▼ Background In Australasia, Head and Neck Cancer (HNC) is more common and survivable than ever before. As a result, the population prevalence of HNC patients (including HNC survivors) in our communities is ever-increasing. Most of these patients are, at least, partially dentate, and are at high risk of dental disease. Most will seek dental care in the private sector with general dentists. Problematically, little is known about the capacity of general dentists to provide treatment for these complex patients. This study, which is believed to be the first of its kind in this space, will seek to describe the knowledge, attitudes and behaviour of general dentists providing treatment for HNC patients. Methods A cross-sectional survey of a random and representative sample of 800 New Zealand general dentists was undertaken. The survey was self-administered, from a digital platform. Participants were invited into the study by email, and this email contained an embedded hyperlink to the survey. Data were collected on sample demographics and various aspects of the knowledge, attitude and behaviour of general dentists treating patients with a history of HNC. A total of 156 surveys were completed (20.4% response rate). Data was analysed using SPSS software (version 25), by the investigator. Qualitative interview of four key informants was undertaken to provide depth of understanding to the data. Results The majority (73.4%) of registered general dentists who responded to the survey invitation felt that providing dental treatment for HNC patients fell within their scope of practice, however one quarter of dentists had not seen any HNC patients during the previous year in practice, and only 1 in 20 had seen significant numbers (20 or more HNC patients). HNC patient flow was positively associated with the dentist being female, in a metropolitan-based practice, having had work experience in the public sector and currently working in both public and private practices. General dentists were found to have sufficient knowledge to treat HNC patients, in practise if not in theory. On average, dentists answered 9.6 questions correctly out of total of 15 questions (64% of questions answered correctly, on average). HNC patient flow over the preceding year, being aged 50 to 59 years, and having 30 to 39 years of clinical experience, were associated with superior knowledge. The study found that dentists lacked confidence in providing dental treatment for HNC patients. Only one quarter of New Zealand graduates and about 10% of overseas graduates reported that they were confident treating HNC patients. Willingness to provide basic dental treatment for a HNC patient was related to the invasiveness of the treatment itself; typically, the more invasive a procedure, the fewer dentists who would consider undertaking it. Similarly, a clinician’s experience providing basic dental care to HNC patients during the previous year in practice was also dependent on procedural invasiveness. ▼ Background: Although head and neck cancer (HNC) is the sixth most common cancer type by incidence globally, few studies have focused on real world treatment patterns of this disease. A large, randomized study of erythropoietin stimulating agents (ESAs) produced worsened outcomes in patients with HNC. Real-world data on the treatment patterns HNC, and the use and outcomes of ESAs in this population is currently lacking. Methods: SEER Medicare was used to evaluate patients aged ≥65 years with a first diagnosis of HNC between 2002 and 2007. Logistic regression was used to evaluate predictors of treatment. An interrupted time series was used to estimate the changes in ESA use among patients receiving chemotherapy over time. Propensity weighted models predicted survival, thromboembolism (TEE), and disease recurrence outcomes associated with ESA exposure. Results: Characteristics associated with no treatment for HNC included being unmarried (adjusted OR=2.00, 95% CI: 1.75, 2.27) or African American (adjusted OR=1.65, 95% CI: 1.37, 1.98). ESA use was 50.0% among patients receiving chemotherapy and 3.1% among patients not receiving chemotherapy. ESA use was 50.0% among patients receiving chemotherapy and 3.1% among patients not receiving chemotherapy. Combination regimens such as cetuximab+taxane+platinum (63.8%) and taxane+platinum (56.0%) were associated with the highest rates of ESA use, while single-agent cetuximab was associated with the lowest rate of ESA use (22.3%). Use of ESAs significantly declined from 40.3% of patients receiving chemotherapy in January 2007 to 16.7% in August 2007. Patients receiving an ESA were more likely to have a TEE (adjusted HR=1.19 [95% CI: 1.31, 1.81), experience disease recurrence (adjusted HR=1.09 [95% CI: 1.00, 1.16]), or die (adjusted HR=1.17 [95% CI: 1.06, 1.30]). Discussion: Disparities in the diagnosis and treatment of patients with HNC were present, despite the uniform insurance status of patients in the database. Significant declines in ESA occurred following FDA action in January 2007. ESA use increased among HNC patients in the immediate aftermath of previous disclosure of negative trial data (June 2003). ▼ Energy transfer from molecular species has been a long standing topic of profound interest to the chemical physics community. It is worth noting however, that to date, most studies have preferentially focused on chemically stable molecular species. While the literature does contain numerous examples of energy transfer of small radical or chemically unstable species, there have been extremely few studies which have actually probed highly vibrationally excited species. This apparent lack of attention should not be confused with a lack of interest. On the contrary, given the prevalence of vibrationally excited radicals in complex chemical systems such as planetary atmospheres and combustion, it is highly desirable to gain a complete understanding of the energetic deactivation processes of these delicate species. More often than not, the limiting factor which prevents examination of such species is a lack of spectroscopic information which is necessary for the identification as well as modeling of the corresponding species. In this thesis, we explore the use of time-resolved Fourier transform infrared emission spectroscopy, coupled with ab initio quantum chemical calculations, as a means of characterizing the vibrationally excited energy transfer dynamics from hydrogen isocyanide (HNC) as well as the ketenyl (HCCO) radical. It has been determined that each of these radical species can be generated in appreciable relative concentrations and with excess internal energy, following the 193 nm photolysis of specific stable molecular precursors. Through variation of the associated inert atomic collider species, and repeated spectral fitting analysis, it becomes feasible to obtain a measure the time-resolved average internal energy (as a function of the collider species), and hence a measure of the vibrational energy transfer efficiency of each radical species. It is observed that both HNC and HCCO exhibit enhanced vibrational energy transfer, for all average internal energies, relative to comparably sized stable molecular species. The roles and associated benefits of enhanced energy transfer of these radical species, to combustion chemistry (HCCO) as well as the interstellar medium (HNC), will be discussed.

▼ In Scotland, further education colleges provide 28% of all higher education; this includes over half of part-time undergraduate higher education. This provision has contributed to wider participation in higher education in Scotland by “non traditional” students and to progress towards a mass system of higher education within a learning society. This thesis is a case study of higher education students in a Scottish further education college. It explores the nature of the students’ experience and its relevance to institutional management and higher education policy. Evidence is drawn from the college’s records, from focus groups and from a questionnaire survey of whole year groups (full-time and part-time students) over three successive years. The theoretical focus is upon a new definition of lifelong learning as learning integrated with life, drawn from literature on motive, motivation, participation and retention. The research explores the students’ experiences of combining study with work and family life. The student experience is found to be heterogeneous, complex and distinct from the stereotype of a young full-time university student. Vocational motives predominate and there is evidence of a significant investment of meaning, expectation and purpose in the experience of higher education. The students’ ability to balance and integrate learning with life is a determining factor in the achievement of sustained participation. The quality of support networks both in college and in the students’ work and family lives are found to be more significant than personal or demographic characteristics. The case study contributes to current thinking about the professional role of college senior managers in creating a student-centred institutional culture that responds to the complexity of the students’ experience. A case is made for a review of the current inequity of financial support for full-time and part-time higher education students and of the marginal status of colleges in the development of higher education policy.
