1. Bardt, Janna. Die Implementierung eines Kinderpalliativzimmers auf einer Normalstation - Eine qualitative Studie.

Degree: PhD, Medizinische Fakultät, 2017, Georg-August-Universität Göttingen

In the context of an enhancement of the (medical) care structure for children receiving palliative care the Medical Care Unit at University of Göttingen has installed a special Children’s Palliative Care Room (CPCR). The room was implemented at a unit with curative purpose. This thesis investigated how employees involved in this process experienced it and what expectations they had of this CPCR. To answer the research question a qualitative study design was applied conducting 17 interviews with employees of different professions. The interviews followed a half-standardized interview procedure and were recorded and then transcribed with the help of the software “f4transkript”. The interview transcipts were analyzed according the principles of grounded theory (Glaser/Strauss; Strauss/Corbin) applying MAXQDA. As a result it can be assessed that the employees had very diverse perceptions of the implementation process of the CPCR. Being confronted with the CPCR the employees revealed the feeling of a ‘class society’ across the unit as a central phenomenon. The obvious main reason for this feeling was reported to lie in significant discrepancies regarding the equipment of the new CPC Room in comparison to the rest of the unit. This again can be understood as a reflection of the overall dissatisfaction of the staff with single aspects of the implementation process. It could be assessed that the negative atmosphere was triggered by organizational difficulties during the implementation phase as well as difficulties in defining their own tasks and responsibilities. The staff reacted distant to the ‘class society’. This reaction was influenced by the staff’s fear of being confronted with dying patients in a stronger and more psychologically stressing way. Besides, the interviewees reported a lack of integration into the implementation process of the CPC Room. Intra- and interprofessional conflicts made the cooperation within the team additionally more complicated. Keeping a distance resulted in a resignation which was strengthened in combination with unclear task descriptions and unsecure knowledge about tasks and goals of children’s palliative medicine. Despite these difficulties during the implementation process some employees saw the CPC Room as a chance to broaden their own competencies. A new professional focus could be established and although there were many challenges it was perceived as a positive complementary to their daily work. The result of this study show how important it is to take employees’ perspectives into account while planning and establishing a CPC Room at a regular care unit and the role these perspectives play for working in such a room.
