Introduction Myoma are common benign tumors of the uterus and can be found in up to 35% of women above the age of 35. Myoma often become symptomatic and the therapy of choice for women who want to become pregnant in the future is the laparoscopic myomaenukleation. Because of the good vascularisation blood loss is the most common and severe intraoperative complication. The reliable control of the blood loss is thus the most important requirement for a succesful intervention. The problem is known for a long time and there exist numerous techniques to reduce blood loss. So far vessel clamps have never be used, even though there exist field-tested clamps on the market and the technique is used in other fields of medecine. Method In the prospective, randomised and multicenter study 158 patients were included. 89 patients underwent conventional laparoskopic surgery. The 69 patients in the study group underwent the same procedure and the additional intraoperative clipping of the uterine arteries. In both groups the following values were measured: intraoperative blood loss, decrease of haemoglobin, transfusion rates, weight and number of removed myoma, complications, length of surgery and the resistance and pulsatile indices of the uterine artery. Results The randomisation was successful. There was no significant difference between both groups concerning the preoperative haemoglobin value and the weight and number of removed myoma. In the study group the mean postoperative blood loss was 1,3 g/dl compared to a 1,5 /dl in the control group (p = 0,055). The mean intraoperative blood loss in the study group was 135ml and 150ml in the control group (p = 0,096). There were no transfusion needed in any group. The average length of surgery was 136 minutes in the study group and 120 minutes in the control group. There occured no pathologic changes in the resistance and pulsatile indices. No complications that could be connected to the intervention occured. A correlation between the size and the number of removed myoma and the blood loss was shown. Conclusion There was no significant difference concerning the decrease of haemoglobin, the intraoperative blood loss and the transfusion rate. Further studies are needed to evaluate if the technique allows to reduce blood loss in patients which very large or numerous myoma.
