▼ The implementation of interprofessional education into the curricula of health professional education plays a key role in preparing students for interprofessional collaboration in patient care. However, the development and implementation of interprofessional education in existing curricula is challenging and thus far rarely implemented in Germany. There is a need for research with regard to planning and implementing interprofessional education. Student participation has already shown positive effects for the mono-professional curriculum development. This paper examines the effects of student participation in the context of interprofessional curriculum development.
In an exploratory, qualitative study (Behrend et al. 2019a), focus group discussions were conducted with students (
n = 5) and faculty members (
n = 5) who jointly planned and implemented interprofessional education into health professions education. The experiences and perceptions of the focus group participants were audio-recorded and transcribed. The transcripts were analyzed qualitatively according to Kuckartz (2016).
The qualitative analysis showed that student participation was perceived as having a positive influence on the process and the results of interprofessional curriculum development. The students' motivation, the balance between clarity on tasks and student autonomy and a low-hierarchy team atmosphere were identified as facilitating factors for student participation. Challenges included the risk of role conflicts between faculty members and students and the risk of over demanding of the students.
The statements, from students and faculty members, indicate that students develop professionally by working in interprofessional projects due to acquiring knowledge and skills, and reflecting on their attitude towards interprofessional collaboration. The students also had a motivating influence on the faculty members. They perceived the students as future ambassadors for interprofessional collaboration in the workplace.
In this qualitative study, the reported experiences and perceptions of the focus group participants, provided multiple qualitative evidence for a positive, complementary effect of student participation in interprofessional curriculum development. Facilitating factors for student participation were identified, which should be actively nurtured in future interprofessional curriculum development. The participating students developed professionally and reflected on their attitude towards interprofessional collaboration. Through this, they may play an important role in improving interprofessional collaboration in their future workplaces.
The results of this work extend findings on the positive effects of student participation in mono-professional curriculum development onto the interprofessional context. This may motivate other schools to involve students more actively in the development of interprofessional education.

▼ Genetic host factors play a critical role in susceptibility to malaria and its
severity. Malaria tropica caused by Plasmodium falciparum results in over one
million deaths annually. Toll-like-receptors have been shown in vitro to be
involved in recognition of P. falciparum. The glycosylphosphatidylinositol
anchor of P. falciparum induces signaling in host cells via TLR-2 und TLR-4.
The interaction of the microbial ligand and the respective TLR results in the
release of proinflammatory cytokines via the TLR/IL-2-receptor (TIR) domain.
Thus, TLR may contribute as key host molecules to inflammatory responses
including cytokine production and fever in malaria. In mice, P. falciparum GPI
induces the symptoms of severe malaria while the effect of P. falciparum GPI
on human immune cells remains unknown. In a case-control study among 870
Ghanaian children we examined the influence of TLR-2- and -4 polymorphisms in
susceptibility to severe malaria. Additionally we examined the role of
synthetic P. falciparum GPI in the proinflammatory immune response in in vitro
stimulation assays using murine and human cell lines. TLR-2 polymorphisms
frequently seen in Caucasians and Asians were completely absent in all
Ghanaian children. However, we detected a rare and previously undescribed
mutation (Leu658Pro) which impairs signaling via TLR-2 in vitro. On the
contrary, the TLR-4 polymorphism Asp299Gly was observed at an extraordinarily
higher rate than in Europeans. In healthy controls it was found in 17.6%; in
severe malaria patients it occurred at a significantly higher frequency
(24.1%, P < 0.05). The TLR-4 polymorphism Thr399Ile was found at similarly
high rates, in 2.4% of healthy children and in 6.2% of patients with severe
malaria (P = 0.02). The polymorphisms TLR-4 Asp299Gly and TLR-4 Thr399Ile
conferred an 1.5- and 2.6-fold risk of severe malaria. Synthetic P. falciparum
GPI did not induce cytokine production in the stimulated cells. These findings
suggest an important role of TLR in malaria susceptibility in vivo and their
potential influence on the progression of the disease. Especially in Africa
common TLR-4 variants predispose to severe malaria. The absence of a
proinflammatory immune response to stimulation with synthetic P. falciparum
GPI observed here does not mean necessarily that TLR do not recognize P.
falciparum GPI or that it does not play a role in the pathogenesis of malaria.
This is more likely explained by structural differences between the synthetic
P. falciparum GPI and natural GPI.

▼ The aim of this study was to investigate longitudinal over four years changes
of taste and smell in patients with multiple sclerosis (MS) and correlate
these results to the disease activity. Initially there was made a study at the
Charité of
Berlin evaluating their olfactory and gustatory possibilities and
compared with the new results. 20 MS patients (16 with relapsing remitting MS,
four with chronique progressive MS) were tested. The Threshold Discrimination
Identification test was used for subjective olfactometry (Sniffin’ Sticks) and
an objective olfactometry was made. The Taste Strip Test (TST) was used to
evaluate gustatory function. To evaluate the neurological system the Expanded
Disability Status Scale (EDSS) was used, the Becks Depression Inventory (BDI)
was used to detect a depression and the Mini Mental Status Examination (MMSE)
was used to check the cognition of the patients. The changes in the TST score
correlated negatively with the disease progression, expressed in the changes
of the EDSS score (r=-0.81). The changes of the TDI score correlated with the
self estimation of smell on the Visual Analogue Scale (r=0.47). The number of
relapses in this period of time correlated with the changes of the
discrimination test score (r=-0.48) and the change of the discrimination score
correlated with the change of the TST score (r=0.68). This might be indicative
to be a sign for an affection of the central nerve system. The results of the
objective olfactometry underlined parts of the results of the TDI test but
some results were so different that there were made a focus on the TDI test.
The patients were aware of their olfactory capacity. Olfactory and gustatory
capacity might be useful parameters to estimate disease activity in MS
patients. As the discrimination of smell is processed in higher central
regions of the central nervous system (CNS), the results suggest that
olfactory und gustatory dysfunction in MS patients occurs due to CNS damage
rather than to peripheral damage of the olfactory nerve.

▼ Improved diagnostic methods of cyto- and molecular genetics illustrate that
acute myeloid leukemias (AML) represent a heterogeneous group of different
sensitivity to chemotherapy and prognosis. The differential expression of
apoptosis-associated proteins plays a major role in this regard. In this
study, a selection of pro-and antiapoptotic proteins of the inhibitors of
apoptosis proteins (IAP) family (XIAP, cIAP1, survivin) and other apoptosis-
associated proteins (caspase -3, Bcl -2, Bcl -xL , Bax) were examined for
their level of expression and prognostic significance in 137 cryopreserved AML
samples of children and adults. 62 of the 92 examined adult patients were
treated according the AMLCG-92-study-protocol. All examined children (
n=45)
were treated according the protocol of the AMLBFM-93 study. The expression
levels were examined by immunoblot or flowcytometry. Matching the
antiapoptotic effect of XIAP, high expression of XIAP, in both AML of
childhood and adulthood, was associated with a lower overall survival: adults
[9 months (
n=21) vs 19 months (
n=41), p<0,05]; children [30 months (
n=10) vs.
41 months (
n=34), p=0,02]. In addition, a high XIAP expression correlated with
intermediate / poor karyotype in AML of adult [mean relative optical density
(mROD) 3563;
n=69 vs mROD 184;
n=5; p=0.011). In AML of childhood a high
expression of XIAP (mROD 2509 ± 503,
n=30, p<0.002) and Bcl-XL (mROD 2615 ±
439, p<0.001) correlated with intermediate / poor karyotype. Furthermore, XIAP
expression correlated with myelomonocytic French–American–British (FAB)
subtypes M4/M5 (p=0.05) and expression of monocytic markers (CD 14, CD 36; p <
0.05; CD 4, HLA-DR; p < 0.01) in AML blasts of adults. For the antiapototic
protein survivin, a shorter overall survival [27 Monate (
n=10) vs. 41 Monate
(
n=34); p < 0.026] was associated with high expression levels for AML in
children. Comparing expression in view of age specific differences, different
expression levels were shown for XIAP (p < 0.05). Matching the antiapoptotic
function of XIAP the higher expression was detected in the unfavorable group
of adult AML. However, a higher expression of pro-apoptotic proteins Bax (p <
0.001) and procaspase-3 (p < 0.05) was detected in the adult group, too.
However, a significant variability in the expression of the different IAP in
primary AML was apparent in this study, indicating a differential regulation
of these proteins. Some IAP seem to play a prognostic role in AML and probably
new prognostic groups of AML can be defined. The aim of current and future
research is the identification of specific patient risk groups, risk adapted
and targeted therapies for better responses and outcomes.

Eingriffes für Patienten diskutiert. Das Ziel unserer retrospektiven Studie
war einerseits, die Effektivitä
t der Operation hinsichtlich der Veränderung
von klinischen Symptomen zu evaluieren. Andererseits analysierten wir den
regionalen zerebralen Blutfluss unter Verwendung von Acetazolamid stimulierten
Perfusions-SPECT Untersuchungen. Methodik: Diese ein zentrische Studie wurde
in der Klinik für Neurochirurgie mit zerebrovaskulärem Schwerpunkt des
Vivantes Klinikums im Friedrichshain durchgeführt. Es handelt sich um eine
nicht selektive Erhebung, die den Fokus auf patientenzentrierte Versorgung
legt. Es erfolgte eine Analyse aller Patienten, die sich zwischen August 2008
und Juni 2011 einer extrakraniellen-intrakraniellen Bypass Operation
unterzogen. Einschlusskriterien: mindestens ein symptomatischer Verschluss der
A. carotis interna mittels digitaler Subtraktionsangiographie nachgewiesen und
eine eingeschränkte oder aufgehobene relative zerebrovaskuläre
Reservekapazitä
t nachgewiesen mittels 99m Tc-HMPAO –SPECT. Ergebnisse: Von den
ursprünglich 21 Patienten wurden vier nicht operiert und daher ausgeschlossen,
da sie keine eingeschränkte oder aufgehobene Reservekapazitä
t zeigten. 17
Patienten erfüllten alle Einschlusskriterien und wurden operiert. Die
perioperative Morbiditä
t und Letalitä
t beliefen sich auf 11,8% (
n=zwei) bzw.
0%. Alle Bypässe waren während der Nachsorge (zehn bis 46 Monate) durchgängig.
Es konnte kein signifikanter Anstieg der lokalen Perfusion in zuvor minder-
durchbluteten Arealen nachgewiesen werden. Jedoch zeigte sich eine
signifikante Reduktion der Anzahl der Patienten, die unter transitorisch
ischämischen Attacken (postoperativ vier Patienten im Vergleich zu neun
Patienten präoperativ, p=0.020) oder Kopfschmerzen (postoperativ zwei
Patienten im Vergleich zu sechs Patienten präoperativ, p=0.041) litten. In
allen Fällen wurde eine nicht signifikante Reduktion (p>0.05) der Symptome wie
Hemiparese, Aphasie oder visuelle Defizite in Frequenz und Intensitä
t erzielt.
Diskussion: Die Bypass-Operation kann als Erfolg gewertet werden, da sie eine
signifikante Reduktion der Anzahl der Patienten, die unter transitorisch
ischämischen Attacken oder Kopfschmerzen leiden, ermöglicht. Hinzu kommt, dass
eine deutliche, wenn auch nicht signifikante Tendenz zur Besserung weiterer
klinischer Symptome nachgewiesen werden konnte. Nichtsdestotrotz ließ sich
mittels Perfusions-SPECT Untersuchung kein signifikanter Anstieg der Perfusion
belegen. Zusammenfassend erachten wir es für sinnvoll, Bypass-Operationen im
größeren Rahmen durchzuführen. Wir halten diese Technik für eine überzeugende
Möglichkeit, die für die beschriebene Patientengruppe in Betracht gezogen
werden sollte.

▼ Vasovasostomy (VVS) and tubulovasostomy (TVS) are the treatment options of
first choice to restore fertility after vasectomy. Aim of this study was to
determine the outcome of vasectomy reversal and its predictors of success. :
Intraoperative azoospermia was found in 148 of 678 cases (21, 8 %) depending
on obstructive interval: < 5 years in 4, 1 %, 5-10 years in 21, 7 % and > 10
years in 42, 8 %. Postoperatively overall patency rate was 90, 9 % (602/662)
depending significantly on the anastomosis technique: VVS+VVS in 94, 2 %,
VVS+TVS in 84, 9 % and bilateral TVS in 70, 6 % of the patients. The resulting
overall pregnancy rate was 71, 1 % (404/568) depending significantly on the
maternal age: < 30 years in 84, 3 %, 30-34 years in 77, 1 %, 35-39 years in
60, 1 % and ≥ 40 years in only 22, 0 %. The best results were achieved with
the same female partner. After failed vasectomy reversal a second reversal
procedure was a good option with pregnancy rate of 51 %.

▼ Objective: Junior physicians have been shown to exhibit exceptionally high
burnout levels. With this study, we aimed to investigate the relationship
between work experience, medical career stage, work load, on-call stress, and
burnout-level in a population of university physicians. Methods: The influence
of demographic and professional characteristics on work load and burnout
levels from the Maslach Burnout Inventory were determined amongst 702 of 1797
(response rate 39.1 %) university-based clinicians using generalized linear
estimation models. Results: MBI burnout levels were comparable to other
studies on hospital-based physicians with 51.4 % meeting cut-off criteria for
pronounced levels of emotional exhaustion and 53.8 % for depersonalization.
With increasing experience, emotional exhaustion (P < .001) and
depersonalization (P = .002) decreased. Long working hours and regular night
shifts posed additional independent risk factors. 39.6 % of physicians met the
cut-off value for a high grade of depersonalization if working days, and 61.4
% if providing regular night shifts. Compared to 29.6 % of senior physicians,
84.6 % of residents provided regular night shifts (P < .001), and residents
reported longer weekly hours if their schedule required night shifts (P =
.005). Conclusion: Having little work experience and high workloads with a
disadvantageous combination of night shifts and long working hours, physicians
in early medical career presented an especially high burnout risk. Early
intervention in case of high workload and on-call stress may have a particular
preventive benefit in this group.

▼ Recent studies on prenatal bone formation and bone development around the
developing teeth are lacking. While molecular mechanisms underlying the tooth-
bone-interface have been examined, morphological contexts were given little
attention. This study closely examines such morphologically substantial bone
development processes of developing teeth and the neighboring bone in the
fetal maxilla applying for the first time computer-based and three-dimensional
methods. Our main focus lies on bone development processes and the formation
of peridental and interdental bone around the developing teeth of fetuses in
the bell stage (CRL 160-270 mm, approx. 18-28 WG). Five series of histological
sections were microscopically examined; three were reconstructed in 3D using
AnalySIS® 3D to illustrate different developmental stages of 160 mm CRL, 225
mm CRL and 270 mm CRL. We aimed at determining the histo-morphological
conditions of bone development processes (apposition zones, resorption zones,
and inactive zones). Distances from the dental follicles of the developing
teeth to the surrounding bone in mesial, distal, palatal, and vestibular
directions were measured. Possible interactions or regularities within the
modifying process regarding the development of single teeth and their distance
to the surrounding bone were investigated. We for the first time confirm also
for the upper jaw that the developing teeth are enclosed by bone in bony
crypts; they develop within the latter. The formation of interdental septa and
a complete compartmentalization of the single developing teeth remain undone
in a mesio-distal direction until a developmental stage of 270 mm CRL (WG 28).
The developing primary canine is an exception with its bony septa being formed
firstly. The surrounding bone never completely encloses the neighboring
developing teeth within the investigation period; the bony septum remains open
in a central position. The canine lacks a complete bony margin in a vestibular
direction that cannot be counterbalanced until WG 28. Resorption processes
dominate around the developing teeth. At the outward side of the bone, bone
formation predominates. These processes imply a “centrifugal” bone formation
with a concurrent volume increase of the developing teeth. Open questions
remain with the distance measurements from the single developing teeth to the
neighboring maxillary bone. The measured distances of the middle fetus are
often bigger than those of the older fetus. Individual growth spurts and a
possible physiological variability of the fetuses cannot be excluded in this
study. However, our distance measurement results make it plausible that the
distances follow a prospectively developing periodontal space.

einer Dysfunktion des Aktinzytoskeletts einher. Ziel vorliegender Arbeit war
es, den Einfluss des Aktinzytoskeletts auf den Prozess der
Mikrogliaaktivierung näher zu erforschen und zu charakterisieren. Das Modell
zur Untersuchung dieser Aktinstabilisierungsprozesse basiert auf der
gelsolindefizienten Mauslinie (Gsn-/-) und dem Einsatz pharmazeutischer
Interventionen mit Jasplakinolid und Cytochalasin D. Während Jasplakinolid zur
Stabilisierung (Polymerisation) des Aktinfilamentes führt, resultieren
Interventionen mit Cytochalsin D in einer Destabilisierung (Depolymerisation).
Die Arbeit zeigt, dass gezielte Störungen des Auf- und Abbaus von
Aktinfilamenten in klassisch aktivierter Mikroglia (mittels LPS) keinen
Einfluss auf die Transkription von typischen „M1“ Genen haben. Unterdessen
resultierten Eingriffe in die Aktinfilamentstabilitä
t bei alternativer
Aktivierung von Mikroglia (mittels IL-4) in einer deutlichen Herabregulierung
der Transkriptionsrate typischer „M2“ Gene, was auf einen verminderten
Transport des Transkriptionsaktivators pStat6 in den Nukleus zurückzuführen
ist. Auf funktionaler Ebene führen Veränderungen der Aktindynamik zu
reduzierter Freisetzung von Stickstoffmonoxid, TNF-, IL-6 und IGF-1. Als Grund
für die verringerte NO-Sekretion konnte eine reduzierte iNOS-
Proteinexpressionsrate im Zytoplasma und eine verminderte Aufnahme von Arginin
gezeigt werden. Dagegen ist die LPS-induzierte Freisetzung von IL-1b, das dem
„nicht-klassischen“ Freisetzungsmechanismus über Golgi-Apparat und ER folgt,
unter Stabilisierung des Aktinzytoskeletts erhöht. Die in vitro Experimente
vorliegender Arbeit zeigen, dass Störungen der Aktindynamik zu deutlich
herabgesetzter Migration, Proliferation und Phagozytose von Mikrogliazellen
führen. Zudem konnte bei der Gsn-/- Mauslinie eine verringerte
Proliferationsrate IBA-1-expremierender Zellen nach Hirnischämie und ein
verringerter Anstieg an IBA-1+ Zellen im Nucleus facialis nach Axotomie des
Nervus facialis festgestellt werden. Desweiteren beweist die Arbeit, dass das
intrazelluläre Calciumniveau (Basislinie und ATP-induziert) in Gsn-/-
Mikroglia signifikant erhöht ist. Zusammenfassend kann festgehalten werden,
dass gezielte Störungen der Aktindynamik sowohl klassische als auch
alternative Mikrogliaaktivierungsprozesse abschwächen. Während alternative
(IL-4 vermittelte) Mikrogliaaktivierung mit einer deutlichen
Herabregulierungen auf Gentranskriptionsebene assoziiert ist, finden die
klassischen (LPS vermittelten) Aktivierungsprozesse auf post-transkriptionaler
Ebene statt und führen zu Beeinträchtigungen von Substrataufnahme-, Transport-
und Freisetzungsmechanismen.

▼ Einleitung: An ihren beiden Enden verfügen die menschlichen
Geschlechtschromosomen über je einen identischen Sequenzabschnitt. Diese
beiden pseudoautosomalen Regionen (PAR) ermöglichen den ansonsten
unterschiedlichen Geschlechtschromosomen eine korrekte Paarung und
nachfolgende Aufteilung während der Meiose des Mannes. Die Grenze zwischen
pseudoautosomaler Region 1 (PAR1) und den spezifischen gonosomalen Sequenzen
entstand während der Aufspaltung der Menschenaffen. Nachdem die
pseudoautosomale Region an ihrem proximalen Ende Material eingebüß
t hatte,
gilt sie als fortan stabil. Bei einem Screening auf Kopiezahlpolymorphismen
haben wir in 15 voneinander unabhängigen Familien eine Duplikation unmittelbar
proximal von PAR1 entdeckt. Obwohl diese Duplikation eine X- spezifische
Sequenz beinhaltete, wurde sie ausschließlich von Vater zu Sohn vererbt. Dies
ließ vermuten, dass das Duplikon auf dem Y-Chromosom positioniert ist. Handelt
es sich bei der Duplikation um eine evolutionär ältere Variante von PAR1 im
Sinne einer „Non-Deletion“? Methodik: Um diese Fragestellung anzugehen, wurden
folgende Verfahren durchgeführt: aCGH, FISH, PCR mit Gelelektrophorese, BAC-
vermittelte gezielte paired-end Hochdurchsatzsequenzierung, Sequenzierung nach
Sanger, Einzelmolekülsequenzierung (PacBio) und Y-chromosomale
Haplogruppenbestimmung mittels Y-STR Typisierung. Ergebnisse: Diese genomische
Analyse zeigte, dass es sich nicht um eine ältere, sondern um eine jüngere
bisher unbekannte Variante der PAR1 handelt. Dieser die PAR1 um 105 kbp
verlängernde Polymorphismus entstand durch eine nicht-allelische homologe
Rekombination zwischen dem X- und dem Y-Chromosom, welche durch den nur 548 bp
langen Repeat LTR6B vermittelt wurde. Die Identifikation der reziproken
Deletion auf dem X-Chromosom einer Familie und das Vorkommen der Variante in
verschiedenen Y-chromosomalen Haplogruppen zeigen, dass es sich dabei um ein
wiederkehrendes genomisches Rearrangement in der menschlichen Population

▼ Einleitung: Die dynamischen Eigenschaften des adulten Methyloms wurden
kürzlich im zentralen Nervensystem beschrieben. Ob äußere Reize auch die DNA-
Methylierungsmuster des peripheren Nervensystems beeinflussen können, wurde
bisher nicht untersucht. Die Hypothese der vorliegenden Arbeit war, dass eine
periphere Nervenschädigung Methylierungsveränderungen der DNA im
Spinalganglion L5 hervorrufen kann. Dies wurde anhand eines neuropathischen
Schmerzmodells nach Spinalnervenligatur (SNL) bei Ratten untersucht. Methodik:
Mittels „Reduced representation bisulfite sequencing“ (RRBS) wurden die
Methylome von acht Spinalganglien (vier Kontrollen, vier SNL) 24h nach SNL
analysiert. RRBS ermöglicht die genomweite Untersuchung von DNA Methylierungen
mit einer Auflösungsgenauigkeit einzelner Basenpaare. Dabei wird die DNA
zuerst mittels eines Restriktionsenzyms in Fragmente geschnitten, die
mindestens 2 Cytosin-phosphat-Guanin-Dinukleotide (CpGs) enthalten. Die
nachfolgende Behandlung mit Bisulfit führt zur Desaminierung der
unmethylierten Cytosinbasen in Uracilbasen, ohne die anderen Basen zu
beeinflussen. Abschließend werden die DNA Fragmente amplifiziert, sequenziert
und an das Referenzgenom angeglichen. Ergebnisse: Es zeigten sich
weitverbreitete, hoch signifikante (p≤10-4) Cytosin Hyper- und
Hypomethylierungen in etwa 1% der durch RRBS erfassten 1.4 Millionen CpGs.
Diese wurden unter dem Begriff „Dynamisch differenziell methylierte CpGs“
(dDMCs) zusammengefasst. Die epigenetische Umgestaltung erfolgte größtenteils
außerhalb der sogenannten CpG Inseln. 56% der beobachteten Veränderungen
befanden sich in Promoter- oder Genregionen, insbesondere in Genen der
„axonalen Wegfindung“ (p<10-11). Die vorliegende Arbeit fand keine Korrelation
zwischen den Methylierungs-veränderungen und der Variation der Gen Expression
und reiht sich damit in die wachsende Zahl von Studien mit genomweiten
Analysen ein, die für eine komplexere Interaktion zwischen Methylom und
Transkriptom sprechen. Die anderen 44% der dynamisch veränderten CpG
Methylierungsmuster befanden sich außerhalb von Genen. Diese intergenischen
dDMCs traten in Clustern auf, wobei sich benachbarte dDMCs immer in die
gleiche Richtung veränderten. Ihre Positionen wurden mit intergenischen
Gewebe-spezifischen differenziell methylierten CpGs (tDMCs) von Leber, Milz,
Spinalganglion L4 und Skelettmuskel, verglichen. Bemerkenswert war, dass sich
die Positionen der dDMCs mit denjenigen der tDMCs deckten (p<10-15) und fast
nie den invarianten Teil des Methyloms betrafen (diejenigen CpGs, die in allen
Geweben identisch sind). Das intergenische Methylom lässt sich damit in zwei
Teile gliedern: ein invarianter Teil – welcher zwischen unterschiedlichen
Geweben und unter verschiedenen Bedingungen unverändert stabil bleibt – und
ein plastischer Teil, der auf Umwelteinflüsse wie z.B. die Verletzung des

▼ Congenital diaphragmatic hernia (CDH) is a rare birth defect characterized by
malformation of the diaphragm and prolapse of abdominal organs into the
thoracic cavity. Complicating factors are pulmonary hypoplasia and pulmonary
hypertension. Even after a successful operation, these patients commonly need
prolonged assisted ventilation. Patients and Methods: We evaluated
retrospectively the course of disease of 26 newborns with CDH, born between
1993 and 2003 and treated at the Department of Neonatology at University
Hospital Charité,
Berlin. In addition, data from lung function tests (infant
whole-body plethysmography, tidal breathing analysis and occlusion tests),
carried out at a median age of 32 days were analyzed. With the help of these
data, we looked for prognostic indicators for a reduced pulmonary outcome.
Results: The majority of patients with CDH had a pathological lung function.
Respiratory compliance, resistance, thoracic gas volume, respiratory rate and
tidal volume differed significantly from the parameters of controls. Further
analysis showed an association between duration and intensity of mechanical
ventilation and pulmonary outcome. An early prenatal diagnosis of CDH, a non-
left-sided hernia, the use of a patch for operative repair or complications
like pleural effusion and pulmonary hypertension significantly increased
likelihood of abnormal results from subsequent lung function tests. And where
there was an impaired pulmonary function, patients were more likely to be
hospitalized longer, especially those children with an obstructive-restrictive
pattern. Conclusion: Even after successful surgery, most children with a
congenital diaphragmatic hernia have a reduced lung function. With certain
risk factors it is more likely for these patients to develop a ventilation
disorder. This underlines the necessity of a protective ventilation strategy,
in order to reduce additional pulmonary sequelae due to barotrauma and oxygen
toxicity. It seems sensible to perform early lung function tests on all
children with CDH and to refer them to a specialist paediatric pulmonology
unit for continuous follow-up.

▼ This thesis presents a survey of supra- and y-condylar fractures of the
humerus in children who were treated in the Department of Pediatric Surgery at
the Virchow Campus of the Charité Universitätsmedizin
Berlin between
01-01-2005 to 31-12-2010. The focus of this retrospective study is the
identification of disorders of peripheral blood supply, motor and sensory
functions before and after surgery. The surgeons in the Department of
Pediatric Surgery of the Charité observed an increasing number of disorders of
peripheral blood supply, motor and/or sensory functions. Therefore, this work
relates the current treatment outcomes to similar studies and especially to a
former study performed in the same Department focusing on the period from
01-04-1996 to 30-04-2001. The study includes 355 supracondylar and 16
y-condylar fractures of the humerus, which are classified according to
Baumann. 49% of all cases were type Baumann I fractures, 21% Baumann II and
30% Baumann III. The age ranged from four months to 17.4 years (middle 5.9
years). The most fractures affected males (56%). Correlated fractures were
identified very seldom (3%). Preoperative disorders of blood supply, motor
and/ or sensory functions were found in 7% of the cases. Postoperative
disorders in blood supply were not found. There was also non compartment
syndrome or Volkmann contracture identified. Disorders of peripheral sensory
and motor function were found 43 times (11.6%). The ulnar nerve was affected
in 65% of all postoperative nervale disorders. Out of the affected 43
children, 22 (51%) were actually further examined. Only one of them had a
hyperesthesia in the second thru fifth finger, another had a hypesthesia in
the fifth finger. Disorders of blood supply or motor function were not found
at all. Overall, the observation of the surgeons in the Department of
Pediatric Surgery of the Charité was correct. There were currently indeed a
higher absolute number of disorders of peripheral blood supply, motor and/ or
sensory function. However, they also treated more children with fractures of
the distal humerus in general. Considering this increased total number of
treated distal humerus fractures the relative number of disorders of
peripheral blood supply, motor and/ or sensory function decreased from 18.1%
to 11.6% in comparison to the previous study. The number of preoperative
disorders shows a slightly increase from 5.2 to 5.9%. The share of
postoperative disorders that was found in this study ranges in the level,
which is also shown in other studies (4 to 18.1%).

Alter haben entscheidenen Einfluss auf den klinischen Erfolg von
Organtransplantationen und die Qualitä
t von Spenderorganen. Da die Zahl der
älteren Transplantatempfänger steigt und zunehmend Organe von älteren Spendern
transplantiert werden, ergeben sich drängende Herausforderungen für die
Transplantationsmedizin. Klinisch konnte für die Transplantation von älteren
Spenderorganen eine Assoziation mit schlechterem Patienten- und
Transplantatüberleben sowie häufigeren akuten Abstoßungsreaktionen und
verzögerter Transplantatfunktion gezeigt werden. Um diesen klinischen
Phänomenen zugrundeliegende Mechanismen zu ergründen, wurde in der
vorliegenden Arbeit ein Mausmodell heterotoper, komplett vaskularisierter
Herztransplantation mit MHC-disparaten jungen (acht bis zwölf Wochen)
DBA/2-Empfängern und jungen oder alten (18 Monate) C57BL/6 Spendern etabliert.
Von alten Spendermäusen gewonnene Herzen zeigten eine signifikant
beschleunigte Transplantatabstoßung im Vergleich zu jungen Spenderherzen.
Dieser Unterschied im Transplantatüberleben war begleitet von höheren Indizes
in der ISHLT Abstoßungsklassifikation und verstärkter Transplantatinfiltration
durch CD4+ und CD8+
T- Zellen. Das Alter des transplantierten Organs hatte
weiterhin Einfluss auf Charakteristiken der systemischen Alloimmunreaktion des
Empfängers, dargestellt durch erhöhte Häufigkeit von milzständigen CD8+
Effektor-
T-Zellen und CD8+IFN-γ+
T-Zellen bei Transplantation von älteren
Spenderorganen, zusätzlich zu erhöhten Häufigkeiten von alloreaktiven IFN-γ-
und IL-6-produzierenden Splenozyten sowie ausgeprägterer alloreaktiver
Proliferation von Splenozyten bei Restimulation mit donorspezifischen
Antigenen. Um Effekte des Parenchyms der transplantierten Organe von denen der
mitübertragenen Passagierleukozyten zu unterscheiden, wurden chimäre
Spendertiere generiert durch Transplantation von Knochenmark junger syngener
Tiere in alte und junge zukünftige Spendertiere sechs Wochen vor
Organentnahme. Nach Transplantation dieser chimären Spenderherzen (junges
Parenchym mit jungen Passagierleukozyten oder altes Parenchym mit jungen
Passagierleukozyten) waren Transplantatüberleben, Abstoßungsindizes und
systemische Empfängerantworten vergleichbar, was auf eine entscheidende Rolle
von organständigen Passagierleukozyten bei der Vermittlung von verstärkten
Immunreaktionen nach Transplantation alter Spenderorgane hindeutete. Mit Fokus
auf dendritische Zellen als eine Untergruppe der Passagierleukozyten wurden
nun organständige dendritische Zellen durch Vorbehandlung der Spendertiere mit
liposomalen Clodronat aus den Spenderherzen depletiert. Unterschiede in
Transplantatüberleben und Abstoßungsindizes waren aufgehoben nach
Transplantation von Spenderherzen vorbehandelter Tiere, ebenso waren die
systemischen Immunantworten unabhängig vom Alter der Spendertiere. Weiterhin

▼ Introduction Pelvic tilt influences acetabular orientation (AO). Anteversion
(AA) and inclination (AI) define anatomical acetabular orientation. Pelvic
tilt determines functional acetabular orientation because of its spatial
relocation. Anterior pelvic plane is used for calculation of anatomical AO,
whereas functional AO is defined by the table plane of operating table or
medical imaging. The following analysis shows, if there is an influence of
pelvic tilt on functional AO. Secondary was verified, if anteversion and
inclination give equivalent information for correct AO. Material and methods
Computed tomography – based three dimensional reconstruction of bony pelvic
structures provided the basis of the following analysis. 138 pelvises were
determined with the patients supine. APP was distinguished by the most ventral
points of the right and left Spina iliaca anterior superior and both pubic
tubercles, while the angle between APP and coronal plane defined pelvic tilt.
Results Pelvic tilt supine amounted to -0,1°±5,5°. 79% of the collective
showed a physiological pelvic tilt. AA amounted to 18,3°±4,5° relative to APP
and 18,5°±4,6° relative to table plane. AI was 46,9°±3,7° for both reference
planes. AA and pelvic tilt correlated positive relative to APP (all: p < 0,
01, r = 0,497, R2 = 0,320) and negative relative to table plane (all: p < 0,
01, r = -0,366, R2 = 0,117). 29 persons showed increased posterior or anterior
pelvic tilt of -10,3°±4,5° respectively 8,5°±1,8°. These individuals showed a
difference of AA between both reference planes. AA differed from 8,3°±3,1°
(females) and 7,7°±3,6° (males) with extensive posterior tilt and from
6,2°±1,0° (females) and 5,4°±2,0° (males) with extensive anterior tilt. AI
showed less marked differences of 2,2°±0,8° for extensive pelvic tilt.
Discussion Pelvic tilt compensated for increased AO. Individuals with high
anatomical AA showed high pelvic tilt, which reduced functional AA. Conversely
individuals with low anatomical AA had low pelvic tilt and therefore enlarged
functional AA. Pelvic tilt is considered by APP, but it is not by table plane.
Hence in individuals with physiological pelvic tilt both reference planes can
be used for correct AO. By contrast table plane is no reliable reference plane
for correct anatomical AO in individuals with extensive pelvic tilt.
Conclusion Anatomical anteversion influences pelvic tilt. Pelvic tilt has a
compensating influence on functional acetabular orientation. In conclusion
results of this study serve to further understand the functional anatomy of
the acetabulum and demonstrate the importance of pelvic tilt for correct AO.

▼ Objectives: The aim was to investigate the performance of an experimental
self-adhesive resin cement used as a post and core build-up material in
comparison to two commercially available core build-up materials. Materials
and methods: 40 human maxillary incisors were endodontically treated and
prepared for post insertion (RelyX Fiber post). Post and core build-ups were
performed using one of the four investigated materials: two build-up
composites based on an etch-and-rinse adhesive approach (LuxaCore Dual – LC
und Clearfil Core – CC) and one self-adhesive resin cement in two application
modes (Handmix and Automix). The samples were subjected to thermo-mechanical
loading (TML). Maximum load capability (Fmax) was tested in a universal
testing machine. Margin integrity was evaluated using Scanning Electron
Microscopy. Physical properties were additionally examined. Results: Fmax was
significantly affected by the core build-up material (p<0.0005; 1-way ANOVA).
CC [481 (158)
N] achieved higher Fmax than LC [226 (80)
N], Handmix [205 (115)
N] and Automix [197 (134)
N] (p<0.05; Tukey-B). The percentage of perfect
continuous margin in enamel after TML differed significantly among the groups
(p<0.0005; Kruskal Wallis). CC gained significantly higher percentages of
perfect margin compared to the other groups. Physical properties were
significantly affected by the build-up material (p<0.0005; ANOVA). CC and LC
demonstrated higher flexural strength in comparison to the self-adhesive
groups (p<0.05; Scheffé). Water sorption was significantly higher in the
self-adhesive groups (p<0.05; Scheffé). Conclusion: Within the limitations of
the present in vitro study it can be concluded that the investigated self-
adhesive resin cement is not suitable as a core build-up material due to its
low maximum load capability, low margin integrity and weak mechanical
properties. Clinical relevance: The investigated self-adhesive resin cement
cannot be recommended as a core build-up material.

▼ Coronary artery disease (CAD) with its manifestations chronic ischemic heart
disease and acute myocardial infarction was the leading cause of death in
Germany in the year 2012 with 14.2% of all deaths. Invasive conventional
coronary angiography (CCA) serves as the reference standard in the detection
of coronary artery stenosis and in-stent restenosis (ISR). Noninvasive
coronary angiography using multislice computed tomography (CTA) is a promising
tool for patients with a low-to-intermediate likelihood for CAD due to its low
risk of complications and high sensitivity and specificity in the detection of
CAD. The aim of this work was to investigate 1. the diagnostic accuracy of the
combination of myocardial perfusion imaging (CTP) and CTA compared to CTA
alone for the assessment of coronary artery stents, 2. the time efficiency and
agreement of perfusion parameters of two different myocardial perfusion
software releases and 3. the image quality using two different bolus tracking
methods for CTA image acquisition. The combination of CTA and CTP compared to
CTA alone achieved a higher diagnostic accuracy in the detection of ISR on the
patient level (87% vs. 71%, p < 0.001) and on the stent level (89% vs. 76%, p
< 0.001; publication 1). The number of non-diagnostic stents was significantly
reduced using the additional CTP information (p < 0.001). The evaluation of
the two software releases for CTP analysis showed that with the new software
version significantly less analysis time is required compared to the previous
software version (p < 0.001). The diagnostic accuracy was not significantly
different between the two software versions (publication 2; p = 0.169). In the
analysis of two different bolus tracking methods for the initiation of CTA
(publication 3) both a manual and a fixed threshold bolus tracking method
achieved a reliable image quality. The evaluation of the image quality of the
coronary arteries showed no significant differences between the two groups (p
> 0.41). The present work confirms that the non-invasive coronary angiography
using CT is a constantly developing and promising tool and - under certain
conditions - an alternative to CCA in the diagnosis of CAD.

▼ Introduction: Asymmetric dimethylarginine is a competitive inhibitor of the
NO-sythase and an important factor of the regulations of vessels. It is
synthesized through posttranslational modification of proteins and released by
proteolyses into circulation. In literature patients with chronic kidney
disease or dialysis had higher ADMA levels. Iodonated radiocontrast media can
heavily damage the kidney, which can lead to a complete loss of kidney
function. Especially patients with predamaged kidneys have a higher risk.
Because of the rising usage of radiocontrast media, the importance of
identification of biomarkers for the prognosis after application rises as
well. The ADMA-NO-System might be taking part in the pathogenesis of a
contrast induced nephropathy. So this work examines the behavior and the force
of expression of ADMA to renal events. Methods: Therefore 330 patients with
preexisting diabetes mellitus or renal impairment, who were undergoing a
percutanous coronary angiography, were included and the levels of ADMA,
creatinine and cystatin c before and 24 hours after contrast media application
were measured. To determine the development creatinine was also measured 48
hours and tree months after the application. At three month follow up data
about clinical relevant events were collected, too. Results: As result ADMA
rises after contrast media application. But there was no relationship between
ADMA and the incidence of an acute renal failure. For longer prognosis ADMA
levels before and after contrast media application were predictive for
dialysis and major adverse renal events (MARE). Additional patients with MARE
had a significant higher increase of the ADMA levels than other patients.
Diabetes mellitus, anemia, congestive heart failure and the hydration of
patients emerged as important influencing factors in the assessment of ADMA.
Conclusion: In conclusion the ADMA-regulation is influenced by contrast media.
Patients with poorer prognosis demonstrated a different behavior by a higher
increase of ADMA. For the short term outcome ADMA seems to be less relevant.

▼ Background: Although joining patient self-help groups is recommended to
patients with ankylosing spondylitis (AS) the evidence for benefits of
membership is limited. Objective: To evaluate differences between AS patients
who are members of a patient self- help group and AS patients who are non-
members. Methods: An anonymous questionnaire which consisted of 82 questions
regarding demographics, diagnosis, smoking habits, acquisition of information
about the disease, disease activity, functional status, patient satisfaction,
quality of life, treatment and work disability and educational level was
distributed to AS patients who were members of the German AS organisation
(DVMB) and non-members. Results: Originally 1273 patients responded (1068
members and 205 non-members). To achieve comparability a 2:1-matching of
members to non-members regarding age, gender and disease duration was
performed. In the matched population members (
n=366) and non-members (
n= 183)
were similar regarding age (47.0 vs. 46.3 years), disease duration (21.1 vs.
20.1 years), gender (63.9% vs. 63.9% male) and HLA-B27 positivity (89.0% vs.
94.9%). Members and non-members differed significantly: members more often
felt that they were well informed (62.2% vs. 35.4%), more often used the
information provided by the patient organisation (58.5% vs. 11.5%), had a
better functional status (BASFI 3.4 vs. 3.9) and less work days missed during
the last year (15.1 days vs. 31.2 days). The results stayed unchanged after
co-variance analysis. Conclusion: There are numerous benefits associated with
the membership in an AS patient self- help group. AS patients who are members
feel better informed, have a better functional status and a healthier life
style, and, overall, seem to cope better with the disease than non-members.

(ESBL-E) are associated with increased mortality. Outcome differences due to
various species of ESBL-E or ESBL genotypes are not well investigated. We
conducted a cohort study to assess risk factors for mortality in cases of
ESBL-E bacteremia (K. pneumoniae or E. coli) and the risk factors for sepsis
with organ failure. All consecutive patients of our institution from 2008 to
2011 with bacteremia due to ESBL-E were included. Basic epidemiological data,
underlying comorbidities, origin of bacteremia, severity of sepsis and delay
of appropriate anti-infective treatment were collected. Isolates were PCR-
screened for the presence of ESBL genes and plasmid-mediated AmpC
β-lactamases. Cox proportional hazard regression on mortality and
multivariable logistic regression on risk factors for sepsis with organ
failure was conducted. 219 cases were included in the analysis: 73.1% due to
E. coli, 26.9% due to K. pneumoniae. There was no significant difference in
hospital mortality (ESBL-E. coli, 23.8% vs. ESBL-K. pneumoniae 27.1%,
p=0.724). However, the risk of sepsis with organ failure was associated in
cases of K. pneumoniae bacteremia (OR 4.5, p<0.001) and patients with liver
disease (OR 3.4, p=0.004) or renal disease (OR 6.8, p<0.001). We found
significant differences in clinical presentation of ESBL-E bacteremia due to
K. pneumoniae compared to E. coli. As K. pneumoniae cases showed a more
serious clinical presentation as E. coli cases and were associated with
different risk factors, treatment and prevention strategies should be adjusted
accordingly.

▼ Background: Antioxidants represent an important protective mechanism against
the destructive effects of free radicals in the human organism. These can
arise due to various internal and external influences, such as UV and infrared
radiation, chemical toxicants, infectious diseases or nicotine and alcohol
abuse. The aim of this study was to investigate the impact of individual
lifestyle habits, such as diet and stress factors, on the antioxidant status
of the skin. Furthermore possible differences between ethnical population
groups, age and social groups were examined. Materials and Methods: For this
purpose the cutaneous carotenoid concentration was determined in healthy
volunteers of different social and ethnical background using a mobile,
miniaturized measurement device based on reflection spectroscopy. Moreover
individual data on dietary and lifestyle habits was assessed. In a further
step, the clinical use of a topically applied cream containing highly
concentrated antioxidants was investigated for treatment and prevention of
palmar plantar erythrodysesthesia in patients receiving chemotherapy with
pegylated liposomal doxorubicine. Results: It was shown that especially a
healthy diet, rich in fruit and vegetables, and avoidance of stress are
equally important for a high antioxidant status. The antioxidant status is a
multifactorial process that is influenced by stimulating, as well as reducing
factors. The nutritional and stress behavior is of particular importance.
Individual changes in diet and lifestyle can quickly be shown by using the
above mentioned non-invasive spectroscopic mean measuring carotenoids as
biomarkers. In addition to that, the measurements are well suited to provide a
biofeedback about present individual lifestyle and eating habits or even
motivate to a healthier lifestyle. Furthermore the clinical application of a
cream containing a high concentration of antioxidants showed positive
therapeutic and preventive effects against palmar plantar erythrodysesthesia.
Discussion: Due to its handiness, mobility and non-invasiveness, the used
spectroscopic device is a well suited tool to investigate the antioxidant
status in a larger amount of subjects. Both the determination of cutaneous
antioxidants by means of the described method as well as the topical
application of concentrated antioxidants can be effectively used for
individual health promotion and disease prevention.

▼ Neoplasm of adrenal glands are frequent. In a normal population benign
neoplasm dominates. For these the value of CT and MRI in diagnostics is well
examined. Both are suited to predict benignancy. However, for patient with a
high risk for adrenal metastases only few data is available. This study
examined whether CT and MRI are sufficiently accurate to predict benignancy in
a high-risk group. A diagnostic algorithm for patients with a lung carcinoma
(LuCa) and an adrenal neoplasm (AN) is derived from our data. This
retrospectives study included patients with LuCa and at least one AN, if at
least one MRI with chemical shift imaging (CS-MRI) and optionally an
unenhanced CT (nCT) existed and if the nature of the AN was known either by
histologic proof or follow-up. 51 patients with 65 AN were enrolled. In all
cases a CS-MRI and in 41 a nCT was available. The signal loss in CS-MRI and
the density in nCT were measured. Threshold values for assuming benignancy
were fixed: in nCT a density of ≤10 HU, in CS-MRI a signal loss of >50%. For
CS-MRI a sensitivity of 88.9% and a specificity of 93.6% were found (negative
prediction value (NPV) 95.7%, positive prediction value (PPV) 84.2%, accuracy
92.3%. A ROC analysis confirmed that the threshold value of 50% signal loss
offers a good differentiation between benign and malign neoplasm in a high-
risk group. In nCT a sensitivity of 92.3% and a specificity of 78.6% were
found (NPV 95.6%, PPV 66.6%, exactness 82.9%). From this data an algorithm was
derived for patients with LuCa and AN. As nCT and CS-MRI are equivalent
concerning the NPV, it is possible to perform nCT first. If benignancy of AN
cannot be proven, a CS-MRI can provided further specificity. With this
algorithm staging of LuCa patients can be non-invasively preformed with
clinically sufficient accuracy. If the dignity of AN remains unclear a PET-CT
and possibly histologic proof may be necessary in potentially operable
patients. Our data on the performance of CS-MRI and nCT in a high-risk group
is comparable with published data found in a normal population. In conclusion,
the proposed diagnostic algorithm using nCT and CS-MRI for patients with a
high risk for adrenal metastases can, with sufficient accuracy, predict the
benignity of an adrenal neoplasm.

▼ Oxygen contributes to the pathogenesis of neonatal brain damage, leading to
neurocognitive impairment of prematurely born infants in later life (17,120).
We have identified a critical role for receptor-mediated neuronal apoptosis in
the immature rodent brain. The contribution of the intrinsic apoptotic pathway
accompanied by activation of caspase-2 under hyperoxia in the neonatal brain
still remains elusive. Inhibition of caspases appears a promising strategy for
neuroprotection in neonatal brain injury. 6-day-old Han Wistar rats were
randomly assigned to four groups and treated as follows: (1) normoxia (21% O2)
and vehicle i.p., (2) normoxia and 1mg TRP601/kg/KG i.p., (3) hyperoxia (80%
O2) and vehicle i.p. (4) hyperoxia and 1 mg TRP601/kg/KG i.p. We applied a
recently developed pentapeptide-based group II caspase-inhibitor (5-(2,6
-difluoro-phenoxy) -3(R,S)-(2(S)-(2(S)-(3-methoxy-carbonyl-2(S)-(3-methyl-2(S
)-((quino-line-2-carbonyl)-amino) butyrylamino)pro-
pionylamino)3methylbutyrylamino)propionyl-amino)-4-oxo-pentanoic-acid-
methylester; TRP601). 12 or 24h after treatment, postnatal day-6 pups were
killed. They were transcardially perfused with PBS and decapitated. Brains
were removed and prepared for Fluoro-Jade B staining, Western blot analysis
and detection of caspase-2 and -3 enzymatic activities. Data were analyzed by
R Project for Statistical Computing and presented as mean±standard error.
Group effects were assessed by ANOVA, followed by independent sample
t-test
multiple comparison. Adjusted P-values of <0.05 were considered statistically
significant. We report that elevated oxygen (hyperoxia) levels trigger a
marked increase in active caspase-2 expression, resulting in an initiation of
the intrinsic apoptotic pathway with upregulation of key proteins, namely,
cytochrome c, Apaf-1 and AIF, whereas Bid and the anti-apoptotic Bcl-2 are
downregulated. These results coincide with an upregulation of caspase-3
activity and marked neurodegeneration. However, single treatment with TRP601
at the beginning of hyperoxia reversed the detrimental effects in this model.
The present study reveals the involvement of caspase-2 in combination with the
intrinsic apoptotic cascade in hyperoxia-induced injury to the immature
central nervous system. We report for the first time that TRP601 provides
strong evidence for neuroprotective action against hyperoxia-induced apoptosis
influencing intrinsic caspase-dependent and also caspase-independent pathways.
Our results are in line with a recently published report of a protective
effect of TRP601 in other rodent models of perinatal rodent brain injury,
(128) and suggest different potential molecular mechanisms that may entertain
this tissue-protective effect of TRP601. TRP601 seems to be a auspicious
prospect for neuroprotection in neonatology.

▼ Purpose: Spectacle independence after cataract surgery is one of the main goals in refractive cataract surgery today. The surgeon can choose between a vast variety of intraocular lenses. One of the most recent generations of intraocular lenses is the Tecnis Symfony IOL which uses a new achromatic diffractive lens design. The purpose of this study was to evaluate this extended depth of focus (EDOF) IOL and to compare it to a monofocal intraocular lens.
Methoden: This was a prospective non-randomized clinical study including 60 eyes of 30 patients undergoing cataract surgery with the implantation of the EDOF-IOL Tecnis Symfony IOL (ZXR00, Abbot Medical Optics Inc, Santa Ana, CA, USA) (EDOF group: 30 eyes of 15 patients) or the monofocal Tecnis Monofokal 1-Piece IOL (ZCB00, Abbot Medical Optics Inc, Santa Ana, CA, USA) (monofocal group: 30 eyes of 15 patients). The visual acuity in far, intermediate and near distance, the depth of focus, contrast sensitivity and glare vision unter photopic and mesopic conditions, higher-order aberrations and the quality of life (National Eye Institute Refractive Error of Life Instrument 42 Questionnaire) were evaluated during a 4-week and 3-month follow-up.
Results: There was a significant better contrast sensitivity and glare vision unter photopic conditions and a significant better contrast sensitivity unter mesopic conditions for the monofocal group (p=0,041). No differences were found in the glare vision under mesopic conditions and in the ability to drive a car (p=0,485). The EDOF group showed significant better monocular and binocular intermediate and near visual acuity (p<0,001). No significant differences were found when looking at higher order aberrations between the groups (p=0,655). Concerning the questionnaire no differences were found in the categories “glare” (p=0,384) and “satisfaction” (p=0,14). However, in the category “dependence on correction” the EDOF group showed significantly better results (p=0,001).
Conclusion: The EDOF IOL is a good contribution to the spectrum of intraocular lenses. It provides better intermediate and near visual acuity compared to a monofocal lens. The worse results in contrast sensitivity and glare vision seem to have a limited clinical relevance when looking at driving ability and quality of life.

preoperatively and six months postoperatively, the fatty degeneration of
gluteus medius muscle was quantified using magnetic resonance tomography.
Subsequently, the analysis of the osseous and muscular damage and the
varianceanalysis of the muscle damage as a function of severity of the
acetabular bone defect were performed. Results: The present study includes
four patients with defect type 1, twelve patients with defekts type 2A, 2B and
2C and six patients with defects typ 3A and 3B according to the Paprosky
classification. In the cohort, a significant increase in the muscular fatty
degeneration of the gluteus medius muscle can be demonstrated (preoperatively
30%± 28,6; postoperatively 40% ± 30,7; p = 0,002). Preoperatively, patients
with first revision manifest a significantly lower extend of muscle damage
compared to patients with multiple revisions (fatty degeneration: first
revision 20% ± 22,6; multiple revisions 45% ± 31,0; p = 0,045%). A
preoperative high-grade osseous defect of the acetabulum is associated with
significantly higher damage of gluteus medius muscle than low-grade osseous
defects (p = 0,006). Conclusion: The present study shows that a preoperative
high-grade bone damage of the acetabulum is associated with significantly
higher muscle degeneration. With patients with multiple revision and/or higher
osseous deficits, the possible functional deficits have to be reasoned and
implants for reduction of the risk of dislocation have to be available
intraoperatively. Large femoral heads, dual mobility cups and constrained
liner represent possible options.

▼ Background: The acquired factor XIII deficiency after major operations is
correlated with perioperative blood loss and bleeding disorders. This study
investigated the effect of a prophylactic factor XIII application on the
perioperative blood loss and the rate of blood transfusions for alcohol
dependent patients undergoing a neck dissection. Methods: In this prospective
double-blind clinical study 23 alcohol dependent patients with a carcinoma, an
elective neck dissection and a daily minimum consumption of 60 g of alcohol
were randomized into two groups. The Fibrogammin group (
n=11) was treated with
2500 U Fibrogammin® preoperatively and four times 1250 U postoperatively. The
placebo group (
n=12) received 0.9 % saline solution instead. The blood loss
and transfused blood products were measured intraoperatively and daily up to
10 days after surgery or until the discharge from hospital. Results: In
between Fibrogammin and placebo group there was no significant difference of
blood loss according to the multivariate testing method (p=0.59) as well as
the univariate testing method (intraoperative p=0.83; postoperative p=0.74;
perioperative p=0.88). A tendency of decreased bleeding was found at 1. POD
(1. POD: p=0.24). Intraoperative blood products were administered to 1 patient
in the Fibrogammin (1 EK and 2 FFP) and 2 patients from the placebo group (3
EK respectively 2 EK). 1 patient in each group received a transfusion
postoperatively (Fibrogammin group: 1 EK, placebo group: 3 EK). The need of
transfusions did not differ within the groups (p=1.00). The length of hospital
stay in the Fibrogammin group tended to be 2 days shorter (p=0.41). After the
neck dissection, the factor XIII levels within the placebo group decreased
significantly by 30 %. 40 % of patients from the placebo group developed an
acquired factor XIII deficiency (≤ 70 %). The patients with an acquired factor
XIII deficiency showed a significantly enhanced blood loss intraoperatively
and at the 2nd POD (intraop: p=0.02; 2. POD: p=0.02). In the Fibrogammin
group, the value remained within the range of reference. Conclusions: Within
both groups the results did not show a significant reduction of perioperative
blood loss or decreased need for blood transfusions after the prophylactic
application of factor XIII. However, the descriptive results indicate that the
detection of factor XIII levels and the selective application of factor XIII
can be beneficial for alcohol-dependent patients with a high perioperative
blood loss.

▼ Background: Psoriasis vulgaris is a chronic disease that predominantly affects
the skin. The prevalence is about 2-3 % in the European population with a
variation among different ethnic groups. So far systemic reviews focused
predominantly on short-term treatment, whereas symptoms usually persist
throughout life. Objective: Evaluation of effective systemic long-term
treatments for moderate-to-severe psoriasis. Methods: A systematic literature
research in Medline, Embase and the Cochrane Li-brary was conducted with a
search strategy similar to the german Psoriasis S3 guide-line. The proportion
of patients with a decrease in the Psoriasis Area Severity Index (PASI) of at
least 75 % and the reduction in the Dermatology Life Quality Index (DLQI) at
different points in time during therapy were gathered. Studies have been
summarized in terms of study designs and survey periods. Among these
appropriate studies were included in the meta-analysis. Particular attention
was given to the sta-tistical approach with the handling of dropouts. Results:
33 publications reporting on 27 trials with active treatment of 6575 patients
were included in the systematic review. 7 randomized controlled trials were
eligible for the meta-analysis. After 24 weeks of treatment infliximab 5 mg/kg
bodyweight [risk dif-ference (RD) 78%, 95% confidence interval (CI) 72–83%]
and ustekinumab 90 mg (RD 77%, 95% CI 71–83%) were the most efficacious active
agents. Adalimumab 40 mg (RD: 60%, 95% CI 45–74%) showed results within the
range of different etanercept dosages (etanercept 50 mg once weekly: RD 62%,
95% CI, 52–72%), (etanercept 25 mg twice weekly: RD 45%, 95% CI 34–56%),
(etanercept 50 mg twice weekly: RD 56%, 95% CI 49–62%) and (etanercept 50 mg
twice weekly until week 12, then 25 mg twice weekly: RD 50%, 95% CI 42–57%).
In studies lasting longer than 24 weeks a decrease in efficacy for inflximab,
adalimumab and etanercept was observed. Conclusion: More sufficient data is
needed to draw reliable conclusions in efficacy of long-term treatments beyond
24 weeks and head-to-head comparisons of different ac-tive agents are
necessary.

▼ Background: There is growing evidence that inflammatory processes are involved
in the pathophysiology of depression. Immunmodulatory mechanisms may be part
of an effective antidepressant treatment. Lithium augmentation (LA) is one of
the treatment strategies with the best evidence concerning the efficacy for
treatment resistant depression (TRD). The impact of immune parameters on
lithium augmentation has not been investigated yet. The aim of this study was
to explore for the first time, if immune parameters such as cytokines change
under LA and if cytokines and brain-derived neurotrophic factor (BDNF) are
potential predictors of response to LA. Furthermore the relationship between
cytokines and BDNF as well as clinical characteristics was investigated.
Methods: As a subproject of a prospective multicenter study investigating
response prediction to LA in unipolar TRD (ICD10 F32.1-3 und F33.1-3) serum
levels of the immune parameters interleukin (IL)-2, IL-4, IL-6, IL-8, IL-10,
granulocyte-macrophage colony-stimulating factor (GM-CSF), Interferon-γ
(IFN-γ), tumor necrosis factor-α (TNF-α) were measured in 95 patients, BDNF in
104 patients before and after four weeks of LA. Depression severity was
assessed weekly using the Hamilton Depression Rating Scale (HAMD-17). Results:
Significant changes of cytokine levels under LA could not be found. Cytokines
before LA had no significant influence on the response or remission
probability. BDNF before LA showed a significant effect on the response
probability (Hazard Ratio (HR) 0.903, 95%KI 0.83-0.99, p=0.023) and an
influence with a trend to significance to remission probability to LA (HR
0.894, 95% KI 0.8-1.00, p=0.052) – cytokines had no effect. Patients with a
faster speed of onset of the depressive episode differed from patients with a
slower onset of the depressive episode in terms of cytokine levels (IL-2,
IL-4, IL-6, IL-8, IL-10, IFN-γ: p<0.05, TNF-α: p<0.1). BDNF was positively
correlated with IL-4 (p=0.009), GM-CSF (p=0.028) and IFN-γ (p=0.042) before LA
and IL-4, GM-CSF (p=0.012), IL-8 (p=0.067), IL-10 (p=0.081) and IFN- γ
(p=0.057) under LA. Conclusion: This is the first clinical study investigating
the influence of LA on cytokines. There was no modulating effect of LA on
peripheral cytokines. The data of this study does not indicate that cytokines
can predict treatment response to LA, whereas patients with low BDNF levels
are related with a better response to LA. Differences in clinical
characteristics of depression may correlate with immunological parameters and
there may be a connection between cytokines and BDNF. These results need
further evaluation. The findings contribute to the knowledge of cytokines and
BDNF as a state marker in affective disorders.

▼ Abstract Introduction: Multiple sclerosis (MS) is an immune-mediated,
inflammatory disease of the central nervous system. Although its etiology
remains unclear, some scientific studies indicate, that the mucosal immune
system may play an important role in the regulation of autoimmunity. The
purpose of this study was to evaluate the changes in the composition of the
intestinal flora of patients with MS Methods: Stool samples were collected
from MS outpatients (
N = 54) in the MS Center of Oberhavel Kliniken in
Hennigsdorf and from control persons (
N = 54) attending the General Internal
Medicine Outpatient Clinic of the Charité Hospital in
Berlin. They were
investigated with fluorescence in situ hybridization (FISH) techniques, with
probes specific for six bacterial groups: Atopobium, Bacteroidaceae,
Bifidobacteriae, Enterobacteriaceae, Eubacterium rectale and Faecalibacterium
prausnitzii. Results: Atopobium bacteria occured significantly more often in
MS patients than in controls. Concentrations of Atopobium and Bifidobacterium
were significantly higher, while the concentration of Bacteroidaceae laws
significantly lower in MS patients. The concentrations of Atopobium and
Bifidobacteriae in MS patients reached levels, which are typically for
Firmicutes and Bacteroidetes. The composition of the intestinal flora in
patients with MS did not depend on the clinical form of the disease, disease
activity, degree of disability, type of therapy and the place of residence.
Conclusion: The structure of the fecal flora in MS patients in regards to the
presence of bacterial groups corresponded to results of previous observations
in other autoimmune diseases. The evidence suggests that bacterial dysbiosis
of the intestinal flora could affect the immunological and metabolic
characteristics of MS patients. Some factors related to the evolution of the
human intestinal flora and its environmentally induced changes may suggest a
connection to the known MS-typical epidemiological patterns.
