▼ Gay, bisexual, and other men who have sex with men (GBMSM) are disproportionately burdened by HIV. In sub-Saharan Africa, the prevalence of HIV among GBMSM is 2-4 times greater compared to that of the general male population. In Kenya, the Ministry of Health recognizes the importance that GBMSM play in the national HIV epidemic and there is an urgent need to expand HIV prevention and treatment interventions among this population. Evidence from observational, experimental, and modeling studies demonstrates that antiretroviral therapy (ART) used to treat HIV infection improves individual health and reduces transmission to negative sexual partners. As a result, the expansion of ART has been prioritized by national and international organizations and there has been sustained interest to implement find, test, link, and retain strategies in combination HIV prevention interventions. Gay, bisexual, and other men who have sex with men face numerous barriers that limit engagement in prevention and care services including stigma related to gay or homosexual identity, social isolation, discrimination by health care workers and medical professionals, and criminalization of same-sex behaviors. The Anza Mapema Study implemented a find, test, link, and retain model that was specifically targeted to GBMSM of Kisumu, Kenya in order to improve access to testing, increase awareness of HIV status, reduce barriers to ART, reduce ART initiation delays, and retain men in treatment and prevention services. In the studies examined here, we sought to identify the sociodemographic, behavioral, psychosocial, and clinical characteristics associated with: 1) HIV infection status; 2) viral suppression among HIV-positive GBMSM who were offered treatment and followed for 12 months; and 3) missed follow-up visits among HIV-negative participants enrolled in prevention and education services. ▼ Free-roaming dogs, Canis lupus familiaris, can be both a public health and conservation concern. Between 2010 and 2013 we identified 2,649 dogs in four rural villages in Tanzania. We characterized dog demography and ownership practices and investigated whether vaccination influences dog population dynamics. We found that adult dogs had higher survival than puppies in all villages. We observed a male-biased sex ratio across all age classes and higher adult male dog survival. Within the vaccination villages, vaccinated dogs had a decreased risk of death. However, overall mortality in one non-vaccination village was significantly higher than in the two vaccination villages and other non-vaccination village. Dogs in poor body condition had lower survival than dogs in ideal body condition in all villages. Sickness and spotted hyena, Crocuta crocuta, predation were the two main causes of dog death. Reproductive patterns were similar between vaccination and non-vaccination villages and we observed an overall male-biased litter sex ratio regardless of the mother’s body condition. Dogs were owned and used primarily for livestock and household protection. We found that dog ownership was related to livestock ownership, household size, education, and house type. Thus, the number of dogs increases with household wealth. Stable isotope analyses of dog hair confirmed survey data and indicated that dogs were fed primarily a corn-based diet similar to human diet in the villages.Free-roaming domestic dogs in rural communities exist in the context of their human owners as well as the surrounding wildlife. Our results demonstrate that vaccination alone does not impact domestic dog population dynamics and that they may be mediated by humans. Understanding the role of dogs and their care within these communities is important for planning and implement rabies control measures such as mass dog vaccination. ▼ ABSTRACT Background Male circumcision (MC) is approximately 60% effective against heterosexual HIV transmission among men. MC is being scaled-up across East and southern Africa. If men engage in riskier sex after becoming circumcised (termed “risk compensation”), the protective effect of MC could be diminished. Several governments in East and southern Africa have signaled their intention to scale-up Infant Male Circumcision (IMC) to mitigate potential risk compensation and because the procedure is simpler, safer and less expensive. Methods Data for this dissertation come from two sources. The first is the randomized controlled trial of male circumcision for HIV prevention and post-trial follow-up data collected between 2002 and 2010. The trial randomized 2,784 HIV negative men to immediate or delayed circumcision. After a median of 24 months of follow-up, uncircumcised men were offered the surgery and follow-up continued to 72-months. Generalized estimating equations models with incorporation inverse probability of treatment and censor weights were used to estimate the association between circumcision status and four time-varying measures of sexual risk-taking behavior: no condom use at last sex, sex the same day as meeting someone in the previous six months, >2 sexual partners in the previous six months and exchange of sex for gifts or money in the previous six months. The second data source is the Mtoto Msafi study and post-study routine infant circumcision service provision. The case-control study was conducted in 2010 at five government health facilities in western Kenya. Cases were mothers and fathers accepting circumcision for their son. Controls were parents who declined IMC services. A questionnaire comprising 41 questions was administered. Clinical variables relating to the circumcision procedure, including clinical provider, experience level of provider, age of the infant, and adverse event data were collected during the study and after study completion. Results In the risk compensation analysis, no condom use at last sex increased modestly over time for both circumcised and uncircumcised men (OR for 6-month increase in time 1.06). Compared to uncircumcised men, circumcised men had increased odds of no condom use at last sex (OR=1.17, p=0.006). There was no evidence of risk compensation in the other sexual behavioral outcome variables. In the infant circumcision analysis, factors associated with accepting IMC among mothers in multivariable logistic regression modeling were: father circumcised, both partners Luo (vs. father uncircumcised, both partners Luo OR=5.47, p<0.001) and agreeing with the father about the IMC decision (OR=5.00, p<0.001). Among fathers, factors associated with accepting IMC were: being circumcised and Luo (vs. uncircumcised and Luo OR=3.96, p=<0.001) and having higher endorsement of MC (OR=3.79, p<0.001).
