Abstract: Chronic kidney disease (CKD) is associated with high morbidity and mortality rates, main causes related with cardiovascular disease (CVD) and bone mineral disorder (CKD-BMD). Advanced glycation end-products (AGEs) are uremic toxins that play a role on development of CVD and CKD-BMD. The measurement of skin autofluorescence (sAF) is a noninvasive method to assess the level of AGEs in tissue, validated in CKD patients. AIMS: Evaluate in hemodialysis (HD) patients the relationships among AGEs detected via sAF, serum and dietary intake, and metabolic and structural parameters of bone and cardiovascular disease, including vascular calcification (VC) evaluation. METHODS: An observational, transversal study that had included 32 patients undergoing HD at Hospital das Clínicas da UNICAMP and Hospital das Clínicas da Faculdade de Medicina de Botucatu; and 37 healthy subjects (Control Group). All subjects performed biochemical tests, ankle-braquial index (ABI) measurement by Doppler ultrasound, AGEs-Saf and quantification of dietary AGEs, carboxymethyl-lysine (CML) and methylglyoxal (MG) intakes by three-day dietary record. HD Group underwent hip, hands and lateral abdominal x-rays; transthoracic echocardiography; bone densitometry; serum CML; and semi-quantitative bone histomorphometric analysis. RESULTS: AGEs-sAF levels were elevated according to age in both HD and Control groups, however were higher in CKD patients (2.8 UA vs. 2.4 UA; p = 0.005). Dietary intake of total AGEs (11,943 ± 5,581 KU vs. 8,564 ± 3,702 KU; p = 0.016), CML (4,213 ± 3,941 KU vs. 2,560 ± 1,792 KU; p = 0.003) and MG (14,648 ± 10,902 nmol vs. 10,664 ± 5,480 nmol; p = 0.018) were higher at Control Group. Total AGEs ingestion was associated with lower values of cardiac ejection fraction (R = -0.451 ; p = 0.035) and higher ABI (R = 0.456 ; p = 0.038). The most part of patientes that underwent bone biopsy was diagnosed with osteoporosis (N = 12 ; 57%), and this group had lower total AGEs intake. The most frequent osteodistrophy tipe was osteitis fibrosa (OF) , wich was associated with higher CML intake and AGEs-sAF. CONCLUSION : Our study suggests AGEs association with sistolic cardiac disfunction, vascular stiffness and bone disorders related with BMD-CKD like OF and osteoporosis in HD patients. Abstract: OBJECTIVE: To evaluate how medical professionalism is redefined by beliefs, values and culture of the Brazilian population. METHODS: Initially, a review was carried out in the literature with terms "medical professionalism", "profissionalismo medico", "profissionalismo ou profissões em saúde", "profissionalismo e educação médica" from publications ranging from 1997 to 2017, accessing databases PubMed, Education Resources Information Center (ERIC) and Scientific Electronic Library Online (SciELO). Original data were collected in a two-staged mixed methods research for the construction of a Brazilian framework of medical professionalism competencies. The first stage used the e-Delphi method to obtain health education expert opinion. The second stage collected the perception of Unified Health System users and health professionals from different regions through Focal Groups (FG). Guests who accepted to participate in e-Delphi answered three questionnaires (Q1 to Q3). Q1 asked participants to indicate six important characteristics or behaviors of a good medical professional. Q2 requested the validation of the list of these attributes from attributes identified by more than 50% of the experts in the previous stage (Q1). Q3 selected and sorted the ten most important Q2 items. In all the questionnaires, panelists were allowed to express their views on the categorizations and textual composition of competencies. The FG of patients and health professionals occurred in five municipalities of different sizes, each in a region of Brazil, which were recorded and are under content analysis after transcription. FG data were compared to the categories extracted by e-Delphi consensus. RESULTS: The narrative bibliographic review identified a vast literature originating in European and North American countries, but few studies in other regions. Despite the lack of consensus, most publications include domains such as patient care, interpersonal and communication skills, personal and professional development. Ninety-two e-Delphi panelists answered the Q1 and 59% completed the three stages. Female respondents prevailed (55%) and male and female physicians were the main category of panelists (48% and 38%, respectively) Competencies were categorized into six domains, in descending order: "communication and collaboration skills", "commitment to technical performance", "commitment to ethics" "cultural competence", "reflective practice and self-knowledge" and "social responsibility". The number of communication and collaboration skills cited was higher than others were. The FGs of patients and health team professionals added other skills and behaviors such as "acceptance and appreciation of the companions", "appropriate use of information and communication technologies", "availability to answer questions and clarify issues of patients and companions", "identifies social health determinants".
