Abstract: Introduction: Myocardial infarction (MI) is one of the most frequent causes for the development of heart failure. Therapeutic strategies that modulate ventricular remodeling (VR) have been indicated after MI. Physical exercise and atorvastatin may exert effects on myocardial mechanics, ventricular performance, and collagen and fibrosis metabolism. Objectives: To evaluate the effects of physical exercise and atorvastatin on the improvement of ventricular remodeling after myocardial infarction induced in experimental rats. Materials and methods: We evaluated 108 male Wistar rats at 4 weeks of age submitted to permanent ligation of the anterior interventricular artery or sham. The animals were divided into 8 groups: SHAM (n=7), SHAM/TR (n=6), SHAM/AT (n=5), SHAM/TR/AT (n=8), IM/TR (n=9), IM/AT (n=10), IM/TR/AT (n=13). On the 1st postoperative day, AT groups received atorvastatin (10mg/kg/day); on the 2nd day, the treadmill training protocol was started for 8 weeks. Left ventricular (LV) pressure-volume and histological catheter analyzes were performed, as well as the exhaustion test at the 4 and 8 week post-training periods. Results: The ratio of LV weight to animal weight in the infarcted groups was higher in the training and atorvastatin groups. The training in association with atorvastatin reduced the cross-sectional area of the cardiomyocytes when compared to the animals of the IM groups (IM/TR/AT 273.5 ± 66.9?m2 vs. IM 293.4 ± 65.9?m2, IM/TR 299.2 ± 72.9?m2, IM/AT 299.0 ± 58.6?m2, P<0.01). From the hemodynamic data the minimum time pressure/derivative derivative (dP/dt min) in the IM groups treated with atorvastatin, independent or non-training, showed higher values compared to the IM animals (IM 0.00180 ± 0.0002mmHg/s vs. IM/TR/AT 0.00234 ± 0.0005mmHg/s vs. IM/AT; P<0, 05). The content of collagen and fibrosis in the LV and we observed that the training associated or not with atorvastatin was efficient to decrease mainly the occurrence of fibrosis in the infarcted groups (IM 12.4 ± 0.76%, IM/TR 4.9 ± 1.6%, IM/AT 12.3 ± 3.8% and IM/TR/AT 7.0 ± 3.3%; P<0.05). The distance analysis showed that the trained animals performed better than the IM group (IM 265.5 ± 147.8m vs. IM/TR 673.4 ± 334.3m, IM/TR/AT 641.1 ± 341.0m, SHAM/TR 729.6 ± 184.6 and SHAM/TR/AT 604.3 ± 244.8m; P<0.01). Abstract: Introduction: prolonged mechanical ventilation (PMV) and difficult weaning are associated with reduced thickness and weakness of the main inspiratory muscle, the diaphragm. Powerbreathe® equipment is a handheld device that, in addition to providing two forms of inspiratory muscle training (EIMT), provides inspiratory muscle energy and power information. Objectives: to evaluate the general characteristics, MV and weaning time, inspiratory pressures (MIP) and rapid shallow breathing index (RSBI) of patients selected for IMT in the TMIE and intermittent nebulization groups; analyze, and compare pressure, power, flow, volume, and energy variables between Powerbreathe® manual (GPM) and automatic (GPA) groups. Method: were selected tracheostomized patients undergoing ventilatory weaning with adequate gas exchange, with no hemodynamic instability, musculoskeletal disorders or neuromuscular disease. The patients included were randomized to the TMIE: Powerbreathe® (GP) groups, and divided according to the level of consciousness, Glasgow coma scale (GCS) ? 8 to GPM, with a loading of 30% of MIP; and GCS ? 9 to GPA group, with automatically load adjusted. Intermittent Nebulization Group (GNI): adaptation of oxygen through a collar tracheal, with progressive increasing time of MV disconnection. Patients were followed up until 48 hours of continuous nebulization. Results: 132 patients were selected and 104 included, randomized to EIMT (n = 51), divided into GPA (n = 25), GPM (n = 26), GNI (n=53). The sample was homogeneous, except for Apache II, with higher value in GPM (p = 0.02). The weaning time was not statistically different, p = 0.23, GP (8.55 ± 6.48 days), GNI (10.86 ± 6.48 days); and the lowest success rate (75%) was for GPM group. The highest VMI time was GNI group (28.68 ± 15.75 days), with no significant difference (p = 0.21). There was a significant increase in MIP to GNI (p = 0.001) and GPA (0.007), with significant reduction to RSBI in GNI (p = 0.03). The GPM group presented the highest values of pressure (p = 0.00001), power (p = 0.0007) and energy (p = 0.003). Conclusion: these critical ill patients, with PMV have IMT indication, whose modalities must be individualized, in order to reduce energy expenditure for neurological patients and to adjust loads and training frequencies in the GPA group.
