Abstract: This study aimed to investigate the relationship between the engagement in advanced activities of daily living (AADL) and cognitive performance in community-dwelling seniors. The data presented is drawn from the population-based study Frailty Profile of Elderly Brazilians (FIBRA-Unicamp). The sample comprised 2.549 older adults without cognitive impairment suggestive of dementia, and 65.71% females, mean age of 72.32 years (±5.55) and education of 4.37 years (±3.99). Information on sociodemographic characteristics were collected (gender, age, education and family income) and health conditions (number of diseases and depressive symptoms). The self-reported AADL were grouped into physical, social and intellectual. Cognitive performance was assessed using the Mini Mental State Examination (MMSE), considering the global score and guidance fields, memory, attention and calculation, language and constructive praxis. The mean scores of MMEE were significantly higher among men, younger individuals and those who had more education, higher income, less related diseases and had no depressive symptoms. The analysis of multivariate linear regression and hierarchical regression analysis performed in blocks (sociodemographic variables, health and physical, social and intellectual AADL) have shown that engaging in intellectual AADL were positively associated with total MMSE score (p=<0.01, R2=0.014 and p=<0.01, R2=0.013 respectively). Regarding cognitive domains, though modest, multivariate regression analysis showed an association between the intellectual AADL and guidance fields, attention / calculation, language and constructive praxis (p=0.017, R2=0.005; p=<0.042, R2=0.008; p=<0.001, R2=0.021 e p=<0.001, R2=0.021 respectively). Social AADL joined the domains of memory (p=0.024, R2=0.002) e language (p=0,023, R2=0.004). There was no association between engaging in vigorous physical AADL and cognitive performance. Years of education and family income were the variables associated more robustly to the total MMSE score and your domains (except only between family income and memory). The results suggest that engagement in social and intellectual AADL may have a protective role in cognitive aging and engaging in AADL may represent a viable strategy for promoting mental health among the elderly Advisors/Committee Members: UNIVERSIDADE ESTADUAL DE CAMPINAS (CRUESP), Yassuda, Mônica Sanches (advisor), Universidade Estadual de Campinas. Degree: 2015, Universidade Estadual de Campinas

Abstract: The purpose of this study was to investigate the association between depressive symptoms and self-reported chronic diseases and health problems among community-dwelling elderly. The data were collected between 2008 and 2009 as part of the Frailty of Brazilian Elderly (FIBRA) Study, a population-based, cross-sectional descriptive investigation on frailty in Brazilian elderly. The sample consisted of 2,546 elderly people who were evaluated for depressive symptoms using the 15-item Geriatric Depression Scale (GDS-15). People with scores of 6 or above were considered to have depressive symptoms. They were living in seven Brazilian urban areas: Campinas (SP), Belém (PA), Parnaíba (PI), Campina Grande (PB), Poços de Caldas (MG), Ivoti (RS) and Ermelino Matarazzo - District of São Paulo (SP). Descriptive statistics and stepwise univariate and multivariate logistic regression analyses were performed. Significance level was set at 5% (p?0.05). Of the total sample, the average age was 72.3 years (SD = 5.5) and 65.8% were women. The GDS-15 scores ranged between 0 and 15, with a mean of 3.5 (SD = 2.7) and the prevalence of depressive symptoms was 20.5%. There was a significant difference (p<0,001) in the prevalence of depressive symptoms between genders. Hypertension (62.2%), arthritis (43.6%), memory difficulty (54.5%), fear of falling (57.3%) and sleep problem (48.2%) were the most frequently reported events. Sadness was 3.16 times more likely to be associated with depression (95% confidence interval [CI] 2.48-4.03). The GDS-15 do not specifically evaluate the sadness, as it happens in the complete version of the GDS-30. Understanding the signs, symptoms and causes is the important step to overcoming the difficulties to differentiate when sadness is related depression in older people. Elderly at higher risk of depression were those who reported arthritis, sadness, urinary incontinence, fecal incontinence, bed-confinement at home due to illness or due to surgery and aged between 75-79 years Advisors/Committee Members: UNIVERSIDADE ESTADUAL DE CAMPINAS (CRUESP), Guariento, Maria Elena, 1955- (advisor), Neri, Anita Liberalesso, 1946- (coadvisor), Universidade Estadual de Campinas. 3. Pereira, Alexandre Alves, 1976-. Comparação de duas definições de fragilidade na avaliação de risco para morte em idosos participantes do estudo fibra Campinas: Comparison of two frailty definitions in risk assessment for death of elderly people in campinas fibra study.

Degree: 2016, Universidade Estadual de Campinas

Abstract: Objective: To Compare two operational definition of frailty in risk assessment of death in community-dwelling elderly people. Methods: We used two databases. The first was the database of the FIBRA study (Fragilidade em Idosos Brasileiros), that was composed of 900 older adults resident in Campinas. The second was the database of the Mortality Information System (SIM) of Campinas that was assessed every 6 months from 2009 to 2013. There were included 689 older adults (72.1 ± 5.3; 68.8% women) who scored above the cutoff point for the Mini-Mental State Examination. We used two operational definitions to evaluate frailty: the frailty phenotype (exhaustion, unintentional weight loss, weakness, slowness, low activity) that identifies the frailty by the presence of three or more of the components, and the frailty index as a result of deficits accumulated by the individuals along their life course. The statistical analysis included Chi-square, Mann-Whitney and Kruskal-Wallis tests to compare categorical variables; Cox regression to evaluate the association between frailty and mortality, and Kaplan-Meier survival curve analysis. Results: The incidence of mortality was 8.2%. The prevalence of frail elderly according to the frailty index was 38.8%, the pre-frail were 51.6% and those who were robust were 9.6%. The average frailty index was 0.23 ± 0.10, ranging from 0.03 to 0.62. The average value was higher in women than in men. There was no association between frailty index and mortality, but being male and being age 75 and older corresponded to higher risk to death. The prevalence of frail elderly was higher in the frailty index model than in the frailty phenotype model According to the frailty phenotype, frail older adults had higher risk of death than did the non-frail. Conclusion: The frailty phenotype model was a better predictor of mortality in community-dwelling elderly than the frailty index model did. The comparison of operational definitions of frailty can provide insights into the accuracy in predicting adverse health outcomes in elderly people in different populations Advisors/Committee Members: UNIVERSIDADE ESTADUAL DE CAMPINAS (CRUESP), Neri, Anita Liberalesso, 1946- (advisor), Neri, Anita Liberalesso (advisor), Universidade Estadual de Campinas.
