▼ Background: Peri-implantitis is an inflammatory disease process that presents with bone loss around implant fixtures. Without the proper treatment, it can lead to the eventual loss of the implant. However, controversy lies over the effectiveness of treatment modalities as the criteria for disease diagnosis and success of treatment varies in the literature. With an increasing global burden of disease comes incentive to find predictable treatment options for peri-implantitis. Wnt4 has been shown in prior studies to attenuate osteoporotic bone loss and prior studies in our lab using a murine model for peri-implantitis, found transgenic Wnt4 mice to exhibit the same pattern of attenuated bone loss in the maxilla. Objective: To identify whether experimental recombinant Wnt4 (rWnt4) injection shows attenuated bone loss compared to a control PBS injection, in a murine model of ligature induced peri-implantitis.Methods: Four-week-old male C57BL/6J mice had their left maxillary molars extracted. Following eight weeks of healing, titanium implants were placed in the extraction sites and allowed to integrate for four weeks. After osseointegration, evaluated by presence and lack of mobility in the implants, a 6-0 ligature was tied under the head of the implant in half of the mice to induce peri-implantitis. Half of the mice did not receive ligature placement around the implants. Each group was then divided again to either receive daily control PBS injections or experimental rWnt4 injections intraperitoneally for 30 days. Four weeks after ligature placement and daily injections, the mice were sacrificed. The maxillae were imaged using micro-computerized tomography (μCT) scanning and linear and volumetric bone loss were analyzed. Tissues from the representative samples (based on proximity to mean measurements) were stained with hematoxylin and eosin (H&E), Picrosirius Red for collagen, and Tartrate Resistant Acid Phosphatase (TRAP) for osteoclasts. Immunohistochemistry (IHC) was also performed and analyzed for NF-κB, MMP-9, and Cox-2.Results: Clinically, four weeks after ligature placement there was increased soft tissue edema present around implants from the ligature induced peri-implantitis group compared to the no-ligature group. Radiographically, linear bone loss analysis showed statistically greater bone loss in the ligature compared to no ligature group for both the PBS and rWnt4 injection groups. The same finding was observed for the volumetric bone loss analysis around implants. There was no statistical difference found in bone loss between the rWnt4 injected groups compared to PBS injections for both the ligature and no ligature groups. TRAP staining for osteoclasts showed statistically greater numbers of osteoclasts in the PBS injection ligature group compared to PBS injection no ligature group. Picrosirius red staining showed more organized collagen fibril distribution in rWnt4 ligature vs. PBS ligature groups. ▼ Jumonji Domain Containing 6 (JMJD6) is a histone arginine demethylase that preferentially removes methyl groups from dimethylated arginine 2 of histone 3 (H3R2me2) and arginine 3 of histone 4 (H4R3me2), thereby enabling dynamic regulation of transcription. JMJD6 also regulates gene expression by modulating RNA splicing, suggesting that JMJD6 is a multifaceted epigenetic regulator. Clinically, JMJD6 overexpression is strongly linked to poor prognosis in various human cancers, including breast and lung. JMJD6 is also reported to promote cancer cell migration/invasion and angiogenic sprouting, two phenotypes of which are well known for cancer stem cell (CSCs; also known as cancer initiating cells) characteristics. JMJD6 is also known to be essential for the differentiation of multiple tissues and cells during embryogenesis. Recent studies revealed that histone methylation played a critical role in controlling stemness of normal stem cells. Although CSCs share molecular and phenotypic characteristics with normal stem cells, a considerable knowledge gap remains in our understanding of epigenetic regulation of oral CSCs particularly by histone demethylases.To investigate potential involvement of histone demethylases in oral CSCs, we screened expression of 22 known histone demethylases in CSC-enriched oral squamous cell carcinoma (OSCC) populations. Among them, JMJD6 was unequivocally overexpressed in all tested CSC-enriched OSCC populations. Subsequent functional analysis showed that knockdown of endogenous JMJD6 in OSCC strongly suppressed CSC phenotypes (e.g., decreased self-renewal capacity and migration). Conversely, ectopic expression of JMJD6 enhanced the CSC phenotypes. Interestingly, expression of CSC-related genes (e.g., pluripotency transcription factors and cytokines) was markedly affected by modulating JMJD6 expression. We further found that JMJD6 regulates the expression of CSC-specific cytokines (i.e., IL-4, IL-5, MIP-1α, MIP-1β, IFNα, and IFNβ) by binding to their promoter regions. Our study suggests that JMJD6 promotes oral CSC phenotype by epigenetically regulating its target genes.

▼ Introduction and objective: Non-carious cervical lesions (NCCLs) are a very interesting topic in the field of restorative dentistry. This is for several reasons: (1) It considered one of the most common lesions in the permanent teeth. (2) The best restorative materials indicated for NCCLs is still a controversial subject, and (3) despite many studies done and still ongoing, there are no consensuses about bonding agent systems and their bounding effectiveness.Currently, the restorative options for cervical lesions (in general) are composite resin, glass ionomer, gold foil, and amalgam. Each of these options have different characteristics and properties. For example, gold foil is one of the best dental materials with respect to the physical properties; however, for conservative and esthetic reasons, composite is one of the best restorative dental materials. Glass ionomer comes after composite as an esthetic direct restorative material. Besides this, glass ionomer uniquely has an anti-carious property by releasing fluoride ions. In advanced NCCLs, since the tooth already lost part of its structure, it is preferred to restore these cases with restorations that do not require further preparation for retention like composite resin and glass ionomer. Consequently, composite and glass ionomer are favored over amalgam and gold foil for esthetic and conservative purposes in NCCLs cases. Hence, in this study, we want to compare composite resin and glass ionomer as a restoration option for non-carious cervical lesions (NCCLs). More specifically, we want to evaluate the marginal integrity and retention rate of composite resin and glass ionomer materials as a restoration option for NCCLs. The aim of this study is to sum up the evidence from multiple systematic reviews, clinical trials, and observational studies that have evaluated the marginal integrity of resin composite and glass ionomer restorations placed in non-carious cervical lesions. In other words, our purpose is to compare glass ionomer and resin composite as restoration for non-carious cervical lesions using an evidence based dentistry approach by going through the Comparative Effectiveness and Efficacy Research and Analysis for Practice (CEERAP) in order to find out which one of these two materials has better marginal integrity. Methods:Systematic reviews with or without meta-analysis, observational studies, and randomized clinical trials that assess the effectiveness of glass ionomer, composite resin, and/or adhesive systems, such as a restorative material for NCCLs were chosen. Cochrane library, PubMed, ADA Center for Evidence-Based Dentistry, Google Scholar, Web of Science databases were searched from September 2015 to April 2016. The review was restricted to papers with English language and papers studying permanent teeth with a time frame of no less than one year. ▼ Osteoporosis is a metabolic bone disease where decreased bone quality and quantity increases the rate of fractures. It serves as one of the largest skeletal aging problems worldwide. Like many other chronic diseases, osteoporosis affects the majority of the elderly population, especially females after menopause. The prevalence of bone disease and fractures is projected to increase markedly as the population increases(1). Bone disease and many other factors related to fractures will have an enormous impact on the future well-being of Americans and worldwide. NELL-1 is an osteogenic, secretory molecule previously shown to enhance bone regeneration in various rodent and ovine skeletal defect models. Systemic delivery of NELL-1 via intravenous (IV) injections not only increases murine bone mineral density (BMD) and percent bone volume, but also increases new bone formation throughout the overall skeleton(2). Astoundingly, in the past, we have shown that systemic delivery of NELL-1 induces robust bone formation in healthy mice, which were injected intravenously every 7 days. Here, we investigate the potential of systemic administration of PEGylated NELL-1 (NELL-PEG) every 7 days to see the increases in femoral BMD, percent bone volume and also new bone formation throughout the overall skeleton after four weeks of treatment to reverse ovariectomy (OVX)-induced osteoporotic bone loss in mice. First, we have successfully established osteoporotic mouse model that mimics the human post-menopausal osteoporosis by surgical removal of ovaries. To monitor the changes in BMD in vivo dual-energy X-ray absorptiometry (DXA) was performed biweekly. After four weeks of treatment, animals were sacrificed for microCT and histological analyses.Overall, these findings implied that systemic delivery of NELL-PEG in OVX mice would provide a novel strategy for osteoporosis therapy; nonetheless, further studies are needed to determine the optimum dosage and the efficacy of various routes of drug administration.Keywords: NELL-1, PEGylation, osteoporosis, systemic osteogenic therapy

Non-extraction treatment of Class II subdivision
malocclusion, with symmetrical forces has not been described in the
literature. This study evaluated the use of symmetrical forces with
face-bow therapy in conjunction with full fixed appliances of 47
patients (28 females and 19 males) that were treated by the same
practitioner. The pretreatment and post-treatment records were
taken at an average age of 12.3 and 14.4 years respectively. The
results showed that the Caballero index (The distance from the tip
of the mesio-buccal cusp of the upper first molar to the
mesio-buccal groove of the lower first molar on the class II side.)
decreased from 1.86 to 0.12 mm, making this statistically
significant. The midline showed a distinctive improvement having a
pretreatment value of 1.33 mm and a post treatment value of 0.29
mm. Arch length had a modest gain of 1.15 mm with the left side
having the biggest contribution. The maxillary intermolar position
remained stable with a 0.3 mm net change. The ANB angle decreased
from 3.5 to 2.1 degrees. The upper incisor in relation to S-N and
lower incisor inclination increased by 3.3 degrees. Finally, the
mandibular plane angle increased in value from 31.2 degrees to 32.2
post treatment. Within the limits of this study and based on the
test of equivalence, it is concluded that when symmetrical forces
are used in non extraction therapy for Class II subdivision growing
patients, the class II malocclusion is corrected, but some
asymmetry in the molar position remains.

The purpose of this study was to investigate the
effects of taper sizes on fracture resistance in root sections and
determine if there is a difference in the fracture resistance of
different regions within the same root. Thirty-six single-rooted
mandibular premolars were divided into three groups for three
different taper sizes (.02, .04, and .06). The specimens were
instrumented and sectioned. Each section was photographed and the
net average surface area was calculated using the ImageJ software
(NIH, Bethesda, MD). Each individual root section was then
subjected to a load to fracture test using the MTS 858 machine
(MTS, Eden Praire, MN). A ratio of the load to fracture and average
surface area was calculated. Statistical analysis was conducted
using ANOVA, Tukey-Kramer, Pairwise Spearman, and Random Intercept
Models. It was determined that fracture resistance increase with
larger taper size and was higher in the apical
region.

INTRODUCTION: The present study evaluated the
effectiveness of SureSmile technology when used as an adjunctive
tool in orthodontic treatment. The study was designed to evaluate
the effectiveness of SureSmile archwires to produce the final tooth
position prescribed by the SureSmile virtual treatment plan.
METHODS: Digital models of the SureSmile virtual treatment plan and
final treatment outcome were digitally superimposed to determine
areas of discrepancy. Discrepancy less than 0.5 mm with respect to
mesial-distal, facial-lingual, vertical dimensions, and discrepancy
less than 2 degrees for crown torque, crown tip, and crown rotation
were considered to be clinically acceptable. RESULTS: Discrepancy
was within clinically acceptable limits with respect to the
mesial-distal dimension for all teeth except upper 2nd molars and
upper lateral incisors. In the facial-lingual dimension, clinically
acceptable results were observed for upper canines and lateral
incisors, lower 1st molars, lower 1st and 2nd premolars, and lower
canines. Discrepancy in the vertical dimension was minimal with
only lower 2nd molars exceeding clinically acceptable limits.
Discrepancy exceeded clinically acceptable levels for crown torque,
crown tip, and crown rotation with the exception of crown torque on
lower 2nd premolars and crown tip on lower 1st molars and lower 2nd
premolars. CONCLUSION: The effectiveness of SureSmile treatment to
achieve predicted final tooth position is highly variable for tooth
type and dimension of movement.

Endosseous implants are important options for
restoring edentulous dental sites. Patients and practitioners are
interested in implant systems that can osseointegrate faster and
predictably in compromised patients. Recent research has shown that
utilizing biomimetic peptides (i.e. RGD and SNa15) on titanium
surfaces could improve osteoblastic responses. RGD is a peptide
sequence important in binding nearly half of all integrins and is
important in cellular adhesion. SNa15 is a modified peptide
sequence from the N-terminus of the salivary protein statherin,
which has been shown to be important in calcium phosphate mineral
binding and nucleation. The aim of this study was to compare the in
vitro outcomes associated with osseointegration (osteoblast-like
cell adhesion, spreading, proliferation, and differentiation)
between the titanium surfaces coated with RGD, SNa15, or a
combination of the two biomimetic peptides via a covalently bound
elastin-like biopolymer (p-). The co-immobilized titanium surfaces
had either a 1:1 molar ratio of biomimetic peptides on separate
biopolymers (pRGD and pSNa15) or a coating containing both peptides
on a single biopolymer (pRGD-SNa15). Cellular adhesion, spreading,
and proliferation was assessed with fluorescent labeling of actin,
vinculin, and/or nuclear DNA and viewed with a light microscope.
ELISA was used to determine early- and late-stage osteoblastic
differentiation by measuring alkaline phosphatase and osteocalcin,
respectively. Our results indicate that while titanium surfaces
coated with only pRGD displayed greater cell adhesion, there were
nonsignificant differences between titanium surfaces coated with
elastin-like peptide polymers in terms of other
osseointegration-related outcomes. We also noted a pattern that
titanium surfaces coated with hydrophobic RGD-containing polymers
were associated with an abnormal late stage osteoblastic
differentiation pattern. In conclusion, we suggest that the
co-immobilization of these biomimetic peptides via elastin-like
polymers should be optimized in further research to improve their
clinical applicability.

Statement of problem: The use of short dental
implants is usually confined to areas of the mouth with limited
vertical bone height. As a result, longer implant crowns are
required to restore the occlusion with the opposing arch, leading
to a disproportionate crown-to-implant ratio (C/I ratio). Purpose:
The purpose of this study was to evaluate the stress distribution
patterns on crestal bone of short implants plateau-designed
connected with different crown heights, and to compare these
findings with those of regular implants. Material and Methods: Two
implant designs (∅5 mm×6 mm and ∅5 mm×11 mm) and abutments (∅5 mm×9
mm and ∅5 mm×5 mm) were scanned using a micro-CT scanner. STL
surface models were subsequently extracted to generate surfaces.
All models were imported to Abaqus CAE for three-dimensional
non-linear finite element analysis. For each implant model,
loadings of 100, 200, and 700 N were simulated in 2 directions:
pure vertical and at an oblique angle of 15 degrees. Results: At
100 and 200 N oblique loads, peak compressive and tensile stresses
ranged from 18.30 to 53.57 MPa and 7.39 to 43.95 MPa, respectively.
At 700 N oblique loads, the ∅5 mm×6 mm implant with long abutment
model exceeded the compressive and tensile strength of cortical
bone, at 193.4 and 158.6 MPa correspondingly. For 700 N oblique
loads, increasing crown height increased peak compressive stress by
28.6 % and peak tensile stress by 28.4 %. Stress distribution
patterns showed the highest amounts of stress located at the
implant crest module in cortical bone. Conclusion: Stress
distribution patterns for the oblique loads of 100 and 200 N were
similar in all models, regardless of the C/I ratios. All peak
stresses were within physiological tolerance. At 700 N loads,
increasing crown height resulted in bone failure.

Introduction: Few methods exist to study central
nervous system processes following intraoral somatosensory
stimulation using functional magnetic resonance imaging (fMRI), due
to inherent technical difficulties associated with this imaging
tool. Our goal was to develop and perform feasibility testing of a
novel device capable of delivering valid and reliable intraoral
somatosensory stimuli at dental chair-side and during MRI. Methods:
Details of a device designed to deliver intraoral dynamic pressure
stimuli are described. Device testing took place in three settings:
a) laboratory testing to assess range of stimulus force
intensities, b) dental chair-side to assess reliability, validity
and discriminant ability in force-pain relationship; and c) MRI to
evaluate magnetic compatibility and ability to evoke brain
activation in painfree subjects similar to those described in the
literature. Results: A novel device capable of delivering valid and
reliable intraoral somatosensory stimulation was developed
(ICC=0.89, 0.78-1 [95% CI]). Psychophysical data analysis showed
high discriminant ability in differentiating painfree controls from
AO cases (sensitivity=100%, specificity=86.7%, area under ROC
curve=0.99). fMRI results of intraoral dynamic pressure pain in
painfree subjects revealed activation of brain areas typically
associated with acute pain processing including thalamus, primary
and secondary somatosensory, insular, and prefrontal cortices.
Conclusions: A novel psychophysical method to deliver dynamic
intraoral pressure stimulation was developed and validated,
allowing non-invasive exploration of cortical and subcortical
mechanisms of intraoral somatosensation.

Statement of problem. Pneumatization of the
maxillary sinus limits the quantity of alveolar bone available for
implant placement and may result in a lack of primary stability and
difficulty in achieving osseointegration. Purpose. The purpose of
this study was to retrospectively analyze a cohort of patients who
had implants placed in the posterior maxilla and calculate the
prevalence of sinus augmentation and identify factors associated
with it. Material and methods. With IRB approval, dental records
from a population of patients who had implants placed in the
maxillary posterior region, between January 2000 and December 2004,
were used to create a data base. Independent variables were divided
into continuous (age of the patient at stage 1 implant surgery
(S1), time between the extraction and S1, time between the
extraction and sinus augmentation, time between the sinus
augmentation and S1) and categorical (gender, implant failure,
American Society of Anesthesiologists system classification,
smoking, osteoporosis, residual crestal bone height, implant
position, implant proximity, prostheses type, and implant diameter
and length). The dependent variable was the incidence of a sinus
augmentation procedure. Simple logistic regression was used to
assess the influence of each of the factors on the presence of
sinus augmentation (P<.05). Results. The final database included
502 maxillary posterior implants with an overall survival rate of
93.2% over a mean follow-up period of 35.7 months. Of 502 implants,
272 (54.2%) implants were associated with a sinus augmentation
procedure. Among variables, residual crestal bone height
(P<.001), implant position (P<.001), implant iv proximity
(P<.001), prosthesis type (P<.001), implant failure
(P<.01), and implant diameter (P<.01), were statistically
associated with sinus augmentation. Conclusions. Within the
limitations of this retrospective study, the results suggest that
more than half (54.2%) of the maxillary posterior implants were
involved with a sinus augmentation procedure. The prevalence of
sinus augmentation increased with decreased residual crestal bone
height, more posterior implant locations, and complete or partial
edentulism. Sinus augmentation was significantly associated with
implant failure and wide implants.

Purpose: The central focus of this retrospective
chart review is the comparison of survival rates for dental
implants placed by residents in Oral and Maxillofacial Surgery and
Graduate Periodontics in coordination with restorative treatment
provided by residents in Graduate Prosthodontics at the University
of Minnesota School of Dentistry. Materials and Methods: Records
for 246 patients treated at the University of Minnesota School of
Dentistry, Minneapolis, Minnesota between 1994 and 2008 met the
inclusion criteria for this study. Potential records were initially
identified using an existing database maintained by the school’s
Implant Program that contains dates, record numbers, implant types,
and departmental affiliations of the restorative and surgical
clinicians who planned the implant surgeries. A total of 968
patients with 2,591 implants in this database had a resident in
Graduate Prosthodontics listed as the restorative clinician and a
resident associated with either Oral and Maxillofacial Surgery or
Graduate Periodontics listed as the surgical clinician. The 246
records in this review were selected based on the patient having a
recorded visit to any department in the School of Dentistry within
the last 12 months. Approval for this review was obtained through
the Institutional Review Board. The principle investigator
performed a manual review of the patient records and collected data
relating to implant survival, grafting, restoration survival, and
technical complications. Statistical analysis was performed by
consultants from the Biostatistical Design and Analysis Center.
Results: A final total of 228 patients and 707 implants were
available for analysis. The exclusion of 17 charts was based on
final implant placement being performed by faculty or restorative
work provided by faculty or dental students. A total of 39 failures
were observed in 29 patients. The 5-year survival rate for all
implants observed in this study was 95.3%, which includes failures
due to implant fracture (0.99%). Oral and Maxillofacial Surgery
residents treated 106 patients with 329 implants and a 5-year
survival rate of 95.4%. Graduate Periodontics residents treated 134
patients with 378 implants and a 5- year survival rate of 96.9%.
Survival rates for both departments were calculated without
failures due to implant fracture. Accepting a threshold value for
significance of p<0.05, Cox Proportional Hazards Regression
demonstrated no significant difference in survival rates between
the two departments as the pvalues obtained were 0.59 when all
failures were included and 0.4 when failures due to fracture were
excluded. Prosthetic utilization of the implants included single
crowns (49.65%), fixed partial dental prostheses (23.76%),
mandibular fixed complete dental prostheses (9.90%), maxillary
removable complete dental prostheses with implant supported fixed
bars (6.65%), mandibular removable complete dental…

Purpose: To compare the microleakage of a
self-etching sealant with a traditional phosphoric acid-etched
sealant under non-contaminated and saliva-contaminated conditions.
Methods: Seventy-eight sound extracted human molars were used
(n=13). Teeth in groups 1 and 2 were cleaned with pumice,
phosphoric acid etched, rinsed, drying agent applied, sealant
placed (UltraSeal XT plus) and light cured. Teeth in groups 3 and 4
were sealed using the same steps as groups 1 and 2 with the
addition of a bonding agent placed underneath the sealant. Teeth in
groups 5 and 6 were cleaned with a proprietary flour pumice and
rinsed prior to being sealed with a self-etching sealant (Enamel
Loc®). Teeth in groups 2, 4 and 6 were contaminated with saliva and
thoroughly air-dried prior to the sealant placement. All teeth were
subjected to a thermocycling process, stained with silver nitrate,
sectioned, and the images of sealant on the occlusal surface were
recorded. Microleakage distance was measured and subjected to a
two-way ANOVA analysis. Results: Significantly larger microleakage
distances were found for the selfetching sealant compared to the
traditional sealant group and those sealants that had a bonding
agent placed underneath them. (P=0.0001). Saliva contamination did
not significantly affect the microleakage distance (P=0.1674).
Conclusions: Under the conditions used in this in-vitro study, the
self-etching sealant, regardless of contamination condition, had
extensive microleakage distances in comparison to little
microleakage in the traditional phosphoric acid-etched sealant
and/or those that had a bonding agent used underneath
them.

An understanding of tooth anatomy is an important
cornerstone in providing successful endodontic treatment. Detailed
images of tooth anatomy using microcomputed tomography allow
measurements of pulpal floor anatomy, canal configurations, canal
dinemsions, root wall thickness, presence of calcifications, and
apical anatomy. The aim of this study of eighteen mandibular second
molars using micro CT was to investigate various aspects of tooth
anatomy. The most frequently found mesial root canal configuration
was Vertucci Type 7 (1-2-1-2) in 33% of samples. Distal canals were
most frequently Vertucci Type 1(1), with 61% of samples showing
this configuration. 11% of samples had two canals, 44% of samples
had three canals, 33% of samples had four canals, and 11% of
samples had five canals at some point along the root. On average,
root wall thickness between the mesiobuccal root and the furcation
was the thinnest at 1.23 mm. The mesiolingual canal root wall
thickness was on average 1.29 mm, and the distal was 1.41 mm. 78%
of samples had calcifications present in both the pulp chamber and
within the canals; 17% of samples were free from calcification in
both the pulp chamber and within the canals; one (6%) sample had
calcification in the chamber only. Other reported data included
canal dimensions at one millimeter increments along the root,
pulpal floor anatomy, and apical anatomy.

With the introduction of Highly Active
Anti-Retroviral Therapy (HAART), HIVinfected patients have had
long-term survival rates increase dramatically. A significant
sequela from HIV-infection and side effect from some HAART
medications is the loss of bone mineral density. The purpose of
this study was to investigate the effects of a common HAART
medication, tenofovir, on bone cells. It was hypothesized that
tenofovir alters the gene expression of osteoblasts and
osteoclasts, which creates an imbalance in the bone remodeling
system and skeletal change. To test this, mice were treated with
tenofovir, then serum was obtained to determine changes in
osteocalcin, osteopontin, and carboxyl telopeptides of type I
collagen activity. Furthermore, changes in osteoclast numbers were
obtained by tartrate resistant acid phosphatase staining of mouse
femurs. While other studies have shown that gene expression is
altered in tenofovir treated cells, the present study did not find
a significant difference in osteocalcin, osteopontin or carboxyl
telopeptide of type I collagen activity for tenofovir mice.
However, tenofovir decreased the number of osteoclasts in tartrate
resistant acid phosphatase stained sections in mice. Given that HIV
patients taking tenofovir medication have an increase in
osteoporosis and the results of this study indicate tenofovir alone
creates osteopetrotic effect, further studies are needed to
determine the relationship between HIV infection and
tenofovir.

The aim of the present study was to investigate if
a relationship exists between certain skeletal and dental
characteristics and the amount of incisal wear of mandibular
incisors. In this retrospective, cross-sectional study, an index
specifically designed to measure incisal wear on dental study casts
was used to examine mandibular incisal wear in 215 subjects using
pre-treatment records from the University of Minnesota orthodontic
clinic. Multiple linear regression analysis (R2 = 0.3886) resulted
in a regression model with five independent variables. Age, gender,
incisor irregularity, UI-SN and Sn-GoGn were all shown to be
significantly associated with increased mandibular incisal wear.
Based on these findings, a prudent clinician may be able to use
dental and skeletal relationships to better predict patients who
are at a risk for increased tooth wear.

Introduction: Panoramic images are an important
part of a patient’s dental and orthodontic record. They provide
broad coverage and are a valuable screening tool for potential
abnormalities and normal dental development. Dental students learn
much about panoramic image anatomy during dental school but are not
usually taught a systematic method of image interpretation. The use
of an eye-tracker will allow visualization and statistical analysis
of interpretation method to find differences between newer and
experienced clinicians. Information gathered can be used to
recommend an interpretation method to newer clinicians as they
begin their dental career. Methods and Materials: Two groups of
clinicians were created: ten clinicians with more than five years
clinical experience (experienced clinicians) and ten clinicians
with five years or less experience (newer clinicians). Five
panoramic images, three which contained no significant findings and
two which had significant findings, were used in the study. An
eye-tracker and the eye-tracking software were used to record and
evaluate the interpretation methods to find differences between the
groups. Results: Newer clinician’s interpretation path entered more
areas of pathology or abnormality in portions of the image other
than the dentition. Areas of abnormality or pathology within the
dentition were entered equally by the interpretations paths of both
groups. Newer clinicians had longer interpretation times and their
interpretations included more fixation points and covered more
image area. Experienced clinicians were quicker with interpretation
and showed more of an interpretation pattern. Conclusions: Newer
clinicians are more complete (interpret the entire image), have
more fixation points and spend longer time interpreting. Newer
clinicians often had no pattern to interpretation, but had
interpretation paths which entered areas of pathology or
abnormality due to completeness of interpretation. Experienced
clinicians showed much more pattern (systematic approach) to
interpretation, but were less complete. In both groups,
interpretations times were significantly longer when the entire
image was interpreted.

INTRODUCTION: The literature regarding survival of
short implants is conflicting. Furthermore, excessive
crown-to-implant ratios have been cited in the literature as being
detrimental to implant survival. Hence, the purpose of this study
was to determine the 3- year survival estimates, and the

roughened, plateau-design implants. Additionally, risk factors for
failure were evaluated. MATERIAL AND METHOD: This retrospective
cohort study involved 341 patients who possessed at least 1 single
≤6mm length plateau-design implant supported restoration that had
been surgically placed between August 2000 and June 2007. An
electronic chart review was conducted to acquire data on patient
demographics, implant location, bone density, length of time in
function and/or implant failure, as well as the most recent digital
periapical radiographs in which the entire crown and implant were
visible. Data analysis was conducted utilizing appropriate
univariate and multivariate regression statistics, as well as
non-parametric Kaplan-Meier survival analysis in order to determine
the overall 3-year survival rate. RESULTS: Data from 573 single
implant-supported fixed restorations were tabulated and included in
the study. 13 implants failed overall. The 3-year Kaplan-Meier
survival analysis adjusted for clustered observations was found to
be 93.9%. The average (SD) crown-to-implant ratio of implants that
were in function was 2.06 (0.4), and the average (SD)
crown-to-implant ratio of those that failed was 2.02 (0.3). This
difference was not statistically significant. Results from the
multivariate model found quality of bone, and history of adjacent
endodontically treated teeth to be statistically associated with
implant failure, with hazard ratios of 13.2 and 2.7 respectively (P
≤ 0.05). CONCLUSIONS:The results of this study suggest that the
survival of short, roughened, plateau-design dental implants is
comparable to implants of traditional lengths, which suggests that
these implants are a clinically acceptable option where alveolar
bone height is limited or anatomic limitations
exist.

Introduction: The use of digital models for
simulation of orthodontic treatment is currently hampered by
time-consuming manual segmentation processes which rely on the user
to manually define tooth boundaries for separation into individual
tooth objects. A new software tool has been developed that almost
completely automates the segmentation process requiring little
input from the user. The software combines novel methods to analyze
the 3D mesh curvatures that make up a digital model, identify the
gingival margin for separation of the teeth from the gingival
tissues, repair the gingival margins, and then identify the high
curvature vertices on the gingival margins to determine
segmentation boundaries between teeth. Methods: Thirty pretreatment
models (15 upper, 15 lower) with varying amounts of crowding and
spacing were digitally scanned using an R700 digital model scanner
(3Shape, Copenhagen, Denmark). Each digital model was then
segmented using the newly developed software tool and then visually
evaluated for segmentation accuracy. To determine inter- and intra-
operator repeatability, two different examiners participated in
this study. To use as a comparison, the same 30 digital models were
also segmented using 3Shape’s OrthoAnalyzer® and evaluated for
segmentation accuracy in the same manner. Results: Of the 387
possible separators on the 30 models tested, 380 were correctly
placed by the new software tool (98.2% accuracy, 5 omitted
separators, 2 misplaced separators). On a tooth by tooth basis, a
total of 77 errors were observed on 417 teeth; 67 being minor
errors that did not affect the segmentation of the models and 10
being major errors that did affect the segmentation of the models
(83.0% success in accurate tooth anatomy reproduction after
segmentation). Both inter- and intra- operator repeatability was
high. Using OrthoAnalyzer® to segment the same 30 digital models,
all 387 separators were correctly placed and there were 14 tooth
anatomy reproduction errors. Conclusions: Initial results indicate
that mesh segmentation algorithms can be developed to accurately
segment digital dental models in most situations while requiring
little user time. Further development of these algorithms could
provide the orthodontic practitioner fast, easy treatment
simulation for help in treatment planning.

▼ Background: Corticosteroid medications have been researched extensively in oral surgery procedures for the proposed reduction in trismus, swelling, and pain. No consensus has been determined for the most efficacious type, timing, and dosage of medication thus far. In addition little is known about the usage of corticosteroids for periodontal and implant related procedures. The aims of this review are to help clinicians understand the usage of corticosteroid medications in various dental surgeries. Methods: The PubMed-MEDLINE and the Cochrane-CENTRAL databases were searched through and up till June 2015 to identify appropriate studies regarding this aim. Appropriate studies were those reporting on the usage of corticosteroids related to its pathophysiology, surgical related outcomes, and patient related outcomes in dental procedures. Conclusions: The search yielded 256 unique papers after selection resulted in 12 publications that met the eligibility criteria. In general the usage of corticosteroids in third molar extractions improved post operatives outcomes related to edema, trismus, and a slight reduction in pain. However, It cannot be recommended to use corticosteroids for pain management. Due to the various types, routes, and dosages of corticosteroid used in studies, no specific drug, route, or dosage can be recommended by literature. The usage of corticosteroids for periodontal and implant related procedures has not been investigated. Further research is required to investigate the possible benefits of corticosteroids on reduction of surgical swelling in periodontal and implant related surgeries.

▼ Parodontiitti on maailmanlaajuisesti erittäin yleinen sairaus. Jopa 50% maailman aikuisväestöstä kärsii parodontiitista, ja 11 % taudin vaikea-asteisesta muodosta. Vaikea-asteinen parodontiitti onkin ihmiskunnan kuudenneksi yleisin sairaus.
Parodontiitin ehkäisyssä on paljon parantamisen varaa. Tauti aiheuttaa suurta haittaa yhteiskunnallisesti, oraalisesti ja systeemisesti. Perinteisesti on keskitytty korjaamaan taudin aiheuttamia vaurioita, ja huomio on kiinnittynyt lähinnä suuren hoidontarpeen potilaisiin. EFP:n (European Federation of Periodontology) mukaan olisi aiheellista lisätä painotusta parodontiitin ennaltaehkäisyyn. Ehkäisevän hoidon suunnitelma tulisi räätälöidä yksilöllisesti potilaan tarpeiden mukaan riskianalyysin ja diagnoosin perusteella.
Suuressa mittakaavassa tulisi lisätä ihmisten tietoutta ienverenvuodosta varhaisena merkkinä parodontiitista. Lisäksi pitäisi saada hammaslääkärit mittaamaan ientaskut rutiinisti kaikilta potilailta. Suun terveydenhuollon henkilöstön tulisi aktiivisesti opastaa potilaita suun terveydenhoidossa ja parodontiitin ennaltaehkäisyssä. Toisaalta potilaalla on omahoidon välinein ja -tuottein vain rajalliset keinot hoitaa sairauttaan, ja on tärkeää, että potilaat pääsevät tehokkaaseen ehkäisevään hoitoon.
Tämän syventävän tutkielman tavoitteena on koota uudistuneet ohjeet parodontiitin ennaltaehkäisemiseksi. Ohjeet on eritelty yksilön, suun terveydenhuollon tiimin ja julkisten viranomaisten tasoille. Työn käsittelyssä sivuutetaan peri-implantti mukosiitin ja peri-implantiitin ennaltaehkäisy, sekä tulehduskipulääkkeiden, antibioottien ja matala-annoksisen doksisykliinin käyttö hoitomuotona. Toteutan syventävän tutkielmani kirjallisuuskatsauksena.

▼ Midpalatal expansion has been used in orthodontics to address maxillary deficiency in the transverse dimension. The separation of the palate causes micro tears along the suture and repair ensues in the resulting space. However, even after a period of stabilization, relapse can occur to adversely affect the orthodontic treatment outcomes. Currently, there is no consensus as to the cause of relapse; whether, it is mediated by the bony palatine shelves or the dense connective tissue covering them. Therefore, elucidating the molecular and cellular mechanism involved in the midpalatal suture expansion can be used as a foundation to explore ways to reduce relapse in orthodontic patients who undergo maxillary expansion. This is especially critical in many craniofacial patients, such as those with cleft lip and/or palate (CLP) malformations, in which maxillary expansion provides a sound foundation for dental development. The current therapeutic modalities do not provide a stable maintenance of the expansion, as relapse can occur in up to 45% of patients. In addition, prior research has shown that the anti-resorptive drug class of bisphosphonates (BP) can reduce the relapse ratio following expansion. However, the process by which BP affect relapse is still unclear. The long-term goal of this study is to understand the etiology of relapse and to diminish relapse after maxillary expansion. This study hypothesized that oral myofibroblasts in soft tissue played a role in relapse following palatal expansion and that BP decreased relapse by inhibiting myofibroblast differentiation of normal human oral fibroblasts (NHOFs). The specific aims of this study were (1) to examine the changes that BP have on myofibroblasts and their differentiation of NHOFs; and (2) to establish a mouse model for studying palatal expansion using a modified expander. BP inhibited gene expression and protein expression of α-SMA in NHOF cells in a time-dependent and dose-dependent manner. A functional collagen matrix assay demonstrated the anti-contractile effect of BP on myofibroblasts. The establishment of the mouse model involved mice that underwent 7 days of palatal expansion using an expander with a helical spring and arms that loop around the central incisors. Midpalatal skeletal expansion was successfully achieved in mice. After expansion, the control group (n=15) were injected with physiologic saline solution (5mg/kg, 0.9% sodium chloride) and the experimental group (n=15) were subcutaneously injected with 0.1mg/kg of Zoledronate dissolved in physiologic saline solution. The expanders were then be converted to retention appliances and allowed 7 days of retention. At the end of retention, half from each group was immediately sacrificed. The other half was sacrificed after undergoing 7 days of relapse in which the retention device were removed. ▼ Lääketieteen menetelmien kehittyessä kasvaa kysyntä ja tarve uusille menetelmille potilaiden hoidossa. Tutkimuksen myötä on saatu tietoa kantasoluista, niiden erilaistamisesta ja kasvatuksesta laboratorio-olosuhteissa. Kasvua ja erilaistumista ohjaavien tekijöiden vaikuttaessa soluja voidaan ohjata erilaistumaan haluttua suuntaa kohti. Luukudospuutosten hoidossa on käytetty biomateriaaleja jo muutamia kymmeniä vuosia. Viime vuosina on alettu käyttämään erilaistettuja autologisia kantasoluja yhdessä biomateriaalien kanssa. Biomateriaalit voivat olla luuta tai luuta korvaavaa materiaalia, joita voidaan tarpeen mukaan muotoilla haluttuun muotoon. Alan julkaisuissa on esitetty jo muutamia aiheeseen soveltuvia potilastapauksia, joten aiheeseen liittyvä aktiivinen tutkimustyö on maailmalla käynnissä.
Tässä tutkielmassa käsitellään kirjallisuuskatsauksen muodossa nykyhetken vertaisarvioitua tietoa rasvakudosperäisten kantasolujen ja kasvutekijöiden käytöstä kallon alueen luukudospuutosten hoidossa. Tutkielma sisältää myös pienen osan käytännön työn kuvaamisesta.

▼ Tutkielma on systemaattinen kirjallisuuskatsaus, jonka aiheena on korjaava karieshoito lapsilla keskittyen lähinnä hoidossa käytettäviin materiaaleihin. Tutkimuksen lähteenä on tieteellinen lähdeaineisto. Haut on tehty avainsanoilla MEDLINESTA ja Pubmedistä. Korjaavaa hoitoa tarvitaan lähinnä karieksen takia, jota pyritään hallitsemaan ennaltaehkäisevästi, jotta korjaavalta hoidolta vältyttäisiin. Korjaavaa hoitoa kuitenkin tarvitaan, mikäli kariesvaurio on jo edennyt niin pitkälle, että on vaarana pulpan tulehtuminen.
Amalgaami oli 1980-luvun lopulle asti paljon käytetty materiaali korjaavassa karieshoidossa ja on edelleenkin muualla kuin pohjoismaissa. 1980-luvulla tiedostettiin amalgaamin mahdollisia ympäristöhaittoja sen elohopeapitoisuudesta johtuen ja suositeltiin vaihtoehtoisia materiaaleja erityisesti lasten karieshoidossa. Tutkimusten perusteella täytteiden elinkaari on lyhentynyt samaan aikaan, kun amalgaamin käyttö on vaihtunut tekniikkasensitiivisempiin materiaaleihin. Toisaalta karies on edelleenkin yleistä lasten ja nuorten keskuudessa nykypäivän Suomessa, esim. 12-vuotiailla on keskimäärin 1,2 karieksen vaurioittamaa hammasta.
Tavoite materiaalivalinnassa on, että täyte kestää maitohampaan elinkaaren loppuun saakka. Materiaalit kestävät hampaissa vähemmän aikaa potilailla, joiden kariesriski on suuri. Paikkojen kestävyyden tutkiminen on haasteellista; elinkaarianalyysiä on käytetty tähän tarkoitukseen muun muassa kohorttitutkimuksissa.
Materiaalien valinnan suhteen ei ole olemassa yhtenäistä kansainvälistä käytäntöä. Suomessa käytetyimmät materiaalit maitohampaiden korjaavassa hoidossa ovat nykyisin lasi-ionomeeri, resiinivahvisteinen lasi-ionomeeri, kompomeeri ja vähäisessä määrin myös yhdistelmämuovi. Lasten pysyvien hampaiden korjaavassa hoidossa käytetään nykyisin lähinnä yhdistelmämuovia. Teräskruunut ovat tutkimusten mukaan erittäin kestäviä. Suomessa teräskruunuja käytetään kuitenkin vielä melko vähän.
Korjaavan hoidon materiaali tulee valita joka kerta tilannekohtaisesti. Korjaavassa hoidossa materiaalivalintaa ohjaa lapsen ko-operaatio ja käyttöalue. Kompomeeri on suositeltava materiaali maitohampaaseen aina, mikäli lapsen yhteistyökyky on riittävä, jolloin työn tekninen suoritus on mahdollista toteuttaa hyvin. Tärkeintä on kuitenkin karieksen hallinta.

▼ Tämän tutkimuksen tarkoituksena on tutkia lastenreumapotilaiden purentaa ja sen mahdollista muuttumista lastenreumapotilaiden kasvaessa. Tutkimusongelmana on selvittää, millaisia purennallisia piirteitä esiintyy lastenreumapotilailla ja miten ne poikkeavat terveiden lasten vastaavista piirteistä. Tutkimusaineistona on Pohjois-Suomessa asuvia lastenreumaa sairastavia lapsia. Aineisto on Päivi Arvosen väitöskirjatyön aineisto.
Lastenreumapotilaita on seurattu useina eri käyntikertoina säännöllisin väliajoin. Tähän tutkimukseen otettu huomioon sellaiset potilaat, joilla on ainakin kolme käyntiä vastaanotolla ja huomioon on otettu potilaiden ensimmäinen, toinen ja kolmas käynti. Vastaanotolla potilaiden tiedot on kerätty paperilomakkeelle, joista tiedot on siirretty tietokoneelle taulukointia ja arvojen laskemista varten. Arvojen laskemisessa on käytetty apuna Excel-taulukointiohjelmaa, jonka avulla on esitetty taulukkomuodossa lastenreumapotilaiden purentaa ja sen muutoksia.
Tuloksista huomattiin, että ensimmäisellä käynnillä horisontaalinen ylipurenta (HYP) oli 10%:lla potilaista yli 4 mm. Vertikaalinen ylipurenta (VYP) puolestaan oli 17%:lla potilaista yli 4 mm. Kun verrataan lastenreumapotilaiden 1. ja 3. käynnin arvoja, HYP suureni 20%:lla ja VYP suureni 23%:lla. Ensimmäisellä käynnillä 13%:lla potilaista oli avopurenta. Avopurenta paheni 7%:lla potilaista. Ensimmäisellä käynnillä AII-purenta oli 10%:lla potilaista. AII-purenta pahentui 10%:lla potilaista. Maksimaalinen avaus pieneni ensimmäisen ja kolmannen käynnin välillä 17%:lla potilaista.

▼ Kirjallisuuskatsauksen tavoitteena oli selvittää eroavatko aikuisten oikomishoitopotilaiden, joilta on poistettu 4 ensimmäistä premolaaria, hoitotulokset muiden aikuisten oikomishoitopotilaiden hoitotuloksista. Lähteinä käytettiin Terveysportti, Pubmed ja Medline tietokantoja sekä ortodontian oppikirjoja. Kaikkiaan mukaan otettiin 26 oikomishoitoa käsittelevää artikkelia, jotka pääasiassa käsittelivät aikuisten poistopotilaiden oikomishoitoa. Katsauksessa havaittiin, että aikuisten poistopotilaiden hoitotulokset ovat samankaltaisia kuin muillakin potilasryhmillä. Poistoilla voi olla vaikutusta hoidon kestoon, mutta muilla tekijöillä kuten kojeiden rikkoutumisilla, väliin jääneillä hoitokäynneillä, oikojan taidoilla ja alkuperäisellä purentavirheellä voi olla myös vaikutusta. Biprotruusiivisilla potilailla premolaaripoistot osana oikomishoitona voivat parantaa kasvojen profiilia. Mitään varmoja johtopäätöksiä ei tämän katsauksen perusteella voida tehdä.
