▼ This study identified and examined the nature and extent of external factors influencing growth and development of selected SMMEs in Durban, KwaZulu-Natal. The importance of the research lies in the contribution of its findings and recommendations to ensure the survival of SMMEs. The target population was identified as 300 owners of SMMEs in Durban, KwaZulu Natal, with a sample of 169 selected on a convenience basis, to which a questionnaire was distributed.
Regardless of the problems experienced by SMMEs, it is vital to stimulate their growth and development, as this sector is likely to be the job creators of the future South Africa, as well as the providers of a continuation level of employment opportunities (Antonie, 2010). The rate at which SMMEs spring up is encouraging, considering that one of the cardinal goals of establishing SMMEs, is the eradication of unemployment and poverty.
It was found that, not all of the respondents understand the potential of SMMEs to gain a competitive advantage; however, the majority agreed that Durban is an accessible and convenient location to do business, and that Government should partner with SMMEs in job creation and poverty alleviation. In addition, more than 80 percent of the respondents believe SMMEs will thrive with more access to loans and financial support. More than half of the respondents indicated that they often need to bribe government officials in the process of obtaining a business license and/or registration, while the findings also show that, when the process is fair, open and transparent, respondents believe it facilitates SMME growth and development. Recommendations on how factors identified can be better managed, so as to support, rather than negate, SMMEs' quest for growth and development, include government facilitation of workshops, and more visible training.

Conduct a randomised, double blind, placebo controlled study in order to determine the sphere of action of the imponderable remedy Pink 30CH on healthy volunteers who recorded the signs and symptoms produced in order to determine the substances potential usefulness in a future clinical setting according to the Law of Similars.
Determine and highlight the commonalities shared between the symptoms and themes produced by remedy Pink 30CH and the other selected imponderable remedies.
In the homoeopathic drug proving of remedy Pink 30CH, provers were uninformed to both the nature of the substance as well as to the potency selected and used for proving purposes. Neither the provers nor the research investigator had any knowledge of who received the verum or the placebo until the end of the proving.
Thirty (30) provers were selected after meeting the inclusion criteria of which thirty percent (30%) of the subjects received placebo in a randomised fashion. Verum and placebo were dispensed to the proving body in a set of six (6) powders which were taken sublingually three (3) times daily or until any proving symptoms were experienced.
All provers were examined and made to record in their journals before, during and after the administration of the proving substance so as to serve as their own intra-individual controls. At the end of the proving period all journals were then recalled and all proving data recorded was then collated and edited into a repertory and materia medica format, which was then used to formulate a homoeopathic drug picture of the remedy that could be used in future clinical settings.
The homoeopathic drug picture thus derived was then analysed with the aim of highlighting the important themes that were elicited during the proving. These symptoms and themes were then related and compared to seven (7) other imponderable remedies: Luna (Moonlight); Magnetis Polus Arcticus (North pole of the magnet); Magnetis Polus Australis (South pole of the magnet); Positronium (Anti-matter); Radium bromatum (Radium bromide); Sol (Sunlight) and X-ray, in order to expand the overall understanding of the commonalities shared by the imponderabilia as an entirety.

▼ Introduction
This dissertation entails a homoeopathic proving of ivory from the male African elephant (Loxodonta africana) 30CH with a subsequent comparison to Lac Loxodonta africana.
Objectives
The primary objective of this proving was to determine the effects of homoeopathically prepared ivory from the male African elephant (Loxodonta africana) in a 30CH dilution and was achieved by administering the remedy to a group of healthy individuals (provers) who will document all symptoms that arise as a result of taking the remedy. These symptoms will be used to identify the therapeutic indications of homoeopathic ivory. With these specific indications being documented the remedy can then be utilized in the sick individual, that present with similar symptoms, to induce a cure.
A further objective of this proving is to report any variation that may exist in the comparison of two remedies, namely Lac Loxodonta africana (milk derived from the African elephant) and the remedy used in this proving, ivory from the male African elephant (Loxodonta africana).
Methodology
The substance was triturated up until the 3CH and subsequently converted into a liquid potency to be potentised up until the 30CH. Granules were then impregnated with the 30CH liquid potency. Ten impregnated granules were then placed in each individual
ii
lactose powder sachets. A total of six powders were dispensed to the proving participants.
The proving was conducted as a double blind placebo controlled study with a total of twenty-six (26) provers that met the inclusion criteria (Appendix A). The group was made up of both homoeopathic students as well as the general public of varying ages, race and gender. The total group was randomly divided into two groups, twenty (20) of which received the homoeopathic remedy, the remainder (6) received placebo. A full case history of each prover was taken before commencing the proving as well as on completion of the study. Each individual prover kept a journal, starting a week before the proving, which was continued while taking the remedy and ceased when all symptoms had abated.
Once all provers had completed the proving, the information received from the provers through the journals from both groups was collated, assessed and analyzed.
A comparison was then made between this proving and Lac Loxodonta africana to assess whether any similarities or differences were evident. The comparison was made on symptom similarities and rubric analysis.
Results
The proving of ivory from the African elephant (Loxodonta africana) revealed a variety of symptoms. A total of 32 systems were affected in the twenty provers who received the remedy. 716 symptoms were recorded, 83 of which were new symptoms. The systems that were predominately affected were the mind, head and extremities.
Many symptoms were confirmed to be similar to those identified in the proving of Lac Loxodonta africana, though differences were also acknowledged.

Background: In order to develop a balanced healthcare system, healthcare
integration and inter-professional communication is important and allows for
optimum healthcare benefits for a patient and improves cost-effectiveness. The
chiropractic profession has been trying to improve inter-professional
communication with the medical profession. Kimberly Hospital Complex (KHC) is
a tertiary provincial hospital situated in the Northern Cape and since 1998, a
permanent chiropractic post exists at this hospital, making it the only state
hospital in South Africa with a full-time chiropractic clinic and post.
Purpose: To determine the knowledge and perceptions of the medical staff
about chiropractic at KHC.
Method: This study was achieved by means of a questionnaire, which was
modified to suit a South African context by means of a focus group. The
questionnaire was personally delivered to 975 medical staff members at KHC. A
response rate of 30% (n = 292) was achieved and the data was analysed using
SPSS version 15 (SPSS Inc., Chicago, III, USA).
Results: The mean age of the respondents was 37.3 years and most were
female (78.9%, n = 289). Doctors (62.5%, n = 54) and therapists (61.6%, n = 10)
had a higher knowledge percentage score than nurses (48%, n = 213) or other
healthcare professions (56.8%, n = 15). Doctors (77.8%, n = 42), therapists
(100%, n = 10) and other healthcare professions (69.2%, n = 9) were more
inclined to think that chiropractic is an alternative healthcare service, while
nurses perceived chiropractic as a primary healthcare service (43.3%, n = 91).
Many respondents were unaware of the fact that Diagnostics, Emergency
Medical Care, Pharmacology and Radiology are included in the chiropractic
curriculum and that chiropractic leads to a Master’s degree. Seventy five percent
(n = 203) believed that chiropractors are competent in the general medical
iv
management of patients, but they would still rather refer patients to
physiotherapists and orthopaedic surgeons. Despite the poor level of knowledge
of chiropractic, 79.2% (n = 224) believed that it is sufficiently different from
physiotherapy to warrant two separate professions and few (24%, n = 69)
perceived it as unscientific. A large proportion of the respondents (80.3%, n =
228) believe that chiropractic is not well promoted in South Africa and only 20.8%
(n = 59) felt that they know enough about the profession to advise a patient. The
majority wanted to learn more about the chiropractic profession (95.8%, n = 277),
especially pertaining to the scope and the treatment employed by chiropractors.
Seventy-nine percent (n = 212) believed that patients benefit from chiropractic at
KHC and 95.4% (n = 268) felt that South African hospitals would benefit from
chiropractic care.
Conclusion: Due to the poor level of knowledge at KHC, an…

The prevalence of neck pain in musculoskeletal practice is second only to that of low back pain (Vernon et al., 2007). There is a growing interest in neck pain research due to the escalating disability burden and compensation costs associated with neck pain (Côte et al., 2003). Manual therapies are commonly used in the treatment of neck pain (Côte et al., 2003). After an extensive literature review by Haldeman et al. (2008) they found that manual therapy techniques have some benefit but no one technique was clearly superior to the next. Therefore, the purpose of this study is to compare three commonly used manual therapy techniques in the treatment of chronic mechanical neck pain.
OBJECTIVES
The purpose of this study was to compare three different manual therapy techniques (SMT, MET and PNF) which are commonly used in the treatment of chronic MNP in terms of range of motion, pain and disability.
METHOD
Forty-five patients with chronic mechanical neck pain were obtained through non-probability convenience sampling and assigned into one of three treatment groups (15 per group) using a computer generated randomized table. The three different treatment groups were: Spinal Manipulative Therapy (SMT), Muscle Energy Technique (MET) and Proprioceptive Neuromuscular Facilitation (PNF). Each group received six treatments over a period of three weeks with a follow-up consultation. Measurements were taken at the first, third and sixth treatment and at the follow-up consultation.
SPSS version 15.0 was used to analyse the data. A p value of <0.05 was considered as statistically significant. An intra-group analysis was done using repeated measures ANOVA testing to assess the time effect for each outcome separately. For inter-group analyses the time x group interaction effect was assessed using repeated measures ANOVA testing, and profile plots were used to assess the trend and direction of the effects.
RESULTS
Intra-group analysis of the results revealed that all three groups improved significantly between the first and the final consultation, for all measures. Inter-group analysis of the data did not show any difference between the three groups by the end of the final consultation. However, extension range of motion appeared to improve slightly faster in the PNF group
iv
but it was not significant when compared to the other two groups. Therefore, there was no statistical significance between the three groups.
CONCLUSION
It was concluded that all three treatment groups responded equally to the treatment, thus, suggesting that MET or PNF techniques can be used if SMT is contra-indicated.

▼ Dissertation in partial compliance with the requirements for a Master's Degree in
Technology: Chiropractic, in the Department of Chiropractic, Durban University of
Technology, 2010.
Background: Chiropractic in South Africa seems to be gaining acceptance by medicine
with increased recognition from the private healthcare sector. This trend is reflected by
the recognition of private healthcare providers of chiropractic services. Integration would
accelerate the growth of the chiropractic profession in this country. It is therefore
important to understand how chiropractic is currently perceived with respect to
integration into the KwaZulu-Natal (KZN) healthcare system. As well as to determine
factors perceived to facilitate or hinder this integration.
Objectives: To explore and describe the perceptions of selected stakeholders about
the integration of the chiropractic profession into the KZN healthcare system.
Method: The sample included ten selected stakeholders within the KZN healthcare
sector. Each participant participated in a semi-structured interview. Questions included
participants‟ experience of chiropractic, the role and scope of chiropractic practice, and
key developmental issues affecting integration. Interviews were captured on a digital
voice recorder and transcribed into text. Data was analysed by the use of NVivo
software (NVivo 8, developed and designed in Australia, copyright 2008 QSR
International Pty Ltd. ABN 47 006 357 213).
Results: The majority of participants (n = 7) had a positive experience of chiropractic,
but few (n= 2) recognised the diagnostic role of chiropractic. All participants, except two
doctors, believed that integrating chiropractic into the public healthcare system would
benefit the healthcare fraternity, the chiropractic profession and patients.
However, hindering factors perceived by the participants included: chiropractors
practicing non-evidence based techniques; chiropractic being registered with a different
council and being taught at a different institution to conventional medical professionals;
and most importantly a lack of knowledge of the profession. Facilitating factors were
III
perceived to be: increased education of stakeholders about chiropractic; improved
communication between chiropractors and medical doctors; improved marketing
strategy; and lastly improved patient management.
Conclusions: A positive experience of chiropractic is directly affected by a positive
exposure to the profession. The profession itself is responsible for dispelling some of
the confusion it has created, by collectively practicing evidence based medicine, and
marketing a united message to stakeholders.

Introduction
All the studies on the perception of homoeopathy in South Africa conducted so far have found that the group least knowledgeable about homoeopathy is the African group. However, no study yet has focused on this group, and tried to find out more detail about their perceptions of homoeopathy.
The study took place in Mnambithi, a town in the northern part of KwaZulu Natal within the Uthukela District with a population of about 200 000 people.
Aim of the study
The overall aim of this study was to determine the perception, knowledge and utilization of homoeopathy amongst African adults resident in Mnambithi, KwaZulu Natal.
Methodology
The survey method was employed to conduct this study. The research instrument was a self-administered questionnaire. The number of questionnaires completed was 1034, distributed according to suburbs 10.6%, centre of town
iii
9.8%, former township areas 58.7% and rural areas 20.6% which approximately reflected the proportion of the population resident in those areas. The data was primarily analyzed by means of descriptive statistics using frequency tables.

The sample consisted of 50.3% males and 49.5% females, with the largest group of respondents being 41 years old and above. Most were unemployed (61.8%). The educational standard was high, with 43.8% of respondents having matric and 20.4% having a diploma or degree. Most respondents (98.6%) had not heard of homoeopathy before. Only 0.1% of respondents had consulted a homoeopath before. 83.8% of respondents answered that they would consider consulting a homoeopath in the future, and 43.3% indicated they were interested in learning more about homoeopathy. The lack of knowledge about homoeopathy and yet being interested in learning more, is a similar finding to other perception studies.
Conclusions
It can be concluded from the results that the level of knowledge of homoeopathy amongst respondents was minimal, with only 10 out of 1034 respondents having heard of homoeopathy. Questions relating to the perception of homoeopathy were restricted to those who had heard of homoeopathy, but because of the small number, no conclusions regarding perception can be drawn.

Forward head posture (FHP) is a common postural abnormality, often
associated with myofascial trigger points which can result in head and neck pain. The
craniovertebral (CV) angle lies between a horizontal line running through C7 spinous
process and a line connecting C7 spinous process to the tragus of the ear. The
smaller the angle the greater the FHP. Cervical musculoskeletal abnormalities have
often been linked to headache types, most especially episodic tension-type
headache (ETTH) and cervicogenic headaches (CGH). Objectives: To determine
whether an association exists between FHP, distance of the external auditory meatus
(EAM) from the plumbline and cervical range of motion and the presentation of ETTH
and CGH. Method: This was a quantitative comparative study (n=60) comparing
three equal groups, one with ETTH, CGH and healthy controls. The FHP of the
Subjects FHP was assessed by measuring the CV angle. A lateral digital photograph
was taken to assess the distance of the external auditory meatus from the plumbline.
Lastly, cervical range of motion was measured. The two symptomatic groups also
received a headache diary for a fourteen day period monitoring frequency, intensity
and duration of their headaches. Result: The two symptomatic groups had a smaller
CV angle and a greater distance from the plumbline (p<0.05) than the asymptomatic
group. The asymptomatic group had a significantly greater flexion (p=0.009),
extension (p=0.038) and left rotation (p=0.018) range of motion than the two
symptomatic groups. The CGH group had a significant positive correlation between
the distance of the EAM from the plumbline and the intensity of headaches. The
ETTH group had a significant positive correlation between the right craniovertebral
angle and the mean duration of headaches. Conclusion: Therefore, it can be
concluded that patients presenting with ETTH and/or CGH may have associated
postural abnormalities that may act as a trigger or a contributory factor to the
presenting headache.

▼ Dissertation submitted in partial compliance with the requirements for a Masters Degree in Technology: Chiropractic, Durban University of Technology, 2011.
The relationship between TMD and dysfunction in the cervical spine has been reported in the literature and there are many case studies which have shown favourable results when treatment was aimed at the TMJ, cervical spine relationship. However, the numerous TMD treatment protocols described in the literature concerning this relationship, and the effectiveness of these treatments have not been well established. In spite of this many chiropractors treat TMD. TMD is a multifactoral condition and conservative treatment options need to be further investigated in order to determine if manual interventions directed at the cercival spine in the treatment of TMD are beneficial.
Objectives: The purpose of this study was to compare myofascial trigger point therapy and manipulative therapy of either the TMJ, cervical spine or a combination of the two in order to determine their effectiveness for the TMD.
Method: Thirty participants with TMD were randomly assigned to one of three treatment groups. Participants in each group received two treatments per week for two weeks with a follow up consultation in the third week. Data were collected before the commencement of the first, second, and fourth treatments and at the follow up consultation. Outcome measures included algometer readings, CROM, Mouth opening readings, NRS and a disability questionnaire. SPSS version 15.0 was used for analysis of the data. A p value <0.05 was considered as statistically significant. Multivariate testing was used for intra- and inter-group comparisons. Profile plots were generated to assess the direction and trend of the effect and to visually compare the trends in the different treatment groups.
Results: Inter-group comparisons did not reveal any statistically significant different improvements between the three treatment groups.
Conclusion: All groups responded favourably to treatment and showed trends towards improvement. However, statistically analysis revealed that no one treatment protocol was superior to the other. Although no definitive inferences may be drawn regarding the effectiveness of each treatment approach, within group trends indicated that the combination of the two treatment approaches may be preferred.

Background: Neck pain is an extremely common condition and the treatment of neck pain forms an integral part of chiropractic practice. The optimal treatment of neck pain is provided when practitioners incorporate available evidence, experience, and knowledge regarding the clinical presentation of the patient into their treatment regimes. Current evidence suggests that a combination of manual therapy, specifically manipulation and/or mobilization, and rehabilitation may offer the optimum treatment for mechanical neck pain. However, numerous factors other than available evidence, experience and clinical presentation may influence treatment choices made by practitioners. Through the assessment of practice patterns, one may asses if the optimal treatment for a neck pain is being provided by practitioners and assess if factors specific to a practitioner may influence the treatment of neck pain.
Objectives: The aim of this study is to determine the chiropractic treatment and management of mechanical neck pain, to compare this to evidence based recommendations for the conservative treatment of mechanical neck pain and to assess if factors other than the available evidence may influence the treatment of mechanical neck pain.
Method: A quantitative, cross-sectional descriptive survey compiled using available literature and validated by means of a focus group and pilot testing, was administered to chiropractors practicing in KwaZulu-Natal. Upon completion of the questionnaire, the data was coded into an Excel spread sheet and imported into IBM SPSS version 20 for statistical analysis. This research protocol was approved by the Durban University of Technology Institutional Research Ethics Committee (REC 82/13) and the study took place from March to July 2014.
Results: Ninety-six practitioners responded to the study which is a response rate of greater than 70%. Practitioners favoured the use of spinal manipulation, auxiliary therapeutic techniques (specifically those which were manual in nature), rehabilitation, and numerous forms of education. Specific variations in treatment pattern existed when comparing various patient presentations indicating that practitioner based factors impacted on treatment choices made by practitioners. The most significant findings included the increased utilisation of auxiliary therapeutic techniques by female practitioners, the increased utilisation of traction by practitioners identifying with the straight philosophy of chiropractic. Other significant findings included the increased utilisation of cervical collars by practitioners of increased age and experience and the increased utilisation of auxiliary therapeutic techniques by practitioners who did not attend health related conferences at least once every second year or…

The purpose of this study was to evaluate Chinese parents' buying behaviour towards baby milk powder in the context of China's One-Child policy. The study examined the Chinese culture, social trends, the influence of product attributes among urban Chinese parents and parents' product knowledge of baby milk powder. This study is an exploratory study. A questionnaire was used to collect the data from 400 respondents at Shangdong University, China. There are 312 respondents' questionnaires used to analyse the results. The results were discovered through the use of four research objectives. Firstly, the results reflect that family and friends do have an influence on buying decisions on baby milk powder. There parents rely more on word-of-mouth communication and shop assistants play a negative role in parents' buying decision. Secondly, mothers need to opt for baby milk powder for their babies due to the flood of women into the job market and incomes from dual earners. Thirdly, parents perceive high price with good quality; the preferred brand by the parents have a significant impact on their brand loyalty and parents generally hold positive views of foreign-made baby milk powder. Fourthly, parents have a high level of subjective and objective knowledge of baby milk powder. This study was limited to the sample size and the geographic area. Consequently, results of this study can not be regarded as representative of the entire Chinese population. The results may help marketers develop more effective marketing programmes to affect consumers' buying decision. In addition, this study is one of a few studies that apply the theory of buying behaviour in the context of China's One-Child policy in the marketing field.

The South African tourism industry is becoming an important service sector in this country, which is attracting increasing international tourists in recent years. China, as a fast developing country, will be exerting greater influence on the development of tourism worldwide over the next decade. In particular, the impact of rapid growth of China's outbound tourism market has been felt by South Africa. A feature that South Africa's tourism marketers need to understand is the different demographics of the Chinese population and their different perceptions on SouthAfrican tourism. The purpose of this study is to identify, analyze and evaluate factors that could impact positively and/or negatively on the image of South Africa as a tourist destination among prospective tourists from China. It examines the opinions and perceptions of Chinese tourists in Shanghai. The research evaluates the kinds of perceptions and attitudes held by Chinese tourists towards South African tourism. The study was conducted with the assistance of fieldworkers in Shanghai who helped the research complete all the interviews and questionnaires. Data was analysed through the Statistical Package of Social Sciences (SPSS), and findings, conclusions, and recommendations are presented. The study provides the recommendations to South African tourism organizations to take into considerationthe seriousness of Chinese tourists' understanding and perceptions towards South Africa as an emerging tourism destination as results indicate that many Chinese tourists do not quite understand South Africa and lack proper knowledge and information. Findings indicated that, although many Chinese tourists do know about South Africa as a tourist destination, they do

The aim of this research is to analyze the introduction of lean manufacturing in a selected South African organization viz. Smiths Manufacturing. The research, together with its conclusions, may identify problematic areas which, once addressed, will improve the implementation in other areas and identify the benefits, if any, of lean manufacturing. Three research objectives exist and two methods of research were used to solve the research questions. The first and second research objectives (i.e. to analyze the manner in which lean manufacturing was introduced in Smiths Manufacturing and its benefit on the company's competitive position and future existence) were analyzed qualitatively with the aid of personal interviews. The third research objective (i.e. to determine the financial benefit of lean manufacturing introduction on the respective assembly line) was analyzed quantitatively. This research revealed three pertinent findings: Firstly, Smiths Manufacturing has adhered to eight of Liker's 13 Step Implementation Procedure with two areas of non-conformance being identified and insufficient information present to comment on the remaining three steps. Secondly, improvements in the manufacturing process have enabled the company to be more price competitive. Areas of improvement were in labour, raw materials and finished goods. Purpose: Chronic ankle instability (CAl) is characterised by ankle pain, weakness, edema, crepitus, adhesions, restrictions and ligamentous laxity. Various treatment options target a variety of aspects of this condition. However, there is a paucity of literature with regards to combined treatment choices. The purpose of this study was to investigate the relative effectiveness of combining manipulation with rehabilitation, compared to rehabilitation only, in participants with CAl. Methods: The study was a single blinded, randomised and comparative clinical trial at a chiropractic day clinic. Thirty participants with grade I and II CAl were recruited and randomly allocated into one of two treatment groups. Fifteen participants received a combination of manipulation and rehabilitation (coupled peroneal muscle strengthening and proprioception) and fifteen received the rehabilitation only programme. All six treatments in each group were conducted over five weeks. Results: A P value of <0.05 was considered statistically significant. The intra-group outcomes in the Manipulation and Rehabilitation Group indicate that statistically significant improvements were achieved for all six parameters in this study (VAS p<0.001; FAOI p<0.001; Algometer p<0.001; motion palpation p<0.001; WBO p=0.001 and BBS p<0.001). This is in comparison to the three outcomes where statistical significance was achieved (VAS p<0.001; FAOI p<0.001 and BBS p<0.001) in the Rehabilitation only Group. Inter-group analysis revealed statistically significant improvement in favour of the Manipulation and Rehabilitation Group for VAS (p<0.001); algometer readings (p=0.002) and Motion palpation findings (p<0.001). Conclusions: The findings of this study show that manipulation in combination with rehabilitation is relatively more effective than rehabilitation only for most outcome measures. Introduction
Although the South African Government adopted a primary health care approach to health care service provision in order to ensure equitable access to and utilization of health care services to all communities, the country continues to face challenges regarding access and utilisation of health care services especially in the rural communities. Antenatal care which is mostly provided at primary health care level is regarded as the cornerstone for the success of the maternal and child health care programme. Therefore, poor access to and under-utilisation of health care services could potentially influence the success of this programme and pregnancy outcomes.
Aim of the study
The aim of the study was to determine whether pregnant women from KwaMkhizwana rural community had access to and were utilising antenatal care services.
Methodology
A qualitative, exploratory, descriptive and contextual study was conducted guided by Thaddeus and Maine’s three delays model. Purposive sampling of the pregnant women and all categories of nurses who were employed in the three health care facilities in the area was done. Data was collected in two phases through in-depth semi-structured interviews with both the pregnant women and the nurses respectively between February and March 2016. The sample size was guided by data saturation. All data were analysed using the Tesch’s method of data analysis.
Study findings
Six major themes and several sub-themes emerged from the interviews with both Phase 1 and Phase 2 participants. The major themes included: 1) access to health care and emergency services, 2) availability of human and material resources, 3) social and cultural beliefs, 4) past pregnancy experiences, 5) communication and transparency regarding health care service delivery and 6) quality of antenatal care services.
Summary of the findings
The pregnant women encountered several challenges which led to delays in seeking, reaching and receiving antenatal care. Most of the pregnant women participants related limited access to health care, with under-utilisation of antenatal services. They were unhappy about the antenatal care services they received in the three available health care facilities in the area, which made these facilities to be inaccessible and underutilised. The nurse participants recognised the challenges facing the pregnant women regarding the access and utilisation of antenatal care services, together with the challenges faced by the nurses while working in the three available health care facilities in the area.
Recommendations
The recommendations that were made included: to consider building a centrally located fixed primary health care clinic that would ensure equal access to health care services, strengthening the implementation of policies regarding the referral system and ambulance services, ensuring sustainable availability of human…

One of the major issues following the genocide in Rwanda in 1994 was what to do with the huge number of people (around 100 000) accused of crimes during the genocide. Western legal approaches dealt with a handful of such cases at huge expense but the vast majority of the accused languished in prison. The government decided to employ a modified version of Gacaca - the traditional way of dealing with disputes and lower level crimes at community level.
Using a qualitative research methodology and employing focus groups and individual interviews as data collection tools, this research investigate perceptions about the operation of Gacaca in three Rwandan communities, with particular reference to truth, justice, forgiveness and reconciliation.
The research suggests that in the three communities, Gacaca was seen as bringing the truth out into the open and to provide a measure of justice, although limitations were noted in both of these respects. Given the enormity of the genocide crimes, however, there seemed to be little progress in the areas of forgiveness and reconciliation.

Cardiomyopathies and their resultant heart failure (HF) remain a
major cause of cardiovascular morbidity and mortality (Wood and Picard, 2004).
Idiopathic dilated cardiomyopathy (IDCMO) is a primary myocardial disease of
unknown cause, characterized by left ventricular (LV) or biventricular dilatation
and impaired myocardial contractility. Dilated cardiomyopathy (DCMO), along
with rheumatic heart disease and hypertension (HPT), is one of the leading
causes of HF in Africa. In fact, in an epidemiology study of 884 patients in
Soweto, IDCMO was the second major cause of HF. Thirty five percent of
patients in the study, with HF, had IDCMO (Sliwa, Damasceno, Mayosi, 2005).
Methodology: Patients referred to the cardiomyopathy (CMO) clinic at Chris
Hani Baragwanath hospital, situated in the echocardiographic lab, were recruited,
provided they satisfied the exclusion and inclusion criteria and were enrolled after
obtaining voluntary informed consent. From May 2009 to September 2010, 70
patients with IDCMO were recruited for this trial. Patients with DCMO were
identified by means of echocardiographic criteria which included a left ventricular
ejection fraction (LVEF) of less than 45% and an end diastolic dimension (EDD)
of greater than of 52 mm (2D in long parasternal axis).
Results: In the present study the prevalence of left ventricular (LV) thrombus in
patients with IDCMO was 18.6%. When using Univariate logistic regression, the
only independent predictors of LV thrombus formation was LVEF and age.
However, when multivariate logistic regression analysis was applied to the data,
the only predictor with a significant association was age. The reason for this is
not clear. It is postulated that perhaps younger patients have differences in the
pathophysiology of their disease such as a greater smoldering inflammatory
component which may therefore predispose them to thrombus formation. For
example the presence of IL-6 may be important in the formation of LV clot in
cases of LV dysfunction (Sosin, Bhatia, Davis, Lip, 2003). The association
between LVEF and LV thrombus was borderline significant.
Conclusion: The prevalence of LV thrombus formation in this cohort of patients
with IDCMO was 18.6%. Echocardiographic parameters alone cannot predict
which patients are more likely to develop thrombus formation.

▼ Submitted in partial compliance with the requirements for the Master’s Degree in Technology: Chiropractic, Durban University of Technology, Durban, South Africa, 2016.
Background: Neck pain is a condition that is becoming more common throughout the world and most people can expect to experience some degree of neck pain sometime during their life. There is a need for early diagnosis and follow-up of neck pain to assess a patient’s level of self-rated disability due to the escalating disability burden and compensation costs associated with neck pain. For this reason, disability questionnaires are increasingly used for clinical assessment and as an outcome measurement for the treatment of neck pain. Translation and cross-cultural adaptation of disability questionnaires have thus become increasingly necessary when dealing with different cultural groups.
Methods: This is a quantitative validity and reliability assessment study comparing the English versions of the Neck Disability Index (NDI) and the Quadruple Visual Analogue Scale (QVAS) to their translated Afrikaans counterparts. The first step was to translate the questionnaires into Afrikaans. The translated questionnaires were then scrutinised and critiqued by an Expert group, who are fluent in both the Afrikaans and English languages, and back translated to English in order or establish their face validity. A small pilot study was conducted with the original and translated versions of the questionnaires to establish their content validity.
The translated and original versions of the NDI and QVAS were given to a study group to complete with a suitable time delay between the completions of the questionnaires. The results from the study group were statistically analysed to establish concurrent validity and reliability.
Results: There were 50 participants each completing one English NDI and QVAS and one Afrikaans NDI and QVAS. The results indicate high reliability for both the Afrikaans NDI (α = 0.900) and QVAS (α = 0.883). The Afrikaans NDI and QVAS are deemed reliable compared to their English counterparts.
The results indicated a significant level of concurrent validity for both the NDI and the QVAS.
Both the Afrikaans versions of the NDI and QVAS were deemed reliable and concurrent validity was established. The NDI and QVAS were successfully translated and the Afrikaans versions can now be used in the Afrikaans population as viable alternatives to the English NDI and QVAS.

This study discusses the impact of modernisation of rural homestead living spaces on dwellers in a selected area of Umbumbulu, south of Durban Kwa-Zulu Natal South Africa. The study was conducted after a change was noticed within the rural homesteads built environment. Factors responsible for the changes in building/ dwelling shape, size, style, as well as the choice of materials (SSSM) used were discussed. The study further examined the impact of the listed changes within the social context of Umbumbulu’s rural dwellers, while addressing in-depth questions around the topic of modernisation, especially within the confines of rural homesteads and living spaces.
A qualitative research approach was employed where an interpretative research paradigm was chosen as a theoretical framework for the study. Data consisted of seven semi structured interviews. The research design consisted of themes, the analysis, as well as the findings in relation to literature.
The conclusion showed what the rural dwellers understand about modernisation in a rural context, as well as how it has impacted the changes in building/ dwelling shape, size, style, as well as in the choice of materials used. Three identifiable themes were discussed namely: 1. modernisation; 2. change in building shape, style, size and materials; and 3. positive living experience. Links were made between the themes which in turn indicate how modernisation has had an impact on rural homestead living spaces and dwellers.

The years succeeding 1990 have seen a significant increase in the release of mainstream film featuring transgendered characters. The inclusion of such characters in popular film becomes a point of interest as transgendered identities differ from the hegemonic heterosexism of the audiences at whom these films are targeted. This investigation aims to gain a better understanding of how audience members read gendered identity through the visual appearance of drag queen characters in mainstream film. Due to the emblematic contrast between the male body and a hyper-feminine dress aesthetic, drag queens pose an overt visual challenge to the normative expectation of anatomical sex determining gender and gendered expression. This investigation is conducted from the paradigmatic perspective that recognises the impossibility of a ‘correct’ reading of dress aesthetics and is thus concerned with discovering the various gendered meanings audience members may attach to drag costume in film. This interpretivist standpoint, however, is held in conjunction with the critical understanding that prevalent contemporary socio-political constructs with regard to gender and dress will undoubtedly affect these perceptions. Segments from selected Hollywood films featuring drag queen protagonists were screened for a heterogeneous focus group and the subsequent discussion analysed through critical discourse analysis. Academic discourse concerning the socially constructed gender dichotomy and the debated subversive potential of the drag act is reviewed in order to provide a theoretical framework for analysing the participants’ comprehension of gendered performance. Gendered associations with dress and the body together with film theory are examined to better understand how an audience may perceive gendered identity through drag costume in film and what affect this may have on their conception of sartorial gendered expressions in reality. Finally, to situate and provide further context for this investigation, Queer theory critiques of the representation and reception of transgendered characters in past mainstream films are considered.

This study investigates how language is used to communicate meaning in bank advertisements. It also examines stylistics in advertising with specific focus on selected bank advertisements in South African and Nigerian newspapers and magazines. Stylistics is a branch of linguistics which studies the principles, and effect of choice and usage of different language elements in rendering thought and emotion under different conditions of communication. Advertising is a form of communication used to help sell products and services. Adverts are not only designed to fascinate, but also to achieve their persuasive goal. This study shows how language is used in bank advertisements to convey messages to the public. The stylistic elements employed in the analysis of bank adverts included graphology, phonology, lexis, syntax and cohesion. The use of capitalization and repetition for emphasis, phonemes and Gothic writing to attract the attention of readers and images to stimulate customers’ aspirations were some of the findings of the study. Given that studies on language use are still of high interest to linguists, this study critically interrogates the effectiveness of language choice in bank advertisements. The conclusion is that stylistic devices are important in advertising as they attract customers to the services and products being advertised.

Background
An increase in the world's population of ageing people is occurring not only in developed countries but also in developing countries. In South Africa, the proportion of the population aged 50 and over increased from 14.8% in 2006 to 15% in 2009 and is predicted to be 19% by 2030. This means that the supply of services for the elderly people should match the demand at all times, otherwise the quality of life of these senior citizens will be compromised. This study aimed at developing a model that would improve the quality of life for elderly people living in the uMhlathuze and uMlalazi sub-districts of the uThungulu district, KwaZulu-Natal.
Methodology
A qualitative, exploratory, descriptive design was applied for this study. A semi-structured interview guide based on the Health Related Quality of Life Theory and Maslow’s hierarchy of needs was used. Random sampling was used to select the elderly participants. Purposive sampling was used for the selection of the chairpersons of non-governmental organisations, and the District Programme Manager. Audits were conducted in the clubs that elderly people with chronic disease participate in. Data analysis followed Tesch’s steps after which themes and categories were formulated.
Results
Three major themes that emerged from the data analysis were social well-being of elderly people, physiological factors and psychological factors. The results revealed that elderly people experience poor living conditions and suffer poverty due to a number of factors which including the high unemployment rate of their children. Often the children are involved in substance abuse using the elderly person’s money pension money, which leaves them without any food items in the household. According to the District Programme Manager, there was a project on integrated chronic disease management that was conducted at uThungulu district by the Department of Health. The integrated chronic disease management focused on the population in general of all ages, yet in this study the focus has been on elderly people, which is why the researcher developed a model to improve the QoL of elderly people, due to their unique needs.
Conclusion
This research study gathered information regarding social, economic, health and environmental factors in rural areas which will help in bringing issues of elderly people’s quality of life to awareness. This research will deepen the knowledge and skills of professionals on ageing issues, especially in rural areas/communities.

The study arose as a result of a merger between the former Technikon Natal and former M.L. Sultan Technikon, culminating in the formation of a new institution in 2002, called the Durban Institute of Technology (now known as the Durban University of Technology). The focus of the study was on the impact of mergers on lower level employee motivation and staff morale at the Durban University of Technology. Furthermore, the study focused on examining the perceptions of lower level employees towards the merger process which included the pre-merger, during and post-merger phases. A mixed methods research design was used in this study. The quantitative sample was n=50. In addition, six in-depth interviews were conducted for the qualitative part of the study.
This study concluded that the merger had a negative impact on the perceptions of lower level employees regarding the merger process in the post-merger phase. The results indicated that staff morale of lower level employees was low post-merger. Some lower level employees proved to be less satisfied than others who adopted the new changes as a result of the merger. Interestingly, the results indicated a high level of motivation amongst lower level employees post-merger. Minimal communication originated from top management to lower level employees. It was recommended, amongst other suggestions that communication and training should be strengthened amongst lower level employees, supervisors and management in higher education institutions. Whilst lower level employees may have low skills and education, an attempt should be made to involve lower level employees early on and throughout higher education mergers.

INTRODUCTION
Globally, the use of alternative therapies by the public as well as healthcare workers has been widely documented. In South Africa, no studies have been done on alternative therapies specifically related to nurses. The vast amount of media attention with regards to alternative therapies related to the positive and negative outcomes has sparked major interest in this topic. The aim of the study was to determine the perceptions of professional nurses at nine hospitals within the uMgungundlovu District, South Africa, towards alternative therapies in a nursing context.
OBJECTIVES OF THE STUDY
The objectives of the study were to determine the professional nurses’ perceptions as well as knowledge of alternative therapies. The possible barriers in providing alternative therapies in nursing practice was also explored in order to make recommendations to guide professional nurses with regard to alternative therapies in patient care.
METHODOLOGY
A quantitative exploratory research approach underpinned this study. A survey questionnaire was used to obtain information from professional nurses who had consented to participate. The total population of professional nurses was 1218. A total of 616 questionnaires were distributed to participants at the nine participating hospitals. Three hundred and eighty six (386) questionnaires were returned, which resulted in a 63% response rate.
FINDINGS
It was evident from the data that professional nurses used various forms of alternative therapies. The results revealed that almost 69% (n=265) used prayer/spiritual healing, followed by 53.9% (n=207) who used nutritional supplements, whilst 51.9% (n=199) used music therapy. Participants rated their satisfaction on the information received regarding alternative therapies in nursing education as follows: prayer/spiritual healing 71.1% (n=271), nutritional supplements 71.7% (n=273), music therapy 57.9% (n=220), massage therapy 46.7% (n=216) and support groups 80.3% (n=306).
About two thirds (n=272) felt that alternative therapies was beneficial rather than a threat to patient’s health. However, 60% (n=229) agreed that the use of alternative therapies that were not tested in a scientific manner should be discouraged. More than 80% (n=313) of the sample thought that health professionals should have the ability to advise patients in the clinical setting about the most commonly used alternative therapies.
More than half of the sample (n=254) stated that they did encounter patients in the clinical setting who enquired about alternative therapies. Only 39.6% (n=153) stated that they felt confident to advise patients about alternative therapies, whilst 26.7% (n=103) stated that they were unable to do so.
As the evidence base for the use of alternative therapies grows, so too does the demand increases for the integration of alternative therapies in nursing…

Activated sludge (AS) flocs are paramount in biological treatment of wastewater, are comprised of microbial consortia with organic and inorganic material bound together by extra polymeric substances (EPS). The filamentous bacteria play a vital role in the floc formation process by providing the necessary structural support. Presence of epiphytic attachment on selected filamentous bacteria is a commonly occurring phenomenon in activated sludge samples. Different theories have been proposed to describe this phenomenon; however, not much research has been carried out to explore the profundity of the attachment. In this study, an attempt has been made to elucidate the intrinsic nature of the epiphytic attachment between the bacterial rods and filamentous bacteria based on microscopic (morphological and structural) analysis. Characterization of these epiphytes were performed using fluorescence in situ hybridization (FISH) at group level using Alpha, Beta and Gamma Proteo-bacterial probes. Morphological characteristics of filament hosts and the bacterial rods at the interface region was assessed using scanning electron microscopy (SEM). The SEM micrographs indicated that the attachment was facilitated by more than the EPS layer. Further ultrastructural examination using transmission electron microscopy (TEM) indicated a possible cell-to-cell interaction between epiphytes and the selected filaments. Fibrillar structures resembling amyloid-like proteins were observed within the filament cell targeted by the epiphytes. An interaction was apparent between the amyloid like proteins and the epiphytes as exhibited by the direction of fibrillar structures pointing towards the
approaching epiphytes. Common bacterial appendages such as pili and fimbria were absent at the interface and further noted was the presence of cell membrane extensions on the epiphytic bacteria protruding towards the targeted filamentous cell. The sheath of host filaments however, remained intact and unpenetrated, during colonization. Amyloid-like fibrils at interface may potentially play the role of attachment sites for the attaching epiphytes, as attachment facilitating appendages were not visualized.

In this research, the woven fibre microfiltration (WFMF) fabric which is produced locally in South Africa is used as a membrane material. It is cheaper in price in comparison with the current commercial membrane materials that are in use. The WFMF is also more robust when compared with the commercial membrane materials thus is able to withstand harsh working conditions. From previous studies on the WFMF, it has been shown that it can be used as a membrane material without any compromise to permeate quality. This research seeks to optimize the working conditions of this membrane material (WFMF) with an aim of achieving lower running costs and better anti fouling strategies in comparison to the commercial MBRs.
The objectives and aims of this research was to come up with a MBR system whose running cost is lower than that for the commercial systems, which can be adapted for use in any environment, especially in the hardship regions where its robustness would be an added advantage. The performance of the WFMF submerged MBR was also optimised including antifouling operating regimes.
This study was done in a pilot plant that was set up at Veolia wastewater treatment plant, Durban Metro Southern Works. The feed water for the pilot plant was pumped from the return activated sludge mixing chamber by means of a submersible pump. The MLSS concentration of the feed water was about 12 g/l. The various investigations that were conducted in the course of this research included the effect of spacing between membrane modules, relaxation steps and frequencies, evaluation of aeration rates and evaluation of coarse vs. fine bubbles which were all aimed at optimizing the performance of the immersed WFMF MBR. The permeate was checked for turbidity and COD levels to ensure that they were within the accepted water standards.
From the experiments it is shown that the critical flux increased with an increase in aeration rate which is in concurrence with the literature and a starting flux of 30 LMH was chosen for the running of the pilot plant for the various experimental runs to be carried out. For the pipe diffuser height effect experimental run, the best results were achieved at a height of 5 cm below the membrane modules and the use of a pipe diffuser gave better results than the use of a disc diffuser. For the membrane module spacing effect the best results were obtained at the smallest possible width i.e. 3.5 mm. The best relaxation step sequence was found to be 9 mins on and 1 min off. COD, turbidity and DO was continuously determined during the course of the experimentation.
Further studies should be done on use of the disc diffuser with increased surface area of aeration holes and also hole sizes of smaller diameters to check on its effectiveness as a means of reducing fouling on the membrane surface.

The emergence of a highly competitive and integrated international economy, rapid technological innovation, and a growing knowledge base will continue to have a profound impact on the lives of communities. In recent years there has been an international movement towards educational reform, particularly in science education, to meet the need for a sustainable environment, economy, and society. Science education is a key element in developing scientific literacy among today’s and tomorrow’s youth. Recent decades have seen an exponential growth in scientific applications, and one would expect an accompanied increase in science interests in the classroom, as well as an increased understanding of basic science ideas and ways of thinking will follow. However, research has shown that this is not the case.
In this study, the researcher, through the research methodology, took an in-depth look at whether the curriculum reform in the Life Science curriculum was forthcoming to lifelong learning. The key questions guiding this study are as follows: (a) What are the perceptions and expectations of grade 12 learners regarding the quality of the Life Sciences curriculum in respect of lifelong learning; (b) What are the perceptions and expectations of grade 12 learners regarding the their Life Sciences educators; (c) What are the perceptions of the grade 12 Life Sciences educators of the relevance of the grade 12 Life Sciences curriculum with a view to lifelong learning; (d) How does the grade 12 Life Sciences curriculum impact on lifelong learning of learners.
This study falls within the realm of mixed methods study. Mixed methods encompass both a quantitative and a qualitative study. The study was conducted in the district of Pinetown because in 2013 this district produced the best results in Kwa-Zulu Natal in the National Senior Certificate Life Sciences examination. This study used simple random sampling to obtain the learner participants. The researcher conducted the study in 16 schools and 25 learners from each school were randomly selected. The educator sample comprised 75 grade 12 Life Sciences educators. The SERVQUAL questionnaire was administered to the 400 learners that were randomly selected while; the educator questionnaire was administered to the sample of 75 educators.
The findings of the research revealed that educators experienced many challenges which hindered the successful implementation of the Life Sciences curriculum effectively. The quality of the Life Sciences curriculum itself did not that hinge on lifelong learning. It did not have very much relevance to the lives of the learners and as such the learners were learning about things that were abstract to them. Another finding was that educators found it challenging to relate the grade 12 Life Sciences curriculum to the everyday experiences of the learners because time was a limiting factor. The exam driven nature…

The thesis examines the operations and outcomes of the Self Help Groups (SHGs) in Matetsi ward. It examines the causes of conflicts and consequences of the conflicts in the SHGs. This is achieved by adopting the conflict transformation approach. Succinctly, the theory views conflict as not necessarily destructive, but as a vehicle for change. This therefore, has implications for one’s understanding of the causes of conflicts. SHGs have been adopted by rural women so that they can work collectively to address their socio-economic problems. The study was conducted in Matetsi ward, Hwange District where conflicts within the groups were broadly detected. The information used in the study were produced from face-to-face interviews, focus group discussions, minute books and testimonials throughout field work in the designated ward. The study exposed that conflicts in the study location emanated from delays in loan repayments by SHG members. Individuals’ failure to fulfil their obligations of saving and returning loans on time created tensions which in turn resulted in conflict among the group members. The conflicts became complex and recurred due to unaddressed animosity, fear and anger, resulting in frustrations in the groups. Conflicts in the groups also led to the collapse of some of the groups. The research findings indicated that SHGs were not only sources of income for rural women, but also represented a yearning for self-sufficiency among rural women in spite of the socio-economic strife in Hwange District.

The overall aim of this study was to examine the circumstances under which violence occured among learners at Phatheka Primary School, situated in the township of Umlazi, and in doing so explore the causes of violence among learners at this school. The study further aimed to explore the complicit role of the school as an agent in shaping the manner in which learners relate to violence and vis à vis shaping learner behaviour. The research was driven by three objectives: Firstly, to investigate the gendered, class and ethnic nature of perpetrators and victims of school violence. Secondly, to examine the intra-group interactions among peers and their positioning of each other in terms of violent behaviour. Lastly, to explore how the discipline regime of the school shaped the constructions of violent and peaceful behaviour among learners.
The qualitative research approach was utilised to obtain detailed and rich data. Ten educators and ten learners were purposively chosen from one urban primary school in the Umlazi Township. The ten learners that were interviewed, were identified from teachers’ observations of conflict situations at the school, in the classroom and on the playground. The ten most senior educators formed the sample of educators for this study. Seniority was determined according to the number of years the educator had been teaching at this school. Four of the most senior male educators and six of the most senior female educators were selected.
The data was collected through face-to-face interviews and focus group interviews. The composition of the group consisted of learners with different interests and cultures in order to stimulate debate. The questions asked in the focus group interviews were similar to the questions used in individual interviews to broaden the data by comparing the responses of respondents when they were on their own, to their responses in a group situation.. The data analysis process was organised according to the research questions and based on themes that emerged from the contents of the interviews. A list of themes was formed of each transcript. These themes were then grouped and organised according to similarities. The list of themes was compared to the data and codes were allocated. The data was then divided and organised into categories, relevant themes and sub-themes.
The findings of this study revealed that violence at this school was mainly interpersonal. Physical violence at this school took the form of hitting, kicking, punching, slapping and other acts that caused physical pain or injury. This study found that many learners believed that certain types of muthi provided advantage to the user in conflicts and fights. Many boys at this school used vernacular expressions of stick fighting to reinforce their dominance over other boys, as metaphors of manhood that bolstered their position among peers. Physical confrontation often…
