Este trabalho prospectivo, duplo-cego randomizado, avaliou o efeito da analgesia preemptiva do cetoprofeno e do parecoxibe. Sessenta pacientes foram submetidos à cirurgia para remoção de terceiros molares inferiores bilaterais inclusos, sendo operado um lado de cada vez. O paciente foi seu próprio controle. Os pacientes foram separados em dois grupos de 30 pacientes. No grupo parecoxibe, na primeira operação foi usado o parecoxibe ou placebo, endovenoso, 30 minutos antes da cirurgia e imediatamente após a operação foi feita a administração do placebo ou parecoxibe, garantindo ao paciente receber parecoxibe antes ou após a operação. O lado oposto foi operado após duas semanas da primeira cirurgia e o paciente que havia recebido parecoxibe antes e placebo depois da operação recebeu placebo antes e parecoxibe depois da operação e o que havia recebido placebo antes e parecoxibe depois recebeu parecoxibe antes e placebo depois. Nos 30 pacientes do grupo cetoprofeno, o modelo foi o mesmo, substituindo-se apenas o parecoxibe pelo cetoprofeno. O paciente pôde utilizar como medicação resgate a dipirona, sempre que necessário para controlar a dor pós-operatória. Após a operação foi fornecido para todos os pacientes um questionário, contendo a escala analógica visual (EAV), a escala descritiva de dor (EDD) e uma tabela para informar o consumo de medicação resgate. Foi avaliada a dor pós-operatória por meio da EAV, da EDD e pelo consumo de medicação resgate. Não houve diferença estatisticamente relevante quanto a intensidade da dor com o uso do parecoxibe ou do cetoprofeno antes ou depois do procedimento cirúrgico. Ao comparar a analgesia proporcionada pelo cetoprofeno e pelo parecoxibe os resultados mostraram que o parecoxibe administrado antes do procedimento cirúrgico foi mais eficaz do que o cetoprofeno no controle da dor na quarta hora do pós-operatório (p=0,041), mas foi menos eficaz após 24h (p=0,003). Quando se comparou a analgesia proporcionada por esses fármacos usados após a operação, o parecoxibe foi mais eficaz do que o cetoprofeno após 6 e 8h do procedimento (p=0,003 e 0,023, respectivamente).

This is a prospective, double-blind randomized, cross over experiment, to evaluate the effect of the preemptive analgesia of ketoprofen and parecoxib. Mais informações são necessárias para definir se o tratamento percutâneo da coartação da aorta com stents é superior à angioplastia com cateter-balão. De julho de 2000 a maio de 2003, 21 adolescentes e adultos com coartação da aorta focal e média de idade de 24 anos (DP 11 anos) foram submetidos, consecutivamente, a implante de stents (grupo 1). Os resultados foram comparados com os obtidos em um grupo histórico de 15 pacientes com média de idade de 18 anos (DP 10 anos) (p = 0,103) submetidos a angioplastia (grupo 2) nos últimos 18 anos. Após o procedimento, a redução do gradiente sistólico foi maior (99% [DP 2%] versus 87% [DP 17%]; p = 0,015), o gradiente residual foi menor (0,4 mmHg [DP 1,4 mmHg] versus 5,9 mmHg [DP 7,9 mmHg]; p = 0,019), o ganho no local da coartação foi maior (333% [DP 172%] \"versus1̈90% [DP 104%]; p = 0,007) e o diâmetro da coartação foi maior (16,9 mm [DP 2,9 mm] versus 12,9 mm [DP 3,2 mm]; p < 0,001) no grupo 1. Alterações da parede da aorta, incluindo dissecções, abaulamentos e aneurismas, foram observadas em oito pacientes do grupo 2 (53%) e em um do grupo 1 (7%) (p < 0,001). Não houve complicações maiores. Cateterismo (n = 33) ou ressonância magnética (n = 2) de controle foram realizados em seguimento mediano de um ano para o grupo 1 e um ano e meio para o grupo 2 (p = 0,005). A redução do gradiente sistólico persistiu em ambos os grupos; entretanto, gradientes tardios mais altos foram observados no grupo 2 (mediana de 0 mmHg para o grupo 1 versus 3 mmHg para o grupo 2; p = 0,014). Não houve perdas no diâmetro da coartação no grupo 1 e houve ganho tardio no grupo 2 (16,7 mm [DP 2,9 mm] versus 14,6 mm [DP 3,9 mm]; p = 0,075). No grupo 1, dois pacientes necessitaram de novo implante de stent em decorrência da formação de aneurisma ou fratura da malha do stent. No grupo 2, as anormalidades da parede aórtica não progrediram e um paciente necessitou de redilatação em decorrência da recoartação. A pressão arterial sistêmica foi semelhante em ambos os grupos durante o seguimento (sistólica: 126 mmHg [DP 12 mmHg] no grupo 1 versus 120 mmHg [DP 15 mmHg] no grupo 2; diastólica: 81 mmHg [DP 11 mmHg] no grupo 1 versus 80 mmHg [DP 10 mmHg] no grupo 2; p = 0,149 e p = 0,975, respectivamente). Apesar de os desfechos clínicos terem sido satisfatórios e similares com ambas as técnicas, o uso de stents propiciou resultados mais previsíveis e uniformes para alívio da estenose, minimizando também o risco de desenvolvimento de alterações da parede da aorta.

▼ INTRODUÇÃO: A estimulação cerebral profunda (ECP) é usada para tratar a doença de Parkinson (DP) avançada. A estimulação do núcleo subtalâmico (NST) melhora os sintomas de parkinsonismo, mas seu mecanismo de ação permanece pouco compreendido. A ressonância magnética functional (RMf) pode auxiliar no estudo de áreas cerebrais motoras de forma a melhorar o entendimento dos mecanismos de ação da ECP. O objetivo desta tese foi de desenvolver e testar técnicas de RMf para estudo de pacientes com DP tratados com ECP. MÉTODOS: Foram realizados testes para verificar interferência do sistema de ECP nas imagens de RMf e realizadas adaptações de modo a diminuir os artefatos na imagem. A seguir foram examinados pacientes com DP em quatro condições: durante a movimentação da mão direita (antes do implante dos eletrodos para ECP, depois da cirurgia sem estímulo elétrico e depois da cirurgia com estímulo elétrico no NST) e durante o repouso com a estimulação elétrica ligada e desligada. No total foram examinados dez pacientes, dos quais oito foram comparados a voluntários normais. Foram realizadas três sessões de RMf em cada condição, utilizando dois tipos de seqüências: em bloco (BL) e relacionada a eventos (RE). As imagens foram analisadas calculando-se a freqüência de atividade cerebral detectada em 19 áreas sensitivo-motoras. Dados comportamentais durante o exame e a evolução clínica foram analisados. RESULTADOS: Os pacientes não apresentaram complicações decorrentes dos exames de RMf. Após modificações na técnica de implante do eletrodo e ajustes do intervalo pós-cirúrgico para o exame, os artefatos nas imagens foram reduzidos. Nos controles, as sequencias RE mostraram mais atividade que BL nas áreas: área suplementar motora, área pré-motora (pM) direita, cíngulo anterior (Cga), cíngulo posterior (Cgp), tálamo bilateral e putamen direito. Durante a movimentação da mão dominante, comparando as seqüências BL e RE dos pacientes com DP, não houve diferença significativa considerando todos experimentos pré-operatórios e pósoperat órios sem o estímulo elétrico. Com o estímulo elétrico ligado, nas seqüências BL houve maior atividade em relação às RE, na área sensitivomotora primária (SM1) esquerda. Durante o repouso, quando a estimulação elétrica foi ligada houve maior atividade das seguintes áreas: cerebelo direito, SM1 esquerda, Cgp, pM bilateral, mesencéfalo esquerdo e Cga. Houve melhora clínica dos pacientes e tanto a avaliação motora na primeira semana pós-operatória, na fase sem medicação, bem como a avaliação global em seis meses estiveram correlacionadas com os parâmetros motores aferidos durante os exames de RMf. CONCLUSÕES: Foi desenvolvido método para a aplicação da RMf em pacientes com DP submetidos à ECP que permitiu as seguintes observações: as seqüências BL mostraram maior freqüência de atividade na SM1 nos pacientes com DP e ECP ligada, enquanto as seqüências RE mostraram maior freqüência de atividade nos voluntários normais nas áreas motoras primárias e secundárias; e o estímulo elétrico no repouso produziu atividade… Advisors/Committee Members: Amaro Júnior, Edson, Leite, Claudia da Costa.

OBJETIVOS. Verificar os efeitos da terapia hormonal com derivados estrogênicos e progestogênicos, isolados ou associados à metiltestosterona, na sexualidade e nos sintomas climatéricos em mulheres na pós-menopausa e comparar os dois tipos de terapia hormonal. CASUÍSTICA. Selecionaram-se sessenta mulheres sexualmente ativas, com queixas sexuais, com relacionamento estável com parceiro capacitado para o coito, com idade de 42 a sessenta anos (média etária 52,1 + 4 anos) e tempo de menopausa de um a 28 anos (média 5,6 anos). Excluíram-se mulheres com doenças sistêmicas, doenças psiquiátricas, endócrinas, distopias genitais, tabagistas e usuárias de terapia hormonal ou de medicamentos que apresentavam interferência na sexualidade. METODOLOGIA. Realizou-se estudo de coorte progressiva, duplo-cego, randomizado, com duração de 12 meses. As mulheres foram divididas em dois grupos: EP (n=29), medicadas com estrogênios conjugados (EEC) 0,625 mg + acetato de medroxiprogesterona (AMP) 2,5 mg + placebo, e grupo EP+A (n=31), medicadas com EEC 0,625 mg + AMP 2,5 mg + metiltestosterona 2,0 mg. Para estudar a sexualidade, utilizou-se o Questionário Sexual do Hospital das Clínicas e foram avaliados o desejo sexual, a excitação e a capacidade orgástica nas atividades desenvolvidas com o parceiro, o interesse sexual não vinculado, exclusivamente, às atividades desenvolvidas com o parceiro, a dispareunia, a secura vaginal e a freqüência sexual. Para as análises estatísticas, utilizaram-se Modelos Lineares Gerais para Medidas Repetidas, Análise de Variância (ANOVA), Modelos de Regressão Logística Multinomial e Qui-quadrado de Pearson. O nível de significância foi de 5%. RESULTADOS. Nos grupos EP e EP+A, houve aumento no escore de desejo sexual vinculado, exclusivamente, às atividades desenvolvidas com o parceiro (F=18,334; p<0,001), no escore de excitação sexual (F=14,022; p < 0,001), na capacidade orgástica (F=34,650; p < 0,001) e na freqüência sexual (F=7,687; p=0,008), bem como redução da secura vaginal (?2=44,153; p<0,001), da dispareunia (?2=34,447; p < 0,001) e do índice menopausal de Kupperman (F=158,460; p < 0,001). A análise comparativa entre os grupos EP e EP+A mostrou maior interesse sexual não vinculado, exclusivamente, às atividades com o parceiro (?2=11,551; p=0,021) e mais altos índices de Castelli I (F=8,542; p < 0,001) e índices de Castelli II (F=11,500; p < 0,001) no grupo EP+A. Não se observaram hirsutismo nem alopécia em nenhum dos grupos; acne grau I foi observada em duas mulheres do grupo EP e em 13 do grupo EP+A. CONCLUSÕES. As terapias hormonais com derivados estrogênicos e progestogênicos, isolados ou associados à metiltestosterona, causaram impacto positivo em todos os parâmetros sexuais e nos sintomas climatéricos analisados. A associação de metiltestosterona ao tratamento estro-progestacional aumentou o interesse sexual não vinculado, exclusivamente, às atividades com o parceiro e os índices de Castelli I e II. ▼ This study addresses the research question “How Do I Inspire Personal and Social Change Through My Theater Practice?” I implement the theory and practice of H.E.A.T., a fusion theater system, combining use of theater arts as healing practice, educational asset, activist tool, and an art form. I research different ways that theater can affect change, focusing specifically on the use of history in performance. I dramatically interpret a period of history where performance and poetry contributed to change. I utilize qualitative methods including performance ethnography, auto ethnography, arts-based research, and historical research. I describe the fieldwork in conflict zones in the Middle East, which led to the scripting of a full-length play, and the presentation of the play, which included discussion groups and audience participation through post-show events. The dissertation is a bricolage, combining scholarly chapters, performative writing, and scripted theater. The work explores ways of employing theater as a change agent by using history as an inspiration. In the city of Cordoba, Spain, in the 10th and 11th century Muslims and Jews lived in a state of relative peace. Looking at medieval Cordoba I explore the Judeo-Arabic poetry of the time, asking: Can what happened in Cordoba be a model for performance and peacebuilding? Based on historical research, the Judeo-Arabic poetry of ancient Al-Andalusia, and the theory of performative peacebuilding, the dramatically scripted section of the dissertation will take place in two realms: Present-day conflict zones in the Middle East; and medieval Cordoba where two ancient characters convey a story of coexistence through poetic expression. In three decades of working as a theater artist, I have come to believe that my work must be dedicated to facilitating change. The sacred and ancient art of theater needs to be meaningful to 21st-century life so that we can use it to awaken, heal, educate and repair the world. ▼ AIM: This doctoral thesis focuses on the measurement of efficiency and effectiveness in the health care sector, and the chosen field of study is end stage renal disease (ESRD) in Greece. The specific objectives are: i) To measure, at the health system level (macro-level), technical and scale efficiency of dialysis units and to perform a longitudinal productivity analysis, while concurrently focusing on the observed differences between the public and private sectors and ii) To compare, at the medical intervention level (micro-level), the effectiveness of the three known modalities of Renal Replacement Therapy (RRT), namely in-center hemodialysis (HD), peritoneal dialysis (PD) and renal transplantation (Tx). MATERIAL AND METHODS: All dialysis units operating in Greece from 1993 (N=73) to 2004 (N=124) were involved in the measurement of efficiency. Non-parametric Data Envelopment Analysis (DEA) was used with two inputs, nurses and dialysis machines and one output, the annual number of dialysis sessions. Technical and scale efficiency were determined for every unit in each of the study years, by implementing variable and constant returns to scale (VRS, CRS), input oriented DEA models. Total Factor Productivity was measured with Malmquist’s index which was decomposed into three components indicating changes in pure technical efficiency, scale efficiency and technology. Effectiveness of RRT methods was determined with cost-utility analysis (CUA). A representative sample of patients on HD (N=642), PD (N=65) and Tx (N=167) were self-administered the KDQOL-SF (Kidney Disease Quality of Life Short Form) instrument, incorporating the generic QoL instrument SF-36, from which the preference-based SF-6D utility index was derived. Utilities were also obtained from direct methods, namely visual analog scale (VAS), time tradeoff (TTO) and standard gamble (SG), and patients’ willingness-to-pay for a kidney transplant was measured. Lifelong QALYs (Quality Adjusted Life Years) were calculated by considering individual expected remaining life years. HD and PD cost analysis was performed from the societal perspective, whereas the cost of Tx was adopted from a reimbursement proposal formulated by the Hellenic National Transplant Organization. In the CUA, both cost and QALYs were discounted at a common rate of 5%. RESULTS: The DEA VRS models, for the twelve study years, produced efficiency scores ranging from 66,18% to 74,19%. Differences are observed between the public and private sectors, with scores varying between 62,3-70,5% and 75,0-89,7% respectively. The CRS models logically produced lower scores for all units (39,5-53,5%). ▼ Enzymes that are involved in the synthesis and processing of oligosaccharides, such as glycosidases, are important catalysts for the specific assembly of oligosaccharide structures on proteins. Design and preparation of selective inhibitors of these enzymes are of high interest as these molecules can be used to modulate cellular functions. Moreover, they may provide potential drugs in new therapeutic strategies. In particular, swainsonine (Swa), a natural inhibitor of Golgi α-mannosidase II, reduces certain tumors and hematological dysfunctions. Nevertheless, some side effects resulted in limitations for the development of this compound in medicinal treatments. Recently, our laboratory has developed a new combinatorial methodology for the disclosure of glycosidase inhibitors based on the dihydroxypyrrolidine scaffold found in Swa. This method allowed the preparation of a new family of selective and competitive a-mannosidase inhibitors with inhibitory activities (Ki values) ranging from 3 µM to 135 nM for the best congeners toward α-mannosidase from jack bean, a reliable model enzyme for mammalian Golgi α-mannosidase II. The aim of the present work was to design and synthesise optimized inhibitors of α-mannosidases II based on a 2-aminomethyl-3,4-dihydroxypyrrolidine scaffold and to assess their potential as new anti cancer leads. In a first time, a structural study was carried out through the analysis of x-ray structures of the best inhibitors previously developed in our laboratory complexed to Golgi α-mannosidase II from Drosophila melanogaster (dGMII), which presents a high sequence identity with the human enzyme. The binding mode of these inhibitors could be thus established. This study combined with docking experiments allowed us to design inhibitors of α-mannosidase II, with improved efficacy. The best congener (3R,4R,5R)-3,4-dihydroxy-5-({[(1R)-2-hydroxy-1-phenylethyl]amino}methyl)-1-methylpyrrolidin-2-one displayed an enhanced inhibitory potency against dGMII (IC50 = 0.5 µM vs 80 µM for the first generation inhibitor). Introduction of a polar substituent on the C(5) position of the pyrrolidine ring accounted for this improved activity as revealed by the x-ray structure of this new derivative complexed to dGMII which displayed additional interactions with active site residues. In a second time, these new α-mannosidase inhibitors have been evaluated as anti cancer agents and their effects were determined on human cancer cells from glioblastoma and melanoma, two tumors associated with a high proliferative and invasive potential, multiple resistance toward conventional chemotherapeutic agents, and poor prognosis. As these compounds were not able to inhibit cellular growth, probably due to a poor cellular uptake, derivatization of our inhibitors into more lipophilic esters, ethers and amides derivatives was envisaged. ▼ Photodynamic therapy (PDT) can be used to cause vascular collapse and blood flow stasis of the irradiated pathological neovascularisation appearing in several diseases such as age-related macular degeneration (AMD) and cancer. We hypothesized that this PDT-related interruption of vessel integrity may lead to an increased transvascular passage of drugs that could be used as a drug delivery pathway. Thus, preceding the occlusion of the pathological vasculature, PDT could be used for instance as a local drug delivery pathway to administrate an anti-angiogenic drug in the case of AMD or chemotherapy in the case of cancer, potentially improving combination therapies. In the case of chorioneovessels (CNV) due to AMD, the recurrence of the exudative AMD component of the weeks/months after PDT, due to the re-opening and/or re-growth of neovessels might be avoided by adding an anti-angiogenic factor such as anti-VEGF or anti-inflammatory drug before, during or shortly after PDT. In the case of cancer, the starvation of tumour cells induced by the PDT occlusion of blood vessels feeding the tumour might be combined with a chemotherapeutic agent for the direct kill of the cancer cells themselves. It has been reported that following the light application in PDT, a physiological cascade of responses on the one hand leads to vascular occlusion but may also induce a vascular permeability enhancement. The aim of this thesis is to find conditions where this increase in leakage due to PDT can be observed, to characterize it and to take advantage of this phenomenon to develop the basis of a novel combination therapy approach. Hence in this thesis, pre-clinical experiments were performed in the vasculature of the chorioallantoic membrane model (CAM) of the chicken embryo and in the dorsal skinfold optical chamber of nude mice observed by intravital microscopy (IVM). In the CAM, no PDT-induced leakage of a fluorescent dye (FITC-dextran) was observed unless an anti-aggregating factor, such as aspirin was added. In the chicken embryo model, delaying the blood clot appears to be an essential process to allow effective potential drug delivery. In the dorsal skinfold of the nude mouse, the inflammatory response after PDT was observed and quantified. This revealed that PDT induces a time dependent acute inflammatory response as shown by increased number of leukocytes "rolling" along the vessel wall after treatment. This was observed over a 2 hour period following PDT. The quantification of the microvascular leakage showed a continuous FITC-dextran leakage from the vasculature treated by PDT to the interstitial space. This local leakage was clearly increased by the inflammatory status of the tissue (observed by quantifying the rolling leukocytes and by histology). This concept has the potential to improve the drug delivery of anti-angiogenic drugs in the eyes of patients treated for AMD and could also be applied to improve the uptake of cytostatic drugs in tumours. ▼ A major challenge for effective gene therapy is systemic delivery of viruses carrying therapeutic genes into affected tissue. The immunogenic nature of human adenoviruses (Ads) limits their use for intratumoral (IT) injection in gene therapy. Ads transfection is further hampered by the fluctuating presence of Coxsackie and Adenovirus Receptor (CAR) and integrins on the cells’ surface. To circumvent these limitations we developed a novel approach wherein Ads are encapsulated inside the shell of lyophilized, lipid-encapsulated, perfluorocarbon microbubbles (MBs)/ultrasound (US) contrast agents, which act as delivery vehicles for a sitespecific gene transfer system. We performed infection studies with Ad.GFP (Green Fluorescent Protein), Ad.mda-7 (melanoma differentiation associated gene 7) and CTV.mda-7 on human DU145 and mouse prostate cancer cells as well as observed enhanced GFP expression when Ad.GFP was delivered by MBs and US. Our results show that US breaks open the MB/Ads complexes by undergoing cavitation at the sonoporated site, which allows Ads to transfer their transgene only to the sonoporated region. Cavitation collapse of the MBs creates small shockwaves that increase cell permeability by forming temporary micropores on the cell surface bypassing the receptormediated dependence of Ads for transfection. Fetal bovine serum (FBS) containing complement did not allow the unprotected Ads to infect the cells; however, MBs complexed with Ad.GFP did infect DU145 and TRAMP-C2 cells in a FBS rich media. We studied MB assisted gene delivery of reporter (GFP) and therapeutic genes (p53, Rb, Rb2 (p130) and Mda-7/IL-24) into prostate cancer (PC) xenografted in immune-compromised athymic mice. The results demonstrated that MBs protect the host from unspecific viral immune response thus protecting the viral payload and allowing for intravenous (IV) injection rather than IT injection. Additionally, Ad gene transfer was enhanced at the targeted/sonoporated mice tumor xenografts. This research demonstrated mda-7’s efficacy in reducing primary (treated) and untreated tumors that simulated the presence of metastasis in athymic mice xenograft models bearing human PC cells. Bystander anti-tumor activity of mda-7, a secreted cytokine was noted for non-targeted tumors. Earlier in vitro studies on the combination of radiation and gene therapy (Ad.p53, Ad.Rb, and Ad.p130) demonstrated an increase in the percentage of cell death for DU145 cells. We also studied UTMD (ultrasound targeted microbubble destruction) gene therapy in combination with external beam radiation for radiation resistant PC. The results demonstrated an enhanced therapeutic benefit of tumor suppressor genes in radiation resistant PC. ▼ There has been a rising interest in addiction medicine and addiction treatment in both the medical and behavioral health science fields. Research suggests having a family member with a substance abuse problem has negative impacts on both physical and mental health (Orford, Copello, Velleman, & Templeton, 2010a). Despite advances toward understanding the experiences of family members affected by a loved one’s addiction, the siblings of substance abusers have been largely excluded from scientific research and literature. As a result, little is known about how siblings experience the impacts of a brother or sister’s addiction; even less is known about the experiences of adolescent siblings sharing a home with a substance-abusing sibling.The purpose of this study was to explore the experiences of and meanings made by adolescents living with the phenomenon of a sibling’s addiction. Five adolescents participated in a semi-structured interview exploring the question: What is it like to be the brother or sister of a person with a substance abuse problem? Interpretative Phenomenological Analysis was used to analyze data and six themes were identified: personal impact; familial impact; social impact; coping strategies; shared ways of knowing, being, and seeing; and ways of understanding. The results indicated siblings experience profound emotional and relational impacts, which include stress, anxiety, sadness, and anger as a result of the trauma, betrayal, and grief associated with a sibling’s substance abuse. Experiences of invalidation within the family and stigmatization within the community were associated with strained relationships and increased isolation. A comparison of the results to existing research on adult siblings of substance abusers revealed the negative impacts experienced by adolescent siblings of substance abusers continue into adulthood. Participants’ ability to identify and describe these negative impacts directly contributes to the health care field’s current dearth of data on the subject. Results challenge the misconception that siblings of substance abusers are “doing fine” and highlight an opportunity for researchers and treatment providers to expand their knowledge of this largely underrepresented population. Participants’ perspectives on expanding interventions for affected family members to include the siblings of substance abusers are also discussed. ▼ Introduction : La mortalité associée à l’insuffisance rénale aiguë (acute kidney injury ‘’AKI’’) aux soins intensifs pédiatriques (SIP) dépasse les 50%. Des études antérieures sur la thérapie de remplacement rénal (TRR) ont fait ressortir plusieurs facteurs de risque de mortalité dont le syndrome de défaillance multiviscérale (SDMV) et la surcharge liquidienne ≥ 10 à 20% avant l’initiation de la TRR. L’objectif de cette étude était d’identifier les principaux facteurs de risque de mortalité à 28 jours après l’initiation de la TRR chez les patients atteints d’AKI aux SIP. Méthode : Il s’agit d’une étude de cohorte rétrospective aux SIP d’un centre tertiaire. Tous les enfants ayant reçus de la TRR continue ou de l’hémodialyse intermittente pour AKI, entre janvier 1998 et décembre 2014, ont été inclus. Les facteurs de risque de mortalité ont été préalablement identifiés par quatre intensivistes et deux néphrologues pédiatres et analysés à l’aide d’une régression logistique multivariée. Résultats : Quatre-vingt-dix patients ont été inclus. L’âge médian était de 9 [2-14] ans. La principale indication d’initiation de la TRR était la surcharge liquidienne (64,2%). La durée médiane d’hospitalisation aux SIP était de 18,5 [8,0-31,0] jours. Quarante patients (44,4%) sont décédés dans les 28 jours suivant l’initiation de la TRR et quarante-cinq (50,0%) avant la sortie des SIP. Le score de PELOD ≥ 20 (OR 4,66 ; 95%CI 1,68-12,92) et la surcharge liquidienne ≥ 15% (OR 9,31; 95%CI 2,16-40,11) à l’initiation de la TRR étaient associés de façon indépendante à la mortalité. Conclusion : Cette étude a permis de faire ressortir deux facteurs de risque de mortalité à 28 jours à l’initiation de la TRR : la surcharge liquidienne et la sévérité du SDMV mesurée par le score de PELOD. ▼ The rate of musculoskeletal and mental health disorders is high in the Canadian Armed Forces, and can preclude service members from being employable and deployable. The aim of this thesis was to analyze the effectiveness of the Return to Duty intervention at decreasing morbidity and assisting service members to be retained by the Canadian Armed Forces. Using a multiple baseline single subject design, the physical and mental function of the participants was measured repetitively pre, during and post-intervention. In total, 23 participants were enrolled in the Return to Duty intervention, where they received physical training with health and work literacy once a week for 10 weeks in a group. Analysis was performed with descriptive statistics and visual analysis using Minimally Important Difference for significance. Six months post-intervention, return to duty outcome was measured. The confirmed return to duty rate was 39.1% at 6 months post-intervention, with an additional 34.8% expecting to return to duty. Amongst the measures, the 4 Squares Step Test, Work Ability Index and SF-36v2 physical component improved in respectively 70.6%, 47.1% and 66.7% of the confirmed and expected favorable outcome group (73.9% of the sample). Deterioration in function was found mostly in the unfavorable group, notably in mental health measures; possibly indicating that mental health could be a mediator of return to duty. Overall, most of the repeated measures did not change meaningfully, potentially suggesting an ecological model of return to work. The study found a superior confirmed return to duty rate compared to nationally in the Canadian Armed Forces, and a superior expected favorable return to duty rate, compared to any bases/wings. The return to duty processes in the Canadian Armed Forces have not been previously published therefore, this thesis provides a novel contribution to the scientific literature. Furthermore, this thesis helps to understand factors that might increase the retention of the service members in the Canadian Armed Forces. Finally, this work makes recommendations for future studies in return to duty in armed forces.

▼ In light of increasing system demands, system regulations, and constrained resources, those living and working with dementia in the long-term care sector are vulnerable to oppressive care practices. This is true so long as our understanding of how social power affects the ways in which dementia care knowledge is created, shared, and enacted remains limited. Based on prolonged field observations and on informal and formal interviews with care recipients, family members, and staff, the aim of this critical qualitative research was to examine the culture of dementia care knowledge in two sites: a specialized dementia care unit in a long-term home, and an affiliated adult day program. Three key insights arise from this study. The first is that normalizing inclusiveness in generating dementia knowledge is akin to democratizing hierarchical relationships in long-term dementia care work environments; the more that knowledge is co-constructed under ethical conditions of discourse, the more point-of-care staff can contribute to care planning and provision. Second is the temporal distinction between two prominent yet somewhat contradictory care norms: that of contextualizing a responsive behavior, and that of using force to complete daily care tasks. These co-existing care routines not only contest one another, they manifest along different timelines. Third, care providers and recipients can benefit from a three-pronged approach to generating dementia care knowledge wherein an iterative pattern of internalizing, externalizing, and socializing care knowledge yields collectively held organizational knowledge. This study re-conceptualizes “culture” in ways that consider the broader (often oppressive) social forces at play in constituting dementia care knowledge. In addition to its methodological contributions to critical qualitative health research, the study’s implications are relevant to those who espouse ethical and non-coercive dementia care practices, and to knowledge translation scholars who appreciate that as a part of context, the knowledge culture needs to be understood in terms that make clear the influence of social power among and between the culture’s constituents and the intervening knowledge translators.

The research project Â´This is my Face: Audio-visual practice as collaborative sense-making among men living with HIV in ChileÂ´ is an interdisciplinary project that explores Â´collaborative mise en-scÃ¨neÂ´ as a method to further understand the sense-making processes around the biographical disruption caused by HIV. It combines Anthropology and Arts methods as part of the PhD in Anthropology, Media and Performance, a practice-based program that fosters interdisciplinary approaches to the production of original knowledge, based on self-reflexive and critical research practices (The University of Manchester, 2018). Relying on the specific competences of photography and film and the co-creation of an ethnographic context based in hermeneutic reflexivity, the collaborators on the project created and explored representations of critical life events, in order to make sense of the disruption HIV brought to their lives. The collaborators were highly stigmatised individuals living with HIV, which hindered their possibilities for sharing narratives and for reflection, and as such, made it more difficult for them to come to terms with a diagnosis they described as a Â´fractureÂ´ in their lives. This project analyses the creative process of Â´collaborative mise-en-scÃ¨neÂ´ as a way to provide further opportunities for reflexivity and sense making, a method that departs from their everyday face-to-face encounters as means of understanding what they are going through. Representations of life events emerged from our practice, as well as evocations, which provided a means by which to understand their experiences with HIV, and opened up ways to resignify their past experiences and projections of the future. Photography and film offered their specific expressive competences to the project, but also gave the possibility of making visible the collaboratorsÂ´ experiences in order to promote a dialogue with others, moving beyond our creative encounters. Therefore, their evocations became Â´statementsÂ´ of what it means to live with HIV in Chile, and at the same time, by taking part in its creation, it provided access to the particularities of the sense-making process in which those images were embedded. This collaborative creative process opened up ways to highlight the relevance for sense-making in face-to-face encounters, demonstrating that hermeneutic reflexivity as a practice-based form of mutual questioning can promote a critical engagement with life trajectories and with others beyond our practice.
