparative study. This comparative study identified differences across the case study countries in the designs of the built memorials, in legislation enacted after the atrocities, the responses to the perpetrators, the marking of anniversaries, and in the management of tributes left at the sites. ▼ The World Health Organisation (WHO) declared tuberculosis (TB) a global emergency, and this infectious disease remains a health threat by being the leading cause of death amongst adults (Naidoo, Dick & Cooper, 2008:55). In 2005, South Africa was ranked seventh in the world for having the highest TB rate and the lowest TB success rate in the world. As a professional nurse involved in the tuberculosis programme at a clinic in the Nelson Mandela Bay Municipality at local government level for approximately three years, the researcher observed that the morale of professional nurses who provide TB services appeared to be low. They also appeared to be frustrated because they feel that they are not winning the battle with regard to the TB epidemic in their communities irrespective of their efforts to try and curb the spread of the disease. The extent of the workload per person also appeared to add to the low morale and frustration of the professional nurses rendering TB health services because they feel that they are unable to manage everything. The objectives of the study were therefore to explore and describe how professional nurses experienced the management of health services being rendered to TB patients in Sub district B of the Nelson Mandela Bay Municipality (NMBM) in order to make recommendations that could be used by the district manager to address the research findings. The research study was based on a qualitative, explorative, descriptive and contextual research design. The research population consisted of all professional nurses who worked in the TB services of Sub district B. Non-probability, purposive sampling was used to select the participants for the study. Seven in-depth and three follow-up interviews were conducted before data saturation was achieved. The data gathered during the interview process by the researcher were transcribed and coded by an independent coder using Tech’s model for data analysis. Ethical considerations were adhered to throughout the research study. The aspect of trustworthiness according to Guba’s model was implemented in the research study and included credibility, applicability, consistency and neutrality. iii One theme, two sub themes and categories were identified relating to the diverse experiences expressed by the participants relating to the management of health services being rendered to TB patients. The experiences expressed by the professional nurses included both negative and positive experiences. The negative experiences expressed by the participants were for example, a lack of resources as hampering adequate service delivery, a concern regarding the number of staff contracting TB due to a lack of infection control measures, a difference in conditions of service between the two local authorities and the DOTS supporters as being a threat to patient confidentiality. The positive experiences expressed by the participants included experiences relating to job satisfaction in rendering TB health services, the DOTS supporters as being supportive to the staff, the TB meetings…

▼ After 1994 with the inception of the South African Democratic Government, the health care delivery system was one of the areas that had to be reviewed. The vehicle for a comprehensive health care system was based on primary health care which encouraged people and patients to take responsibility for their health by being involved in all aspects of their care. For this purpose the Batho Pele principles were introduced, a concept which is informed by 8 principles, namely: consultation, service standards, access, courtesy, information, openness and transparency, redress and value for money. These principles are meant to restore the dignity and the rights of patients which are paramount in the Constitution of South Africa. (Constitution of South Africa 108 of 1996 Chapter 2) The nurses claim that since the introduction of the Batho Pele Principles, patients and their families have been “impossible” towards nurses, making unnecessary and sometimes impossible demands. The nurses also experience patients and their families as being informed of their “rights” but not of their responsibilities as patients. They were also unaware of the rights of the nurses. The objectives of the study were:- To explore and describe the experiences of professional nurses working in the outpatient departments of the introduction of the Batho Pele principles in state hospitals, and to Recommend guidelines that will enhance better understanding and implementation of the Batho Pele principles by the professional nurses. The study is founded on a qualitative research paradigm based on explorative, descriptive and contextual framework. The data was collected from focus groups from each hospital of the Port Elizabeth Hospital Complex. Each group consisted of four to six willing participants all of them were professional nurses who have worked at least five or more years in the outpatient department of the Port Elizabeth Hospital Complex. Data was collected via semi-structured audio-taped interviews together with the researcher’s field notes. Data analysis was done using Tesch’s data analysis spiral. The assistance of independent coder reinforced the truth value of the findings. Themes and subthemes emerged from the data that was collected and revealed that the professional nurses experienced that the Batho Pele Principles as a good policy, but that it was difficulty to uphold due to inadequate planning of health services prior to implementation of the Batho Pele Principles. They experience not getting from their management and they also experience that there was lack of discipline in their institutions. Based on these findings, guidelines that will recommend better implementation of the Batho Pele Principles by nurses were compiled by the researcher and future research in this regard was recommended Advisors/Committee Members: James, S Dr.

▼ Myanmar, formerly known as Burma, is a country in Southeast Asia with a population close to 60 million people, which suffers from underdeveloped infrastructure. With the change of government and restoration of democracy starting from 2011, policy reforms are anticipated leading to large-scale economic development and growth. This growth is expected to result in rapidly increasing trade volumes. Myanmar’s maritime infrastructure needs upgrades: existing ports are mainly up-river, have limited draught and need continuous dredging. Currently, six deep sea port projects are being planned, but it is not known whether these sites have been selected based on rational, commercial and technical considerations or on (geo)political interests from the past. If these sites have been selected based on outdated political considerations, rather than commercial, well-grounded and sustainable development, they may struggle to obtain finances and support. Especially in the past, port sites were often allocated based on (geo)political considerations. However, because site selection is critical to port development and the long-term success and growth of a port, research into optimum and sustainable site selection is of significant value. The research objective is development of a strategy for deep sea port site selection and port development in Myanmar, by developing, validating and applying a framework for sustainable site selection, integrating a stakeholder-inclusive approach and ecosystem services. This framework is developed based on literature and desk study, and a comprehensive case study conducted in Myanmar. The strategy elaborates upon the optimum site selection (process), conceptual lay-outs and biggest challenges. Following the developed two-step site selection process, a short list with the sites Pathein, Yangon, Mawlamyine and Dawei was obtained. By means of a Multi-Actor Multi-Criteria Analysis and a cluster-based ranking, Mawlamyine and Yangon ended first. Yangon seems like a logical choice for deep sea port development, as it is the economic center of the country with the best hinterland connectivity. Despite pressure from stakeholders, it is concluded that Yangon is not a suitable location for deep sea port development because of physical restraints. However, it is a crucial port and must be part of the strategy. Yangon and Thilawa port should maintain and strengthen their current activities, and this appears best in combination with a deep sea port near Kalegauk Island (near Mawlamyine), which is connected with a high-quality road connection to Thailand and the Greater Mekong Subregion. Besides, this site offers many opportunities with respect to Ecosystem Services, by using Ecosystem-Based design principles. This port will accommodate the large Post-Panamax vessels and Yangon and its hinterland will be connected to it by sea, road and rail. Yangon port functions as a feeder port or extended gateway in this combination. This combination of ports can for instance be found in Thailand, Cambodia and Vietnam. ▼ The research investigates the devolution of decision-making at the Nelson Mandela Bay Municipality (NMBM). The research sets out to investigate the extent to which the Municipality offers resources to its politically elected representatives and whether the support enables a devolution that includes local citizens in decision-making. The study was demarcated within the NMBM, using an empirical survey, a population group composed of councillors and officials who work very closely with politicians in mobilizing the community in public participation. The data collection tool of choice for the study was a selfadministered questionnaire, supplemented by document analysis, unstructured interviews and the researcher’s observations. The purpose of the study is to investigate reasons why devolution is not having the desired results at NMBM, Investigate and find alternatives to how devolved decision-making can be enhanced, using the viable local political process at the NMBM and similar municipalities. The study argues from a theoretical base of good governance and public participation. The findings made were inter alia: Politicians in public office are not using the resources at their disposal optimally; tussles amongst politicians and with officials do have a negative effect on devolved decision-making; and vandalism and closing down of ward council offices are not purely a reflection of a ward councillor’s failure to account to the constituency, and the ward committee system at NMBM has not since 2009. The recommendation is that deliberate efforts need to be made to establish democratically elected ward committees in all sixty wards of NMBM. The establishment of ward committees has to be a joint effort between political office bearers, the broader political landscape and officials as previous efforts have failed to produce legitimate structures. ▼ Modern problems are complex and are characterized by the existence of multiple actors, multiple perspectives, conflicting interest as well as key uncertainties. This dissertation aims to explore the operational challenges of marine services in the port of Durban. The researcher was interested in exploring marine operational challenges and their impact on the maritime industry as well as stakeholder benefits from the operation of marine services. The purpose of employing systems thinking was to recognize relationships and interconnectedness between elements at play in the marine services operations. Understanding behavior of marine services systems was critical for gaining insight into the behaviors and patterns underlying these complex systems. The research questions related to the operational challenges being experienced within the port of Durban; the stakeholders who are impacted by these challenges; and the recommendations that can be made to mitigate the impact of these challenges. The research objectives aimed to explore the dynamics of the marine services’ system. Furthermore, the research aimed to look at how the current levels of offering can be improved within the port of Durban. The research problem statement presented the level of connection between the South African economy and the port of Durban, acknowledging that the port of Durban is the gateway to international markets. Marine systems are complex and dynamic requiring the system thinking approach to understand the structure of the problem situation. A conceptual model of a dynamic marine service system was developed by employing a qualitative paradigm. Data was collected via interviews and observations from purposive marine employees who possessed knowledge and expertise of the global maritime industry. Using a thematic analysis the findings indicated that investment in infrastructure is the main marine services operational challenge confronted by the department. The volume of cargo triggers ship evolution where ship builders begin to build bigger size vessels to cater for volume throughput in international ports. The ship size evolution leads to the gap in the shipping industry and port infrastructure which require huge investments to close the gap. Advisors/Committee Members: Green, Paul. (advisor).

▼ In South Africa, suicidal behaviour in the younger generations has become rapidly part of a major public health problem. It is a well-known fact that adolescence is a critical period of progression in a teenager’s development which makes them more susceptible and vulnerable to suicide. Statistics have shown that there has been an increase in suicide among adolescents within the last decade (Schlebusch, 2005). The main aim of this research study is to explore the perceptions of adolescents regarding the factors that contribute to adolescent suicide. This study reviews literature concerning various risks factors for adolescent suicidality. It also described major social and psychological theories of suicide and the extent to which these studies support the importance to adolescent suicide. In the literature review theories of Bronfenbrenner’s Ecological Systems Theory, Durkheim’s sociological theory of suicide, Freud’s psychological perspective and Baumeister’s Escape Theory were discussed. A qualitative research design will be used by using visual participatory methodology techniques namely photo voice and reflective writing. A sample of 12 grade 11 learners, male and female, was purposely selected to participate in the research study. The focus of the study is to investigate the risk factors underlying adolescent suicidal behaviour in schools in the Northern Areas of Port Elizabeth, as well as to explore the stressors experienced by adolescents that could contribute to them having a higher suicide risk. Another focus point will be to look at prevention strategies as an antipode for suicidal behaviour. The main findings that emerged from the data analysis include contributing factors namely: socio-economic factors, personal factors, psychological factors and educational and school-related factors. The main prevention strategy themes emerged from findings derived from the data and include: parental involvement, religion, professional help and support groups and confiding in someone that you trust.

▼ Purpose of this treatise- This Paper aims to investigate the Sustainability of Health Facility Committees in the Nelson Mandela Bay Health District. Design/methodology/approach – This study consists of a literature review and a pilot study. Qualitative research approach was used in order to obtain descriptive data from the targeted group. The primary sources of data collection the researcher used were from the members of the committee, health facility manager, chairperson and the health promoter who are members of the health committees. Focus group discussions with health committees were conducted to provide rich in-depth data. Literature and journal articles were also used to provide secondary data to corroborate findings. Research limitations- A major limitation to this study is that due to the nature of the nature of the research report it was not possible to assess the sustainability of health Facility Committees from other areas in the Nelson Mandela Bay Health District. Findings-The study has revealed that Community Health committees are sustained by the commitment and passion members have for the work done in the facilities and health committees. The study also revealed that social cohesion plays a major part in the sustainability of Community Health Committees (CHC). Lack of involvement by ward councilors, support from the Health Department, uncertainty of responsibilities by the health committees and limited skills were indicated as major setbacks threatening the sustainability of Community Health Committees. It can thus be concluded that these limitations must be properly addressed in order to enable and uphold the sustainability of Community Health Committees. Original/value -So far, there has been limited research which has been undertaken with regards to the subject of Sustainability of Health Facility Committees in Nelson Mandela Bay Health District. This study will aid in enabling a better understanding of what sustains Community Health Committees and the Challenges facing such communities in order to enable individuals and the parties involved to better formulate solutions to overcome these challenges in Nelson Mandela Bay.

▼ The business environment and the economy of the society today are continuously witnessing the impact of globalisation. Telecommunication advancement and transportation has greatly made the impact of globalisation to be felt. Globalisation though argued to have its merits and its demerits have resulted in fierce competition amongst businesses. The banking industry is not isolated from these competitions. Any business wishing to survive and stay profitable in the recent world must be able to compete in the global economy. Hence, for any bank to sustain itself and remain profitable it must be able to withstand the competition in the environment it operates. To be able to compete in the banking industry means delivering better services to customers than competitors. This study presents how the South African banks can satisfy their customers and remain profitable in the face of competition. The South African banking industry consists of local and foreign banks and these banks compete for customers in the environment in which they are located. Increase in different branches by different banks has made the competition even fierce. In other to suggest recommendations for the South African banks to satisfy their customers and become profitable, the researcher identified some problem areas that need to be improved upon. These include customer loyalty, relationship banking and electronic banking. These variables were researched to understand how they affect the banks and the customers and where tested using both a qualitative and a quantitative analysis to ascertain if the customers were satisfied with the banks‟ approach of managing these variables. The results indicated that the customers were satisfied to a lesser extent. Any bank wishing to satisfy its customers and remain profitable in other to compete in the industry needs to satisfy the customer to a large extent. Therefore, recommendations where suggested based on the empirical results to help improve the banking industry‟s ways of attracting and retaining customers. ▼ Globalization of the world’s economy, containerization, intermodalism and specialization have reshaped transport systems and the industries that are considered crucial for the international distribution of goods such as the port industry. Simultaneously, economies of location, economies of scope, economies of scale, optimization of production factors, and clustering of industries have triggered port regionalization and inland integration of port services especially those provided by container terminals. In this integration dry ports have emerged as a vital intermodal platform for the effective and efficient distribution of containerized cargo. Dry ports have enabled port and hinterland expansion increasing the competitiveness of container terminals at seaports. In consequence, container terminals and dry ports are establishing formal and informal relationships to strengthen the competitiveness of their hinterlands and to improve their role in the physical distribution of goods. This study assesses the characteristics of relationships between container terminals and dry ports. Such assessment is conducted based on a set of relationship characteristics proposed in a relationship assessment model for customer/supplier, in which dry ports are given the role of suppliers of port services to container terminals. In addition, the research assesses the impact of the relationships between container terminals and dry ports on inter-port competition. The main findings of the research led to conclude relationships between container terminals and dry ports are characterized by medium mutuality, low particularity, low co-operation, low conflict, low intensity, low interpersonal inconsistency, high power/dependence and medium trust. Additionally, it was concluded that such relationship characteristics impact inter-port competition in two main ways. In one hand by driving container terminals to maximize the utilization of dry port’s capabilities such as container transport/delivery, container storage, customs clearance, information systems and intermodal connections to industrial clusters. On the other hand, by constructing channels of interaction through which dry port’s benefits for hinterlands such as increase of container terminal capacity, reduction of road congestion, increase of modal shift and hinterland expansion are used as leverage in competition for containerized cargo.

▼ This thesis investigates the challenges facing developmental local government in South Africa, using the Nelson Mandela Metropolitan Municipality as a reference. The thesis comprises eight chapters. The study is based on the assumption that the Nelson Mandela Metropolitan Municipality, like other municipalities in South Africa, is confronted by numerous challenges in implementing its constitutional developmental mandate conferred on it by the Constitution of the Republic of South Africa Act 108 of 1996. It is vital that local government understands and contextualises these challenges, so that appropriate interventions may be developed. The widespread recent service delivery protests which, in many instances, have turned violent, have sounded an alarm that cannot be ignored. If local government is already struggling to fulfill its traditional mandate of service delivery, then it would find it difficult to spearhead social and economic transformation and development. This study proposes to provide a brief historical background on the evolution and transformation of local government in South Africa. Issues such as the new status and developmental mandate of local government, the extent to which local government has succeeded in complying with its developmental mandate, as well as the challenges it has encountered along the way, will be addressed. Recommendations are presented on how the existing status quo can be changed to enhance service delivery and development and enable low government to fulfil its developmental role more efficient and effectively, with particular reference to the Nelson Mandela Metropolitan Municipality. The empirical survey and research methodology employed in the study is described. This is followed by the operationalisation of the survey questionnaire used for gathering the data needed for analysis. The research findings of the empirical survey are then statistically analysed and reported. The concept of models is introduced, and selected models are explained. This is followed by an explanation of the proposed normative model for monitoring and evaluating service delivery and development in the Nelson Mandela Metropolitan Municipality, for possible replication in other South African municipalities. Various recommendations flowing from the results of the empirical study, namely the responses made by the respondents during the empirical survey, are proposed in the final chapter. ▼ The South African Department of Health, like the health departments of many other countries, has reviewed its policies to focus on the delivery of comprehensive Primary Health Care (PHC). The South African health care sector is undergoing major restructuring in an attempt to address the inadequacies resulting from the fragmentation and duplication of health services in apartheid South Africa. Following this restructuring, the decentralisation to health services has been adopted as the model for both the governance and management of health issues (Department of Health, 2002:7). Before 1994, local government health departments were rendering certain primary health care services in terms of the Health Act 63 of 1977. Post 1994, the Constitution of the Republic of South Africa 1996 (Act 108 of 1996) classified primary health care as a provincial function. Based on this classification, primary health care services in South Africa are now being provided by two authorities, namely local government and provincial government, in the same community. Thus, in the Nelson Mandela Bay Municipality District, primary health care services are rendered by two authorities, namely the Nelson Mandela Bay Municipality and the Nelson Mandela Health District of the Eastern Cape Department of Health. These authorities are targeting the same community, with the same PHC package, with different sets of conditions of service, salary structures, infrastructure, accountability and authority. Such differences are believed to have impeded functional integration, depleted human resource capacity in rendering an effective and efficient PHC system and resulted in inefficient budget spending by both authorities. The problems of location, duplication and fragmentation of primary health care provision in the Nelson Mandela Bay Municipality District are not conducive to optimal service rendering. This will be resolved only once a unified, single integrated health service has been established. This study was undertaken to explore and describe the views of health practitioners with respect to the location of primary health care within the Nelson Mandela Bay Municipality District. The research design of this study was a quantitative, explorative, descriptive survey. Healthcare practitioners, like management, doctors and nurses, were asked to respond to a structured questionnaire. The findings of the study indicate that while health practitioners may hold diverse views on where primary health care should be located, they agree that a unified, single PHC authority would be desirable. Although primary health care is a combination of task-orientated basic health services and the process of community development, it is important that the authority of choice should ensure the highest possible quality through an integrated process, taking into account local needs. ▼ Cardiovascular disease contributes to mortality and morbidity statistics worldwide and in South Africa. The current focus in health care revolves around activities aimed at preventing the development of cardiovascular disease, rather than the treatment of disease. The identification of risk factors that can predispose a patient to the development of cardiovascular disease is an essential component of any cardiovascular disease management programme. It is necessary that in the management of these risk factors, they are not considered to be isolated, but inter-related. Through the provision of point-of-care cardiovascular risk screening and monitoring services as well as disease-related counselling, the community pharmacist, as a readily accessible source of healthcare, can play an essential role in the cardiovascular disease management process. The aim of this study was to describe the nature of the services provided by community pharmacists with respect to cardiovascular risk and disease management in the Nelson Mandela Metropole. The research design was a non-experimental, descriptive study using a crosssectional survey method. Data was obtained through the utilisation of a questionnaire. The questionnaire consisted of three sections and was administered to community pharmacies in the Nelson Mandela Metropole, that provided cardiovascular point-of-care screening services. The community pharmacists correctly identified cardiovascular risk factors such as obesity (76.6 percent; 36, n=47) and smoking (27.7 percent; 13, n=47). Other cardiovascular risk factors such as abdominal obesity (4.2 percent; 2, n=47), gender (2.1 percent; 1, n=47) and family history (4.2 percent; 2, n=47) were largely ignored by the pharmacists. Point-of-care testing services were readily available in the pharmacies, with all of the pharmacies providing blood glucose and blood pressure measurements. Blood cholesterol measurements were only provided in 87.8 percent (36, n=41) of the pharmacies. The services were generally provided in a clinic facility, with 90.2 percent (37, n=41) of the pharmacies having a clinic facility available. Pharmacists were involved in the provision of point-of-care services, with 85.4 percent (35, n=41) of the pharmacies indicating that the pharmacists participated. Pharmacists readily provided counselling prior (70.7 percent; 29, n=41) to and after (80.5 percent; 33, n=41) the conduction of the screening services on areas such as lifestyle modification and treatment options. Only 15 percent (7, n=47) of the pharmacists indicated that they were aware of Cardiovascular Risk Calculator Tools and none of the pharmacists indicated that they had utilised such a tool. Pharmacists recommended frequent monitoring (60.5 percent; 26, n=43) and lifestyle modification (67.4 percent; 29, n=43) to patients, if the result of their screening service was within normal limits. ▼ Pregnancy challenges the health care system in a unique way in that it involves at least two individuals – the woman and the fetus. The death rates of both pregnant women (maternal mortality) and newborns (perinatal mortality) are often used to indicate the quality of care the health system is providing. In terms of maternal and perinatal outcomes South Africa scores poorly compared to other upper-middle income countries (Penn-Kekana & Blaauw, 2002:14). The high stillbirth rate compared to the neonatal death rate reflects poor quality of antenatal care. Maternal and perinatal mortality is recognised as a problem and as a priority for action in the Millennium Development Goals (Thieren & Beusenberg, 2005:11). The Saving Mothers (Pattinson, 2002: 37-135) and Saving Babies (Pattinson, 2004:4-35) reports describe the causes and avoidable factors of these deaths with recommendations on how to improve care. The quality of care during the antenatal period may impact on the health of the pregnant woman and the outcome of the pregnancy, in particular on the still birth rate. In primary health care services there are many factors which may impact on and influence the quality of antenatal care. For example with the implementation of the comprehensive primary health care services package (Department of Health, 2001a:21-35) changes at clinic level resulted in a large number of primary health care professional nurses having to provide antenatal care, who previously may only have worked with one aspect of the primary health care package such as minor ailments or childcare. Because skills of midwifery or antenatal care, had not been practiced by some of these professional nurses, perhaps since completion of basic training, their level of competence has declined, and they have not been exposed to new developments in the field of midwifery. The practice of primary health care nurses is also influenced by the impact of diseases not specifically related to pregnancy like HIV/AIDS and tuberculosis. The principles of quality antenatal care are known (Chalmers et al. 2001:203) but despite the knowledge about these principles the maternal and perinatal mortality remains high. The Basic Antenatal Care quality improvement package is designed to assist clinical management and decision making in antenatal care. The implementation of the BANC package may influence the quality of antenatal care positively, which in turn may impact on the outcome of pregnancy for the mother and her baby.
