▼ Background: The burden of End Stage Renal Disease (ESRD) is a growing concern worldwide. According to the United States Renal Data System for 2010, the prevalence and incidence of hemodialysis in Taiwan is the highest in the world. Hemodialysis patients commonly have complications, including osteoporotic fracture-associated diseases, which affect the quality of life.
Objective: This study aimed to explore the incidences of various osteoporotic fractures and related risk factors.
Methods: A nationwide prospective study based on the Taiwan National Health Insurance database was conducted during 2001â2007. The study analyzed the incidence of osteoporotic fractures and medical resource utilization in ESRD patients. The International Classification of Diseases (ICD) 9-CM diagnosis codes and treatment codes were identified as the inclusion criteria for osteoporotic fractures. Data were statistically analyzed by means of descriptive analysis, independent t-test, Chi-square test, and logistic regression.
Results: A total of 4016 Taiwanese people with ESRD in hemodialysis were identified in 2001, and 729 injuries occurred during the 6-year follow-up period. The cumulative incidences of lower limb fractures, upper limb fractures, and vertebral fractures were 6.94%, 5.60%, and 4.75%, respectively. Hemodialysis patients with and without osteoporotic fractures were significantly different by sex and geographic region. A higher incidence of fractures was observed in females than in males.
Conclusions: In conclusion, the information regarding clinical symptoms was not available from our database because the National Health Insurance database has ICD 9-CM diagnosis codes only. This study also confirms the strong influence of dialysis treatment on all types of orthopedic injuries and osteoporotic fractures. We hope to present the risk factors as an important point through which to notify hemodialysis patients to prevent orthopedic injuries and osteoporotic fractures, so that these patients will regularly bone density measurements. Therefore, policy makers should take the prevention of osteoporotic fractures in ESRD patients into consideration.

▼ Background: Diabetic retinopathy is one of major causes of visual disturbance and blindness in the patients with diabetes. Early treatment for retinopathy could be achieved by early detection through eye examinations and reduce the patientâs visual loss. However, the rate of eye examinations was low in the patients with diabetes in Taiwan. Itâs needed to further study about the reasons for poor compliance of eye examinations in the patients with diabetes.
Objectives: To estimate compliance of eye examinations in the patients with diabetes, to examine factors that affect compliance, and to determine reasons for compliance and non-compliance.
Data source and Sample size: Data were obtained from the 2009 Nation Health Interview Survey (NHIS) in Taiwan. A total of 1040 patients with diabetes 12 years of age or older were enrolled in this study.
Methods: A cross-sectional population study. The Andersen model was used as the analytic framework, and all variables were categorized into three factors: predisposing, need, and enabling factors. The chi-square test, independent-sample t test, and logistic regression were used to analyze the association between these variables and the use of eye examinations.
Results: Of all patients with diabetes, only 45.2% had an eye examination in the past years. By logistic regression, the probability of an eye examination among the patients with diabetes increased with checking sugar level themselves, taking care of their feet, having attended diabetes support group, not living in a rural area, having eye symptoms, having longer duration of diabetes, and taking insulin for sugar control. Hakka people and patients with frequent hospitalizations were less likely to use eye examinations.
Conclusion: The study results showed that the utilization rate of eye examinations was low. Itâs important to improve the utilization of eye examinations through education about diabetic retinopathy and eye examination. We must aggressively transfer the patients with no any symptoms of eye, early stage of diabetes, mild degree of diabetes and frequent hospitalization to ophthalmology clinic for eye examinations. At last, itâs also needed to resolve the problems of insufficient resources of ophthalmology in a rural area.

▼ With the initiation of national health insurance, the ecology of medicine and organization of hospitals in Taiwan underwent rapid change. The beginning of a global budget aggravated the impact of hospitals, especially the district hospitals which were already facing most of the pressure. Over half (51.61 %) of the above district hospitals chose to focus on long term care, nursing homes making up the great majority.
This research evaluates the efficiency of two district hospital-based nursing homes by Data Envelopment Analysis (DEA). The purpose of this research includes:
First, evaluate the efficiency of two district hospital-based nursing homes, including overall efficiency, purely technological efficiency and scale efficiency. Second, assess the relative efficiency relationships of the two nursing homes. Third, probe the differences between relatively efficient and relatively inefficient groups in input variable and output variable. Fourth, with the analysis results, suggest adjustments of input and output to every relative inefficiency groups, as the health care manager's reference.
The research approach is to consult relevant documents and the purpose of this study is to choose input and output variables. These variables are screened by Pearson analysis for isotonicity relation. Process the study by DEA for relatively efficient analysis, then direct against the relatively inefficient groups for discussion of efficiency reference set and slack variable analysis.
The result of study shows:
First, overall efficiency: There are 6 relatively inefficient groups (37.5%) in the first nursing home; There are 8 groups relatively inefficiently attended to (50%) in the second nursing home; There are 18 groups relatively inefficient (56.25%) in the combined first and second nursing home groups.
Second, purely technological efficiency: There are 4 groups relatively inefficiently attended to (25.0%) in the first nursing home; There are 5 groups relatively inefficiently attended to (31.25%) in the second nursing home; There are 15 groups relatively inefficient (46.9%) in the combined first and second nursing home groups.
Third, scale efficiency: There are 6 groups relatively inefficiently attended to (37.5%) in the first nursing home; There are 8 groups relatively inefficiently attended to (50%) in the second nursing home; There are 18 groups relatively inefficient (56.25%) in the combined first and second nursing home groups.
Fourth, the slack variable analysis: the input items that must improve are foreign caretaker labor hours, nurse assist labor hours and nurse labor hours into sequentially, the output one is most important in increasing the income range.

▼ Background: Informed consent is the autonomy of the patient who involves decision-making after being informed of and understanding the physician.
Objectives: This study sought to investigate and compare patientsâ and physiciansâ perceptions of informed consent and the factors that influence their viewpoints on informed consent.
Methods: The subjects were citizens without medical background and physicians working in four hospitals. Self-structured questionnaire was used to gather information. The physicians were recruited from two medical centers, a regional hospital and a district hospital in Kaohsiung. The data is analyzed by SPSS 14.0 and used descriptive statistics, item analysis, factor analysis, chi-square test, Fisher's exact probability test, t-test, and ANOVA.
Results: The response rates were 97% in citizens (n=891) and 79 % in physicians (n=158). As for the concept of informed consent, 55.7% citizens thought that doctor should respect the opinion from the patient and family during the informed consent process. Furthermore, 91.2% the citizens prefer to know the information about their health condition from the physicians rather than from family (8.8%). Regarding the sequence of informing the physical condition, 29.9% citizens also prefer directly from the physician. As for the decision-making process, 55% respondents thought that patientâs own decision is the most important. 52.5% physicians thought that doctor should respect the opinion from both the patient and family and the bad news should be informed by physicians (88.0%) rather than by family. As for the sequence of knowing the bad news, 46.2% physicians preferred to inform family first and inform patient after discussing with family. In decision-making process, 48.7% physicians thought that physicianâs opinion was still more important than opinion of patients and family. Further analysis revealed that patientsâ gender and the level of family visited hospital have significant difference on the perception of informed consent (p=0.027; p=0.000); gender, age, educational background and living locations also have significant difference on âwho to deliver the bad news.â (p=0.006; p=0.004; p=0.035; p=0.012); Citizensâ age, educational background and career have significant influence on their opinion of informed consent of recently visiting doctor (p=0.014; p=0.006; p=0.001). The variables in the physiciansâ background have no relation with the means of informing and decision-making. The gender and position of the physician have significant effect on the opinion of practice of informed consent (p=0.015 and p=0.001).
Conclusions: We concluded that the perceptions of informed consent, there was no difference between physicians and citizens; however the citizen chose the patient first, and the physician chose the family first during the process of informed consent. Physicians have better perceptions of informed consent than the common citizens.

The purpose of this research is (1) to find out the overall trends of the common avoidable hospitalization diseases in Taiwan since the implementation of the National Health Insurance (NHI), (2) to compare the different distributions of the common avoidable hospitalization diseases across age groups, genders, regions, and the hospitalâs characters, (3) to make the related useful information and suggestions of improving avoidable hospitalizations to the policy makers, hospitals, and the general public.
Methodï¼
Standardized rates of avoidable hospitalization from 1997 to 2004 were derived from the National Health Insurance database and the Taiwan census data. In this research, we focus on the four main diseasesï¼Asthma, Pneumonia, Diabetes, Hypertension as the avoidable hospitalization diseases. Logistic regression and chi-squire tests are used to detect and describes trends in data.
Resultsï¼
During the 8-year research period, AHCs (Avoidable Hospitalization Conditions)of Asthma and Hypertension decline overall, conversely, AHCs of Pneumonia and Diabetes show increasing trends overallï¼in the hospital characters aspects, AHCs are mostly occurred in the local hospitalsï¼from the viewpoint of the patientsâ characters, AHCs of Asthma and Pneumonia are always occurred in children male, on the other hand, AHCs of Diabetes and Hypertension are always occurred in female and above 65 years oldï¼in the regional aspect, the AHCs of Asthma and Pneumonia are comparatively higher in east Taiwan.
Conclusionï¼
According to the results of this research, we can find that the avoidable hospitalizations in Taiwan still need to be improved. We suggest the policy makers and the hospitals may use the ãavoidable hospitalizationãas an indicator to monitor the quality of care and the outpatient care quality in Taiwanâs healthcare system. Thus can not only reduce the unnecessary expenditure, but also can offer the general public better quality of healthcare and improve their quality of life.

With the carrying on the step of the society and the changing of the health care insurance system, the medical behavior has changed. The health care organization should not only focus on professional field, but also looking for operating forever.
Objectiveï¼
1.To understand the recent condition of health care organizational ethics in others counties by paper research.
2.To develop the questionary of health care organizational ethics.
3.To understand the recent condition of health care organizational ethics in Taiwan by questionary, and compare the importance and feasibility.
Methodsï¼
The target of this questionary includes clinicians, nurses, the staff of medical laboratory and pharmacist, administrators and others. The location of the questionary includes medical center, territorial hospitals, regional hospitals and primary hospitals. The program SPSS 13.0 will be used for descriptive statistics, factor analysisï¼relability test and t test in this research.
Result and Conclusionï¼
There was significant difference in the intervieweeâs consideration between importance and feasibility of health care organizational ethics survey, whatever in every aspect or item. Classify all interviewee by different level of hospital but same position, or same level of hospital but different positions, there was also significant difference in the intervieweeâs consideration of importance or feasibility by different level of hospital or positions, except nurses.

▼ The purpose of this study is to evaluate the health related quality of life (QoL) of head and neck cancer (HNC) patients with cancer-free survival after treatment and to investigate the factors correlated with their health-related QoL. The European Organization for Research and Treatment of Cancer (EORTC) Core Quality of Life Questionnaire (QLQ-C30) questionnaire and the EORTC head and neck cancer-specific questionnaire (QLQ H&N-35) were self-administered by all participating patients. Sociodemographic data were collected using a questionnaire specifically designed for the study and through the medical chart reviewed. The patients who have been treated radiotherapy in our department of radiation oncology were invited to participate Health-related quality of life was assessed 3 months or later after completion of radiotherapy. Data relating to sociodemographic factors, disease characteristics and treatment factors of HNC survivors were analyzed. A total of 141 head and neck cancer patients completed the questionnaires. After data analysis, the results are described as follows: In the results of EORTC QLQ-C30 questionnaire, the score of âglobal health statusâ is 69.34. Most of the patient could maintain good functional quality of life. âFatigueâ is the most impaired symptom score in the patient. In the results of EORTC QLQ-H&N35, the three head and neck specific symptom scores indicating the most impaired in QOL were âWeight lossâ, âNutrition supplementsâ and âdry mouthâ. After analysis through the multiple regression model, we found sex, age, tumor subsite, radiotherapy technique and surgery were the independent factors significantly correlated with the QoL scales
This study is helpful for us to understand the QoL status and the factors affecting the QoL for patients with head and neck cancer after radiotherapy.

▼ The purpose of this research aims to (1) construct the predict model by system dynamics on the outpatient visits; (2) investigate the influence on hospital performance after drug price policies made; and (3) offer feasible schemes by scenario simulation. National Health Insurance (NHI) system intends to safeguard the right to health care for all residents in Taiwan. NHI system has gained high public satisfaction so far. However, medical expenditures and pharmaceutical costs have propelled the rapid growth in recent years because of the stipulation of the insurance premium rate, the general push for improved health care quality, and inclusion of new drugs as well as new technologies. To prevent the deficit of the NHI systemâs finances from widening further, the Bureau of NHI is urged to reform the reimbursement policies, including Adjustments of Drug List and Payment Scheme, Global Budget Payment System, and Adjust Reasonably Reimbursement Criteria as well. Based on the statistics, a quarter of the total revenues were for the drug cost. Legally mandated drug price adjustments have a great influence on the financial burden of the medical institutions.
On the basis of the simulation analysis, the major findings of this research are summarized as follows: (1) The adjustments of the prescription depend on the amount of the profit gained from the drugs; (2) The increase the refillable prescriptions for patients with chronic illnesses enhances the profit of the drugs; (3) With the implementation of the system dynamics, the counterintuitive phenomenon can be identified and used to guide the future policies of the medical institutions. This research also suggests that the total comprehension of finding the leader loop in the dynamic complexity system within the management of the medical institutions is beneficial in providing the relative adjustment policies for a hospital.

▼ Patients of stoma caused by the colorectal disease or trauma are increasing and most of the patients are keen to have their stoma closed as early as possible to lower the burdens in their life. However, stoma often associated with complications such as wound. The interval between constructing and closing of stoma is still controversial.
This study will figure out a way to predict an appropriate timing of stoma closure to get better outcome by using NHI database, from January 1, 2007 to December 31, 2008, in Taiwan.
In 463 stoma closure patients, mean age: 65.94 years, ratio of male to female: 1.66, 70.19% Colostomy, 29.81% Ileostomy, mean timing: 156.73 days, mean length of stay: 10.81 days, mean fees: 57,698.06 TWD, only one death in hospital, 35 readmission within 30 days after discharged; with α = 0.05 level, the timing is significant in comorbidity, primary diagnosis; the outcomes are associated with the timing of stoma closure, comorbidity, patientsâ age and gender, hospital ownership and physician annual volume.
Through this study, a perfect timing of stoma closure should be determined by the characteristics of patient and physician. Physicians could predict the timing through the stratified analysis and well organize the resource of hospital to improve the quality of medical care.

▼ Objectives: Study wanted to explore the factors that will affect the total medical expense in the patients who receive laparoscopic cholecystectomy (LC). We also to confer the influencing factor that will associate with the difference of reports the expense under the case payment system.
Methods:Retrospective study . Collected from year 2003 to 2007, received LC in a general teaching hospital in Kaohsiung city. We also adopt the chart review and combined with the health insurance expense data to explore the important factors that were associated with total hospitalized expenses, declaration of expense differences, and profits. The methods of multiple linear and logistic regressions were needed.
Results: 1539 subjects, 613 male and 926 female. The average age was 54.4 , and 1313 subjects were hospitalized from outpatient. All subjectâs average hospitalized days were 3.79 and medical expenses were 42528.1 dollars. The frequencies of the type of declaration about ânot exceedâ, âexceed but actuallyâ, and âexceed but no actuallyâ were 88.8%, 8.6%, and 2.6%, in sequence. The average declaration of expense differences was 14484.1 dollars. The significant factors that were associated with total hospitalized expenses were the age, surgical year, source of hospitalize, major symptom, combine disease, a complication after surgery, hospitalized days, type of declaration. In the other linear regression model, we found the age, surgery year, source of hospitalize, major symptom, high technology examination before surgery, combine disease, a complication after surgery, hospitalized days, and physicianâs surgery quantity per year had been statistically significant with the declaration of expense differences. For the odds of hospitalâs profits, the significant factors include the surgery year, source of hospitalize, major symptom, high technology examination before surgery, and hospitalized days.
Conclusion: We found a few significant factors that were associated with dependent variable in three regression models in this study. The major factor is hospitalized days that were a stronger influence total hospitalized expenses, declaration of expense differences, and hospitalâs profits. The hospitalâs superintendent can carry on the management through the appropriate method to control the medical resource consumes.

▼ Background: Tue Businessweek magazine proposed lists of attending doctors in medical centers and regional hospitals based on the recommendation of hospital presidents, directors and supervisors of medical societies, and surveyed doctors. The current study aims to investigate the criteria existing in Businessweekâs selection and factors related to the nomination of Taiwanâs best 100 doctors. Material and Method: Subjects are the 667 doctors nominated in Businessweekâs â100 Especially Recommended Doctorsâ and â600 Doctor-Recommended Doctors,â published respectively on May 5th and October 3rd, 2008. Biographical data of the subjects is firstly reorganized according to such items as specialty, hospital, education, professional training, experience and current status; introduction to specialties on websites of the hospitals where the subjects serve is also taken into consideration during data reorganization. Secondly, additional social network variables, including special positions in such non-hospital and non-school institutions as societies, foundations and publishing companies, are implemented. Collected data is analyzed by means of descriptive statistics, chi-square test and logistic regression. Results: Out of the 667 doctors, the 131 ones especially recommended by Businessweek tend to either serve in northern Taiwan or have graduated from National Taiwan University. In terms of social network, âdirectorsâ of a department/branch/center are 3.082 times more likely to be nominated among the 131 than those who do not have the title, while those with special positions in medical societies are 5.314 times more likely to be nominated than those without any special position. Conclusion: The physicians not only training in the medical profession, but also need to expand social network.

Ventilator-dependent patients have a great and far-reaching impact on families. Their primary caregivers, who are usually the ventilator-dependent patientsâ families, have played important roles during the caring process. Those caregivers are burdened with a lot of responsibility and pressure. It is crucial to find out how to assist the caregivers within selected methods of caring and to provide them with suitable needs in order to reduce their psychological burden. Therefore, the purpose of this study is to explore the related factors and to compare the burden and the needs of the primary caregivers in respiratory care ward and home care on long-term ventilator-dependent patients.
Designï¼
The survey adopts cross-sectional and purposive sampling by using a structured questionnaire to collect information. Both Burden and Needs Questionnaireâs reliability is respectively 0.923 and 0.943; their KMO is 0.828 and 0.829 ; their Expert Validity (CVI) is 0.88 and 0.91. The questionnaire has reached the ideal value of reliability and validity. The sampling subjects are from respiratory care ward and home care centers in southern Taiwan. Total 260 out of 280 questionnaires are returned (the response rate has reached 92.9%) and there are 241 valid samples. The questionnaires are used for data collection and are divided into four sections; âpatient basic informationâ , âfamily basic informationâ, âcaregiver burden scaleâ, and âcaregiver need scale â. The collected data is analyzed with descriptive statistics-test, one-way ANOVA, Pearsonâs correlation and multiple regressions by using SPSS Windows version 12.0.
Resultï¼
(1) Patients from Home-care centre are normally younger in comparison with the patients from respiratory care wards; their average usage of mechanical ventilator is 30.98 month longer; they tend to have more conscious and tracheotomy patients. According to the demography, these two groups of the patients have a significant difference. (2) The average age of primary home care and respiratory care ward caregivers is 50â¼52; most of them are female; most of the care givers are the patientsâ children. (3) "Physiological burdens" and "physiological needs" of home care centers caregivers are greater than the caregivers in respiratory care ward. They have reached a significant difference in statistic. (4) The primary caregivers have a declining health, and their workload and demands are increasing. (5) There is a significant positive correlation between the primary caregiversâ burden and needs. (6) Base on the numbers of admitting to the ICU, the hospital and the current health status of the primary caregivers; the workload is predictable.
Conclusionï¼
Hopefully the result of this study can provide the Government to formulate a long-term-care insurance. Focus on providing the services to different needs for the caregivers; reduce the caregiversâ burden on caring the mechanical ventilator patients. The purpose of this study is to explore the relationship of patientsâ medication adherence with the statins therapy and inpatient visit the in Taiwan.
Methodsï¼
A corhort study was implemented to use the National Health Insurance Research Databaseï¼NHIRDï¼between January 2001 and December 2007, to analysis the patient who had taken the medicine of statins at the first time and never had diagnosis of coronary heart diseaseï¼CHDï¼. The patient were followed until occurred first inpatient visit with coronary heart disease or end of study periodï¼Dec. 31 2007ï¼. The medication adherence was calculated as medication possession ratioï¼MPRï¼which was sum of days of supply of the statins medications divided by follow-up duration.
Resultï¼
In the analyzed of 19371 patients who use statins therapy, 11642 patientsï¼59.17%ï¼had high medication adherenceï¼MPRâ§80ï¼ï¼and the rest patientsï¼40.83%ï¼had been included in low medication adherenceï¼MPRï¼80ï¼ï¼. There were significantly higher adherence for patients were male, elderly, CCI=1, in medical center and in rural area. The hospitalized risk increased with MPR<80 %, male, elderly, and more comorbidities. The hospitalization costs showed comorbidities with MPR<80 %, male, elderly, more comorbidities, patient in medical center, teaching-hoapital and suburban.
Conclutionï¼
Lipid-lowering therapy, statins medication is well known to reduce cardiovascular risk across a wide range of patients. It is very important to follow-up and compliance with statins therapy. This study finded that good adherence to statin treatment was associated with lower risk of occurred CHD.

▼ In this study, we investigated the factors influencing the preference of the public for self-paid health examination centers. We employed the questionnaire survey method, used citizens of Chiayi City as the subjects, and recruited people exercising at the Chiayi City Park. Of the 400 questionnaires distributed, 378 were recovered; however, 12 invalid questionnaires containing incomplete or uniform responses were eliminated, resulting in 366 valid questionnaires (91.5% valid recovery rate). The data underwent descriptive analysis, factor analysis, reliability analysis, an independent samples t-test, and one-way analysis of variance by using SPSS18.0 for Windows statistical software suite. Based on the analysis results, we reached the following conclusions:
(a) Participants presented a mid-upper level score on the scale of factors for self-paid health examination center preference. Health care personnel received the highest score on the scale, followed by environmental factors, health examination services, and health management. Marketing factors received the lowest score.
(b) The participants based their selection of self-paid health examination centers on the following 10 factors: the professional skills of physicians, the professional skills of nursing staff, advanced health examination equipment, the detailed descriptions of pre-examination precautions provided by physicians, clear explanations of examination procedures by nursing staff, the polite attitudes of nursing staff, the polite attitudes of physicians, hygienic considerations regarding the examination environment, transportation convenience, and reasonable price.
(c) Participants who differed in sex, age, education, occupation, marital status, and annual household income exhibited significant differences in the factors determining their preference for self-paid health examination centers.
(d) Significant differences were observed in the factors that influenced the self-paid health examination center preference of participants with differing habits, such as smoking, drinking, chewing betel nut, exercising, use of medication or health supplements, and reading health information.
These findings can benefit self-paid health examination centers, which must comprehend and fulfill the needs of the public to retain market competitiveness.

▼ ããThe majority of cancer patients in Taiwan seek non-mainstream complementary and alternative medicine (CAM) care; increasing numbers of breast cancer patients self-administer CAM. However, few studies have examined breast cancer patients who use CAM. The purpose of this study is to (a) explore breast cancer patientsâ usage of CAM, including the types, duration, and cost; (b) and factors related to the duration of using CAM by women. The study design was a cross-sectional survey. A total of 158 women who had received operations in the past year were recruited; among them 111 women (70.3%) used CAM after their operation. The results were as follows. First, the women using CAM had a the mean age of 54.92; a total of 83.8% were married; 72% had a junior or senior high school educational level, 80.2% practiced Daoism or Buddhism; 64.2% were employed; 98.9% had an annual income below NT $900,000; 35.1% received their primary income from their spouses; 89.9% were residents of Kaohsiung county; and 94.3% had heard of CAM. Second, regarding CAM use, most of the women used 1 to 4 types of CAM. The most common CAM types were biologically based therapies (72.1%) with vitamins and organic food and mind-body interventions (66.7%) with exercise. The average duration of CAM use was 12.62 ± 7.85 months. The most common reason for using CAM was to boost physical energy (90.1%), and most patients (77.5%) stated that their major source of support for CAM use were their family and friends. In addition, most of the patients received their information from books, newspapers , magazines, or from participating in related lectures. Approximately 88.3% of the patients had not informed their health providers of their CAM use. The primary reason for not informing their health care provider was âIt is not necessary to ask my health care provider regarding the usage of CAMâ (66.7%). The significant factors related to the womenâs duration of CAM usage were occupations and circumstances following their operation. These factors explained 73.2% of the variance, and the factors related to CAM use explained 53.8% of the variance. This study suggests that health care providers should consider information regarding CAM usage among breast cancer patients, and health providers should study and conduct research on CAM. In addition, we provide appropriate suggestions for breast cancer patients regarding safe, appropriate, and supportive CAM usage.

Influenza often results in serious illness or death in the elderly and utilizes a considerable amount of health care resources as well. At present, influenza vaccine is the most effective way of preventing influenza and can reduce the possibility of serious illness, complications, and death. At this stage, the rate of influenza vaccine administered has gradually increased; health policies recommend that the elderly should receive regular annual vaccination to prevent influenza. Therefore, by understanding the current situation of sequential vaccination in the elderly in Taiwan, observing long-term vaccination changes, and comparing the differences in medical resource utilization, a better health policy for improving the health of the elderly and better utilization of limited medical resources can be expected.
Study Design
We conducted a retrospective cohort study using the database from National Health Insurance to analyze the influenza vaccination status and medical resource utilization from 2001 to 2007. The propensity score was used for pairing. Individuals were divided into two large groups: those who received influenza vaccine and those who did not receive the vaccine during the vaccination period. Furthermore, each group was divided into individuals who were never vaccinated, who received vaccination but discontinued, and those received sequential vaccination. The vaccination situations were cross-compiled and compared, and the outpatients who utilized medical resources in clinics, hospitals, or emergency rooms in 2007 due to acute or chronic respiratory disease were observed, regardless of the cause of death.
Population Studied
We tracked the vaccination situation of individuals who were ≥65 years of age over 6 vaccination periods and the medical resource utilization a year later.
Principle Findings
The propensity score was used to pair ages, gender, and the extent of disease. From 2001 to 2007, 54,958 people have been tracked. In the past, in the sequential vaccination group, a decrease in the cost of clinical visits was observed when patients were vaccination in the vaccination period. A decrease in hospital costs has also been observed if patients have breaks between vaccinations. In addition, among patients who received vaccination in the vaccination period, the number of clinical visits and the risk of death decreased; however, decrease in the number of emergency room or hospital visits was not observed. Therefore, if vaccination is continued for 6 years, the number of hospital visits and risk of death can decrease; however, the number of clinical visits may not decrease after the first vaccination. On comparing the group vaccinated for the first time, the group with breaks between vaccinations, and the group that received sequential vaccination, it was observed that sequential vaccination can significantly decrease the risk of death.

▼ The environment of the health care industry resulting in a considerable change in recent years by the NHI system uncertainty, rising health care costs, intra-industry competition, the rise of consumer awareness, as well as the patient's requirements to improve the quality of medical care and other factors impact. Therefore, health care providers have thinking business strategy in the future of medical institutions, and actively pursue a diversification of health services, extend the medical professional services related to health checks which is no constraints by NHI system, and at their own expense systemic health checks. Armed forces hospital is not only provide health check service to military personnel but also to provide the public at their own expense health check service. Many domestic and foreign literature shows that use of customer relationship management effectively can improve service quality. Therefore, how to use customer relationship management to enhance service quality of health check, to explore new customers (public) and retaining customers (military personnel), strengthen the competitiveness is an important issue facing the Armed forces hospitals.
In this research, the content of health check service quality are four dimensions to form by the environmental facilities, the health check process, personnel services and health management;the overall customer satisfaction for a single item; the customer loyalty is primarily to repurchase intentions and recommended four questions of. The structure of this research was constructed by "health check service quality", "customer satisfaction", " customer loyalty" the three dimensions together with background information on the dimensions of the formation; and distinguish between "health check service in current" and " health check services in the use of customer relationship management to adjust or increase the service measures " in two part. Explore the relationship and differences between the various dimensions.
This research took the example of an armed forces regional hospital. We use the convenient sample method to obtain questionnaires. The result of this research discovers that the demographic factors have no statistical difference with the health check service quality and the overall customer satisfaction; demographic factors of marital status and work patterns (military personnel) for partial questions of the customer loyalty have significant difference. There is moderate to high positive correlation between health check service quality and overall customer satisfaction. With the application of stepwise multiple regression analysis, health management in the health check service quality can efficiently predict overall satisfaction of the health check as a whole explanatory power and statistical-significance. ▼ Objectives: This Study wanted to analysis the characteristics of the elder adult who had hospitalized with pneumonia. We also evaluated the factors that will affect the unexpected readmission in elderly pneumonia patients.
Methods: This is a retrospective cohort study design. The study data was collected 341 pneumonia patients who have hospitalized in a general teaching hospital in Kaohsiung city from year 2009 to 2010. The study population was divided into two groups, the sample size of the old group (age >= 65yrs), and the young group (age < 65yrs) was 173 and 168, respectively. The methods of stepwise multiple logistic regressions were needed to evaluate the association between aging and different days of unplanned readmission in adult pneumonia patients.
Results: All the 341 adult pneumonia patients, we found 613 male and 926 female. The demography characteristic of the study subjects, the means of age was 61.9yrs (s.d. = 19.3yrs), and BMI was 23.4 kg/m2 (s.d. = 4.5 kg/m2). The percentage of ICD-9-CM that code 486 was 95.6%. Most patients were community-acquired pneumonia (98.8%), hospitalized from emergency room (85.3%), and admission in general wards (92.7%). The unplanned readmission within 14/30 days, 60 days, and 90days were 9.1%, 11.7%, and 15.0%, respectively. The significant factors that were associated with readmission within 14 days include age, Hb, hospitalized days, hypertension, and other disease. When we used the multiple logistic regression analysis to adjust the other variables, only age still significant with readmission within 14 days (the crude OR of the old group was 4.561, adjusted OR was 2.714, 95% CI of OR from 1.002 to 7.353). In the stepwise multiple logistic regression models, the variable that was associated with readmission with 14 or 30 days were age (>= 65yrs, OR = 3.025), WBC (>=10750 mm3, OR=2.917), and Hb (>=12.4 g/dL, OR=0.390). We remain the elderly subjects to evaluate the factor that will influence readmission states. In all the stepwise logistic regression models, we found the experience with used endotracheal tube in the hospitalized period were the significant increases the readmission rate within 14 or 30 days, 60 days, and 90 days.
Conclusion: In our study shows that the situations of unexpected readmission in pneumonia patients were strong association with aging. We suggest that the indicator of medical quality should be adjusted before we comparison the readmission rate in the different institute. The major factors that will be associated to affect the readmission states were endotracheal tube used (significant with 14 or 30 days readmission rate), CRP level (significant with 60 days and 90 days readmission rate), and Hb level (significant with 60 days and 90 days readmission rate).

▼ With regard to infectious diseases, the most economical, direct, and efficient way to prevent them is timely inoculation and a comprehensive policy of vaccination. Such steps not only reduce the overall mortality rate, but also lessen a patientâs susceptibility to serious complications once infected, and therefore their length of hospital stay. It is the foundation of disease prevention in all countries, and should be the primary focus of every public health department.
This survey is based on a health belief model and a self-constructed questionnaire. Its sample base are parents whose children have visited one of two local hospitals, each of which is in a different administrative region. A total of 350 questionnaires were distributed. Recoveries were 270, of which 237 were useable. The effective response rate, therefore, is 67.7%. The useable recoveries were analyzed by SPSS, 17th edition, and verified and assumed by mean, standard deviation, t-test, one-way ANOVA, Pearson correlation analysis and Logistic regression analysis. The most influential factors on parentsâ decision whether or not to accept streptococcus pneumoniae vaccination (SPV) were as follows:
1.The greater the understanding of SPV and its policy, the greater the number of vaccinations
2.The perceived importance of good health
3. Age variability
4. The interrelationship between the perception and the policy of vaccination, the benefits of - and barriers to â action, and the virulence and severity of the disease
The results of this research suggest the public perception of SPV is the most important factor governing its efficacy. It is recommended, therefore, that public health departments campaign for SPV in a variety of different ways,( e.g. in newspapers and magazines, on TV, at pediatric clinics, at health centers, etc.) in order to establish a free and open flow of information to the public at large. It is in the hope of reducing the current mortality rate, length and cost of hospital stay and the serious complications arising from infection, that we offer the following data as reference for future planning.

▼ Objective: The expense of treating Diabetes Mellitus and its complication is the 4th largest part of all in Taiwan Health insurance system. Since continuously multidiscipline team service is the best strategy for Diabetic care. This study is aimed to investigate the relationship between COC (continuity of care) and risk of diabetic foot in Taiwan.
Method: We used Taiwan National Health Insurance database to make a longitudinal study during 2001 to 2007. Only adult diabetes patients who had more than 4 times ambulatory records in 2001-2002 were enrolled. The outcomes from 2003 to 2007 were analyzed as following: risk of amputation, re-amputation, and death. COC and medical expense and organization of care were counted as independent variables.
Result: Re-amputation rate is 23.26%, 1/3 amputated diabetes have been received â§ 3 times amputation. The highest risk of amputation and re-amputation is high COC, which are 1.26 times (p <0.001) and 1.62 times (p <0.001). Amputated diabetes patient had 3 times (p <0.001) risk of specific diabetes associated death. Different medical organization type was not related to the risk of amputation. Low COC was related to poorer vascular and neurological outcome.
Conclusion: We believed that diabetic foot care was a missing circle of the chain of multidiscipline diabetes care chain.

▼ After Taiwan National Health Insurance implemented in 1995, health care management
systems of hospitals in Taiwan had gradually turned in financial-oriented. On the other hands,
people had paid less attention and little mention to basic ethics-oriented topics. Ethics of Health
Care Administration (EHA) combined business ethics and medical ethics and extended a broader
connotation within it. Except general guidelines of medical ethics, EHA was extended to
considerations for management and policy conducts. This research has surveyed the EHA
implementation in hospitals in Taiwan and influenced investigated the relationships amoung the
differences of hospitals characteristics, the superintendentsâ characteristics, and the
superintendentsâ perceptions toward EHA.
A cross-sectional study with structured self-completion questionnaires had been done.
Questionnaires were distributed during May and August, 2009, and 135 were returned, with a
response rate of 27.89%. Responses were gathered from superintendents of hospitals at a district
level or higher. Results delivered by two sections in this research:
Quantitative Section:
1. Superintendents with different hospitals characteristics had positive attitude and
identifications toward the EHA implementation degree in hospitals.
2. Superintendents with older age or deeper experiences had higher appraisal of the EHA
implementation degree in hospitals.
3. Responses for the EHA implementation degree in hospitals, the scores in
themselves-hospitals were higher than the scores in others-hospitals.
4. Responses with higher satisfaction with present health care environment could with a
higher EHA implementation degree in their hospitals.
Qualitative Section:
1. Responses for the perceptions of internal operations in health care institutions were
more focused on positive ways of operational management in health care institutions,
included the agreements of human-based management, health care quality, cost control,
sustainable development under the specific operating model by a certain hospital.
2. Responses for the perceptions of external environment of health care institutions were
more focused on policies and systems within medical or health care field included the
regard for the possibility of health care policiesâ and health care insurancesâ reforms.
Ethics of health care administration can be implemented with a well conduct health care
infrastructure and policy supported management system.

The competition among these health care service organizations are forging these facilities into cost effective systems through, promoting various quality control programs, elevating clinic & treatment capabilities, and improving quality of health care services.
This study is to investigate how the medical staffsâ recognitions are, and at what priority to initiate proper actions, while promoting Taiwan Quality Indicator Project in a medical center in southern Taiwan and its relative influences in clinic quality. The method is to conduct a survey of four hundred and twenty eight (428) clinic personnel from six (6) departments â Emergency Dept., Surgery Dept., Intensive Care Unit, Obstetric & Gynecology Dept., Nursing Dept. and, Psychlogical Dept. - of the sampled medical center. A total of 428 copies of inquires were released to the sampled clinic personnel on April 1st, 2007; and those were collected back with a returning rate of 86.9%, on April 30th, 2007. Software package SPSS is applied to analyze the distribution of this survey. And methods of chi-square test, ANOVA are used to investigate the influence to health care quality caused by various different personnel characteristics.
Conclusions are as follows:
1.The results demonstrate that 52.4% clinic personnel do not fully understand about TQIP. And among them, Nurses, Resident Doctors and, Chief Doctors are the most. While the clinic personnel completely understand TQIP are mostly Registered Nurses and, Attending Physicians. Obvious differences of understanding about TQIP exist, apparently, per different job titles.
2.The importance of TQIP given by the sampled persons is 4.08, categorized as âimportantâ. Without discrimination, same importance is indicated by all clinic personnel of different job titles.
3.The index/indicators of TQIP are accepted as capable to evaluate health care quality of their unit/department by 64.1% of the sampled personnel. Thus, the index/indicator chosen are adequate and proper.
4.69.4% of sampled clinic personnel agree that quality improvement policies are prepared and are realized in their unit/department. The results of TQIP in those units/departments illustrate positive quality improvements of health care service.
5.On the whole, after participating TQIP, the sampled hospital finds quality improvement in health care. The positive influences in health care are, sequentially from more to less, elevating concept of quality control, understanding quality status of the hospital, assessing quality problem of clinic service unit/department, improving health care service quality, improving capabilities in problem solving, improving operation processes & procedures, improving in methods of data collection, better team work, accelerating information computerization in the hospital, better communication and coordination among unit/department.

▼ Objectiveï¼ Influenza is an important public health problem, and may seriously damage the elderly population. Although influenza vaccination is a safe, and can significantly reduces morbidity and influenza -related complication mortality in elderly people, however, the influenza vaccination rate did not reach expected goals. Therefore, to explore the potential factors that affect the vaccination rate becomes a critical research question. The purpose of this study was to apply the Health Belief Model, which includes demographic, health state and healthy behaviour, knowledge about influenza and influenza vaccine, perceived susceptility, perceived seriousness, perceived benefit and barrier, and cues to action variables, to understand factors motivating the community elderly people to receive influenza vaccination.
Design: The sampling designs were stratified and systematic sampling with total 700 sample. During June 2007 , a structured questionnaire was sent to the elderly peopleï¼â§65 yearsï¼in Kaohsiung cityã
Result: The male and female sample distribution rates were equally in 50%, average age was 72.87 years old and 59.6% elderly people had chronic disease (such as hypertension, heart disease and diabetes). Chi-square tests showed that âchronic diseaseâ, âhealth examinationâ, âalways seek care from specific physicianâ, âexercise â, â recognize about influenza and influenza vaccinationâ, â perceived seriousnessâ, â perceived benefit and barrierâ, were significantly associated with having ever been vaccinated. Logistic regression results indicated that âthink the influenza vaccination can effectively prevent influenzaâ, âregular health examinationâ, âcues to actionâ and âperceived barriersâ were found to be the significant predictors of acceptance of influenza vaccination.
Conclusionsï¼The study results indicated that health care workerâs attitude about influenza and influenza vaccination will directly influence community elderly people's cognition. Therefore, by educating the health care worker about influenza seriousness and the vaccination effectiveness may successfully promote vaccination rate. Through health education activities in the community, it may increase correct cognition about influenza and the benefit of vaccination. In addition, the health department needs to strengthen the health policy marketing in order to increase influenza vaccinations rate, reduce morbidity and influenza -related complication mortality.

▼ Objective: Since its implication, case payment system prevailed and increased cases number in the following years. Hospitals in Taiwan face continous challenge with emerging policies such as global budget and TW-DRG system, which is soon on the way. Remarkable medical resource comsuption is seen in patients with congenital heart deseases, with presence of structural heart defects at birth. The corrective treatment of congenital heart diseases, surgical and transcather, is usually undertaken in large-scale Hospitals in Taiwan.Congenital heart diseases. Items of case payment for congenital heart diseases treatment were implented years ago. However, in the literature there is yet no research about the results of its implentation.
This study focus on three objectives:
1.To study of medical resource consumption of case payment system with congenital heart diseases.
2.To study the effect of patient and provider attributes on medical resource consuption in the case payment with congenital heart diseases.
3.To provide the evidence-based information for forcoming NHI policies.
Materials and Methods:
Retrospectively, claims data from Bureau of National Health Research Insurance (BNHI) for resource utilization of case payment for congenital heart diseases was analyzed. The data includes DD(Inpatient expenditures by admissions) and HOSB(Registry for contracted medical facilities) files ranged from 1997 to 2005. Data items meeting the criteria of both CHD and case payment were extracted. The relationships between patient factors (age, sex, DRG code), healthcare provider (contract type, accreditation type, ownership, area) and resource utilization (length of stay, expenditure) were studied.
Results:
A total of 4,366 admissions for CHD case payment was enrolled. The mean patientsâ age is 14.56 years. Female accounts for 55.46 % of the admissions. Among them there are 3954 open heart surgeries and 412 transcatheter treatments. Average hospital day is 11.6 and 3.37 days respectively. Average payment per case is NT215,355 and NT61,819 respectively. Different degrees of resource utilization occur with different patient or hospital characteristics, with statistical signifcance. More resource utilization tends to occur in extremes of age groups, e.g., newborn and elderly populations, regardless open heart surgery or transcatheter treatment cases. Also more hospital fee occurred in private hospital than public hospital, but less in medical centers when compared to metropolitian hospitals. For regression analysis of dependence of resource consumption on patient and hospital factors, the overall power of explanation is higher in transcather treatment cases. Among the factors influencing medical resource utilization, age
group and ownership are respectively the most significant factors.
Conclusion:

Objective: The purpose of this study is to investigate the associations between the different education background of young female consumers, with and without medical education, and their buying behavior in the chain cosmeceutical shop based on concept of E.K.B. model ( Engel, Blackwell and Kollatt model ) , the decision making process model ( Engel, Blackwell and Kollatt ,1993) .
Design: The study was empirically tested using data collected from 365 questionnaires answered by the female consumers in big Kaohsiung area. The structural equation modeling technique was used to evaluate the causal model and confirmatory factor analysis was performed to examine the reliability and validity of the measurement model. Additionally, the linear regression was used to test the null hypothesis.
Measurement: Questionnaire administered items measuring the buying behavior of young female consumers, store selection and consumer behavior, the satisfaction of chain cosmeceutical stores, the professional image of pharmacists and life style, in chain cosmeceutical stores.
Results: The findings indicated that the satisfaction of chain cosmeceutical stores, the professional image of pharmacists, life style and educational background significantly affected the store selection and consumer buying behavior.
Conclusion: This study provides a means to understand what factors impact on the young femalesâ buying behavior in chain cosmeceutical stores and how this may affect their future consumption. In addition, this study reveals marketing implications relevant for chain cosmeceutical stores considering consumer segmentation of young females, and applies this customer segmentation as marketing strategy.

▼ The development and the utility of high-tech medical equipments are increasing relative to the health expenditure growth. These high-tech equipments do not necessary benefit the quality of patient care but increase the service utilization for hospitals. This study aims to describe the difference of equipments distribution between academic medical centers and metropolitan hospitals and to examine the association between the change of the kinds, number, and utility of high-tech medical equipments and the health service utilization.
Two databases were used in this study. First, a secondary data from the annual hospital survey published in 2003 and 2004 by Taiwan Hospital Association was used. The data contained information on 8 health service volumes, kinds, number and utility of 17 kinds of high-tech medical equipments, 4 hospital characteristics, and 3 kinds of staff ratio. Second, Statistical Yearbook of Department of Social Affairs, Ministry of Interior provided data on Tthe percentage of 65+ years old population in every location. The 8 kinds of health service volumes varience were designed as dependent variables and the 3 independent variables were separately change of kinds, number and utility of high-tech medical equipments. There were 59 samples, 8 academic medical centers and 51 metropolitan hospitals, included. The t-test showed differences between academic medical centers and metropolitan hospitals in change of kinds, number, utility of high-tech medical equipments and health service volumes varience. Person correlation presented how these variables correlated to each other. Regression analyses predicted the health service utilization from those variables.
Generally speaking, the academic medical centers only had significant higher varience of average daily emergency roomvisitsï¼t = 3.59, p = 0.01ï¼than metropolitan hospitals did but there was no significant difference in 3 independent variables. Besides, correlation among medical equipments, health service utilization and themselves shows that the change of total high-tech medical equipment kinds was significant positive correlated with the change of total number of high-tech medical equipmentsï¼r = 0.44ï¼; the change of total number of high-tech medical equipment was significant positive correlated with average daily emergency roomvisits varienceï¼r = 0.28ï¼and average daily physical examination of outpatient department visits varianceï¼r = 0.30ï¼; the average utility of high-tech medical equipment was significant positive correlated with average daily outpatient department visits varienceï¼r = 0.27ï¼. However, the 3 independent variables have no overall significant effect on change of health service utilization.
Based on the above findings, although literature indicated that the kinds and number of high-tech medical equipments somehow might stimulate the demand for health service, we did not find the same results. It could be concluded that hospitalsâ purchasing of equipment do not impose a significant impact on raising the medical service utilization.…
