▼ Over the last 25 years, ovarian stimulation for IVF treatment has gradually become more complex, time consuming and expensive. Recently, the downsides of ovarian stimulation have attracted increasing attention. Currently, a shift in the focus of IVF is occurring from striving for maximizing instant success 'at all costs' to developing safer and more patient friendly protocols in which the risks of treatment are minimized while optimizing the chance of a singleton live birth. Strategies involving single embryo transfer and milder ovarian stimulation protocols) have been proposed to provide such a patient friendly strategy. An important concern regarding the use of mild treatment strategies remains the reduction in the per cycle chance of pregnancy. The principle aim of this thesis was to identify means of improving the efficacy and therefore the uptake of milder treatment strategies for IVF such as mild ovarian stimulation and SET. In this thesis it was shown that fear regarding the use of mild stimulation for a reduction in ovarian response or an increase of drop-outs (when the number of treatment cycles needed is increased) is unjustified. Indeed, the retrieval of a modest number of oocytes following mild ovarian stimulation was, in contrast to following conventional ovarian stimulation, associated with optimal pregnancy outcomes. Additionally, patients were found to be willing to undergo an increased number of treatment cycles as long as a mild ovarian stimulation protocol was applied. Furthermore, prediction models were developed to reduce the chance of cancellation following mild stimulation and to provide an evidence based method to identify women who may qualify for the transfer of a single versus two embryos. After external validation the first model should be able to reduce the chance of cancellation following mild stimulation to an average level, the second should lead to an increase the overall pregnancy rate per transfer while maintaining a low number of multiple pregnancies. These interventions could increase the efficacy and implementation of mild treatment strategies.

▼ How is the church addressing the difficult issue of infertility? Pastors may not be adequately trained to handle the demon of infertility, which is a growing epidemic. Thirteen Scriptural remedies for the struggle of infertility are proposed in the form of a manual which is designed to equip and be implemented by churches. The hope is that churches will effectuate either a group dynamic or individual canvas for a detailed analysis of what Scripture teaches about infertility. Research involves surveying pastors and infertile couples about what they believe is being done in churches to relieve the sting of infertility. Results prove that there is an undeniable need for support for couples within the church involving this agonizing issue. A biblically-based manual is presented that focuses specifically on this burdensome trial. Utilizing it, churches will effectively be able to minister to couples and help them forge through the fire of infertility.

▼ The objective of this study was to determine the relationship among thrombophilia, inherited and acquired and unexplained female infertility. This was a prospective study, in which 278 women, with several fertility problems and interval age 36,2 years, were prospectively included. These women were divided in three studying groups, based in the following inclusion criteria: A). 96 women with unexplained infertility. Some of the infertile couples may have undergone unsuccessful IVF attempt without any conception. B).73 women with recurrent pregnancy losses. C). 109 women with IVF cycles, who had experienced a successful conception followed by an unexplained abortion. Moreover we studied 61 fertile women who have conceived naturally (control group). These women were tested in antithrombin, protein C and protein S deficiencies, Factor-V Leiden, FII G202210A mutation (PTM), MTHFR C677T and A1298C polymorphisms (inherited thrombophilic factors), homocysteine levels in women's blood (mixted thrombophilic factor), PTTLa, immunoglobulin (Ig) M and IgG anticardiolipin antibodies (ACAs), immunoglobulin (Ig) M, IgA and IgG antiB2-glycoprotein antibodies (antiB2GpI), phospholipid profile (anti-phosphatidilserine, anti-phosphatidilinositole, anti-phosphtidilaithanolamine, anti-phosphatidilcholine and anti-phosphatidilglycerole antibodies levels, which are the less frequent antiphospholipid antibodies) (acquired thrombophilic factors) and anti-thyroid peroxidase (anti-TPO) and anti-thyroglobulin (anti-TG) antibodies. Afterwards, we examined the possible relationship among the inherited and acquired thrombophilic factors and female infertility, investigating the presence of statistically important difference (p<0,05) among the percentage of the factors in study and control groups. We also examined the relative risk of infertility in women with thrombophilia studying the Odds ratio (O.R.) with 95% CI (confidence interval) for these factors. Ιn our study there was not observed a relationship between antithrombin, protein C or protein S, FV-Leiden, PTM mutation, MTHFR A1298C polymoprphism, homocysteine levels, PTTLa, ACA IgG antibodies, anti-beta2GPIgG or IgM antibodies and female infertility. On the contrary, we have found a relationship between MTHFR C677T polymorphism, homozygous and heterozygous, ACA IgM antibodies, anti-beta2GPIgA and female infertility. Moreover, there was identified a association between phospholipid profile and unexplained infertility or abortions in IVF treatment but not with recurrent pregnancy losses. Finally, this study demonstrates a strong association between the presence of thyroid autoantibodies (ATA-TG and ATA-TPO) and female infertility.

▼ La morphogénèse des spermatozoïdes a lieu pendant la spermiogénèse, la dernière phase de la spermatogenèse.Cette différenciation est accompagnée de modifications drastiques de l’enveloppe nucléaire associées à un fort remodelage chromatinien. Notre travail été centré sur l'enveloppe nucléaire des spermatides pendant la spermiogénèse humaine. Nous avons notamment caractérisé la lamina nucléaire, réseau protéique composant de l’enveloppe nucléaire, et ses protéines partenaires potentiellement impliqués dans les liaisons lamine-chromatine: les protéines à domaine LEM, LBR, la protéine chromatinienne BAF et BAF-L.Nous avons révélé la présence exclusive des lamines de type B concentrées au pôle postérieur à la fin de la spermiogénèse dans les spermatozoïdes, et nous avons identifié et caractérisé l’isoforme testicule-spécifique lamine B3 humain.Nous avons découvert et caractérisé la lamine A2, un isoforme méiotique du gène LMNA, chez l'homme et la souris.Nous avons ensuite étudié ces protéines chez des patients atteints de globozoospermie, et nous avons pu identifier BAF comme un nouveau biomarqueur de l’immaturité des noyaux de spermatozoïdes. Par ailleurs, nous avons identifié la deuxième mutation perte de fonction dans le gène SUN5 chez 3 patients apparentés, et validé son implication dans la mise en place de la jonction tête-flagelle des spermatozoïdes.Notre caractérisation de la lamina nucléaire et ses protéines partenaires au cours de la spermiogénèse humaine apporte une meilleure compréhension de son rôle dans la différenciation des spermatides en spermatozoïdeset peut être la base de nouveaux champs d’investigation cas d’infertilité liés à des anomalies nucléaires.
The morphogenesis of mature spermatozoa takes place during spermiogenesis, the last phase of spermatogenesis.This differentiation involves drastic changes in the nuclear envelope associated with profound chromatin remodelling.Our work was focused on the nuclear envelope of spermatids during human spermatogenesis.We have characterized, the nuclear lamina, a protein meshwork component of the nuclear envelope, and its protein partners potentially implicated in the linkage lamin-chromatin: LEM-domain proteins, LBR, chromatinien protein BAF and BAF-L.Our study revealed the exclusive presence of B-type lamins concentrated at the posterior pole of mature spermatozoaat the end of spermiogenisis and we have identified and characterised the testis-specific isoform lamin B3 in human.We have also discovered and characterized the lamin A2, a meiotic isoform expressed from the LMNA gene in human and mouse. By studying abnormal globozoospermic spermatozoa, we were able to identify BAF as a potential biomarker of spermatozoa nucleus immaturity.Moreover, we have identified the second loss-of-function mutation in the nuclear envelope protein SUN5 in three related patients, and thus demonstrated its involvement in the formation of the spermatozoa head-tail junction.Our characterization of the nuclear lamina and its protein partners during human spermiogenesis, provides…

Strategies involving single embryo transfer and milder ovarian stimulation protocols) have been proposed to provide such a patient friendly strategy. An important concern regarding the use of mild treatment strategies remains the reduction in the per cycle chance of pregnancy. The principle aim of this thesis was to identify means of improving the efficacy and therefore the uptake of milder treatment strategies for IVF such as mild ovarian stimulation and SET. In this thesis it was shown that fear regarding the use of mild stimulation for a reduction in ovarian response or an increase of drop-outs (when the number of treatment cycles needed is increased) is unjustified. Indeed, the retrieval of a modest number of oocytes following mild ovarian stimulation was, in contrast to following conventional ovarian stimulation, associated with optimal pregnancy outcomes. Additionally, patients were found to be willing to undergo an increased number of treatment cycles as long as a mild ovarian stimulation protocol was applied. Furthermore, prediction models were developed to reduce the chance of cancellation following mild stimulation and to provide an evidence based method to identify women who may qualify for the transfer of a single versus two embryos. After external validation the first model should be able to reduce the chance of cancellation following mild stimulation to an average level, the second should lead to an increase the overall pregnancy rate per transfer while maintaining a low number of multiple pregnancies. These interventions could increase the efficacy and implementation of mild treatment strategies.

Infertility is defined as the inability to conceive after a minimum of one year of regular intercourse without contraception (Carlson et al, 2002). This may occur as primary infertility, where individuals have never had a biological child, or secondary infertility where individuals have had at least one previous documented conception (Greer et al, 2003). Infertility, in the African setting, is seen as a violation of the social norm. It contributes to psychological distress and marital instability as well as the loss of social security, social status and gender identity. Parenthood is considered culturally mandatory making childlessness unacceptable. Not only does Africa have the highest fertility rates in the world, Africa also has the highest number of infertility cases globally (Dyer et al, 2005; Ragone & Twine, 2000). Unexplained infertility is diagnosed when the routine investigation of semen analysis, tubal patency and assessment of ovulation show no abnormality and the couple have engaged in regular sexual intercourse. Unexplained infertility is thus a diagnosis of exclusion. When a previously identified cause of infertility is corrected, yet infertility persists a diagnosis of unexplained infertility is also given (Behrman et al, 1988). Before a diagnosis of unexplained infertility can be made four aspects are generally assessed. Each aspect encompasses a different aspect of the reproductive process: the number and quality of sperm, maturation and release of the ova, barriers to fertilization and barriers to implantation and maintenance of pregnancy (Cooper-Hilbert, 1999). The aim of this research was to evaluate the efficacy of homoeopathic similimum treatment in females pre-diagnosed with unexplained infertility using case studies. Cases were evaluated using fertility and parameters related to fertility which include basal body temperatures and midcycle cervical mucous changes. In addition, general parameters such as general well-being, premenstrual symptoms, dysmenorrhoea and sexual function were also evaluated. Concomitant symptoms were included in the analysis of each participant’s holistic case. The male partner was also required to be free of any structural and functional pathology.

▼ In the first part of study, our objective was to determine the effect of CoQ10 supplementation of culture media on preimplantation mouse and human embryo development. CoQ10 supplementation of culture media did not improve mouse or human embryo development in vitro. Since the results appeared to be negative, we decided to move on to research the effect of age on female infertility.
In the second part, we investigated the effect of female age and ovarian stimulation protocols on IUI outcome in 411 infertile women. We found that the ongoing/live birth rate per cycle in women ≤ 37 years was significantly higher than in older patients.
In the third section, we determined if very young age (≤25 yrs) has an impact on pregnancy outcome in women undergoing IVF-ET. Our results demonstrating lower pregnancy rates in very young patients and egg donors compared to the patients in their early thirties were surprising.

▼ Uterine transplants (UTx) are currently being researched as a method by which a woman with uterine factor infertility (UFI) can become pregnant and gestate her own genetically related child. Current criteria for ethically selecting UTx recipients and donors are limited. At its core, UTx is an organ donation being used as an assisted reproductive technology (ART), therefore this thesis will use the considerations and discussions surrounding organ donations and ARTs to inform an assessment of the current ethical criteria for receiving a UTx and the ethical criteria for donating one’s uterus. The discussions of organ donations and ARTs will also help in proposing and assessing potential additions to the criteria for both UTx candidates and donors in clinical practice and in addressing lingering questions involved in the ethical clinical use of UTx such as allocation of uteri and payment for the procedure.

▼ Objectives: Infertility is a public health problem which in Africa south of the Sahara affects up to 40 percent of women. As such, infertility has devastating psychosocial consequences on affected couples. The World Health Organization estimates that there are 60-80 million infertile couples worldwide and the majority of whom are in developing countries. The objective of this study was to investigate and determine the factors associated with infertility in women attending the gynaecology clinic at the University Teaching Hospital (UTH) in Lusaka, Zambia.
Methods: Women attending the gynaecology clinic for infertility were enrolled using an unmatched case-control study design. Controls were randomly selected from women in the labour ward. A preformed investigator administered questionnaire was used to collect data. Bivariate analysis was used to determine association between infertility and demographic and previous gynaecological history. Odds ratios were used to determine statistical significance. Associations with a p<0.05 or those with clinical significance were further analysed by multivariate logistic regression.
Results: One hundred and thirty women were selected as cases and 260 as controls. Overall, primary infertility was found to affect 50/130 (38.5 percent) of the women while secondary infertility was found in 80/130 (61.5 percent). In multivariate logistic regression analysis, the following factors were independently associated with infertility: marital status, age at menarche, frequency of menses, ever use of contraception, ever had a manual vacuum aspiration and past pelvic procedure or infection. Though on bivariate analysis, age >35 years and partner alcohol use were associated with infertility, this was not the case in the multivariate logistic regression analysis when potential confounders were considered. The odds of a married woman having infertility was 9 times that of an unmarried woman (OR 9.09, 95% confidence interval 2.22 to 37.29). Irregular menses, was over 20 times that of a woman with regular cycles to be associated with infertility (OR 20.72, 95% CI 2.52-170.56). Past ever use of contraception was associated with 83 percent less odds of infertility (OR 0.27, 95 CI 0.14-0.57). Investigations were ordered for infertility were hormonal, HSG, ultrasound and semen analysis though in most cases the investigation was not done or results were not available.
Conclusion: There was a higher proportion of secondary infertility than primary infertility in this study. Marital status, age at menarche, frequency of menses, ever use of contraception, ever had a manual vacuum evacuation and past pelvic procedure or infection were independently associated with infertility though other factors, like behavioural and weight, were not considered. It appeared that ovulation problems as manifest by irregular menses could be amenable to ovulation induction agents though suspected tubal damage (after MVA and pelvic surgery) could be problematic in treating. ▼ Many women of reproductive age who survive cancer discover the additional challenge of infertility caused by their cancer treatment. This study sought to understand the simultaneous crises of cancer and infertility that require invasive medical treatment relating to life and legacy among young adult female cancer survivors. This reproductive trauma has the potential to affect approximately 70,000 women of reproductive age (15-39) diagnosed with cancer annually in the United States. For women who are newly diagnosed with cancer, considerations related to the impact of cancer treatment on their ability to have children add to the complexity of emotion and decision-making around their treatment. For some women, the loss of their fertility is just as, if not more, important than their cancer diagnosis and can result in distress, depression, anxiety, anger, loneliness, and regret. Those who are able to find meaning in their traumatic loss, however, may experience posttraumatic growth, which has significant implications for survivorship. Data were collected from interviews, a demographic and background survey, and participant collaboration on a preliminary results report. Eleven women were interviewed and seven women participated in analyzing the results. Thematic analysis was used to examine the data, after which six themes were identified that describe the experience of iatrogenic infertility among young adult female cancer survivors. The themes pertain to fertility loss; emotional strain; liminality and identity shifts; relationship tension; systemic comparisons between cancer care and infertility sectors; and infertility openness and advocacy. Among the five domains of posttraumatic growth, spirituality was the domain least identified with change among women currently experiencing infertility.This research has implications for all professionals who work with young adults with cancer and assist in the process of meaning-making after trauma and loss. It raises awareness of what women with cancer confront after going through cancer treatment in their quest to have children in survivorship. It reveals gaps in service, areas of retraumatization between the cancer and infertility sectors, and the deep concern that women have regarding their fertility and the great loss that is felt once it is eradicated from cancer treatment.

It documents, analyses and reflects upon the evolution of a project, that over nine years, examined the challenges of designing and implementing a clinical trial design that could capture the effect of using Ovarium compositum to support the outcomes of women undergoing infertility treatment at Jersey General Hospital. Aims: The initial aim of the research project was to design and conduct a randomised clinical trial of a homeopathic product, Ovarium compositum, as an additional treatment for women undergoing infertility treatment at Jersey General Hospital. When the trial did not meet it's recruitment target the focus of the project switched to an investigation of the reasons for trial failure. The need for a pragmatist philosophical worldview to bring structure and understanding to the process of writing and rewriting the thesis became increasingly clear as the project evolved. The aims are divided into two categories, methodological and outcomes. ▼ A book of poems, Simple Machines asks in what respect the human body can be considered a simple machine in the tradition of the inclined plane, the lever, and the pulley. It often ends up frustrated—infertility is particularly devastating to the speaker of thesepoems—at how unpredictable and alienating one’s own body can be. Simple Machines takes up language as another possible “simple machine”; it experiments with composition methods that might be considered “mechanical” or actually use machines, literally. Forexample, in the first section, I use the online Oxford English Dictionary’s reverse-search feature to create word lists from which to compose poems. In the second section, I conduct erasures of scholarly texts about machines’ surprisingly human capabilities. In the thirdsection, I also present “poem machines” in homage to the language machine in Swift’s Gulliver’s Travels—paper structures that can be bound in a book and later detached, assembled, and manipulated to make poems. These restrictive writing methods allow for thekind of invention created by the necessity of stringent regulations. They mimic the proposed mechanical nature of the body—if the language itself has hidden structures made available by technology, perhaps the human body and mind do, as well.Donna Haraway’s “A Cyborg Manifesto” argues that machines are inextricably linked to our bodies, and we can exploit that connection for our own creativity. VernorVinge, in “The Coming Technological Singularity: How to Survive in the Post-Human Era,” argues for “human/computer symbiosis in art” as a process of “Intelligence Amplification (IA),” an alternative to Artificial Intelligence (AI). This dissertation aligns itself with the idea of IA—I intend a “merging of competence,” to use Vinge’s term, between my ability as apoet and the computer’s ability to search great quantities of words to find groups of words whose members have surprising similarities, as made available by electronic dictionaries. Modernist literary concerns propose a poetics invested in technological innovation, humankind’s triumph over nature via technology—William Carlos Williams, in his introduction to The Wedge, calls a poem “a small (or large) machine made of words.” In thatvein, this project tests the hypothesis that our machines, our bodies, and our language sync up—if we think of the body as a machine, do we expect too much from it? Or do we expect the wrong things? Or do machines hold unexpected abilities we can use for our own artisticaims, perhaps in “collaboration”? And what kind of poetry would that make possible? I propose that the answers to these questions depend upon and spring up from Haraway’s assertion that “[o]ur machines are disturbingly lively, and we ourselves frighteningly inert.”

▼ Gonadotropin-releasing hormone (GnRH) is a primary and essential regulator of vertebrate reproduction. When GnRH neurons are compromised, GnRH expression is attenuated, producing the condition Idiopathic Hypogonadotropic Hypogonadism (IHH). IHH is characterized by infertility and/or delayed or absent puberty. When this condition is accompanied by anosmia, it is termed Kallmann syndrome. Although it is clearly a crucial factor in the reproduction of mammalian species, little is known about the genetic regulators of GnRH neuronal ontogeny. In the work presented herein, we elucidate developmental and genetic mechanisms that control the migration and maturation of GnRH neurons, and therefore illuminate the novel mechanisms responsible for IHH. Furthermore, we have identified new genes influencing the relationship between nasal development and GnRH neuron migration- further elucidating the etiology of Kallmann syndrome. The two genes that are explored in the work presented here are the closely related homeodomain transcription factors, sine oculis-related homeobox 3 (Six3) and Six6. First, we present data demonstrating that Six3 regulates GnRH neuron migration through actions external to the GnRH neuron. Six3 heterozygous mice displayed disrupted migration of GnRH neurons and severe olfactory impairment. Interestingly when the role of Six3 specifically within the GnRH neuron was investigated using a GnRH specific cre-recombinase, we found that Six3 acts within the neuron to upregulate GnRH neuron number. In addition, we investigated the actions of the closely related gene Six6. Six6 was found to be essential for survival of GnRH neurons and maintenance of fertility in a Six6 knock-out mouse. Six6 was found to play a similar role within the GnRH neuron in enabling fertility; however, within the GnRH neuron, Six6 acts to regulate GnRH expression. These investigations have enabled us to garner knowledge concerning the novel regulatory mechanisms of this fascinating hormone; and, has allowed us to shed light on the causes of infertility plaguing ~7% of all couples.

non-contraception couple to achieve pregnancy in one year.
Generally, infertility is classified as either primary (67-71%)
when no pregnancy has ever occurred or secondary (29-33%) where
there has been a pregnancy, regardless of the outcome. It affects
13-18% of couples of reproductive age worldwide and is treatable in
many cases [1]. Although, infertility is not life threatening, it
is psychologically a traumatic condition associated with social
stigma in certain societies resulting in intense social and
parental pressure to attain biological parenthood; hence many
infertile couples choose to undergo assisted reproductive
technology as a form of treatment. In recent years, it has been
demonstrated that up to 50% of infertility cases are associated
with male factor because of spermatogenic failure; interestingly in
which ~70% of the underlying etiological cause(s) are known but
remaining 30% are referred to as idiopathic infertility [2] Thus,
the etiology of male infertility is complex and can be reflected in
a variety of conditions like defective spermatogenesis, congenital
absence of bilateral vas deferens (CABVD), infections, and
immunological disorders such as antisperm-antibodies, diabetes,
obesity, pituitary disorders, and imbalance in levels of
gonadotrophic hormones. Genetic factors are considered to be
significant in male infertility [3]. Hence, the aim of the study is
to investigate the chromosomal aberrations, azoospermia factor
(AZF), cystic fibrosis transmembrane regulator (CFTR) gene
mutations in the peripheral blood lymphocytes (PBL) of infertile
men with abnormal sperm parameter and compare it with healthy
fertile control subjects to delineate the genetic causes of male
infertility. This thesis comprises of six chapters. The first
chapter summarizes about male factor infertility: incidence,
classification, etiology, genetic factors, and treatment options.
In around 50% of infertile couples, male-related causes are
involved, such as impaired sperma

▼ Phosphate (Pi) is a mineral that is essential for life and is found in many important biological molecules as well as comprising the structural material of bone and teeth. However, when animals fail to excrete ingested Pi, Pi overload occurs. Pi overload causes hyperphosphatemia associated with multiple aging-like phenotypes, including
infertility. We have employed two mouse models to study the effects of Pi overload on female fertility. The first model is Klotho mutant mice, which suffer Pi overload due to decreased renal Pi excretion. The second model is wild-type mice fed a high Pi diet, which develop Pi overload due to increased intestinal Pi absorption. Both mouse models of Pi overload are infertile, do not undergo proper puberty, do not ovulate, and have hypotrophic uteri. These animals have aberrant hypothalamic-pituitary-gonadal (HPG) axis function typified by low expression of Kiss1 mRNA in the hypothalamus, altered serum luteinizing hormone (LH) levels, and low mRNA expression of steroidogenic enzymes in the ovary. However, the HPG axis in these animals appears to be functional if stimulated with the proper hormonal cues. Estradiol administration stimulates Kiss1 expression in the anteroventral periventricular nucleus while inhibiting Kiss1 expression in the arcuate nucleus of the hypothalamus. Acute administration of Kisspeptin-10 elicits an LH surge. Additionally, gonadotropin administration induces ovulation. These results suggest that the HPG axis in female mouse models of Pi overload is capable of proper function but that upstream hormonal cues may be absent in these mice. We propose that low Kiss1 expression is the primary cause of HPG axis dysfunction in these mice since it is a master regulator of puberty onset in females. Our models of Pi overload-induced female
infertility may serve as a novel rodent model system in which to study sexual dysfunction and hormonal aberrations in women and children with chronic kidney disease who suffer from Pi overload due to decreased renal Pi excretion.

Advancing maternal age is a well-established risk factor associated with chromosome segregation errors in oocytes. In this thesis, I employed real-time high resolution imaging to examine the onset of aneuploidy and mechanisms regulating chromosome segregation in mouse oocytes. Specifically I determined 1) that although considerable cohesion loss occurs during MI, bivalent dynamics are not grossly affected, instead premature separation of dyads in MII was found to be the major segregation defect; 2) aged oocytes have decreased levels of SAC proteins on the kinetochores and possess both a lowered ability to maintain SAC arrest and re-establish bivalent biorientation following spindle disruption; 3) a genetic basis to aneuploidy susceptibility is likely to exist as suggested by the distinct aneuploidy phenotypes of two different mouse strains and 4) demonstrated that Ndc80 N-terminal modification was able to impose a robust SAC signalling, in doing so prevent chromosome mis-segregation in young mouse oocytes. Altogether this thesis directly examined the precise timing of chromosome segregation errors, and re-emphasized the role of a weakened SAC in maternal age-related aneuploidy.

▼ This study was an exploration on the impact of cultural and psychosocial
dynamics in the management and treatment of infertility. It also endeavoured to assess
the knowledge, attitudes, perceptions regarding infertility and its causes among
communities in Nairobi.
This study used descriptive research approach. The researcher used nonprobabilistic
sampling to administer semi-structured questionnaires, which were
distributed to forty-three respondents within Diamond Trust Bank. Secondly, the
researcher used the case study approach to carry out in-depth interviews with four
individuals' (two males and two females) who are suffering infertility. Finally, four
medical doctors in the field of reproductive health, specifically gynaecologists were
selected purposively and interviewed.
The study revealed that persons suffering from infertility were stigmatised.
Infertility was perceived from the standpoint of traditional practices and taboos such as
lack of respect for elders, curses, religious beliefs and "incompatibility of blood between
the spouses". Most communities were found to have inadequate knowledge about the
causes of infertility; however, those with higher education levels i.e. secondary, and
'~
postgraduate levels had knowledge on the medical ca-us.es of infertility. Community perceptions elevated the man thus led to bias and prejudice against women in instances
of childlessness. The results from all communities sampled indicated that women who
are infertile are mistreated thus the condition of infertility is feminised.
The dominant type of infertility revealed is the secondary form of the condition that
is found to result from a history of STIs, injury, poor corrective surgery or growths in
women's' reproductive systems. Facilities for the management of infertility were found
to be concentrated in the urban areas and at a high cost.

Management and treatment of infertility is a crucial factor for the publics well being.
Dealing with infertility within the populace is challenging due to the sensitive and
personal nature of the condition. Cultural factors playa big role in interpretations of
infertility therefore there is great need to create awareness and address infertility as a
medical condition right from the grassroots. In addition, affordable and easy accessibility
to medical facilities and doctors should be facilitated in all parts of the country.
This study recommends that the department of Public Health should have
reproductive health experts at the district level to help those who cannot access urban
treatment. It also recommended reduction of adoption process by removing some of the
bureaucracies, which makes the process too long and tedious.

▼ Antisperm antibodies (ASA) are thought to be a predominate cause of immune
infertility by interfering with various aspects of sperm function in both the male and the female reproductive tracts. The precise mechanism by which these antibodies contribute to
infertility, as well as their etiology, remains to be established. ASA are present in a variety of biological substrates, such as genital tract secretions, and the blood sera of both males and females. Although not all ASA underly
infertility, a substantial body of research suggests that certain ASA, referred to as sperm immobilizing antibodies (SI-Abs) and sperm agglutinating antibodies, significantly impair sperm transportation in the female reproductive tract. High titers of sperm agglutinating or sperm immobilizing antibodies have been associated with reproductive failure. CD52g is a GPI anchored glycoprotein found on mature sperm and in seminal plasma (SP). Antibodies against a male reproductive tract-specific epitope of CD52g are known to readily agglutinate sperm. The current study sought to develop an ELISA to quantify the prevalence of CD52g antibodies in the sera of male and female patients with
infertility, and to determine if there was a correlation between the prevalence of CD52g antibodies and the prevalence of sperm agglutinating antibodies in the sera of these patients. Ultimately, CD52g antibodies were only detected in the sera of patients (21%) with sperm agglutinating antibodies. While detecting CD52g antibodies in sera via an ELISA proved challenging, the results of this study corroborate research demonstrating that CD52g antibodies have a remarkable capacity to agglutinate sperm. Elucidation of the mechanisms underlying this immune response would advance our understanding of immune modulation in human reproductive tracts, further the diagnosis of immune
infertility, and are currently providing the basis for the development of a potent dual purpose immunocontraceptive, that both prevents unintended pregnancy, and prevents the transmission of sexually transmitted infections (STIs).

▼ BACKGROUND: Current estimates indicate approximately 6.7% of women in the US are infertile.
Infertility can arise from many different conditions and can be both attributed to male and female factors. In women, measures of ovarian reserve such as AMH level or FSH level on day three of the menstrual cycle can be used to predict reproductive success. In conjunction with additional tests and lifestyle screening, these values help to guide the course of treatment, with some patients moving eventually to in vitro fertilization. In vitro fertilization therapy is cost-prohibitive if the patient does not have insurance coverage, an issue for the majority of patients across the United States.
AIMS: To evaluate the correlation between cycle number, age, and AMH level with achieving a successful live birth through a statistical analysis of over 50,000 cycles of in vitro fertilization.
Methods: Data was obtained from Practice Highway, a company that owns the electronic medical record (EMR) program eIVF, used by over 140 reproductive endocrinology practices across the country. The database provided a total of 88 variables and 138,526 IVF cycle observations ranging from 01/01/2005 to 09/09/2015. A thorough review of available literature was performed to determine the need for the correlations analyzed in this paper. Utilizing this information in conjunction with the scope of this paper, the number of variables was reduced to four. Linear regression and multivariate regression was performed on the four main study variables to identify possible correlations.
Results: The results of our study indicate that there is not a strong correlational relationship between the variables analyzed. There was not a significant correlation between age and whether a live birth was achieved. As expected, the strongest relationship identified through our study is the decrease in AMH as age increases.
Conclusions: Our study results indicate that there is no correlation between AMH and live birth, suggesting that this biomarker alone is insufficient to be used by physicians as a means of advising patients on IVF success. Additionally, measures such as number of cycles completed and age at cycle start can be useful when counseling patients on their probability of success with assisted reproductive technologies.

▼ BACKGROUND: Infertility remains a challenge for some couples globally. Recent evidence shows that one in every six couples will face fertility problem in their reproductive age. For the past four decades, Assisted Reproductive Technology (ART) has been an effective intervention for infertility. The complexities associated with the ART presents many challenges to couples including psychological, social, and emotional difficulties. These challenges underscore the relevance of coping particularly during the process of the ART.
Objectives: This study therefore sought to understand the challenges associated with ART and the various coping strategies infertile couples adopt when seeking ART.
Methods: The study employed convergent parallel mixed method design. Exploratory phenomenological approach and cross-sectional survey were used for the qualitative and the quantitative studies respectively. Overall, 12 women and 6 men participated in the qualitative study, whereas 211 women participated in the quantitative study. The qualitative study employed purposive sampling technique and the quantitative used convenience sampling technique to recruit the respondents. The qualitative data was collected using in-depth interviews and the Ways of Coping questionnaire was used to collect the quantitative data. Analysis of the quantitative data was done using content analysis technique whereas the quantitative data was analysed using the Stata version 14.0. Descriptive statistics such as the mean, frequencies were determined from the quantitative data. Inferential statistics such as Chi-square test of independence, Fishers exact test, one-way analysis of variance and Kruskal Wallis were used to determine association between each of the predictors of coping and levels of coping strategy.
Results: The study revealed couples with infertility problems highly value biological children due to social stigmatization about childlessness. Some respondents indicated that they have suffered various forms of psychological, emotional, and social afflictions because of their inability to have their own children. Respondents utilised ART the best alternative solution to infertility problem. Factors such as advanced age of the woman, pressure from families, particularly mothers-in-law, and friends influenced respondents’ uptake of ART. Furthermore, various ART uptake challenges, identified included anxiety about treatment outcome, high cost of ART, and long distance to ART centres. The study respondents used different coping strategies to cope with ART. These included confrontational, distance, self-control, social support, acceptance responsibility, escape-avoidance, problem solving and positive reappraisal coping strategy. The most highly used coping strategy was positive-reappraisal (n = 104, 49.3%). Fertility centre and cause of infertility were the two covariates that were significantly related to overall coping strategy. None of the covariates were associated with the positive reappraisal coping strategy.

▼ Objective – This thesis was written to analyze the effects of Traditional Chinese Medicine (acupuncture as well as herbs) and naturopathic medicine in the treatment of idiopathic infertility. Research articles included looked at the efficacy of nutritional supplementation, herbal supplementation, acupuncture, and Chinese patent remedies. Basic Procedures – The majority of the research articles were based upon searches in PubMed. Other major resources include The Textbook of Natural Medicine (4th Edition) and the Women’s Encyclopedia of Natural Medicine by Tori Hudson ND. Main Findings – Naturopathic medicine and Traditional Chinese Medicine appear to be efficacious alternatives in the treatment of idiopathic infertility. Principal Conclusions – Lifestyle modification should be addressed with patients with idiopathic infertility. Research has found that smoking, excessive caffeine, and a low or high BMI could be implicated in the diagnosis of idiopathic infertility. According to research, emphasis should also be placed upon a nutrient rich diet. Without important vitamins and minerals such as B vitamins, vitamin E, zinc, and selenium in order to produce viable offspring. Herbal supplementation should be included in patients who have difficulty regulating hormones. Acupuncture and Chinese herbs may also prove to be effective in the treatment of idiopathic infertility especially when used in combination with IVF techniques.

▼ Background: Fertility problems in women often cause mental stress. Feelings that women describe during the infertility investigation is jealousy, sorrow, guilt, pain, and anxiety. These women need support and encouragement from the nurse during the investigation. Therefore it is important that nurses understand the women's experiences. Previous studies on women's experiences of infertility are based on interviews. Therefore, this study will focus on women's own stories about their experiences during infertility investigation. Aim: The aim of the study was to describe women's experiences of infertility during the time the investigation is in progress. Method: The study was based on narratives, which in this case means an analysis of biographies. Three autobiographies were analyzed according to Dahlborg-Lyckhages (2006) description of analysis of narratives. Results: Four themes and seven subthemes emerged from the analysis. The results show that women's experiences are characterized by being inadequate, feeling guilt, envy, and the nurse's attitudes that can give sense of desperation or hope. The women's experiences affect their everyday lives and their emotions make them isolate themselves. Conclusion: Motherhood is a central part of the woman's life and identity, to be infertile is perceived as to major pressure that may result in mental stress. To reduce the infertile women´s suffering and mental stress it is important that the nurse shows empathy and understanding

▼ Si la fécondation in vitro est une innovation biomédicale récente, datant seulement de 1978, le recours à l’Assistance Médicale à la Procréation (AMP) est rapidement entré dans les mœurs, au point que ses procédés participent aujourd’hui à plus de 3 % des naissances françaises. Les cliniciens et les biologistes de la reproduction jouent un rôle incontournable dans ce champ. Chefs d’orchestre de la rencontre des gamètes, ils sont également les principaux acteurs décisionnaires de l’accès et de la poursuite des traitements de l’infertilité. Pourtant, leur pouvoir décisionnel a moins été étudié que les conséquences de leur intervention en termes de vécu de l’infertilité, de parenté, de filiation et de genre. C’est précisément ce manque que cette recherche tente de pallier. Au croisement des sociologies de la santé, des professions, des organisations et de l’action, cette étude vise à éclairer la multiplicité des pratiques et des représentations des spécialistes de l’infertilité à travers le prisme des décisions médicales. Elle repose sur deux enquêtes, menées pendant plus de deux ans au sein de plusieurs centres d’AMP de la région parisienne, comprenant des observations de consultations et de réunions pluridisciplinaires, et des entretiens avec des patients et des spécialistes de l’infertilité. S’appuyant sur ce corpus de données, et guidée par un souci constant d’articuler les dimensions micro et macro sociologiques, cette investigation s’attache d’abord à resituer les contextes globaux et locaux dans lesquels s’inscrivent les choix des professionnels, puis à analyser les décisions relatives à l’accès à l’AMP et aux techniques médicales. In fine, cette recherche montre que la sélection des patients et les stratégies médicales, structurées dans des cultures propres à chaque centre, apparaissent comme des compromis trouvés par les acteurs qui composent avec un ensemble de contraintes pour adopter des pratiques en adéquation avec leurs valeurs et leurs représentations.
The in vitro fertilization is a recent biomedical innovation, dating from 1978. Nevertheless, the use of Assisted Reproductive Technologies (ARTs) become widely accepted, to the point where these technics contribute today to more than three per cent of French childbirth. Clinical practitioners and biologists play a key-role in this field. They are in charge of the gametes meeting and they are also the main decision makers on the access and the pursuit of infertility treatments. However, their decisional power has been less studied than the consequences of their intervention in terms of infertility experience, kinship, filiation and gender. ▼ Klinefelter Syndrome (KS) is a sex chromosome abnormality causing a variety of clinical features and infertility. KS occurs when a male individual has an extra X chromosome resulting in a karyotype of 47,XXY. Much research has been done to look at the fertility of men with KS and in the mid-1990s a surgical technique, testicular sperm extraction (TESE), was shown to be effective in preserving the fertility of some men. Less is understood about the experiences of the men who utilize this procedure. The purpose of this study was to explore the experiences of men with KS who have undergone TESE to understand the decision making process and to identify opportunities to support men with KS undergoing this process. Individuals 18 years and older with self-reported KS (n=2) were recruited from an online support network and by word-of-mouth to participate in a semi-structured interview focusing on the feelings about TESE, the challenges they faced with their condition, and the supports they utilized. The main motivation for undergoing TESE was a desire for biological children. Participants reported a number of different challenges including: coping with the clinical features of KS phenotype, acceptance of their infertility after a failed TESE procedure, passing on KS and the need for hormone

supplementation. The supports these men utilized included: parents, a partner, and other non-familial supports, such as physicians and online support groups. None of the men in this study were offered an appointment with a genetic counselor. However, genetic counselors may have a role in the care of a man with KS by helping men with KS through the decision making process to undergo TESE, setting reasonable expectations for the procedure and providing support after the procedure.

▼ Infertility is the inability to conceive after at least 12 months of unprotected sexual intercourse (Dooley, Nolan, & Sarma, 2011; Kelly-Weeder, 2012; World Health Organization [WHO], 2016). It is estimated that 8-12% of all heterosexual couples, or 80 million people worldwide, are infertile (Leon, 2010; WHO, 2002). Research on
infertility has given preference to the experiences of women and couples who seek medical assistance for
infertility (Almeling & Waggoner, 2013), while research on
infertility experiences of heterosexual men are still rare given that approximately 50% of
infertility cases involve men’s
infertility (Culley, Hudson, & Lohan, 2013; Pacey, 2009). The current study assessed the diverse experiences of eight men through their process of
infertility utilizing Grounded Theory to analyze and establish a theoretical understanding of the experiences. The study found that the experiences of men with
infertility differ based upon whether they were diagnosed with male-factor
infertility, or female-factor or unknown
infertility. Men with male-factor
infertility were found to experience a response to diagnosis and treatment which included more avoidance and distancing behaviors in response to distress and emotional pain. Men diagnosed with female-factor or unknown
infertility tended to respond to diagnosis and treatment with active coping skills which sought to find a resolution to the problem of
infertility as a means of working to support their wives. Themes that arose among the participants was the impact of societal messages, responses to diagnosis, responses to treatment, impact on relationships, impacts on the self, and recommendations from the men to medical and mental health professionals. Variables which were found to impact men’s adherence to the model included racial and ethnic identity, income, access to treatment, and identity development. Clinical implications for the findings of this study and future research directions are also discussed.

▼ It is well documented that fertility in pigs is reduced in summer and fall compared to winter and spring. Termed as seasonal
infertility, problems with follicular development and corpora lutea formation and other unknown factors can combine to delay puberty in gilts, increase wean to estrus interval in sows, and increase rates of pregnancy failure and reduced litter size. The array of seasonal problems associate with the confounded effects of both heat stress and changing photoperiod. Although the modern domesticated pig is considered a “non-seasonal” breeder, its ancestor, the European wild boar, and other well characterized seasonal species, breed seasonally in response to changing photoperiod and hot temperature. Perception of photoperiod through the pineal gland alters patterns of melatonin which modulate the hypothalamic-pituitary axis and function of the ovary to control reproduction. Seasonal
infertility problems are then at least partially mediated, through the seasonal changes in the duration of nighttime secretion of melatonin, which is shortest in summer and slowly increases into fall.
Because both photoperiod and heat stress are associated with seasonal
infertility, two studies were designed to test whether short-term supplemental melatonin given to mimic the extended nighttime melatonin pattern observed in the peak fertility season of winter, could improve fertility in summer and fall under periods of heat stress. In both studies, exogenous melatonin was fed during proestrus and into early gestation, coinciding with the follicular and early luteal phases. Experiment 1 was performed in a single replicate using prepubertal gilts (n=36). Females were randomly assigned by age and weight and allocated in a 2 x 3 factorial treatment design to receive Melatonin (MEL, 5mg) or Control (CON, placebo) once a day while housed in one of three environmental rooms (32 °C) that provided: 1) 8 h of light and heat (8H); 2) 16 h light and heat (16H); or 3) 24 h light and heat (24H). All gilts received an i.m. injection of P.G. 600, fence line boar exposure, and artificial insemination at estrus. Following P.G. 600, ovaries were assessed by ultrasound and reproductive tracts collected to assess pregnancy, litter size, fetal and placental measures at 33 days of gestation. Continuous and categorical data were analyzed using the GLM and GENMOD procedures of SAS for the main effects of treatment and room. Temperature, humidity, ammonia and light intensity for all the three rooms were assessed. There was no effect of MEL or room on estrus (91.6 ± 11.8%), or on follicle development (14.3 ± 1.5 large follicles/gilt in 91.6 ± 15.9% of the gilts). However, there was a tendency (P = 0.08) for MEL to improve pregnancy rate compared to CON (87.8% vs. 63.3%) but with no effect of room. The number of fetuses, healthy fetuses, abnormal fetuses and placental efficiency were not affected. The results of this study suggest that P.G. 600 alleviates the effects of heat stress on expression of estrus but only melatonin had beneficial effects…

▼ This thesis details a series of experiments investigating infertility in mated sows. The particular problem of delayed return to oestrus in sows following a normal mating to a fertile boar, was studied. The vaginal biopsy and ultrasonic methods of pregnancy diagnosis were tested for use in diagnosis of pregnancy in herds where a large proportion of sows showed a delayed return to oestrus. These methods were found unsuitable in this instance and so plasma progesterone and oestrogen levels were examined in sows,18 days after mating, in an attempt to identify affected sows. Facilities for hormone analysis were established and workable assays for progesterone, oestrogen and corticoid levels developed.
Two laboratory experiments accompanied these field trials. The first was to study the temporal relationships between progesterone, oestrogen, and corticoid levels during the oestrous cycle and early pregnancy in sows. The second was to look at the effects of ACTH administration on plasma progesterone, oestrogen, corticoid, and LH levels and upon oestrus and ovulation.
These experiments are described and the results discussed in this thesis. An integrating discussion and summary is included to link each section and to combine the conclusions of this thesis.
