Abstract: Introduction: Literacy can influence different areas of cognition and illiterate individuals present worse performance in tasks of verbal fluency, calculation, memory and constructive praxis. Currently, most cognitive screening instruments for Alzheimer's disease (AD) are influenced by schooling. The diagnostic accuracy of usual cognitive screening tests (Mini-Mental State Examination, Verbal Fluency, Clock Drawing Test) has not been directly compared to that of tests supposedly more appropriate for low schooling. This represents an important gap in the neuropsychiatric literature. Objective: Carry out a systematic review on cognitive screening tests for AD in elderly Brazilians with low levels of schooling. To compare the diagnostic accuracy of usual cognitive tests to tests that might be more appropriate for low schooling in AD screening. Method: For the systematic review, the Prisma method was used for the selection of articles and 9 studies were indexed by the descriptors screening, illiterate, Alzheimer, education and cognition. The empirical study on diagnostic accuracy consisted of a clinical sample of 117 elderly between illiterate and schooled (1 to 4 years of schooling), who attended an outpatient Geriatric clinic. The patients were classified into four groups according to their cognitive status and educational level: literate controls (n = 39), illiterate controls (n = 30), literate AD (n = 30) and illiterate AD (n = 18). The tests were compared as follows: Black and white Figure Memory Test from the Brief Cognitive Screening Battery (BCSB) versus Colored Figure Memory Test from the BCSB; Clock Drawing Test (TDR) versus Clock Reading Test (TLR); Verbal Fluency (FV) animal category versus VF grocery category; Constructive Praxis from the Consortium to Establish a Registry for Alzheimer's Disease (CERAD) versus the Stick Design Test (SDT). Results: The 9 articles that fulfilled the inclusion criteria for the systematic review presented instruments evaluating a single domain, a combination of instruments and batteries containing several instruments. In the empirical study, the means of the illiterate and schooled control group did not present significant differences in the tests: Black and White Figure Memory Test from the BCSB (immediate), Colored Figure Memory Test the BCSB (Late), Clock Reading Test and Verbal Fluency, animals and grocery category. The means of the clinical groups in the two schooling levels presented a significant difference in most of the tests, except in the copy of Praxis Constructiva of CERAD and the Stick Design Test in the school group and in the group of illiterates in the copy and recovery of the Stick Design Test. ROC curve analyses showed equivalent diagnostic accuracy between the appropriate tests for low schooling and the usual tests. Conclusion: For valid cognitive screening it is important to consider sociocultural and educational factors in the interpretation of results.
