Abstract: Total parenteral nutrition (TPN) consists of the intravenous administration of the necessary ingredients for the survival and maintenance of the patient's homeostasis. Over the years this technique has been increased and contributes to the treatment of several types of patients including the seriously ill. However, TPN may be related to alterations in the lipid and hepatic profile. The most common hepatic and biliary complications in PN are: cholestasis and hepatic steatosis. Changes in the lipid profile are more related to hypertriglyceridemia and low HDL values, mainly in patients in inflammatory response. The aim of this study was to evaluate the changes in the lipid and hepatic profile of pediatric patients submitted to TPN therapy hospitalized in the Clinical Hospital of the State University of Campinas - Unicamp. It is a descriptive and historical cohort study, in which 195 pediatric patients (January 2012 to February 2016, from 0 up to 19 years old) who received TPN were evaluated. The hepatic profile was assessed from alanine aminotransferase (ALT), aspartate aminotransferase (AST), alkaline phosphatase, gamma glutamyl tranpeptidase (GGT), total bilirubin. The albumin and transthyretin were also evaluated. The lipid profile was evaluated from values of cholesterol, HDL and triglycerides (TG). Laboratory and clinical nutritional status, evaluation and outcome were also measured. According to the hepatic profile, the majority of patients presented normal values of AST (79.2%), ALT (74.9%), GGT (56.4%) and alkaline phosphatase (68.1%). Total bilirubin was altered in 68.5% and transthyretin (87.3%) and albumin (65.1%) were low in most patients. The data regarding the lipid profile showed normal cholesterol in 84.6%, low HDL in 94.5%, high TG in 81.5% and high C-reactive protein in 92.7% of patients. With multivariate logistic regression it was possible to identify that the chances of patients under NPT with high GGT values are 2.46 times higher if the patient is older than two years of age, 2.26 times higher if not a surgical patient, 3 times higher if you have a high cholesterol and 2.39 times higher if you have high triglycerides. The chance of elevated ALT is 4.57 times higher in patients who present high cholesterol. It was concluded that the prevalence of elevated ALT, AST, GGT and alkaline phosphatase were within the percentages described in the literature. From the association of alterations of the hepatic and lipid profile it was possible to verify that patients with hypercholesterolemia are more likely to make alterations of ALT, and GGT. Patients with hypertriglyceridemia are more likely to have GGT changes.
