▼ Abstract: Myofascial pain (MP), caused by myofascial trigger points (MTrP), is of the main cause of headache and neck pain. It is associated with muscular stiffness, stress and can influence the quality of life of symptomatic patients. Ultrasound images of dysfunctional tissue have been used aiming to objectively evaluate the myofascia trigger point. Aiming to evaluate the effect of acupuncture and electroacupuncture on the treatment of MP at the upper trapezius muscle, sixty women aged between 18 and 40 years old, body max index between 19 and 30 Kg/m2, presenting at least one trigger point at the upper trapezius and local or referred pain for more than six months were randomized in to and three groups: electroacupuncture (EAC), acupuncture (AC) and SHAM-acupuncture (SHAM). Both examiner (CEEM) and volunteers were blinded to the treatments. The selected acupoints were VB20, VB21, F3 and IG4, besides a maximum of 2 "ashi points" in each upper trapezius. The effectiveness of treatment (eight sessions), as well as it maintenance till one month follow up, were evaluated concerning the intensity of pain (Visual analog scale: VAS), range of motion (fleximetry), quality of life (SF-36), stress (area under the curve to the ground: AUCG; cortisol awaking response: CAR) and ultrasound image (area of MTrP). Influencing factors and the menstrual cycle phases were monitored. According to its distribution, data were analyzed by pared t-test, Wilcoxon signed rank, ANOVA repeated measures, Friedman test, ANOVA or Kruskal Wallis. Pearson¿s correlation was applied. The level of significance was set in ?= 0.05. It was observed reduction on general pain (GP) on treated groups after all sessions (EA: P<0.001; AC: P<0.001). After treatment, intensity of pain on the right trapezius (RTPz) decreased in the EA (P<0.001) and AC (P=0.025) groups, but on the left trapezius (LTPz) it was only observed in the EA group (P<0.001). There was interaction of factor "time of evaluation" with factor "group" for GP in the AC and EA groups; and for pain at the RTPz and LTPz only in the EA group. There was increase on rotation to the left in the EA group (P=0.049) and on inclination and rotation to the right on group AC (P=0.005; P=0.032). There was increase on AUCG (P=0.006) and CAR (P<0.001) in the AC group. While SHAM presented significant increase only on bodily pain (P=0.005), EA and AC showed increase at the same following domains, with respectively P values: physical function (P=0.011; P=0.016), role physical (P=0.027; P=0.13), bodily pain (P=0.010; P=0.003), general health (P=0.017; P=0.011), vitality (P=0.010; P=0.011), mental health (P=0.018; P=0.014) and physical components (P=0.019; 0.002). It was observed a decrease on MTrP area (US) in the groups EA (TPzD: P<0.001; TPzE: P=0.001) and AC (RTPz e LTPz P<0.001), and in the SHAM groups only at the LTPz (P=0.036). ▼ Abstract: Persistent pain related to temporomandibular disorders (TMD) is recognized as the third most prevalent chronic condition of pain in the world. However, the factors involved in the transition from the acute to the chronic phase remain uncertain. Furthermore, there are subgroups of patients with TMD that are noresponders to treatment. A heuristic model of causal influences of this dysfunction has proposed two major intermediate phenotypes: psychological distress and pain amplification (hyperalgesia and allodynia), contribute to the TMD onset and persistence. Furthermore, for chronic care of patient with TMD is difficult to determine specifically what can or cannot work for every patient individually. Knowledge of persistent factors and treatment approaches that emphasize the flexibility and satisfy the individual necessities of these patients may represent a new direction in research for the treatment of chronic pain associated with TMD. Thus, the aim of this study was to investigate factors (psychological distress and pain amplification) related to the process of TMD chronic facial pain development. To do so, first the literature review was presented in this study, then we present two articles developed at the Piracicaba Dental School and also an article during an exchange program at the University of North Carolina, both with cross-sectional design (case-control studies), which address psychological distress factors and sleep quality in patients with different manifestations of TMD-related pain. First two studies compared TMD subgroups that were classified according to the presence or absence of widespread pain in order to evaluate quality of life domains and which components most affect the functional capacity of facial pain patients. Later, we also investigated possible correlations between the intensity of facial pain and depressive symptoms, and also, somatization. For data collection we used, respectively, the ShortForm-36v2® Health Surveys and RDC/TMD axis II history questionnaire. The results showed that patients with localized facial pain and patients with generalized body pain share impairments on emotional aspects. Functional capacity was only affected by the pain and the presence of widespread pain. Moreover, regardless of the group assessed, there was a positive correlation not only between facial pain and depression, but also with somatization. In the latest study, our aim was to investigate associations between sleep quality and signs of hyperalgesia and allodynia in patients with TMD and controls, respectively evaluated by the Pittsburgh Sleep Quality Index and the Sensory Qualitative Test. Our findings showed that poor sleep quality is associated with noxious thermal and mechanical stimuli (hyperalgesia), but it is not associated with allodynia. ▼ Abstract: The aims of this study were : (1) to assess physical (electromyographic activity) and psychological (anxiety) factors in subjects with temporomandibular disorders (TMD) and controls; (2) to investigate which of these factors is the most meaningful in the painful symptoms of these subjects and; (3) to seek painful, electromyographic and anxiety indicators able to make the distinction between these subjects. Forty-one volunteers with 18 to 42 years-old were evaluated and, were divided into 02 groups: (1) TMD group (n = 20) and, (2) Control group (n = 21) according to Axis I of Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD). State and Trait Anxiety levels were assessed with the self-applied questionnaire State and Trait Anxiety Inventory (STAI). The pain was measured with algometry, which allowed the quantification of palpation pain (DP) and the pressure pain threshold (LDP). The volunteers were submitted to an electromyographic examination in order to register the masseter and the anterior temporalis muscle activity during natural mastication. After the qualitative analysis of the electromyographic signal, duration (DUR), amplitude (Root Mean Square (RMS)) and frequency (median frequency (FMED)) of the contraction phase of each masticatory cycle were selected as the study variables. For statistical analysis, t test and Wilcoxon test were used for data comparison, the Pearson and Spearman partial correlation coefficient were used in the study of association between variables and, the logistic regression was used to evaluate the variables that have greater ability to distinguish between groups. As results of the physical factors, there were found lower mean averages of LDP and higher mean averages of DP in the TMD group, both indicating higher pain levels and, lower values in the left masseter muscle FMED of TMD group. In the psychological factors, state and trait anxiety levels were significantly higher in TMD group. There was not observed statistically significant association between pain and anxiety variables in the study of linear correlation and; when the association between pain and electromyographic activity was tested, there was found an increase in pain associated to greater amplitude of the right anterior temporalis muscle. When there was investigated which variables had greater ability to distinguish between groups, the State-Anxiety and FMED were the most significant. This study concludes that there is difference in perception of pain in masticatory muscles of women with TMD and controls, and that pain may relate to increased activity of the anterior temporalis muscle. There were no associations between pain and anxiety, but state anxiety was found to have a great ability to distinguish between subjects with the presence or absence of TMD, suggesting the importance of evaluating not only physical but psychological factors in these patients.

▼ Abstract: Temporomandibular Dysfunction may affect the masticatory muscles, temporomandibular joint (TMJ) or both, has high prevalence in women and most common symptoms is pain. It was proposed to determine minimal effective concentration of piperine to activate the transient receptor potential vanilloid subfamily 1 (TRPV1) in the right TMJ in Wistar female rats under the conditions: normal, chronic inflammation in the TMJ, chronic stress and inflammation and stress combination. It was developed two randomized double-blind experimental studies (n. 2633-1). In study I, 48 animals were randomly divides into six groups, each group received 30 ?l into the TMJ one of this solutions: standard solution (80% sterile saline, 10% Tween 80 and 10% ethyl alcohol) or 1, 2, 3, 4 and 5 ?g of piperine diluted in 100 ml of standard solution. In study II, 144 rats were randomly divided into groups: A - chronic inflammation in the right TMJ induced by Freund's Complete Adjuvant; B - chronic stress caused by chronic stress model; C - association of these conditions. These groups were divided (n = 8), and it was injected into the TMJ 30 ?l of the same solutions as previously describe. In both studies, rats in diestrous phase of the hormonal cycle, after injection of the solution, were assessed for nociceptive behavior, which consisted in quantify how many times the rats flinched its head and the time spent, in seconds (s), to rub the orofacial region. Each head flinch followed uniform pattern of 1 second duration, and the behaviors were expressed as time function, allowing the sum. For statistical comparison, it was used variance analysis and Tukey-Kramer (P<0.05). In study I, there was a significant difference for rubbing orofacial region between groups of 2 ?g and 5 ?g (100,37 ± 63,81 s, 100 ± 60,5 s, respectively) and control (33,37 ± 18,48 s), and the sum of the behaviors between 2?g (130,87 ± 257,88 s) and control (62,75 ± 14,81 s). In study II, in group A, there were statistical differences for head flinch behavior between 5?g (69,5 ± 16,44 s) and control (41,13 ± 15,06 s) groups; for rubbing orofacial region between 4?g (51,5 ± 28,73 s) and control (14,71 ± 7,54 s), and for the sum, among 4 and 5?g (105,37 ± 22,64 s, 115,50 ± 35,14 s, respectively) and controls (52,86 ± 17,46 s). In group B, for head flinching behavior there was significant difference between 4 ?g (85,87 ± 19,21 s) and control (49,87 ± 10,7 s); rubbing orofacial region, between 5 ?g (48,25 ± 27,25 s) and control (3,.75 ± 12,69 s). In group C, there was no significant difference in the subgroups. Thus, 0,02 ?g/ml of piperine showed the lowest concentration effective to cause noxious stimulation in normal condition, the solution of 0,04 ?g/ml of piperine, for groups of chronic joint inflammation and chronic stress and for association conditions group there was not statistical significant difference

▼ Abstract: Catecolamine-O-methyltransferase (COMT) is an enzyme with broad biological functions, including pain modulation, exerted through metabolization of substrates such as dopamine, epinephrine, and norepinephrine. It is well known that COMT activity is genetically polymorphic in humans, and correlates to individual pain perception and efficiency in its remission among temporomandibular disorder (TMD) patients treated with propranolol. Thus, our first aim was to investigate new single nucleotide polymorphism (SNP) that might mark for this benefit. Indeed, we have found a new SNP, rs165774 (G>A), that was associated with TMD or intermediate phenotypes in two different cohorts. This polymorphism, in turn, is in close proximity to a second SNP, rs165895 (T>C), both in the 3' untranslated region (3'UTR) of an alternative COMT mRNA that has not yet been characterized. Therefore, we also aimed to (i) verify the expression of this transcript in different human tissues and cell systems, tracking it through RT-PCR (Reverse Transcriptase - Polymerase Chain Reaction); (ii) predict the crystalline structure of the enzyme encoded by it, modeling it with discrete molecular dynamics; (iii) express it in a cell system and compare its relative expression and enzymatic activity to the conventional isoform, using RT-PCR and HPLC (High Performance Liquid Chromatography), respectively; and (iv) verify the effects of SNPs rs165774 and rs165895 on the transcript, creating different mutants by site-directed mutagenesis and observing their effects on the relative expression and enzymatic activity. Our findings show that (i) the alternative COMT transcript is expressed in different human tissues and cell systems; (ii) the crystalline structure of its enzyme exhibits a unique C-terminus that is distinct from the conventional isoform; and that this transcript is (iii) less relatively expressed and its enzyme is less active than the conventional transcript and enzyme, respectively, and (iv) is regulated, at transcriptional level, by the SNPs rs165774 e rs165895. With this study, we conclude that SNP SNPs rs165774 is a strong genetic marker for TMD; that along with SNPs rs165895, they are located in the 3'UTR of an alternative COMT mRNA which proved, for the first time, to be able of encoding an alternative isoform of the enzyme that exhibits enzymatic activity; and that this alternative COMT mRNA is regulated, at transcriptional level, by both SNPs

▼ Abstract: Temporomandibular disorders (TMD) are common chronic orofacial pathology conditions characterized by pain in the temporomandibular joint and/or muscles of mastication. The TMD is a common pain condition, especially in women during their reproductive phase. Thus the objectives of this thesis were (1) to evaluate whether the hormonal fluctuations that occur during the menstrual cycle alter pain sensitivity (PS), maximum bite force (MBF) and masticatory performance (MP) to volunteers with TMD, (2) evaluate if jaw movements and sleep quality would change with treatment with stabilizing occlusal splint and (3) compare the diagnosis based on the Diagnostic Criteria for Temporomandibular Disorders (DC / TMD) to evaluate the reliability and calibration between formal training in relation to self-instruction with documents and film. For this purpose, fifty volunteers were selected and divided into two groups of 25 participants: (1) volunteers with regular menstrual cycles and TMD, and (2) voluntary with TMD and using oral contraceptives. Pain sensitivity (PS) by using a visual analog scale, maximum bite force (MBF) using sensors and masticatory performance (MP) through screening were analyzed. These variables were measured for a period of four phases of a complete menstrual cycle. The same 50 volunteers diagnosed with PS participated in the second study, pain intensity (visual analogue scale), quality of sleep (Epworth Sleep Scale (ESS) and Index of Pittsburgh Sleep Quality (PSQI) were analyzed, and mandibular movements - range of motion and chewing movements (kinesiography) before and after two months of treatment with stabilizing splints. Repeated measures GLIMMIX were used for data analysis followed by Tukey¿s test (P ? .05). For the third study a different sample was used, a total of 32 patients (27 patients with TMD and 9 asymptomatic) were examined using DC/TMD. Six examiners were divided into two groups with different training, (1) formal training and calibration at a DC/TMD training center and (2) self-instruction with the use of documents and video. After the first evaluation, the second group went through the formal training, considered the group (3) self-instruction + course. The reliability assessment was performed over a full day comprising in a clinical examination of 16 patients for each of the three groups. The Cohen's kappa coefficient was used to calculate the reliability of the DC/TMD diagnoses. Regarding the results, differences between the PS in luteal phase (Fourth evaluation) and ovulatory phase (third evaluation) were found (p = 0.01) in both groups. No differences in MBF (P = 0.34) or MP (P = 0.43) were found between the experimental and control groups. Sensitivity to pain was reduced after treatment (P = 0.0001). The treatment improved range of motion, increasing mouth opening (P = 0.0001) and anterior-posterior movement (P = 0.01) and the maximum opening speed (P = 0.0001) and closing (P = 0.04) during mastication after treatment.
