Abstract: The medical practice in Brazil is characterized by a large number evaluation requests for specialists in neurology. At the same time, in a country of continental dimensions, it is impossible to equip each emergency unit with structure and team for emergency neurological care. The consequence is a high number of patients transfers to neurosurgery referral centers and delay on the time until treatment. The advances on data transmission allowed telemedicine to become an important tool to address this problem. In this context, the use of smartphones for neurological screening can be an important tool to reduce this problem, as long as its safety and efficiency is verified. OBJECTIVES: Evaluate the safety, effectiveness and reproducibility of the neurological patients screening made through smartphones. Evaluate the deficiencies in the neurological patients referencing system and the impact of the implementation of this tool to solve the problem. METHODS: Clinical, retrospective and multicenter study. Data from medical records of patients submitted to neurological evaluation (age, sex, initial diagnosis, severity, computed tomography [CT] of the skull, initial conduct, time to treatment, hospitalization time, transportation for evaluation and final result) were obtained. The evaluation performed by independent neurosurgeons, using smartphones, was compared with the medical record data, assessing the ability to identify pathological changes in the tomography, identify potentially serious patients and the safety and effectiveness of indicating transfer of patients to referral neurosurgery services. The correlations between time to care, hospitalization time and final results were analyzed using the Pearson chi-square test. The internal-rate agreement and the agreement between the reviewers and the patients' records were analyzed using the method of Cohen's Kappa. RESULTS: The study included 232 cases, 141 men (60.8%) and 91 women (39.2%). The distribution, according to age, was 10 to 102 years, with an average of 48 years (median of 49). The initial diagnosis of the medical records were: 119 cases (51.3%) of TBI; 35 (15.1%) of stroke; 27 (11.6%) of headache; 15 (6.5%) seizure; 4 (1.7%) of HSA; 4 (1.7%) tumor and 28 (12.1%) miscellaneous. In 78 cases (33.6%) there was transport of the patient from other cities to the neurosurgery services. The time between the neurological event and the medical care ranged from 1 hour to 730 hours, with an average time of 18 hours (median of five hours). Among total patients, 105 (45.3%) were discharged after evaluation, 101 (43.5%) were hospitalized and 26 (11.2%) underwent surgery. The telemedicine evaluators presented an accuracy of 95.69% to identify pathological alteration in the CT, with concordance of 0.858. Regarding the identification of severe neurological pathology, the accuracy was 95.26%, with concordance of 0.858. Regarding the conduct, the evaluators presented a concordance of 0.672, increasing to 100% in the cases in which the patient underwent surgery. Abstract: Background: Temporal lobe epilepsy is the most common type of focal epilepsy in adolescents and adults. The origin of the seizures is the mesial aspect of the temporal lobe in 2/3 of the individuals. The main cause for mesial temporal lobe epilepsy is hippocampal sclerosis, with 65% of cases becoming the anatomical basis for the surgical treatment of mesial temporal lobe epilepsies. Several surgical accesses, with the objective of preserving structures of the temporal lobe, were described as selective without an objective structural evaluation on the effectiveness in the preservation of the temporal lobe neocortex and temporal stem. Objectives: To determine if there is postoperative temporal lobe injury through advanced imaging methods in selective transsylvian amygdalohippocampectomy. Methods: Fifty-seven patients with a diagnosis of refractory epilepsy of the temporal lobe treated surgically with selective techniques were analyzed: transinsular amygdalohippocampectomy (TI) with 29 patients and transuncus amygdalohippocampectomy (TU) with 27 patients. The inferior frontal occipital fasciculus (IFOF), uncinate fasciculus (UF), optic radiation (OR) were evaluated by diffusion tensor analysis (DTI) as well the relaxometry T2 of the neocortex, compared to the control group of 30 patients with a diagnosis of refractory temporal lobe epilepsy still not undergoing surgical treatment (CO). Results: On the IFOF and UF, the anisotropy fraction, voxels and fibers numbers of the surgical group (TI and TU) decreased in comparison to the CO group (p <0.05), a decrease only in the number of voxels of the RO in relation to the group TI and CO group (p <0.05) and increase in the relaxometry time of the surgical group compared to the CO group (p <0.05%).
