▼ In this paper, we discuss the development of an adverse events (AEs) questionnaire for the registration and documentation of adverse events (AEs) in a clinical trial with nonsteroidal anti-inflammatory drugs (NSAIDs) for the treatment of pain in canine cancer patients. The aim of the research project is to assess the usefulness of robenacoxib in comparison to carprofen in the palliative treatment of canine cancer patients. The assessment of the treatment efficacy is done by scoring quality of life (QoL), pain and AEs. In literature, QoL scoring systems en pain scoring systems can be easily found (Lynch, 2010) (Iliopoulou, 2013) (Lavan, 2013). But in most clinical trials, AEs are recorded spontaneously or by interviewing owners (Edamura, 2012) (Vial, 2012) (Flor, 2013). We developed an AE questionnaire owners can fill out at home every week during the treatment period. The research question of this thesis is whether we succeeded in developing an AE questionnaire for the clinical trial with NSAIDs. During the development of the AE questionnaire another research question emerged. Namely, if the AE questionnaire might also be useful for owners with canine cancer patients undergoing chemotherapy at the Clinic of Companion Animals at the University of
Utrecht.
In order to create the AE questionnaire, a selection of expected AEs in treatment with NSAIDs was made. This selection is based upon the mechanisms of action of NSAIDs and the known AEs reported by the European Medicines Agency (European Medicines Agency, 2011). We adapted the VCOG CTCAE v1.1, a grading system for AEs observed during chemotherapy in cats and dogs (Veterinary Cooperative Oncology Group, 2011). After translation and a first review, the developed questionnaire was tested twice. The first time its adequacy was tested in a group of canine patients without AEs. The second time its adequacy was tested in a group of canine cancer patients treated with chemotherapy. Based upon the first test, the final version of the AE was formed. Finally, owners of the second test group were asked for feedback on the AE questionnaire. In both tests, we find that owners think that the AE questionnaire we developed is clear and easy to fill out. The results of the survey reported that owners do not think that filling out the AE questionnaire is time consuming. But only 50% (4/8) of the owners think the AE questionnaire has added value.
In this study, we find that the AE questionnaire is adequate for the use in the research project with NSAIDs. However, we believe that the AE questionnaire should be adapted before it can be used in a clinical setting with canine cancer patients undergoing chemotherapy.

▼ The NMDA antagonist Perzinfotel was tested for anaesthetic sparing properties in telemetered dogs and cats when used in a pre-anaesthetic protocol. In dogs, IV, SQ and IM administration of perzinfotel (10-30 mg/kg) decreased the mean isoflurane MAC values by 32 – 44%. The greatest MAC reduction (59%) was observed with a combination of 20 mg/kg perzinfotel and 0.2 mg/kg butorphanol. Thus, all doses of perzinfotel resulted in a significant reduction of MAC. A dose of 30 mg/kg resulted in significantly higher HR and DAP. Administration of perzinfotel as a pre-anesthetic treatment prior to isoflurane anesthesia improves anesthetic safety by reducing inhalant anesthetic requirements. In cats, IV, IM and SQ administration of perzinfotel (2.5 – 15 mg/kg) significantly decreased mean isoflurane MAC by 43.3 – 68.0%. HR tended to be lower (not significant) and blood pressure was higher compared to placebo. Perzinfotel decreased isoflurane MAC and increased BIS and blood pressure values in anesthetized cats. The administration of perzinfotel as pre-anesthetic medication prior to isoflurane anesthesia should improve anesthetic safety by reducing inhalant anesthetic requirements and improving cardiovascular function during anesthesia. To compare implanted telemetry (DSI) with semi-implanted telemetry (VAP) and a tail cuff, cats were implanted with telemeters and a VAP device. At 4 different time points and under normo- and hypertension, blood pressure was measured with the implanted devices and with NIBP. When comparing VAP and NIPB with DSI, VAP had a correlation coefficient (R2) between 0.8487 and 0.9972 while NIPB had a R2 between 0.7478 and 0.9689. The analgesic properties of perzinfotel, PLA-695 and Carprofen were compared in an induced synovitis pain model in dogs. Compared to baseline values, peak vertical force (PVF) and vertical impulse (VI) for both the negative control and perzinfotel groups had significantly lower forces at 2 and 4 hours. No differences from baseline values for PVF and VI were seen in the PLA-695 or carprofen groups at any measurement time points. Between group comparisons found significantly higher PVF and VI values in the carprofen group vs. the no treatment and perzinfotel groups at 2 and 4 hours. The PLA-695 group had higher impulse values vs. no treatment at 2 hours. The last study investigated the effects on cardiovascular parameters, if any, of a commercially available combination of metaflumizone and amitraz administered to healthy, telemetered beagles that were subsequently sedated with dexmedetomidine. Dogs were sedated first without any pre-treatment and then after pre-treatment with metaflumizone and amitraz. Baseline values of all parameters were within normal limits for all dogs before the first anesthetic event. At 10 and 20 minutes after onset of sedation, oxygen saturation as measured by pulse oxymetry was significantly higher for dogs that were pre-treated with metaflumizone and amitraz. ▼ Objective—To evaluate tissue reaction to Vicryl Plus in cats
Study Design—Double-blinded, prospective, randomized clinical trial.
Animals—Cats (n=22) who were submitted for elective ovariectomy
Methods—Elective ovariectomy was performed by one single surgeon via a standardized abdominal incision, which was sutured in 3 layers with either Vicryl or Vicryl Plus. Wounds were evaluated during recovery and at four time intervals for pain, subdermal swelling of the wound edges, redness, crusting and subdermal thickness by one single investigator.
Results—Nineteen animals met the inclusion criteria. In eight cats Vicryl was used and in eleven cats Vicryl Plus. There was more subdermal swelling in the Vicryl group after 8 to 9 days and after 16 to 17 days (p<0.05). There was no significant difference in the other parameters.
Conclusion—The addition of triclosan to the coating of Vicryl may have a reducing effect in tissue reaction to suture material in wound healing process in cats.
Clinical Relevance—Less tissue reaction may possibly occur in cats after wound closure with Vicryl Plus compared to closure with Vicryl. Further research is warranted.

▼ It was hypothesized that the fiber-guided Nd:YAG laser with its incisional and hemostatic action, could be useful for meticulous tissue dissection in a subcapsular partial prostatectomy in dogs, and in laparoscopic surgery of dogs and cats. Prior to clinical use, its action was investigated in vitro. Spatial distribution of Nd:YAG laser-induced temperature distribution was investigated in a polyacrylamide gel using a dynamic color schlieren technique, comparing contact and free beam laser exposure. Short-term (up to 20s), laser-induced, thermal damage was investigated in ex vivo canine prostate tissue. Tissue temperature was measured using thermocouple probes. Prediction of thermal damage using Arrhenius theory was discussed and compared to the in vitro damage threshold, determined by histological evaluation. Contact mode laser application caused vaporization of tissue. Mean extend of tissue damage underneath the vaporization crater floor was 4 mm coagulation necrosis. The tissue damage results were comparable to the schlieren temperature distributions, indicating that the schlieren technique can be used to model laser-tissue interaction. The laser was used in subcapsular partial prostatectomy in 4 mongrel dogs and in 8 dogs with prostate carcinoma (PCA). The urethra and dorsal capsule were spared and urinary continence was maintained postoperatively. It was concluded that laser assisted subcapsular partial prostatectomy is a potential palliative treatment for PCA in dogs and that the Nd:YAG laser is capable of precise prostate tissue dissection with acceptable thermal damage margins, comparable to the in vitro study. The Nd:YAG laser was compared to bipolar electrocoagulation (BEC) forceps in laparoscopic ovariectomy (lapOVE) in dogs and cats. The Remorgida forceps, combining BEC and sharp incision in one instrument, was compared to laser surgery in lapOVE in dogs. Laparoscopic laser ovariectomy was useful but did not have a significant advantage over BEC-OVE. The main reason for this is that blood vessels of >2 mm in diameter can not be properly coagulated with the laser, and BEC forceps were still needed to prevent mesovarial hemorrhage. The Remorgida forceps reduced surgical time, was not associated with an increased complication rate, produced less fume, and was used as a stand-alone device in ovariectomy. Under certain circumstances (pedicles containing much adipose tissue) the laser performed better than Remorgida. Laser surgery took more time (~2 min) than bipolar electrosurgery in lapOVE. Obesity significantly increased surgery duration in dogs, but not in cats because of the small amount of adipose tissue in the ovarian ligament of cats. No major difficulties or complications were encountered during or after surgery. In cats, low-pressure pneumoperitoneum, easy manipulation of the ovaries and a minimal amount of fat in the ovarian ligament facilitated excellent surgical access and outcome. ▼ The dog is one of the few species to develop spontaneous prostate carcinoma (PCA) and is thus an attractive model for the study of the disease in humans. Many of the features of the disease in the dog are similar to its human counterpart, however a number of aspects of the pathogenesis, diagnosis and management of the disease are different between dogs and people. The first aim of the thesis was to investigate aspects of the pathogenesis of PCA in dogs. This thesis presents some similarities and some differences between dogs and humans in the pathogenesis of PCA. The expression of COX-2 is not related to inflammation or to the degree of differentiation in canine PCA as it is in humans. However, similar to human PCA, androgen ablation is associated with increased inflammation in the neoplastic tissue. Like humans, dogs with PCA have a shorter CAG-I repeat in the AR gene, but variations in length are much smaller than in humans. Differences in CAG repeat length and plasma androgens that may explain ethnic differences in the prevalence of PCA in humans cannot explain the predisposition of the Bouvier des Flandres to develop PCA. There is growing evidence of a potential protective effect of androgens in the development of PCA in dogs. The second aim of this thesis was to develop therapeutic tools for the management of PCA in dogs. Treatment of canine PCA remains palliative as it is effective in relieving clinical signs but cannot offer a cure. This thesis introduces an effective surgical technique for short-term management of PCA in dogs, provided there is no urethral obstruction. Photodynamic therapy is a promising treatment modality for cancer in general and prostate carcinoma in particular, however technical improvements to allow more efficient and even administration of light to prostatic tissue are necessary to make it useful in the management of clinical cases. Additional research needs to be performed to develop new techniques to remove canine PCA without recurrence or detrimental side effects. Total prostatectomy techniques, while preserving continence in the dog, partial prostatectomy with effective adjunctive radio-, chemo- or immunotherapy, or local minimally invasive interventional techniques can be mentioned as examples.

▼ Insulinoma (INS), which causes clinical signs associated with hypoglycaemia, is the most common pancreatic neuroendocrine tumour (pNET) of dogs and humans. Ten tot fifteen percent of human INS metastasise to regional lymph nodes and the liver, and these are referred to as ‘malignant INS’. Surgical excision of malignant INS is rarely complete and malignant INS generally respond poorly to traditional chemotherapeutic agent regimens. Therefore, recurrence is likely, leading to decreased survival times in affected individuals. In the dog, the biological course of INS resembles that of malignant INS in humans, since >95% of these canine tumours metastasise. In the dog, as in humans, surgery along with optional post-operative medical therapy, remains the recommended approach when possible. However, the prognosis for canine INS is still poor, because primary INS is frequently inoperable and/or micrometastases are missed during surgery. Therefore, novel approaches are needed to improve diagnosis, therapy and prognosis for both canine and human INS. Comparative oncology aims to utilise spontaneous tumours in pet animals as sophisticated models for the study of human cancer biology and therapy. Since the release of the canine genome in 2005, cancer in dogs has been repeatedly emphasised as an excellent model for humans. Naturally-occurring cancer in dogs and humans share similarities in histology, tumour biology, and response to conventional therapies. Regarding the close resemblance to human malignant INS, canine INS forms an interesting study model for human INS. Novel targets for human and canine INS therapy may be identified by uncovering the pathways underlying tumourigenesis of canine INS. The aims of this study were to: 1.Develop a novel surgical technique for resection of canine insulinomas and abdominal lymph node metastases; 2.Establish reliable prognostic biomarkers for canine insulinomas that facilitate optimal patient management; 3.Identify novel druggable targets in canine and human insulinomas. It was demonstrated that partial pancreatectomy using a bipolar vessel-sealing device (BVSD) was a safe and viable alternative to conventional methods of pancreatectomy for canine INS. BVSD pancreatectomy in dogs with INS significantly decreased operative and hospitalisation times and was not associated with more clinical complications than conventional pancreatectomy. Tumour size, TNM stage, stromal fibrosis and the Ki67 index proved to be useful parameters to predict clinical behaviour of canine INS. Novel druggable genes in canine and human INS were identified, using quantitative real-time PCR, and microarray analysis. Using flow cytometric analysis, athymic mice and zebrafish embryo xenografts assays, CD90 was identified as insulinoma cancer stem cell marker with the potential to serve as a druggable target.

▼ In this thesis some recent technological developments in human surgery are evaluated for their potential use in veterinary medicine by introducing them as surgical innovations for canine gonadectomy. Barbed sutures achieve wound apposition without surgical knot tying and thus avoid knot-associated negative consequences (lengthy placement, impaired wound healing around bulky knots, and the effect of unsightly knots on cosmetics). A study in 9 dogs found that celiotomy closure was easily achieved, no intraoperative or postoperative complications occurred, and ultrasonographic characteristics during wound healing were comparable to conventional sutures. Ovariectomy is technically less complicated, less time consuming, and is associated with decreased morbidity (smaller incision, less intraoperative trauma, decreased discomfort) compared with ovariohysterectomy. For the incidence of long-term urogenital problems, including endometritis/pyometra and urinary incontinence, no differences between techniques were observed. This makes ovariectomy the preferred method for elective gonadectomy. Neuromuscular blocking agents induce abdominal muscle wall relaxation and might lead to a larger intra-abdominal working space and improved surgical efficiency. However, the duration of ovarian pedicle transection in 40 dogs was not significantly changed (5.7 ± 2.3 versus 5.9 ± 1.9 minutes), arterial blood pressure decreased 5%, and the occurrence of intraoperative complications did not differ. There was no advantage to add NMB to the anesthetic protocol. Newly developed energy-based hemostatic surgical devices were evaluated during laparoscopic ovariectomy. When a bipolar electrocoagulation grasping forceps was compared with monopolar electrocoagulation on 103 dogs, it decreased laparoscopic ovariectomy time (41 versus 53 minutes), decreased intraoperative hemorrhage (8 versus 13% dogs), and facilitated exteriorization of the ovaries. Obesity (56 versus 42 minutes) and intraoperative mesovarial bleeding (56 versus 46 minutes) increased surgical time. Dog age, estrus, and pseudopregnancy did not influence surgical time. When a vessel sealing device was compared to the bipolar electrocoagulation grasping forceps in 40 dogs, the surgical time was reduced (3.5 ± 2.7 versus 8.1 ± 4.2 minutes), with less bleeding (0 versus 10%), and with less thermal coagulation damage based on histological scoring. Surgical times were influenced by surgeon experience, and ovarian pedicle fat scores. When the vessel sealing device was compared to a bipolar electrocoagulation cutting forceps in 21 dogs, excision of the ovaries was faster (2.5 ± 1.2 versus 4.8 ± 1.8 minutes), bleeding occurred less (0 versus 29%), and thermal coagulation damage was less (93.4 ± 85.8 versus 130.9 ± 101.2 µm). Surgical times increased in obese animals. A vessel sealing device for open ovariectomy was compared with conventional suture ligation in 20 dogs, and resulted in reduced surgical times (2.22 ± 0.58 versus 4.10 ± 1.13 min).
