▼ Hypothesis: There is no difference in patient compliance regarding appointment keeping behavior, broken appliances, wearing orthodontic auxiliaries, or oral hygiene maintenance in the Medicaid and non-Medicaid orthodontic populations studied. Objective: To determine if perceived problems with the orthodontic Medicaid population are justified by examining whether patients whose orthodontic treatment is covered by Medicaid have more late and failed appointments, more broken appliances and are less compliant with orthodontic auxiliaries wear and oral hygiene maintenance than patients who are paying for orthodontic services themselves. Methods: A retrospective chart review was conducted at two sites: 1) Wicker Park Orthodontics – a private orthodontic practice in Chicago and 2) the University of Illinois at Chicago Orthodontic Clinic. Charts of 30 Medicaid and 30 non-Medicaid patients were reviewed at each site. From each chart, the following information was recorded: mean percentage of failed and late appointments, number of broken appliances, auxiliary wear, and oral hygiene maintenance. Student t-tests and Chi-square tests were performed to determine if there were any statistically significant differences in the aforementioned categories between the Medicaid and non-Medicaid orthodontic patients. Results: The student t-tests showed no statistically significant differences between the two groups with regards to the mean percentage of late and failed appointments (p = 0.107, p = 0.393). Cross-tabulation and Chi-square results also showed no statically significant differences between the two groups with regards to the number of broken appliances, auxiliary wear, and oral hygiene (p = 0.075-0.600). Conclusions: The results of this study indicate there is a lack of evidence behind orthodontists’ perceptions that Medicaid orthodontic patients are less compliant than non-Medicaid orthodontic patients. While there are still some difficulties in treating Medicaid patients, such as low fee reimbursement and cumbersome paperwork, the current study will hopefully alleviate some of the doubts practitioners may have regarding this population’s compliance making them more open to participating in the Medicaid program to help reduce the access to care issue faced by these patients. ▼ The concept of cephalometric growth prediction has been around for decades, but it has only been readily available as in-office programs in the recent past. No objective attempts have been made to validate the accuracy of the growth prediction algorithms in these programs. This retrospective study compares the predictive ability of these currently available programs to provide a measuring stick for growth prediction. Hypothesis: The three algorithms (Bolton/Ricketts in Dolphin Imaging™ and Ricketts in RMODS® software, JOE CEPH®) tested in this study provide accurate growth predictions when compared to actual observed growth of untreated orthodontic subjects. Objective: To evaluate and compare the accuracy of three computerized growth prediction algorithms based on lateral cephalograms. Methods: Longitudinal sets of cephalograms from 56 untreated caucasian subjects were studied. Each subject had three cephalograms, with the first time point (T1) taken between the ages of 8 and 11 and subsequent cephalograms taken 2 years (T2) and 4 years (T3) after T1, that were all traced using Dolphin Imaging™. Two-year and four-year growth predictions were then performed using the Ricketts growth prediction algorithm (Alg 1) and the Bolton growth prediction algorithm in Dolphin Imaging™ (Alg 2) and the Ricketts growth prediction algorithm in RMODS® software, JOE CEPH® (Alg 3). The predictions were superimposed with their corresponding tracing of actual growth. Statistics were performed to determine the accuracy of the predictions as compared to actual growth using a clinical reference mean of 1.5 mm and to compare the algorithms to each other. Results: Alg 1 predicted 17 of 18 landmarks for the 2-year predictions for males, 18 of 18 landmarks for the 2-year predictions for females and 14 of 18 for the 4-year predictions for both males and females. Alg 2 predicted 18 of 18 landmarks for all 2-year predictions. For the 4-year predictions, Alg 2 predicted 15 of 18 and 12 of 18 using chronologic age for males and females, respectively. It also predicted 13 of 18 and 16 of 18 using skeletal age for males and females, respectively. Alg 3 predicted 18 of 18 landmarks for all the 2-year predictions. Alg 1 was the most different and Alg 2 and Alg 3 showed to be more similar. Conclusion: Overall, the three growth prediction algorithms tested provided accurate growth predictions when compared to observed growth of untreated orthodontic subjects. ▼ Hypothesis: We hypothesize that there is no mean difference in gingival buccal attachment levels between pre and post-treatment for each of the non-surgically expanded adults and non-expanded adult groups. We also hypothesize that there is no mean difference between non-surgically expanded adults and non-expanded adults for all variables measured in this study: clinical crown height, transarch width, dental angulation, and palatal vault angle. Objective: To evaluate nonsurgical adult maxillary expansion and assess the gingival buccal attachment levels pre and post-treatment in adult patients with constricted arches where the maxillary arch was expanded. Methods: Initial and final study models were evaluated for 26 adults treated with nonsurgical maxillary expansion and compared with 31 adult orthodontic patients who did not require expansion. Study models were scanned and digitized with the Lythos Digital Impression System (Ormco). Linear measurements were completed in Ortho Insight 3D (clinical crown height and transarch width). Angular measurements were completed in Dolphin Imaging (dental angulation and palatal vault angle). Results: Patients who underwent nonsurgical adult expansion showed a mean increase in transarch width for first premolars (3.2+/-1.5mm), second premolars (3.1+/-1.0mm), and first molars (2.5+/-1.4mm). The expansion group showed a mean increase in clinical crown height of the right first premolar (0.5 +/- 0.7mm), right second premolar (0.4 +/- 0.6mm), left first premolar (0.4+/- 0.7mm), left second premolar (0.4 +/- 0.3mm), and left first molar (0.2 +/- 0.6mm). Conclusions: There was a mean difference in gingival buccal attachment levels post-treatment for each of the non-surgically expanded adults and non-expanded adult groups. There was a statistically significant increase between non-surgically expanded adults and non-expanded adults for clinical crown height, transarch width and dental angulation, especially in premolar areas. ▼ Hypothesis: We hypothesize that there is no statistical significant mean difference nor association of the vertical location and sagittal dimension between the minimum cross sectional area (MCSA) of airway in CBCT scans and the minimum sagittal linear dimension (MSLD) of airway in 2D reconstructed lateral cephalograms (RLC) in an orthodontic population. Objective: To investigate the mean difference and association of the vertical location and sagittal dimension between MCSA of airway in CBCT scans and MSLD of airway in 2D RLC. Methods: CBCT-scans of 91 patients, grouped into three age groups (≤20, 21-40 and >40 years), were utilized to perform a 3D assessment of the upper airway and to reconstruct lateral and postero- anterior cephalograms (PAC). Airway volume, MCSA, vertical level and sagittal dimension of MCSA in CBCT and MSLD of airway in 2D RLC as well as linear transverse measurements in 2D reconstructed PAC were computed and analyzed. Results: In all age groups, there was no significant mean difference between the vertical location of MSCA in CBCT and MSLD in 2D RLC. However, there was a significant mean difference between the sagittal dimension of MCSA in CBCT and MSLD in 2D RLC (p< 0.05). In all age groups, correlation tests showed moderate-strong significant correlation between the vertical location of MSCA in CBCT and MSLD in 2D RLC (0.611 ≥ r ≤ 0.727). Pearson correlation test showed strong significant correlation between the sagittal dimension of MSCA in CBCT and MSLD in 2D RLC (r = 0.873) in the youngest age group. Conclusion: Two dimensional reconstructed lateral cephalograms may be used as screening tool to identify the vertical location of minimum sagittal measurement of oropharyngeal airway. However, comprehensive assessment of airway characteristics is better achieved with CBCT-based 3D evaluation. Funding: UIC College of Dentistry, Department of Orthodontics. ▼ Hypothesis: Effects of unilateral maxillary premolar extraction treatment on dental arch form and midline deviation do not differ significantly from bilateral maxillary premolar extraction treatment. Distances of corresponding points between right and left sides of the dental arch are approximately the same within each group. Objective: A retrospective study evaluating post-treatment maxillary arch forms and midlines for asymmetry in Class II subdivisions treated with unilateral and bilateral maxillary premolar extractions. Methods: Using GeoMagicTM and MATLABTM softwares, best fit curves expressed as quadratic polynomials were generated from post-treatment digital models of fifteen Class II subdivisions treated with unilateral and twenty with bilateral maxillary premolar extractions. Transverse and sagittal measurements were recorded to analyze asymmetry in arch form. Midline deviation relative to the mid-palatal raphe was calculated. Dental models were superimposed on constructed reference planes to generate average post-treatment arches. Statistical comparisons were performed with significance level set at p ≤ 0.05. Results: Within the unilateral extraction group, anterior, anterior-middle and middle segments showed significant differences in arch form between right and left sides transversely, and all segments of the arch aside from the posterior segment in the sagittal dimension. Bilateral premolar extraction treatment did not show significant differences in arch form between right and left sides in either dimension. Significant differences were found between the groups in anterior and anterior-middle segments of the arch transversely, and in the middle and middle-posterior segments sagittally. A significant difference in midline deviation relative to the mid-sagittal plane was observed between the groups. Superimposed average arches showed similar results. Conclusions: Unilateral maxillary extraction treatment generally results in a narrower and more posteriorly displaced arch form on the extraction side, with a deviated maxillary midline from the mid-palatal raphe towards the extraction side of the arch. ▼ Introduction: The purpose of this randomized controlled trial was to evaluate the effect of AcceleDent Aura device on maxillary and mandibular arch alignment and pain during orthodontic treatment with SureSmile. Methods: Seventy-one subjects undergoing non-extraction comprehensive orthodontic treatment with SureSmile were randomly assigned into experimental group that used AcceleDent device and control group that used no additional device. Little’s irregularity index was used to evaluate maxillary and mandibular anterior teeth alignment at four time points – initial, 3 months, 6 months, and 12 month or end of treatment. Pain intensity and compliance with AcceleDent were recorded. Results: Out of total initial sample, data from sixty-one subjects were analyzed. There were no statistical significant differences in the initial upper and lower arches irregularity indices between the two groups, p-value > 0.05. At all follow up time points, the mean irregularity indices were consecutively smaller for both upper and lower arches. Within each group these differences were statistically significant with p-value <0.001. However, the mean differences in irregularity index were not statistically significant between the experimental and control groups at any time points. There was no difference in pain levels between the two groups during initial 3 months of the treatment for the 3 days evaluated; however, pain was significantly lower in the experimental group from the third month until the end of the treatment, p-value = 0.006. Average compliance with AcceleDent use was 78%. Conclusion: The AcceleDent Aura device had no effect on arch alignment. Likewise, device did not have any effect on pain level during initial three months. After 3 months until the end of the treatment, AcceleDent group had significantly less pain compared to the control group during the initial 3 days post-adjustment appointments. ▼ Dental Monitoring™ is a software designed to assist orthodontists in the remote monitoring of orthodontic patients. Dental Monitoring™ software allows patients to take pictures of their teeth through the use of their smartphone. These images are then uploaded to the Dental Monitoring™ platform where a patented algorithm calculates and tracks the current movement of the teeth. This software has the potential for orthodontic patients to have fewer appointments and have better communication with their orthodontist. Despite the potential that this platform can offer, currently there are no studies to validate this technology. To validate this technology, 10 wax typodonts were initially scanned with an iTero® Element™ to produce an initial 3D model that Dental Monitoring™ then used as a baseline for their calculations. Invisalign® trays were then created for each of the 10 wax typodonts; each of the 10 wax typodonts had a series of 10 Invisalign® trays. Once an Invisalign® tray was placed, the typodont was placed in hot water bath set at 45o Celsius for five minutes. After the wax was cooled and the Invisalign® tray was removed, a Dental Monitoring™ photo exam and iTero® scan was taken. The 3D STL file produced from Dental Monitoring™ and the 3D STL file from the iTero® Element™ were then superimposed in GOM™ 3D software to calculate a global deviation from approximately 200,000 surface points between the two models. Results show that there was no clinically significant difference between the reconstructed digital dental models generated by the iTero® Element™ intraoral scanner and reconstructed digital dental models generated by the Dental Monitoring™ application in photo mode. Clinical significance was determined to be 0.5 mm based on the American Board of Orthodontics (ABO) standards of clinical significance. Results did demonstrate a statistically significant difference in global deviations over successive Dental Monitoring™ photo exams. Reconstructed digital dental models produced from Dental Monitoring™ in photo mode did progressively increase in global deviations, therefore becoming less accurate over time. These deviations were not clinically significant and never exceeded the threshold of 0.5 mm. ▼ Introduction: The purpose of this study was to evaluate the association of various linear and angular parameters of subjects’ mandibles with different craniofacial skeletal patterns, utilizing 3D CBCT images from three private practices in CO and IL, USA. Methods: Pre-treatment 3D CBCT images from 331 non-growing subjects were used. Two-dimensional cephalometric images generated by Dolphin Imaging Software were utilized for categorizing the samples. The samples were categorized into Class I, Class II, and Class III skeletal types; and subcategorized into hyperdivergent, normodivergent, and hypodivergent vertical growth patterns. Dimensions of linear and angular variables from 3D images i.e. ramus height, ramus breadth, condylar length, coronoid process length, anterior mandible height, and convergence angles of the ramus and of the mandibular base, amongst the skeletal categories were evaluated. ANOVA and Student's t-test statistical analyses were conducted to determine differences of the outlined variables amongst the skeletal types, and Pearson correlation was performed to evaluate the correlation of each variable. Results: There was a significant difference in ramus height observed between Class I and III skeletal types, and between males and females. Significant difference in anterior mandibular height was observed between males and females. There were statistically significant mean differences of the studied variables of the mandibles, among the vertical patterns in all skeletal groups. Positive correlation of various parameters of the mandibles was observed most in Class I/normodivergent subjects. Conclusions: This is the first study to compare linear and angular variables of the mandibles in all skeletal types, using 3D CBCT images. This study provides a preliminary 3D norm for all skeletal types and genders. Ramus height can be used as an indicator for gender differentiation. In orthodontics, practitioners commonly use panoramic radiographs to assess external orthodontically induced apical root resorption. Unfortunately, using panoramic radiographs for this purpose may not lead to reliable and accurate conclusions due to multiple drawbacks of the technique. This research focuses on assessment of crown/root ratio measurements of teeth from dried skulls and the possible effect of patient positioning in the panoramic machine on the measurements. We also evaluated the accuracy of the crown/root ratio of incisors and canines measured on panoramic radiographs at ideal head position compared to the same measurements performed on cone beam computer tomographs (CBCT). The sample consisted of 24 dried skulls. Panoramic and CBCT radiographs were taken. Panoramic radiographs were taken under 7 different head positions. Baseline head position (0 ) was established when the horizontal laser orientation line passed through Porion-Orbitale. The skull was then tilted +5o, +10o, +15o upward and -5o, -10, -15o downward. Crown/root ratios of all teeth were measured on panoramic radiographs. Only incisors and canines were measured on the CBCT. We found upward/downward head tilt to significantly affect crown/root ratio measurements of all groups of teeth except lower incisors. Maxillary teeth were more sensitive to upward head tilt, while downward head tilt was more forgiving, affecting only molars. There were no differences in crown/root ratio measurements of lower incisors and canines between panoramic radiographs at ideal head position and CBCT scans. However, there were differences in crown/root ratio measurements of upper incisors and canines between panoramic radiographs at ideal head position and CBCT scans. ▼ Hypothesis: There is no significant difference in cephalometric measurements when the skull is rotated at different degrees along the sagittal axis vs when the skull is oriented ideally at zero degrees. Objective: Identify potential measurement errors on lateral cephalometric radiographs due to head rotation along the sagittal axis and assess for variability between different degrees of rotation. Methods: Existing iCAT CBCT images of 24 de-identified dry human skulls were used in this study. Lateral cephalometric images were extracted from CBCT images at 11 different skull positions, 0° to ±15°, in 3° intervals, using prospective projection. Lateral cephalograms were traced and analyzed for variability between different angles of rotation. Results: The following rotations from zero had statistically significant mean differences: +3°-SNB; +6°-SNB, NMe; +9°-SNA, SNB, ArGoMe, NMe, SGo; +12°-SNA, SNB, ArGoMe, SNMP, NMe; +15°-SNA, SNB, ANB, FMA, U1SN, SGn-FH ArGoMe, SNMP, GoMe, NMe, SGo; -3°-SNA, SNB; -6°-SNB, FMA, U1SN, NMe; -9°-SNB, FMA, U1SN, SN, NMe, SGo; -12°-FMA, SGnFH, SNMP, SN, GoMe, NMe, SGo; -15°-FMA, SGnFH, SNMP, GoMe, ANSPNS, NMe, SGo. SNB showed statistically significant mean differences in each positive distance, p<0.05. Linear horizontal measurements SN, ANSPNS, and angular measurements NAApo, L1MP and U1PP, did not show statistically significant mean differences in any of the positive distances, p>0.05. Angular measurements ANB, NAApo, ArGoMe, L1MP, and U1PP did not indicate statistically significant mean differences in any of the negative distances, p>0.05. Angular variables NAApo, L1MP, and U1PP did not indicate statistically significant mean differences in any of the distances from zero, p> 0.05. ±3° showed statistically significant mean differences, p<0.05, only on the angular variables SNB (positive and negative) and SNA (negative). Conclusions: Cephalometric measurement errors increased as skull rotation increased from 0° to ±15°. NAApo, L1MP, and U1PP were the most reliable measurements and had minimal change. There is no mean difference in study variables between surgical and non-surgical subjects based on Pediatric Sleep Questionnaire (PSQ) and Sleep-Related Breathing Disorders (SRBD) risk. 2. There is no relationship between cephalometric variables and response to rapid maxillary expansion (RME) as measured by change in PSQ scores. Objective: To investigate the difference in study variables between surgical and non-surgical pediatric subjects with SRBD after RME treatment. Methods: Subjects were referred to an orthodontist by ENT physicians due to airway/sleep-related symptoms and treated with RME. Lateral cephalograms were taken at baseline. PSQs were administered to caregivers at baseline and again after RME removal to measure subjects’ response to treatment. Subjects were divided into two groups: surgical - had tonsillectomy and adenoidectomy (T&A) performed (n=15), and non-surgical - did not have T&A performed (n=43). The subjects were further classified into subgroups positive responders and non-responders based on PSQ scores and the percentage change in the score from baseline. Results: Independent Student’s t-tests indicated statistically significant mean differences in the cephalometric variables ANB(°), PAS(mm), and PNS-ad2(mm) between the two study groups non-surgical positive responders (n=37) and surgical positive responders (n=13). Statistically significant mean differences were also found in the variables facial axis(°) and PAS(mm) between high-risk non-surgical positive responders (n=14) and high-risk surgical positive responders (n=9). The Pearson correlation test did not show a statistically significant correlation between any of the cephalometric variables and the percentage change in the total PSQ-SRBD score from baseline. Conclusions: Positive responders who had undergone T&A had higher mean ANB, lower mean PAS, and higher mean PNS-ad2 measurements. For SRBD high-risk positive responders, those who had undergone T&A had lower mean facial axis and PAS measurements. Baseline cephalometric variables may be employed as a screening tool in determining response to RME. ▼ Cone Beam Computed Tomography (CBCT) has become an increasingly important tool in treatment planning and diagnosis in orthodontics. The popularity of CBCT has increased tremendously among dentists and orthodontists. Unfortunately, the increase in the ionizing radiation and reports in the lay press about radiation overdose with CT scanners created arguments against CBCT as the imaging technique of choice for comprehensive orthodontic assessment. The purpose of this feasibility study is to find a threshold number of projections produced by the Adaptive-Steepest-Descent-Projection-Onto-Convex-Sets (ASD-POCS) algorithm from the original iCAT 3D CBCT to be used reliably for 2D evaluation of orthodontic diagnostic records. Virtual 3D digital images of craniofacial structures were made by the iCAT CBCT machine (Imaging Sciences International, Hatfield, Pa) of four subjects. The ASD-POCS algorithm was used to derive different compression rates by reconstructing the 3D digital images using lower number of projections. Five different compression rates were compared (320iCAT, 300 UC, 151 UC, 76 UC, and 39 UC). Dolphin 3D (Version 11.7, Dolphin Imaging, Chartsworth, CA) was used to project the 3D images of different compression rates into lateral and postero-anterior (PA) cephalograms. The accuracy of landmark identification in cepahlograms derived from different compression rates was used to evaluate the quality of compressions. The preliminary results of this feasibility study based on the small sample (n=4), showed that there is no mean difference of error distances between a set of landmark locations in both lateral and PA cephalometric radiographs derived from CBCT images at different compression rates. None of the means of error distances of landmark locations in different compressions rates exceeded 1.5 mm error margin for both lateral and PA cephalometric radiographs. ▼ A study was performed to determine the use of a specific reconstruction algorithm (RA), adaptive-steepest-descent-projection-onto-convex-sets (ASD-POCS) on the image quality of orthodontic radiographic images constructed from two cone beam computed tomography (CBCT) scans. Four different image reconstructions were prepared using ASD-POCS. Reconstructed images varied among each other in the number of basis images. These were RA 300, same as the i-CAT image (control) and reducing the number of basis images to RA 150, RA 79, and RA 33. All the CBCT images with different numbers of basis images were used to reconstruct the multiplanar images of the panoramic view, lateral cephalogram, and posterior-anterior cephalogram manually using Dolphin 3D software. Seventy three examiners evaluated the reconstructed 2D radiographs (panoramic, lateral, and posterior-anterior cephalogram) for the purpose of general screening, diagnosis, and treatment planning from the reconstructed CBCT images. The examiners were orthodontists, orthodontic residents, oral surgeons, oral surgery residents and oral and maxillofacial radiologists. Using the preferences (Yes/No) and 10 point rating scale the examiners evaluated if the images were diagnostic for screening, diagnosis, and treatment planning by identification of specified landmarks. Non-parametric tests were used to determine statistical significant differences. Preferences of all the images among participants were statistically significant (p<0.05). Mostly, for all 2D reconstructed images, RA 300 was preferred over i-CAT (control) and had the highest mean rank. For the panoramic radiograph and posterior-anterior radiograph, it was found that the quality of the images with the full number of basis images RA 300 and RA 150 were not significantly different from the i-CAT reconstructed images. For the lateral cephalogram, it was found that the images generated from RA 300, RA 150, and RA 79 number of basis images were not significantly different, resulting in usable images with up to one half and one fourth reduction in number of projections. This shows that reconstruction algorithm ASD-POCS could be used to reduce the number of basis images while maintaining the image quality for the purpose of 2D orthodontic records. ▼ Hypothesis: There is no statistically significant mean difference in vertical dimension changes between the quad helix expander and the bonded rapid palatal expander during phase I treatment for growing skeletal class I and class II subjects. Objective: The objective of this study was to determine if there is a significantly different effect on vertical changes during palatal expansion in phase I treatment using a quad helix expander or a bonded rapid palatal expander in growing skeletal class I and class II subjects. Methods: This retrospective study looked at two treatment groups, a quad helix expander group and a bonded rapid palatal expander group, before treatment (T1) and after the completion of phase I treatment (T2) to assess changes in the vertical dimension after palatal expansion treatment. Each treatment group was also compared to an untreated predicted growth model. All subjects were growing patients with either a class I or class II skeletal pattern. Lateral cephalograms taken before treatment and after the completion of expansion and phase I treatment were traced using Dolphin Imaging, and these cephalometric tracings were used to analyze the changes in vertical dimension. Results: When the quad helix and bonded rapid palatal expander groups were compared to each other, no difference was found at T1, but significant differences at T2 were found for the variables convexity, lower facial height, total facial height, facial axis, and FMA. When the two treatment groups at T2 were compared to their respective untreated predicted growth models, a significant difference was found for the variable lower facial height for the quad helix group and for the variable U6-PP for the bonded expander group. Conclusions: The differences found at T2 between the two groups suggested that the quad helix expander had more control over skeletal vertical measurements than the bonded rapid palatal expander, based on this sample. When comparing treatment results to untreated predicted growth values, the quad helix expander appeared to maintain lower facial height better than the bonded rapid palatal expander, and the bonded rapid palatal expander appeared to maintain the maxillary first molar vertical height better than the quad helix expander. Overall, both the quad helix expander and the bonded rapid palatal expander showed minimal vertical changes during palatal expansion treatment. ▼ Objective: To investigate the three dimensional outcome root inclination (facio-lingual torque) and angulation (mesio-distal tip) of SureSmileTM (SS) treated cases. Hypothesis: The mean tip and torque discrepancy of the of the SureSmileTM target model (treatment simulation) and the final CBCT (outcome) is not different than the mean tip and torque discrepancy of the initial CBCT and corresponding SureSmileTM therapeutic model (initial model). Methods: Initial SS CBCT (therapeutic or initial CBCT), SS therapeutic model (initial model), post-treatment CBCT (outcome or final CBCT), and SS target model (plan or simulation) were collected for 40 consecutively finished SS cases. DolphinTM 3D root anaylsis software was used to measure the tip and torque values for SS target model and post-treatment CBCT of 30 randomly selected cases. Discrepancies between these variables were compared against the mean discrepancies of the initial CBCT and initial model for 10 randomly selected cases, which was a baseline for expected mean discrepancy of like samples. One sample t-tests were conducted to assess if a difference between tooth tip and torque outcome vs. plan discrepancies were different that found for initial model vs. initial CBCT discrepancies, overall and for each tooth type (i.e. maxillary central incisor, maxillary lateral incisor, etc.). Results: Tests of the final discrepancy to the baseline initial discrepancy were found to be statistically significant for both tip and torque overall, and for many tooth types. Conclusion: Although statically different, the overall mean discrepancy of the SS target models to the outcome CBCT are within clinically acceptable range (±2.5˚). Mean outcome discrepancies were also found to be statistically significant for several tooth types, but clinical significance (beyond ±2.5˚) was limited to the maxillary and mandibular second molars for tip, and the maxillary second molar and mandibular central and lateral for torque. Overall, tip outcomes were closer to the plan than torque outcomes and for most tooth types. The mandibular arch had more teeth with mean discrepancies of statistical significance, fewer outcomes within 2.5˚ of the plan, and had greater overall discrepancy from the plan. Further investigation is necessary to better delineate the cause of these findings. ▼ Hypothesis: Oropharyngeal 2D and 3D airway variables will be associated with MAS treatment response in terms of change in respiratory disturbance index (RDI) and change in minimum oxygen desaturation (SaO2). The association between oropharyngeal 2D and 3D airway variables with MAS treatment response will depend on sleep position, sleep state, mandibular protrusion, initial OSA severity, BMI, and neck circumference. Objective: To identify the association between oropharyngeal airway dimensions derived from cone beam computed tomography (CBCT) and MAS treatment response, and to determine whether it is affected by mandibular protrusion, sleep state, sleep position, initial OSA severity, BMI, and neck circumference. Methods: Thirty-three adults were assessed retrospectively. Dolphin 3D was used to measure the airway in pretreatment CBCTs. Airway volume, length, and minimal cross-sectional area (CSA) were calculated, as well as transverse and A-P dimensions and minimal CSA location. Pre- and post-treatment polysomnograms (PSGs) assessed OSA severity via RDI, and changes in minimal SaO2, supine and non-supine RDI, and NREM and REM RDI. Results: This study included 23 males and 10 females. Ten, fifteen, and eight initially presented with mild, moderate, and severe OSA, respectively. Oropharyngeal 2D and 3D airway variables were associated with treatment response. Multivariate models explained treatment response, wherein initial OSA severity was a primary predictor in four models, and the combination of total airway volume and initial BMI were predictors in two models. Conclusions: Patients with higher initial OSA severity and smaller airway volumes may have increased response to MAS therapy. Decreases in airway volume due to skeletal rather than soft tissue obstruction may enable better MAS treatment response. Since MAS targets upper airway, patients with superior airway constriction illustrate increased treatment response potential as well as decreased titration to achieve desirable outcome. ▼ In this retrospective, cross-sectional study, our objective was to use a previously published cephalometric method to measure the maxillary and mandibular molar and premolar dentoalveolar heights of orthodontically untreated male and female subjects past their peak growth spurt as determined by CVMS stage V. Methods: A total sample size of 218 subjects with full permanent dentition that met defined inclusion and exclusion criteria, and grouping criteria were analyzed. Approximately 50% of the sample was obtained from the University of Illinois-Chicago Department of Orthodontics, and 50% from private offices. The sample was diverse in ethnic backgrounds and representative of an urban practice setting, with ages ranging from 13-56 years old. Using cephalometric gropuing criteria, subjects were then assigned to one of the following groups: Class I normodivergent, Class II hyperdivergent or hypodivergent, or Class III hyperdivergent or hypodivergent. Cephalometric images were traced using Dolphin Imaging (v11.7) and data was analyzed using independent t-tests, one-way ANOVA, and Bonferroni post-hoc tests (p<0.05), using SPSS statistical software (v22.0). Results: Several statistically significant differences in mean dentoalveolar height measurements were found when comparing Class I subjects to the other groups, and when comparing hyperdivergent groups to hypodivergent groups, and when comparing Class II groups to Class III groups. Statistically significant differences ranged from 1.11mm-4.55mm. Gender dimorphism was also demonstrated, both in mean differences in dentoalveolar heights, and in the magnitude of differences between groups of the same gender. Because of the gender dimorphism, all groups were also separated by gender. Conclusions: The comparisons in this study showed strong trends of short maxillary posterior dentoalveolar heights in Class II hypodivergent subjects, and short mandibular posterior dentoalveolar heights of Class III hyperdivergent subjects.
