▼ Abstract 1: The Association of Length of Hospital Stays and Number of ED Visits with Executive Functioning Among Childhood Cancer Survivors Background: Childhood cancer survivors are at increased risk of executive dysfunction. Stimulating environment; whether at school, day care, or home results in cognitive growth. Children with complicated health issues visit Emergency Departments (ED) multiple times in a school year and spend a very high number of days in the hospital, which can interfere with normal development. Objective: The current study aims to understand the association of total of hospital days and number of ED visits with executive functioning of children with cancer. Methods: We used a cohort of 77 childhood cancer survivors for this study. Parents were asked to complete several surveys and additional information was collected from the medical record and administrative data sets. Total hospital days and number of ED visits and were derived from the medical record and administrative dataset and executive functioning was assessed by a parent-rated instrument, the Behavior Rating Inventory of Executive Functions (BRIEF). Multiple linear regressions were used to determine the relationship between total hospital days and BRIEF scores and number of ED visits and BRIEF scores. Results: There is a positive correlation between number of ED visits and executive functioning, as measured by the BRIEF parent report scale. Moreover, there is a positive correlation between total hospital days and global score of executive dysfunction on the BRIEF. In multiple linear regressions, children with higher number of ED visits had more problems reported in multiple aspects of executive functioning; moreover, children with higher number of total hospital days had more problems with Global Executive Composite (GEC) component of executive functioning after adjusting for co-variables. Conclusion: An association was found between ED visits and length of hospitalization on parent-reported executive functioning. ED visits and hospitalization are often related to medical complications, and may reflect severity of illness, but may also influence children’s ability to participate in school and other aspects of normal development. Follow-up prospective studies with a longer duration are needed to explore potential confounds and understand the course of this relationship. Abstract 2: The Association of Neighborhood Poverty Status and Executive Functioning Among Childhood Cancer Survivors. Background: Childhood cancer survivors are at increased risk of developing executive dysfunction. Low socioeconomic status (SES) has been associated with poor executive functioning in childhood cancer survivors previously, but all the previous studies have used traditional measures of SES, based on parents’ education level, family annual income and occupation. I examine how women “move on,” or are unable to, after a diagnosis of breast cancer. I interviewed 80 survivors of various types and stages of breast cancer to explore the relationship between how breast cancer survivors think about cancer and how they manage the daily consequences of this disease, including its effects on identity. My main objective was to examine the cognitive strategies and social practices survivors employ for living with (a history of having had) cancer. Cancer experience is undoubtedly shaped by factors like disease stage and type, treatments received, time since diagnosis and treatment, age, and social location. But many of my participants, across categories, described bracketing some aspects of their experience while holding onto a certain degree of ontological insecurity as they redefined their lives after, with, or beyond cancer. Drawing on ontological insecurity enabled them to remain attuned to their selves: they used cancer to help them redraw boundaries in their lives and focus attention on their selves in ways they were not doing previously. While self-regulatory health practices, feminist ideologies/the women’s health movement, and environmental awareness overlap in breast cancer survivorship to produce activated patients and actualized subjectivities, my participants also discussed how the existential and medical uncertainties of their experiences led them to create new spaces for meaning in their lives. However, while many of my participants wanted to use cancer as a catalyst for self-growth or change across life domains, certainly not all of them were able to do so. Survivorship programs are critical in this regard. They can help survivors harness the uncertainty they feel instead of allowing it to become paralyzing or debilitating. Many survivors need help framing ontological insecurity as a resource to employ, not something to move beyond; but, moreover, they need spaces in which they can acknowledge these uncertainties as part of their new realities.

Study was conducted on hundred at risk heart patients. Information on various aspects like demographic profile, socio-economic profile, background information, blood lipid profile, anthropometric measurements and dietary pattern of subjects were assessed. From the data it was observed that the problem was more prevalent among the patients above 50 years of age and about 33, 34 and 33 per cent subjects had a positive family history of cardiac disease, diabetes and hypertension. As far as diet is concerned, intake of cereals, meat and meat product, roots and tubers, sugar and jaggery and visible fat was significantly reduced after the onset of disease but found to be still higher than RDA. Overall nutrient intake of calories, carbohydrates, fat and protein was also found to be much higher than RDA. Though, there was a significant reduction in the dietary consumption pattern after the onset of disease but still on the higher side as comparison to Recommended Daily Allowances. Majority of female patients had a grade I obesity as per classification of Body Mass Index because of their sedentary lifestyle. Abnormal lipid profile fraction was also observed in selected patients about 40 percent had a border line risk of total cholesterol. Intervention package was developed which include various audio/ visual modules to impart nutrition education on selected patients. The developed package was validated in terms of gain in knowledge which was found to be 14.11 and 10.46 in male and female patients, respectively. It can be inferred that intervention package can be an effective measure for bringing favourable and significant changes in the nutritional profile as well as in nutritional knowledge of cardiac subjects, which may lead to improvement in cardiac state and thus helps in retardation of many other complications. Sidewise suitable recipes especially for cardiac patients were also prepared from therapeutic food items such as Arjuna bark, Aloe-vera, Bottle gourd, Linseed, Dalchini, Geloy and Oat. newline

▼ Though much study has been conducted on HIV/AIDS and its scope, the area that seems to have been relegated to the back of most studies is about stigma and how it affects the overall outcome of the existing measures to mitigate the effects of the pandemic to the infected, affected and the community at large. Despite all the efforts that have been put in place, there are still many cases of treatment failure. Treatment success is when the measures being employed help the infected live and leads a normal life even when testing HIV/AIDS positive while failure would imply where the condition of the patient deteriorates progressively inspite of being on treatment. Most studies are concentrated on these known factors, that is, non-adherence, drug toxicity and potency of the antiretroviral regimen but studies on the stigma and its contribution to treatment failure has been neglected. This is the gap that my study was aimed at exploring as a way of knowledge furtherance. The study sought to establish the level of treatment failure among HIV patients undergoing treatment, to identify the causes of stigma among HIV patients undergoing treatment, to determine how stigma contributes to the failure in treatment among HIV patients undergoing treatment and to find out strategies of stigma reduction among HIV patients undergoing treatment. This research problem was studied through the use of a descriptive survey research design. The target population of this study was 298 respondents comprising 23 staff members and 275 HIV patients undergoing treatment at Mbagathi District Hospital. The researcher used purposeful sampling to select 13 staff members dealing with HIV treatment and simple random sampling to select 155 HIV patients undergoing treatment using random number tables. Qualitative data was collected by use of interview guides while quantitative data was collected by use of semi-structured questionnaires. The quantitative data in this research was analyzed by descriptive statistics using statistical package for social sciences (SPSS V 21.0). The study concludes that people often avoid being associated with HIV, with devastating effects on prevention, care and treatment. Stigma can reduce the likelihood of people using condoms and accessing preventive services such as educational meetings and counselling. People who hold stigmatizing attitudes are also less likely to adopt preventive behaviours. The study also concludes that stigma often prevents disclosure of an HIV-positive status to partners, providers and family members, which in turn deter behaviours that can prevent further spread of HIV, such as condom use, or mitigate its impact, such as care seeking. The study further concludes that stigma deters use of health facilities and adversely affects quality of health services. 　　The experience of cancer challenges the body and the mind. A solid body of evidence has amassed on what help breast cancer patients cope with adversities. In the field of positive psychology, the concepts of hope and optimism further our understanding of the role future expectancies play in goal-directed pursuits and in psychological adjustment to adverse events. 　　Snyder’s theory of hope (2002) conceptualizes that goal directed behaviours have two components – pathways and agency thinking. Pathways thinking is associated with finding the routes to achieve a desired goal. Agency thinking provides the mental energy to initiate and persist in pursuing goals. Hope is consistently linked with better outcomes in academics, athletic, health care and community settings (Snyder et al., 2005). 　　The literature on optimism (Scheier & Carver, 1985) describes the characteristics of optimists and pessimists. Optimists hold positive future expectancies with positive emotions keeping individuals actively engaged in their goals despite frustrations. Pessimists are more inclined to use avoidant coping in response to the negative emotions engendered in their negative expectancies. 　　In the present project, hope and optimism are investigated in a group of breast cancer patients who are recently diagnosed with recurrent, advanced or metastatic disease in Hong Kong. To the best of the author’s knowledge, no other studies have investigated future expectancies in advanced breast cancer patients in a longitudinal design. Findings suggested that agency hope and optimism together account for 27 - 29% of the variation in concurrent distress; and 52 - 54% of the distress three months later. Optimism was an independent predictor of concurrent and three-month psychological distress. However agency hope was not an independent predictor. The results of the present study are compared with a published study (Ho, et al., 2011) of hope and optimism in a group of oral cavity cancer survivors, who did not have an active disease. In both the advanced breast and oral cavity cancer patient groups, hope and optimism together predicted distress. But optimism was a stronger predictor relative to agency hope in the advanced breast cancer patients whose disease outcome was less certain. 　　The present study also attempts to decipher the cognitive processes postulated to be mediators in the relationship between future expectancies and psychological adjustment. In the advanced breast cancer group, attention to negative information mediated the relationship between optimism and concurrent anxiety and depression. In the oral cavity cancer survivors, negative cancer-related rumination mediated in the relationship between agency hope and depression, and in the relationship between optimism and anxiety and depression. The implications of the findings are discussed in light of the possible different cognitive processes of patients who are faced with two different disease conditions.

▼ End stage renal failure refers to an irreversible kidney dysfunction in which the kidneys fail to remove toxins from the blood stream. Haemodialysis is an effective treatment modality to sustain the lives of patients with end stage renal failure by removing waste products like urea, creatinine, and excess fluid. It involves a challenging regimen including dietary restrictions to ensure long-term survival. Failure to adhere with the regimen can result in fatal consequences. Renal patient discomfort related to non-adherence is commonly seen in clinical settings; however, there is currently no evidence-based fluid or dietary education program available to haemodialysis patients. This dissertation aims to identify and evaluate current evidence for the effectiveness of dietary and fluid control programs in the treatment of haemodialysis patients, to assess the transferability and feasibility of implementing a nurse-led education program regarding dietary and fluid control in haemodialysis patients, and to develop an evidence-based, nurse-led fluid and dietary control program for haemodialysis patients. Four electronic bibliographical databases including PubMed, Cochrane library, PsycInfo and CINAHL and two searching engines including Google scholar and ProQuest were used to identify studies that examined the effectiveness of educational programs or interventions on fluid or dietary control for patients on haemodialysis. Forty-five unique studies were identified as potentially relevant. Eleven of those studies met the selection criteria and were evaluated in this dissertation. Appraisal instrument was used to evaluate the quality of the selected studies. Six studies and three studies showed statistically significant in reduction of serum phosphate level and interdialytic weight gain respectively after educational intervention. Critical evaluation of the available studies led to an evidence-based, nurse-led, fluid and dietary control program for haemodialysis patients that followed the guideline development process of the Scottish Intercollegiate Guideline Network. Comparison on the similarity of patient characteristics, staff competence, and organizational settings of the evaluated studies were similar to those of the target unit. Therefore, the proposed program may be transferable and feasible. Furthermore, a cost-benefit analysis showed that the benefit of the fluid and dietary control program to patients outweighs the cost needed to implement the program. This dissertation outlines a proposed twenty-week program including marketing of the program, training of staff in the targeted renal unit, pilot testing, and application of the proposed program. Evaluation of the program will focus on three categories: patient outcome, health care provider outcome, and organization outcome. ▼ Background and Research Problem: It is well-known that nurses around the world are exposed to various forms of violence at their workplaces. In psychiatric facilities, many of these incidents are perpetrated by patients against nurses. There is a perception that the current legislation and regulations in the country do not adequately protect psychiatric nurses (health care workers in general) against workplace violence. The preliminary literature suggested that most quantitative studies on workplace violence in psychiatric facilities have concentrated on secondary and tertiary psychiatric hospitals looking at the prevalence, the association between demographic factors and violence behaviour, nurses‟ therapeutic responses, and the impacts on the quality of care. Studies on workplace violence in a psychiatric facility for adults with intellectual disability in the country are limited. Secondly, it appeared from the preliminary literature review that similar studies have not looked at this phenomenon from the individual resilience perspective.Therefore, this study will seek to determine the factors associated with the psychological response of nurses‟ victims of inpatient violence in a psychiatric facility for adults with intellectual disability in Cape Town.Aim: To determine factors associated with the psychological response of nurses‟ victims of in-patient violence in a psychiatric facility for adults with intellectual disability in Cape Town.Objectives: To describe the (1) individual resilience of nurses working at a psychiatricfacility for adults with intellectual disability; (2) psychological response of nurses‟ victims of in-patients‟ violence at a psychiatric facility for adults with intellectual disability; and (3) association between the individual resilience characteristics and the psychological responses of nurses victims of in-patients‟ violence in a psychiatric facility for adults with intellectual disability.Methodology: Descriptive-exploratory design using a quantitative approach was used.All categories of nurses (professional nurse, enrolled nurse and enrolled nursing assistance) working at the psychiatric facility for adults with intellectual disability were eligible for the study. Convenient sampling was conducted to select 127 participants who met the inclusion criteria. Self-administered questionnaire was used to collect data. Data was analysed using the SAS V9.3 computer programme. Ethical clearance was obtained from University as well as approval from the management of the psychiatric facility prior to approaching the study participants. Results: The results of the study showed high level of resilience among nurses on Assessment of Resilience Scale (82.9%, n=104); nurses psychological responses to violence were equally distributed between avoidance (mean =4.65 and SD=1.36), intrusion (mean= 4.55 and SD=1.50), hyper arousal (mean=4.46 and SD=1.60) resulting in total mean of 13.67 (SD=4.14) on the revised Impact of Event Scale (IES-R). This study focused on two novel elements: studying the phenomenon of ‘clinician anonymity’ and its effects on inter-professional communication in hospital practice and applying design science theory and methodology to understand and address communication problems. Healthcare is characterized by “more to do, more to know, more to manage, and more people involved than ever before.” This reality brings challenges to staff working in rapidly changing teams and stressful environments. The inability to recognize colleagues and the general lack of familiarity within teams undermines the effective collaboration that is critical to the delivery of patient care. This phenomenon, termed ‘clinician anonymity,’ was neither well defined nor understood, and few studies focused on interventions that addressed its causes and effects. In order to evaluate how an intervention, both the design of an artifact and the process of its design, influence the phenomenon of clinician anonymity, a longitudinal, mixed methods (observations, interviews, surveys) study was conducted at a large urban teaching hospital in Toronto, Canada. Based on existing frameworks and drawing from initial observations, a theoretical model of clinician anonymity was developed. The model informed the initial requirements for ‘Face2Name,’ a tool designed to act on the communication practices linked to anonymity. Pre- and post-intervention data comprising of 158 hours of observations over 8 months, 14 semi-structured interviews, and over 250 surveys were collected at four points in time. Results showed strong evidence of clinician anonymity in hospitals and revealed that the predominant cause is the nature of teaching hospitals, characterized by rapidly changing fluid teams, organizational structures and power dynamics between professions that dictate how collaborative processes and standard procedures are performed. Evaluation of the intervention confirmed the benefits of a visual tool on recall and staff satisfaction and user feedback contributed to making each subsequent iteration better suited to staff workflow and the hospital environment. Results also revealed that the process of designing and deploying the tool was more effective in addressing the problem of anonymity than the artifact itself, confirming a main tenet of design science that the intervention is both process and resulting product. Findings from this study are of value within and also outside the hospital, generalizable to other environments with fluid teams or with similar institutional cultures. Moreover, the reflective process intrinsic to design science provided considerable insights on how to conduct future design studies in the context of communication research.

▼ Context: Among cancer survivors, multi-morbidity is common, with the majority of survivors co-managing at least one additional condition alongside their cancer care needs. A co-diagnosis of diabetes, a heterogeneous group of metabolic disorders, occurs in 16-29% of all cancer survivors. Both diabetes and cancer place considerable stress on the individuals and populations affected by these conditions. Research has identified multiple factors associated with better outcomes for specific cancers and diabetes when considered separately; however, there is a need to develop a holistic understanding of the impact of co-managing these conditions. Additionally, health disparities for cancer and diabetes among specific racial and socioeconomic groups for mortality and several health-related outcomes have persisted for decades. This study was guided by Link and Phelan’s theory of ‘fundamental causes,’ a social epidemiological theory that aims to identify the fundamental social causes of health inequalities by exploring four key relationships between socioeconomic status and health outcomes. According to the theory of ‘fundamental causes’ theory socioeconomic disparities in health are rooted in unequal resource distribution which positions those with more resources in a better position to adopt behaviors that will benefit their health. Guided by this theoretical framework, this dissertation examines the impact of social factors on co-managing diabetes and cancer on multiple risk factors and disease outcomes. The overall goal of this dissertation is to examine the impact of social factors and diabetic status on multiple outcomes in a nationally representative sample of cancer survivors, as articulated in the three specific aims below. Specific Aims: The aims of this dissertation were to: (1) identify factors associated with the co-occurrence of diabetes and cancer and factors known to be associated with higher risks for poor outcomes (i.e., obesity and physical inactivity); (2) examine differences in self-assessed health (i.e., proxy for mortality), physical and mental quality of life between diabetic cancer survivors and non-diabetic survivors; and (3) examine differences in multiple psychosocial outcomes for diabetic cancer survivors and non-diabetic cancer survivors. Design, Setting and Patients: All aims of this study utilize data from the U.S. Medical Expenditures Panel Survey Household Component (MEPS-HC) and the supplemental self-administered survey “Experiences with Cancer” collected from 2011-2012. The study sample is restricted to non-institutionalized civilians who reported having a diagnosis of cancer during their lifetime who completed the supplemental questionnaire. Head injury is the major cause of death for individuals under 35 years old in the United States of America, and a similar picture exists in South Africa. A major cause of traumatic brain injury is motor vehicle accidents. In addition, the advances in modem medical technology increase the chances for survival, for example, sophisticated medical diagnostic techniques such as computerised tomography (CT), and magnetic resonance imaging (MRI). Advances in emergency trauma and neurosurgical procedures have also increased the number of survivors following a head injury. The symptoms resulting from head injury include disturbed physical, cognitive, psychological and behavioural functioning. Long-term management of these symptoms is usually required. A body of evidence exists that demonstrates the value of rehabilitation during the first year of recovery from traumatic brain injury. Head injury cases often result in litigation in that the injured person may be entitled to compensation for the injuries sustained. Considerable forensic debate exists around the issue of whether the victim's symptoms are attributable to organic brain damage, or whether they reflect a pre-existing functional psychological state of psychiatric illness, or personality disorder. Further still, the individual may be 'malingering' or faking postconcussion symptoms of headache, dizziness, fatigue, memory deficit, impaired concentration, irritability, anxiety, insomnia, concern about bodily functions, and hypersensitivity to light and noise. Alternatively, a common diagnosis is traumatic neurosis with a psychogenic basis, and in the context of litigation is referred to as "compensation neurosis". Specifically, the issue of contention revolves around concussion, and mild or minor head injuries. This study proposed that both neuropathophysiological and psychosocial symptoms occur in the event of a traumatic brain injury. This view supports the traditional model that pain or the sequelae of head injury are not affected by compensation. Therefore, the objective of the study was that follow-up assessment after compensation pay-outs should produce similar results to the assessment results obtained during the litigation process. The hypotheses of the study were essentially confirmed. The neuropsychological error scores produced in the assessments during litigation, and two-to-four years after the completion of litigation were similar. However, similar to other studies, variations were evident. Four exceptions were demonstrated in that significant improvement was evident with regard to shotterm verbal memory delayed recall for paragraphs, and for immediate recall (without and after interference) in verbal memory for a word list. These improvements therefore pertain to verbal memory. The fourth improvement occurred with respect to manual dexterity and visual-motor work speed, although the improvement may be peripheral or primary (neurological). ▼ Risk factors and incidence of syphilis in HIV-infected persons, the HOPS, 1999-2015 BACKGROUND: The incidence of syphilis has been increasing, especially among gay, bisexual and other men who have sex with men (MSM) for at least a decade in the United States (U.S.). We assessed incidence, temporal trends and associated risk factors for newly diagnosed syphilis infections among HIV infected patients in care during a 15-year period. METHODS: We analyzed data from the HIV Outpatient Study (HOPS) cohort participants seen at ten U.S. HIV clinical practice sites from January 1, 1999 to June 30, 2015. New syphilis cases were defined based on a combination of established laboratory parameters as well as clinical diagnoses. We assessed incidence rates of syphilis by patient socio-demographic, clinical and behavioral characteristics, and performed multivariable Cox proportional hazards regression analyses of risk factors for new syphilis infections. RESULTS: We studied 6888 HIV-infected participants, among whom 641 had one or more new syphilis diagnoses during a median follow-up of 5.2 years (Interquartile range: 2.0 to 10.8). Most study participants were male (78%) and aged 31-50 years (Table) and 56% of participants were MSM, 28% heterosexuals, 10% persons who inject drugs (PWID) and 6% other/unknown risk. There were a total of 799 syphilis diagnoses for an overall incidence of 1.8 per 100 person-years (95% Confidence Interval [CI] 1.6-1.9). The crude incidence rate was higher among MSM than heterosexuals (2.6 vs. 0.7, P <0.001), was higher among participants aged 18-30 years than over 50 years (3.0 vs. 0.8, P<0.001), and was elevated among non Hispanic blacks vs. white and Hispanic/Latino participants (Table). Rates of diagnosed syphilis were highest in the most recent time period, 2011 2015, as compared with prior periods. In multivariable analyses, the independent risk factors for syphilis included (all P< 0.001): being aged 18-30 years (hazard ratio [HR] 1.8, CI 1.5-2.1) vs older, having MSM HIV risk (HR 4.4, CI 3.6-5.5) vs other HIV risks, being black, non Hispanic (HR 1.8, CI 1.5-2.1) vs other race/ethnicities, and being observed during 2011-2015 (HR 2.3, CI 1.7-3.0) vs earlier periods. CONCLUSIONS: The steady increases in the syphilis incidence rate through 2015, particularly among HIV-infected patients who are younger, black, non-Hispanic and MSM, reflect ongoing sexual risk. ▼ Background: - Patients have the right to expect Quality of care. Patient satisfaction with nursing care is considered an important factor in explaining patients' perceptions of service quality. Care assessed to be high quality according to clinical, economic or other provider-defined criteria is far from ideal if as a result of that care the patient is unhappy or dissatisfied. There is, then, a sound rationale for making the organization and delivery of health care responsive to consumer opinion. Objective: -The objective of this study is to assess quality of nursing care as indicated by patients’ satisfaction. Methods: - A Cross-Sectional Survey was conducted at Public Hospitals in Addis Ababa, namely Tikur Anbessa, Saint Paul and Zewditu Memorial Hospitals from July, 2004 to April, 2005. The study population was adult patients who were admitted into the medical, surgical and gynecological wards of study Hospitals for at least two nights. The data collection tool was a modified ‘Newcastle Satisfaction with Nursing Scale’ (NSNS). Data analysis was computed by SPSS. Ethical clearance was secured from Addis Ababa University, Medical faculty and from the study Hospitals. Participation in this study was voluntary and based on patient’s ability to give informed consent. Result: - A total of 660 adult patients from medical, surgical and gynecological wards were approached from the study hospitals and 631 of them participated in this study (Non-response ii i rate was 4.4%).Participants who were females, age group 41 – 50 years of old, having low income, less educated, patients who were admitted in third classes, and have no history of previous admission were more satisfied. The over all rating of satisfaction was 67%.The top aspects that patients scored highest for their satisfaction with nursing care were the amount of freedom given, nurses capability in their work and nurses treatment of patients as an individual. The aspects with which patients were least satisfied were the amount and type of information they received regarding their condition and treatment and also the amount nurses knew about patients’ care. Age was the significant predictor of patient satisfaction with nursing care (p<0.05). The need of improving interpersonal relation ship of nurses with their patients was recommended.
