Interação social e comunicação de crianças com alteração neuromotora sob a ótica de familiares/cuidadores e do fonoaudiólogo: Social interaction and communication of children with neuromotor disabilities from the perspective of family/caregivers and the speech and language pathologist.

Degree: 2011, Universidade Estadual de Campinas

Abstract: Introduction: Several factors such as maternal malnutrition, extreme prematurity, and various syndromes, among others, could cause changes in child development. In this case, timely intervention can promote the overall progress of these children at risk, considering that this action involves the participation of family / carers and a multidisciplinary team, of which the audiologist. In Brazil, there are few standardized protocols for assessment of language. In the areas of occupational and physical therapy, one of the instruments used for pediatric evaluation of disability is the PEDI - Pediatric Evaluation of Disability Inventory that proved viable for investigation of aspects of communication and social interaction of children at risk making up this study. Objectives: To analyze the social interaction and communication of children with neuromotor from the perspective of family / caregivers and the Speech and Language Pathologist and to characterize the profile of children and families / caregivers. Subjects and Methods: The study was approved by CEP-FCM/UNICAMP and submitted for approval at the institution of the study. Participated in 35 family members / caregivers of 38 children aged 12 to 36 months, with neuromotor and risk factors for the development, attended the Early Stimulation Programme of APAE of a midsize city in the interior of São Paulo and a Speech and Language Pathologist (therapist and researcher). Data collection was done through three sources: (i) study of medical records to characterize the profiles of children in age, sex, diagnosis and time of attendance at the institution, (ii) information gathering with family / caregivers to characterize their profiles as to age, education and occupation, and (iii) implementation of the PEDI, has been used the first part of this instrument, focused social function skills, which include aspects of communication and social interaction. The data collection procedures involved with the family / caregivers, the interview method and with the professional, clinical trial method. We performed descriptive statistics, inferential and factorial data compared to the reference values of the PEDI. Results: Regarding the distribution of diagnosis, children with cerebral palsy was more frequent. Most family members / caregivers were mothers (88%) and a small portion, grandparents (6%) and parents (3%), aged between 19 and 45 years, withpredominance of high school education and occupation as reported work from home (67%). The responses of family members / caregivers showed greater agreement on communication skills (68%), followed by the skills of home / community (56%) and social interaction (53%). The responses of the PEDI showed developmental delays in communication skills and social interaction of children studied in relation to the values of the instrument both in the perspective of family / caregivers and the professional / researcher. Conclusions: In general, the majority of children in the study presents reference values below those of the PEDI, showing… Advisors/Committee Members: UNIVERSIDADE ESTADUAL DE CAMPINAS (CRUESP), Chun, Regina Yu Shon, 1958- (advisor), Universidade Estadual de Campinas. Estudo da atenção à família no cuidado de crianças e adolescentes com alterações de linguagem = perspectiva dos familiares utilizando a Classificação Internacional de Funcionalidade, Incapacidade e Saúde para crianças e jovens (CIF-CJ): Study of family attention in the care of children and adolescents with language impairment : families' perspective through The International Classification of Functioning, Disability and Health for children and youth (ICF-CY).

Degree: 2016, Universidade Estadual de Campinas

Abstract: The health assistance has been the subject of attention in various areas of knowledge, such as Pediatrics and Speech and Language Pathology. The family attention, as research subject and health intervention field, requires focus their modes of assistance and share the care, in a wider health perspective. This broader understanding beyond the biological dimension of the disease, presupposes a biopsychosocial model as proposed by the World Health Organization (WHO) in the creation of the International Classification of Functioning, Disability and Health - version for children and youth (ICF-CY), analysis referential applied in this study. Purpose: Investigate the perception of family members regarding the speech and language disorders of children and adolescents under speech and language therapy and the impact of family participation in the therapeutic process using the ICF-CY. Method: Longitudinal qualitative research approach in which 32 family members of 25 children and adolescents with speech and language disorders, under speech and language therapy. For the data collection it was conducted a survey of children and adolescents medical records, to characterize the participants¿ profiles semi-structured interviews with family members, analysis of speech and language pathology reports and written, graphic and video records of 26 group sessions with family members, which occurred from August 2012 to June 2015, transcribed for categorization using the ICF-CY. Results: In both interviews and attendance records of family members, all participants addressed initially, in the ICF-CY component of body Functions and Structures, domains such as fluency and articulation, showing concern with organic aspects. Most reported different repercussions of the speech/language disorders in domains such as dealing with stress and speaking activity among others, qualified from mild to severe, initially, wherein these qualifiers were modified to without alteration (without problem) to moderate during the therapeutic process. Participants statements were classified in the Environmental Factors component, as facilitators in domains such immediate family¿s attitudes and health services, systems and policies, and as barriers in the social attitudes. Conclusion: These findings demonstrate that the children and adolescents¿ speech and language disorders, in the families¿ perception, are firstly addressed at the organic dimension. As of the therapeutic care, the findings demonstrate, as repercussion, the expansion of the look of the family members to other issues and implications of everyday life in speech/language. The results corroborate the importance of the ICF-CY usage as a health care analysis tool, by incorporating functionality and participation aspects, fostering the promotion health and care opportunities in the life of this population group, which may contribute to the formation of health professionals in the theoretical approach adopted Advisors/Committee Members: UNIVERSIDADE ESTADUAL DE CAMPINAS (CRUESP), Zanolli, Maria de Lurdes, 1954- (advisor), Chun, Regina Yu Shon, 1958- (coadvisor), Universidade Estadual de Campinas. 3. Zerbeto, Amanda Brait, 1988-. Estudo da percepção de crianças e adolescentes com alterações de fala e linguagem utilizando a CIF-CJ: Study of the perception of children and adolescents with speech and language disorders using the ICF-CY.

Degree: 2017, Universidade Estadual de Campinas

Abstract: Introduction: Speech and language disorders may cause implications in social participation and functionality in children and adolescents. In Speech-Language Pathology, not always the social impact of speech disorder is contemplated, limiting the therapy to biological aspects. And one framework that interests to language and speech area, by the biopsychosocial approach of the individual, is the International Classification of Functioning, Disability and Health - children and youth version (ICF-CY). Objective: To characterize longitudinally the perception of children and adolescents with speech disorders and children and adolescents with typical language development, about language, participation, and functionality aspects, using the ICF-CY. Method: Descriptive and analytical, and longitudinal research of qualitative and quantitative approach. The sample was composed of 60 children and adolescents: 30 participants with speech disorders (SD Group) and 30 with typical language development (TD Group). I collected the data in two moments: beginning of research and six months later. For the data collect: I interviewed the participants with a semi structured questionnaire; the legal guardians filled out a structured questionnaire with sociodemographic information and health history; and I consulted the medical records of SD participants. From the data collected, language, participation, and functionality aspects were analyzed according ICF-CY domains. I designated one qualifier and described qualitatively each ICF-CY domain. For quantitative analysis, I used: Wilcoxon Test for the data comparison between Interview 1 and 2 in each group; Mann-Whitney Test for the comparison between ICF-CY qualifiers in SD and TD Groups in the same interview; the significance level adopted for the statistical tests was 5% (p?0.05). For qualitative analysis I used the thematic content analysis. Results: In Interview 1, most of selected ICF-CY domains were statically significant different between the groups, and the SD Group presented more difficult. The SD participants reported articulation and fluency difficulty with repercussion in daily activities, such as: conversation, to relate to family members and friends, carrying out daily routine, and handling stress. The family members and acquaintances presented barriers attitudes, such as to correct the participant's speech and give nicknames related to their speech. TD Group did not report these difficulties. Comparing Interviews 1 and 2, in TD Group, three domains related to relating and attitudes of family members and friends presented significant changes. In SD Group, the difficulty level decreases with SLP therapy in many domains: articulation and fluency functions, social relation, daily activities, barrier attitudes of people, and handling stress. Conclusion: The use of ICF-CY and Body Function, Activities and Participation, and Environmental Factors components allows the professional to characterize language, participation, and functionality aspects, and the impact of the speech… Advisors/Committee Members: UNIVERSIDADE ESTADUAL DE CAMPINAS (CRUESP), Zanolli, Maria de Lurdes, 1954- (advisor), Chun, Regina Yu Shon, 1958- (coadvisor), Universidade Estadual de Campinas.
