Abstract: Amyotrophic Lateral Sclerosis (ALS) is a neurodegenerative disease with inexorable course. Classically known by its motor symptoms, ALS clinical spectrum also includes varied stages of cognitive and Behavioural deficits, affecting up to 50% of studied individuals according to populational studies. Clinical, molecular genetical, neuroimaging and pathological data suggests that there is a pathophysiological continuum between ALS and Frontotemporal Dementia (FTD) and increasing attention has been lately given to extramotor manifestations in ALS. Although, formal neuropsychological assessment is extense and infeasible in clinical outpatient routine and impaired by motor deficits. Thus, specific tools are needed in order to properly evaluate ALS individuals. Among internationally validated tools, none had been translated and validated to our population until the beginning of this study. The prevalence and profile of cognitive and Behavioural deficits according to international criteria in Brazilian ALS patients were also not know. Furthermore, the anatomical substrate of extramotor findings in ALS has conflicting findings in literature. Thus, we performed the present study in order to translate and validate a useful tool for cognitive screening in Brazilian ALS patients, named ALS-CBS-Br. Also, we aimed to describe prevalence rates and profile of cognitive (ALSci) and Behavioural (ALSbi) deficits in the studied ALS cohort according to international criteria. Lastly, we performed a neuroimage study with Magnetic Resonance Imaging (MRI) and multimodal evaluation assessing cortical grey matter thickness, deep grey matter structures volume and white matter diffusivity with comparison between ALS subgroups according to cognitive and Behavioural status and a healthy controls subgroup. For such, 76 ALS individuals and 38 healthy controls were recruited. ALS-CBS-Br had accuracy of 90.6% on detecting ALSci in comparison to gold standard neuropsychological evaluation, with proposed cut-off score of 10 out of 20, proving itself as a useful tool for cognitive screening for ALS individuals. We described prevalence of 20% of ALSci in non-demented ALS individuals, with executive, language and verbal memory impairment. Also, we identified prevalence of 23% of Behavioural alterations (ALSbi) in non-demented C9orf72-negative ALS individuals, highlighting apathy as the most frequent frontotemporal deficit, but also with high prevalence rates of dysphoria, anxiety and irritability. Among 7 studied individuals with ALS type 8, caused by VAPB gene mutation, 2 (28,6%) were diagnosed as ALSbi, corroborating the hypothesis that this [is not a purely motor disease. Neuroimaging assessment demonstrated volumetric reduction of amygdalae and left thalamus in comparison with cognitively intact ALS individuals (ALSni) and healthy controls, with significant correlation between these structures MRI parameters and cognitive data. Abstract: Background: Temporal lobe epilepsy is the most common type of focal epilepsy in adolescents and adults. The origin of the seizures is the mesial aspect of the temporal lobe in 2/3 of the individuals. The main cause for mesial temporal lobe epilepsy is hippocampal sclerosis, with 65% of cases becoming the anatomical basis for the surgical treatment of mesial temporal lobe epilepsies. Several surgical accesses, with the objective of preserving structures of the temporal lobe, were described as selective without an objective structural evaluation on the effectiveness in the preservation of the temporal lobe neocortex and temporal stem. Objectives: To determine if there is postoperative temporal lobe injury through advanced imaging methods in selective transsylvian amygdalohippocampectomy. Methods: Fifty-seven patients with a diagnosis of refractory epilepsy of the temporal lobe treated surgically with selective techniques were analyzed: transinsular amygdalohippocampectomy (TI) with 29 patients and transuncus amygdalohippocampectomy (TU) with 27 patients. The inferior frontal occipital fasciculus (IFOF), uncinate fasciculus (UF), optic radiation (OR) were evaluated by diffusion tensor analysis (DTI) as well the relaxometry T2 of the neocortex, compared to the control group of 30 patients with a diagnosis of refractory temporal lobe epilepsy still not undergoing surgical treatment (CO). Results: On the IFOF and UF, the anisotropy fraction, voxels and fibers numbers of the surgical group (TI and TU) decreased in comparison to the CO group (p <0.05), a decrease only in the number of voxels of the RO in relation to the group TI and CO group (p <0.05) and increase in the relaxometry time of the surgical group compared to the CO group (p <0.05%).
