Abstract: Chronic kidney disease (CKD) is associated with high morbidity and mortality rates, main causes related with cardiovascular disease (CVD) and bone mineral disorder (CKD-BMD). Advanced glycation end-products (AGEs) are uremic toxins that play a role on development of CVD and CKD-BMD. The measurement of skin autofluorescence (sAF) is a noninvasive method to assess the level of AGEs in tissue, validated in CKD patients. AIMS: Evaluate in hemodialysis (HD) patients the relationships among AGEs detected via sAF, serum and dietary intake, and metabolic and structural parameters of bone and cardiovascular disease, including vascular calcification (VC) evaluation. METHODS: An observational, transversal study that had included 32 patients undergoing HD at Hospital das Clínicas da UNICAMP and Hospital das Clínicas da Faculdade de Medicina de Botucatu; and 37 healthy subjects (Control Group). All subjects performed biochemical tests, ankle-braquial index (ABI) measurement by Doppler ultrasound, AGEs-Saf and quantification of dietary AGEs, carboxymethyl-lysine (CML) and methylglyoxal (MG) intakes by three-day dietary record. HD Group underwent hip, hands and lateral abdominal x-rays; transthoracic echocardiography; bone densitometry; serum CML; and semi-quantitative bone histomorphometric analysis. RESULTS: AGEs-sAF levels were elevated according to age in both HD and Control groups, however were higher in CKD patients (2.8 UA vs. 2.4 UA; p = 0.005). Dietary intake of total AGEs (11,943 ± 5,581 KU vs. 8,564 ± 3,702 KU; p = 0.016), CML (4,213 ± 3,941 KU vs. 2,560 ± 1,792 KU; p = 0.003) and MG (14,648 ± 10,902 nmol vs. 10,664 ± 5,480 nmol; p = 0.018) were higher at Control Group. Total AGEs ingestion was associated with lower values of cardiac ejection fraction (R = -0.451 ; p = 0.035) and higher ABI (R = 0.456 ; p = 0.038). The most part of patientes that underwent bone biopsy was diagnosed with osteoporosis (N = 12 ; 57%), and this group had lower total AGEs intake. The most frequent osteodistrophy tipe was osteitis fibrosa (OF) , wich was associated with higher CML intake and AGEs-sAF. CONCLUSION : Our study suggests AGEs association with sistolic cardiac disfunction, vascular stiffness and bone disorders related with BMD-CKD like OF and osteoporosis in HD patients.
