To investigate chloroform (CETC) and methanol (METC) extracts of leaves of Trichilia connaroides (Wight.andArn.) Bentv. for antihyperlipidaemic activity in normal, hypercholesterolemic and hyperhomocysteinemic animals, the possible mechanism of lipid lowering property, antioxidant effect in hypercholesterolemic, hyperhomocysteinemic and diabetic animals and on the secondary complications of diabetes mellitus. A dose of 100mg/kg body weight of extracts was chosen for the study. METHODS Extraction of freshly collected leaves of Trichilia connaroides was carried out by conventional method and chemicals tests were performed to confirm the presence of phytochemicals in the extracts. Test animals were rendered hypercholesterolemic by maintaining them on highfat diet (HFD) in place of normal diet, hyperhomocysteinemic, by oral administration of L-Methionine (1gm / kg) and diabetic by intraperitonial administration of streptozotocin (60mg/kg). Lipid lowering activity of extracts were assessed in normal,hypercholesterolemic and hyperhomocysteinemic animals by measuring the serum triglyceride (TG), total cholesterol (TC) and high density lipoprotein cholesterol (HDL-c). Low density lipoprotein-cholesterol (LDL-c) and very low density lipoprotein-cholesterol (VLDL-c) were calculated. Hypercholesterolemia progression test and hypercholesterolemia regression test were performed to understand the mechanisn of serum cholesterol reduction, and fructose load test and olive oil load test were performed to understand the mechanism of triglyceride reduction. Antioxidant activity of extracts in pro-oxidant state of hypercholesterolemia, hyperhomocysteinemia and diabetic animals were determined by assessing levels of products of lipid peroxidation (LPO) (expressed as MDA), superoxide dismutase (SOD), catalase (CAT) and glutathione reductase (GSH) in the heart homogenate of hyperhomocysteinemic and in the liver homogenate of hypercholesterolemic and diabetic animals.

▼ Hypercholesterolemia is one of the most prevalent, yet underdiagnosed diseases faced by the medical community today. Its prevalence can largely be attributed to diet, lack of exercise, and lifestyle choices such as smoking or drinking, but there is also a genetic component. Familial Hypercholesterolemia is the genetic disorder in which a person is unable to properly eliminate levels of low-density lipoprotein, mostly due to an ineffective receptor in the liver. Hypercholesterolemia has been positively correlated with the prevalence of cardiovascular disease, and patients with the severe homozygous familial hypercholesterolemia typically have abbreviated lifespans. In these situations, and also those less acutely dire, it’s necessary to rely on medication to help maintain one’s cholesterol levels to within low risk ranges. High does statin therapy has been shown to be the most effective therapy for maintaining LDL cholesterol. It has become the standard regardless of the cause of hypercholesterolemia because of its few side effects, its high tolerability, its ease of administration, its safety, and most of all because of its immense efficacy. This has not, however, prevented the exploration into other types of cholesterol therapies that may work in concurrence with statins. Drug classes such as PCSK9 inhibitors, ApoB inhibitors, MTP inhibitors, and thyromimetics have all been explored with varying success. Each of these potential therapies has a separate mechanism of action, allowing for modulation in conjunction with statins. PCSK9 inhibitors and ApoB inhibitors appear to provide the most upside by virtue of LDL lowering capabilities, followed by a drug known as ezetimibe that reduces dietary cholesterol uptake in the gut. MTP inhibitors have been shown to be effective therapies for homozygous familial hypercholesterolemia specifically due to their function of lowering LDL particle creation rather than LDL receptor number or function as statins and PCSK9 inhibitors do. Thyromimetics have yet to yield an effective therapy for cholesterol treatment, but the hope remains alive that this could come to fruition in the future. ▼ Cardiovascular disease (CVD) is a major contributor to chronic disability and death globally while hypercholesterolemia is a major independent risk factor. Because of adverse effects of currently used CVD drugs (e.g. statins), safer alternatives including natural products might be considered as options for treatment of hypercholesterolemia. Studies have shown that treatment with flaxseed lignan, mainly as secoisolariciresinol diglucoside (SDG), can safely decrease blood cholesterol. However, purified SDG production is expensive; processes that concentrate the natural SDG polymer to 50% or more SDG from flax yield a more cost effective product. Unfortunately, the relative bioavailability of pure SDG and SDG polymer are unknown and the bioactivity of these two forms of SDG are also unknown. This study compared the relative pharmacokinetics of purified SDG and SDG polymer in a single oral dose (40 mg/kg SDG dose) in rat. The concentration of SDG and SDG polymer including secoisolariciresinol (SECO), enterodiol (ED), and enterolactone (ENL) were determined in plasma after administration. The aglycone SECO could only be detected in early plasma samples at 0.25 to 4 h after administration. Unconjugated ED was detected and quantified after 8 (mean ± SD: 3.4 ± 3.3 ng/mL) and 12 h (6.2 ± 3.3 ng/mL) in most subjects, while total ED (unconjugated ED and ED conjugate) was detected in plasma from 2 h to 16 h in most rats. The time of the maximum plasma concentration (Tmax) of total ED in rats fed SDG was 11.7 ± 1.1 h and 10.9 ± 1.9 h for rats fed the SDG polymer. The maximum concentration of total ED (Cmax) for rats fed SDG was 262.2 ± 170.8 ng/mL while for those fed SDG polymer Cmax was 207.2 ± 115.5 ng/mL. Total enterolactone (ENL) was determined for all plasma samples. Tmax of total ENL in rats fed purified SDG was 12.6 ± 1.5 h while for those fed SDG polymer Tmax was 12.7 ± 3 h. Cmax of total ENL in the purified SDG and SDG polymer fed rats was 81.6 ± 23.8 ng/mL, and 65.9 ± 19.6 ng/mL, respectively. The relative bioavailability of total ED and ENL of SDG polymer is 111% and 89%, respectively, when compared to purified SDG. No significant difference in Cmax, Tmax, and AUC of total ED and ENL of purified SDG and SDG polymer was found. In addition, we investigated the effects of chronic daily oral purified SDG (6 mg/kg) or an equivalent dosage of SDG polymer in female Wistar rats fed a 1% cholesterol diet for 1 week before initiation of purified SDG or SDG polymer doses for 23 days. A significant reduction in normalized liver weight was observed in the group treated with purified SDG when compared to high cholesterol control. Both purified SDG and SDG polymer not significantly, but clinically induced a reduction in serum TAG (19% and 15%, respectively) and increase in HDL-C (15% and 24%, respectively). This thesis studies individuals with heterozygous familial hypercholesterolemia (the most common metabolic disorder that is inherited in an autosomal dominant trait, is due to a mutation in the LDL receptor), who are followed in the Lipid Clinic of Cardiology Department at Hippocration University Hospital of Athens, starting monitoring from January 1987 to December 2002. During the extended monitoring, a re-evaluation and recording of the terminal points of the study was performed at 15 (2002) and 30 years (2016). We recorded the lipid parameters, blood pressure (BP), BMI, waist/hip ratio, presence of diabetes, smoking, dietary habbits, their medication, physical exercise and all known prognostic factors of cardiovascular events. All individuals performed a stress test, according to the Bruce protocol. The following parameters of the test were recorded: duration in sec, exercise capacity in METs, the predicted heart rate (HR,%), systolic BP at rest, at 3, 6 and 9 minutes and its maximum value, diastolic BP and HR at the same times and finally the recovery HR at maximum stress test. Individuals with familial hypercholesterolemia as in our study are of high cardiovascular risk. The primary end point of our study was the first cardiovascular event (fatal or non MI, unstable angina or sudden death). We found that smoking, hypertension, increased LDL/HDL ratio and fibrinogen levels were the most important predictors of cardiovascular events during 15 years of monitoring. Especially the LDL/HDL ratio >8 was associated with 66% greater risk while with coexistance of hypertriglyceridemia >200mg/dl the risk is tripled. After 15 years of observation we found that the maximum exercise capacity, HR recovery in the first minute and the levels of pulse pressure (difference between systolic-diastolic BP) are important predictors for coronary heart disease in patients with heterozygous hypercholesterolemia. There was a gradual increase in the cumulative risk of cardiovascular events as it grows by 15%, 24% and 32% after 10, 20 and 30 years of observation, respectively. There was a significant risk association with male gender (3.2 times higher risk), older people, menopause and the increased ratio of waist/hip circumference. Also the only lipid parameter that was higher in people who developed cardiovascular disease was lp (α). After 30 consecutive years of monitoring of patients with heterozygous hypercholesterolemia we conclude that the maximum exercise capacity (METs), the HR and its changes in comparison with the rest as well as the maximum values of systolic and diastolic BP, all are strong predictors for Cardiovascular Disease.

▼ Abstract To facilitate the diagnosis of hypercholesterolemia, we wanted to create a simple and rapid method for diagnosing familial hypercholesterolemia in a homogenous population. The PCR method for the FH-Helsinki mutation detected 25 FH-Helsinki positive patients, two of whom had no clinical signs of FH, but had a positive family history for the disease. The method is exceptionally useful in Northern Finland, where 62% of the FH patients carry the FH-Helsinki mutation. The role of polymorphisms and mutations of the apo B particle as etiologic factors of hypercholesterolemia was studied in a population of moderately hypercholesterolemic individuals. The catabolism of the patients' own LDL was compared to that of a healthy and normocholesterolemic donor, and no major differences were observed. However, the presence of the XbaI cutting site was associated with elevated cholesterol values and a slightly lowered LDL catabolic rate. Patients homozygous for the EcoRI cutting site also had a slow LDL catabolic rate and slightly elevated cholesterol values. The MspI and Ins/del polymorphisms of the apo B particle were not associated with variations in LDL catabolism. The e 4 allele of apolipoprotein E was slightly more frequent in our hypercholesterolemic population than in the average population. The lipid values did not differ significantly between the apo E phenotypes in moderately hypercholesterolemic individuals, nor could we detect any differences in the catabolic rates of their LDL according to the apo E phenotype (individuals with the phenotype apo E 2/2 were excluded from the study). In our population of CAD patients, the frequency of the e 4 allele was lower than in CAD populations from Southern Finland (0.23 vs. 0.32), suggesting that apo E 4 is not so strongly associated with coronary disease in Northern Finland as in other populations. The E 4 phenotype was associated with slightly smaller LDL cholesterol reductions by colestipol and lovastatin treatment compared with patients with the phenotype 2/3. The lipid risk factors of male and female CAD patients were studied in a group of patients admitted to one ward of the Oulu University Hospital. We found the males to have the typical high LDL cholesterol and low HDL cholesterol lipid pattern, but women with two- or three-vessel CAD had high LDL and low HDL cholesterol associated with high VLDL lipids, and hypertension, diabetes or smoking. Pharmacological treatment of hypercholesterolemia was studied by comparing lovastatin to colestipol, and in a separate study where a new drug, enprostil was used. Enprostil, whose main effect is on the gastrointestinal tract, would be a useful alternative for long-term treatment of hypercholesterolemia. Unfortunately, however, gastrointestinal side-effects limit its long-term use. Colestipol reduced plasma LDL cholesterol and elevated plasma HDL cholesterol and triglycerides, but it, too, caused gastrointestinal side-effects. ▼ Abstract: Total parenteral nutrition (TPN) consists of the intravenous administration of the necessary ingredients for the survival and maintenance of the patient's homeostasis. Over the years this technique has been increased and contributes to the treatment of several types of patients including the seriously ill. However, TPN may be related to alterations in the lipid and hepatic profile. The most common hepatic and biliary complications in PN are: cholestasis and hepatic steatosis. Changes in the lipid profile are more related to hypertriglyceridemia and low HDL values, mainly in patients in inflammatory response. The aim of this study was to evaluate the changes in the lipid and hepatic profile of pediatric patients submitted to TPN therapy hospitalized in the Clinical Hospital of the State University of Campinas - Unicamp. It is a descriptive and historical cohort study, in which 195 pediatric patients (January 2012 to February 2016, from 0 up to 19 years old) who received TPN were evaluated. The hepatic profile was assessed from alanine aminotransferase (ALT), aspartate aminotransferase (AST), alkaline phosphatase, gamma glutamyl tranpeptidase (GGT), total bilirubin. The albumin and transthyretin were also evaluated. The lipid profile was evaluated from values of cholesterol, HDL and triglycerides (TG). Laboratory and clinical nutritional status, evaluation and outcome were also measured. According to the hepatic profile, the majority of patients presented normal values of AST (79.2%), ALT (74.9%), GGT (56.4%) and alkaline phosphatase (68.1%). Total bilirubin was altered in 68.5% and transthyretin (87.3%) and albumin (65.1%) were low in most patients. The data regarding the lipid profile showed normal cholesterol in 84.6%, low HDL in 94.5%, high TG in 81.5% and high C-reactive protein in 92.7% of patients. With multivariate logistic regression it was possible to identify that the chances of patients under NPT with high GGT values are 2.46 times higher if the patient is older than two years of age, 2.26 times higher if not a surgical patient, 3 times higher if you have a high cholesterol and 2.39 times higher if you have high triglycerides. The chance of elevated ALT is 4.57 times higher in patients who present high cholesterol. It was concluded that the prevalence of elevated ALT, AST, GGT and alkaline phosphatase were within the percentages described in the literature. From the association of alterations of the hepatic and lipid profile it was possible to verify that patients with hypercholesterolemia are more likely to make alterations of ALT, and GGT. Patients with hypertriglyceridemia are more likely to have GGT changes. ▼ Abstract: Cardiovascular diseases (CVD) are the major cause of death in the occidental world. Atherosclerosis is the main process responsible for the pathogenesis of these diseases. Metabolic disturbances such as dyslipidemias, hypertension, insulin resistance and obesity are well established atherosclerosis risk factors. Currently, different pharmacological and dietetic interventions have been used to reduce and avoid CVD. Caloric restriction is one of these strategies, which is associated with increases in life span and reductions of age related chronic diseases. The mitochondrial uncoupler 2,4 dinitrophenol (DNP) has been considered as a caloric restriction mimetic tool. We have demonstrated that a mouse model of human familial hypercholesterolemia exhibits metabolic and redox disturbances highly relevant for metabolic syndrome and atherosclerosis. Thus, the aim of this study was to evaluate the effects of caloric restriction and mild mitochondrial uncoupling induced by DNP on the metabolism and atherosclerosis development in hypercholesterolemic LDL receptor deficient mice (R0). Thus, R0 mice were compared after 3 months in three conditions: R0 fed ad libitum (control), R0 under caloric restriction (R0-RC) obtained by providing food in alternate days, and R0-DNP fed ad libitum and treated with DNP 1 mg/L in the drinking water. The caloric restriction regimen, despite reducing food intake, induced unexpected adverse metabolic effects, namely, increased plasma total-, VLDL- and LDL-cholesterol, and increased visceral and subcutaneous adipose tissue masses. In addition, R0-RC mice became hyperglycemic, hyperinsulinemic, glucose intolerant and insulin resistant. Moreover, R0-RC mice presented elevated plasma levels of the inflammatory markers TNF? and C-reactive protein. Some benefits regarding systemic and tissue redox state were observed in R0-RC, however, atherosclerosis lesion areas were increased about 3 fold in R0-RC mice. DNP treatment had minimal effects on the lipid and glucose metabolism of R0-DNP mice. It decreased plasma free fatty acids, but did not change other plasma lipids, inflammatory markers, glucose homeostasis, and the size of adipose depots. On the other hand, marked effects of DNP treatment on the tissue redox state were observed, namely, reduction in the generation of mitochondrial oxidative reactive species and in the liver content of carbonyl protein. The spontaneous atherosclerosis development was significantly reduced in the R0-DNP mice, independently other well established risk factors such as plasma cholesterol and inflammatory mediators. Therefore, the caloric restriction regimen is not indicated for the condition of familial hypercholesterolemia due to LDL receptor deficiency. ▼ Rationale: Risk factors for cardiovascular disease (CVD) are both modifiable and non-modifiable. Modifiable risk factors associated with CVD poor diet quality, physical inactivity and tobacco consumption. Individuals with familial hypercholesterolemia (FH) have inherited genetic disorders affecting low-density lipoprotein receptors. These individuals are predisposed to a greater risk of CVD due to increased low-density lipoprotein cholesterol (LDL-C) levels. Dietary modifications remain an important part of CVD risk reduction in FH. Methods: This study was a pre and post intervention design that ran over eight weeks. We recruited FH participants through GP practices in Dunedin and Southland. Dietary assessment for each participant was carried out using a prompted 24-hour recall and a semi-quantitative FFQ. The FFQ was used to create a simulated 7-day diet record for each participant. This was done at baseline and repeated at the end of the study. Nutrient analyses of diets were done using Kai-culator©. Blood lipids were compared pre and post intervention. The blood lipid measurements were taken for total cholesterol (TC), high density lipoprotein cholesterol (HDL-C), LDL-C, triglycerides (TAG), and TC:HDL ratio. Spearman’s correlations and paired t-tests were carried out to determining changes in nutrient intakes and their associations with lipid changes. Results: No significant differences were found between pre and post mean levels of TC, HDL-C, LDL-C, TAG or TC:HDL-C. There were significant reductions in intakes of biscuits (p=0.002), eggs and egg dishes (p=0.027), bread based dishes (p=0.012) and processed meat (p=0.008). There were also significant increases in intakes of fish and seafood (p=0.003), nuts (p=0.002),fruit (p=0.012), and vegetables (p=0.016) and table spread (p=0.048). This related to a significant reduction in intakes of saturated fatty acids (p<0.001), cholesterol (p=0.003) and an increase in monounsaturated (p=0.01) and polyunsaturated fatty acids (p=0.035). Qualitative feedback indicated that the majority of participants found the study to be helpful in increasing their understanding of heart healthy eating habits, and in changing their dietary practices. Conclusions: This short term intervention showed that tailored dietary advice was effective in changing nutrient and food group intakes towards a more optimal diet. ▼ Hypercholesterolemia, a condition marked by high levels of cholesterol in the blood affects over 30 million adults in the USA and is responsible for 2.6 million deaths and 29.7 million disability life adjusted years worldwide. It is caused by genetic mutations but environmental factors such as diet, smoking, age and sex may aggravate the condition. The significant mutations are seen in APOB, LDLR, LDLRAP1, and PCSK9 genes. The high plasma cholesterol tends to deposit in organs leading to atherosclerosis, tendon xanthomas and athropathies. Diagnosis is by measuring LDL levels with fasting LDL levels >190 mg/dL in adults in the absence of any other causes leading to a suspicion of the disease. Genetic tests are used to confirm familial hypercholesterolemia. First line treatment is lifestyle adjustments including diet, smoking cessation and regular physical exercise. Pharmacological therapy includes statins alone or with adjuvants such as Ezetimibe, Niacin, Fibrates, Bile acid sequestrants or the newer Lomitapide and Mipomersen. A new class of drugs called PCSK9 inhibitors have been developed to treat hypercholesterolemia. They block the PCSK9 enzyme that binds to LDL receptors leading to their degradation. The effect of these drugs is less LDLR degradation, therefore more available receptors to bind and process cholesterol. Currently in the market are the monoclonal antibodies against PCSK9 namely Evolocumab and Alirocumab. Bococizumab has passed Phase II clinical trials and is proceeding to Phase III before approval for market distribution. Despite their excellent efficacy in lowering LDL, these drugs are relatively expensive and available only as subcutaneous injections. This challenge is being address by research into other inhibitors of this enzyme. They will be in the form of adnectins, small molecules, RNA interference and vaccines. This these will discuss further the currently available PCSK9 inhibitors, those under development as well as giving an overview of hypercholesterolemia, especially the familial type. ▼ Hypercholesterolemia is a significant risk factor in the development of cardiovascular disease and is associated with chronic leukocyte adhesion in the microvasculature. While the underlying mechanisms behind this have yet to be determined, it may be possible that hypercholesterolemia impairs the fluid shear stress (FSS) inactivation of neutrophils through the rigidifying effect of cholesterol on membrane fluidity. FSS restricts surface expression of CD18 integrins through cathepsin B (ctsB) proteolysis, which minimizes neutrophil adhesivity. If hypercholesterolemia blocks FSS mechanotransduction, then the inhibition of CD18 cleavage may link pathologic blood cholesterol elevations with dysregulated neutrophil adhesion. We hypothesized that elevated cholesterol contributes to dysregulated neutrophil adhesion by impairing ctsB FSS-induced CD18 cleavage through membrane fluidity changes. In the first part of this study, we demonstrated that FSS-induced CD18 cleavage is a robust response of neutrophils and involves selective cleavage of macrophage 1-antigen (Mac1) through ctsB proteolysis. The second part of this study confirmed that ctsB regulates neutrophil adhesion through its proteolytic actions on Mac1, an important integrin involved in adhesion and chemotaxis. Specifically, ctsB accelerated neutrophil motility through an effect on Mac1 integrins during pseudopod retraction. Furthermore, by using a flow-based assay to quantify the mechanoregulation of neutrophil adhesivity, we demonstrated that FSS-induced ctsB release promoted neutrophil detachment from platelet-coated substrates and unstimulated endothelium. For the third part of this study, we linked cholesterol-related membrane fluidity changes with the ability of FSS to restrict neutrophil adhesion through Mac1. We also determined that pathologic cholesterol elevations associated with hypercholesterolemia could block FSS-induced Mac1 cleavage and were linked to disrupted tissue blood flow. This was accomplished using low-density lipoprotein receptor deficient (LDLR-/-) mice fed a high-fat diet. Ultimately, the results provided in the present study confirmed that cholesterol-related changes in membrane fluidity blocked the ability of ctsB to regulate neutrophil adhesion through FSS-induced Mac1 cleavage. This implicates an impaired neutrophil FSS mechanotransduction response in the dysregulation of neutrophil adhesion associated with hypercholesterolemia. Since dysregulated adhesion may be one of the earliest upstream features of cardiovascular disease associated with hypercholesterolemia, the present study provides a foundation for identifying a new mechanobiological factor in the pathobiology of microcirculatory dysfunction.

▼ Cardiovascular disease (CVD) is one of the leading causes of death in the developingworld, killing one Australian every twelve minutes (www.heartfoundation.com.au).Hypercholesterolemia and long term elevated LDL-cholesterol (LDL-C) levels arelinked to the formation of atherosclerotic plaques, which can eventually lead tomyocardial infarction or stroke. It is therefore essential to lower LDL-C levels in orderto reduce cardiovascular (CV) risk.The low density lipoprotein receptor (LDLR) removes and degrades LDL particlesfrom circulation. Proprotein convertase subtilisin kexin type 9 (PCSK9) regulatescholesterol metabolism by degrading the LDLR. A number of pharmaceuticalcompanies have created therapeutic agents known as PCSK9 inhibitors, which bind toand block PCSK9. PCSK9 inhibitors in combination with existing lipid loweringtreatments have been shown to significantly decrease circulating LDL-C levels in awide variety of subjects. The aim of thesis was to examine the biology of PCSK9 insubjects with familial hypercholesterolemia (FH), in subjects at high CV risk and insubjects treated with a CETP inhibitor.The ex vivo and in vitro studies in this thesis demonstrated that statins increased,recombinant PCSK9 decreased, and the PCSK9 inhibitor restored LDLR cell surfaceexpression in all heFH and receptor defective hoFH subjects. However, LDLR cellsurface expression was not affected in receptor negative hoFH subjects. In subjects athigh CV risk the administration of ascending atorvastatin doses increased plasmaPCSK9 levels at each individual dose. Subjects with diabetes who were treated withatorvastatin also had significantly higher plasma PCSK9 levels compared to subjectswithout diabetes. However, the administration of the CETP inhibitor, torcetrapib hadno effect on circulating PCSK9 levels in subjects with or without diabetes.These results from this thesis provided a novel insight into PCSK9 biology and haveimportant clinical implications for subjects diagnosed with FH and in subjects at highCV risk. The results from these studies support current clinical trial results and providestrong in vitro and ex vivo evidence for the use of PCSK9 inhibitors in all heFH,receptor defective hoFH and statin treated subjects at high CV risk with or withouttype 2 diabetes.

▼ Background: Familial Hypercholesterolemia (FH) is estimated to affect one in every five hundred people. FH is a lifelong condition that if untreated can lead to premature coronary heart disease (CHD) and possibly death. In spite of the high prevalence, internationally FH is both underdiagnosed and undertreated, leaving a large population of people at a greatly increased risk for premature CHD. There are effective treatments available that can significantly improve long term survival. Methods: The prevalence of FH at CCHMC for individuals between age 2 and 21 was calculated using i2b2, a research data warehouse at CCHMC. A chart review was completed on 180 patients who were seen between 1/1/2011 and 7/15/2013 at CCHMC and met the inclusion criteria. A 42 item survey was developed to evaluate parental knowledge, attitudes and interest in genetic counseling and genetic testing. 171 surveys were sent out, 86 included an educational brochure created by the FH Foundation and 85 did not include the brochure. Results: The prevalence of FH in the selected population was 0.04%. 1% of individuals between the ages of 9 and 11 received cholesterol screening at CCHMC. Of the 180 individuals included in the chart review, 146 (81%) had seen a lipid specialist, 47 (26%) had a diagnosis of FH, 105 (58%) had a diagnosis of dyslipidemia or hyperlipidemia, 28 (16%) had been prescribed a statin, 0 (0%) had discussed FH with a genetic counselor. Of the 171 surveys sent, 76 surveys were completed with a response rate of 44%. Respondents whose child had been prescribed a statin had a higher mean knowledge score than respondents who children had not been prescribed a statin (p = 0.0267). If the respondent reported that a healthcare provider had talked to them about FH there was a trend towards significant associated with knowledge score (p = 0.0606). Receiving the educational brochure, having a diagnosis of FH, reported use of the internet to look up FH, annual house hold income and a reported family history of hypercholesterolemia were not associated with increased knowledge. Conclusions: Results from this study supports existing data regarding the under diagnosis of FH and the lack of adherence to routine cholesterol screening. Participants who reported a health care provider had previously spoken to them about FH were more knowledgeable compared to those who had not. Given the important role healthcare providers play in patient education, incorporating genetic counselors into the counseling and risk assessment of these patients and families may increase overall disease knowledge as well as adherence to cholesterol screening and genetic testing guidelines. L’hypercholestérolémie familiale à transmission autosomique dominante (ADH), caractérisée par une élévation des taux plasmatiques en cholestérol total et LDL-C, est due à des altérations de 4 gènes : LDLR, APOB, PCSK9 et APOE. L’objectif principal de cette thèse est d’identifier de nouveaux gènes impliqués dans l’ADH. L’identification de nouveaux gènes sera suivie de l’étude des mécanismes physiopathologiques liés à leurs mutations. Un deuxième objectif est de calculer le score génétique (GRS) chez tous les individus appartenant à 5 familles où une mutation FH a déjà été identifiée afin de déterminer si une forme polygénique expliquerait les cas de phénocopies observés. Parallèlement, nous avons mené une étude dans la population libanaise caractérisée par une fréquence élevée de dyslipidémie et qui représente un outil d’étude remarquable au plan génétique du fait de l’existence d’une forte homogénéité du fond génétique.Ce projet de recherche a permis de révéler un gène candidat pouvant être impliqué dans l’ADH : LRP6. De plus il a permis de remettre en question le rôle du récepteur LRP6 jusqu’à présent considéré comme un protagoniste important dans l’internalisation des LDL. Des études supplémentaires sont encore nécessaires afin de confirmer ou non l’implication de ce gène dans l’ADH et de déterminer son rôle exact dans le métabolisme du cholestérol. Concernant le score polygénique, nous avons montré que le GRS ne peut pas être considéré comme un outil de diagnostic pour différencier les sujets avec une hypercholestérolémie monogénique de ceux avec une hypercholestérolémie polygénique et ne peut pas être utilisé pour expliquer les cas de phénocopies

Atherosclerosis and its cardiovascular complications are the leading causes of morbidity and mortality in industrialized countries. Hypercholesterolemia is one of the major cardiovascular risk factors and it affects one in 20 subjects in the general population. Autosomal dominant hypercholesterolemia (ADH), characterized by elevated plasma total cholesterol and LDL-C levels, is due to alterations in 4 genes: LDLR, APOB, PCSK9 and APOE. The fundamental work of Brown and Goldstein revealed the important role of the mutations in the LDLR gene in ADH and contributed to the development of a major class of cholesterol-lowering drugs: statins. Similarly, the discovery by Abifadel et al. in 2003 of the first hypercholesterolemic mutations of PCSK9 was the starting point of an adventure which resulted, 12 years later, in the development of a new class of cholesterol-lowering drugs: anti-PCSK9 antibodies. The main objective of this thesis is to discover new genes, major genetic factors and modifiers involved in ADH. The identification of new genes will be followed by the study of the pathophysiological mechanisms linked to their mutations. The published reports of the effectiveness of simvastatin in treatment of hypercholesterolemia were mostly conducted in western populations, and only few studies in Asian or domestic populations have ever been reported. By regulations from Bureau of National Health Insurance, the effective dosage of lipid lowering agents should be started from lower dose, such as 20 mg of simvastatin per day. Whether this dosage of simvastatin is effective for treatment of patients with hypercholesterolemia and the efficacy and safety of such dosage are the objects of this study. Materials and Methods. After the approval of IRB in a medical center located at north Taiwan, a randomized 12-week study was conducted to evaluate the efficacy and safety of 20mg/day simvastatin treatment on hypercholesterolemia. By randomization 65 patients, followed up at cardiovascular outpatient department under the diagnosis of primary hypercholesterolemia, were enrolled into a 4-week washout period, and finally 49 intent-to-treat patients entered a 12-week treatment with 20mg simvastatin per day, given through oral routine in the evening. Demographic and laboratory data were obtained before and after treatment. The primary efficacy measure was the changes from baseline in lipid parameters. Tolerability was assessed in terms of the overall incidence of adverse experiences and the most commonly reported adverse experiences. Results. The per-protocol analyses included 39 hypercholesterolemic patients whom completed 12 weeks of therapy. Total cholesterol and LDL cholesterol at the end of the study period showed significant reductions by 22.5ï¼ (p<0.001) and 29.8ï¼(p<0.001), respectively. Triglyceride also showed a significant reduction by 31.8% (p=0.006), whereas total alkaline phosphatase and calcium showed a weak and insignificant change over the study period. The female group had significantly greater reduction in triglyceride than that in the male group, and the non-smoking group also had significantly greater reduction in triglyceride than that in smoking group after 12-week treatment. There were 17 studied cases (34.7%) had minor transient but clinical insignificant increases in serum aspartate aminotransferase and alanine aminotransferase, and 7 cases (14.3%) experienced symptom of painful muscle, of whom 3 cases (6.1%) dropped out this study. Conclusion. Our results, although obtained from a small scale of hypercholesterolemic patients, suggest a probable positive efficacy and good tolerability with only few minor side effects of simvastatin on blood lipids. A aterosclerose é uma doença inflamatória crônica e proliferativa que tem início quando fatores de risco alteram o endotélio vascular. As partículas da nanoemulsão lipídica LDE concentram-se em sítios inflamatórios e de intensa proliferação celular, como acontece nas lesões ateroscleróticas. A carmustina, um fármaco antiproliferativo usado na quimioterapia do câncer, não foi ainda explorada no tratamento da aterosclerose. Em trabalhos anteriores, mostrou-se que a associação com a LDE reduz drasticamente a toxicidade da carmustina, o que já foi demonstrado em pacientes com câncer avançado. O propósito do estudo é avaliar se a carmustina associada à LDE pode promover o efeito antiproliferativo nas lesões ateroscleróticas induzidas em coelhos. No presente trabalho, dezoito coelhos machos da raça New Zealand, receberam dieta rica em colesterol a 1% durante 8 semanas. Depois de 4 semanas foram divididos em dois grupos: grupo controle, que recebeu injeção endovenosa contendo apenas solução salina e grupo tratado, que recebeu LDE-carmustina na dose de 4mg/Kg semanalmente durante 4 semanas. Foram avaliados perfil hematológico, lipídico, bioquímico, ponderal e o consumo de ração. Após a eutanásia, foram medidas as lesões ateroscleróticas macroscópicas. Em seguida, o arco aórtico foi analisado por morfometria e por imunohistoquímica. Observou-se que não houve diferença no perfil ponderal e no consumo de ração entre os grupos de estudo. Não houve toxicidade hematológica, hepática e renal no grupo tratado. No perfil lipídico, ao final do estudo, as concentrações de colesterol total, não HDL-C e triglicerídeos aumentaram em todos os grupos. Portanto, o tratamento com LDE-carmustina inibiu as lesões ateroscleróticas em aproximadamente 90%, comparado ao grupo controle. LDE-carmustina também reduziu a presença de macrófagos, de células de músculo liso e das células reguladoras de linfócitos T na íntima arterial, bem como a expressão protéica de MMP9, das citocinas inflamatórias e dos receptores de lipoproteínas. O tratamento da aterosclerose induzida em coelho com LDE-carmustina resultou em marcante redução das lesões na aorta, da proliferação e invasão da íntima por macrófagos e células musculares lisas, características da doença, além dos fatores inflamatórios. Tendo em vista que, associada à LDE, a carmustina tem baixa toxicidade, a nova preparação LDE-carmustina tem grande potencial para a terapêutica das doenças cardiovasculares de natureza aterosclerótica

Atherosclerosis is a chronic inflammatory and proliferative disease that starts when risk factors alter vascular endothelium. The particles of the lipid nanoemulsion LDE concentrate on inflammatory sites and with intense cell proliferation, as occurs in atherosclerotic lesions. O pré e pós-condicionamento isquêmico são fenômenos cardioprotetores que limitam o dano tecidual no infarto agudo do miocárdio. Seus efeitos benéficos ocorrem através da ativação de complexas vias sinalizadoras intracelulares, que têm a inibição da formação do poro de transição de permeabilidade mitocondrial como efetor final comum. A presença de fatores de risco, como a hipercolesterolemia, pode interferir nos resultados finais quando se lança mão de tais medidas cardioprotetoras. Entretanto, há na literatura achados contraditórios que não mostram interferência do colesterol elevado sobre a cardioproteção do pré-condicionamento isquêmico. No pós-condicionamento isquêmico, mais recentemente descrito, as primeiras evidências também mostram resultados conflitantes, que carecem de mais estudos. A baixa disponibilidade do óxido nítrico presente na hipercolesterolemia está associada a níveis elevados de dimetilarginina assimétrica, um inibidor da óxido nítrico sintetase, e pode ser o responsável por abolir os benefícios de ambas as medidas cardioprotetoras aqui estudadas. Procuramos, neste trabalho, avaliar a influência da hipercolesterolemia sobre o pré e pós-condicionamento isquêmico, e correlacionar os níveis de colesterol total e dimetilarginina assimétrica com o tamanho do infarto do miocárdio experimental em ratos anestesiados. Nós encontramos um maior tamanho do infarto nos animais hipercolesterolêmicos apesar do uso destas medidas cardioprotetoras.

Ischemic pre and postconditioning are cardioprotective phenomena that limit heart tissue damage in acute myocardial infarction. Its beneficial effects occur through the activation of complex intracellular signaling pathways, which have the inhibition of the formation of the mitochondria permeability transition pore as common final effector. The presence of risk factors such as hypercholesterolemia may interfere with final results when it makes use of such cardioprotective measures. However, there are contradictory findings in the literature which show no interference of high cholesterol on cardioprotection of ischemic preconditioning. In ischemic postconditioning, more recently described, the initial evidences also shows conflicting results, which require further studies. The low availability of nitric oxide present in hypercholesterolemia is associated with high levels of asymmetric dimethylarginine, an inhibitor of nitric oxide synthase, and may be responsible for removing the benefits of both cardioprotective measures studied here. The objective of our study was to assess the effect of hypercholesterolemia on ischemic pre and postconditioning and correlate the levels of total cholesterol and asymmetric dimethylarginine with the size of experimental myocardial infarction in anesthetized rats. We found a larger myocardial infarction in the hypercholesterolemic animals despite the use of these cardioprotective measures.

Introdução: Os fitoesteróis (FE) são reconhecidos por reduzirem a concentração plasmática de LDL-colesterol, sendo importantes coadjuvantes no tratamento da hipercolesterolemia moderada. Entretanto, estudos publicados recentemente demonstram resultados conflitantes em relação à eficiência dos FE na prevenção da aterosclerose. Além disso, algumas investigações evidenciaram que o aumento da concentração plasmática de FE está positivamente relacionado ao risco de desenvolvimento de aterosclerose. Com a finalidade de elucidar a sua ação sobre esses parâmetros, o objetivo deste estudo foi avaliar os efeitos da suplementação de FE no desenvolvimento da aterosclerose em camundongos com ablação gênica para o receptor de LDL (LDLr-KO). Métodos: Os animais foram alimentados durante 16 semanas, com dieta rica em gordura (40% do valor calórico total da dieta), suplementada (grupo FE; 2%, n=10) ou não (Controle; n=10) com FE. Foram avaliadas as concentrações plasmáticas e hepáticas de colesterol, triglicérides, FE (beta-sitosterol, campesterol e latosterol). Na aorta dos animais, determinaram-se as concentrações de colesterol total, colesterol livre e éster e FE, além do infiltrado de macrófagos e infiltrado de lípides. Nos macrófagos do peritôneo dos animais, os quais assemelham-se aos presentes na artéria, avaliou-se a expressão de RNA mensageiro dos genes envolvidos no efluxo e influxo de colesterol (ABCA1, ABCG1, LOX1 e CD36). Também determinou-se as concentrações de FE no intestino e baço dos animais. Resultados: Conforme esperado, o consumo de FE induziu elevação plasmática dos principais FE, campesterol e de beta-sitosterol, reduzindo a concentração de colesterol no plasma. Houve aumento nas concentrações hepáticas de triglicérides e FE, entretanto, não foram observadas diferenças entre os grupos nas expressões de RNA mensageiro de genes lipolíticos (CPT, PPAR alfa) e lipogênicos (SREBP1-c, MTP, LXR e PPAR gamma) no fígado. Não houve, também, alteração no SREBP2, gene relacionado à síntese de colesterol. O conteúdo de colesterol total na artéria foi menor nos animais do grupo FE, não diferindo entre as formas livre e éster. As concentrações de FE na artéria foram iguais entre os grupos. A área de lesão no grupo FE foi menor em relação ao grupo-controle. A suplementação com FE induziu redução na expressão de RNA mensageiro de ABCG1, não interferindo na expressão dos outros genes estudados na artéria. Conclusão: Os achados deste estudo demonstram que a elevação de FE no plasma não induziu o seu acúmulo na parede da artéria e preveniu o desenvolvimento da aterosclerose
