Reabilitação cardiovascular : avaliação de protocolo não exaustivo para determinação de treinamento aeróbico e efetividade de programa de intervenção para manutenção do comportamento de atividade física: Cardiovascular rehabilitation : evaluation of non-exhaustive protocol to determine aerobic training and effectiveness of intervention program for maintaining the physical activity behavior.

Degree: 2014, Universidade Estadual de Campinas

Abstract: For implementation of its methodology, this study conducted low-risk patients to training (hypertension and coronary artery disease) referred to a Cardiac Rehabilitation Program (CRP) was divided into two steps. Objectives: The Step I aimed to evaluate the benefits of an aerobic exercise training based on non-exhaustive Double Efforts Protocol (NEDE) for determining exercise intensity to improve aerobic capacity and cardiovascular risk factors. This step employed a quasi-experimental design (n=67) over a 12-week follow-up of physical training. The Protocol of Physical training was performed 3 times per week, lasting 35 minutes, with intensity given by the critical speed grounded in PDE. The NEDE was based on the results of four tests of aerobic exercise performed randomly at different intensities on a treadmill, using estimates of delta oxygen consumption and delta heart rate obtained in each test for determination of critical speed. The Step II aimed to evaluate the effectiveness of an educational intervention program (EIP) after discharge from the CRP in relation to the physical activity maintenance, aerobic capacity data and cardiovascular risk factors. This step employed experimental design, including all patients who have completed Step I of the study (n=58). Patients were randomized into control group (CG) and intervention group (IG). EIP is an intervention combining reinforcement of self-efficacy and post-volitional strategy to aid the implementation of the intention to act (action planning strategies and coping with obstacles). Data analysis: In Step I, the data were subjected to analyzes Paired t-test and Wilcoxon Signed-Rank test to compare repeated measures (normal and non-normal distribution). In Step II, the data were submitted to analysis of Wilcoxon Signed-Rank test and the Mann-Whitney Model for comparison of independent (groups) and dependent measures (time). It was adopted as the significance level p<0.05. Results: At the final follow-up of Step I, there was an increase of 14.4% in VO2 peak (p<0.0001) and an increased in the critical speed reached in PDE of 12.3% (p<0.0001). Furthermore, there was a reduction in waist circumference of 2.8% (p<0.0001) and an increase of 6.2% in HDL-cholesterol (p=0.0159). At the end of follow Step II, there was a significant increase in physical activity given by self-related in IG compared to CG (p<0.001) and increase in leisure activities in IG at T3 (p<0.01); decrease in VO2 peak and METs peak in the CG (p<0.05) and maintenance in IG, and increase of VO2 peak estimated in IG at T3 (p<0.05). Waist circumference, body weight and body mass index increased in CG (p<0.05) and remained in the GI T3. There was a decrease in total cholesterol in IG at T3, compared to the CG (p< 0.05). Conclusion: The NEDE showed to be a promising method for prescribing exercise training among patients with cardiovascular disease with benefits in relation to physical capacity and cardiovascular risk markers such as waist circumference and HDL-cholesterol. The EIP showed evidence… Advisors/Committee Members: UNIVERSIDADE ESTADUAL DE CAMPINAS (CRUESP), Gallani, Maria Cecilia Bueno Jayme, 1966- (advisor), Gobatto, Claudio Alexandre, 1964- (coadvisor), Universidade Estadual de Campinas.
