▼ Introdução: Este estudo realizou-se no ACES X Cacém-Queluz entre Outubro e Dezembro de 2011. Teve como objectivo conhecer a auto percepção do estado de saúde dos indivíduos que recorreram a junta médica na USP do ACES X, para atribuição de grau de incapacidade entre Outubro de 2011 e Dezembro de 2011. Material e métodos: Este estudo é observacional, transversal, analítico de abordagem quantitativa. O estudo incidiu sobre 59 indivíduos que recorreram à junta médica na USP do ACES X. Foram excluídos deste estudo indivíduos que devido à sua situação mental se encontravam incapacitados para assinar o consentimento informado, menores de 18 anos, indivíduos que devido a défice de visão se encontravam incapacitados para ler e/ou assinar o consentimento informado, não sabiam ler e escrever, eram surdos-mudos, não dominavam a língua Portuguesa e os indivíduos que recusaram participar no estudo após solicitação. Elaborou-se um questionário sociodemográfico, especificamente para este estudo. Para conhecer a percepção que os indivíduos tinham da sua saúde utilizou-se o questionário SF- 36 – SHORT – FORM 36, versão 1.0, validado na população portuguesa por Ribeiro (2005). As variáveis independentes consideradas foram o grau de incapacidade atribuído em junta medica; foram ainda consideradas como variáveis independentes dados sociodemográficos (idade, sexo, estado civil legal, viver conjugalmente, viver numa instituição, ter apoio domiciliário, tipo de apoio domiciliário, naturalidade, nível de escolaridade, profissão, ocupação nas ultimas duas semanas, doença que motivou junta medica, data de diagnóstico da doença). Consideraram-se como variáveis dependentes as dimensões do questionário SF-36 (funcionamento físico, desempenho físico, dor corporal, saúde geral, vitalidade, funcionamento social, desempenho emocional, saúde mental) e as componentes física e mental. Para se proceder a análise de dados, utilizou-se o programa informático SPSS versão 20.0, tendo sido feita análise uni e bivariada. Resultados A idade média dos indivíduos que participaram neste estudo foi 55 anos, sendo que 54,2% eram do sexo feminino; 71,2% viviam conjugalmente com alguém, 94,9% tinham naturalidade Portuguesa; 69,5% tinham um nível de escolaridade correspondente ao ensino secundário ou menos. O grupo dos trabalhadores dos serviços pessoais, de protecção e segurança e vendedores foi a mais representado com 28,8%; 44,1% dos indivíduos referiram estar reformados, 27,1% referiram ter exercido uma profissão ou um trabalho mesmo que não remunerado nas últimas duas semanas e 16,9% referiram ter estado doentes desde há 3 meses ou mais. Setenta e quatro virgula cinco indivíduos que solicitaram atribuição de grau de incapacidade fizeram-no devido a doença neoplásica, sendo a neoplasia da mama a mais referida com 20,3%. Cinquenta e quatro vírgula dois indivíduos referiram que a doença que motivou pedido de grau de incapacidade lhe tinha sido diagnosticada há 1 ano ou menos de 1 ano. The Department of Defense enlists approximately 180,000 new military recruits each year and is considered the largest employer of young adults in the United States. Socio-economic support and educational tuition assistance are the two primary reasons indicated to enlist in the military. In recent studies, two other variables, delinquency and adverse childhood experiences (ACEs), have also been shown to be linked to military enlistment. This study investigates these four variables as predictors of military enlistment. Predictors of military enlistment represent critical human and social influences on young adults making the transition from adolescence to adulthood and this knowledge is important for effective clinical and macro social work practice. For young adults with a history of delinquency and ACEs, the military may be one of the only organizations able to facilitate this transition. To investigate these variables as predictors of military enlistment, the study used the National Longitudinal Survey of Youth (NLSY) (1997) dataset to conduct a logistic regression analysis of four independent variables (socio-economic status, educational achievement, delinquency, and ACEs) on the dependent variable, military enlistment. The sample was formed through random selection of matching military enlistees with the same age, race, and gender characteristics to those who never enlisted. The independent variables were defined using data from the NLSY97 dataset for household income, educational level, delinquency index, and an ACEs score was constructed from ten variables from the NLSY97 dataset. The results of the study found that the interaction of delinquency with ACEs produced the strongest predictive factor of military enlistment (p < .05), which was followed by the interaction of delinquency with educational achievement (p < .05).

Previous research has documented the negative impact of Adverse Childhood Experiences (ACEs) on adolescents’ physical health, mental health and health risk behaviors (Bielas et al., 2016; Brown & Shillington, 2017; Isohookana et al., 2016). Although Latino adolescents may experience a higher prevalence of ACEs (Llabre et al., 2017), they have been largely overlooked in the research literature. Not all adolescents who experience adversity develop negative outcomes. Previous research has found emotion regulation, religious meaning-making, self-efficacy, locus of control, purpose, community support, family cohesion, and social support to promote resilience among individuals who experience adversity (Bellis et al., 2017; Cardoso & Thompson, 2010; Culpin, Stapinski, Miles, Araya & Joinson, 2015; Day & Kearney, 2016; Hamby, Grych & Banyard, 2018; Roh et al., 2015; Schweizer et al., 2016). Despite this, ACEs research has focused on negative outcomes instead of identifying protective factors that may help adolescents build resilience. In order to address these gaps, the present study sought to (1) determine the relation between ACEs and Latino adolescents’ subjective well-being (SWB) and (2) identify protective factors that could moderate that relationship. Eighty-eight Latino adolescents completed measures of ACEs, SWB, emotion regulation, religious meaning-making, self-efficacy, locus of control, purpose, community support, family cohesion and social support. Results suggest that the more ACEs adolescents experienced, the lower their SWB. Moreover, external locus of control, internal locus of control, family cohesion and purpose moderated the association between ACEs and SWB. The results suggest that helping Latino adolescents build a high sense of purpose, a high family cohesion, a low external locus of control and a high internal locus of control can promote a higher SWB despite experiencing ACEs. O início da reforma dos cuidados de saúde primários aconteceu em 2005, com o Programa do XVII Governo Constitucional, que tinha como objetivo “atribuir uma particular relevância à reestruturação dos centros de saúde, pela proximidade ao cidadão e pelo contributo que dão à melhoria dos níveis de vida dos Portugueses. ” (Decreto-Lei n.º 88, 2005, p. 3606) Em 2006, surgiram as Unidades de Saúde Familiar (USF) como o primeiro movimento da reforma. As USF são unidades funcionais com autonomia administrativa, técnica e funcional que se organizaram de forma voluntária, que visam a prestação de serviços de qualidade e de proximidade, baseando-se numa nova forma de organizar a acessibilidade do utente à saúde. Em 2008 foram criados os Agrupamentos de Centros de Saúde (ACeS), segundo movimento da reforma. Os ACeS têm como objetivos a eficiência e economia de escala, aplicados à agregação de recursos e das estruturas de gestão. Estes agrupamentos passaram a ser constituídos por diversas unidades funcionais, entre elas as USF e as Unidades de Cuidados de Saúde Personalizados (UCSP).Através da realização de um estudo de caso no ACeS de Oeiras, observaram-se os resultados de seis dos indicadores comuns contratualizados pelas USF e pelas UCSP, entre 2009 e 2011, com o objetivo de compreender as novas formas de organização. Estes indicadores representam quatro áreas: o acesso, o desempenho assistencial, a satisfação e o desempenho económico. O estudo permitiu concluir que as USF obtiveram melhores resultados no triénio em análise, e que as UCSP levaram mais tempo a adaptar-se aos sistemas de informação e à nova forma organizativa, mas no último ano em análise (2011), os seus resultados melhoraram.

The primary health care reform began in 2005 with the XVII Constitutional Government Programme. This Program aimed "to give a strong input to the restructuration of the health centers, due to their proximity to the citizens and due to the contribution they make on the improvement of the life quality of the Portuguese people. "(Decreto-Lei n. º 88, 2005, p.3606). In 2006 were created the Family-Health Units (USF). These voluntarily self-created teams have administrative, technical and functional autonomy and aim to assure health services marked by outstanding quality and proximity, based on a new form of organization. In order to increase the scale efficiency and economy were created, in 2008, the Health-Centers Groups (ACeS), clusters which role was to aggregate resources and management structures, that are presently composed of several functional units such as the USF and UCSP. In this study we approached the ACeS Oeiras. We analyzed the results of six indicators (People access, assistance performance, satisfaction and economic performance) that are common to USFs and UCSP, between 2009 and 2011. Os cuidados de saúde primários são a base do Sistema Nacional de Saúde visto que são o primeiro contacto que os cidadãos têm com o sistema de saúde. Com o Decreto-Lei nº 28/2008, de 22 de Fevereiro, estabeleceu-se o enquadramento legal necessário à criação dos agrupamentos de centros de saúde do Serviço Nacional de Saúde, abreviadamente designados por ACES que prestam os cuidados de saúde primários. Com esta nova estrutura, pensamos ser importante avaliar a satisfação profissional dos colaboradores dos ACES, assim como o seu nível de burnout/engagement, para contribuirmos para a sua adaptação à mudança. Estando desde 2009 em curso a implementação do ACES Oeste Sul, com base numa amostra de 206 colaboradores desta organização de saúde, o presente estudo tem como objectivo principal avaliar o nível de satisfação profissional e o engagement dos profissionais de saúde que aí desempenham as suas funções. Para avaliar estes factores foram utilizados a Escala de Satisfação Profissional, de Luís Graça (1999) e a Utrecht Work Engagement Scale (UWES) de Schaufeli e Bakker (2003) adaptada para a população portuguesa por Alexandra Marques Pinto. Os resultados obtidos permitiram verificar que no geral os profissionais se encontram satisfeitos, embora surjam alguns indicadores de não satisfação ao nível das dimensões: condições de trabalho e saúde; remuneração e segurança no emprego; e que nas três dimensões do engagement: vigor, dedicação e absorção, os profissionais se encontram na sua maioria vinculados com o trabalho.

The primary health care are the foundation of the National Health Care System due to the fact they are the first contact the citizens have with the system health care. With the legislation Decreto – Lei nº 28/2008 dated 22nd February, was established the legal compliance for the aggregation of the health centers of Health National Service, namely ACES responsible for the primary health care. With this new structure , we think it is important assess the level of satisfaction of the professionals of the ACES together with his level of burnout and engagement and by this way contribute to their adaptation to the changes in progress. Initiated in 2009 the implementation of the ACES Oeste Sul, the principal objective of this study, based in a sample of 206 professionals of this ACES, is to assess the levels of job satisfaction and engagement of them. To proceed with this assessment were used the following tool scales: Escala de Satisfação Profissional, from Luis Graça (1999) and Utrecht Work Engagement Scale (UWES) from Schaufeli and Baker (2003), adapted to the Portuguese population by Alexandra Marques Pinto. The results of this assessment on a generic way are showing that the professionals are satisfied, but there are specific indicators of no satisfaction on the following dimensions: working conditions and healthy; salaries and job security; and in the three dimensions of the engagement: vigor; dedication and absorption, the professionals are in its majority connected with their work.

▼ Working memory (WM) is a crucial component of cognitive function that affects learning, reasoning, and problem solving, all of which are important for daily functioning. Therefore, addressing factors that can impact working memory, such as stress, are incredibly relevant to understanding WM efficiency. WM is an important component of higher order cognitive function and high WM capacity has been shown to be important for academic and occupational performance. Thus, understanding the relationship between stress-related factors and WM could aid in identifying strategies to mitigate the deleterious effects of stress on working memory. Although some previous research has indicated a negative impact of acute stress on WM, other research has indicated no impact or even a positive impact of stress on WM. As the relationship between acute stress and WM is mixed, examining other stress-related factors may provide further insight into the relationship. The current study examines how adverse childhood experiences (ACEs) and acute stress influence WM, and how frontal theta and alpha activity are affected by WM task demands. Participants completed a working memory task while their EEG was recorded. Participants then completed the PANAS to assess their current emotional state. Following the PANAS, participants viewed a stressful or neutral video as an acute stress induction, followed by a second PANAS to ensure effectiveness of stress induction. Participants then completed the WM task a second time. Finally, the participants completed the ACEs questionnaire. Bayesian linear mixed effects models were used to examine the relationships between ACEs, acute stress, WM, and frontal theta and alpha frequencies. Findings suggest there is not enough evidence to support a relationship between acute stress, ACEs, WM, and WM-related theta and alpha. While the current study did not reveal a relationship, future research should explore how acute stress and exposure to specific stressors during childhood could explain individual differences in WM.

▼ Faith-based ministries and non-profit organizations focused on strengthening families and family preservation may wrap services around families, but may underestimate the intergenerational impact of childhood adversity and trauma on the family system. Understanding the neurological, educational, and health outcomes as a result of adverse childhood experiences is key to developing evidenced-based, therapeutic programs to best assist clients in strengthening the internal protective factors of hope, resilience, spirituality, and attachment, in an effort to avoid harmful intergenerational beliefs, attitudes, actions, and habits following their exit from programs. The aim of this online, mixed methods study was to examine the relationship between adverse childhood experiences and the internal protective factors of hope, resilience, spirituality, and attachment, through the lens of Attachment theory (Bowlby, 1957) and Resiliency theory (Walsh, 1996). The data collected in this study were obtained through participants in the Buckner Children and Family Services, Inc. Family Pathways Program. Participants were single parents, enrolled in a higher education program, between the ages of 18-45 years of age, male or female, and a resident at one of eight Family Pathways locations in Texas. The voluntary, confidential study was accessed online and consisted of questions from: a demographic questionnaire; the Adverse Childhood Experiences Questionnaire (ACES-Q), the Adult Hope Scale (AHS); the Resilience Scale (RS); the Spiritual Well-Being Scale (SWBS); and the Revised Adult Attachment Scale (RAAS). In addition, a total of five qualitative questions, one after each of the quantitative inventories, gathered additional insight from the participant’s viewpoint and responses were coded into themes. Pearson product-moment correlation analysis was used to examine the relationship between adversity and the internal protective factors of hope, resilience, spirituality and attachment. A response rate of 38% was achieved from a total of 134 possible participants. Yet, their demographic variables were representative of the overall program demographics. The respondents scored higher than the national average (12.5%) in overall ACEs scores with 60.8% of the population scoring 4 or more. The results of the quantitative data revealed that those with higher ACEs scores also had increased attachment anxiety. In the domain of ACEs Abuse, those who suffered physical, verbal, or sexual abuse as a child had a weak to moderate negative correlation with Hope, the subscale of Hope Pathway, Spirituality, the subscale of Spirituality Existential, and a strong negative correlation with the subscales of Attachment Depend and Attachment Close. ▼ Problem: There is a significant dose-dependent relationship between traumatic experiences faced in childhood and poor adult health outcomes including substance abuse, chronic psychological and physical disease, and early mortality. Childhood trauma alters the chemistry of the developing brain and body, negatively influencing long-term health. The Adverse Childhood Experiences (ACE) screening tool is a binary response questionnaire consisting of ten questions that assess for history of childhood trauma. Despite mounting evidence to support the use of this tool and the great potential to enhance preventive, trauma-informed care, screening of adults for Adverse Childhood Experiences in primary care is not common practice. Perceived barriers to implementation cited by providers include time constraints, lack of training and confidence in screening and management of patients with a history of childhood trauma. Duchesne Clinic began the process of screening for ACEs by admission staff during the qualifying process. However, providers required support and guidance as they began to discuss the screening results with their patients and integrate ACE screening scores into plans of care. Project Aim: The aim of this project was to implement a guided reflective practice intervention with Duchesne Clinic providers to facilitate management of patients with high ACE scores. Project Method: This was a quality improvement (QI) DNP project at a safety net clinic in Kansas City, KS. Pre and post-intervention survey data was analyzed to assess changes in providers’ confidence and perceived barriers to management of patients with high ACE scores after four weeks of guided reflective practice intervention. Additionally, thematic analysis was performed to identify themes expressed by the providers in their reflection of their encounters with patients and their perceptions regarding management of patients with high ACE scores. Findings: Each of the seven questions on the ACE Provider Questionnaire yielded mean gains ranging between 0.25 to 1.2. A thematic analysis of the notes taken during all the interviews over the four-week guided reflective practice intervention produced the following themes: time as a barrier; feeling unprepared and unable to help patients with trauma history; re-traumatization; and strategies and tools for caring for patients with a history of trauma. Conclusions: Guided reflective practice was a successful tool to assist the providers at Duchesne Clinic to gain confidence and reduce the barriers to caring for patients with high ACE scores. Additionally, the sessions facilitated rich, honest and productive conversations between the Project Director, the Medical Director of Duchesne and each of the five providers and yielded a set of best practices for ACE screening and management at Duchesne Clinic. ▼ Researchers and practitioners have increasingly focused on institutional responses to campus gender-based violence/harassment, yet they have paid far less attention to graduate student experiences than to undergraduate student experiences. Graduate students operate in a different context from undergraduates, and therefore specific knowledge of gender-based violence/harassment in the lives of graduate students is needed. The purpose of this exploratory, nonexperimental study was to better understand the prevalence of adult gender-based violence/harassment and adverse childhood experiences (ACEs) among graduate students, as well as to understand the relationship between those experiences and participants’ mental health and academic functioning. The study’s theoretical framework combined critical adult learning theories with cognitive perspectives on adult learning, including the neurobiology of trauma. Data used in the current study were originally collected as part of an institutional campus climate survey on gender-based violence; responses from n = 684 of the randomly selected participants were used in the current study’s analyses. Participants commonly reported both adult gender-based violence/harassment experiences and ACEs. The results of two OLS regressions indicated that experiencing more types of adult gender-based violence/harassment or more types of ACEs was associated with higher levels of negative affect and lower levels of mindfulness. Among participants who experienced gender-based violence/harassment in graduate school, independent samples t-tests showed that individuals who reported at least occasional academic functioning difficulties had lower levels of mindfulness and higher levels of negative affect than those who did not experience difficulties. Overall, the findings suggest the need for trauma-informed policies and practices within graduate education and higher education in general. Advisors/Committee Members: Dr. Abigail H. ▼ According to Felitti and colleagues (1998), a significant portion of the general population has been exposed to adverse childhood experiences (ACEs) with subsequent and persistent, dose-related, negative consequences to physical and mental health. Debilitating disturbances to biopsychosocial well-being are significant and frequently lead to fatality in adulthood. After reviewing the prevalence and severity of ACEs, this thesis presents an overview of the literature outlining the biological, social, and psychological factors contributing to the development and progression of disease in the brain and body. Additionally, current trauma-informed interventions are summarized. Three experienced clinicians share practical advice for implementing evidence-based, trauma-informed mental health services. Suggestions for integrating components of empirical literature with practical advice are provided to meet the biopsychosocial needs of those with histories of multiple adverse childhood experiences. A sample integrated curriculum for middle school students who have been exposed to multiple adverse experiences is included. The proposed, strengths-based curriculum enhances trauma-focused cognitive behavioral therapy (TF-CBT), the gold standard for trauma-informed care, with principles from schema-focused cognitive behavioral therapy (SFCBT; Young, Klosko, & Weishaar, 2003); neurobiological and mindfulness research; motivational interviewing; existential psychology; and, attachment theory. Consistent with new recommendations for children with complex trauma histories (Cohen, Mannarino, Kliethermes, & Murray, 2012), the proposed Building Healthy Core Beliefs curriculum is the skill-building, phase one of a three-phase program. In accord with guidelines from TF-CBT, the skills learned in this curriculum include: (a) recognizing, normalizing, and managing the "fight-flight-freeze" response (b) recognizing, identifying, and managing emotions; (c) differentiating inaccurate thoughts from accurate, helpful thoughts; (d) using positive self-talk; (e) practicing mindfulness and meditative exercises; (f) completing a program with sequenced steps and specific tasks; (g) recognizing and celebrating accomplishments; and, (h) building healthy core beliefs. The four healthy core beliefs promoted are: (a) I am loved; (b) I am lovable; (c) I am worthy; and, (d) I am capable of achieving my goals. This curriculum prepares clients for the second-phase of integrated trauma-informed care, i.e., life-scripting. Sample letters to parents or guardian are provided to maintain communication between therapist and caregiver. Homework is designed to encourage positive parent-child interactions.

▼ Actualmente, dadas as restrições orçamentais, a medição da utilidade como instrumento de avaliação e de tomada de decisão no sector da saúde torna-se pertinente. Assim, apresenta-se uma análise dos ganhos em utilidade gerados pela implementação de programas de fisioterapia nos serviços de fisioterapia do Agrupamento de Centros de Saúde Alto Trás-os-Montes I – Nordeste. O cálculo da utilidade gerada, que é fundamental para a prossecução de análises custo-utilidade, realiza-se com recurso ao Short Form-6 Dimensions (SF- 6D). O instrumento de medida da utilidade é aplicado em dois momentos de tempo em conjunto com um questionário que permite caracterizar a população. A variação na utilidade, medida antes e após a intervenção de fisioterapia, permite calcular o ganho em utilidade. A caracterização sócio-demográfica dos utentes permite a aplicação de um conjunto de testes que identificam as características que determinam tais ganhos. Conclui-se que cerca de 78% dos utentes inquiridos verifica uma melhoria no seu estado geral de saúde e que, em média, a variação na utilidade é de cerca de 0,10. O valor médio de utilidade, após a aplicação de um programa de fisioterapia, é de 0,863 sendo que a dimensão relativa à dor física é a que melhores resultados evidencia. Os ganhos mais elevados de utilidade foram encontrados, em média, para os homens, indivíduos com idades entre os 26 e os 45 anos, trabalhadores manuais, reformados, com patologia ortopédica, sem outra patologia, que realizam entre 46 a 60 sessões de fisioterapia e utentes que referem ter conhecimento de alternativas ao serviço público oferecido no seu concelho mas que não ponderam a possibilidade de realizar tratamentos fora do seu concelho de residência. A situação face à profissão, a patologia, o número de sessões realizadas e a isenção/pagamento de taxa moderadora são variáveis que determinam ganhos de utilidade. Nowadays, given the budgetary constraints, the measurement of utility as a tool for evaluation and decision-making in the health sector becomes relevant. Thus, this study presents an analysis of gains in utility generated by the implementation of physical therapy programs in physical therapy services in the Group Health Centre (ACES) Alto Trás-os-Montes I - Nordeste. The computation of utility generated, which is fundamental to the pursuit of cost-utility analysis, is carried out using the Short Form-6 Dimensions (SF-6D). The utility measurement instrument is applied in two moments of time together with a questionnaire that allows characterizing the population. The change in utility, measured before and after the intervention of physical therapy, allows the computation of utility gains. The individuals’ socio-demographic characterisation allows the application of a set of tests that identify the characteristics that determine such gains. It is concluded that about 78% of those individuals surveyed improved their overall health and, on average, the change in utility is about 0,10.
