Research Objective Patients with chronic kidney disease (CKD) are at increased risk for atherosclerosis and peripheral artery disease (PAD). PAD has received far less attention than coronary artery disease (CAD) in CKD patients. Few studies have examined risk factors for PAD in CKD. We studied the possible related risk factors and benefit of hypertension treatment in CKD patients with PAD. Data Sources We included 129 patients of both sexes with stages 3 to 5 of CKD, as described by the Kidney Outcome Quality Initiatives (K/DOQI ) classification, without receiving dialysis therapy, not previously diagnosed with PAD. Study Design The following information were collected within six month period, including demographic characteristics, history of hypertension, anti-hypertension drug, diabetes, smoking, and pre-existing cardiovascular disease, body mass index (BMI), fasting blood glucose, HbA1c, total cholesterol, triglyceride(TG), high density lipoprotein cholesterol (HDL), low density lipoprotein cholesterol(LDL), calcium (Ca), phosphate(P), Albumin, uric acid, urine protein. Ankle-brachial index (ABI) is a noninvasive diagnostic test that is efficient in detecting asymptomatic PAD with ABI <0.9. Findings There were 22 (17.7 %) participants with PAD. Higher systolic blood pressure (SBP), higher diastolic blood pressure (DBP), higher pulse pressure showed strong association with PAD in CKD patients. On further analysis, significant fewer patients treated with calcium channel blocker (CCB) in hypertensive CKD patients with PAD (Ï2 =7.055, p =0.008). The multivariate logistic regression analysis in hypertensive patients demonstrated the risk factors for PAD was pulse pressure, and Calcium channel blocker treatment may correlate with decreasing PAD formation (odds ratio= 0.232, 95% CI=0.07-0.73, p =0.013) in CKD patients. Conclusion There is a high prevalence rate of PAD in population of CKD, especially those with hypertension. ABI should be routinely examined in these patients who can benefit earlier from therapeutic measures. Background : The oral administration of Glucosamine and intra-articular (IA) injection of Hyaluronic Acid were used to treat osteoarthritis. Several studies supported that clinical pain symptoms could be alleviated, and activities of daily life improved after the completion of IA knee hyaluronic acid injections. However, the efficacy of oral administration of Glucosamine was inconclusive. In Taiwan, the Taiwan's National Health Insurance (NHI) made the strict formulations for the both of âoral administration of Glucosamine âand âIA knee hyaluronic acid injections.â But, there were no evidence to prove that whether the combined treatments from both oral administration of Glucosamine and IA knee hyaluronic acid injections were more effective than the treatment with the IA knee hyaluronic acid injections alone. Objectives : The porpose of this study was to examine whether the effects of treatment combined with both âoral administration of Glucosamine and IA knee hyaluronic acid injectionsâ on clinical pain symptoms and activities of daily living were better than âIA knee hyaluronic acid injectionsâ alone. Methods : We randomly assigned 60 patients with symptomatic knee osteoarthritis, at the department of Orthopedics, Kaohsiung Municipal Union Hospital, between 2008 and 2009, to receive oral administration of Glucosamine and IA knee hyaluronic acid injections or oral administration of Placebo and IA knee hyaluronic acid injections. In this study, clinical pain symptoms, and activities of daily living were assessed by using Lequesne's Index, visual analogue scale index, Berg Balance Test, and Biodex Balance Test before and after the treatment in 60 patients with the knee OA. Result : The mean age was 66.6 years, and 70 percent were woman. There was a significant efficacy before and after treatments both in the treatment group(p<0.001) and the placebo control group(p<0.001). In the other hand, there was not the statistical significance in the efficacy between the treatment group and the control group(p=0.869). Conclusion : The combined treatment with both oral administration of Glucosamine and IA knee hyaluronic acid injections was not significantly better than the IA knee hyaluronic acid injections alone in reducing knee pain or promoting the function of daily life. Nonsteroidal anti-inflammatory drugs (NSAIDs) are one of the most frequently prescribed classes of drugs worldwide. Due to their excellent effects for analgesic, anti-inflammatory and antipyretic, most elder population used frequently for osteoarthritis. Gastrointestinal symptoms and ulceration associated with NSAIDs are common. Such ulcers may cause pain, bleeding, or perforation. It leads to other medical problems. The aim of this study was to examine the utilizations of medical resources associated with inpatients with NSAID related peptic ulcer disease and compared to non-NSAID related peptic ulcer disease. The study used the database from a teaching hospital in southern Taiwan. Inpatients who were identified ICD-9 CM codes as peptic ulcer diseases from January 1st of 2008 to December 31 of 2008 were included in this study. We also examined the indications for usage of NSAIDs, the date of prescription before the index date. The results showed 17.6% of inpatients with peptic ulcer disease related to NSAIDs. Inpatients with NSAIDs related peptic ulcer disease compared with inpatients with non-NSAIDs related peptic ulcer disease had significant difference in age and comorbidity. Although rapid urease test positive rate for Helicobacter pylori was higher in group of non-NSAID related peptic ulcer disease, it seemed underestimate because of the test number of patients was low. The average total direct medical cost of inpatients with NSAID related peptic ulcer was NT 36,491 and non-NSAID related peptic ulcer was NT 37,266.1. There were no significant difference in medical costs of standard care , intensive care , blood products,endoscopy, endoscopic hemostasis, surgery, CT scan , ultrasound, laboratory tests,medications, doctorâs service between the two groups. Diagnostic and therapeutic procedures were no statistically significant difference, including blood transfusion,CT scan, endoscopic hemostasis, surgery, symptoms presentations, and intensive care. In this study, the duration for using NSAID was within 30 days for inpatients associated with NSAID related peptic ulcer, which had a substantial excess numbers of ulcer hospitalization. Since the common disease for using NSAID is osteoarthritis,which is very popular in elderly, therefore, we suggested that the policy makers of the National Health Insurance should be aware that preventive usage of proton pump inhibitors for the elder population who need frequent use of NSAIDs might decrease NSAID related ulcer complications.
