Abstract: Hepatitis C virus (HCV) is one of the major causes of liver diseases, including cirrhosis and hepatocellular carcinoma. The main reason to treat HCV infection is to eradicate the virus and prevent complications related to the chronic infection. The first study evaluated the rates of sustained virological response (SVR) , treatment discontinuation (TD) due to adverse events (AE), and severe anemia (hemoglobin >8.5g/dL) in patients with chronic HCV infection treated with triple therapy based on first generation protease inhibitors (PI). We included 203 patients: 132 treated with telaprevir (TVR) and 71 with boceprevir (BOC). TD rate due to AE was 19.2% and anemia was the mains reason (38.5%). Risk factors for TD were higher Charlson's comorbidity Index [(CCI) OR 1.85, CI 1.05-3.25)] for BOC, and higher bilirubin count (OR 1.02, CI 1.01-1.04) and lower body mass index (OR 0.98, CI 0.96-0.99) for TVR. Severe anemia occurred 17.2% of the patients. Risk factors for this outcome were lower estimated glomerular filtration rate (eGFR) (OR 0.95, CI 0.91-0.98) for patients treated with TVR and higher CCI (OR 2.21, CI 1.04-4.67) and higher ribavirin (RBV) dosage (OR 0.84, CI 0.72-0.99) for those patients treated with BOC. SVR rates for patients treated with TVR and BOC were 57.3% and 27.3%, respectively. The second study evaluated the TD rates and its risk factors among patients with advanced liver fibrosis treated with all-oral direct antiviral agents (DAA). We included 214 patients: 180 were treated with sofosbuvir (SOF) + daclatasvir (DCV) ± RBV; 31 received SOF + simeprevir (SMV) ± RBV e 3 had SOF + RBV. TD rate was 8.9% and liver decompensation (42%) was the main reason. Among patients with Child-Pugh B or C cirrhosis (31), 10 (32.2%) prematurely interrupted treatment. Risk factors for TD in univariate analysis were higher age (p=0.0252), higher CCI (p= 0.0078), higher Model for end stage liver disease (MELD) (p<0.0001), higher FIB-4 index (p=0.0122), and lower hemoglobin (p=0.0185) at baseline. Multivariate analysis demonstrated that higher age (OR 1.1, CI 1.02-1.19) and higher MELD (OR 1.27, CI 1.03-1.56) were associated with premature treatment interruption. The third study evaluated SVR rates and its associated factors in patients treated with all-oral DAA. We included 527 patients: 51,6% were cirrhotic, 76,8% were infected with genotype (GT) 1, 20,7% had GT3, and 2,5% had GT2. Most patients used SOF + DCV + RBV (60,7%) or SOF + SMV (25,6%). Global SVR rate by intention-to-treat was 90.5%. SVR rate was higher in patients with GT1 (92.1%), followed by GT2 (84.6%), and GT3 (84.4%). SVR rate was higher in non-cirrhotic (94.2%) compared to cirrhotic patients (87.1%; p=0.0071). Lower CCI was associated with SVR (p=0.0014). Among cirrhotic patients, RVS rate was higher in those with Child-Pugh A (88.7%) compared to Child-Pugh B or C (80%). Abstract: Vibration controlled transient elastography (VCTE) is one of the most validated noninvasive test modalities for liver fibrosis staging and steatosis grading in chronic hepatitis C (CHC), however, diagnostic performance indicators may have been hampered by an imperfect gold standard. Considerable inter and intraobserver variability may hinder the accuracy for liver biopsy and, therefore, its comparators. Moreover, VCTE results are internally validated solely on the basis of variability and success rate and lack reproducible quality indicators. In this study we aimed to evaluate different aspects regarding diagnostic performance of VCTE and factors associated with accuracy. Adult patients with CHC were prospectively enrolled from an outpatient university hospital viral hepatitis clinic for consecutive liver biopsy and VCTE. Anthropomorphic, clinical and biochemical data were analysed along with technical factors for possible association with accuracy, including elastogram quality. Liver biopsies underwent both conventional histological examination by an experienced hepatopathologist and digital morphometric analysis. Controlled Attenuation Parameter results from VCTE measurements were recorded and studied in comparison with digital morphometric quantification in liver biopsy fragments and for possible association with elastogram quality. In a preliminary analysis, 182 adult patients with a median age of 55 years and median body mass index of 26.71 kg/m2 were analyzed. Abstract: Hepatitis C (HCV) is the main cause of liver transplantation (LT) worldwide and has universal recurrence after LT. It is responsible for higher morbidity and mortality when compared to other causes of LT. Antiviral treatment with sustained virological response (SVR) is recommended in order to avoid graft fibrosis and development of cirrhosis. Two categories of treatment were evaluated: interferon-based and direct-acting antivirals (DAA). The first study included 127 patients who underwent LT due to decompensated cirrhosis or hepatocellular carcinoma (HCC) secondary to HCV infection. Fifty-six patients had histological evidence of HCV recurrence, 42 started interferon-based therapy and 37 completed treatment, which consisted of conventional or pegylated interferon associated with ribavirin (RBV). Adverse events (AE) such as anemia (75.7% of patients) or neutropenia (65.6%) were common, requiring RBV dose reduction (75.7%), erythropoietin use (59.5%), filgrastim use (56.8%) and interferon dose reduction (48.6%) for management. Graft rejection occurred in 48.6% of patients treated, which was associated to antiviral therapy in 38.9% of cases. The overall SVR rate was 54.1%. SVR was associated to longer follow up after treatment (median 66.5 versus 37 months for non-SVR patients, P=0.03), reduced rates of liver disease progression (15% versus 64.7%, P=0.0028) and death (5% versus 35.3%, P=0.033). Regardless of the result of therapy (SVR or not), there was a significant difference between treated and untreated patients regarding the occurrence of death and months of survival (P<0.001). The second study included 55 patients treated with DAA (sofosbuvir and daclatasvir) associated with RBV, for 12 or 24 weeks (58% and 42% of patients, respectively). The majority of patients was previously interferon treatment-experienced (67%), 65% with HCV genotype 1 and 35% genotype 3 infection. Liver fibrosis staging through biopsy had only been performed on 42% of patients in the two years prior to beginning of treatment and most patients had mild to moderate fibrosis (26% Metavir F1, 43% F2, 9% F3, 4% F4). RBV doses ranged from 250 to 1250 mg daily, 10.1 mg/kg on average. Anemia was the main AE (65%) and 20% of patients had severe anemia (Hemoglobin<10g/dL), requiring reduction or suspension of RBV dosage in 20% and 5% of patients, respectively. There were no treatment interruptions due to severe AE. The SVR rate was 98%, with one treatment failure (a cirrhotic, interferon-experienced, genotype 3 infected patient who was treated for 12 weeks). There were no rejection episodes during antiviral treatment, but one patient had a rejection diagnosis 12 weeks after the end of treatment (EOT). During follow-up after the beginning of treatment (20 months, on average) there were no deaths and only one patient had decompensation of liver disease, with the occurrence of jaundice, ascites, encephalopathy and esophageal variceal bleeding 12 weeks after EOT.
