▼ The periodontitis is an inflammatory disease condition caused by a variety of
oral microorganisms. A hyper reactive host response regulation is triggering
the disease progression in a small part of the world wide population – around
10% at most – resulting either in a local or generalized aggressive form of
manifestation. Bleeding is a final act in a process of tissue and capillary
damage initiated by bacterial presence and modulated by pro-inflammatory
cytokines. Calibrated assignment of bleeding intensity as a parameter for the
severity of the disease is considered golden standard for diagnosing
gingivitis – an inflammatory condition restricted to the soft tissues.
Periodontal inflammation regularly causes attachment loss and is accompanied
by loss of alveolar bone and a phenomenon named “pocketing”. The severity of
this disease condition is currently characterized by assessing either the
probing pocket depths (PPD) or the attachment loss (AL). The evaluation of
bleeding tendency as Bleeding on Probing (BoP) is currently an additional
dichotomous finding. The aim of the present study was to assess the
suitability of a visual calibration of a Pocket Bleeding Index (PoB-Index) to
reflect the real inflammatory status of each single probed periodontal pocket.
By matching single grades of the index suggested – i.e. the single visually
assigned bleeding grades - with parameters such as bleeding duration (stop
watch) and amount of blood sampled by capillary tube correlation coefficients
were calculated. Correlations between PoB-Index and the probing depth and PoBI
and the amount of gingival crevicular fluid (GCF) sampled as exudates that
follow a gradient were calculated also to objectify the visual calibration of
bleeding intensity. The capillary dilatation is initiated as first step in a
cascade of inflammatory reactions in front of all clinical signs and results
in a measurable increase of the GCF. Since periodontitis progresses
episodically, i.e. phases of activity are followed by phases of inactivity the
capillary dilatation follows the same pattern. The mean age of 17
participating patients recruited was 32 years; 10 of them were non-smokers
whereas 7 were smoking. All patients enrolled did not have any periodontitis
therapy or received any antibiotics prescribed for the last 12 months. All
patients underwent intensive instructions in oral hygiene repeatedly, thus the
allocated plaque index remained for the duration of the study < 25% (API <
25%). After completion of instrumentation (S/RP) all patients received
Amoxicillin (500 mg) and Metronidazol (250 mg) x 3 daily for 10 days per os
prescribed. The assessment of the study parameters was carried out at baseline
after completion of the initial therapy (before S/RP), at month 3, 6 and 9
past active therapy. In total 575 sites were evaluated and monitored. For
improving the reproducibility, all measurements were undertaken under equal

▼ The objective of the 11 year retrospective study was to elaborate on a model
for long-term forecasting of the survival of teeth and to identify the
respective main periodontal factors. The analysis is based on data from a
random sample of 108 patients who underwent at least 8 years of special
parodontal treatment in the department of Periodontology at Charité
Berlin.
This treatment mainly consists of instructions and perpetual training towards
dental hygiene, scaling, root-smoothing and dental surgery when necessary. The
gender relation in the sample was exactly 54/54 with a mean age of 50.1 years
at the beginning of the treatment. At the first dental inspection the mean
number of teeth in situ was 24.69 (minimum=4, maximum=32, standard
deviation=4.9). During the "initial therapy" 28 out of 2667 teeth had to be
extracted before starting the main therapy in which 211 out of 2639 teeth
could not be preserved, i.e. had to be extracted. 57% (20%) of the patients
had mean probing depths 4-6 mm (> 6 mm). Approximately 50% of the patients
showed a generalized moderate periodontitis and 10% actually got a generalized
heavy periodontitis. Because of the well known poor explanatory power and
their often misleading results, the popular binary presentation of the data
was solely done to meet traditional requirements. The main goal was to model
the multi-causal relationship between periodontal parameters and the command
variable "survive/not-survive". The multivariate statistical models "Logistic
regression" and "Tree analysis" were used to detect periodontal parameters
with multivariate significant highest impact with respect to the command
variable. Because of the apparent and well-known difference both statistical
models distinguish between single- and multi-rooted teeth. To reduce the
unmanageable high variety of parameters for a tooth, in the first step, two
scores were calculated that subsume the most important parameters: Score1 =
0.289 . ST distal + 0.332 . AttV bukkal + 0.359 . AttV mesial + 0.339 . AttV
oral Score2 = 0.503 . ST bukkal + 0.255 . AttV bukkal + 0.284 . AttV mesial +
0.313 . AttV oral. Score1 relates on single-rooted and Score2 is appropriate
for multi-rooted teeth. Both functions were used as hyper-parameters in both
statistical models. In contrast to the traditional results, both scores show
the necessity of the usage of individual measures instead of the so called
"approximate" values. The ROC-curves for Score 1 and Score2 with respect to
the command-variable provide an AUC of as high as 0.77 and 0.79 respectively.
The main results of the tree-analysis for single-rooted teeth are: Score1,
vitality, loose-fitting, smoking habits, Gender, upper/lower jaw and age-
related bone loss and their interactions are the most important indicators in
a multivariate model predicting the command variable "survive/not-survive".

▼ The aim of this study was to examine the revascularization of augmented bone
from the anterior iliac crest after a reduced healing period (3 months) and
different covermethods. For this 9 patients were augmented with autogenous
bone in the maxilla. The transplants were covered in one quadrant with a
bioresorbable membrane and in the other with periost only, according to the
split-mouth-technique. The goal was to supply the edentolous patients with
prosthetics based on implants. The implantation site was prepared with a
trephine-bur. The obtained sample was examined histological under a
lightmicroscope by digital camera and computer. The relation of newly formed
bone to necrotic bone and to connective tissue was determined. The average
results of the examination showed 46 % newly formed bone with membrane and 39
% newly formed bone without membrane. The amount of connective tissue was 28 %
with or without membrane-cover. 26 % necrotic bone was found with and 33 %
without membrane. Thus, the use of a membrane showed no significant advantage
in comparison to the covering with periost. The reduced healing period of the
augmented bone before implantation resulted in no disadvantage.

▼ Bisphosphonates are drugs which are used for the treatment of osteoporoses,
hypercalcamia, morbus paget, multiple myeloma and cancer associated with
metastases of the bone because of their ability to inhibit bone resorption.
Especially aminobisphosphonates given intravenousely every month are under
suspicion of being the trigger to the multifunctional effect of the
bisphosphonate associated osteonecrosis of the jaw (BRONJ). Since there is no
gold standard for the diagnoses of ONJ, the aim of our studies was to validate
the different methods in their valency. Methods used in hospital were used and
compared to assist the treatment which is strongly related to the instant of
time. 28 Patients of different ages, sex, oncology disease and social
background were included in our studies. All 28 patients were treated with
intravenous bisphosphonates for a time period of 6 months to 7 years and
developed a clinically assured ONJ. None of the patients were treated by
radiotherapy in the neck or head area. A clinically indicated panoramic x-ray,
a magnetic resonance image and nuclear bone scanning were taken before any
surgical treatment was undertaken. The patients were asked standardised
questions of the German ONJ-register. Afterwards data was compared with the
medical file. The clinical presentation showed exposed bone in 96.4% of cases,
in which pus (43%) and fistula formation (14%) was associated. The clear
subjective and objective improvement after antibiotic therapy underlines the
hypothesis of a multifunctional effect leading to ONJ by bacterial
colonisation in the jaw- an osteomyelitis. In 96% of cases, it was possible to
show a chronological coherence between a dental treatment and the development
of the ONJ. Typical radiological findings shown in the panoramic x-ray were
sclerosis, non-healing extraction sites and osteolytic lesions. However the
panoramic x-ray showed false negative results in 25% of cases. The third phase
of nuclear bone scanning showed an increased nuclear uptake in the jaw in all
of the cases. Three phase nuclear bone scans were performed routinely on
patients with metastatic disease as a part of their clinical evaluation. In
one case it was even possible to confirm a non symptomatic ONJ after the
nuclear bone scanning showed positive results. The nuclear bone scanning is a
very sensitive method which might assist a positive therapy for the ONJ. We
recommend every patient, having a higher risk of getting an ONJ, a diagnostic
program as shown in our studies. With the help of the MRI, ONJ could be
detected in every patient.
