Abstract: The research aims to reconstitute how the sanitary service was structured in Campinas, a city in the state of São Paulo, Brazil, from the institutionalization process of sanitary policies during Brazil¿s First Republic. This structuring was carried out in the city by means of the São Paulo State legislation, characterized by the Sanitary State Service and from the municipal legislative formulation. From the relation resulting of bibliographical and documental sources, we propose to historically reconstitute how health services were deployed in the city of Campinas, given its economical relevance in the state of São Paulo. We highlight the city¿s and its inhabitants¿ behavior during this period and when the health policies were widely organized and affected the city¿s social arrangement. Abstract: Chagas disease (DC), discovered by Carlos Chagas in 1909, is a disease caused by the protozoan Trypanosoma cruzi. This disease affects millions of people in South and Central America. Nowadays, it is estimated that there are in Brazil between 1.9 million and 4.6 million people infected by this protozoan. In 1991, a campaign was launched to eliminate the vector transmission of the disease in the Southern Cone countries, which resulted in the reduction of acute cases of the disease. The objectives of the study were: to implant genotyping techniques and to classify T. cruzi isolates present in patients with Chagas disease treated by the Chagas Disease Study Group (GEDoCh); correlate with the clinical form and their origin, evaluate the blood culture results and evaluate the efficiency of serological kits. Four tubes were collected with 6mL EDTA for blood culture and a 4 mL dry tube for serum collection. Serological tests and genotyping were performed at the Laboratory of Diagnosis of Infectious Diseases by Techniques of Molecular Biology. Of the 54 samples collected, 26 (48%) were positive in blood culture. Samples of female patients obtained a higher percentage of positive blood culture, as well as in patients with cardiac clinical form and in patients treated with incomplete treatment with benznidazole. After genotyping in DTUs, the TC II was the most prevalent DTU, especially in patients with the cardiac form of CD. In the serological tests, the TESA-cruzi test solved a case of non-reactive serology and it was not possible to correlate the type of DTU found with the serological tests. Abstract: Current laboratory diagnosis of chronic Chagas disease relies on conventional serological tests but false-positive and false-negative results may occur. Hemoculture can also be used as a diagnostic method but its sensitivity in chronic phase is limited due to the low/intermittent parasitemia. Molecular diagnosis methods have been studied and the main technique that has been extensively tested is the Polymerase Chain Reaction (PCR). This study aimed to evaluate the qualitative and quantitative PCR performance for diagnostic purposes in patients with megaesophagus. We studied DNA blood samples from 26 patients with negative or inconclusive conventional serology for Chagas disease and from 33 patients with positive serology. The most suitable target for qualitative PCR presenting high positivity was the Sat-DNA of T. cruzi. When compared to the quantitative results, the concordance between the two molecular methods in both groups was 72.72%. In addition, we observed that all patients with positive results of hemoculture had positive qualitative PCR. The qPCR methodology although highly sensitive could not detect minimal amounts of T. cruzi DNA in one patient with positive hemoculture. Our results indicate a possible misdiagnosis of patients with megaesophagus given by the negative or inconclusive serology for Chagas disease.
