Abstract: The polycystic ovary syndrome (PCOS) is an endocrine heterogeneous disease with unknown etiology. It affects about 8% of women in reproductive age. Among the main features of this disease is the hyperandrogenism, infertility and chronic anovulation. The PCOS also have high prevalence of disorders that lead to the metabolic syndrome, such as obesity, insulin resistance, hyperinsulinemia, hyperlipidemia, hypertension and diabetes mellitus type 2. The pharmacological measures, such as diet and physical exercise, have been considered as a first-line strategic on disease prevention and treatment of that. Regarding this presented situation, the aim of this study was to assessusing an animal model of androgenization, PCOS relationship between metabolic syndrome and aerobic exercise effects on the chronic treatment of these disorders. The animals groups were sedentary PCOS (PCOSsed), PCOS exercise (PCOSexe), sedentary control (SEDcon) and exercise control (EXEcon). The animals had free access to water and food. The training methodology was swimming with pleasant water temperature, during 1 hour per day, 5 times a week, over 8 weeks with progressive overload. The anthropometric, metabolic, cardiovascular, renal and intracellularly analysis were carried out considering measures such as body mass ratings, blood pressure, heart rate, lipid profile, glycemic profile, insulin analyses, glomerular filtration, renal sodium handling and intracellular signaling pathways. In the skeletal muscle was evaluated intracellular signaling through the proteins IR, IRS-1, Akt, GLUT-4 and Angiotensin II receptors (AT1R and AT2R) in kidneys. The PCOSsed animals showed the parameters obesity, glucose intolerance, hyperinsulinemia, hypertension, and increased heart rate significantly higher than trained groups, after 8 weeks of experiments. The obesity, insulin resistance and hypertension results were positively correlated with high cholesterol, triglycerides and HDL-C levels. For the kidney function¿s test, the trained groups (PCOSexe and EXEcon) showed lower renal sodium excretion than sedentary groups. However, only in PCOSexe animals the decrease occurred primarily by reducing excretion in the proximal portion, which was offset by an increase in sodium excretion in the distal portion. Another renal response in animals PCOSexe observed was a decrease in renal excretion of potassium. Natriuretic reduction in trained animals may have occurred due to the homeostatic mechanism for compensatory reduction in blood pressure and heart rate. The analysis of AT1R and AT2R, PCOSsed animals have showed less expression of these receptors. On the other hand, the PCOSexe group has presented a significant increase in the expression of AT2R when compared to PCOSsed, which have influenced in the elevation of the reason AT1R/AT2R. Abstract: Introduction: Breast carcinoma may present different clinical and pathological responses to neoadjuvant chemotherapy (NAC), with worse prognosis for those with non-complete pathologic response (non-pCR). Neo-Bioscore is an important prognostic marker of NAC. We analyzed the predictive factors of complete clinical (cCR) and pathological response (pCR) in breast and axilla and also the prognostic factors for women with non-pCR carcinoma according to a new model of Neo-Bioscore presentation. Methodology: This is a retrospective study that included women with breast carcinoma submitted to NAC from 01/01/13 to 12/31/15. The main outcomes were disease-free survival (DFS), overall survival (OS), cCR and pCR in breast and axilla. The total follow-up was 45 months. The investigated factors were menopausal status, Ki67 expression, estrogen receptor (ER), histological grade, molecular subtype, tumor size, axilla status and stage. The Clustered Neo-Bioscore ranked the women at low, low-intermediate, high-intermediate and high risk according to DFS. The prognostic accuracy between Clustered Neo-Bioscore and Neo-Biosco was performed by the ROC curve. The predictive analysis for cCR and pCR by odds ratio (OR) was performed by multinomial logistic regression. The prognostic evaluation by Hazard Ratio (HR) of women with nRPc was performed by Log-rank and Cox Regression, for a statistical significance p-value < 0.05. Results: A total of 310 patients submitted to NAC presented a predominance of T3/T4 (64%), stage III (62%), positive axilla (74%), Ki67 > 20% (74%) and non-luminal A (81%). The percentage of cCR/pCR, relative to non-cCR/non-pCR for breast, axilla and breast plus axilla were respectively 53.2%/16.5%, 76.3%/36.8% and 50.6%/13.9%. Triple-negative (TN) carcinoma was the main predictor of pCR in breast (OR = 22.81, 7.13-72.96), axilla (OR = 64.00, 8.90-461.40) and breast plus axilla (OR = 36.06, 8.77-148.26). The Ki67 > 50% expression was a predictor of cCR in breast (OR = 2.00, 1.31-3.06) and breast plus axilla (OR = 1.67, 1.10-1.45), but not for axilla. The negative ER carcinomas, Ki67 > 50%, HER2 and TN subtypes were predictors of cCR and pCR. 267 non-pCR carcinomas had worse DFS-3a (HR = 3.88, 1.18-11.95), but not OS-3y (p > 0.05). Clustered Neo-Bioscore showed similar prognostic accuracy to Neo-Bioscore (0.76 vs 0.78, p > 0.05). Carcinomas classified as low-intermediate, high-intermediate and high risk presented respectively 2.6 x, 14 x and 22.2 x greater risk of tumor recurrence than low risk. The III pathological stage had poor prognostic for low risk carcinoma (HR-3y = 13.0, 1.60-106.1). The TN carcinoma (HR-3y = 3.60, 1.19-10.92), II (HR-3y = 5.35, 1.19-24.01) and III (HR-3y = 6.56; 1.29-33.32) tumor stages were poor prognostic factors in carcinomas of low-intermediate risk. Conclusion: TN and HER2 carcinomas were the main independent predictors in women with pCR in breast, axilla and breast plus axilla, including to confirm pCR in patients with cCR. Abstract: Objective: To correlate the cephalometric and anthropometric measures with the severity of obstructive sleep apnea syndrome (OSAS) using the apnea-hypopnea index (AHI). Patients and Methods: The study is presented in two articles: the first is a retrospective study of 93 patients with OSAS, carried out between July 2010 and July 2012, patient age ranging from 19 to 80 years; the second was also a retrospective study of 102 patients with OSAS, carried out between from July 2010 and July 2013, patient age ranging from 20 to 70 years, divided into three age groups (?20?40, ?40?60 and ?60 years). The following measures were analyzed in both studies: body mass index (BMI), neck circumference (NC), abdominal circumference (AC), pelvic circumference (PC), angles formed between the base of the cranium and the maxilla (SNA) and the mandible (SNB), difference between SNA and SNB (ANB), distance between the mandibular plane and the hyoid bone (MP-H), space between the base of the tongue and the posterior pharyngeal wall (PAS) and distance between the posterior nasal spine and the tip of the uvula (PNS-P). The means, standard deviation and Pearson¿s correlation were analyzed. Results: In the first study, it was observed that the AHI showed a significant correlation with: BMI (r=0.207, p=0.047), NC (r=0.365, p<0.001), AC (r=0.337, p=0.001), PNS-P (r=0.282, p=0.006) and MP-H (r=0.235, p=0.023). In the second study, there was no correlation with BMI in patients aged 20 to 40 years old. In patients over 60 years of age, there was no significant correlation of cephalometric and anthropometric measures with AHI. However, the same alterations found in the entire sample were also observed in the 40 to 60 year age group. Conclusions: The anthropometric measures BMI, NC and AC and cephalometric measures MP-H and PNS-P may be used as indicators that are predictive of the severity of OSAS.
