▼ Infertility is the inability of a sexually active,
non-contraception couple to achieve pregnancy in one year.
Generally, infertility is classified as either primary (67-71%)
when no pregnancy has ever occurred or secondary (29-33%) where
there has been a pregnancy, regardless of the outcome. It affects
13-18% of couples of reproductive age worldwide and is treatable in
many cases [1]. Although, infertility is not life threatening, it
is psychologically a traumatic condition associated with social
stigma in certain societies resulting in intense social and
parental pressure to attain biological parenthood; hence many
infertile couples choose to undergo assisted reproductive
technology as a form of treatment. In recent years, it has been
demonstrated that up to 50% of infertility cases are associated
with male factor because of spermatogenic failure; interestingly in
which ~70% of the underlying etiological cause(s) are known but
remaining 30% are referred to as idiopathic infertility [2] Thus,
the etiology of male infertility is complex and can be reflected in
a variety of conditions like defective spermatogenesis, congenital
absence of bilateral vas deferens (CABVD), infections, and
immunological disorders such as antisperm-antibodies, diabetes,
obesity, pituitary disorders, and imbalance in levels of
gonadotrophic hormones. Genetic factors are considered to be
significant in male infertility [3]. Hence, the aim of the study is
to investigate the chromosomal aberrations, azoospermia factor
(AZF), cystic fibrosis transmembrane regulator (CFTR) gene
mutations in the peripheral blood lymphocytes (PBL) of infertile
men with abnormal sperm parameter and compare it with healthy
fertile control subjects to delineate the genetic causes of male
infertility. This thesis comprises of six chapters. The first
chapter summarizes about male factor infertility: incidence,
classification, etiology, genetic factors, and treatment options.
In around 50% of infertile couples, male-related causes are
involved, such as impaired sperma

▼ Gender scholars argue that in Western culture hegemonic gender beliefs are widespread and stable, and individuals' access to resources and power are contingent upon their ability to conform to these ideals (Ridgeway and Correll 2004 ; West and Zimmerman 2002). However, they also argue that gender is a fluid and dynamic aspect of identity, as demonstrated by individuals' ability to revise personal notions of gender based on other salient features of identity, including, race, class, age, sexual orientation or physical (dis)abilities (Connell 1995 ; Fenstermaker and West 2002 ; Kimmel 2006 ; Ridgeway and Correll 2004 ; West and Zimmerman 2002). This dissertation uses male infertility as a case study for exploring what happens when the perceived foundations of masculinity are destabilized in the lives of heterosexual married men. Male factor infertility is a clinical condition that affects nearly half of the more than seven million infertile couples in the United States. For this original research study, I employed mixed qualitative research methods, including ethnographic observation in male infertility clinics, in-depth interviews with male infertility specialists, clinic staff, infertile men and their wives, and content analysis of medical texts and conference addresses, in order to examine how medical institutions and individuals socially construct male infertility. I argue that the social processes of constructing gender and negotiating masculinity are tightly intertwined with the process of constructing disease. Early in this dissertation I show that preconceived notions regarding women's responsibility for all aspects of reproduction have historically shaped popular understandings of male infertility and hindered the development and organization of male infertility as a medical specialty. I describe how medical practices are designed around preconceived notions of men as sexually knowledgeable and experienced, yet all the while strive to protect presumably fragile masculine identities. I detail the various ways infertile men work to redefine their condition in terms that downplay infertility's threat to masculinity. Throughout this dissertation I argue that gender-constructing and disease-constructing processes are inextricable, because notions of gender inform understandings of disease. When disease presents a threat to gender identities, individuals create new understandings of disease in order to negotiate and stabilize their personal gender identities

▼ Introduction: Infertility is the inability of a couple to conceive after one year of regular unprotected sexual intercourse. In the Ghanaian society women bear the brunt for a couple’s inability to have children. Studies put male factors and female factors on the same level of causation. There seem to be little knowledge about male infertility in our communities. This study sought to assess knowledge about male infertility, identify some of the factors associated with knowledge about male infertility and explore some perceptions about male infertility. It also determined the factors that influenced people’s perception about male infertility in the Kumasi metropolis.
Methods: The study was a cross-sectional quantitative study. Multistage sampling method was used in selecting respondents. Two hundred participants from the Kumasi metropolis were involved in the study. A structured questionnaire was used to collect data on participants’ socio-demographic information, knowledge about male infertility and perceptions about male infertility. Data was entered with Microsoft Excel. STATA software version 14 was used to perform chi square test for associations and multivariate logistic regression analysis.
Results: Sixty-six percent of the respondents had poor knowledge about male infertility. Over sixty-two percent of women had good knowledge about male infertility compared with men. Married respondents were six times more likely to have good knowledge about male infertility. Fifty eight percent thought male infertility was not common. Over 70% thought men were embarrassed to discuss male infertility issues. Over 80% thought that men did not put in effort to preserve their fertility and would not avail themselves for fertility diagnosis.
Conclusion: These findings suggest that more effort should be put into educating the public on male infertility. This will reduce the prevalence of male infertility and reduce the stigma and emotional turmoil women in childless marriages go through.

▼ In recent years, there has been some suggestion of an increase in
male factor
infertility in the industrialized countries with a decline in sperm counts and a rise in sperm pathology.
Male factor
infertility is a multifactorial phenomenon that is
observed in approximately half of infertile couples and affects one man in 20 in the general population. The potential causes of
male infertility arise from a number of factors including genetic, lifestyle factors and chronic diseases. However, a high proportion of infertile
male patients have now been shown to have defective sperm functions related to oxidative stress. Oxidative stress in semen has been speculated as one of the major factors causing
male infertility and has been identified in 30-80% of cases of
male infertility. While oxidative stress is accepted as a significant pathology, there is currently an inadequate knowledge of the exact mechanisms by which oxidative stress develops in
male infertility, as well as a lack of an easy and reliable method for the measurement of seminal oxidative stress in routine clinical use. The main objective of this doctoral thesis is to investigate the underlying causes for oxidative stress in infertile men and the mechanisms by which oxidative stress develops. Furthermore it will also examine the effectiveness of an oral antioxidant therapy for treatment of seminal oxidative stress. During these doctoral studies experiments were designed with the aims of:
• Developing a standardized protocol for the measurement of seminal oxidative stress, that can be conducted in the average clinical laboratory with minimal additional equipment (NBT Assay)
• Examining the causes for oxidative stress in semen. Obesity has previously been identified as a cause of systemic oxidative stress. Therefore I examined if obesity causes oxidative stress to sperm. Seminal inflammation and its role in oxidative damage in semen are also investigated.
• Determination if antioxidant supplementation is an effective treatment of oxidative sperm damage.
• Assessment of the relation between Oxidative stress and sperm DNA methylation. Previous studies have linked
male infertility with epigenetic abnormalities of the
male genome. Since oxidative stress has been shown to interfere with somatic cell epigenetic programming I investigated the possibility of a similar link in sperm.
It is hoped that advances outlined in this thesis will have made a significant contribution to the diagnosis, prevention and treatment of the
male infertility.

discussed. The introduction gives a general overview of both male infertility and
azoospermia. Diagnostic, genetic and therapeutic aspects of male infertility according to the
current literature are discussed. The chapter on diagnosis of obstructions of the genital tract
describes a study on the incidence and management of subtotal obstructions in men with
severe oligozoospermia. Furthermore, genetic risk factors are investigated in tv.ro studies: in
men with congenital bilateral absence of the vas deferens the relationship with cystic fibrosis
is investigated and in men with either azoospermia or severe oligozoospermia cytogenetic
abnormalities, Y chromosome deletions and cystic fibrosis gene mutations are determined.
Practical advises for genetic investigations and genetic counselling are given.
The chapter on management of male reproductive tract obstructions describes the results of
transurethral resection of cystic lesions in the prostatic gland, causing ejaculatory duct
obstruction. In a review on microsurgical treatment of the male genital tract, different
operative techniques, outcome and prognostic factors are discussed extensively, including our
own results. Finally. the technique of epididymal sperm extraction and intracytoplasmic
sperm injection are presented together with the results of these combined treatments for men
with irreparable obstructions of the seminal path.
The scope of this thesis is to give an overview of recent developments in the diagnosis and
treatment of obstructive male infertility, and to discuss the genetic risk factors associated with
these forms of male infertility.

▼ Cystic fibrosis (CF) and congenital absence of the vas deferens (CAVD) are conditions caused by mutations in the CFTR gene. Ninety-five percent of men with CF and/or CAVD have infertility due to azoospermia. Until recently, men with CF were not likely to live into adulthood and because of this, issues surrounding CF-related infertility did not receive much attention. However, as improved care and treatments continue to extend the life expectancy for individuals with CF, the need to understand how affected men are feeling about issues surrounding CF-related infertility is becoming increasingly relevant. The purpose of this study is to explore the experiences of men with CF and/or CAVD in disclosing their infertility to romantic partners, the effects that both disclosure and associated infertility have had on their romantic relationships and their thoughts on reproduction. We used an anonymous online survey and received 37 responses. We found that nearly half of our participants (44.4%) desired more information at the time of learning about CF-related infertility, and that some wished they had learned this information at an earlier age. Almost all participants (97.2%) had disclosed CF-related infertility to their romantic partner and felt supported after doing so. While many men stated they were concerned that their romantic partners would react negatively to learning about CF-related infertility, few (21.4%) felt their partners actually had negative reactions after disclosure. In addition, the majority of men (61.1%) in this study felt like CF-related infertility did not affect their ability to establish romantic relationships. Finally, 75% of men indicated they think about having or already have children, and most men discussed this with their romantic partner. This is one of the first studies to explore the issues men with CF and infertility face and the results provide valuable insight for other men affected by CF and CAVD, as well as the healthcare professionals who provide care for them.

▼ Male infertility in humans has increased in the last few decades and could be as high as 40%, while up to 50% of these men have ''unexplained'' (idiopathic)
infertility. Although newly developed molecular techniques have great value in detecting subtle causes of
male infertility, more detailed sperm functional tests are required to identify compromised fertility, especially in a clinical set-up. Since ethical constraints often preclude the pursuit of many basic research questions in humans, non-human primates (NHPs) have been identified as key models in human-related studies. NHPs are often used in studies on
male fertility/
infertility, IVF or assisted reproductive technology (ART) procedures,
male contraception and reproductive toxicology. However, comparing results of NHP and human studies require that techniques used for assessment must be objective, standardized and sensitive to recognize compromised sperm function. The aim of this study was to evaluate standard sperm functional tests and develop new functional tests using NHP sperm, specifically from vervet monkeys (Chlorocebus aethiops), chacma baboons (Papio ursinus) and rhesus monkeys (Macaca mulatta), for application in human and NHP studies and to ultimately develop a basic primate model. The sperm functions investigated included sperm motility, longevity, vitality, DNA integrity, acrosome reaction, and hyperactivation. The sperm functional tests evaluated were: CASA motility analysis; Sperm Longevity test; Eosin-Nigrosin and Hoechst and Propidium Iodide staining, as well as the use of WST-1 cytotoxicity assay for vitality; the TUNEL assay for DNA integrity; Acrosome Intactness Test; and induction of hyperactivation via stimulants. The validity of each test was investigated by inhibiting sperm function through the use of copper sulphate and cadmium chloride. All functional tests were successfully performed across all three species, except the TUNEL assay for DNA integrity, and was further used for validation testing. Validation testing proved that all sperm functional parameters were significantly affected by the highest concentrations of the chemicals (250 µg/ml CuSO4 and 500 µg/ml CdCl2) and if not significant, trends of reduction were seen. The tests employed were therefore sensitive to the inhibitory effect of the metals. By evaluating these established sperm functional tests we found that primates would serve as good models for research study. Furthermore, we optimized and modified techniques for sperm and functional analysis in these three primate species and this study will standardize protocols for use in future studies on
male infertility. Additionally, comparing human and NHP sperm function can possibly reveal or explain the high
infertility rates in humans.

▼ As the number and age of human couples turning to assisted reproductive technology (ART) continues to increase, it is essential for clinicians to understand infertility threats related to both female and male patients. The objective of this study was to evaluate the association between age, environment, and reproductive success in male patients having participated in assisted reproductive technology. In corresponding experiments, male infertility variables such as; age, lifestyle exposures, body mass index (BMI), and infertility length with current partner (ILCP) were investigated. A retrospective collection of clinical male patient data from 2011 to 2014 was evaluated. Thirty-five variables were collected from an original sample of 132 patients and correlated for relationships related to male fertility. A negative relationship was observed between pregnancy and male age, IVF pregnancy and male age, male age and semen volume, and male age and semen progressive motility. A negative correlation was also revealed among alcohol usage and semen volume and alcohol usage and total motile sperm/specimen. Additionally, a positive correlation was observed between ILCP and percent normal semen. The goal of the following study, the clinician survey, was to evaluate and compare differences in opinions. Questions pertained to male infertility factors and fertility clinic practices. Clinicians responded with the following opinion rates; 67.9% felt semen analysis was an effective predictor, 32.7% reported no idea if DNA fragmentation was a predictor, 58.5% were in agreement that male age had somewhat significance, 80.1% responded that genetics and/or epigenetics displayed somewhat or significant influence (41.5% and 39.6%), 58.5% believed male exposure/environmental factors displayed significance, 53.9% felt access to more male information would enable better care. The most commonly seen descriptive variable clinicians reported was ILCP (70.8%), the most important semen characteristic was sperm count (84.6%), the most commonly seen urological variable was vasectomy (77.8%), smoking was the most commonly seen environmental exposure (74.5%), and medication use was the most commonly seen medical variable (84.8%). Clinicians described that 39.1% of patient charts were <25.0% completed and 63.0% of clinicians acknowledged that the industry was not providing adequate male reproductive information to infertility patients.

▼ Introduction: Typha capensis, commonly referred to as bulrush also called "love reed'' growing in Southern Africa's wetlands, is one of South Africa indigenous medicinal plants that are traditionally used to treat
male fertility problems and various other ailments. Previous studies revealed that T. capensis has indeed a beneficial effect on
male reproductive functions and aging
male symptoms. The T.capensis rhizomes are used in traditional medicine during pregnancy to ensure easy delivery, for venereal diseases, dysmenorrhea, diarrhoea, dysentery, and to enhance the
male potency and libido. Typha genuses contain flavones and other phenolic compounds, which exhibit anti-oxidative capacity. Materials and Methods: This study encompasses three parts (part 1: Exposure of different cell lines to crude aqueous extracts of T. capensis rhizomes; part 2: HPLC analysis of Typha capensis crude rhizome extract and exposure of different cell lines to the F1 fraction of the summer season; part 3: Compound identification by means of NMR spectrometric analysis and exposure of different cell lines to bioactive compounds (Quercetin and Naringenin) isolated from T. capensis rhizomes. Part 1: TM3-Leydig cells and LNCaP cells incubated with different concentrations of crude aqueous extract of T. capensis rhizomes (0.01, 0.02, 0.1, 1, 10 and 100 μg/ml) and control (without extract)
for 24 and 96 hours, after incubation. The following parameters were evaluated: cell morphology and viability (determined by means of MTT assay). Part 2: The crude extract HPLC profiles were obtained by preparing the extracts for different seasons (Autumn, Winter, Spring, Summer). TM3-Leydig cells, LNCaP cells and PWR-1E cells incubated
with different concentrations T. capensis rhizomes extract F1 fraction of the summer season (0.01, 0.02, 0.1, 1, 10 and 100 μg/ml) and control (without extract) for 24 and 96 hours, after incubation. The following parameters were evaluated: cell morphology was observed and recorded, viability (determined by means of MTT assay), testosterone production (testosterone ELISA test), cell early apoptosis (determined by means of Annexin V-Cy3 binding), DNA fragmentation (determined by means of the TUNEL assay).
Part 3: NMR spectrometric analysis was performed on a 13C spectra were recorded at 400 MHz. TM3-Leydig cells and LNCaP cells incubated with different concentrations of bioactive compounds (Quercetin and Naringenin) isolated from T. capensis rhizomes, for acute exposure (24, 96 hours) and chronic exposure (96 hours), after incubation, the following parameters were evaluated: cell morphology and viability (determined by means of MTT assay), testosterone production (testosterone ELISA test), cell early apoptosis (determined by means of Annexin V-Cy3 binding) and DNA fragmentation (determined by means of the TUNEL assay). ▼ The mammalian sex chromosomes are enriched in large (>10kb), nearly identical (>99%) segmental duplications containing testis-expressed gene families. These genomic regions are highly variable in size, ranging from a single duplication present in a palindromic orientation (singleton palindrome) to vast duplications consisting of 10-100s copies present in both palindromic and tandem orientation (palindrome array, or ampliconic array). Both singleton palindromes and palindrome arrays harbor palindrome-associated testis-expressed gene families. In contrast to the majority of single copy sex-linked genes which are post-meiotically silenced, palindrome-associated gene families are expressed post-meiotically. Palindrome-associated gene families independently evolved, with only 30% shared between humans and mice. Furthermore, palindrome-associated gene families are rapidly evolving at the sequence level and can vary substantially in gene copy number, which are hallmarks of genes involved in fertility and speciation. These observations suggest that palindrome-associated genes have evolved mechanisms to evade the post-meiotic gene silencing which single copy X- and Y-linked genes are
subject to and that palindrome-associated gene families play important post-meiotic roles in
male fertility. In this thesis, I explore the evolutionary history, the importance of palindrome structure, and the functions of genes harbored within palindromes.
Using genetically engineered mouse models, I manipulated the structure and copy number of segmental duplications in two singleton palindromes (Mageb5 and 4930567H17Rik) and two palindrome arrays (Slx and Slxl1). My studies demonstrate the post-meiotic expression of palindrome-associated genes correlates with gene copy number and is not dependent on their palindromic orientation. The singleton palindrome gene families, Mageb5 and 4930567H17Rik, are not dosage sensitive as males with a single copy of either gene exhibit no defects in fertility or spermatogenesis as compared to wild-type males. My investigation of Slx and Slxl1 gene families reveal they are Mus-lineage specific, partially redundant, and essential for
male fertility. The complete deletion of both Slx and Slxl1, results in post-meiotic spermatogenetic defects and
male infertility. Additionally, alterations in Slxl1, but not Slx, gene copy number distorts the ratio of
male to female offspring. This unequal transmission of X and Y chromosomes is indicative of meiotic drive, the non-Mendelian transmission of chromosomes. We hypothesize that Slxl1 competes with a related palindromic array associated gene family on the Y-chromosome, Sly, as alterations in the ratio of Slxl1 to Sly copy number impact the offspring sex ratio. We believe meiotic drive contributed to the massive amplification of Slxl1 and Sly on the mouse sex chromosomes. ▼ A infertilidade é um problema significante na população mundial, afetando mais de 15% dos casais em idade reprodutiva. Isoladamente o homem contribui com 50% na infertilidade conjugal. As causas genéticas são responsáveis por 60% dos casos de infertilidade idiopática. O polimorfismo em genes que codificam enzimas detoxificantes de fase II pode alterar sua expressão ou função, modificando o biometabolismo de compostos tóxicos ao sistema reprodutor masculino. Os genes GSTM1 e GSTT1 codificam enzimas de mesmo nome, são fundamentais no processo de detoxificação dos xenobióticos endógenos ou exógenos, facilitando sua excreção. O objetivo principal do presente estudo foi detectar o polimorfismo dos genes GSTM1 e GSTT1 em homens com infertilidade idiopática pelo Serviço de Reprodução Humana do Hospital das Clínicas de Goiânia (HC), segundo o laudo dos espermogramas. Ao total foram avaliadas 304 amostras de pacientes com idade entre 15 e 69 anos, diagnosticados com infertilidade idiopática, no período de 2004 a 2006. Os resultados observados em amostras de DNA de sêmen, freqüências genotípicas de 28,6% (30/105) para o genótipo GSTM1/T1(nulo) em indivíduos normais e 50% (6/128) para o GSTM1/T1(nulo) em indivíduos alterados, demonstrando significativa relação estatística entre o polimorfismo do gene GSTM1 e GSTT1 com a infertilidade masculina idiopática. A alteração seminal associada ao genótipo polimórfico de maior freqüência GSTM1/T1 (nulo) é a alteração de número (oligozoospermia) possuindo maior freqüência tanto para GSTM1 (nulo) (78,9%) quanto GSTT1 (nulo) (73,7%), apesar de não ser significante. Analisando as freqüências genotípicas das amostras de sangue não encontramos associações estatísticas com a infertilidade masculina, já que 46,6% (34/76) são GSTM1/T1 (nulo) em indivíduos normais e 58, 4% (73/125) são GSTM1/T1 (nulo) em indivíduos alterados. Não foi encontrada influência significativa do consumo de álcool, cigarro, caxumba e uso de xenobióticos analisados nos parâmetros básicos dos espermogramas analisados. Indivíduos polimórficos para os genes GSTM1 e GSTT1 estão mais susceptíveis a redução na qualidade seminal e possivelmente infertilidade, sendo que indivíduos oligospérmicos são os mais afetados pelo polimorfismo gênico.
The infertility is a significant problem in the world-wide population, more than affecting 15% of the couples in reproductive age. Separately the man contributes with 50% in the conjugal infertility. The genetic causes are responsible for 60% of the cases of idiopathic infertility. Male infertility is a relatively common condition affecting 1 in 20 men of reproductive age. The aetiology of this condition is thought to involve the excessive generation of reactive oxygen species (ROS) by human spermatozoa; however the cause and sub-cellular origins of this aberrant activity is unknown. The objective of this doctoral thesis was to determine the importance of sperm mitochondria in creating oxidative stress associated with defective sperm function and subsequently to investigate causes of increased mitochondrial ROS generation. In the first study, intracellular measurement of mitochondrial ROS generation and lipid peroxidation was performed using the fluorescent probes MitoSOX red and BODIPY C11 in conjunction with flow cytometry. Effects on sperm movement were measured by computer-assisted sperm analysis. Defective human spermatozoa spontaneously generated mitochondrial ROS in a manner that was negatively correlated with motility. The induction of mitochondria ROS resulted in peroxidative damage to the midpiece and a loss of sperm movement that could be prevented by the concomitant presence of α-tocopherol. Concluding that the sperm mitochondria make a significant contribution to the oxidative stress exhibited by defective human spermatozoa. In order to investigate causes of irregular mitochondrial ROS production, we demonstrated that defective human sperm populations are characterized by high cellular contents of both total and free fatty acids using gas chromatography. The free unsaturated fatty acid content of these cells was positively correlated with the induction of mitochondrial superoxide generation. This relationship was causal and mediated by the range of unsaturated fatty acids that are present in human spermatozoa. Direct exposure of these cells to free unsaturated fatty acids stimulated mitochondrial superoxide generation and precipitated a loss of motility and an increase in oxidative DNA damage, two key attributes of male infertility. We followed with examination of the ability of human spermatozoa to undergo apoptosis and generate mitochondrial ROS. We report that these cells are prevented from entering the apoptotic pathway as long as the PI3kinase/AKT complex is phosphorylated. If PI3 kinase phosphorylation is inhibited with wortmannin then these cells default to an intrinsic apoptotic cascade characterized by caspase activation in the cytosol, annexin V binding to the cell surface, mitochondrial ROS generation, cytoplasmic vacuolization, oxidative DNA damage and motility loss. However the physical architecture of the cell subsequently prevents endonucleases released from the mitochondria or activated in the cytosol from penetrating the sperm nucleus and, as a result DNA fragmentation does not occur although oxidative DNA adducts can clearly be detected. ▼ Les gènes du chromosomeX sont majoritairement inactivés au cours de la méiose mâle. Chez la souris, seulement 6% d’entre eux sont réactivés au cours des stades post-méiotiques. Parmi eux le gène Uba1X codant pour l’enzyme activatrice de l'ubiquitine, UBA1 qui produit trois transcrits dont deux sont ubiquitaires mais le troisième prédomine dans les cellules post¬-méiotiques : les spermatides. Nos travaux montrent que les 5’UTR, seul différence entre ces trois transcrits, déterminent la localisation et la dose relative des isoformes nucléaire et cytoplasmique de la protéine UBA1. Nous avons mis en évidence chez la souris que le transcrit spermatide-spécifique code pour l'isoforme nucléaire, exprimée fortement dans les spermatides suggérant un rôle de la protéine UBA1 dans la dégradation des histones lors du remodelage chromatinien. Nous avons détecté deux mutations dans la région spermatide-spécifique du gène : une délétion de 13pb et une transition G>A, chacune portée par un patient infertile, et non retrouvée dans notre population témoin. Les analyses ont montré que la délétion de 13pb induit un épissage anormal du transcrit spermatide-spécifique et que la transition G>A pouvait réduire le taux d’expression du transcrit spermatide-spécifique. Ces mutations pourraient induire l'infertilité des deux patients. En parallèle nous avons pu démontrer que les mutations dans le gène codant pour la protamine PRM1 décrites dans la littérature c.102G>T et c.-107G>C ne sont pas liées à l'infertilité masculine et que le variant est un polymorphisme fréquemment retrouvé dans la population congolaise.
The majority of genes on the X chromosome are repressed during meiosis and only 6% of them are expressed in post meiotic germ cells. One of these genes is Ubal, encoding the ubiquitin-activating enzyme UBA1. Ubal produces three different transcripts, two of which are ubiquitously expressed while the third is predominant in the post meiotic germ cells: the spermatids. Our study shows that the 5’UTR, which is the only difference between these transcripts, determines the localization and the relative dose of the nuclear and cytoplasmic isoform of the UBA1 protein. The spermatid-specific transcript encodes for the nuclear isoform in the spermatids in the mouse suggesting that the UBA1 protein is implicated in chromatin remodeling during spermiogenesis. We have detected two mutations in the spermatid-specific region of the UBA1 gene in two infertile men: a deletion of 13bp and a G>A transition, neither of which was found in our cohort of fertile men. The deletion of 13bp diminishes the correct splicing of the spermatid-specific transcript and that the G>A transition may reduce expression of the spermatid-specific transcript. These results show that the UBA1 gene is involved in spermiogenesis, and reactivated in spermatids by its spermatid-specific transcript and that the mutations identified may induce infertility by reducing UBA1 levels in spermatids. Infertility is estimated to occur in 15-20% of couples in the United States.
Although infertility has psychosocial consequences of varying intensity for both
individuals and couples, available research on infertility predominantly focuses on
the woman and her experience. Few studies directly address the male experience
of infertility qualitatively.
This qualitative study explored the experience of infertility by men, and
their perceptions and expectations of healthcare providers and the healthcare
system. King's (1981) Theory of Goal Attainment provided the theoretical
framework. The setting was a fertility clinic in the Midwest. A purposive sample
of six men with a diagnosis of infertility comprised the sample. Data were
collected through a demographic questionnaire and open-ended questions, and
analyzed using Colaizzi's (1978) method of data analysis.
Results indicated complex psychosocial and environmental factors
impacted the males in their experience of infertility. Three themes emerged as
male perceptions of the infertility experience: (a) goal blockage consisting of
feelings of isolation and disappointment, (b) goal adjustment with realistic
situational appraisals; as well as embracement of alternative measures to foster
goal attainment including positive perceptual adjustments in response to goal
blockage, and, (c) active involvement and re-engagement in adjusted goal
attainment. Three themes emerged on how healthcare providers can assist men
living with infertility: (a) empathy, (b) education, and, (c) guidance. Implications
for future nursing practice and education indicate an increased need for
anticipatory guidance, and development of a holistic, individualized approach to
infertility.

▼ Miesten kokemuksia tahattomasta lapsettomuudesta on tutkittu vähän. Tutkimusta on tehty enemmän naisten tai pariskuntien kokemuksista, jolloin miesten kuvaamat kokemukset ovat jääneet niukoiksi. Isän ja miehen rooli perheessä on muuttunut muiden yhteiskunnallisten muutosten myötä, eikä isäksi tulemista nähdä enää vain perheen jatkamisen välttämättömyytenä, vaan toiveena isäksi tulemisesta.
Tämän pro gradu -tutkielman tarkoituksen oli tutkia miesten kokemuksia tahattomasta lapsettomuudesta, sen seurauksista sekä sosiaalisesta tuesta. Tutkimus on osa Tampereen yliopiston lapsettomuushanketta.
Empiirisen tutkimuksen aineisto kerättiin syksyllä 2018 sähköisellä e-lomakkeella. Lomake sisälsi taustatietoja kartoittavia strukturoituja sekä avoimia kysymyksiä liittyen lapsettomuuden kokemuksiin ja
sosiaaliseen tukeen. Viisi e-lomakkeeseen vastanneesta miehestä antoi yhteystietonsa ja luvan jatkohaastattelua varten. Puhelinhaastattelut tehtiin joulukuussa 2018. Tämän tutkielman aineisto muodostui e-lomakkeen vastauksista (n=12) ja puhelimitse toteutetuista teemahaastatteluista (n=3). Tutkimusaineisto analysointiin aineistolähtöisellä sisällönanalyysillä.
Miehet kokivat tahattoman lapsettomuuden raskaana emotionaalisena prosessina, johon liittyi tunteiden käsittelyn haasteita. Miehen rooli lapsettomuudessa nähtiin ristiriitaisena, sillä yhteiskunnallisesti rakennetut stereotyyppiset roolit vaikuttivat miehen omiin, mutta myös muiden odotuksiin miehen toiminnasta puolison rinnalla. Miehet kyseenalaistivat asetetut rooliodotukset ja pyrkivät tasa-arvoistamaan lapsettomuuden kokemusta naisten rinnalla. Lapsettomuushoidot koettiin uuvuttaviksi ja kokemukset hoidon laadusta olivat ristiriitaisia. Osa miehistä koki itsensä täysin ulkopuoliseksi lapsettomuushoidoissa, joissa kaikki huomio kiinnitettiin puolisoon. Muut ihmiset kohtasivat lapsettoman miehen myötätuntoisesti, mutta valitettavan usein kohtaamiseen liittyi ajattelematon ja ymmärtämätön suhtautuminen, joka johti lapsettomuuden salailemiseen. Lapsettomuus aiheutti henkistä kasvua ja elämän tuomiin muutoksiin opettelemista. Miehet saivat monipuolista tukea laajalta tukiverkolta, ja puolison tuki vertaistuen ohella koettiin elintärkeäksi. Miehet jäivät kaipaamaan ystävien rohkeaa kohtaamista sekä kokonaisvaltaisesti pariskunnan huomioivaa hoitohenkilökunnan informatiivista tukea. Hoitohenkilökunnan toivottiin ohjaavan pariskuntia vertaistuen ja muiden tukipalveluiden äärelle.
Johtopäätöksenä voidaan todeta, että lapsettomuudesta kärsivät miehet tarvitsevat tasa-arvoista huomiota sekä rooliodotuksia rikkovaa tukea kaikilla yhteiskunnan sosiaalisten suhteiden tasoilla. Hoidossa huomiota tulisi kiinnittää pariskuntien yksilöllisiin tarpeisiin ja miesten tunteiden käsittelyyn. ▼ Infertility affects 10-15% of couples, making it one of the most frequent health disorders for reproductive aged individuals. Over 20% of cases are idiopathic, and it is commonly thought that many individuals have an underlying genetic etiology. Identifying genes involved in unexplained infertility provides information to support diagnosis, genetic counseling and potential therapeutic intervention.
One approach to identifying genes involved in infertility is to examine the phenotype-genotype correlation in subjects with a clinical phenotype accompanied by a balanced chromosomal aberration (BCA), as is the foundation of gene discovery in the Developmental Genome Anatomy Project (DGAP). Participant DGAP230 has severe oligospermia and 46,XY,t(20;22)(q13.3;q11.2). While BCAs may reduce fertility by production of unbalanced gametes, a chromosomal rearrangement may also disrupt or dysregulate genes important in fertility.
Using large-insert genome sequencing, chromosomal breakpoints were determined with nucleotide-level precision. Investigation of genes in the topologically associated domains at the sites of the rearrangement and subsequent use of the novel 3C-PCR technique revealed exclusive dysregulation of synaptonemal complex protein 2 (SYCP2) from the derivative chromosome 20 (der(20)) allele, which resides 1.5 Mb centromeric to the der(20) breakpoint. 4C-seq from the SYCP2 promoter revealed interactions eight Mb downstream of the der(20) breakpoint in chromosome 22, supporting a model of enhancer adoption.
SYCP2 encodes synaptonemal complex protein 2, a member of the synaptonemal complex involved in male meiosis I homologous chromosome synapsis. To assess its impact on meiosis, we misexpressed RED1, the functional axial element homolog of SYCP2 in Saccharomyces cerevisiae. By performing Red1 immunolocalization on surface-spread meiotic nuclei, we discovered that excess Red1 forms polycomplexes, disrupting the structural integrity of the synaptonemal complex by preventing incorporation of the transverse filament Zip1. This model highlights aberrant meiosis as the defect in spermatogenesis leading to severe oligospermia. Finally, we reveal two novel frameshift mutations in SYCP2 identified in azoospermic men, which further support a mechanism of SYCP2-mediated infertility.
This thesis provides the first evidence of SYCP2-mediated infertility in humans and illuminates that rearrangement breakpoints should be considered as an alternative etiology to that of segregation of unbalanced gametes in infertile men harboring BCAs.

▼ Protein interacting with C kinase 1 (PICK1) is a higly conserved peripheral membrane protein involved in protein trafficking, a function that has been well characterized in neurons. PICK1 contains an N-terminal PDZ (PSD-95/Dlg/ZO-1) domain, which is known to mediate protein – protein interaction; a central BAR (Bin/amphiphysin/Rvs) domain, which is known to be homologous to the BAR domain of arfaptin 2 and binds to negatively charged lipid; and an acidic C terminus. Our lab has proposed the model that the PDZ domain of PICK1 binds to the cargo proteins, sorting the cargo proteins to specific vesicle complex; while the BAR domain of PICK1 binds to the lipid membrane structure and facilitates the formation of vesicle. Here, we report that male mice deficient in PICK1 are infertile and have a phenotype resembling the human disease globozoospermia. The primary defect in the testes of Pick1-knockout mice was fragmentation of acrosomes in the early stages of spermiogenesis. This fragmentation was followed by defects in nuclear elongation and mitochondrial sheath formation, leading to round-headed sperm, reduced sperm count, and severely impaired sperm motility. We found that PICK1 interacted with Golgi-associated PDZ- and coiled-coil motif–containing protein and the primary catalytic subunit of protein kinase 2 (CK2α′), proteins whose deficiencies lead to globozoospermia in mice. PICK1 was highly expressed in round spermatids and localized to Golgi-derived proacrosomal granules. GOPC colocalized with PICK1 in the Golgi region and facilitated formation of PICK1-positive clusters. Furthermore, there was an increase in apoptosis in the seminiferous tubules of Pick1–/– mice, a phenotype also seen in CK2α′-deficient mice. Our results suggest that PICK1 is involved in vesicle trafficking from the Golgi apparatus to the acrosome and cooperates with other proteins such as GOPC and CK2α′ in acrosome biogenesis.

Infertility John K. Amory MD, MPH Chair of the Supervisory Committee: Dr. Nina Isoherranen, Associate Professor Department of Pharmaceutics Objective: There is currently no effective medical therapy for men with
infertility due to oligoasthenozoospermia (OA). As men with OA have lower concentrations of 13-cis-retinoic acid in their testes, we hypothesized that men with
infertility from OA might have improved sperm counts when treated with isotretinoin (13-cis-retinoic acid). Design and Setting: We conducted a single-site, single-arm, pilot study to determine the impact of therapy with isotretinoin on sperm indices in 19 infertile men with OA. Subjects: Subjects were men between 21 and 60 years of age with
infertility of more than 12 months associated with sperm concentrations below 15 million sperm/ml. Intervention: All men received isotretinoin 20 mg by mouth twice daily for 20 weeks. Outcome Measures: Subjects had semen analyses, physical examinations, routine blood counts, and chemistries every four weeks during treatment. Results: Nineteen men enrolled in the study. Median (25th, 75th) sperm concentration increased from 2.5 (0.1, 5.9) million/ml at baseline to 3.8 (2.1, 13.0) million/ml at the end of treatment (p=0.006). No significant changes in sperm motility were observed. There was a trend towards improved sperm morphology (p=0.056). Six pregnancies (three spontaneous and three from ICSI) and five births occurred during the study. Four of the births, including all three of the spontaneous pregnancies, were observed in men with improvements in sperm counts with isotretinoin therapy. Conclusions: Isotretinoin therapy of men with OA is associated with improvements in sperm production. Additional studies of isotretinoin therapy for men with
infertility from OA are warranted.

▼ textabstractThis thesis describes the treatment of male infertility by in-vitro-fertilization. IVF was
a treatment for infertility caused by tubal blockage. Thereafter IVF was also used as a
treatment for male infertility.IVF is the bringing together of the gametes outside the
female body. After fertilization the embryo is cultured in an incubator before it will be
transplanted to the uterus.
The process of development of the gametes and fertilization is described in chapter
1 of this thesis. A mature sperm and oocyte are formed after mitotic and meiotic
divisions. The head of the spermatozoa is covered by the acrosome. During passage of
the cumulus and the zona pellucida of the oocyte, the acrosome loses its enzymes, the
acrosomal reaction. By means of the enzymatic action of the acrosomal proteins and
the mechanical force of the beating sperm tail, the sperm penetrates the zona pellu·
cida. After contact of the sperm with the cell membrane of the oocyte, the membranes
fuse and the oocyte completes its second meiotic division. After this process the
embryo is formed.
The history of the discovery of the gametes and fertilization and the first successful
attempt to achieve a fertilization in-vitro are described in chapter 2. It was only
from the beginning of the seventies that it was possible to culture human embryos in·
vitro.
Since so many patients today are treated by IVF it is possible to compare the results
of different groups of patients. In chapter 3 the results of IVF for male infertility on
the IVF-laboratory of the Dijkzigt Hospital are described. Patients were grouped according
to the concentration of motile sperm in the ejaculate. Seven groups were formed,
on a logarithmic ascending scale. Results were studied per ovum pick-up and per
patient. Ovum pick-ups with a low sperm concentration did have a lower fertilization
rate (total number of punctures with fertilization/total number of punctures in that
group) than punctures with a high sperm concentration. Punctures with a concen·
tration of motile sperm oflxl06/ml had a fertilization rate of34%, in the group with
4-8x106/ml the fertilization rate was 69% and in the groups with the highest concentrations
the fertilization rate was 90% per ovum pick-up. After fertilization had been
achieved the percentage of pregnancy was the same for all groups. Other criteria to
determine quality of embryo's, such as the number of morphologically good embry·
o's being transplanted or the possibility of cryopreservation showed no difference
between the sperm groups

▼ Passage of spermatozoa through the female reproductive tract is essential for the regulation of fertilization, ensuring that healthy sperm reach the oocyte. Previous studies were devoted to morphological selection of sperm cells by the cervical mucus. However, research prove that the loss of integrity of the sperm plasma membrane is associated with infertile men, irrespective of their normal semen parameters. This indicates that the sperm plasma membrane plays an important role in fertilization. Further studies indicated that sperm surface proteins assist penetration through the female reproductive tract and would therefore provide useful insight in understanding other factors associated with
male infertility. The aim of this project was to determine if there are any differences between sperm surface proteins of fertile donor samples in relation to infertile patient samples using different separation techniques and different detergents. Three different sperm separation techniques were employed, including wash, swim-up (SU) and Percoll density gradient centrifugation (DGC).Parallel to this, the deoxy-ribose
nucleic acid (DNA) fragmentation of these cells were analysed for comparison of the
extent of DNA damage induced due to different separation techniques used. This
provided evidence that the best separation technique is the DGC as it minimises the
amount of DNA fragmentation caused. Four different detergents were used in the process of extracting the membrane proteins from spermatozoa, namely sodium dodecyl sulphate (SDS), saponin,cetyl-trimethyl-ammonium bromide (CTAB), and TWEEN-20. The membrane proteins were then separated on a12% SDS poly-acrylamide gel electrophoresis (PAGE), and analysed by Coomassie blue and silver staining techniques as well as densitometry. Due to the different chemical nature of the detergents that extracted different surface proteins, CTAB (cationic) and SDS (anionic) extracted the most because of its strong solubilising abilities as non-ionic detergents. Common proteins that were extracted in donor samples
included; 115, 92.5, 89, 61, 55.5, 51.5, 47, 44.5, 43, 38.5, 34 and 28 kDa proteins. In patients, commonly occurring proteins included; 92.5, 74.5, 70, 60.5, 51.5, 50, 44.5, 43, 36, 29.5, and 25.5 kDa proteins. Marked differences were found between membrane proteins extracted from donor samples in comparison to patient samples. Identification of these proteins was done using the SwissProt database and a literature search. Mostly non-genomic progesterone receptors were identified; others included oestrogen receptor, a phosphotyrosyl protein, P34H, equatorial segment protein, mannose lectin receptor, human guanylylcyclase
receptor, epididymal protease inhibitor receptor, PH30 and estradiol binding protein. The function of the membrane surface proteins identified in this study plays a vital role in fertilization. In this work, we employed quantitative proteomic strategies to investigate the clinical and the functional aspects of human TEX101 protein.
To facilitate translation of TEX101 into clinic, we first developed and optimized an immunoassay for TEX101 measurement in biological fluids. We then evaluated the performance of TEX101 ELISA in 805 seminal plasma samples of fertile, sub-fertile, and infertile men. We demonstrated the clinical utility of TEX101 to evaluate vasectomy success, to stratify azoospermia forms and subtypes, and to predict the success of sperm retrieval in patients diagnosed with non-obstructive azoospermia.
To gain insights into human TEX101 function, we employed mass spectrometry approaches to investigate the physical and the transient TEX101 interactome. Co-immunoprecipitation-mass spectrometry revealed a testis-specific complex TEX101-DPEP3 which was validated by hybrid immunoassay in testicular tissues and spermatozoa. In addition, we discovered a TEX101 rs35033974 homozygous knockdown model, which carried a missense variation leading to near-complete protein degradation. We then performed global proteomic analysis of TEX101 knockdown spermatozoa, and we identified the transient interactome of TEX101.
New knowledge on TEX101 will provide insights into reproductive biology, facilitate better diagnostics of
male infertility, contribute to rational selection of assisted reproduction treatments, and offer new protein targets to develop non-hormonal
male contraceptives.

▼ Epigenetics refers to the changes in gene expression
that are not accounted for by the changes in DNA sequence. DNA
methylation is one of the main epigenetic mechanisms in mammals. It
contributes to various biological processes such as cellular
differentiation, gametogenesis, and cancer. We investigated DNA
methylation abnormalities in the
male and female reproductive
tract. During gametogenesis germ cells undergo epigenetic
reprogramming, defects of which may lead to compromised
spermatogenesis. Using MethyLight and Illumina GoldenGate assays we
found a broad abnormal epigenetic defect associated with abnormal
semen parameters. We propose that the underlying mechanism may be
improper erasure of DNA methylation during germline reprogramming.;
Ovarian cancer has the highest mortality rate of all gynecologic
malignancies. Survival is strongly stage-dependent but there are no
effective screening methods available. Ovarian cancer is also very
heterogeneous. While each subtype presents with different clinical,
pathological, and therapeutic characteristics, all ovarian cancer
patients are treated uniformly. We used DNA methylation assays to
investigate epigenetic differences between the subtypes and to find
potential blood-based early detection biomarkers. We observed
subtype-specific DNA methylation profiles suggesting that each
subtype might be a distinct disease. Stepwise marker screening
resulted in a panel of four markers that could distinguish pooled
sera of patients from controls, but should be further investigated
on multiple independent serum samples.

▼ One of the main goals of my doctoral training has been
to gain experience working with a variety of types of epidemiologic
data. The three projects that compose this dissertation have
provided me with the opportunity to work with several different
data structures: (1) published summary data, (2) population-based
family data, (3) prospective cohort data, and (4) population-based
case-control data.; For my dissertation, I have employed each of
these data structures to investigate the role of steroid hormones
in the etiology of several urologic diseases. In the last several
decades of epidemiologic research, it has become clear that steroid
hormones are involved in many disease processes. For my research, I
have studied the relationships between steroid hormone exposure and
molecules of steroid hormone response and female bladder cancer, as
well as two
male urologic diseases:
infertility and testicular germ
cell tumors.; In my first paper, I investigated the association
between
male infertility and a polymorphic CAG repeat tract in the
androgen receptor gene (AR) through the meta-analysis of a large
body of existing literature on the topic. I quantitatively
summarized 33 studies, and found that infertile
male cases had
statistically significantly longer mean CAG repeat length than
controls (SMD: 0.19, 95% CI: 0.09-0.29). Meta-analysis of a select
subset of 13 studies that used more stringent case and control
selection criteria revealed a larger difference between cases and
controls (SMD: 0.31, 95% CI: 0.14-0.47). This meta-analysis
provides support for an association between increased androgen
receptor CAG length and idiopathic
male infertility, and brings
together for the first time results from many published reports on
this topic.; In my second paper, I expanded upon the results of the
meta-analysis to conduct an original data analysis studying the
association between androgen receptor CAG repeat length and risk of
testicular germ cell tumors. Increasing rates of testicular germ
cells tumors (TGCTs) over the last 40 years suggest environmental
factors are involved in disease etiology, but familial risk studies
indicate that genetic factors are also important.; I investigated
whether variation in CAG trinucleotide repeat length in exon 1 of
the androgen receptor gene is associated with TGCTs using data from
a population-based family study. Analyses of 273 TGCT case-mother
pairs revealed no association between androgen receptor CAG repeat
length and overall risk of TGCTs. Risk of seminoma was
significantly associated with shorter CAG repeat length, and the
trend was highly significant over decreasing CAG repeat length (CAG
≥20 versus CAG ≤19: OR=0.54 (95% CI: 0.31-0.93), p trend=0.003).
Case-case analyses comparing seminoma cases to nonseminoma and
mixed germ cell tumors confirmed that seminoma cases had shorter
CAG repeat length than other histologies (CAG ≥20 versus CAG ≤19:
OR=0.54 (95%CI: 0.29-1.01), p trend=0.003).; My findings indicate

▼ La malattia diabetica influenza la capacità riproduttiva maschile. Esistono pochi dati riguardanti gli aspetti morfo-funzionali delle vescicole seminali dei pazienti infertili con diabete mellito. Lo scopo di questo studio è stato valutare le caratteristiche ecografiche delle vescicole seminali di pazienti infertili con diabete tipo 2 e neuropatia. Sono stati selezionati 30 pazienti infertili con neuropatia diabetica e senza altre comorbità andrologiche. Due gruppi di controllo differenti sono stati valutati : uomini non diabetici con infertilità idiopatica (n=30) e pazienti infertili con infiammazione a carico delle ghiandole sessuali accessorie(MAGI) (n=30), un modello clinico di infiammazione a carico delle vescicole seminali ben caratterizzato sul piano nosografico. I pazienti ed i controlli sono stati sottoposti a valutazione ecografica della regione prostato-vescicolare per via transrettale endocavitaria dopo 1 giorno di astinenza sessuale prima e 1 ora dopo atto eiaculatorio. I seguenti parametri ecografici sono stati analizzati: (1,2) spessore intervescicolare rilevato in corrispondenza del corpo e del fondo ghiandolare, (3) spessore parietale vescicolare, (4) numero di aree policicliche endoluminali; (5) rapporto tra lo spessore interparietale rilevato al fondo ed al corpo ghiandolare; (6) differenza di spessore interparietale intervescicolare, (7) differenza dello spessore interparietale ghiandolare prima e dopo atto eiaculatorio. I pazienti con DM mostrarono valori medi del rapporto tra lo spessore interparietale rilevato al fondo ghiandolare rispetto lo spessore interparietale rilevato al corpo significativamente maggiori (p<0.05) rispetto gli altri gruppi. Solo i pazienti con MAGI mostrarono un significativo (p <0,05) aumento del numero di di aree policicliche endoluminali. I controlli ed i pazienti con MAGI mostrarono valori non differenti sul piano statistico relativamente allo spessore interparietale rilevato prima e dopo atto eiaculatorio mentre tale parametro risulto' significativamente (p <0,05) ridotto nei pazienti con diabete mellito. In conclusione, questo studio ha dimostrato che i pazienti infertili con DM hanno peculiari caratteristiche ecografiche SV suggestivi di atonia funzionale.

▼ Introduzione: Il diabete mellito (DM) può determinare infertilità maschile agendo a livello pre-testicolare, testicolare e post-testicolare.
Scopo della tesi: valutare la presenza di infertilità maschile e i meccanismi di danno spermatico nei pazienti con DM in età fertile.
Materiali e Metodi: Sono stati arruolati 34 pazienti con DM1, 55 pazienti con DM2, e 100 soggetti fertili sani controllo normale . I pazienti diabetici sono stati ulteriormente suddivisi in 3 gruppi sulla base del compenso glicometabolico e della durata di malattia. Sono stati eseguiti: esame standard del liquido seminale e valutazione citofluorimetrica dei parametri spermatici non convenzionali (frammentazione del DNA nemaspermico, valutazione del grado di vitalità, apoptosi precoce e apoptosi tardiva e del grado di condensazione cromatinica; studio del potenziale di membrana mitocondriale, MMP), formula leucocitaria nell eiaculato mediante analisi citofluorimetrica, valutazione del grado di perossidazione lipidica (PL), e dei livelli spermatici di superossido mitocondriale, ecografia didimo-epididimaria e determinazione sierica in chemiluminescenza di testosterone totale, LH.
Risultati: i pazienti diabetici presentano concentrazione nemaspermica, motilità progressiva e morfologia ridotte rispetto al gruppo di controllo (p<0.05). Il volume dell eiaculato è significativamente inferiore nei pazienti con DM1 (p<0.05), ma sembra non subire variazioni significative nei pazienti con DM2 rispetto al gruppo di controllo. Le concentrazioni di leucociti risultano più elevate solo nei pazienti con DM2 (p<0.05), registrando una riduzione statisticamente significativa della percentuale di linfociti T helper nel liquido seminale e un aumento di quella dei linfociti T suppressor rispetto ai soggetti di controllo (p<0.05)
L analisi dei parametri spermatici non convenzionali ha mostrato un peggioramento dell MMP nei pazienti diabetici (p<0.05). Solo i pazienti con DM2 hanno mostrato una riduzione significativa del grado di vitalità nemaspermica con aumento di apoptosi tardiva rispetto al gruppo di controllo e della frammentazione del DNA rispetto agli altri due gruppi (p<0.05). Quando suddivisi per durata di malattia, i pazienti con DM1 presentano una motilità progressiva minore nei casi di lunga (>10 anni) durata di malattia rispetto agli altri due gruppi, accompagnata da basso MMP dopo 5 anni di malattia (p<0.05).
