我々は、カルシウムフルオロアルミノシリケート粉末混濁液とリン酸水溶液の2液からなるフッ素系ナノシール剤(以下ナノシール剤)を試作し本剤の粒子沈着効果を利用した耐酸性効果を明らかにすることを目的として、人歯を用いAPF製剤との比較研究を行った。耐酸性能の評価には、QLFとFE-SEMを用い、人工齲蝕エナメル質と健全エナメル質の耐酸性能の検討を行った。1.QLFを用いた耐酸性能評価において、人工的初期齲蝕および健全エナメル質の双方でナノシール剤はコントロール群よりも高い耐酸性能を示した。2.人工的初期齲蝕におけるFE-SEM観察において、APFゲルおよびAPF液群では、薬剤塗布直後と脱灰72時間後を比較すると、一部の表面に脱灰の進行が認められた。これに対して、ナノシール剤群では、脱灰処理前後もエナメル質表層に大きな差はみられなかった。3.健全エナメル質表面に沈着したナノ粒子析出物は、FE-SEM観察においてエナメル質表層へ緊密な被覆が確認された。4.健全エナメル質において行われた、塗布・脱灰を繰り返した実験でも、ナノシール剤は安定した耐酸性能を示した。以上より、本剤は、新たな齲蝕予防剤としての使用が期待できると考えられた。(著者抄録)

下顎枝矢状分割術(SSRO)に伴う知覚障害および下歯槽神経血管束の露出と下顎管の走行との関連性について検討した。骨格性下顎前突症の診断でSSROによる下顎後退手術を施行し、術後6ヵ月以上の経過観察が可能であった36例72側を対象とした。下顎枝頬側皮質骨内面と下顎管外側壁との間の海綿骨の厚さが1.9mmより小さいと術後の下歯槽神経知覚障害のリスクが大きくなった。神経露出群では、S-Wスコアが有意に高く下歯槽神経知覚障害の症状を強く認めた。しかし、術後24週では露出群・非露出群ともにS-Wスコアの平均は6ポイント未満となり、術後の下歯槽神経知覚障害は遷延化することなく回復した。S-Wtestにて術後1週で36側が中等度知覚鈍麻〜知覚脱失を示していたが、全症例において経時的に回復傾向を示し、術後24週では6側にParesthesiaおよびHypoesthesiaを残す程度まで回復を認めた。
