Abstract: The AIDS epidemic had an explosive growth, with high mortality and diminished survival until 1995-1996. After the introduction of triple antiretroviral therapy (ART) in 1996, there was a decline in mortality, as well as a significant increase in survival time. These advances have been maintained. However, with increased survival, co-infections such as hepatitis B and C have emerged, and the disease gradually gained more significance among poorer populations. In addition, inadequate living conditions and diseases like tuberculosis (TB) contributed to the new scenario. This study analyzes the effect of social inequalities, HIV/HCV and HIV/TB co-infections in the survival of patients and its risk factors. An evaluation was conducted to assess if these elements are limiting factors for the effect of triple ART. Methods: Non-concurrent prospective cohort study, with AIDS patients diagnosed in 1995-1996, grouped in 4 regions of Brazil. Datas included sociodemographic, epidemiological and clinical information, ART use, primary prophylaxis. The following criteria were used for the definition of cases: Brazilian AIDS case definition, 1 positive anti-HCV ELISA, positive HbsAg, positive baciloscopy and/or X-ray and/or biopsy and/or presumptive diagnosis in cases of TB. Results: A1 ¿ Most of the individuals were male, with low schooling, and heterosexual transmission. Among those with less education, there was a higher proportion of women, of patients under 25 years of age, of injection drug users (IDU) and heterosexuals. The poorest showed higher number of signs and symptoms and lower number of opportunistic infections, although TB was more frequent among them and they had less access to ART (RR=2,73). Survival of those with less educational attainment was shorter in all variables under study, with the exception of type of ART. In the Cox regression analysis, differences in survival were detected according to schooling, diagnostic criterion, number of symptoms, year of diagnosis and ART. A2- 29,5% of the 2821 patients had an anti-HCV test, while IDU and those with less education had higher testing frequency. The prevalence of co-infection was 33,4%, with a prevalence ratio (PR) of 1,54 for males, 2,76 for those with less schooling and 8,55 for IDU. Fever, anemia and asthenia were found to be associated with co-infection, as well as neurotoxoplasmosis. Survival, according to univariate analysis, was shorter in males, in those between 25 and 34 years old and in patients with CD4 T cell count >= 200 cells/ml. In the multivariate analysis, survival was not different between individuals with and without co-infection. A3- Prevalence of HIV/TB co-infection was 25,9%. The most affected were: males (76,3%), IDU (33,8%) patients with low schooling (32,6%) and those between 25 and 34 years old (28,9%). Typical TB signs and symptoms were more common in HIV/TB patients, especially fever (PR=1,49).
