日本は高齢化率が26.7%を占める超高齢社会である(平成27年10月1日現在)。平均寿命が伸びていく中で、介護や治療、何らかの支援を必要としない期間すなわち健康寿命をいかに延伸するかが課題となっているが、近年、高齢者の低栄養が多くみられることが報告されており、要支援や要介護の要因となる可能性が指摘されている。 加齢に伴い残存歯数は減少し、咀嚼や嚥下といった食べる機能も低下していくために、必要な栄養素を充分に摂取できない状況にある高齢者も多い。介護食は、このように低下していく食べる機能を助け、適切な栄養摂取ができるよう食形態を調整した食事である。現在、ユニバーサルデザインフードやスマイルケア食等、市販品として多くの介護食品が存在しており、その需要は今後も拡大していくものとみられる。 青森県は高齢化率28.7%(平成27年2月1日現在)と、超高齢社会の日本の中でもとくに高齢化率の高い地域である。平均寿命は男女とも全国最下位であり、要介護認定率も全国平均を上回っている。日本全体が抱える健康寿命の延伸という課題は、本県においては平均寿命の延伸と併せてとりわけ重要な課題であるといえる。 本県における高齢者の食の現状について、県内に事業所を有する老人福祉施設や在宅の介護者、および給食事業者等を対象に実施した食事提供サービスに関する調査によれば、約6割がなんらかの形で食形態を調整する必要があることが報告されている。また、主として介護食を提供する立場にある人たちを対象とした介護食に関するニーズについての調査から、介護食のレシピや献立例、介護食用の食材、レトルト・冷凍・インスタント等の介護食品、介護食調理の講習会等、介護食提供者の負担を軽減しながら喫食者の満足度を高めるものが求められていることがわかった。特に、介護食のレシピ、介護食品については回答数が多く、「地元の食材をたくさん使った介護食のレシピ」といった回答がみられるなど、県産食材を介護食に利用することへの期待が大きいと推察された。 このようなニーズを踏まえ、県産食材を利用した介護食品と、それを用いた料理のレシピの開発に取り組むことにした。具体的な食材の選定にあたっては、事前に県内高齢者関連施設の給食担当者を対象に調査を実施し、咀嚼や嚥下が困難な方が食べやすくなるように加工されることが望まれる食材について答えてもらった。その結果、山菜や野菜、こんにゃく、もち、魚、イカ、貝類、肉等の食品があげられた。このうち、イカは青森県の地域食材のひとつであり、国内漁獲量ランキングでも上位にある食品である。高齢者の食事に積極的に用いたい良質の動物性たんぱく質給源でもあるが、加熱により硬く咀嚼しにくいテクスチャーになることから、高齢者の食事にはあまり用いられていない実態がみられた。そこで、高齢者ソフト食作りのスキルを応用し、青森県産スルメイカをペースト状にした後、つなぎ食材を加えて成形するという方法により、咀嚼が容易な介護食品を試作した。つなぎ食材として、同じく青森県の重要な地域食材であるナガイモを使用した。試作品を「ソフトイカ」と命名し、これを使った郷土料理(イカメンチ、イカのごろ味噌煮、イカとネギの酢味噌和え)のレシピを考案した。 ソフトイカのテクスチャー測定(硬さ、凝集性、付着性)と官能評価の結果から、ナガイモはすりおろした状態で添加する方法が最も介護食に適していることがわかった。また、ソフトイカを使った郷土料理の官能評価の結果から、食卓に供する際の形としてソフトイカは多用な料理に利用可能であることが示された。 以上のような介護食に関するニーズ、県産食材を利用した介護食品の開発等について、青森県農林水産部総合販売戦略課が平成27年度に設置した「『あおもり食産業』介護食ビジネス研究会」のセミナー・研究会等で、数回にわたり情報発信する機会を得た。参加者の意見や反応から得られた示唆を踏まえ、行政に対して、①県産食材を安価で入手できる仕組みづくり、②生産者・食品製造業者・介護食ユーザーの三者連携のためのシステム構築、③生産者・食品製造業者への支援体制の整備、④県民の介護食に関する認知度向上のための取組み、⑤介護食ビジネス研究会の継続、の5項目について提言した。 これまでの成果をもとに、今後はつなぎ食材としてのナガイモの有用性を活かして、他の県産食材も使用しながら新たな介護食レシピと調理法の開発を行い、WEBや調理講習会等を通じて情報発信していく。介護食に関する地域のニーズを把握しながら、介護食提供者の物理的・精神的負担を軽減するとともに、介護食喫食者の満足度を高め、食支援の一助となるよう取り組んでいきたい。
