Abstract: The combined training (CT, i.e., aerobic training combined with resistance training) is recommended to improve and maintain the cardiorespiratory fitness, strength and muscle mass in elderly. According to literature, the resistance training with blood flow restriction using low intensity (20-30% 1-RM) may increase the muscular strength and muscle mass. Thus, the combined training with blood flow restriction (CTBFR) can be an interesting alternative to improve the functional capacity offering a low mechanical stress to the elderly. PURPOSE: to verify and compare the effect of CT and CTBFR 12 weeks on cytokines (IL-6, IL-10, IL-15, and TNF-?) and C-reactive protein (CRP) in elderly. METHODS: 33 healthy older adults (65.1 ± 4.7 years; 65.3 ± 8.5 kg; 1.6 ± 0.1 m; BMI = 25.0 + 2.6 kg/m²; Triglycerides = 120.08 ± 53.89 mg/dL; Total Cholesterol = 199.67 ± 25.74 mg/dL; HDL-cholesterol = 51.67 ± 10.05 mg/dL; LDL-cholesterol = 124.85 ± 18.29 mg/dL; Insulin = 7.69 ± 3.60 mcUI/mL) were randomly assigned in three groups: CT (n = 11, 2 days/wk walking or running for 40 min, with intensity variation of 55-85% of VO2peak and 2 days/wk resistance training on the leg-press 45º with 4 sets of 10 reps at 70-80% of 1-RM, with 60 s of rest); CTBFR (n = 11, 2 days/wk walking or running for 40 min, with intensity variation of 55-85% of VO2peak and 2 days/wk resistance training with blood flow restriction on the leg-press 45º with 1 set of 30 and 3 sets of 15 reps at 20-30% of 1-RM and cuff placed on the thigh proximal region at 50% of thigh systolic pressure, with 60 s of rest); and control group (n = 11, no exercise). Before and after of the experimental period (12 weeks) were performed the following evaluations: collection of blood samples to biochemistry analysis (lipid profile, insulin, and inflammatory markers), anthropometry, magnetic resonance image of thigh (MRI), muscular strength (1-RM and maximal voluntary ballistic isometric contraction ¿ MVBIC) and cardiopulmonary test. The training-related effects were assessed using a linear mixed model with Tukey post hoc test. RESULTS: The CT and CTBFR groups showed similar increase in cross sectional area of quadriceps (CT: 7.3%, p < 0.001; CTBFR: 7.6%, p < 0.0001), 1-RM (CT: 38.1%, p < 0.001; CTBFR: 35.4%, p < 0.001) and VO2peak (CT: 9.5%, p = 0.04; CTBFR: 10.3%, p = 0.02), described by LIBARDI et al. (2015), without significant changes after the intervention period in inflammatory markers analyzed. Our data shows that the CTBFR and CT did promote similar neuromuscular and cardiorespiratory adaptations, however, from further analysis of effect size and confidence interval, it was possible observe an inflammatory behavior for the CTBFR group and anti-inflammatory for the CT Advisors/Committee Members: UNIVERSIDADE ESTADUAL DE CAMPINAS (CRUESP), Chacon-Mikahil, Mara Patrícia Traína, 1965- (advisor), Universidade Estadual de Campinas.
