Diabetes mellitus is a disorder of glucose metabolism with microvascular and macrovascular complications. Peripheral neuropathy and complications of the lower extremities contribute to increased morbidity and reduced quality of life of diabetic patients. Dryness of the skin of the feet may be an early manifestation of diabetic neuropathy and has been associated with increased risk for development of foot ulcers. There are various methods for the assessment of the dryness of the skin of the feet. However, no study si far has examined the performance of these tests for the diagnosis of peripheral diabetic neuropathy.Aim of this study was to examine the performance of different methods used for the diagnosis of the dryness of the skin of the feet and to look for associations between these methods with indices of peripheral diabetic neuropathyMaterial and Methods: A total of 98 diabetic patients (38 female; mean age 58.8 ± 11.0 years) participated in the study. Dryness of he skin of the feet was assessed by a dermal indicator (NeuropadTM) and the clinical scoring systems of EEMCO [European Group on Efficacy Measurement of Cosmetics: Overall dry skin score (ODS) and specific symptom sum score (SRRC)]. Diagnosis of peripheral neuropathy was assessed by evaluating the neuropathy symptom score (NSS), the neuropathy disability score (NDS), the vibration perception threshold (VPT) and the monofilament test.Results: A total of 10.9% of patients were on monotherapy with metformin, while the majority of patients were receiving insulin (60.9%); 17.4% of patients were receiving combination of two antidiabetic drugs (except insulin), while 9.8% of patients were treated with 3 antidiabetic medicines (except insulin). Statistically significant positive correlations were found between ODS and age, BMI and indicators NSS, NDS and VPT, while a statistically significant negative correlation was found with the monofilament test. SRRC score showed also a statistically significant negative correlation with the ankle-brachial index. The performance (sensitivity and specificity) of NeuropadTM testing was better after15 min than after 10 min of application (right foot: sensitivity 76%, specificity 48%, left foot: sensitivity 79%, specificity 50%). Values of ODS ≥2 and SRRC≥ 4 showed a sensitivity of 48% and 55%, respectively, while the specificity was 60% and 80%,respectively for the diagnosis of peripheral diabetic neuropathy.Conclusion: The dermal indicator test NeuropadTM is superior to ODS and SRRC for the diagnosis of peripheral diabetic neuropathy, because of its simplicity, wide availability and the higher performance in comparison with the ODS and the SRRC. In addition, dry skin is associated with the indices of determination of peripheral diabetic neuropathy.However, the clinical methods used for diagnosis of dry skin have low performance for the diagnosis of this manifestation at the feet of the subjects with type 2 diabetes. Les relations entre douleur, dysautonomie et atteinte des petites fibres nerveuses sont complexes. Notre travail a porté sur les relations entre le phénotypage clinique de la douleur d’une part, et sur la caractérisation neurophysiologique des atteintes autonomes d’autre part, dans le contexte d’atteinte des petites fibres nerveuses. Ceci a fait l’objet de 5 études. La première étude a montré que plusieurs variables cliniques permettaient de caractériser la douleur de façon différentielle entre des patients atteints de neuropathie des petites fibres idiopathique ou secondaire à un syndrome de Sjögren. La deuxième étude a montré que la lésion ou la perte de fonction des petites fibres entraînait plutôt une majoration de la sensibilisation centrale et des anomalies liées aux grosses fibres sensitives chez des patients ayant un syndrome de Sjögren douloureux associé à une neuropathie des petites fibres. La troisième étude a montré que, dans le syndrome du canal carpien, les troubles sensitifs, en dehors des sensations de brûlure, étaient aussi liés de façon prédominante aux grosses fibres, avec cependant une sensibilité diagnostique particulière de l’étude des fibres autonomes vasomotrices par le test à l'EMLA. La quatrième étude a porté sur l’intérêt de la mesure des conductances cutanées par Sudoscan ® dans le suivi des neuropathies amyloïdes et de leur traitement. Enfin, la cinquième étude a permis d’analyser la sensibilité diagnostique respective de deux tests sudoromoteurs (Sudoscan® et Neuropad®) dans le cadre de la détection précoce de la neuropathie amyloïde et aussi pour le suivi des patients, au moins en ce qui concerne le Sudoscan®. Avec tous ces aspects, ce travail a permis de mieux comprendre les relations entre troubles sensitifs et atteinte des petites fibres sensitives et autonomes dans des contextes neuropathiques cliniques, comme le syndrome de Sjögren, le syndrome du canal carpien et la neuropathie amyloïde.

The relationships between pain, autonomic dysfunction and involvement of small nerve fibers are complex. Our work focused on the relationships between clinical phenotyping of pain on the one hand, and the neurophysiological characterization of autonomic disorders on the other hand, in the context of small nerve fiber involvement. This has been the subject of 5 studies. The first study showed that several clinical variables differentially characterize pain between patients with idiopathic small fiber neuropathy or secondary to Sjögren's syndrome. The second study showed that the lesion or loss of function of small fibers led to an increase in central sensitization and abnormalities related to large sensory fibers in patients with painful Sjögren's syndrome associated with small fiber neuropathy. The third study showed that, in carpal tunnel syndrome, sensory disturbances, apart from burning sensations, were also predominantly related to large fibers, with however a particular diagnostic sensitivity of the study of autonomic vasomotor fibers by the EMLA test. Diabetes Mellitus (DM) is a modern plague of humanity. Foot complications are the most serious among DM complications. Diabetic Foot (DF) is the mainstay of foot complications. DF is defined as the foot with ulceration that is associated with neuropathy and/or peripheral arterial disease of the lower limbs in a patient with diabetes.Diabetic neuropathy (DN) is defined as the nerve damage due to DM and may be clinically evident or subclinical. It is characterized as DN after the exclusion of other causes of neuropathy. Clinical studies have repeatedly highlighted the importance of DN as a predisposing factor for ulceration. Recent studies have shown the usefulness and precision of a semi-quantitative method for controlling foot sweating for Diabetic Symmetric Poly-Neuropathy (DSPN) diagnosis. This semi-quantitative method consists of applying an Adhensive Indicator Plaster (AIP) on the plantar surface of each foot, just under at the head of the 1st metatarsal bone. The AIP should be placed on dry skin without callus formation. It contains anhydrous cobalt crystals, which give the plaster a blue color. When the AIP uptakes moisture, it changes its color to pink. When applied to human skin, in case of perspiration loss, the plaster remains blue, in reduced sweat secretion it acquires a picture of a blue-and-pink mosaic, while in normal condition it is fully colored pink.The high degree of reliability and the easiness of use of the IPN suggest that it is proper for self-testing for the identification of peripheral neuropathy in people with DM. The purpose of this study is to examine the predictive value of the above-mentioned semi-quantitative method of sweat excretion in people with DM and its contribution in predicting foot ulceration.221 subjects with DM with and without neuropathy were enrolled in the study. Nobody had any other health problems or was receiving any medication that may altered feet humidity. All patients underwent neurological examination. Neuropathy Disability Score (NDS) was calculated and AIPs were applied on their first visit. Follow-up of the patients was either via telephone or by physical presence. The findings are as follows:1. NDS is the gold standard for prediction of ulceration in the future.2. The AIP has high sensitivity (85%) and extremely high negative predictive value (NPV: 95%) in predicting ulceration as a single method. Its high negative diagnostic value implies that if the result of AIP is normal, then the likelihood of ulceration is extremely low. The result seems to be applicable for up to 7 years.3. If both AIP and NDS are abnormal, then the likelihood of ulceration is 82%. This is attributed to the progressive damage of all the mechanisms of sweating of the legs and nerve degeneration associated with DM.Based on the above, it is proposed that the AIP has to be included in everyday clinical practice as a complementary test to predict foot ulceration in people with diabetes.
