虚血性心疾患と脂質代謝異常との関連についてFramingham Studyを始め多くの報告がある。アポリポ蛋白遺伝子・脂質関連酵素遺伝子解析と脂質代謝・冠動脈疾患との関連についても多くの報告があるが,日本人を対象とした報告,特に冠動脈疾患との関連についての知見は乏しい。遺伝子解析の方法として疾患群と正常群とを比較するAssociation studyと冠動脈病変の重症度の評価により,日本人虚血性心疾患患者におけるアポリポ蛋白多型と血清脂質関連項目・冠動脈病変との関連を明らかにすることを目的とし以下の検討を行った。当科にて冠動脈疾患を疑い冠動脈造影検査を施行した80症例(M:F=48:32,平均年齢57.1歳)を対象とし,AHA Comitte Reportに基づき冠動脈狭窄度により3群に分類し,また冠動脈重症度の2つの指標を算出し検討に用いた。血清脂質関連項目として総コレステロール(以下TC),中性脂肪(以下TG),高比重リボ蛋白コレステロール(以下HDL-C),アポリポ蛋白AI,AII,B,CII,CIII,E(以下Apo AI,apo AII,apo B,apo CII,apo CIII,apo E), Lipoprotein(a)(以下Lp(a)),Remnant like particle-cholesterol(以下RLP-C)を定量した。アポリポ蛋白遺伝子多型はapo AI/CIII/AIV遺伝子多型[Msp I,Sac I restriction fragment length polymorphisnl(以下RFLP)],apo B遺伝子多型[Xba I RFLP,signal peptide(以下SP)多型,variable… (more)
